Barrenness could have dramatic consequences in the Middle Ages, for example expulsion, divorce or conflicts over royal succession. Against this historical background, it seems logical that the fate of the childless ruler would be recounted in the form of a 'Passion narrative'. In the German literature of the Middle Ages, however, there are also interpretative models of a contrary vein to be found. In the year 1220, for instance, Ebernand of Erfurt construed the wedded life of the imperial couple Henry and Kunigunde as a tale of resistance against the royal obligation to reproduce. In his versified legend, composed in the vernacular, the couple secretly agrees not to fulfil society's expectations, but to lead a chaste marriage. Yet above and beyond legend, childless rulers were also subject to the impact of multifarious legal, religious, medical, narrative and discursive factors. We discovered that disruption of the lipid flippases that maintain PtdSer or PtdEth asymmetry in the plasma membrane have opposite effects on phagocytosis in Caenorhabditis elegans embryos. Constitutive PtdSer externalization caused by disruption of the major PtdSer flippase TAT-1 led to increased phagocytosis of cell debris, sometimes leading to two cells engulfing the same debris. In contrast, PtdEth externalization caused by depletion of the major PtdEth flippase TAT-5 or its activator PAD-1 disrupted phagocytosis. These data suggest that PtdSer and PtdEth externalization have opposite effects on phagocytosis. Furthermore, externalizing PtdEth is associated with increased extracellular vesicle release, and we present evidence that the extent of extracellular vesicle accumulation correlates with the extent of phagocytic defects. We examined the frequency of sADRs of psychotropic drugs in pediatric inpatients and explored their potential preventability through following the recommendations of a web-based pediatric drug information system (PDIS). The potential socio-economic impacts of using this online system is also addressed. Routine clinical data from all inpatients treated in a child and adolescent psychiatry department between January 2017 and December 2018 were retrospectively examined for the occurrence of sADRs as defined by the European Medicines Agency. The preventability of the sADRs was assessed based on the information of the PDIS. Furthermore, the expected prolongation of the hospital stay due to sADRs was calculated as well as the associated treatment costs. The study was supported by the Innovation Fund of the Joint Federal Committee, grant number 01NVF16021. In total, 1036 patients were screened of whom 658 (63.5\%) received psychopharmacological treatment. In 53 (8.1\%) of these patients 54 sADRs were documented, of which 37 sADRs were identified as potentially preventable through PDIS. Mitigating sADR through PDIS would likely have prevented prolonged hospital stays and conferred considerable savings for health insurance companies. PDIS provides systematic and evidence-based information about pediatric psychopharmacotherapy and helps to prevent prescribing errors. Therefore, PDIS is a useful tool to increase drug therapy safety in child and adolescent psychiatry. Diese Patienten wurden anhand der Defektlokalisation, d. h. isolierter SSP-, isolierter SSC-, kombinierter SSP+ISP-, kombinierter SSP+SSC-Defekt und kombinierter SSP+SSC+ISP-Defekt in f{\"u}nf Gruppen eingeteilt. F{\"u}r jede Gruppe wurden Matched-Pairs gebildet. Hierzu wurden Patienten gleicher Defektlokalisation und Rotatorenmanschettenrekonstruktion, jedoch ohne Versorgung mit einer LBS-Tenodese ausgew{\"a}hlt, die in Alter und Geschlecht den Patienten „mit Tenodese" entsprachen. Mit 132 Personen (67 „mit Tenodese" und 65 „ohne Tenodese") konnten Matched-Pairs gebildet werden. F{\"u}r die 17 Patienten mit SSP+SSC-Defekt war dies nicht m{\"o}glich, da bei einer derartigen Defektsituation stets eine Tenodese der LBS durchgef{\"u}hrt worden war. Somit wurde 149 Patienten ein Fragebogen zugeschickt, der den Constant-Murley Score, den Simple Shoulder Test und spezielle Schulterfunktionsfragen enthielt. 111 Patienten (63 „mit Tenodese" und 48 „ohne Tenodese") f{\"u}llten die Frageb{\"o}gen aus und sendeten sie zur{\"u}ck. Die Frageb{\"o}gen wurden statistisch ausgewertet und die Ergebnisse der Patienten „ohne Tenodese" denen „mit Tenodese" gegen{\"u}bergestellt - sowohl in den Matched-Pairs, als auch bei der Betrachtung aller Patienten unabh{\"a}ngig von der Lokalisation des Rotatorenmanschettendefektes. Zus{\"a}tzlich wurden bei der Auswertung noch die Gesamtergebnisse des Constant-Scores von zehn Patienten, die aus einer vorherigen Untersuchung bekannt waren, ber{\"u}cksichtigt. Bei der Bewertung der Operationszufriedenheit und beim Simple Shoulder Test zeigten sich keine statistisch signifikanten Unterschiede zwischen den beiden Gruppen. Beim Gesamtvergleich aller Patienten unabh{\"a}ngig von der Defektlokalisation, erzielten die Patienten „ohne Tenodese" (n=53) einen durchschnittlichen Constant-Score von 68,7 Punkten und bei jedem Parameter h{\"o}here Ergebnisse als die Patienten „mit Tenodese" (n=54), deren durchschnittlicher Constant-Score 56,7 Punkte betrug. Abgesehen von den Parametern Arbeitsh{\"o}he und Innenrotation waren diese Unterschiede bei allen vier Sektionen des Constant-Scores statistisch signifikant. Beim Matched-Pair der Patienten mit isoliertem SSP-Defekt lag bei keinem Parameter ein statistisch signifikanter Unterschied zwischen den 26 Patienten mit und den 24 Patienten ohne Tenodese vor, doch erzielten die Patienten „ohne Tenodese" durchschnittlich h{\"o}here Werte. Im Durchschnitt betrug der Constant-Score der Patienten „ohne Tenodese" 68,7 Punkte gegen{\"u}ber 62 Punkten bei den Patienten mit Tenodese. F{\"u}r das Matched-Pair der Patienten mit isoliertem SSC-Defekt wurde aufgrund der geringen Patientenzahl keine Signifikanzpr{\"u}fung durchgef{\"u}hrt, doch zeigte sich auch hier, dass die vier Patienten „mit Tenodese" ein niedrigeres Ergebnis im Constant-Score (63,5 Punkte) erreichten als die f{\"u}nf Patienten ohne LBS-Tenodese (69 Punkte). Die Patienten des Matched-Pairs SSP+ISP-Defekt „ohne Tenodese" (n=8) erreichten, abgesehen von der Innenrotation, bei allen Parametern ein signifikant h{\"o}heres Ergebnis als Patienten „mit Tenodese"(n=9) und erzielten das h{\"o}chste Ergebnis im Constant-Score (85,1 Punkte) von allen Gruppen. Bei einem vorliegendem Drei-Sehnen-Defekt konnte zwischen den beiden Gruppen nur bei der Außenrotation ein statistisch signifikanter Unterschied festgestellt werden, doch waren auch hier die einzelnen Ergebnisse bei Patienten „ohne Tenodese" (n=16) stets h{\"o}her als bei der Gruppe „mit Tenodese" (n=15), die hier das niedrigste Gesamtergebnis (47,6 Punkte) von allen Gruppen erreichte. Obwohl nicht bei jedem Parameter statistisch signifikante Unterschiede herausgefunden werden konnten, kann man dennoch beobachten, dass die Patienten, bei denen keine LBS-Tenodese durchgef{\"u}hrt werden musste, tendenziell bessere Ergebnisse erzielen konnten als die Patienten „mit Tenodese". They belong to the group of fungi or bacteria and their ecological function is mostly unknown. The fungal endophytes of the genus Epichlo{\"e} belong to a special group of endophytes. Epichlo{\"e} endophytes live symbiotically inside cool season grass species and some of them are able to produce alkaloids toxic to vertebrates and insects. Their symbiosis is seen as mutualistic for the following reasons: the fungus provides the plant herbivore resistance by producing alkaloids, and it increases the plant's drought tolerance as well as its biomass production. In return, the grass provides the fungus shelter, nutrients and dispersal. Epichlo{\"e} endophytes are host specific and the ability to produce alkaloids differs between species. In order to estimate intoxication risks in grasslands, it is necessary to detect infection rates of different grass species with Epichlo{\"e} endophytes, and to determine the genotypes and chemotypes of the Epichlo{\"e} species as well as the produced alkaloid concentrations. Factors like land-use intensity or season may have an influence on infection rates and alkaloid concentrations. Also, different methodological approaches may lead to different results. In this doctoral thesis my general aim was to evaluate intoxication risks in German grasslands caused by Epichlo{\"e} endophytes. For that I investigated infection rates of different grass species and the genotypes and chemotypes of their Epichlo{\"e} endophytes in German grasslands (Chapter II). Furthermore, I compared alkaloid concentrations detected with dry and fresh plant weight and different analytical methods. I also detected possible changes on the influence of season or land-use intensity (Chapter III). Additionally, I examined infections with Epichlo{\"e} endophytes and alkaloid concentrations in commercially available grass seed mixtures and determined how that influences the intoxication risk of grazing animals in Europe (Chapter IV). It is of agricultural interest to estimate intoxication risks for grazing livestock on German grasslands due to Epichlo{\"e} infected grass species. Therefore, it is important to investigate which grasses are infected with the Epichlo{\"e} endophyte, if the endophytes have the ability to produce vertebrate and invertebrate toxic alkaloids and if the alkaloids are indeed produced. I showed that Epichlo{\"e} festucae var. lolii infecting agriculturally important Lolium perenne lacked the starting gene for ergovaline biosynthesis. Hence, vertebrate toxic ergovaline was not detected in the majority of the collected L. perenne plants. The detection of alkaloid concentrations is an important tool to estimate intoxication risk for vertebrates, but also invertebrates. My studies showed that the usage of dry plant material is crucial to quantify the correct alkaloid concentrations, and that alkaloid concentrations can vary depending on the detection method. Hence, the usage of validated, similar detection methods is important to be able to compare alkaloid concentrations from different studies. Nevertheless, the trends of seasonal changes and the influence of land-use intensity stayed the same, regardless if dry or fresh plant weight was used. Also, alkaloid concentrations were below toxicity thresholds on population level, regardless of the method used. Two commercially available forage grass and two commercially available turf grass seed mixtures were infected with Epichlo{\"e} endopyhtes and alkaloids were detected. This might contribute to the spreading of Epichlo{\"e} endopyhtes in Germany, therefore seed mixtures should be tested for Epichlo{\"e} infections. Factors contributing to delayed diagnosis include health care professionals' limited knowledge of rare diseases and frequent (co-)occurrence of mental disorders that may complicate and delay the diagnostic process. The ZSE-DUO study aims to assess the benefits of a combination of a physician focusing on somatic aspects with a mental health expert working side by side as a tandem in the diagnostic process. Study design This multi-center, prospective controlled study has a two-phase cohort design. Methods Two cohorts of 682 patients each are sequentially recruited from 11 university-based German Centers for Rare Diseases (CRD): the standard care cohort (control, somatic expertise only) and the innovative care cohort (experimental, combined somatic and mental health expertise). Individuals aged 12 years and older presenting with symptoms and signs which are not explained by current diagnoses will be included. Data will be collected prior to the first visit to the CRD's outpatient clinic (T0), at the first visit (T1) and 12 months thereafter (T2). Outcomes Primary outcome is the percentage of patients with one or more confirmed diagnoses covering the symptomatic spectrum presented. Sample size is calculated to detect a 10 percent increase from 30\% in standard care to 40\% in the innovative dual expert cohort. Secondary outcomes are (a) time to diagnosis/diagnoses explaining the symptomatology; (b) proportion of patients successfully referred from CRD to standard care; (c) costs of diagnosis including incremental cost effectiveness ratios; (d) predictive value of screening instruments administered at T0 to identify patients with mental disorders; (e) patients' quality of life and evaluation of care; and f) physicians' satisfaction with the innovative care approach. Conclusions This is the first multi-center study to investigate the effects of a mental health specialist working in tandem with a somatic expert physician in CRDs. If this innovative approach proves successful, it will be made available on a larger scale nationally and promoted internationally.
