Eine Verbesserung des medianen Überlebens von weniger als 2 auf über 3 Jahre wurde erstmals 1996 durch eine prospektiv randomisierte Studie von Attal et al. gezeigt. Die retrospektive Analyse zweier zeitlich definierter Kohorten, Kohorte 1 (1990-1996) und Kohorte 2 (1997-2002), zur Überprüfung des Therapieerfolges am Universitätsklinikum Würzburg zeigt unter Berücksichtigung aller Patienten keinen Überlebensvorteil für Patienten der 2.Kohorte. Signifikant profitiert haben allerdings Patienten bis max. 65 Jahre der Kohorte 2, die durch die HD-Therapie ein 5-JÜL von 50% (Kohorte 2) vs.32% (Kohorte 1) erreichten, wohingegen sich für ältere Patienten keine signifikant messbaren Überlebensvorteile ergaben. N2 - The median overall survival of patients with multiple myeloma did not exceed more than 2 years for a long time. 1996 Attal et al. published a randomized prospective trial that showed a median survival of more than 3 years in patients who went through a high-dosed chemotherapy and autologous stem cell transplantation. This retrospective analysis in two cohorts compares the overall survival rates of newly diagnosed myeloma patients between 1990-1996 (cohort 1) and 1997-2002 (cohort 2). A significant benefit in 5 year survival rates could be shown for patients younger or max. 65 years(50% C.2 vs. 32% C.1). The overall survival of patients older than 65 years, who did not receive a high-dose therapy, did not improve. The high-dose therapy with autologous stem cell retransfusion seems to be the most important factor to improve the overall survival of patients. Hierbei wurden insbesondere Daten zu Symptomen, Laboruntersuchungen, Lungenfunktionsuntersuchungen sowie radiologischen Untersuchungen gesammelt und ausgewertet. Es wird ein allgemeiner Überblick über die Häufigkeit der einzelnen Symptombereiche sowie Untersuchungsbefunde gegeben. Des weiteren wird anhand eines Bewertungsscores der Erkrankungsverlauf der Patienten abgebildet. In recent studies MRI methods have been developed to measure this parameter noninvasively in mice. Present techniques require additional hardware for cardiac and respiratory gating. In this work a robust self-gated measurement of the local PWV in mice without the need of triggering probes is proposed. Methods The local PWV of 6-months-old wild-type C57BL/6J mice (n=6) was measured in the abdominal aorta with a retrospectively triggered radial Phase Contrast (PC) MR sequence using the flow-area (QA) method. A navigator signal was extracted from the CMR data of highly asymmetric radial projections with short repetition time (TR=3 ms) and post-processed with high-pass and low-pass filters for retrospective cardiac and respiratory gating. The self-gating signal was used for a reconstruction of high-resolution Cine frames of the aortic motion. To assess the local PWV the volume flow Q and the cross-sectional area A of the aorta were determined. The results were compared with the values measured with a triggered Cartesian and an undersampled triggered radial PC-Cine sequence. Results In all examined animals a self-gating signal could be extracted and used for retrospective breath-gating and PC-Cine reconstruction. With the non-triggered measurement PWV values of 2.3±0.2 m/s were determined. These values are in agreement with those measured with the triggered Cartesian (2.4±0.2 m/s) and the triggered radial (2.3±0.2 m/s) measurement. Due to the strong robustness of the radial trajectory against undersampling an acceleration of more than two relative to the prospectively triggered Cartesian sampling could be achieved with the retrospective method. Conclusion With the radial flow-encoding sequence the extraction of a self-gating signal is feasible. The retrospective method enables a robust and fast measurement of the local PWV without the need of additional trigger hardware. Because of the higher risks in patients with obstructive sleep apnoea, the required level of postoperative care for these patients is currently under discussion, and better identification of patients at risk may reduce unnecessary postoperative monitoring. To evaluate the influence of obstructive sleep apnoea, and other risk factors, on peri‐operative complications in children requiring adenotonsillectomy, we performed a retrospective case‐control study that included 1995 patients treated between January 2009 and June 2017. In our analysis, young age (OR 3.8, 95%CI 2.1–7.1), low body weight (OR 2.6, 95%CI 1.5–4.4), obstructive sleep apnoea (OR 2.4, 95%CI 1.5–3.8), pre‐existing craniofacial or syndromal disorders (OR 2.3, 95%CI 1.4–3.8) and adenotonsillectomy, compared with adenoidectomy alone, (OR 7.9, 95%CI 4.7–13.1) were identified as risk factors for complications during or after surgery, p < 0.001. All 13 patients suffering from complications more than 3 h postoperatively had obstructive sleep apnoea plus at least one more of these risk factors. Patients at risk of postoperative complications can therefore be identified by several criteria pre‐operatively, and should be monitored postoperatively using pulse oximetry overnight. For all other patients, postoperative observation on a surgical ward without extra monitoring is sufficient. Admission to paediatric intensive care should be reserved for patients suffering serious intra‐operative complications. Gerade aufgrund der erschwerten Diagnostik und des häufig foudroyanten Verlaufes bei jüngeren Kindern sind eine frühzeitige Diagnosestellung und adäquate Therapie dieser Erkrankung von unermesslichem Wert. Diese retrospektive Studie befasst sich mit Kindern und Jugendlichen unter 18 Jahren, die im Zeitraum vom 01.05.2008 bis 31.05.2013 in der Abteilung für Kinderchirurgie des Universitätsklinikums Würzburg appendektomiert und postoperativ betreut wurden. Simultaneingriffe, wie z.B. Appendektomie bei Malrotation, wurden hierbei ausgeschlossen. Ziel der vorliegenden Dissertation war es unter anderem, das präoperative Management bezüglich der Diagnostik einer Appendizitis darzustellen und zu beurteilen. Hierbei wurde auch die Bedeutung der Leukozytenzahl sowie des C-reaktiven Proteins untersucht. Ein weiterer Fokus lag darauf, die intraoperative Einschätzung des makroskopischen Befundes durch den Operateur dem jeweiligen histopathologischen Befund gegenüberzustellen. Darüber hinaus wurde das Patientengut hinsichtlich des Verteilungsmusters von Alter und Geschlecht beleuchtet und der Frage nachgegangen, ob es einen möglichen Zusammenhang zwischen dem Auftreten einer Appendizitis und der jeweiligen Jahreszeit bzw. Wetterlage gibt. Abschließend lag ein Schwerpunkt der Arbeit darauf, den Stellenwert der laparoskopischen Appendektomie bei Kindern am UKW zu ermitteln und mögliche Vorteile dieses minimalinvasiven Operationsverfahrens gegenüber der konventionellen Appendektomie zu untersuchen. Die Einordnung und Diskussion der Ergebnisse erfolgte anhand der aktuellen Literatur beziehungsweise Studienlage. N2 - Acute appendicitis is the most common cause of acute abdomen beyond infancy and early childhood. Difficulties in diagnosing this disease in combination with an often fulminant course in smaller children emphasize the importance of early diagnosis and adequate therapy. This retrospective study investigates children and adolescents below 18 years of age who have undergone appendectomy and postoperative treatment in the Department for Child Surgery of the University Hospital of Würzburg between 01 May 2008 and 31 May 2013. Patients who received additional surgical interventions (e.g. for malrotation) were excluded. One of the goals of this thesis was to analyze the preoperative management regarding the diagnostics of appendicitis. We focused on the significance of leukocyte count and c-reactive protein. In addition, we compared the intraoperative surgical assessment of the macroscopic findings with the histopathologic results. The distribution of age and sex was analyzed as well as possible correlations between appendicitis and seasons or appendicitis and weather conditions. The final point was to assess the importance of laparoscopic technique for appendectomy in children in the University Hospital of Würzburg and to find possible advantages over a conventional open approach appendectomy. Classification and discussion of the results were performed based on the current state of literature or rather study situation.
