Ab einem im Koronarangiogramm ermittelten Stenosegrad der Koronararterie von >75% wurde das entsprechende Myokardareal als minderperfundiert gewertet. Jetzt erfolgte die Korrelation mit dem Befund der Nuklearmedizin sowie mit der visuellen und berechneten MRT-Auswertung. Die visuelle Auswertung anhand der Graustufen zeigte zum Herzkatheter eine Sensitivität von 96%, eine Spezifität von 8%, zur SPECT eine Sensitivität von 82% und eine Spezifität von 6%. Bei der berechneten Auswertung erfolgte die Einteilung anhand der Perfusionsreserve. Ab einer Perfusionsreserve <1,5 wurde ein minderperfundiertes Myokard erwartet. Diese ergab zum Herzkatheter eine Sensitivität von 93% und eine Spezifität von 30%, zur SPECT eine Sensitivität von 85% und eine Spezifität von 28%. Die niedrige Spezifität in dieser Studie ist möglicherweise darauf zurückführen, dass keine beschwerdefreien Probanden untersucht wurden. In vergleichbaren Studien zeigte sich eine Reduktion der myokardialen Perfusion schon für Koronarstenosen ab 40%, wobei dieses Perfusionsdefizit in der SPECT nicht nachweisbar war. So sind möglicherweise falsch-positive MRT-Befunde tatsächlich falsch-negative Herzkatheter-Befunde und SPECT-Befunde. N2 - 55 patients with symptoms of angina pectoris were studied with MRI first-pass perfusion. MRI measurements were compared to myocardial scintigraphy and to coronary angiography. Based on coronary angiography, at a stenosis of more than 75% we assumed a decreased perfusion of the myocardium. Visual and quantitative MRI findings were correlated to the findings of the scintigraphic studies. The visual MRI findings show in comparison to coronary angiography a sensitivity of 96%, and a specifity of 8%, in comparison to myocardial scintigraphy a sensitivity of 82% and specifity of 6%. For the quantitative findings we calculated the myocardial perfusion reserve. On a myocardial perfusion reserve lower than 1,5, the myocardium was classified as being low perfused. The sensitivity in comparison to myocardial scintigraphy was 85%, specifity 28%. In comparison to angiography sensitivity was 93% and specifity 30%. The low specifity of the MRI measurements in this study might be due to the exclusion of healthy test persons without symptoms. In comparable studies a stenosis of 40% already caused a myocardial perfusion deficit which could not be detected by myocardial scintigraphy.
