Material and Methods: In a total of 158 subjects, 92 suffering from severe periodontitis and 66 matched periodontal healthy controls, PWV, AIx, central and peripheral blood pressure were recorded using an oscillometric device (Arteriograph). Results: Subjects suffering from severe chronic or aggressive periodontitis exhibited significantly higher PWV (p = 0.00004), higher AIx (p = 0.0049) and lower PPA (p = 0.028) than matched periodontal healthy controls. Conclusions: The results of this study confirm the association between periodontal inflammation and increased cardiovascular risk shown by impaired vascular health in case of severe periodontitis. As impaired vascular health is a common finding in patients suffering from severe periodontal disease a concomitant routine cardiovascular evaluation may be advised. Materials and Methods: Forty-five healthy women (24 test/21 placebo) with pregnancy gingivitis in the third trimester of pregnancy were enrolled. At baseline Gingival Index (GI) and Plaque Index (PlI) were assessed at the Ramfjord teeth and venous blood taken for TNF-alpha analysis. Subsequently participants were randomly provided with lozenges to be consumed 2 9 daily until birth (approx. 7 weeks) containing >= 10(8) CFU L. reuteri ATCC PTA 5289 and >= 10(8) CFU L. reuteri DSM 17938 (test) or being devoid of L. reuteri (placebo). Within 2 days after birth recording of GI, PlI and blood sampling were repeated. Results: At baseline, mean GI and mean PlI did not differ significantly between both groups. In the test group mean TNF-alpha serum level was significantly (p < 0.02) lower than in the placebo group. At reevaluation, mean GI and mean PlI of the test group were both significantly (p < 0.0001) lower than in the placebo group. Mean TNF-alpha serum level did no longer differ significantly between the groups. Conclusions: The consumption of L. reuteri lozenges may be a useful adjunct in the control of pregnancy gingivitis. N2 - The purpose of this study was to determine an association between periodontitis and cardiovascular disease. Therefore we analysed the subgingival microbiom of 73 study subjects with periodontitis before and after systematic periodontal therapy and the effect of periodontal therapy on arterial stiffness parameters e.g. pulse-wave velocity and augmentation index, which are indicators for cardiocascular risks. It can be concluded that after periodontal therapy numbers of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis and Tannerella forsythia in the subgingival microbiom have been significantly decreased. There was no significant correlation between the observed reduction of the periodontal pathogens and the changes of the arterial stiffness parameters. Die allgemeinen Lebensumstände der untersuchten Probanden waren dabei jedoch in der Regel wenig standardisiert, so dass eine mögliche Beeinträchtigung der Validität der Studiendaten durch nicht kontrollierte externe Faktoren wie etwa Lebensstil oder Ernährung bislang nicht ausgeschlossen werden konnte. Daher war es das Ziel dieser prospektiven, randomisierten, doppelt verblindeten und placebokontrollierten Interventionsstudie über einen Beobachtungszeitraum von sechs Wochen die Auswirkungen des täglichen Konsums probiotischer, L. reuteri-haltiger Lutschtabletten auf Parameter der oralen Gesundheit von 72 Besatzungsmitgliedern einer Fregatte der Deutschen Marine zu evaluieren, die während einer Einsatzvorbereitung in See unter weitgehend vergleichbaren Lebens- und Ernährungsbedingungen ihren Dienst versahen. Zu Studienbeginn, sowie nach zwei und sechs Wochen wurden an den Ramfjordzähnen (Zähne 16, 21, 24, 36, 41, 44) der Probanden die Anzahl der auf Sondierung blutenden Zahnfleischtaschen (BoP) als primärem Studien-endpunkt erfasst. Darüber hinaus wurden als sekundäre Endpunkte die Taschensondierungstiefe (PPD), das klinische Attachmentniveau (CAL), der Gingival-Index (GI) und der Plaque Control Record (PCR) aufgezeichnet. Mit Hilfe einer doppelt verblindeten Zuteilungsstrategie wurden die Probanden zufällig der L. reuteri-Gruppe (n=36) oder der Placebogruppe (n=36) zuge-ordnet. Sie erhielten nachfolgend einen für die Studiendauer ausreichenden Vorrat an L. reuteri- oder Placebo-Lutschtabletten mit der Anweisung, diese in den nächsten sechs Wochen zweimal täglich zu konsumieren. 30 Probanden der L. reuteri-Gruppe sowie 32 Probanden der Placebogruppe beendeten die Studie mit vollständig erfassten Datensätzen. Ihre Analyse enthüllte für die L. reuteri-Gruppe einen signifikanten (p<0,001) Rückgang der beobachteten BoP-Mittelwerte von initial 41% (±22 SD) aller erfassten Messstellen auf 10 % (±13 SD) nach sechs Wochen. In der Placebogruppe hingegen kam es während des Beobachtungszeitraums zu einer signifikanten (p=0.05) Zunahme der BoP-Mittelwerte gegenüber der Ausgangssituation von initial 37 % (±20 SD) auf 43 % (±17 SD) am Studienende. Auch bezüglich aller sekundären Endpunkte (PPD, CAL, GI, PCR) konnte in der L. reuteri-Gruppe eine signifikante Verbesserung der oralen Gesundheit zwischen Studienbeginn und Studienende beobachtet werden, während sich wiederum in der Placebo-Gruppe im Beobachtungszeitraum eine statistisch verifizierbare Verschlech¬terung aller erfassten sekundären Endpunkte ergab. Die Ergebnisse dieser unter weitgehend kontrollierten Lebens- und Ernährungsbedingungen durchgeführten Untersuchung belegen, dass der regelmäßigem Konsum probiotischer, L. reuteri-haltiger Lutschtabletten unter den Einsatzbedingungen in See nicht nur eine in der Placebogruppe beobachtete Verschlechterung der oralen Gesundheit verhinderte, sondern diese vielmehr im Vergleich zum Ausgangsbefund signifikant verbesserte. Der adjunktive Konsum L. reuteri-haltiger Lutschtabletten könnte daher eine kostengünstige und einfach zu implementierende Maßnahme darstellen, um einer unter militärischen Einsatzbedingungen häufiger zu beobachtenden Verschlechterung der oralen Gesundheit wirksam vorzubeugen. N2 - The inhibitory effect of the regular consumption of probiotic Lactobacillus reuteri strains on the occurrence of oral inflammations has been established by numerous clinical intervention trials. In general however social ,dietary and environmental conditions of the study subjects were not well controlled, so that so far, it could not be precluded, that the validity of the recorded data may have been possibly impaired by external interference factors like lifestyle and nutrition. Therefore it was the aim of this prospective, randomized, double-blind placebo controlled intervention trial to evaluate the impact of the daily consumption of probiotic L. reuteri-containing lozenges on parameters of oral health in 72 crew members of a frigate of the German Navy over an observation period of 6 weeks during a practicing mission at sea under broadly comparable living and dietary conditions. At baseline, as well as after two and six weeks the number of periodontal pockets showing bleeding on probing (BoP) on the Ramfjord teeth (16, 21, 24, 36, 41, 44) were recorded as the primary study outcome. Additionally pocket probing depth (PPD), periodontal attachment level (PAL), Gingival Index (GI) and Plaque Control Record (PCR) were recorded as secondary study outcomes. Using a computer generated random list study subjects were randomly assigned to either the L. reuteri (n=36) or the placebo group (n=36), provided with a supply of experimental L. reuteri- or placebo lozenges and instructed to consume them twice daily for the next 6 weeks. 30 subjects of the L. reuteri and 32 subjects of the placebo group finished the study with complete data sets. For the L. reuteri group statistical analysis revealed a significant (p<0.001) reduction of the observed percentage of BoP positive sites from 41% (±22 SD) at baseline to 10% (±13) at 6 weeks. In the placebo group by contrast a significant increase (p<0.05) of the percentage of BoP positive sites from 37% (±20 SD) at baseline to 43% (±17%) at 6 weeks was recorded. Also regarding the secondary study outcomes (PPD, CAL, GI, PCR) a significant improvement of all assessed parameters was observed in subjects of the L. reuteri group, while in the placebo group a significant deterioration was recorded. The results of this intervention trial performed under broadly controlled comparable living and dietary conditions revealed, that the regular consumption of probiotic L-reuteri-containing lozenges during a practicing mission at sea not only prevented a deterioration of oral health as documented for the placebo group but even improved the status of oral health when compared to baseline values. The adjunctive consumption of L.reuteri-containing lozenges may therefore present a cost-efficient and easy implementable measure for the prevention of oral health deterioration under military operational conditions. Ziel der Studie war die Quantifizierung des Gewinnes an KG sowie die Abhängigkeit des Behandlungsergebnisses von verschiedenen patientenbezogenen Faktoren. MATERIAL UND METHODEN: Die Studienpopulation bestand aus 13 Patienten mit insgesamt 31 Implantaten, bei allen wurde eine Augmentation der KG mittels des ARFs durchgeführt. Es wurden vier verschiedene Messungen der Breite der KG durchgeführt: präoperativ, postoperativ, zum Zeitpunkt der endgültigen prothetischen Versorgung und die durch die orale Inzision mobilisierte Breite der KG. Neben den absoluten Werten wurde die Abhängigkeit des Ergebnisses von verschiedenen patientenbezogenen Faktoren (Alter, Geschlecht, Einheildauer; Position des Implantates) ausgewertet. ERGEBNISSE: Die Breite der KG postoperativ (+3,94 mm, p < 0,0001) und zum Zeitpunkt der prothetischen Restauration (+3,55 mm, p < 0,0001) haben sich signifikant im Vergleich zu den Werten präoperativ erhöht. In der Einheilungsphase kam es zu einer signifikanten Schrumpfung der KG (-0,39 mm, p < 0,0421). Beim Vergleich der verschiedenen Gruppen konnten keine statistisch signifikanten Zusammenhänge festgestellt werden. N2 - OBJECTIVE: The apically relocated flap (ARF) is a surgical procedure to augment keratinized gingiva (KG) around dental implants. The purpose of this study was the quantification of the expected gain of KG as well as the reliance between different patient specific values and the gain of KG. METHODS AND MATERIALS: The study population included 13 patients with a total of 31 dental implants, all of which were uncovered by the technique of the ARF. Four different measurements were performed in order to take the width of the KG: preoperative, postoperative, at the time of the incorporation of the prosthetic restauration as well as at the time of the oral incision. RESULTS: A statistically significant gain of KG postoperative (+3,94 mm, p < 0,0001) and at the time of the incorporation of the prosthetic restauration (+3,55 mm, p < 0,0001) compared to the width of KG preoperative could be found. During healing time, a shrinkage of the KG (- 0,39 mm, p < 0,0421) could be proven. No statistically significant reliance between groups of patient specific values and the gain of KG could be found. CONCLUSIONS: The apically relocated flap is a reliable surgical procedure to augment KG around dental implants. Ziel der vorliegenden Arbeit war es daher zu untersuchen, ob die Bestimmung des gingivalen Biotypes über die Transluzens einer Parodontalsonde durch die bukkale Gingiva, als eine einfach durchzuführende klinische Untersuchung, von der tatsächlichen Dicke des Weichgewebes abhängt. Darüber hinaus wurde erörtert, ob verschiedene parodontale Parameter eher mit einem dicken oder einem dünnen gingivalem Biotyp vergesellschaftet sind. Zuletzt wurden die Studienteilnehmer auf eine bestehende Relation zwischen dem Biotyp des Zahnfleisches und der Form der Frontzahnkronen des Oberkiefers hin untersucht. Zu diesem Zweck wurden an 36 Probanden der gingivale Biotyp über die Transluzens einer Parodontalsonde durch die bukkale marginale Gingiva bestimmt. Anschließend wurde die tatsächliche Dicke der Gingiva auf Höhe des gingivalen Sulkus mit einer individualisierten Messlehre ermittelt. Des weiteren wurden gingivale Parameter (Taschentiefe, Breite der keratinisierten Mukosa, Papillenhöhe) erhoben, sowie die Form der Zahnkronen der Schneidezähne im Oberkiefer anhand von Gipsmodellen bestimmt. Die erhobenen Messwerte wurden anschließend auf Unterschiede zwischen den beiden Gruppen untersucht. Um eine Abhängigkeit von der tatsächlich gemessenen Gewebedicke zu erörtern, wurden zwei Extremgruppen aus den Probanden mit den jeweils sechs höchsten, beziehungsweise niedrigsten Messwerten gebildet. Die statistische Auswertung stellt die Transluzens einer Parodontalsonde durch die bukkale marginale Gingiva als verlässliches Mittel zur Einschätzung des gingivalen Biotyps heraus, wobei anzumerken ist, dass die Sichtbarkeit der Sonde durch die Gingiva nicht ausschließlich von der Gewebedicke beeinflusst wird. Darüber hinaus konnte eine größere Kronenlänge und, äquivalent dazu, eine höhere mesiale Papille für den dicken gingivalen Biotyp dargestellt werden. Bei dem Vergleich der Extremgruppen konnte ausserdem eine signifikant höhere Taschensondierungstiefe und eine breitere befestigte Gingiva für die Gruppe mit einem sehr dicken Gewebe aufgezeigt werden. N2 - The determination of the gingival biotype is an important aid for the dentist to choose the appropriate therapy for pathogenic findings. It represents also a relevant factor for the prognosis of a dental treatment. The aim of the present study was to investigate if the determination of the gingival biotype, by assessing the translucency of a periodontal probe through the buccal gingiva is correlated to the actual thickness of the tissue. In addition several periodontal parameters were analyzed for a relation to a thick or thin gingival biotype. Finally the study population was investigated for a relation of the gingival biotype and the shape of the clinical crown of the incisors in the upper jaw. For this purpose the gingival biotype was determined at 36 subjects by assessing the translucency of a periodontal probe through the buccal gingiva. Subsequently the true gingival thickness was measured with an individualized caliper at the level of the gingival sulcus. In addition gingival parameters (probing depth, width of the attached mucosa, height oft he papilla) were collected and the shape if the incisal crowns in the upper jaw were analyzed using plaster casts from the subjects. The collected data was evaluated for differences between the two groups. To highlight the dependence oft he true gingival thickness, two extreme groups were generated with the six highest and lowest values for the measured gingival thickness. The statistical analysis presents the translucency of a periodontal probe through the buccal gingiva as a reliable and easy way to determine the gingival biotype on a subject. However it has to be considered that the visibility oft he instrument is not solely dependent of the tissue thickness. Additionally a higher crown length and equivalent a greater papilla height was accompanied with a thick gingival biotype. Analyzing the two extreme groups a greater probing depth and a wider band of the attached gingiva could be illustrated for the group with a very thick tissue. Material and methods The periodontal status of 270 patients with moderate CKD randomly selected from a cohort of 5,217 subjects participating in the prospective observational German Chronic Kidney Disease (GCKD) project was analysed by recording bleeding on probing (BOP), probing pocket depth (PPD) and clinical attachment level (CAL). Furthermore, the awareness of the study subjects of their periodontal conditions was evaluated by a self‐reported questionnaire. Results 24.4% of the CKD study patients showed no or only mild signs of periodontal disease, 47.6% displayed moderate and 27% severe periodontitis. Questionnaire data revealed that 62.3% of the study subjects with severe periodontitis were not aware of the presence of the disease, 44.4% denied having received any systematic periodontal therapy so far, although 50% of them indicated to visit their dentist regularly for professional tooth cleanings. Conclusion While the clinical study data confirm an increased prevalence of periodontitis in CKD patients, their self‐reported awareness of periodontitis was low. At baseline, at day 14 and day 42 bleeding on probing (primary outcome), gingival index, plaque control record, probing attachment level, and probing pocket depth were assessed at the Ramfjord teeth. Results At baseline there were no significant differences between the groups. At day 14 and day 42 test group scores of all assessed parameters were significantly improved (P < 0.001) compared to baseline and to the placebo group which by contrast showed a significant (P < 0.001) deterioration of all parameters at the end of the study. Conclusions The consumption of probiotic L. reuteri‐lozenges is an efficacious measure to improve and maintain periodontal health in situations with waning efficacy of personal oral hygiene. Materials and methods This exploratory subanalysis includes patients from the ABPARO‐trial (ClinicalTrials.gov NCT00707369). The per‐protocol collective provided untreated periodontitis patients with high (≥28 kg/m\(^{2}\)) or moderate (21–24 kg/m\(^{2}\)) BMI. Out of the per‐protocol collective, 80 patients were randomly selected and stratified for BMI group, sex, and treatment group (antibiotics/placebo), resulting in 40 overweight and normal‐weight patients. Patients received non‐surgical periodontal therapy and maintenance at 3‐month intervals. Plasma samples from baseline and 27.5 months following initial treatment were used to measure the concentrations of ORM, hsCRP, chemerin, and RBP4. Results At the 27.5‐month examination, ORM and hsCRP decreased noticeably in the overweight group (ORM: p = .001, hsCRP: p = .004) and normal‐weight patients (ORM: p = .007, hsCRP: p < .001). Chemerin decreased in the overweight group (p = .048), and RBP4 concentrations remained stable. Conclusion Non‐surgical periodontal therapy reduced systemically elevated inflammation‐associated biomarkers in periodontitis patients. These improvements were more pronounced in overweight patients than in normal‐weight patients. Der Einfluss der Reflexion und / oder Streuung an der Oberfläche muss als Absorptionsgrad α miteinbezogen werden. Der Absorptionsgrad α ist von der Oberflächenbehandlung und dem Medium zwischen Lichtleiter und Keramikoberfläche abhängig. Die Durchführung einer multiplen linearen Regressionsanalyse zeigt, dass die Schichtstärke, Transluzenz, Helligkeitsstufen und die Farben einen signifikanten Einfluss auf den Transmissionsgrad haben. Den stärksten Einfluss hat die Keramikschichtstärke. Der Transmissionsgrad der Farben B und D unterscheidet sich nicht signifikant vom Transmissionsgrad der Farbe A. Dagegen weist die Keramik der Farbe Bleach einen signifikant höheren und die Keramik der Farbe C einen signifikant niedrigeren Transmissionsgrad auf. Der Absorptionsgrad der glasierten Oberfläche und der Absorptionsgrad bei Belichtung durch die Zwischenmedien ist im Vergleich zum Absorptionsgrad einer gesägten Oberfläche, bzw. bei Belichtung ohne Zwischenmedium signifikant niedriger. N2 - This study examines the transmittance of visible blue light through lithium disilicate reinforced ceramics as a function of the layer thickness, color and translucency. Furthermore, the influence of the surface treatment and the intermediate medium between the light guide and the ceramic surface was investigated. For this purpose, flat samples with a layer thickness of 1.25 mm, 2.5 mm, 5 mm, 7.5 mm of the ceramic IPS e.max CAD were sawn with a saw microtome and then crystallized. The transmittance of the light from an LED light curing unit was measured using a fiber optic spectrophotometer. The thicker, more opaque and darker a ceramic sample, the lower its transmittance. The decrease in the degree of transmission could be described by the Lambert-Beer law. The influence of the reflection and / or scattering on the surface must be included as the degree of absorption α. The degree of absorption α depends on the surface treatment and the medium between the light guide and the ceramic surface. A multiple linear regression analysis shows that the layer thickness, translucency, brightness levels and the colors have a significant influence on the degree of transmission. The thickness of the ceramic layer has the greatest influence. The transmittance of colors B and D does not differ significantly from the transmittance of color A. In contrast, the ceramics of the bleach color have a significantly higher and the ceramic of the color C a significantly lower transmittance. The degree of absorption of the glazed surface and the degree of absorption when exposed to light through the intermediate media is significantly lower compared to the degree of absorption of a sawn surface or when exposed to light without an intermediate medium. In contrast, the difference between the degree of absorption of the polished surface and the degree of absorption of the sawn surface is barely significant when averaging the tests with or without intermediate media. Material and Methods We identified three relevant PICO questions related to soft tissue augmentation procedures and conducted a systematic search of four major electronic databases for clinical studies in systemically healthy patients receiving at least one dental implant and a minimum follow-up of one year after implant placement. The primary outcome was mean difference in marginal bone levels, and secondary outcomes were clinical and patient-related outcomes such as thickness of peri-implant mucosa, bleeding indices, and Pink Esthetic Score. Results We identified 20 publications reporting on 16 relevant comparisons. Studies varied considerably and thus only two meta-analyses could be performed. This systematic review showed that: Soft tissue augmentation either for augmentation of keratinized mucosa or soft tissue volume inconsistently had an effect on marginal bone level changes when compared to no soft tissue augmentation, but consistently improved secondary outcomes. The combination soft and hard tissue augmentation showed no statistically significant difference in terms of marginal bone level changes when compared to hard tissue augmentation alone, but resulted in less marginal soft tissue recession as shown by a meta-analysis. Soft or hard tissue augmentation performed as contour augmentations resulted in comparable marginal bone level changes. Conclusions Peri-implant soft and hard tissues seem to have a bidirectional relationship: “Bone stands hard, but soft tissue is the guard”. A popular combination is Deproteinized Bovine Bone Mineral (DBBM) placed with a native collagen membrane (NCM) tacked to native bone. In this study, we compared the outcome of this treatment option to those achieved with three different graft/membrane combinations with respect to total newly occupied area and the mineralized compound inside. After bi-lateral extraction of two mandibular premolars in five adult beagles L-shaped alveolar defects were created. A total of 20 defects healed for 6 weeks resulting in chronic type bone defects. At baseline, four options were randomly allocated to five defects each: a. DBBM + NCM with a four-pin fixation across the ridge; b. DBBM + RCLC (ribose cross-linked collagen membrane); c. DBBM + NPPM (native porcine pericardium membrane); and d. Ca-sulfate (CS) + RCLC membrane. Membranes in b/c/d were not fixed; complete tensionless wound closure was achieved by CAF. Termination after 3 months and sampling followed, and non-decalcified processing and toluidine blue staining were applied. Microscopic images obtained at standardized magnification were histomorphometrically assessed by ImageJ software (NIH). An ANOVA post hoc test was applied; histomorphometric data are presented in this paper as medians and interquartile ranges (IRs). All sites healed uneventfully, all sites were sampled and block separation followed before Technovit embedding. Two central sections per block for each group were included. Two of five specimen were lost due to processing error and were excluded from group b. New bone area was significantly greater for option b. compared to a. (p = 0.001), c. (p = 0.002), and d. (p = 0.046). Residual non-bone graft area was significantly less for option d. compared to a. (p = 0.026) or c. (p = 0.021). We conclude that collagen membranes with a prolonged resorption/barrier profile combined with bone substitutes featuring different degradation profiles sufficiently support new bone formation. Tacking strategy/membrane fixation appears redundant when using these biomaterials. Materials and methods For this randomized, placebo-controlled, double-blind, parallel group, multi-center phase 3 study, 201 healthy adults were recruited. After baseline recording of plaque index (PI) and gingival index (GI), collection of salivary samples, and dental prophylaxis, subjects were randomly assigned to OCT or placebo mouthwash in a 3:1 ratio. Rinsing was performed twice daily for 30 s. Colony forming units in saliva were determined before and after the first rinse. At day 5, PI, GI, and tooth discoloration index (DI) were assessed. Non-parametric van Elteren tests were applied with a significance level of p < 0.05. Results Treatment with OCT inhibited plaque formation more than treatment with placebo (PI: 0.36 vs. 1.29; p < 0.0001). OCT reduced GI (0.04 vs. placebo 0.00; p = 0.003) and salivary bacterial counts (2.73 vs. placebo 0.24 lgCFU/ml; p < 0.0001). Tooth discoloration was slightly higher under OCT (DI: 0.25 vs. placebo 0.00; p = 0.0011). Mild tongue staining and dysgeusia occurred. Conclusions OCT 0.1% mouthwash inhibits plaque formation over 5 days. It therefore can be recommended when regular oral hygiene is temporarily compromised. Clinical relevance When individual plaque control is compromised, rinsing with octenidine mouthwash is recommended to maintain healthy oral conditions while side effects are limited. Findings were compared with national reference data (DMS V survey) reporting oral health status in age‐related controls. Within‐group comparisons were made between the HPP patients harbouring one versus two alkaline phosphatase liver/bone/kidney type (ALPL) gene variants. Results Of 80 HPP patients (64 female) with a mean age of 46.4 years (range 24–78) and one (n = 55) or two (n = 18) variants (n = 7 lacking testing) within the ALPL gene, those with two variants displayed substantially higher tooth loss rate (14.0 ± 9.3) than those affected by only one ALPL variant (4.1 ± 5.4), who did not differ substantially from healthy DMS V controls. While DMFT score and severe periodontal diseases (PDs) of HPP patients with one variant only increased with progressing age, the two‐variant sub‐cohort age independently exhibited increased DMFT scores and a higher rate of severe PDs. Conclusions HPP patients affected by two variants of the ALPL gene exhibited a higher risk of periodontitis and tooth loss than the general population, while patients with one variant developed clinically relevant oral disease symptoms with progressing ageing. Materials and methods Eighty bovine teeth cut to a length of 18 mm (crown 8 mm, root 10 mm) were randomly assigned to 10 groups (n = 8), receiving orthograde apical plug treatment (APT). Apical plugs were 4 mm in length and made of ProRoot MTA (Dentsply), Medcem MTA (Medcem), TotalFill BC RRM Fast Set Putty (Brasseler), or Medcem Medical Portland Cement (Medcem) plus bismuth oxide (Bi2O3) with and without bovine blood. Further, orthograde (with or without preoperative adhesive coronal dentin sealing) and retrograde APT were compared. Teeth were obturated with gutta-percha and sealer, sealed with composite and stored in distilled water. Tooth color was measured on apical plug, gutta-percha/sealer, and crown surface before treatment versus 24 h, 1, 3, 6, 12, and 24 months after treatment by spectrophotometry. Color difference (ΔE) values were calculated and analyzed by Shapiro-Wilk test, ANOVA with post hoc tests, Friedman test, t test, and post hoc tests with Bonferroni correction (α = .05). Results Tooth discoloration occurred in all groups with no significant differences between HCSCs (p > .05). After 24 months, color changes were prominent on roots but insignificant on crowns. Blood contamination induced a significantly decreased luminescence (p < .05). Blood had a stronger impact on tooth color than Bi2O3. No relevant effects of retrograde placement (p > .05) or preoperative dentin sealing (p > .05) were detected. Conclusions Apical plugs of the tested HCSCs cause discoloration of bovine roots, but not discoloration of bovine tooth crowns within a 24-month period. Clinical relevance APT should be performed carefully while avoiding direct contact with the coronal dentin, and in that case no aesthetic impairments occur. The aim of this explorative study was to investigate the role of Interleukin-(IL)-1, IL-4, GATA-3 and Cyclooxygenase-(COX)-2 polymorphisms after non-surgical periodontal therapy with adjunctive systemic antibiotics (amoxicillin/metronidazole) and subsequent maintenance in a Caucasian population. Analyses were performed using blood samples from periodontitis patients of a multi-center trial (ClinicalTrials.gov NCT00707369=ABPARO-study). Polymorphisms were analyzed using quantitative real-time PCR. Clinical attachment levels (CAL), percentage of sites showing further attachment loss (PSAL) ≥1.3 mm, bleeding on probing (BOP) and plaque score were assessed. Exploratory statistical analysis was performed. A total of 209 samples were genotyped. Patients carrying heterozygous genotypes and single-nucleotide-polymorphisms (SNP) on the GATA-3-IVS4 +1468 gene locus showed less CAL loss than patients carrying wild type. Heterozygous genotypes and SNPs on the IL-1A-889, IL-1B +3954, IL-4-34, IL-4-590, GATA-3-IVS4 +1468 and COX-2-1195 gene loci did not influence CAL. In multivariate analysis, CAL was lower in patients carrying GATA-3 heterozygous genotypes and SNPs than those carrying wild-types. For the first time, effects of different genotypes were analyzed in periodontitis progression after periodontal therapy and during supportive treatment using systemic antibiotics demonstrating a slight association of GATA-3 gene locus with CAL. This result suggests that GATA-3 genotypes are a contributory but non-essential risk factor for periodontal disease progression. Methods: A total of 39 study patients were enrolled. At baseline, bleeding on probing (BoP; primary outcome), gingival index (GI), plaque control record (PCR), recession (REC) and probing pocket depth (PPD) for the calculation of the periodontal inflamed surface area (PISA) were documented. After subsequent professional mechanical plaque removal (PMPR), participants were randomly provided with a supply of sachets containing either a specific collagen peptide preparation (test group; n = 20) or a placebo (placebo group; n = 19) to be consumed dissolved in liquid once daily until reevaluation at day 90. Results: PMPR supplemented with the consumption of the specific collagen peptides resulted in a significantly lower mean percentage of persisting BoP-positive sites than PMPR plus placebo (test: 10.4% baseline vs. 3.0% reevaluation; placebo: 14.2% baseline vs. 9.4% reevaluation; effect size: 0.86). Mean PISA and GI values were also reduced compared to baseline, with a significant difference in favor of the test group (PISA test: 170.6 mm\(^2\) baseline vs. 53.7 mm\(^2\) reevaluation; PISA placebo: 229.4 mm\(^2\) baseline vs. 184.3 mm\(^2\) reevaluation; GI test: 0.5 baseline vs. 0.1 reevaluation; GI placebo: 0.4 baseline vs. 0.3 reevaluation). PCR was also significantly decreased in both experimental groups at revaluation, but the difference between the groups did not reach the level of significance. Conclusions: The supplementary intake of specific collagen peptides may further enhance the anti-inflammatory effect of PMPR in periodontal recall patients. Oswald], Margareta T1 - Mundgesundheit von Hypophosphatasie-Patienten und Gendefektträgern - eine patientenorientierte Querschnittsstudie T1 - Oral health status of hypophosphatasia patients and carriers - a patient focused cross-sectional study N2 - Diese Studie evaluiert die Mundgesundheit von Hypophosphatasie (HPP)-Patienten und beinhaltet die Untersuchungen folgender Kenngrößen: erster Milchzahnverlust, Anzahl fehlender Zähne, DMF/T-Index (Kariös/Fehlend/Gefüllt), Blutung auf Sondierung (BOP), Sondierungstiefen (PPD), klinisches Attachmentniveau (CAL), Zahnlockerungen und kombinierter CDC/AAP Parodontal-Index. Die Daten wurden zwischen HHP-Patienten mit einer und zwei Mutationen auf dem für die alkalische Phosphatase kodierenden Genabschnitt (ALPL-Gen), sowie mit den Ergebnissen der DMS V verglichen, welche die Mundgesundheit der deutschen Allgemeinbevölkerung repräsentiert. 80 Patienten wurden untersucht; 64 von ihnen waren weiblich, 18 männlich. Das Mittlere Alter lag bei 46,4 Jahren. 55 Studienteilnehmer wiesen eine Mutation und 18 zwei Mutationen des ALPL-Gens auf (n=7 keine Testung). Patienten, die von zwei Mutationen des für die alkalische Phosphatase kodierenden Genabschnitts betroffen waren, zeigten ein höheres Risiko für Parodontitis und Zahnverlust als HPP-Patienten mit nur einer Mutation, sowie im Vergleich mit der deutschen Allgemeinbevölkerung. Allerdings stiegen auch bei Patienten mit nur einer Mutation mit zunehmendem Lebensalter die Ausprägungen parodontaler Erkrankungen, sowie die Zahnverlustrate im Vergleich zur Durchschnittsbevölkerung in Deutschland. N2 - This study evaluated the oral health status of adult hypophosphatasia (HPP) patients including the age of first deciduous tooth loss, number of lost permanent teeth, Decayed/Missing/Filled Teeth (DMF/T) Index, Bleeding on Probing (BOP), Probing Pocket Depth (PPD), Clinical Attachment Level (CAL), agomphiasis and periodontal health status according to CCD/AAP criteria. Data were compared inside HHP-patients with one and two ALPL gene mutations and to the findings of the DMS V, which represents the oral health status of the general German population. 80 patients were examined (64 female / 18 male). The mean age were 46.4 years. 55 attendees of the study had one mutation of the ALPL gene and 18 two mutations (7 no testing). HPP-patients affected by two mutations of the ALPL gene showed a higher risk for periodontitis and tooth loss than patients with one mutation and general German population. With increasing age HPP-patients with one mutation also suffer from something more pronounced periodontal diseases and a higher tooth-lost rate than general German population.
