Their invasive nature, however, still carries an increased risk for patients' health. The development of non-invasive diagnostic tools to differentiate between bland steatosis (NAFL) and NASH remains crucial. The aim of this study is the evaluation of investigated circulating microRNAs in combination with new targets in order to optimize the discrimination of NASH patients by non-invasive serum biomarkers. Methods Serum profiles of four microRNAs were evaluated in two cohorts consisting of 137 NAFLD patients and 61 healthy controls. In a binary logistic regression model microRNAs of relevance were detected. Correlation of microRNA appearance with known biomarkers like ALT and CK18-Asp396 was evaluated. A simplified scoring model was developed, combining the levels of microRNA in circulation and CK18-Asp396 fragments. Receiver operating characteristics were used to evaluate the potential of discriminating NASH. Results The new finding of our study is the different profile of circulating miR-21 in NASH patients (p<0.0001). Also, it validates recently published results of miR-122 and miR-192 to be differentially regulated in NAFL and NASH. Combined microRNA expression profiles with CK18-Asp396 fragment level scoring model had a higher potential of NASH prediction compared to other risk biomarkers (AUROC = 0.83, 95% CI = 0.754-0.908; p<0.001). Evaluation of score model for NAFL (Score = 0) and NASH (Score = 4) had shown high rates of sensitivity (91%) and specificity (83%). Conclusions Our study defines candidates for a combined model of miRNAs and CK18-Asp396 levels relevant as a promising expansion for diagnosis and in turn treatment of NASH. This study investigates the potential roles of E-cadherin in mouse Clara cells, as these cells were shown to represent the progenitor/stem cells of the conducting airways and have been implicated as the cell of origin of human non-small cell lung cancer. Postnatal inactivation of E-cadherin affected Clara cell differentiation and compromised airway regeneration under injury conditions. In steady-state adult lung, overexpression of the dominant negative E-cadherin led to an expansion of the bronchiolar stem cells and decreased differentiation concomitant with canonical Wnt signaling activation. Expansion of the bronchiolar stem cell pool was associated with an incessant proliferation of neuroepithelial body-associated Clara cells that ultimately gave rise to bronchiolar hyperplasia. Despite progressive hyperplasia, only a minority of the mice developed pulmonary solid tumors, suggesting that the loss of E-cadherin function leads to tumor formation when additional mutations are sustained. The present study reveals that E-cadherin plays a critical role in the regulation of proliferation and homeostasis of the epithelial cells lining the conducting airways. Zusätzlich konnten im Rahmen eines Längsschnittsvergleiches Veränderungen im Vergleich zu den Wintern 2000/01 sowie 2001/02 eruiert werden. Ergebnisse: Signifikante Unterschiede im Sinne eines gehäuften Verletzungsrisikos ergaben sich für unterschiedliche Körperregionen für die verschiedenen Skitypen. Carvingskifahrer verletzten sich beispielsweise signifikant häufiger am Rumpf (p<0,05), jedoch seltener im Bereich des Kniegelenkes (p<0,05) und des Unterarms (p<0,01). Carvingskifahrer verletzten sich mit zunehmendem Fahrkönnen tendenziell häufiger im Bereich der oberen Extremität und des vorderen Kreuzbandes, während Kniegelenkstraumen insgesamt seltener beobachtet wurden. Kollisionsverletzungen waren bei Carvern nicht häufiger als bei Normalskifahrern zu beobachten. Die Verletzungsraten zeigten sich im longitudinalen Vergleich insgesamt weitgehend konstant. Schlussfolgerungen: Die verwendete Fahrtechnik (eigentliche carvende vs. driftende Fahrtechnik) scheint einen entscheidenden Einfluss auf das Verletzungsmuster auszuüben. N2 - Impact of the carving technique in skiing on orthopaedic injury patterns and the severity of injuries The aim of the study was it to identify the changes of the injury pattern in alpine skiing as a consequence of the introduction of carving skis. Therefore 1383 people who came into the emergency room of the hospital of Garmisch-Partenkirchen during the winter seasons 2003/05 were asked systematically about different parameters leading to the accident by using a standardised questionnaire. The data was evaluated statistically afterwards. In addition a longitudional comparison with the injury rates of the seasons 2000/02 has been done. Significant differences in injury risks could be identified for people using conventional skis compared to those using carving skis. Carvers suffered significantly more often from injuries of the torso (p<0,05),but less often from those of the knee (p<0,05) or the forearm (p<0,01). Carving skiers with better skills tended to gain more injuries of the upper extremity and the anterior cruciate ligament (ACL) while knee trauma in total has been observed less often in more skilled athletes. Collision accidents did not occur more often in carvers than in conventional skiers. The injury rates in total remained widely constant in the longitudional comparison. The actual technique of skiing (carving vs. drifting) seems to play an important role on the injury pattern occuring. The impacts of carving skis on orthopaedic injury patterns should be observed furthermore. In the murine experimental autoimmune encephalomyelitis (EAE) model, antagonism of LIF and genetic deletion of CNTF worsen disease. The potential mechanism of action of these cytokines in EAE is complex, as gp130 is expressed by all neural cells, and could involve immuno-modulation, reduction of oligodendrocyte injury, neuronal protection, or a combination of these actions. In this study we aim to investigate whether the beneficial effects of CNTF/LIF signalling in EAE are associated with axonal protection; and whether this requires signalling through oligodendrocytes. We induced MOG\(_{35-55}\) EAE in CNTF, LIF and double knockout mice. On a CNTF null background, LIF knockout was associated with increased EAE severity (EAE grade 2.1\(\pm\)0.14 vs 2.6\(\pm\)0.19; P<0.05). These mice also showed increased axonal damage relative to LIF heterozygous mice, as indicated by decreased optic nerve parallel diffusivity on MRI (1540\(\pm\)207 \(\mu\)m\(^2\)-/s vs 1310\(\pm\)175 \(\mu\)m\(^2\)-/s; P<0.05), and optic nerve (-12.5%) and spinal cord (-16%) axon densities; and increased serum neurofilament-H levels (2.5 fold increase). No differences in inflammatory cell numbers or peripheral auto-immune T-cell priming were evident. Oligodendrocyte-targeted gp130 knockout mice showed that disruption of CNTF/LIF signalling in these cells has no effect on acute EAE severity. These studies demonstrate that endogenous CNTF and LIF act centrally to protect axons from acute inflammatory destruction via an oligodendrocyte-independent mechanism. Dazu wurde in den medizinischen Datenbanken Medline und Pubmed nach relevanten Studien recherchiert. Nach der Literaturselektion entsprechend festgelegter Auswahlkriterien konnten 44 Studien im Zeitraum von 1962 bis 2005 in die Bewertung einfließen. Diese wurden der Evidenzhierarchie nach der Cochrane Collaboration zugeordnet. Entsprechend der Evidenzstärken und der kritischen Beurteilung der externen und internen Validität wurden die einzelnen Stabilisierungshilfen bewertet. Dabei zeigt sich, dass ältere, weit verbreitete und langzeiterprobte Maßnahmen wie der adhäsive Tape- Verband innovativeren und ausbaufähigen Methoden wie dem propriozeptiven Training weichen. In diesem sensomotorischen Bereich konnten übereinstimmend positive und größtenteils signifikante Ergebnisse ermittelt werden. Auch die Anwendung semirigider und rigider Orthesen zeigte bei der Mehrzahl der Studien einen signifikanten Supinationsschutz. Der präventive Effekt von (Schnür-) Bandagen äußerte sich vornehmlich in der Verbesserung der propriozeptiven Fähigkeiten vor allem instabiler Sprunggelenke. Beim Tape-Verband steht die initiale signifikante Supinationsrestriktion im Vordergrund, was unter anderem mit den Materialeigenschaften sowie vielfältigen und eingeschränkt reproduzierbaren Techniken begründet wird. A total of n = 57 competitive athletes of the German powerlifting federation completed a retrospective survey regarding acute andoveruse injuries. With 224 total injuries, a mean incidence of 1.51 per 1.000 h or 0.49 per year was calculated. Most injuries affected the lower back (20.5%), elbow (11.2%), pelvic region (10.3) and the shoulder (9.8%). Regarding the type of injury acute inflammation (25.9%), muscle strains/sprains (20.5%) and skin lesions (13%) dominated. The mean incidence significantly declined with increasing age and training experience of the athlete. Athletes using a bench press shirt and various regenerative methods like sauna or swimming also showed decreased injury rates. There was no significant correlation between body weight, height or gender and injury incidence. Compared to other sports, the incidence of injuries and overuse syndromes is still low in powerlifting. Die Unberechenbarkeit von Wind, Wasser und anderen Sportlern bergen ein Risiko. In der vorliegenden Arbeit wurden Verletzungen von Windsurfern, Wellenreitern und Kitesurfern im Zeitraum von Mai 2011 bis September 2012 auf Fuerteventura durch erstbehandelnde Ärzte erfasst. Es wurden insgesamt 90 Verletzte, 51 Windsurfer, 13 Surfer und 26 Kitesurfer behandelt. Hierbei zeigte sich, dass sich die meisten Sportler lediglich leichte Verletzungen zuzogen. Schnittwunden dominierten bei Surfern deutlich, diese vor allem im Bereich von Kopf und Hand. Kitesurfer stellten sich vor allem wegen Frakturen vor. Die Verletzungen ereigneten sich eher im Bereich der oberen Extremität und des Kopfes. Windsurfer zogen sich, wie auch in den meisten vorbestehenden Studien, Schnittwunden der unteren Extremität zu. Um einen Vergleich mit einem größeren Kollektiv zu ermöglichen, wurde zwischen dem 30.07.2011 und 30.09.2012 ein bilingualer Onlinefragebogen bereitgestellt und über verschiedene Medien beworben. Dabei wurden Daten von 653 Teilnehmern erhoben (453 Windsurfer, 137 Kitesurfer und 63 Wellenreiter).Hier wurden weitergehende Fragen über Surfmaterial, Verletzungsfolgen und andere Begleitumstände gestellt, die nur retrospektiv erhoben werden konnten, oder den Zeitrahmen einer medizinischen Erstbehandlung deutlich überschritten hätten. Hier zeigte sich durch die Einbeziehung der leichteren Verletzungen ein anderes Bild. Kopfverletzungen wurden unter Windsurfern signifikant häufiger genannt. Kitesurfer gaben in der Umfrage hauptsächlich Verletzungen der unteren Extremitäten an. Hierbei fielen vor allem Verletzungen im Bereich des Bandapparates des Knies auf. Länger andauernde Beschwerden gaben Sportler unserer Befragung kaum an. Dauerhafte Beschwerden kamen nur als Einzelnennungen vor. Schwere Verletzungen bildeten die Ausnahme. Insgesamt scheinen die Sportarten Kitesurfen, Windsurfen und Wellenreiten zumindest im Breiten- bzw. Amateursportbereich unseren Daten zufolge nur mit moderaten Gefahren für die Sportler verbunden zu sein. Erfasst wurden 212 Patienten im Alter von 0 bis 16 Jahren betrachtet, die im Zeitraum vom 01.01.2004 bis zum 31.12.2009 auf Grund einer thermischen Verletzung stationär im Universitätsklinikum Würzburg der Julius-Maximilians-Universität Würzburg behandelt wurden. Den größten Anteil thermischer Verletzungen im Kindesalter stellen Verbrühungen dar. Betroffen sind vor allem Kleinkinder. Verbrennungen finden sich häufiger bei älteren Kindern und Jugendlichen. Jungen sind gefährdeter als Mädchen solche Verletzungen zu erleiden. Verbrühungen treten vermehrt gegen Ende des Jahres auf, während Verbrennungen in den Sommermonaten gehäuft vorkommen. Betroffen ist zumeist die obere Körperhälfte, wobei Verbrühungen meist Brust, Arme und Beine verletzen, Verbrennungen meist Gesicht und Hände. II°- und III°-Verletzungen haben die gleiche Altersverteilung und sind gleich häufig. Die durchschnittliche Krankenhausverweildauer ist bei Verbrennungen höher als es bei Verbrühungen der Fall ist. N2 - On the basis of an retrospective data analysis distribution pattern of burns and scalds should be investigated based on age and gender. 212 patients were mapped in the age of 0 to 16 years, which were treated residential in the university hospital of the Julius-Maximilians-University Würzburg between 01.01.2004 and 31.12.2009. The majority of thermic injuries in childhood are scalds. Affected are primarily infants. Burns are more common with elder children and adolescents. Boys are more endangered than girls to endure such injuries. Scalds occur more likely during winter, whereas burns occur more likely during summer. The upper body is most likely to be affected, whereat scalds mostly chest, arms and legs hurt, burns mostly face and hands. II°- and III°-injuries have the same age pattern and are equal frequent. The average length of stay is with burns higher than it is the case with scalds. Not every III°-injury needs skin graft. The aim of this study was to identify and compare the injury profiles and compare the injury profiles of these three extreme water sports. Materials and Methods These data for this retrospective cohort study were collected through an online standardised questionnaire during the 2017–18 season. The questionnaire included questions about anthropometry, skill level, injury diagnosis, injury mechanism, environmental conditions and training regimes. Results The 626 athletes included reported 2584 injuries. On average, each athlete sustained 4.12 injuries during the season. The most frequent injury location was in the lower extremity, in particular the foot, with 49 (16.4%) injuries in surfing, 344 (18.3%) in wind surfing and 79 (19.7%) in kite surfing. Surfing demonstrated a particularly high rate of head injuries (n = 37; 12.4%). Other frequent injury types were skin lesions (up to 42.1%) and contusions (up to 40.5%). The most common injury across all surfing sports was skin lesions of the foot (wind surfing: 11.7%; kite surfing: 13.2%; surfing: 12.7%). In surfing, skin lesions of the head were frequently observed (n = 24; 8.0%). In surfing, a ‘too large wave’ (n = 18; 24.7%) was main cause of the injury, while in wind surfing (n = 189; 34.5%) and kite surfing (n = 65; 36.7%) ‘own incompetence’ led to the most injuries. Conclusion This unique study compares injury epidemiology and mechanism in the three most popular surfing sports: wind surfing, kite surfing and surfing. However, the validation of this study protocol is lacking. Therefore, this study aimed to identify the validity of injury surveillance in men's professional team sports based on media reports. Methods In a retrospective cohort study, the validity of media-data-relating injuries was investigated in participating teams of the highest two German divisions in men's professional basketball (BB) and handball (HB) in the season 2018/2019. Injury protocols completed by the team physicians were compared to those of sports media injury reports. Results The study population was composed of 133 athletes (54 BB and 79 HB). Of 343 injuries reported by the team physicians, 151 (44%) could be identified by means of sports media reports. Severe injuries (n = 75, 72%) were reported more likely in sports media compared to less severe injuries (n = 76, 32%, p < 0.00001). Odds ratio (OR) was 5.33 (95% CI 3.22-8.82). No differences regarding injury reporting could be seen between the two team sports. Conclusion For severe injuries, media analysis may be a sufficient method for data collection in popular men's professional ball sports. An underestimation of true injury prevalence lies within the range of previous reported investigations concerning the validation of injury surveillance methods. Non-severe injuries could not be verified via media analysis in professional handball and basketball.
