TY - THES A1 - Dreßler, Simone Anneliese T1 - Analyse der postoperativen Versorgung von Patienten mit proximaler Femurfraktur in einer Rehabilitationsklinik T1 - Analysis of postsurgical treatment of patients with fracture of the proximal femur in a rehabilitation centre N2 - Osteoporotische Frakturen sind im Zusammenhang mit proximalen Femurfrakturen beim älteren Menschen von hoher klinischer und sozioökonomischer Bedeutung. Trotz des weltweiten exponentiellen Anstiegs osteoporotischer Fakturen findet vor allem in den primär versorgenden Kliniken eine Identifizierung und adäquate Behandlung von Patienten mit hohem Risiko nur unzureichend statt. Es besteht eine hohe Variabilität innerhalb der Versorgung osteoporotischer Patienten mit PFF in der Rehabilitationsklinik. Eine systematische und evidenzbasierte Osteoporose- und Frakturprävention wird nur teilweise durchgeführt. Innerhalb des Untersuchungszeitraumes von 1993-2002 wurden 795 Patienten in einem hessischen Neuro-Orthopädischen Rehabilitations- Zentrum mit durchgeführter Knochendichtemessung in die Analyse aufgenommen, wovon 156 Patienten wegen einer PFF behandelt wurden. Das Durchschnittsalter der Patienten mit PFF liegt bei 70,4 Jahre und ist um ca. zehn Jahre älter als das Kontrollkollektiv. Mehr als 90% der proximalen Femurfrakturen treten über dem 50. Lebensjahr auf. Knapp die Hälfte der Patienten mit PFF weisen Knochendichtewerte im osteopenischen Bereich auf, fast 50% der Patienten haben nach WHO-Klassifikation eine Osteoporose. Das Frakturrisiko für Patienten mit osteoporotischen Knochendichtewerten nach WHO-Klassifikation ist im Vergleich zu Patienten ohne Osteoporose um 154% erhöht ist. Das Auftreten niedriger Knochendichtewerte scheint eine stärkere Assoziation zu proximalen Femurfrakturen als zu anderen osteoporotischen Frakturtypen zu haben. Präexistente Frakturen sind bei Patienten mit PFF seltener anzutreffen als bei Patienten ohne PFF. Das Frakturrisiko ist durch das Vorliegen früherer Frakturen um die Hälfte niedriger. Radiusfrakturen treten bei Patienten mit PFF häufiger auf als Wirbelfrakturen und sind ein Indikator für potentielle proximale Femurfrakturen, da sie zu einem frühren Zeitpunkt als PFF innerhalb der Osteoporoseerkrankung auftreten. Je älter der Patient zum Zeitpunkt des Rehabilitationsaufenthalts ist, desto häufiger weist er frühere osteoporotische Frakturen auf. Frauen mit PFF geben anamnestisch fast doppelt so häufig prävalente Frakturen an. Innerhalb des Kollektivs mit proximalen Femurfrakturen ist eine hohe Koinzidenz von Sturz- und Frakturereignissen erkennbar. Das Sturzrisiko erhöht sich für Patienten mit positiver Sturzanamnese um 150%. Statistisch sind keine signifikanten Unterschiede zwischen Patienten mit und ohne PFF bezüglich der klassischen Sturzrisikofaktoren erkennbar. Es existiert kein einheitliches Risikoprofil für ältere Menschen, vielmehr handelt es sich um ein sehr inhomogenes Patientenkollektiv. Eine Feststellung des attributalen Anteils eines potentiellen Risikofaktors zum Sturzrisiko ist oft nicht möglich. Fast alle Patienten wurden operativ versorgt. Aufgrund der schnellen postoperativen Belastbarkeit und Mobilisierung werden bevorzugt endoprothetische Operationsverfahren eingesetzt. In den operativen Kliniken zeichnet sich eine gleich bleibend defiziente posttraumatische Osteoporosediagnose über den gesamten Untersuchungszeitraum ab. Innerhalb der Akutkliniken erhalten nur knapp ein Fünftel der Patienten mit PFF eine Osteoporosemedikation. Innerhalb der Rehabilitationsklinik wurden hauptsächlich physiotherapeutische Maßnahmen zur Stärkung der Muskelkraft, Stabilitäts- und Balancetraining angewandt. Unterstützend findet eine adäquate Schmerztherapie und psychologische Betreuung der Patienten statt. Es erfolgt eine solide Abklärung eines potentiellen Vorliegens einer Osteoporose. Im Vergleich zu den primär versorgenden Kliniken ist die Versorgung innerhalb des Rehabilitations- Zentrums besser. Es bestehen jedoch Mängel in der konsequenten Verabreichung einer adäquaten spezifischen medikamentösen Therapie. Fast die Hälfte der Patienten mit PFF und osteoporotischen Knochendichtewerte gehören nach DVO-Definition zur Hochrisikogruppe für weitere osteoporotische Frakturen. Allerdings findet eine nach den heutigen Erkenntnissen der DVO-Leitlinien unzureichende medikamentöse Intervention statt. Nur jeder 10. Patient mit PFF wird mit einem Bisphosphonat als spezifische medikamentöse Therapie versorgt. Eine Nachuntersuchung der Patienten des Rehabilitations- Zentrums ab 2003 nach Implementierung der DVO- Leitlinien wäre sicherlich interessant, um eine systematische Umsetzung leitliniengerechter klinischer Behandlungspfade in der Osteoporose- und Frakturprävention zu erkennen. N2 - Osteoporotic fractures of the proximal femur (PFF)of the elderly are of high clinical and socioeconomic relevance. Despite the worldwide exponential increase in the incidence of osteoporotic fractures, patients with high risk for osteporosis are often not identified during primary care resulting in an inadequate treatment. In the examined rehabilitation centre the treatment of patients with osteoporosis and PFF varies considerably. A systematic and evidence based osteoporosis- and fracture- prevention is only partly implemented. Within the examinated period from 1993 to 2002 795 patients of a hessischen neuro-orthopedic rehabilitation centre with measured bone density were included in the study. 156 of these patients were treated due to fractures of the proximal femur (PFF). The mean age of the patients was 70.4 years being 10 years higher than in the control group. Over 90% of the patients with PFF are older than 50 years. Almost half of the patients with PFF have bone density values in the osteopenic range, the other half having osteoporosis according to the WHO classification. The risk of bone fractures for patients with osteoporotic bone density values is increased by 154% compared to patients without osteoporosis. The association of low bone density values with PFF is higher than to any other type of osteoporotic fracture. Patients with PFF show less fractures in their case history as patients without PFF. The risk for PFF is reduced by pre-existing fractures by 50%. Forearm fractures are more frequent in patients with PFF than vertebral fractures. They may be an indicator for a high risk of PFF due to their earlier incidence in osteoporosis. Women with PFF have twice as often pre-existing fractures than men with PFF. The older a patient at the time of his rehabilitation the more frequent are pre existing osteoporotic fractures. Patients with fractures of the proximal femur (PFF) show a high coincidence of fracture and trauma. The risk to fall is increased by 150% for patients with a case history of falling. No significant differences regarding classical risk factors for falling were found in patients with or without fractures of the proximal femur. The riskprofile for elderly patients was inhomogeneous. The determination of the impact of a potential risk factor on the overall risk for falling was not possible. Almost all patients were treated surgically, endoprosthetic replacement were favoured due to the possibility of early loading and mobilisation of patients. The posttraumatic osteoporosis diagnostics was insufficient in all surgical clinics. In the acute-care hospitals only 20% of patients with fracture of the proximal femur get an osteoporosis medication. Rehabilitation centres concentrated on physiotherapy, pain control and psychological support. In spite of an efficient osteoporosis diagnosis in the rehabilitation centres a specific medication was not prescribed consistently. About 50% of the patients with PFF and low BMD have a high risk for osteoporotic fractures according to the DVO guidelines. The pharmacological intervention seems to be insufficient for these patients, as only 10% are treated with bisphosphonates. An examination of the rehabilitation centre after the publication of the DVO guidelines could show if an implementation into the clinical treatment was possible and effective in prevention of osteoporosis and fractures.
