A1 - Endress, Matthias A1 - Haeusler, Karl Georg T1 - Off-label-dosing of non-vitamin K-dependent oral antagonists in AF patients before and after stroke: results of the prospective multicenter Berlin Atrial Fibrillation Registry JF - Journal of Neurology N2 - Aims We aimed to analyze prevalence and predictors of NOAC off-label under-dosing in AF patients before and after the index stroke. Methods The post hoc analysis included 1080 patients of the investigator-initiated, multicenter prospective Berlin Atrial Fibrillation Registry, designed to analyze medical stroke prevention in AF patients after acute ischemic stroke. Results At stroke onset, an off-label daily dose was prescribed in 61 (25.5%) of 239 NOAC patients with known AF and CHA2DS2-VASc score ≥ 1, of which 52 (21.8%) patients were under-dosed. Under-dosing was associated with age ≥ 80 years in patients on rivaroxaban [OR 2.90, 95% CI 1.05-7.9, P = 0.04; n = 29] or apixaban [OR 3.24, 95% CI 1.04-10.1, P = 0.04; n = 22]. At hospital discharge after the index stroke, NOAC off-label dose on admission was continued in 30 (49.2%) of 61 patients. Overall, 79 (13.7%) of 708 patients prescribed a NOAC at hospital discharge received an off-label dose, of whom 75 (10.6%) patients were under-dosed. Rivaroxaban under-dosing at discharge was associated with age ≥ 80 years [OR 3.49, 95% CI 1.24-9.84, P = 0.02; n = 19]; apixaban under-dosing with body weight ≤ 60 kg [OR 0.06, 95% CI 0.01-0.47, P < 0.01; n = 56], CHA2DS2-VASc score [OR per point 1.47, 95% CI 1.08-2.00, P = 0.01], and HAS-BLED score [OR per point 1.91, 95% CI 1.28-2.84, P < 0.01]. Conclusion At stroke onset, off-label dosing was present in one out of four, and under-dosing in one out of five NOAC patients. Under-dosing of rivaroxaban or apixaban was related to old age. In-hospital treatment after stroke reduced off-label NOAC dosing, but one out of ten NOAC patients was under-dosed at discharge. Eingeschlossen wurden 262 Patienten und Patientinnen (SR-Gruppe n= 167, VHF-Gruppe n=95), die sich von Januar 2010 bis Mai 2013 einer offenen Herz-OP mit begleitender LAA-Amputation an der Klinik und Poliklinik für Herz- Thorax- und thorakale Gefäßchirurgie des Universitätsklinikums Würzburg unterzogen hatten. Erfasst wurden prä-, intra- und postoperative Daten bis zur Entlassung sowie Langzeitdaten nach durchschnittlich 56 Monaten. Die Rate zerebrovaskulärer Ereignisse war im Langzeit-Follow-up in der SR- und VHF-Gruppe nach dem Eingriff nicht signifikant verschieden (>30 d post-OP: 5,1 % vs. 6,1 %, p=0,875). Trotz der LAA-Amputation lag die Gesamtmortalität (>30 d post-OP, alle Todesursachen) in der VHF-Gruppe im Vergleich zur SR-Gruppe risikostratifiziert signifikant höher (26,3 % vs. 7,2 %, HR 2,872, p=0,010). Weiterhin zeigte sich ein Unterschied beim Vergleich von Patienten und Patientinnen mit intermittierendem/chronischem VHF bzw. SR. Hierbei ging ein chronisches VHF (HR=3,614, p=0,006) anders als ein intermittierendes VHF (HR=2,270, p=0,090) mit einem signifikant schlechteren Langzeitüberleben einher. Die aktuelle Datenlage inklusive der vorliegenden Befunde spricht dafür, dass der negative Effekt eines präoperativen VHF auf zerebrovaskuläre Ereignisse bei Patienten und Patientinnen, die sich einer kardiochirurgischen OP unterziehen, durch eine begleitende LAA-Amputation verringert werden kann. Dennoch weist diese Gruppe im Vergleich zu Patienten und Patientinnen mit SR immer noch ein schlechteres Langzeitüberleben auf. Prospektive randomisierte Studien sind notwendig, um allen Betroffenen eine optimale Therapie anbieten zu können. N2 - This longitudinal study aimed to perform a comparative analysis of the effects of an amputation of the left atrial appendage (LAA) in the context of cardiac surgery (kardiochirurgische Operation (OP)) on patients with preoperatively diagnosed sinus rhythm (SR group) and atrial fibrillation (AF group). A total of 262 patients (SR group n=167, AF group n=95) who underwent open-heart surgery with accompanying LAA amputation in the period from January 2010 to May 2013 at the University Medical Centre of Wuerzburg, Germany were included in the study. Pre-, intra- and postoperative data, as well as long-term data after an average of 56 months, were recorded. The rate of cerebrovascular events was not significantly different in the long-term follow-up in the SR and AF groups after the procedure (>30 days post-op: 5.1% vs. 6.1%, p=0.875). Despite the LAA amputation, the overall mortality (>30 d post-OP, all causes of death) was significantly higher in the AF group than in the SR group, risk-stratified (26.3% vs. 7.2%, HR 2.872, p= 0.010). Furthermore, a difference was observed when comparing patients with intermittent/chronic AF respectively SR. In contrast to intermittent AF (HR=2.270, p=0.090), chronic AF (HR=3.614, p=0.006) was associated with a significantly poorer long-term survival rate. The current data situation, including the findings at hand, suggests that the negative effect of preoperative AF on cerebrovascular events in patients undergoing cardiac surgery can be reduced by concomitant LAA amputation. However, this group still has poorer chances of long-term survival compared to patients with SR and prospective randomized studies are thus necessary to be able to offer an optimal therapy to all affected patients.
