T1 - Triplet repeats in clinical subtypes of schizophrenia: variation at the DRPLA (B37 CAG repeat) locus is not associated with periodic catatonia N2 - Clinical evidence for a dominant mode of inheritance and anticipation in periodic catatonia, a distinct subtype of schizophrenia, indicates that genes with triplet repeat expansions or other unstable repetitive elements affecting gene expression may be involved in the etiology of this disorder. Because patients affected with dentatorubral-pallidoluysian atrophy (DRPLA) may present with "schizophrenic" symptoms, we have investigated the DRPLA (B 37 CAG repeat) locus on chromosome 12 in 41 patients with periodic catatonia. The B 37 CAG repeat locus was highly polymorphic but all alleles in both the patient and control group had repeat sizes within the normal range. We conclude that variation at the DRPLA locus is unlikely to be associated with periodic catatonia. The evidence for dominant inheritance and anticipation as well as the high prevalence of human brain genes containing trinucleotide repeats justifies further screening for triplet repeat expansions in periodic catatonia. Durch Kopplungsuntersuchungen konnte eine Kopplung der Erkrankung mit Chromosom 22q13 gezeigt werden. In der Zielregion befindet sich auch das MLC1-Gen (alternative Bezeichnungen WKL1 oder KIAA0027), für welches bereits eine Assoziation mit einer anderen erblichen Hirnerkrankung, der Megalenzephalen Leukoenzephalopathie mit subkortikalen Zysten (MLC), bekannt ist. Im Rahmen dieser Arbeit erfolgte nun eine Mutationsanalyse von MLC1 als Kandidatengen für die Periodische Katatonie am Material einer mehrfach betroffenen Großfamilie. Durch Verlängerung der bekannten partiellen cDNA-Sequenz von MLC1 mittels 5'-RACE ergab sich unter Annahme des von Nomura et al. (1994) beschriebenen offenen Leserasters ein 377 Aminosäuren großes Protein. Die Strukturanalyse des vorhergesagten MLC1-Proteins zeigt die größte Übereinstimmung für den humanen spannungsgesteuerten Kaliumkanal KCNA1. In der Vergangenheit konnte bereits bei anderen neurologischen Erkrankungen ein Zusammenhang mit veränderten Kaliumkanalproteinen nachgewiesen werden. In der bekannten genomischen DNA-Sequenz konnten 12 Exons annotiert werden. Bei der Sequenzierungsanalyse der codierenden Genabschnitte von MLC1 fand sich bei allen erkrankten Mitgliedern der untersuchten Multiplexfamilie ein heterozygoter Austausch von Cytosin zu Adenin an mRNA-Position 1121 (Gen-Bank Accession-Nummer AF319633). Diese Punktmutation führt zu einem Aminosäureaustausch von Leucin zu Methionin im MLC1-Protein. Bei einigen nicht erkrankten Familienmitgliedern ließ sich die veränderte DNA-Sequenz ebenfalls nachweisen, was jedoch durch eine unvollständige Krankheitspenetranz oder einen späteren Erkrankungszeitpunkt begründet sein könnte. In einem Kontrollkollektiv von 327 Probanden aus der Normalbevölkerung sowie bei je einem erkrankten Mitglied von drei anderen mehrfach von periodischer Katatonie betroffenen Familien konnte die Missense-Mutation nicht gefunden werden. In dieser Arbeit wurde die Assoziation einer sinnverändernden Mutation im MLC1-Gen mit dem Auftreten von periodischer Katatonie in einer mehrfach betroffenen Familie gezeigt. Die Aufklärung der Funktion von MLC1 verspricht somit wichtige Erkenntnisse zur Ätiopathogenese sowohl der Megalenzephalen Leukoenzephalopathie mit subkortikalen Zysten als auch der Periodischen Katatonie. N2 - Periodic catatonia is a well defined subgroup of schizophrenia with marked familial liability. In linkage studies it has been linked to chromosome 22q12. The target region includes the MLC1-gene (alternatively named WKL1 or KIAA0027) for which an association with another hereditary neurological disease, namely Megalencephalic Leukoencephalopathy with subcortical Cysts (MLC), is known. In the scope of this thesis a mutational analysis of MLC1 as a candidate gene for periodic catatonia was performed on the samples of a family with multiple affected members. By extension of the partial cDNA of MLC1 using 5´-RACE technique a protein consisting of 377 amino-acids could be deduced within the previously reported open reading frame. In a structure analysis of the MLC1-protein highest identity scores were obtained for the human voltage-gated potassium channel KCNA1. Previously several neurological diseases have been linked to alterated potassium channels. Twelve exons could be annotated in the known genomic DNA sequence. Consecutive sequencing analysis of the coding regions of MLC1 revealed a heterozygous exchange of cytosine for adenine in mRNA-position 1121 (GenBank accession-number AF319633) in all diseased members of the multiplex family under research. This point mutation effects a replacement of leucine by methionine in the MLC1-protein. Several non-diseased family members were also found to be carriers of the point mutation. This might be accounted for by incomplete penetrance or later manifestations of the disease. The missense mutation could neither be found in diseased members of three other families with multiple affected members nor in an unrelated control group of 327 individuals. Association of a missense mutation in the MLC1-gene and the occurrance of periodic catatonia in a multiply affected family could be demonstrated. Accordingly the elucidation of MLC1 function promises important insights into the etiopathogenesis of both megalencephalic leukoencephalopathy with subcortical cysts and periodic catatonia. Für diese Erkrankung war die klinische Abgrenzbarkeit und die familiäre Häufung signifikant und ließ aufgrund der vertikalen Transmission und dem Auftreten über mehrere Generationen und hinweg auf einen Hauptgeneffekt schließen. Nach der Durchführung von Kopplungs-Analysen kristallisierten sich zwei koppelnde Regionen auf den Chromosomen 15 und 22 heraus. Mittels Haplotypanalyse konnte der Genort auf Chromosom 22q13 auf einen knapp 5 Mbp großen Bereich eingeschränkt werden. Da Mutationen im MLC1-Gen bereits im Zusammenhang mit Megalenzephaler Leukoenzephalopathie beschrieben worden waren, wurden in dieser Arbeit zunächst fünf Patienten mit dieser Erkrankung auf Mutationen in kodierenden Bereichen von MLC1 systematisch untersucht. Daran schloss sich eine Analyse dieses Gens bei 140 Patienten mit periodischer Katatonie an. Ein Zusammenhang zwischen Mutationen in MLC1 und dem Auftreten von Megalenzephaler Leukoenzephalopathie wurde untermauert, wohingegen die Ergebnisse eindeutig gegen eine Assoziation mit periodischer Katatonie sprachen. Ein weiteres im Gehirn exprimiertes Kandidatengen (KIAA0767/DIP) wurde in dieser Arbeit untersucht. Dabei wurden sechs SNPs im exonnahen intronischen Bereich entdeckt sowie eine Variante im Exonbereich (p.Glu156Asn). Dies ist eine seltene Normvariante, eine Assoziation zur periodischen Katatonie wurde in einer Fall-Kontroll-Studie ausgeschlossen. Insgesamt wurde durch die systematische Mutationsanalyse die Kandidatenregion auf Chromosom 22q13.3 weiter eingeengt. Gegen einen Zusammenhang zwischen MLC1 und periodischer Katatonie sprechen die vorgestellten validen Ergebnisse. N2 - The present dissertation has carried on the systematic screening for genes involved in periodic catatonia. The clinical definition and the familial clustering of this disorder were of statistical significance. Its vertical transmission and the occurrence across generations led to the conclusion of it being a major gene effect. Linkage scans discovered two linking regions on chromosome 15 and chromosome 22. Using haplotype analysis it was possible to restrict the candidate region on chromosome 22q13 to region of 5 Mbp. In the coding region of the MLC1 gene a variant (p.Leu309Met) segregated in the haplotype associated with periodic catatonia. As detailed descriptions of mutations in the MLC1 gene in connection with megalencephalic leucoencephalopathy already exist, for the present dissertation five patients with the aforesaid disorder were systematically screened for mutations in the coding regions of MLC1. This was followed by an analysis of the according gene in 140 patients with periodic catatonia, which substantiated a correlation between mutations in MLC1 and the occurrence of megalencephalic leucoencephalopathy, whereas the results definitely did not support an association with periodic catatonia. Additionally, a further brain-expressed candidate gene (KIAA0027/DIP) was screened in this dissertation discovering six single nucleotide polymorphisms in the intronic exon-near region and a variant in the exonic region (p.Glu156Asn). This is a rare normal variant, an association with periodic catatonia was excluded in a case-control study. Altogether the systematic mutation analysis enabled a further reduction of the candidate region on chromosome 22q13.3. The research results, which meet the criterion of validity, do not support a correlation between MLC1 and periodic catatonia.
