Mit dem Ziel, m{\"o}gliche Verbesserungen im Katastrophenschutz aus Sicht einer {\"u}berregionalen Einheit aus dem Einsatz ableiten zu k{\"o}nnen, wurde dieser wissenschaftlich evaluiert und die Erkenntnisse in einen {\"u}bergeordneten Kontext gesetzt. Material und Methoden Nach Definition eines konkreten Auswerteprozesses wurden durch ein interdisziplin{\"a}res Expertengremium Einsatzunterlagen und relevante Konzepte des Bayerischen Roten Kreuzes gesichtet. Auf dieser Basis wurden Strategien und Taktiken entwickelt, um die vordefinierten Ziele zu erreichen. Ergebnisse Die Leistungsf{\"a}higkeit der Einsatzeinheiten k{\"o}nnte durch moderne Einsatzmittel (E-Bike, Drohnen, hochgel{\"a}ndeg{\"a}ngige Fahrzeuge) gesteigert werden. Zur Erh{\"o}hung der Reaktionsf{\"a}higkeit k{\"o}nnten neue Schnell-Einsatz-Gruppen (SEG) erforderlich sein, die als Teil von BOS-{\"u}bergreifenden Erkundungseinheiten agieren und in unwegsamen Gel{\"a}nden autark Einsatzauftr{\"a}ge abarbeiten k{\"o}nnen (SEG Erkundung und SEG Gel{\"a}nde-Infrastruktur-Logistik-Transport). Die taktischen Einheiten k{\"o}nnten in die regionale und {\"u}berregionale Katastrophenhilfe eingebunden und synchronisiert werden. Die Weiterverlegung in andere Behandlungseinrichtungen stellt einen sensiblen Abschnitt in der Behandlung und Rehabilitation des Weaningpatienten dar. Ziel der vorliegenden Studie war die Untersuchung des {\"U}berleitungsmanagements und des Interhospitaltransfers von Weaningpatienten unter besonderer Ber{\"u}cksichtigung der Dokumentationsqualit{\"a}t. Methodik Es erfolge eine retrospektive Datenanalyse eines Jahrs (2018) auf 2 Intensivstationen eines Universit{\"a}tsklinikums. Eingeschlossen wurden alle beatmeten Patienten mit folgenden Tracerdiagnosen: COPD, Asthma, Polytrauma, Pneumonie, Sepsis, ARDS und Reanimation (Beatmung > 24 h). Ergebnisse Insgesamt konnten 750 Patienten in die Untersuchung eingeschlossen werden (Alter 64 [52, 8-76; Median, IQR]; 32 \% weiblich). Davon waren 48 (6,4 \%) Patienten zum Zeitpunkt der Verlegung nicht entw{\"o}hnt (v. a. Sepsis und ARDS). Die Routinedokumentation war bei den Abschnitten „Spontaneous Breathing Trial", „Bewertung der Entw{\"o}hungsbereitschaft" und „vermutete Entw{\"o}hnbarkeit" ausreichend, um die Erf{\"u}llung der Parameter der S2k-Leitlinie „Prolongiertes Weaning" ad{\"a}quat zu beurteilen. Vorwiegend wurden diese Patienten mit Tracheostoma (76 \%) in Rehabilitationskliniken (44 \%) mittels spezialisierten Rettungsmitteln des arztbegleiteten Patiententransports verlegt (75 \%). Diskussion Die Verlegung nicht entw{\"o}hnter Patienten nach initialem Intensivaufenthalt ist ein relevantes Thema f{\"u}r den Interhospitaltransfer. Wir untersuchten Schmerz, psychische Belastung und k{\"o}rperliches Funktionsniveau mithilfe des Deutschen Schmerzfragebogens, klinischer Diagnostik und geriatrischer Funktionstests. Ergebnisse Die beiden Teilstichproben unterschieden sich nicht in demografischen Merkmalen. Bez{\"u}glich Schmerzintensit{\"a}t und Beeintr{\"a}chtigung sowie der psychischen Belastung fanden sich ebenfalls keine signifikanten Unterschiede. Lediglich die Anzahl der schmerzbedingt beeintr{\"a}chtigten Tage war vor Corona signifikant h{\"o}her. Die geriatrischen Funktionstests zeigten signifikant bessere Werte w{\"a}hrend der Pandemie an. Diskussion Die vorliegenden Daten zeigen keine Verschlechterung von Schmerz und k{\"o}rperlichem sowie psychischem Wohlbefinden bei SeniorInnen vor dem Hintergrund der Pandemie. Its habit of growth is intermediate between the furcata and laxicephala types of Pilosellae with epigeal and partly hypogeal stolons and a tendency to form clusters. The upper and lower surfaces of the rosette leaves have stellate hairs. The peduncles and the phyllaries are densely covered with dark glandular hairs with yellowish glandular heads. The lateral teeth of the ligules often are separated by incisions. A tetra- und pentaploid transitional taxon between P. macranthela subsp. sylvae-pici and P. officinarum is described as P. ottonis. P. ottonis is tetra- and pentaploid with up to 7 capitula. It is densely covered with dark stalk glands on phyllaries and resembles forms of P. acutifolia in habit. The phyllaries of P. ottonis are covered with numerous epidermal papillae with a diameter of about 10-20 μm and in this aspect resemble P. macranthela subsp. sylvae-pici and P. glomerata. However, in P. officinarum epidermal papillae are absent. The new subspecies belongs to a group of species (H. rotundatum, H. transylvanicum) whose main distribution is in the Balkans. The changeful nomenclatural history of the species name is described. Diagnostic features to distinguish the growth habit-similar species H. murorum, H. rotundatum and H. transylvanicum are discussed. Particular attention is drawn to the importance of the development of the basic leaf cycle. Contrary to previous knowledge, the northwestern distribution limit of H.rotundatum extends now to southeastern Bavaria. Unfortunately, the necessary quality criteria are often mising. Thus, it seems important, that in addition to an app controlling authority highly qualified health care professionals participate in the development of these applications. For reasons of liability, however, the physician must exercise great caution in the selection and recommendation of medical apps, especially considering, that only a few apps are certified as medical devices. There are a large number of medical apps on the market, with only a small proportion being assigned to the field of otorhinolaryngology. The areas of audiology, sleep medicine and allergology are most frequently represented. Althouhgh there is increasing scientific work on this topic in the field of otorhinolaryngology, there is a lack of scientific evidence of contents and results, as is generally the case of medical apps. However, there are other possibilities for users to rate medical apps regarding defined qualitiy criteria such as functionality, scientific integrity, but also data privacy. None of the apps assessed by such a evaluation tool met all the required quality criteria, but the applied instrument helped to better assess the application. However, it was possible to consider the quality criteria in the developmental process of an medical app for the field of otorhinolaryngoglogy.
