Furthermore the quality of life in elderly patients gains importance. Particularly challenging for orthopaedic surgeons is the treatment of extended acetabular defects. This study examines the clinical and radiological outcome of acetabular cup revisions by using a cranial raised press-fit cup. Additionally potential influencing factors were surveyed. Methods: In a retrospective monocentric study 155 patients, who underwent an acetabular cup revision by implanting an oval press-fit cup between 2009 and 2016 were reviewed and scored. An ethical clearance certificate of the University of Wuerzburg was given. Results: The mean follow-up was 53 months. 9 AAOS I defects, 25 AAOS II defects, 100 AAOS III defects and 21 AAOS IV defects were examined. The cumulative cup survival rate by using the Kaplan-Meier-Method was 93,4\% (CI 89,1-97,6\%). The survival rate without any revision was 84,0\% (CI 77,9-90,1\%). A significant reduction of pain on a numeric analogue scale could be proven (5,9 preoperative vs. 1,1 postoperative). The Harris-Hip Score increased significantly from 56,4 preoperative to 73,2 postoperative. In addition an improvement of all dimensions of the Euroqol could be confirmed. The overall state of health was measured by using the SF-12. As a result there was a slightly lower physical health and a slightly higher mental health compared to the reference population. Significant factors of influence on the outcome were detected: Male gender is associated with a higher functional outcome (HHS 87,7 vs. 74,2). Moreover, men numbered their level of pain lower than women did (NAS 0,65 vs. 1,5). Furthermore an association between more extended types of acetabular defects and a lower functional outcome could be confirmed. Additionally the factor age was found to create a significant influence on the functional outcome of the surgery. Patients below the age of 70 years upon the time of surgery reached higher results in Harris-Hip Score than older patients (82,2 vs. 77,3). Septic loosening was associated with a lower functional outcome than aseptic loosening (HHS 74,6 vs. 81,9). Conclusion: By analysing the survival rate, the improvement of function and the reduction of pain, good short- to midterm results were calculated for the surveyed implant system. Factors with a significant influence on the functional outcome were found to be: Gender, underlying type of defect, age upon surgery, septic or aseptic loosening.}, subject = {H{\"u}ftprothese}, language = {de} }
