In vitro-study on guinea-pig small intestine N2 - Eine schwerwiegende Nebenwirkung aller Opioide in der Therapie akuter und chronischer Schmerzen ist die Hemmung der Darmmotilität. In der vorliegenden Arbeit wird untersucht, ob und ggf. über welche Mechanismen die nichtopioidergen Analgetika Paracetamol, Metamizol, Acetylsalicylsäure und das partiell opioiderge Tramadol die Darmperistaltik beeinflussen. Die Experimente wurden an Dünndarmsegmenten des Meerschweinchens in vitro durchgeführt. Ein wesentliches Ergebnis der Arbeit ist, dass Paracetamol die Dünndarmperistaltik konzentrationsabhängig vorübergehend hemmt. Durch Vorbehandlung mit Antagonisten und Inhibitoren der vermuteten Signaltransduktionswege wurden die Mechanismen der Hemmung untersucht. Durch Naloxon und Apamin konnte die hemmende Wirkung von Paracetamol reduziert werden, was zeigt, dass enterische opioiderge Transduktionswege sowie eine Aktivierung von small conductance Ca2+-activated Kaliumkanälen beteiligt sind. Enterisches Stickstoffmonoxid (NO), die Cyclooxygenase und Serotonin spielen dabei als Transduktionsmechanismen keine Rolle. Metamizol und Acetylsalicylsäure haben keinen hemmenden Einfluß auf die Dünndarmperistaltik. Razemisches Tramadol, die beiden Enantiomere (+)- und (-)-Tramadol sowie der Hauptmetabolit O-Desmethyltramadol hemmen konzentrationsabhängig die Darmmotilität; hierbei wirkt (+)-Tramadol stärker als (-)-Tramadol, beide aber deutlich geringer als O-Desmethyltramadol.. An der Wirkung von (+)- und (-)-Tramadol sind opioiderge und adrenerge Mechanismen beteiligt, die Wirkung von O-Desmethyltramadol wird durch Bindung an Opiatrezeptoren vermittelt. N2 - Inhibition of intestinal peristalsis is a major side effect of opioid analgesics. This study examined the effect of the non-opioid analgesics acetaminophen, acetylsalicylic acid, dipyrone and of the opioid-like analgesic tramadol on intestinal peristalsis and some of the mechanisms mediating their antiperistaltic action. The results were obtained with an in vitro preparation of the isolated guinea pig small intestine The results reveal, for the first time, that acetaminophen concentration-dependently impairs intestinal peristalsis. The inhibition caused by acetaminophen involves transmitters acting via endogenous opioidergic pathways and small conductance Ca2+-activated potassium channels, transmission via enteric NO, COX or serotonin does not seem to be involved in the inhibitory action of acetaminophen. Aspirin and dipyrone are devoid of an inhibitory effect. Racemic tramadol, its (+) and (-) enantiomers as well as their major metabolite O-desmethyltramadol concentration-dependently inhibit peristalsis in the guinea pig small intestine. The inhibitory potency of (-)-tramadol to impair intestinal peristalsis is lower than that of (+)-tramadol, and the metabolite O-desmethyltramadol is more potent than either enantiomer. The action of all tramadol forms depends on opioid receptors, and that of (+)- and (-)-tramadol also involves adrenoceptors. Die genauen Mechanismen hinter der Wirkung beider Substanzen sind ungeklärt. Um die Effekte von Butyrat und Acetylsalicylsäure, sowie deren Kombination, auf das Wachstum, die Differenzierung und Apoptose von Kolonkarzinomzellen zu untersuchen, wurden Zellkulturexperimente und Western Blot Analysen durchgeführt. Unter Butyratinkubation kam es zu einer Steigerung der Differenzierung von HT-29-Kolonkarzinomzellen, jedoch nicht von Caco-2 Zellen. Acetylsalicylsäure allein hatte keinen relevanten Effekt auf die Zelldifferenzierung und in Kombination konnte Acetylsalicylsäure die differenzierende Wirkung des Butyrats nicht verstärken. Butyrat und Acetylsalicylsäure hemmten die Proliferation der untersuchten kolorektalen Karzinomzellen. Bei Inkubation mit einer Kombination von Butyrat und Acetylsalicylsäure wurde der antiproliferative Effekt der Einzelsubstanzen deutlich verstärkt. Im Western-Blot konnte diese Beobachtung durch Untersuchung des Proliferationsmarkers PCNA bestätigt werden. Gleichzeitig veränderte sich das Expressionsmuster zellzyklusregulierender Proteine in den untersuchten Zellen: Die Cyclin D3-Expression wurde in Caco-2 Zellen sowie in HT-29 Zellen durch Butyratinkubation erhöht. In Kombination mit Acetylsalicylsäure verstärkte sich die Wirkung von Butyrat auf die Expression von p21Waf1/Cip1 und cdk2: Acetylsalicylsäure und Butyrat in höheren Konzentrationen reduzierten die Expression von cdk2 in HT-29 Zellen, in niedrigen Konzentrationen führte nur die Acetylsalicylsäure-Butyrat-Kombination zu einem Abfall des cdk2-Proteingehalts. Butyrat induzierte die Expression des cdk-Inhibitors p21Waf1/Cip1 in beiden Kolonkarzinomzellinien. Die Wirkung auf p21Waf1/Cip1 war hierbei p53-unabhängig, da es gleichzeitig zu einem Abfall der Konzentration an p53-Protein in HT-29 Zellen kam und in Caco-2 Zellen kein p53 nachweisbar war. Acetylsalicylsäure hatte erst in höherer Konzentration einen Einfluss auf die p21Waf1/Cip1 Proteinexpression in HT-29 Zellen. Die Kombination von Butyrat und Acetylsalicylsäure übertraf in ihrer Wirkung die des Butyrats schon bei Konzentrationen, bei denen die alleinige Butyratinkubation ohne Effekt blieb. Weiterhin induzierte Butyrat, über einen Anstieg der Expression des pro-apoptotischen Faktors Bak, Apoptose in den untersuchten Kolonkarzinomzellen, bei gleichzeitigem Abfall des anti-apoptotischen Bcl-XL. Bei Inkubation mit der Acetylsalicylsäure-Butyrat-Kombination kam es zu einer deutlichen Verstärkung der Apoptoseinduktion, wobei Acetylsalicylsäure die Wirkung des Butyrats auf Bak und Bcl-XL jedoch nicht steigerte. Zudem konnte gezeigt werden, dass der Effekt des Butyrats mit einer Inhibition der Phosphorylierung und Degradation von IkBa, und damit einer Verhinderung der Aktivierung des Transkriptionsfaktors NFkB einhergeht, was die Wirkung von apoptose-induzierenden Substanzen verstärken kann. Zusammenfassend zeigte sich bei Kombination der Einzelsubstanzen Butyrat und Acetylsalicylsäure eine Verstärkung ihrer Wirkung auf die Zellproliferation, sowie die Apoptoseinduktion bei Kolonkarzinomzellen. Dieser Effekt wird durch unterschiedliche molekulare Mechanismen vermittelt, wobei mit p21Waf1/Cip1 und cdk2 erstmals Faktoren ermittelt wurden, auf deren Expression Butyrat und Acetylsalicylsäure additive oder potentiell synergistische Effekte haben. N2 - Numerous experimental and clinical study data support the view of a preventive effect of butyrate and aspirin on the incidence of colorectal carcinomas. The exact underlying mechanisms however remain unclear. In order to examine the effects of butyrate, aspirin and their combination on proliferation, differentiation and apoptosis in colorectal carcinoma cells, cell culture experiments and Western blot analyses were performed. Butyrate-incubation induced differentiation in HT-29 colon carcinoma cells, but not in Caco-2 cells. Aspirin did not exert relevant effects on cell differentiation and did not support the butyrate effects. Both butyrate and aspirin decreased the proliferation of the investigated cell lines. Coincubation with both substrates enhanced the antiproliferative effects of the single substances. In Western blot analyses this observation was confirmed by decreased levels of PCNA, a marker for cell proliferation. At the same time the expression pattern of cell cycle regulating factors was changed: Cyclin D3 expression was induced by butyrate in Caco-2 and HT-29 cells. In combination with aspirin the effect of butyrate on p21Waf1/Cip1 und cdk2 expression was enhanced: Aspirin and butyrate in high concentrations decreased the expression of cdk2 in HT-29 cells whereas in low concentrations only the aspirin-butyrate-combination decreased cdk2 protein. Butyrate induced the expression of the cdk-inhibitor p21Waf1/Cip1 in both cell lines. Butyrate induced p21Waf1/Cip1 independently from p53 as butyrate decreased p53 levels at the same time. Aspirin induced p21Waf1/Cip1 expression when applied at high concentrations. In combination butyrate and aspirin had a stronger effect on p21Waf1/Cip1 expression already at concentrations at which butyrate alone did not have an effect. Also butyrate induced apoptosis by inducing pro-apoptotic Bak and decreasing anti-apoptotic Bcl-XL in the investigated cell lines. When butyrate and aspirin were applied in combination the pro-apoptotic effects of butyrate were enhanced. However aspirin did not have an effect on Bak and Bcl-XL expression. Furthermore it could be shown that the butyrate effects are associated with an inhibition of phosphorylation and subsequent degradation of IkBa. This was followed by an inhibition of the activation of the transcription factor NFkB, which can support the effect of apoptosis-inducing factors. Taken together an enhanced effect of the aspirin-butyrate combination on cell proliferation and apoptosis in colorectal cancer cells was observed. This effect was mediated by different molecular mechanisms. For the first time factors could be identified, p21Waf1/Cip1 and cdk2, on which butyrate and aspirin exert additive and potentially synergistic effects.
