Ätherisch-Öl Komponenten sind etabliert in der Therapie der chronischen und akuten Bronchitis. Eine klinische Studie, die mit Thymianextrakt durchgeführt wurde, ließ auf die klinische Wirksamkeit bei akuter Bronchitis schließen. Zahlreiche pharmakodynamische Effekte konnten in vitro für Thymianextrakt bzw. das ätherische Thymianöl gezeigt werden, jedoch wurde bis jetzt die systemische Verfügbarkeit der betreffenden Verbindungen am Zielorgan noch nicht untersucht. Diesbezügliche Untersuchungen sind notwendig, um die Verknüpfung zwischen den in vitro beobachteten Effekten und der klinischen Wirksamkeit herstellen zu können. Eine pharmakokinetische Studie wurde nach Einnahme einer Einzeldosis BronchipretTP (Filmtabletten mit 60 mg Primelwurzelextrakt und 160 mg Thymianextrakt) durchgeführt, um festzustellen, ob Thymol, das den Hauptbestandteil des ätherischen Thymianöls darstellt, zur Wirksamkeit dieser Zubereitung beitragen kann. Dazu wurde eine Extraktionsmethode für die Bestimmung von Thymol nach enzymatischer Spaltung des Phase-II-Konjugates Thymolsulfat im Plasma sowie von Thymolsulfat und Thymolglucuronid im Urin entwickelt. Es wurde eine neuartige, automatisierte headspace Festphasenmikroextraktionsmethode (HS-SPME) entwickelt, die Extraktion, Anreicherung und Probenaufgabe in einem einzigen Schritt ermöglichte. Die Quantifizierung von Thymol im unteren ng.mL-1 Bereich wurde durch die Verwendung von Gaschromatographie in Kombination mit Flammenionisationsdetektion durchgeführt. Die Automatisierung der Methode war notwendig um sie nach internationalen Richtlinien zur Validierung bioanalytischer Methoden validieren zu können. Dies ist das erste Mal, dass eine Bioverfügbarkeitsstudie bzw. eine pharmakokinetische Studie mit Hilfe der HS-SPME durchgeführt wurde. Thymol selbst war nicht systemisch verfügbar. Es war kein freies Thymol oberhalb von 1,4 ng/mL im Plasma detektierbar. Der systemisch verfügbare Phase-II-Metabolit wurde per LC-MS/MS als Thymolsulfat identifiziert. Thymolsulfat wurde nur im Plasma detektiert, wohingegen sowohl Thymolsulfat als auch Thymolglucuronid im Urin nachgewiesen wurden. Im Urin wurde kein freies Thyxmol oberhalb von 2,1 ng/mL detektiert. Da Thymol nur in Form von Thymolsulfat systemisch verfügbar war, wurde die pharmakokinetische Auswertung an Hand der sich nach enzymatischer Spaltung von Thymolsulfat ergebenden Daten vorgenommen. Die pharmakokinetische Studie wurde nach Verabreichung einer Einzeldosis BronchipretTP (1,08 mg Thymol) mit 12 Probanden durchgeführt. Die Resorption von Thymol war frühzeitig. Bereits nach 20 min konnte Thymol im hydrolisierten Plasma nachgewiesen werden. Maximale Plasmakonzentrationen (cmax) von 93,1±24,5 ng/mL (MW±SD) wurden nach 1,97±0,77 h (tmax) (MW±SD) erreicht. Die Plasmakonzentrationen zeigten einen biphasischen Verlauf und die terminale Eliminationphase setzte nach ca. 10 h ein. Die Eliminationshalbwertszeit errechnete sich zu 10,2 h. Dies betont die Wichtigkeit entsprechend langer Meßzeiträume in Verbindung mit einer hoch sensitiven Analytik, um die Elimination vollständig erfassen zu können. Bezogen auf die verabreichte Thymoldosis (1,08 mg) wurden 16,2±4,5 Prozent (MW±SD) mit unverändertem Thymolgrundgerüst renal eliminiert. Die renale Clearance von 271±156 mL/h (MW±SD) indiziert eine hohe Plasmaeiweißbindung und/oder tubuläre Reabsorption. Die Daten deuten darauf hin, dass die klinische Wirksamkeit nach Applikation einer thymianextrakthaltigen Zubereitung auf Thymolsulfat oder mögliche Phase-I-Metabolite zurückzuführen sein könnte. Die pharmakodynamischen Effekte dieser Verbindungen wurden bislang noch nicht untersucht. Andererseits könnte die Aktivität von Sulfatasen im Lungengewebe die pulmonale Elimination von Thymol erklären. Freies Thymol könnte somit am Respirationstrakt wirksam sein, obwohl es im Plasma nicht detektiert wurde. N2 - Essential oil compounds are established for the therapy of chronic and acute bronchitis. A clinical trial with a preparation containing thyme extract has suggested clinical efficacy for acute bronchitis. Various pharmacodynamic effects have been demonstrated in vitro for thyme extract and the essential thyme oil, respectively, but systemic availability of these compounds in the respective target organs has not been proven yet. Such investigations are necessary to provide the link between in vitro effects and in vivo studies. A pharmacokinetic study following a single dose of BronchipretTP tablets (60 mg primrose extract and 160 mg thyme extract) was performed to determine whether thymol, which is the main compound in the essential oil of thyme, might contribute to the efficacy of this formulation containing thyme extract. Therefore an extraction method for the analysis of thymol after enzymatic cleavage of the phase II conjugates thymol sulfate in plasma as well as thymol sulfate and thymol glucuronide in urine was developed. A novel automated headspace solid-phase microextraction (HS-SPME) method was developed, which provided extraction, concentration and sampling in a single step. Quantification of thymol was consequently achieved at the lower ng×mL-1 level using gas chromatography combined with flame ionization detection. Automation of the method was necessary to test the method according to international guidelines for validation of bioanalytical procedures. This is the first time that HS-SPME has been used for a bioavailability or pharmacokinetic study. Thymol itself was not systemically available because no free thymol could be detected in plasma above 1.4 ng/mL. The systemically available phase II metabolite was identified as thymol sulfate by HPLC-MS/MS. Thymol sulfate was identified in plasma whereas both thymol sulfate and thymol glucuronide were detected in urine. No free thymol was detected in urine above a limit of quantification of 2.1 ng/mL. As thymol was systemically available only in the form of thymol sulfate, pharmacokinetic analysis of the data was carried out based on the data obtained after enzymatic cleavage of thymol sulfate. The pharmacokinetic study comprised a single-dose study with 12 healthy volunteers (1 tablet BronchipretTP equivalent to 1.08 mg thymol). Absorption of thymol was rapid after administration of a single dose of BronchipretTP tablets. Thymol could be detected in hydrolyzed plasma 20 min after application. Maximum plasma levels (cmax) of 93.1±24.5 ng/mL(mean±SD) were reached after 1.97±0.77 h (tmax) (mean±SD). Plasma concentrations showed a biphasic profile and the terminal elimination phase set in after about 10 h and continued up to 38 h. The terminal elimination half life was calculated to be 10.2 h. This stresses the importance of long sampling periods in combination with highly sensitive analytical tools to adequately follow the elimination profile. With regard to the applied thymol dose (1.08 mg) the renally eliminated amount of thymol with unchanged thymol base structure was 16.2±4.5 per cent (mean±SD). The renal clearance of 271±156 mL/h (mean±SD) indicates high protein binding and/or tubular reabsorption. The data indicate that the pharmacological effect observed in vivo after application of preparations containing thyme extract could be due to thymol sulfate or putative phase I metabolites. However, the pharmacodynamic effects of these compounds have not been investigated yet. Also the activities of sulfatases in lung tissue could explain the observed pulmonary elimination of thymol. Free thymol could therefore be effective at the respiratory system, although it was not detected in plasma.
