N2  - Die Schulter stellt ein komplexes Gelenk dar, deren Funktion und Kraft von einer intakten Rotatorenmanschette abhängig sind. Defekte der Rotatorenmanschetten beinhalten isolierte Sehnendefekte sowie Massendefekte mit zwei oder mehr betroffenen Sehnen. Defekte des Musculus supraspinatus und infraspinatus werden als irreparable, postero-superiore Defekte bezeichnet. Diese Studie befaßt sich mit den Ergebnissen der Latissimus dorsi Transposition im Falle von irreparablen postero-superioren Rotatorenmanschettendefekten. Die Zielsetzungen einer verbesserten Zentrierung im glenohumeralen Gelenk, Wiederherstellung von Kraft, Außenrotation und Abduktion sowie Schmerzreduktion sind Gegenstand der Untersuchung. Die Erhebung der Ergebnisse erfolgte klinisch mittels Constant-Score sowie bildgebend mittels Röntgendiagnostik und Kernspintomographie der Schulter.
N2  - The shoulder is a complex joint which function and strength depends on an intact rotator cuff. Defects of the rotator cuff include isolated tendon defects as well as mass defects concerning two or more tendons. A defect of the supraspinatus and infraspinatus muscles are described as postero-superior rotator cuff defects. This study describes the results of latissimus dorsi transposition in case of irreparable, postero-superior mass defects of the rotator cuff. The evaluation concerns the development of the centering of the glenohumeral joint as well as an intended improvement of strength, external rotation, abduction and pain reduction. For the clinical examination we used the Constant score as well as x-ray and MR imaging of the shoulder for the morphological aspects.
