Hierzu wurden 655 Patienten (w: 336, m: 319) eingeschlossen (OP- Zeitraum von 2012-2014). Im Mittel wurde der globale Offset (GO) um -1,14mm verringert (Vergr{\"o}ßerung des femoralen Offset (FO) durch den Eingriff um +4,61 mm und Verminderung des acetabul{\"a}ren Offsets (AO) um -5,74 mm). Bei den Frauen zeigte sich eine Vergr{\"o}ßerung des GO um +0,27 mm (AO -5,08 mm und FO +5,37 mm), bei den M{\"a}nner eine Verkleinerung des GO um -2,62 mm (AO -6,43 mm und FO 3,81 mm). In der Gesamtgruppe zeigten die Parameter der horizontalen Achse AO und FO keine Korrelationen zum Outcome. Jedoch konnte f{\"u}r den GO eine Korrelation zum EuroQol nachgewiesen werden (r=0,097; p<0,05) nachgewiesen werden. In der Gruppe der Frauen konnte eine positive Korrelation zwischen dem AO und dem WOMAC-Gesamtscore (r= 0,132; p<0,05) sowie den Subscores Schmerz (r=0,113; p<0,05) und Funktion (r=0,125; p<0,05) nachgewiesen werden. Die Ergebnisse best{\"a}tigen den Vorteil einer Medialisierung der Pfanne. Bei der Analyse der Parameter der vertikale Achse (vertikaler Offset (VO), H{\"u}ftl{\"a}nge (HL), Hip Center Height (HCH)) ergab sich in der Gesamtgruppe eine negative Korrelation des VO mit den WOMAC-Gesamtscore-Werten (r=-0,109; p<0,01) und den Werten des Subscores WOMAC-Schwierigkeit (r=0,106; p<0,01). Bei der HCH zeigte sich eine schwach positive Korrelation mit dem Subscore WOMAC-Schwierigkeit (r=0,081; p<0,05) und mit dem EurQol (r=0,0117; p<0,01). In der Subgruppe der Frauen ergaben sich bei der HCH eine Korrelation mit dem WOMAC-Gesamtscore (r=0,109; p<0,05) und dem Subscore WOMAC-Funktion (r=0,125; p<0,05). Die HL korrelierte in der Gruppe der M{\"a}nner negativ mit dem WOMAC-Steifheit (r= -0,137; p<0,05). Eine Beckendiskontinuit{\"a}t (PD „pelvic discontinuity") stellt dabei eines der herausforderndsten Szenarien dar. Wir berichten {\"u}ber unsere klinischen Ergebnisse mit einer asymmetrischen H{\"u}ftpfannenkomponente zur Behandlung der PD. Das Implantat ist mit einer extramedull{\"a}ren anatomisch geformten Lasche ausgestattet, die an der glutealen Oberfl{\"a}che des Iliums fixiert wird. Eine zus{\"a}tzliche Fixierung kann durch die Implantation eines intramedull{\"a}ren iliakalen Darmbeinzapfens erfolgen. MATERIAL UND METHODEN: In einer monozentrischen retrospektiven Kohortenstudie analysierten wir prospektiv erhobene Daten von 49 Pat. (35 weiblich, 14 m{\"a}nnlich) mit unilateraler periprothetischer Beckendiskontinuit{\"a}t, die 2009-2017 mittels einer asymmetrischen Acetabulumkomponente operativ behandelt wurden. Das mittlere Alter der Betroffenen zum Zeitpunkt der Indexoperation betrug 72 Jahre (54- 90 Jahre). Die mittlere Nachbeobachtungszeit lag bei 71 Monaten (21-114 Monate). S{\"a}mtliche in der Folge aufgetretenen Komplikationen wurden dokumentiert und die radiologischen und funktionellen Ergebnisse (anhand des Harris Hip Scores (HHS) sowie der visuellen Analogskala f{\"u}r Schmerz (VAS)) wurden ausgewertet. ERGEBNISSE: Mittels Kaplan-Meier-Analyse konnte f{\"u}r unsere Studiengruppe ein 5-Jahres-Implantat{\"u}berleben von 91\% (Konfidenzintervall 77\%-96\%) festgestellt werden). Der mittlere HHS verbesserte sich in unserem Patient*innenkollektiv von 41 pr{\"a}operativ auf 79 bei der letzten Nachuntersuchung (p< .001); die VAS verbesserte sich hochsignifikant von einem Mittelwert von sechs pr{\"a}operativ auf postoperativ eins (p< .001). Die Gesamtrevisionsrate betrug 16\% (n=8). Zwei der Pat. ben{\"o}tigten eine Revision bei aseptischer Lockerung. Bei vier Pat. (8\%) wurde eine periprothetische Infektion festgestellt: ein Patient wurde deshalb mit einer zweizeitigen Revision und ein weiterer mit einer Resektionsarthroplastik behandelt. Die anderen zwei Pat. wurden jeweils mit einem chirurgischen Eingriff im Sinne eines Debridements, ausgiebiger Sp{\"u}lung sowie dem Austausch der mobilen Teile behandelt. Von sechs Pat. (12\%), die eine H{\"u}ftluxation erlitten, war bei zweien die Implantation einer Dual-Mobility-Acetabulumkomponente erforderlich. Methoden: Es wurden die Daten von 73 Probanden mittels der folgenden Fragebogen ausgewertet: Harris Hip Score (HHS), eXtra Short Musculoskeletal Functional Assessment questionnaire (XSFMA), Short Form 36 (SF-36) health survey und Patient Health Questionaire 9 „deutsch" (PHQ-9 D). Zur Schmerzmessung kam eine visuelle Analogskala (VAS von 0-4) zum Einsatz. Daneben wurde die Aktivit{\"a}t mit Hilfe des Schrittz{\"a}hlers Stepwatch™ Activity Monitor (SAM) und eines 25m Gehtests auf Zeit (T25-FW) erfasst. W{\"a}hrend der gesamten Aufzeichnung wurden auch Komplikationen erfasst. Ergebnisse: Zw{\"o}lf Monate nach der Operation verbesserten sich die HHS-Werte signifikant von 55,2 pr{\"a}operativ auf 92,4 (Werte 0 - 100). Der FSFMA Funktionsscore fiel ebenfalls signifikant von 39,4 auf 10,3 und der Beeintrachtigungsscore von 47,0 auf 15,8. Der Score f{\"u}r die Physis (PCS) stieg im SF 36 signifikant von 27,5 pr{\"a}operativ auf 47,5 nach zw{\"o}lf Monaten. Der Score f{\"u}r mentale Gesundheit (MCS) fiel dagegen sogar leicht von 57,6 auf 55,0. Dagegen fiel die Pr{\"a}valenz der mittels PHQ-9 D gemessenen Somatisierungsst{\"o}rungen von elf auf einen Fall. Die Schmerzreduktion durch die Operation zeigte sich durch einen R{\"u}ckgang auf der VAS von 2,41 auf 0,35 zw{\"o}lf Monate postoperativ. This may result in the initiation of bone formation, muscle hypertrophy, and the enhanced production of extracellular matrix constituents, adhesion molecules, and cytoskeletal elements. Bone and muscle mass, resistance to strain, and the stiffness of matrix, cells, and tissues are enhanced, influencing fracture resistance and muscle power. This propagates a dynamic and continuous reciprocity of physicochemical interaction. Secreted growth and differentiation factors are important effectors of mutual interaction. The acute effects of exercise induce the secretion of exosomes with cargo molecules that are capable of mediating the endocrine effects between muscle, bone, and the organism. Long-term changes induce adaptations of the respective tissue secretome that maintain adequate homeostatic conditions. Ein m{\"o}glicher Therapieansatz ist die Rekonstruktion des MPFL. Zahlreiche Operationstechniken sind hierzu in der Literatur beschrieben, darunter Fixationen mit Interferenzschrauben, Bohrkan{\"a}len, Nahtankern oder Nahtfixationen am Knochen oder Weichgewebe. Sie werden meist mit einem autologen Sehnentransplantats durchgef{\"u}hrt und unterscheiden sich haupts{\"a}chlich in der Art dessen patellarer Fixation. Meta-Analysen zeigen nach wie vor hohe Komplikationsraten und teilweise wenig zufriedenstellende Ergebnisse etablierter MPFL-Rekonstruktionen. Ein vielversprechender Ansatz zur Senkung der Komplikationsraten sind patellare Fixationsmethoden, die auf das Einbringen von Fremdmaterial und Bohrungen durch die Patella verzichten. Solche implantat- und/oder bohrkanalfreien Fixationsmethoden sind bereits in der Literatur beschrieben, bis jetzt existieren jedoch nur wenige Studien zu deren biomechanischen Eigenschaften. Ziel dieser Arbeit war der Vergleich biomechanischer Eigenschaften von f{\"u}nf unterschiedlichen implantatfreien patellaren Fixationsmethoden beim MPFL- Ersatz mit dem nativen MPFL und mit nativen Extensorensehnen. Dazu wurde ein porcines Modell benutzt. Die Hypothese dieser Studie war, dass die patellare Weichteilfixation eines autologen Sehnentransplantats ohne Implantate oder Bohrkanal eine vergleichbare prim{\"a}re Ausrissfestigkeit wie das native MPFL zeigt. 60 Patellae und Extensorensehnen wurden aus porcinen Hinterl{\"a}ufen extrahiert und randomisiert auf 6 Versuchsgruppen aufgeteilt (n=10). In den Gruppen 1 und 2 wurden patellare Weichteilfixationen getestet. In den Gruppen 3 und 4 wurden Bohrkanalfixationen mit unterschiedlichem Nahtmaterial durchgef{\"u}hrt (Gruppe 3: resorbierbarer Faden der St{\"a}rke USP 0, Gruppe 4: nicht-resorbierbarer Faden der St{\"a}rke USP 3). In Gruppe 5 erfolgte die patellare Fixation mittels V-f{\"o}rmigen Bohrkanal mit Durchmesser von 4,5 mm. In der sechsten Versuchsgruppe wurde zum Vergleich das native Gewebe getestet. In einer weiteren Versuchsgruppe wurden native porcine Extensorensehnen biomechanisch untersucht. Die biomechanische Testung gliederte sich in mehrere Abschnitte. Zun{\"a}chst erfolgte eine Pr{\"a}konditionierung mit 10 Setzzyklen mit einem Kraftintervall von 5 - 20 N. Es folgte eine zyklische Testphase von 1000 Messzyklen mit einem Kraftintervall von 5 - 50 N. Im Anschluss wurde die maximale Kraft bis zum Versagen angewendet. Dabei wurden die Parameter maximale Ausrisskraft, Yield Load, Steifigkeit, Elongation nach 1000 Zyklen sowie die Versagensart untersucht. In der Versuchsgruppe des nativen Gewebes wurde direkt die maximale Kraft bis zum Versagen angewendet. Die h{\"o}chsten maximalen Ausrisskr{\"a}fte wurden in Gruppen 1 und 2 gemessen (321,8 ± 53,5 N und 242,0 ± 57,4 N) und waren mit dem nativen Gewebe vergleichbar (252,1 ± 129,3 N, p > 0,05). Ein signifikant niedrigerer Yield Load im Vergleich zum nativen Gewebe zeigte sich in den Gruppen 3 (p < 0,01) und 4 (p = 0,01). W{\"a}hrend der zyklischen Belastung war die Elongation der Pr{\"a}parate in Gruppe 3 signifikant h{\"o}her im Vergleich zu den anderen Gruppen (5,3 ± 1 mm, p < 0,05). Alle Gruppen wiesen {\"a}hnliche Steifigkeiten wie das native Gewebe vor (p > 0,05). Die getesteten Fixationsmethoden zeigten signifikante Unterschiede in ihren biomechanischen Eigenschaften (p < 0,05). The aim of this study was to identify and compare the injury profiles and compare the injury profiles of these three extreme water sports. Materials and Methods These data for this retrospective cohort study were collected through an online standardised questionnaire during the 2017-18 season. The questionnaire included questions about anthropometry, skill level, injury diagnosis, injury mechanism, environmental conditions and training regimes. Results The 626 athletes included reported 2584 injuries. On average, each athlete sustained 4.12 injuries during the season. The most frequent injury location was in the lower extremity, in particular the foot, with 49 (16.4\%) injuries in surfing, 344 (18.3\%) in wind surfing and 79 (19.7\%) in kite surfing. Surfing demonstrated a particularly high rate of head injuries (n = 37; 12.4\%). Other frequent injury types were skin lesions (up to 42.1\%) and contusions (up to 40.5\%). The most common injury across all surfing sports was skin lesions of the foot (wind surfing: 11.7\%; kite surfing: 13.2\%; surfing: 12.7\%). In surfing, skin lesions of the head were frequently observed (n = 24; 8.0\%). In surfing, a 'too large wave' (n = 18; 24.7\%) was main cause of the injury, while in wind surfing (n = 189; 34.5\%) and kite surfing (n = 65; 36.7\%) 'own incompetence' led to the most injuries. Conclusion This unique study compares injury epidemiology and mechanism in the three most popular surfing sports: wind surfing, kite surfing and surfing. Methods. 256 THA revisions were retrospectively classified to be infected or not infected. Classification was performed according to the 4 different definitions proposed by the Musculoskeletal Infection Society (MSIS), the Infectious Diseases Society of America (IDSA), the International Consensus Meeting (ICM), and the European Bone and Joint Infection Society (EBJIS). Only chronic PJIs were included. Results. PJI prevalence at revision significantly correlated with the applied PJI definition (p=0.01, Cramer's V=0.093). PJI prevalence was 20.7\% for the MSIS, 25.4\% for the ICM, 28.1\% for the IDSA, and 32.0\% for the EBJIS definition. For synovial fluid white blood cell count, the best ROC-AUC for predicting PJI was 0.953 in combination with the MSIS definition. Conclusion. PJI definition significantly influences the rate of diagnosed PJIs at THA revision. Synovial fluid white blood cell count is a reliable means to rule out PJI. Season 2017-2018 was excluded because of missing video material. Results Video analysis included 175 (53\%) of 329 moderate and severe match injuries. Contact patterns categorized according to the different body sites yielded eight groups of typical injury patterns: one each for the head, shoulders, and ankles, two for the thighs, and three for the knees. Injuries to the head (92\%), ankles (76\%), shoulders (70\%), knees (47\%), and thighs (32\%) were mainly caused by direct contact. The injury proportion of foul play was 19\%. Most injuries (61\%) occurred in the central zone below the basket. More injuries occurred during the second (OR 1.8, p = 0.018) and fourth quarter (OR 1.8, p = 0.022) than during the first and third quarter of the match. Conclusion The eight identified injury patterns differed substantially in their mechanisms. Moderate and severe match injuries to the head, shoulders, knees, and ankles were mainly caused by collision with opponents and teammates. Results The anterolateral approach is by far the most popular approach used for primary total hip arthroplasty, followed by the anterior approach during minimally invasive (55\% for the anterolateral and 29\% for the anterior) and regular surgery (52\% for the anterolateral and 20\% for the anterior). Two-thirds of the orthopaedic surgeons do not use drainages during THA. Moreover, 80\% of the survey participants routinely apply tranexamic acid during surgery. Surgeons who perform minimally invasive surgery for THA use more frequently fast-track-concepts for post-operative rehabilitation. According to the interviewees, the application of fast-track-concepts leads to reduced periods of hospital stay after THA. Conclusion Our data demonstrate that patterns of practice during THA in Germany are in line with the evidence provided by current literature. Methods ALP substrates, bone turnover and mineral metabolism markers, and bone mineral density (BMD) data from EmPATHY, a single-center, observational study of adults (≥ 18 years) with pediatric-onset HPP treated with asfotase alfa (NCT03418389), were collected during routine clinical care and analyzed from baseline through 24 months of treatment. Results Data from 21 patients showed significantly increased ALP activity and reduced urine phosphoethanolamine (PEA)/creatinine (Cr) ratios after baseline through 24 months of asfotase alfa treatment. There were significant transient increases in parathyroid hormone 1-84 (PTH), osteocalcin, and procollagen type 1 N-propeptide (P1NP) levels at 3 and 6 months and in tartrate-resistant acid phosphatase 5b (TRAP5b) levels at 3 months, with a significant decrease in N-terminal telopeptide of type 1 collagen (NTX) levels at 24 months. Lumbar spine BMD T scores continuously increased during treatment. Conclusion Significant changes in bone turnover and mineral metabolism markers after asfotase alfa treatment suggest that treatment-mediated mineralization may enable remodeling and bone turnover on previously unmineralized surfaces. In both legs, the distal femur-mechanical axis angle (FMA), proximal tibia-mechanical axis angle (TMA), and the hip-knee-ankle angle (HKA) were measured. Each alignment angle was assigned to one of Hirschmann's five FMA, five TMA, and seven HKA phenotype categories. Results Three TKAs (1.5\%) underwent reoperation for anterior knee pain or patellofemoral instability in the subgroup of patients with the more valgus phenotypes. There were no implant revisions for component loosening, wear, or tibiofemoral instability. The median Forgotten Joint Score (FJS) was similar between phenotypes. The median Oxford Knee Score (OKS) was similar between the TMA and HKA phenotypes and greatest in the most varus FMA phenotype. The phenotype proportions after calipered KA TKA were comparable to the contralateral leg. Conclusion Unrestricted calipered KA's restoration of the wide range of phenotypes did not result in implant revision or poor FJS and OKS scores at a mean follow-up of 4 years. The few reoperated patients had a more valgus setting of the prosthetic trochlea than recommended for mechanical alignment. Methods Prospective randomized, controlled, single center exploratory trial to compare four distinct exercise interventions, i.e. Resistance Training (RT), Whole Body Vibration Exercise (WBV), Qi Gong (QG) and wearing a Spinal orthosis (SO) for 6 months in men at risk for osteoporosis aged 65-90 years. Primary endpoint was change in isometric one repetition maximum force trunk strength for extension (TSE) and flexion (TSF) compared to baseline, secondary endpoints covered key parameters of geriatric functional assessment, including Handgrip Strength (HS), Chair-Rise-Test (CRT), Usual Gait Speed (UGS) and Timed-Up-and-Go (TUG). Results Altogether 47 men (mean age 77 ±6.1 years) were randomized to RT, (n = 11) WBV (n = 13), QG (n = 10) and SO(n = 13). RT, defined as reference exercise intervention, lead to significant improvements for TSE (p = 0.009) and TSF (p = 0.013) and was significantly superior in the between-group analysis for TSE (p = 0.038). Vibration exercise caused sign. Improvements in TSE (p = 0.014) and CRT (p = 0.005), the Spinal orthosis improved CRT (p = 0.003) and Gait Speed (p = 0.027), while the QG intervention did not attain any sig. Developments. Subgroup analyses revealed most pronounced musculoskeletal progress in vulnerable patients (age ≥ 80 years, pre-sarcopenia, multimorbidity ≥3chronic diseases). Irrespective of the type of exercise, participants ≥80 years experienced significant gains in TSE (p = 0.029) and CRT (p = 0.017). For this purpose, we used 24 stainless steel wire cerclages and 24 ultra-high molecular weight polyethylene and polyester suture tape test bodies. The test bodies were incubated for 30 min. in 100 mg/ml vancomycin solution or equivalent volumes of 0.9\% NaCl. After measuring the initial solution uptake of the test bodies, antibacterial efficacy via agar diffusion test with Staphylococcus aureus and vancomycin elution tests were performed 1, 2, 3, and 6 days after incubation. Vancomycin-loaded tapes as well as vancomycin-loaded cerclage wires demonstrated increased bacterial growth inhibition when compared to NaCl-treated controls. Vancomycin-loaded tapes showed an additional twofold and eightfold increase of bacterial growth inhibition compared to vancomycin-loaded wires at day 1 and 2, respectively. Elution tests at day 1 revealed high levels of vancomycin concentration in vancomycin loaded tapes and wires. Additionally, the concentration in vancomycin loaded tapes was 14-fold higher when compared to vancomycin loaded wires. Incubating suture tapes and cerclage wires in vancomycin solution showed a good short-term antibacterial activity compared to controls. We characterized metabolic glycoengineering in telomerase-immortalized human mesenchymal stromal cells (hMSC-TERT) as a model for primary hMSC, to investigate its applicability in TERT-modified cell lines. The metabolic incorporation of N-azidoacetylmannosamine (Ac\(_4\)ManNAz) and N-alkyneacetylmannosamine (Ac\(_4\)ManNAl) into the glycocalyx as a first step in the glycoengineering process revealed no adverse effects on cell viability or gene expression, and the in vitro multipotency (osteogenic and adipogenic differentiation potential) was maintained under these adapted culture conditions. In the second step, glycoengineered cells were modified with fluorescent dyes using Cu-mediated click chemistry. In these analyses, the two mannose derivatives showed superior incorporation efficiencies compared to glucose and galactose isomers. In time-dependent experiments, the incorporation of Ac\(_4\)ManNAz was detectable for up to six days while Ac\(_4\)ManNAl-derived metabolites were absent after two days.
