Recovery after complete spinal cord injury is very limited even in animal models receiving elaborate combinatorial treatments. Recently, we described an implantable microsystem (microconnector) for low-pressure re-adaption of severed spinal stumps in rat. Here we investigate the long-term structural and functional outcome following microconnector implantation after complete spinal cord transection. Re-adaptation of spinal stumps supports formation of a tissue bridge, glial and vascular cell invasion, motor axon regeneration and myelination, resulting in partial recovery of motor-evoked potentials and a thus far unmet improvement of locomotor behaviour. The recovery lasts for at least 5 months. Despite a late partial decline, motor recovery remains significantly superior to controls. Our findings demonstrate that microsystem technology can foster long-lasting functional improvement after complete spinal injury, providing a new and effective tool for combinatorial therapies. MT allows salvage of the vessel-occluding thrombemboli, which most frequently originate from the left atrium or the left ventricle of the heart or from sites of plaque rupture within large arteries above the heart. Clot composition may influence the efficacy of (intravenous) thrombolysis and MT, respectively. We analyzed 37 human thrombemboli obtained from acute ischemic stroke patients during MT with special emphasis on histological staining of neutrophils and neutrophil extracellular traps (NETs). We found neutrophils as the main cellular component of cerebral thrombemboli but encountered considerable morphological heterogeneity. Neutrophils accumulated in the border region of fibrin-rich structures indicating possible interaction of neutrophils with distinct structural thrombembolus components. Web-like NETs were found in 35 of 37 thrombemboli in varying amounts. NETs were almost exclusively found within fibrin-rich areas. Importantly, stroke etiology, age and present oral anticoagulation was associated with morphological patterns and the amount of neutrophils. Correlation of histological data and imaging data revealed that relative Hounsfield units of cerebral thrombemboli positively correlated with the amount of red blood cells. In summary, our results demonstrate that neutrophils and NETs are substantial constituents of cerebral thrombemboli and contribute to their structural complexity. We here analyzed associated HMGB1 expression in human thromboemboli retrieved via mechanical thrombectomy from 37 stroke patients with large vessel occlusion. HMGB1 was detected in all thromboemboli, accounting for 1.7% (IQR 0.6–6.2%) of the total thromboemboli area and was found to be colocalized with neutrophils and NETs and in spatial proximity to platelets. Correlation analysis revealed that the detection of HMGB1 was strongly related to the number of neutrophils (r = 0.58, p = 0.0002) and platelets (r = 0.51, p = 0.001). Anti-cholinergic treatment is effective in FMS patient subgroups, and cholinergic signaling is a strong modulator of CNS and PNS immune processes. Therefore, we used whole blood small RNA-sequencing of female FMS patients and healthy controls to profile microRNA regulators of cholinergic transcripts (CholinomiRs). We compared microRNA profiles with those from Parkinson's disease (PD) patients with pain as disease controls. We validated the sequencing results with quantitative real-time PCR (qRT-PCR) and identified cholinergic targets. Further, we measured serum cholinesterase activity in FMS patients and healthy controls. Small RNA-sequencing revealed FMS-specific changes in 19 CholinomiRs compared to healthy controls and PD patients. qRT-PCR validated miR-182-5p upregulation, distinguishing FMS patients from healthy controls. mRNA targets of CholinomiRs bone morphogenic protein receptor 2 and interleukin 6 signal transducer were downregulated. Serum acetylcholinesterase levels and cholinesterase activity in FMS patients were unchanged. Our findings identified an FMS-specific CholinomiR signature in whole blood, modulating immune-related gene expression. However, the extent to which ICA-stenosis <50% causes ischemic cerebrovascular events is uncertain. We examined the relation between plaque cross-sectional area and length and the risk of ischemic stroke or TIA among patients with ICA-stenosis of 20–40%. Methods: We retrospectively identified patients admitted to the Department of Neurology, University Hospital of Würzburg, from January 2011 until September 2016 with ischemic stroke or TIA and concomitant ICA-stenosis of 20–40%, either symptomatic or asymptomatic. Plaque length and cross-sectional area were assessed on ultrasound scans. Results: We identified 41 patients with ischemic stroke or TIA and ICA-stenosis of 20–40%; 14 symptomatic and 27 asymptomatic. The plaque cross-sectional area was significantly larger among symptomatic than asymptomatic ICA-stenosis; median values (IQR) were 0.45 (0.21–0.69) cm2 and 0.27 (0.21–0.38) cm2, p = 0.03, respectively. A plaque cross-sectional area ≥0.36 cm2 had a sensitivity of 71% and a specificity of 76% for symptomatic compared with asymptomatic ICA-stenosis. In a sex-adjusted multivariate logistic regression, a plaque cross-sectional area ≥0.36 cm2 and a plaque length ≥1.65 cm were associated with an OR (95% CI) of 5.54 (1.2–25.6), p = 0.028 and 1.78 (0.36–8.73), p = 0.48, respectively, for symptomatic ICA-stenosis. Conclusion: Large plaques might increase the risk of ischemic stroke or TIA among patients with low-grade ICA-stenosis of 20–40%. Sufficiently powered prospective longitudinal cohort studies are needed to definitively test the stroke risk stratification value of carotid plaque length and cross-sectional area in the setting of current optimal medical treatment. JF - Frontiers in Neurology N2 - Background and purpose: Previous studies delivered contradicting results regarding the relation between the presence of an internal carotid artery stenosis (ICAS) and the occurence of white matter lesions (WMLs). We hypothesize that special characteristics related to the ICAS might be related to the WMLs. We examined the relation between the presence of bilateral ICAS, the degree and length of stenosis and ipsi-, contralateral as well as mean white matter lesion load (MWMLL). Methods: In a retrospective cohort, patients with ischemic stroke or transient ischemic attack (TIA) as well as ipsi- and/or contralateral ICAS were identified. The length and degree of ICAS, as well as plaque morphology (hypoechoic, mixed or echogenic), were assessed on ultrasound scans and, if available, the length was also measured on magnetic resonance angiography (MRA) scans, and/or digital subtraction angiography (DSA). The WMLs were assessed in 4 areas separately, (periventricular and deep WMLs on each hemispherer), using the Fazekas scale. The MWMLL was calculated as the mean of these four values. Results: 136 patients with 177 ICAS were identified. A significant correlation between age and MWMLL was observed (Spearman correlation coefficient, ρ = 0.41, p < 0.001). Before adjusting for other risk factors, a significantly positive relation was found between the presence of bilateral ICAS and MWMLL (p = 0.039). The length but not the degree of ICAS showed a very slight trend toward association with ipsilateral WMLs and with MWMLL. In an age-adjusted multivariate logistic regression with MWMLL ≥2 as the outcome measure, atrial fibrillation (OR 3.54, 95% CI 1.12–11.18, p = 0.03), female sex (OR 3.11, 95% CI 1.19–8.11, p = 0.02) and diabetes mellitus (OR 2.76, 95% CI 1.16–6.53, p = 0.02) were significantly related to WMLs, whereas the presence of bilateral stenosis showed a trend toward significance (OR 2.25, 95% CI 0.93–5.45, p = 0.074). No relation was found between plaque morphology and MWMLL, periventricular, or deep WMLs. Conclusion: We have shown a slight correlation between the length of stenosis and the presence of WMLs which might be due to microembolisation originating from the carotid plaque. However, the presence of bilateral ICAS seems also to be related to WMLs which may point to common underlying vascular risk factors contributing to the occurrence of WML. However, a considerable percentage of stroke survivors with symptomatic ICAS (sICAS) have <70% stenosis with a vulnerable plaque. Whether the length of ICAS is associated with high risk of ICVEs is poorly investigated. Our main aim was to investigate the relation between the length of ICAS and the development of ICVEs. Methods: In a retrospective cross-sectional study, we identified 95 arteries with sICAS and another 64 with asymptomatic internal carotid artery stenosis (aICAS) among 121 patients with ICVEs. The degree and length of ICAS as well as plaque echolucency were assessed on ultrasound scans. Results: A statistically significant inverse correlation between the ultrasound-measured length and degree of ICAS was detected for sICAS≥70% (Spearman correlation coefficient ρ = –0.57, p < 0.001, n = 51) but neither for sICAS<70% (ρ = 0.15, p = 0.45, n = 27) nor for aICAS (ρ = 0.07, p = 0.64, n = 54). The median (IQR) length for sICAS<70% and ≥70% was 17 (15–20) and 15 (12–19) mm (p = 0.06), respectively, while that for sICAS<90% and sICAS 90% was 18 (15–21) and 13 (10–16) mm, respectively (p < 0.001). Among patients with ICAS <70%, a cut-off length of ≥16 mm was found for sICAS rather than aICAS with a sensitivity and specificity of 74.1% and 51.1%, respectively. Irrespective of the stenotic degree, plaques of the sICAS compared to aICAS were significantly more often echolucent (43.2 vs. 24.6%, p = 0.02). Conclusion: We found a statistically insignificant tendency for the ultrasound-measured length of sICAS<70% to be longer than that of sICAS≥70%. Moreover, the ultrasound-measured length of sICAS<90% was significantly longer than that of sICAS 90%. Among patients with sICAS≥70%, the degree and length of stenosis were inversely correlated. Larger studies are needed before a clinical implication can be drawn from these results. Since etiology is unknown in 25% of stroke patients and GCA is hardly considered as a cause, we examined in a pilot study, whether screening for GCA after vertebrobasilar stroke might unmask an otherwise missed disease. Consecutive patients with vertebrobasilar stroke were prospectively screened for GCA using erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), hemoglobin, and halo sign of the temporal and vertebral artery on ultrasound. Furthermore, we conducted a systematic literature review for relevant studies. Sixty-five patients were included, and two patients (3.1%) were diagnosed with GCA. Patients with GCA were older in age (median 85 versus 69 years, p = 0.02). ESR and CRP were significantly increased and hemoglobin was significantly lower in GCA patients compared to non-GCA patients (median, 75 versus 11 mm in 1 h, p = 0.001; 3.84 versus 0.25 mg/dl, p = 0.01, 10.4 versus 14.6 mg/dl, p = 0.003, respectively). Multiple stenoses/occlusions in the vertebrobasilar territory affected our two GCA patients (100%), but only five (7.9%) non-GCA patients (p = 0.01). Our literature review identified 13 articles with 136 stroke patients with concomitant GCA. Those were old in age. Headache, increased inflammatory markers, and anemia were frequently reported. Multiple stenoses/occlusions in the vertebrobasilar territory affected around 70% of stroke patients with GCA. Increased inflammatory markers, older age, anemia, and multiple stenoses/occlusions in the vertebrobasilar territory may be regarded as red flags for GCA among patients with vertebrobasilar stroke. Anecdotal reports have shown that vertigo occurs more often in multiple than in single brainstem or cerebellar infarctions. We examined the relation between the location and volume of infarction and vertigo in patients with vertebrobasilar stroke. Methods Consecutive patients with vertebrobasilar stroke were prospectively recruited. The infarction location and volume were assessed in the diffusion‐weighted magnetic resonance imaging. Results Fifty‐nine patients were included, 32 (54.2%) with vertigo and 27 (45.8%) without vertigo. The infarction volume did not correlate with National Institute of Health Stroke Scale (NIHSS) score on admission (Spearman ρ = .077, p = .56) but correlated with modified Rankin Scale (ρ = .37, p = .004) on discharge. In the vertigo group, the proportion of men was lower (53.1% vs. 77.8%, p = .049), fewer patients had focal neurological deficits (65.6% vs. 96.3%, p = .004), patients tended to present later (median [IQR] was 7.5 [4–46] vs. 4 [2–12] hours, p = .052), numerically fewer patients received intravenous thrombolysis (15.6% vs. 37%, p = .06), and the total infarction volume was larger (5.6 vs. 0.42 cm3, p = .008) than in nonvertigo group. In multivariate logistic regression, infarction location either in the cerebellum or in the dorsal brainstem (odds ratio [OR] 16.97, 95% CI 3.1–92.95, p = .001) and a total infarction volume of >0.48 cm3 (OR 4.4, 95% CI 1.05–18.58, p = .043) were related to vertigo. In another multivariate logistic regression, after adjusting for age, sex, intravenous thrombolysis, serum level of white blood cells, and atrial fibrillation, vertigo independently predicted a total infarction volume of >0.48 cm3 (OR 5.75, 95% CI 1.43–23.08, p = .01). Conclusion Infarction location in the cerebellum and/or dorsal brainstem is an independent predictor of vertigo. Furthermore, larger infarction volume in these structures is associated with vertigo. A considerable proportion of patients with vascular vertigo present without focal neurological deficits posing a diagnostic challenge. Irrespective of the stenotic degree, plaques of the symptomatic internal carotid artery stenosis compared to asymptomatic internal carotid artery stenosis were significantly more often echolucent (43.2 vs. 24.6%, p = 0.02). The length but not the degree of internal carotid artery stenosis showed a very slight trend toward association with ipsilateral white matter lesions and with mean white matter lesions load. Conclusion: We found a statistically insignificant tendency for the ultrasound-measured length of symptomatic internal carotid artery stenosis <70% to be longer than that of symptomatic internal carotid artery stenosis ≥70%. Moreover, the ultrasound-measured length of symptomatic internal carotid artery stenosis <90% was significantly longer than that of symptomatic internal carotid artery stenosis 90%. Among patients with symptomatic internal carotid artery stenosis ≥70%, the degree and length of stenosis were inversely correlated. Furthermore, we have shown that a slight correlation exists between the length of stenosis and the presence of ipsilateral white matter lesions which might be due to microembolisation originating from the carotid plaque. Larger studies are needed before a clinical implication can be drawn from these results. N2 - Hintergrund: Stenose der A. carotis interna ≥70% ist eine der führenden Ursachen für ischämische zerebrovaskuläre Ereignisse. Ein beträchtlicher Prozentsatz der Schlaganfall-Überlebenden mit symptomatischer Stenose der A. carotis interna weist jedoch eine Stenose <70% mit einer „vulnerable Plaque“ auf. Ob die Länge der Stenose der A. carotis interna mit einem hohen Risiko für ischämische zerebrovaskuläre Ereignisse oder mit Läsionen der weißen Substanz verbunden ist, wird nur unzureichend untersucht. Unser Hauptziel war es, den Zusammenhang zwischen der Länge der Stenose der Aerteria carotis interna und der Entwicklung von ischämischen zerebrovaskulären Ereignissen sowie der ipsi-, kontralateralen und mittleren Läsionslast der weißen Substanz zu untersuchen. Methode: In einer retrospektiven Querschnittsstudie wurden 168 Patienten mit 208 Stenosen der A. carotis interna identifiziert. Der Stenosegrad und die Stenoselänge sowie die Plaquemorphologie (echoarm, gemischt oder echogen) wurden mittels Ultraschall untersucht. Die Läsionen der weißen Substanz wurden in 4 Bereichen (periventrikuläre und subkortilae Läsionen der weißen Substanz, jeweils auf jeder Hemisphäre) mittels Fazekas-Skala bewertet. Der mittlere dieser vier Werte wurde ebenso berechnet. Ergebnisse: Eine statistisch signifikante inverse Korrelation zwischen der mit Ultraschall gemessenen Länge und dem Stenosegrad der A. carotis interna wurde für eine symptomatische Stenose der A. carotis interna von ≥ 70% festgestellt (Spearman-Korrelationskoeffizient ρ = –0,57, p <0,001, n = 51), jedoch keine bei symptomatischer Stenose der A. carotis interna <70% (ρ = 0,15, p = 0,45, n = 27) und bei asymptomatischer Stenose der A. carotis interna (ρ = 0,07, p = 0,64, n = 54). Die mediane Länge (IQR) für symptomatische Stenosen der A. carotis interna <70% und ≥ 70% betrug 17 (15–20) bzw. 15 (12–19) mm (p = 0,06), die für symptomatische Stenosen der A. carotis interna <90% und symptomatische Stenose der A. carotis interna 90% betrugen 18 (15–21) bzw. 13 (10–16) mm (p <0,001). Bei Patienten mit einer Stenose der A. carotis interna <70% wurde für eine symptomatische Stenose der A. carotis interna einen Grenzwert von ≥ 16 mm gefunden, und nicht für eine asymptomatische Stenose der A. carotis interna mit einer Sensitivität und Spezifität von 74,1% bzw. 51,1%. Unabhängig vom stenotischen Grad waren Plaques der symptomatischen Stenose der A. carotis interna im Vergleich zur asymptomatischen Stenose der A. carotis interna signifikant häufiger echoarm (43,2 vs. 24,6%, p = 0,02). Die Stenoselänge, aber nicht der Stenosegrad der A. carotis interna zeigte einen sehr geringen Trend zur Assoziation mit ipsilateralen Läsionen der weißen Substanz und mit der mittleren Läsionslast der weißen Substanz. However, data in stroke patients on understanding of the informed consent (IC) procedure are sparse. Methods We conducted a cross-sectional study among ischemic stroke patients taking part in the IC procedure of an ongoing cluster-randomized secondary prevention trial. All aspects of the IC procedure were assessed in an interview using a standardized 20-item questionnaire. Responses were collected within 72 h after the IC procedure and analyzed quantitatively and qualitatively. Participants were also asked their main reasons for participation. Results A total of 146 stroke patients (65 ± 12 years old, 38% female) were enrolled. On average, patients recalled 66.4% (95% confidence interval = 65.2%–67.5%) of the content of the IC procedure. Most patients understood that participation was voluntary (99.3%) and that they had the right to withdraw consent (97.1%); 79.1% of the patients recalled the study duration and 56.1% the goal. Only 40.3% could clearly state a benefit of participation, and 28.8% knew their group allocation. Younger age, higher graduation, and allocation to the intervention group were associated with better understanding. Of all patients, 53% exclusively stated a personal and 22% an altruistic reason for participation. Conclusions Whereas understanding of patient rights was high, many patients were unable to recall other important aspects of study content and procedures. Increased attention to older and less educated patients may help to enhance understanding in this patient population. Actual recruitment and retention benefit of an improved IC procedure remains to be tested in a randomized trial. Methods In C57Bl/6 (wildtype, WT), \(hcn2^{+/+}\) and \(hcn2^{-/-}\) mice we used an in vivo model of cerebral ischemia (transient middle cerebral artery occlusion (tMCAO)) to depict a functional impact of HCN2 in stroke formation. Subsequent analyses comprise behavioural tests and hcn2 gene expression assays. Results After 60 min of tMCAO induction in WT mice, we collected tissue samples at 6, 12, and 24 h after reperfusion. In the infarcted neocortex, hcn2 expression analyses revealed a nominal peak of hcn2 expression 6 h after reperfusion with a tendency towards lower expression levels with longer reperfusion times. Hcn2 gene expression levels in infarcted basal ganglia did not change after 6 h and 12 h. Only at 24 h after reperfusion, hcn2 expression significantly decreases by ~55%. However, 30 min of tMCAO in hcn2-/- as well as hcn2+/+ littermates induced similar infarct volumes. K2P3.1-/- animals showed larger infarct volumes and a worse functional outcome following experimentally induced ischemic stroke. Here, we question the role of the closely related K2P channel K2P9.1. METHODS: We combine electrophysiological recordings in brain-slice preparations of wildtype and K2P9.1-/- mice with an in vivo model of cerebral ischemia (transient middle cerebral artery occlusion (tMCAO)) to depict a functional impact of K2P9.1 in stroke formation. RESULTS: Patch-clamp recordings reveal that currents mediated through K2P9.1 can be obtained in slice preparations of the dorsal lateral geniculate nucleus (dLGN) as a model of central nervous relay neurons. Current characteristics are indicative of K2P9.1 as they display an increase upon removal of extracellular divalent cations, an outward rectification and a reversal potential close to the potassium equilibrium potential. Lowering extracellular pH values from 7.35 to 6.0 showed comparable current reductions in neurons from wildtype and K2P9.1-/- mice (68.31 +/- 9.80% and 69.92 +/- 11.65%, respectively). These results could be translated in an in vivo model of cerebral ischemia where infarct volumes and functional outcomes showed a none significant tendency towards smaller infarct volumes in K2P9.1-/- animals compared to wildtype mice 24 hours after 60 min of tMCAO induction (60.50 +/- 17.31 mm3 and 47.10 +/- 19.26 mm3, respectively). CONCLUSIONS: Together with findings from earlier studies on K2P2.1-/- and K2P3.1-/- mice, the results of the present study on K2P9.1-/- mice indicate a differential contribution of K2P channel subtypes to the diverse and complex in vivo effects in rodent models of cerebral ischemia. Methods: In this case–control study, we prospectively recruited 86 patients with a medical history and clinical phenotype suggestive of SFN. Patients underwent neurological examination, quantitative sensory testing (QST), and distal and proximal skin punch biopsy, and were tested for pain-associated gene loci. Fifty-five of these patients additionally underwent pain-related evoked potentials (PREP), corneal confocal microscopy (CCM), and a quantitative sudomotor axon reflex test (QSART). Results: Abnormal distal intraepidermal nerve fiber density (IENFD) (60/86, 70%) and neurological examination (53/86, 62%) most frequently reflected small fiber disease. Adding CCM and/or PREP further increased the number of patients with small fiber impairment to 47/55 (85%). Genetic testing revealed potentially pathogenic gene variants in 14/86 (16%) index patients. QST, QSART, and proximal IENFD were of lower impact. Conclusion: We propose to diagnose SFN primarily based on the results of neurological examination and distal IENFD, with more detailed phenotyping in specialized centers. Die Patienten berichten v.a. von brennenden Schmerzen und Dysästhesien, seltener auch von autonomen Funktionsstörungen. Bei fehlendem Goldstandard und normalen Nervenleitungsstudien ist die Diagnostik erschwert, da selbst nach Spezialuntersuchungen wie Hautstanzbiopsie und quantitativer sensorischer Testung (QST) viele Patienten trotz typischer Anamnese der Diagnosestellung entgehen. Wir rekrutierten 55 Patienten und 31 gesunde Kontrollen. Nach neurologischer Untersuchung und Ausschluss einer Polyneuropathie mittels Elektroneurographie wurden bei allen Studienteilnehmern Hautstanzbiopsien am Ober- und Unterschenkel zur Ermittlung der intraepidermalen Nervenfaserdichte (IENFD) entnommen sowie eine QST zur Funktionsprüfung der kleinen Nervenfasern durchgeführt. Die Studienteilnehmer wurden zudem mit cornealer confocaler Mikroskopie (CCM) und der Ableitung Schmerz-assoziierter evozierter Potentiale (PREP) untersucht. Zur autonomen Testung erfolgte die Messung der Schweißproduktion mittels quantitativem sudomotorischem Axonreflextest (QSART). Die neurologische Untersuchung zeigte in 55% der Patienten Hinweise auf eine Kleinfaserpathologie. Die distale IENFD war bei 62% der Patienten reduziert, die QST bei 22% der Patienten auffällig. Die PREP Latenzen waren in der Patientengruppe länger als bei den Kontrollen, die Amplituden niedriger. Bei der cornealen Innervation zeigte sich eine Reduktion der Nervenfaserdichte, Nervenfaserlänge und Nervenastdichte. Die in QSART gemessenen Parameter zeigten sich zu 86% unauffällig. Während nach klinischer Untersuchung, Hautbiopsie und QST in 53% der Fälle in 2 von 3 Untersuchungen eine Pathologie der kleinen Fasern festgestellt werden konnte, stieg die Rate bei zusätzlicher Anwendung von PREP und CCM auf 80% (ohne Berücksichtigung von QST). Zusammenfassend sollten die klinische Untersuchung und die Hautstanzbiopsie bei allen Patienten mit Verdacht auf SFN erfolgen. PREP und CCM sind unter den verfügbaren zusätzlichen Untersuchungen diagnostisch am wertvollsten. Wichtig ist allerdings, dass bei fehlendem Goldstandard eine SFN auch bei unauffälligen Tests nicht ausgeschlossen werden kann. Zusätzlich können die Mikroneurographie und die genetische Analyse wertvolle Hinweise auf eine Kleinfaserfunktionsstörung und deren Pathophysiologie geben. N2 - Small fiber neuropathy (SFN) forms a subgroup of sensory neuropathies, in which the Aδ- and C-fibers are impaired. Patients mainly report burning pain and dysesthesia, less frequently also autonomic dysfunctions. In absence of a diagnostic gold standard and under normal nerve conduction studies, the diagnosis is difficult. Even after special examinations such as skin punch biopsy and quantitative sensory testing (QST), many patients are not diagnosed despite a typical pain history. We prospectively recruited 55 patients and 31 healthy controls in our study. After neurological examination and exclusion of a polyneuropathy by means of neurological examination and electroneurography, skin punch biopsies were taken from the upper and lower leg of all study participants to determine the intraepidermal nerve fiber density (IENFD). QST was performed to investigate the function of the small nerve fibers. Study participants were also examined with corneal confocal microscopy (CCM) and derivation of pain-related evoked potentials (PREP). For autonomous testing, the sweat production was measured using quantitative sudomotor axon reflex testing (QSART). Neurological examination showed hints for small fiber pathology in 55% of patients. The distal IENFD was reduced in 62% of patients, while QST was abnormal in 22% of patients. The PREP latencies were longer in the patient group than in the controls, the amplitudes were smaller. Corneal innervation showed a reduction of nerve fiber density, nerve fiber length, and nerve branch density in patients compared to controls. The parameters measured in QSART were 86% unremarkable. While after clinical examination, skin biopsy, and QST a pathology of the small fibers could be detected in 53% of the cases in 2 of 3 examinations, the rate increased to 80% with additional application of PREP and CCM (without consideration of QST). In summary, neurological examination should be performed together with skin biopsy in all patients with suspected SFN. PREP and CCM are diagnostically most valuable among the available additional examinations. However, it is important to note that in the absence of a gold standard, SFN cannot be excluded even with normal test results. In addition, microneurography and genetic analysis can provide valuable information about a small fiber dysfunction and its pathophysiology. Neben der Eigenanamnese und der neurologischen Untersuchung erhielten die Patienten eine Suralisneurographie zur Überprüfung der large fibers und eine Quantitative sensorische Testung (QST) zur Detektion einer möglichen Small-fiber-Dysfunktion. Die Vitamin-Bestimmung diente der Untersuchung möglicher Zusammenhänge zwischen der Levodopa-Therapie, eventuell daraus resultierenden Vitamin-Mangelzuständen und einer reduzierten intraepidermalen Nervenfaser-Dichte (IENF-Dichte) beim M. Parkinson. Dagegen war eine pathologisch reduzierte Nervenleitgeschwindigkeit nur bei einem IPD-Patienten nachweisbar. Auffällig war zudem eine negative Korrelation zwischen der Erkrankungsdauer und dem SNAP (Korrelationskoeffizient -0,367, p<0,03). In der Auswertung der Hautbiopsien konnte eine statistisch signifikante Reduktion der myelinisierten Bündel am Unterschenkel der IPD-Patienten festgestellt werden. Bei zehn von 30 IPD-Patienten, jedoch bei keinem der Probanden mit einem atypischen Parkinson-Syndrom, konnte eine verminderte IENF-Dichte nachgewiesen werden. In der statistischen Überprüfung wurde außerdem am Unterschenkel ein signifikanter Unterschied zwischen den IPD-Patienten und der Kontrollkohorte sowie eine negative Korrelation zwischen der Krankheitsdauer und der IENF-Dichte (Korrelationskoeffizient -0,320, p<0,05) festgestellt. Die QST konnte dagegen keinen statistisch signifikanten Unterschied zwischen den einzelnen Kohorten aufzeigen. Im Kontrast dazu fand sich eine längenunabhängige Reduktion der CGRP-positiven und der Substanz P-positiven IENF-Dichte bei den Patienten mit einem idiopathischen Parkinson-Syndrom. Bemerkenswert war zudem eine signifikante Verminderung der Substanz P-positiven intraepidermalen Nervenfasern am Oberschenkel und Rücken bei den APD-Patienten. Eine statistisch signifikante Abweichung der CGRP- und Substanz P-positiven Bündel konnte dagegen nicht festgestellt werden. In der laborchemischen Untersuchung war ein Zusammenhang zwischen den bestimmten Vitamin-Spiegeln und der kumulativen Levodopa-Dosis sowie zwischen den Vitaminen und der IENF-Dichte lediglich bei dem Vitamin B6 nachweisbar. Zusammengefasst erscheint eine Beteiligung des peripheren Nervensystems beim idiopathischen Parkinson als wahrscheinlich, wohingegen bei den atypischen Parkinson-Syndromen vor allem von einer zentralen Genese ausgegangen werden kann. Basierend auf den Ergebnissen der Suralisneurographie und der Bestimmung der myelinisierten Bündel erscheint eine krankheitsbedingte Large-fiber-Beeinträchtigung beim M.Parkinson möglich. Die nachgewiesene längenabhängige Small-fiber-Reduktion bei IPD-Patienten wird vermutlich durch eine axonale Transportstörung verursacht. Einen krankheitsbedingten Erklärungsansatz für die längenunabhängige Reduktion der CGRP-positiven und der Substanz P-positiven IENF-Dichte bei IPD-Patienten liefert der Nachweis von neurotoxischem α-Synuclein in den sensiblen Spinatganglien mit einem daraus resultierenden Untergang von sensorischen Nervenfasern. Aufgrund der geringen Anzahl an Parkinson-Patienten mit sensiblen Symptomen und dem fehlenden Nachweis eines statistisch signifikanten Unterschiedes in der QST liegt der Verdacht nahe, dass die ermittelte intraepidermale Nervenfaserreduktion der IPD-Patienten nicht stark genug ausgeprägt ist, um eine signifikante Abweichung der QST-Ergebnisse zu verursachen. Weiterhin konnte kein Zusammenhang zwischen der kumulativen Levodopa-Menge, den Vitaminen B12, Methylmalonsäure sowie Homocystein und dem Auftreten einer Nervenfaserverminderung nachgewiesen werden, was gegen eine iatrogene Beteiligung des peripheren Nervensystems als Nebenwirkung der Levodopa-Therapie spricht. Das idiopathische Parkinson-Syndrom geht mit einer Reduktion der kleinen Nervenfasern einher, welche vermutlich auf die Grunderkrankung selbst zurückzuführen ist. Die Untersuchung der Haut erscheint somit vielversprechend für die Erforschung der Pathogenese und für die Differentialdiagnostik des M. Parkinson. Methods/Principal Findings: We performed a phase II, double blind, multicentre, placebo controlled trial of pioglitazone in ALS patients under riluzole. 219 patients were randomly assigned to receive 45 mg/day of pioglitazone or placebo (one: one allocation ratio). The primary endpoint was survival. Secondary endpoints included incidence of non-invasive ventilation and tracheotomy, and slopes of ALS-FRS, slow vital capacity, and quality of life as assessed using EUROQoL EQ-5D. The study was conducted under a two-stage group sequential test, allowing to stop for futility or superiority after interim analysis. Shortly after interim analysis, 30 patients under pioglitazone and 24 patients under placebo had died. The trial was stopped for futility; the hazard ratio for primary endpoint was 1.21 (95% CI: 0.71-2.07, p = 0.48). Secondary endpoints were not modified by pioglitazone treatment. Pioglitazone was well tolerated. Conclusion/Significance: Pioglitazone has no beneficial effects on the survival of ALS patients as add-on therapy to riluzole. Formerly regarded as a variant of MS, neuromyelitis optica (NMO)/Devic’s disease is now recognized as a distinct neurological disorder exhibiting characteristic inflammatory and demyelinated foci in the optic nerves and the spinal cord sparing the brain. With the introduction of the double-transgenic “Devic mouse” model featuring spontaneous, adjuvant-free incidence of autoimmune neuroinflammation due to the interaction of transgenic MOG-specific T and B cells, a promising tool was found for the analysis of factors triggering or preventing autoimmunity. The co-inhibitory molecule B7-H1 has been proposed to contribute to the maintenance of peripheral tolerance and to confine autoimmune inflammatory damage via the PD-1/B7-H1 pathway. Compared to Devic B7-H1+/+ mice, Devic B7-H1-/- mice developed clinical symptoms with a remarkably higher incidence rate and faster kinetics emphasized by deteriorated disease courses and a nearly quadrupled mortality rate. Remarkably enlarged immune-cell accumulation in the CNS of Devic B7-H1-/- mice, in particular of activated MOG-specific CD4+ T cells, correlated with the more severe clinical features. Our studies showed that the CNS not only was the major site of myelin-specific CD4+ T-cell activation but also that B7-H1 expression within the target organ significantly influenced T-cell activation and differentiation levels. Analysis at disease maximum revealed augmented accumulation of MOG-specific CD4+ T cells in the peripheral lymphoid organs of Devic B7-H1-/- mice partly due to increased T-cell proliferation rates. Transgenic MOG-specific B cells of Devic B7-H1-/- mice activated MOG-specific CD4+ T cells more efficiently than B cells of Devic B7-H1+/+ mice. This observation indicated a relevant immune-modulating role of B7-H1 on APCs (antigen-presenting cells) in this mouse model. We also assumed altered thymic selection processes to be involved in increased peripheral CD4+ T-cell numbers of Devic B7-H1-/- mice as we found more thymocytes expressing the transgenic MOG-specific T-cell receptor (TCR). Moreover, preliminary in vitro experiments hinted on an enhanced survival of TCRMOG-transgenic CD4+ T cells of Devic B7-H1-/- mice; a mechanism that might as well have led to higher peripheral T-cell accumulation. Elevated levels of MOG-specific CD4+ T cells in the periphery of Devic B7-H1-/- mice could have entailed the higher quantities in the CNS. However, mechanisms such as CNS-specific proliferation and/or apoptosis/survival could also have contributed. This should be addressed in future investigations. Judging from in vitro migration assays and adoptive transfer experiments on RAG-1-/- recipient mice, migratory behavior of MOG-specific CD4+ T cells of Devic B7-H1+/+ and Devic B7-H1-/- mice seemed not to differ. However, enhanced expression of the transmigration-relevant integrin LFA-1 on CD4+ T cells in young symptom-free Devic B7-H1-/- mice might hint on temporally differently pronounced transmigration capacities during the disease course. Moreover, we attributed the earlier conversion of CD4+ T cells into Th1 effector cells in Devic B7-H1-/- mice during the initiation phase to the lack of co-inhibitory signaling via PD-1/B7-H1 possibly leading to an accelerated disease onset. Full blown autoimmune inflammatory processes could have masked these slight effects of B7-H1 in the clinical phase. Accordingly, at peak of the disease, Th1 and Th17 effector functions of peripheral CD4+ T cells were comparable in both mouse groups. Moreover, judging from titers of MOG-specific IgG1 and IgM antibodies, alterations in humoral immunity were not detected. Therefore, clinical differences could not be explained by altered T-cell or B-cell effector functions at disease maximum. B7-H1 rather seemed to take inhibitory effect in the periphery during the initiation phase only and consistently within the target organ by parenchymal expression. Our observations indicate that B7-H1 plays a relevant role in the regulation of T-cell responses in this mouse model for spontaneous CNS autoimmunity. By exerting immune-modulating effects in the preclinical as well as the clinical phase of the disease, B7-H1 contributed to the confinement of the immunopathological tissue damage in Devic B7-H1+/+ mice mirrored by later disease onsets and lower disease scores. As a model for spontaneous autoimmunity featuring a close to 100 % incidence rate, the Devic B7-H1-/- mouse may prove instrumental in clarifying disease-triggering and -limiting factors and in validating novel therapeutic approaches in the field of autoimmune neuroinflammation, in particular the human Devic’s disease. N2 - Multiple Sklerose ist eine Autoimmunerkrankung des zentralen Nervensystems, die durch entzündliche, demyelinisierende Läsionen und neuronalen Tod gekennzeichnet ist. Einst als Variante der MS betrachtet, gilt die Neuromyelitis optica (NMO) / Devic-Krankheit heute als eigenständige neurologische Erkrankung, bei der charakteristische Läsionen in den Sehnerven und im Rückenmark jedoch nicht im Gehirn auftreten. Mit der Einführung des doppelt-transgenen "Devic Maus"-Modells, bei dem es zur spontanen, Adjuvans-freien Inzidenz von autoimmuner Neuroinflammation durch Expression transgener MOG-spezifischer T- und B-Zellen kommt, wurde ein vielversprechendes Werkzeug für die Analyse von Faktoren gefunden, die Autoimmunität auslösen bzw. hemmen können. Das ko-inhibitorische Molekül B7-H1 trägt über den PD-1/B7-H1 Signalweg vermeintlich zur Aufrechterhaltung peripherer Toleranz bei. Devic B7-H1-/ - Mäuse entwickelten im Vergleich zu Devic B7-H1+/ + Mäusen Symptome, die mit deutlich höherer Inzidenz und schnellerer Kinetik einhergingen, unterstrichen von verstärkten Krankheitsverläufen und einer nahezu vervierfachten Sterblichkeit. Die verstärkte Akkumulierung von Immunzellen im ZNS, insbesondere von aktivierten MOG-spezifischen CD4+ T-Zellen, korrelierte mit den schwerwiegenderen klinischen Merkmalen. Unsere Untersuchungen zeigten nicht nur, dass die Aktivierung von myelin-spezifischen CD4+ T-Zellen hauptsächlich im ZNS stattfand, sondern auch, dass im Zielorgan exprimiertes B7-H1 maßgeblich den T-Zell-Aktivierungs- und -Differenzierungsgrad beeinflusste. Analysen am Krankheitsmaximum zeigten eine verstärkte Akkumulierung von MOG-spezifischen CD4+ T-Zellen in den Lymphorganen von Devic B7-H1-/- Mäusen, die wir teils auf erhöhte T-Zell-Proliferation zurückzuführten. Transgene MOG-spezifische B-Zellen der Devic B7-H1-/- Mäuse aktivierten effizienter als B-Zellen der Devic B7-H1+/+ Mäuse MOG-spezifische CD4+ T-Zellen. Dies deutet auf eine wichtige immunmodulierende Rolle von B7-H1 auf Antigen-präsentierenden Zellen in diesem Mausmodell hin. Veränderte Selektionsprozesse im Thymus trugen wohlmöglich zu den höheren CD4+ T-Zellzahlen in der Peripherie bei. Vorläufige in vitro Experimente deuteten auf ein verbessertes Überleben von TCRMOG-transgenen CD4+ T-Zellen aus Devic B7-H1-/- Mäusen hin. Eine erhöhte Anzahl von peripheren MOG-spezifischen CD4+ T-Zellen könnte zu den größeren Mengen im ZNS von Devic B7-H1-/- Mäusen geführt haben. Jedoch sind zusätzliche Mechanismen wie ZNS-spezifische Proliferation und/oder Apoptose bzw. Überleben denkbar. Dies sollte in zukünftigen Untersuchungen genauer analysiert werden. Anhand von in vitro-Migrationsassays und Adoptiver Transfer-Experimenten in RAG-1-/- Mäusen schlossen wir, dass das Migrationsverhalten von MOG-spezifischen CD4+ T-Zellen von Devic B7-H1-/- Mäusen nicht verändert war. Allerdings deutet die verstärkte Expression des transmigrationsrelevanten Intergins LFA-1 auf CD4+ T-Zellen in jungen, symptomfreien Devic B7-H1-/- Mäusen auf im Krankheitsverlauf zeitlich verschieden ausgeprägte Transmigrationskapazitäten hin. Die frühere Differenzierung von peripheren CD4+ T-Zellen in Th1-Effektorzellen in Devic B7-H1-/- Mäusen während der Initiationsphase schrieben wir der fehlenden inhibierenden Wirkung des PD-1/B7-H1 Signalwegs zu, was den früheren Krankheitsbeginn bedingt haben könnte. Stark ausgeprägte autoimmune Entzündungsreaktionen am Krankheitsmaximum maskierten jedoch wahrscheinlich diese schwachen Effekte von B7-H1. Dies wurde durch die Tatsache untermauert, dass am Krankheitsmaximum Th1- und Th17-Effektorfunktionen von peripheren CD4+ T-Zellen in beiden Mausgruppen vergleichbar ausgeprägt waren. Des Weiteren bestanden am Krankheitsmaximum keine Unterschiede in der humoralen Immunität. Die beobachteten klinischen Unterschiede waren demnach nicht durch veränderte periphere T-Zell- oder B-Zell-Effektorfunktionen in dieser Krankheitsphase erklärbar. Vielmehr scheint B7-H1 in der Peripherie ausschließlich während der Initiationsphase der Krankheit und fortwährend im Zielorgan durch seine parenchymale Expression immuninhibierend zu wirken. Unsere Beobachtungen zeigen, dass B7-H1 eine relevante Rolle bei der Immunregulierung im vorliegenden Mausmodell für spontane ZNS-Autoimmunität spielt. Durch immunmodulierende Effekte in der präklinischen sowie der klinischen Phase der Krankheit trug B7-H1 zu der Begrenzung der immunpathologischen Gewebeschädigung in Devic B7-H1+/+ Mäusen bei, sichtbar an einem späteren Krankheitsbeginn und leichteren -verlauf. Als Tiermodell für spontane ZNS-Autoimmunität mit nahezu 100 %iger Inzidenz könnte sich die Devic B7-H1-/- Maus als hilfreich bei der Klärung krankheitsauslösender und -limitierender Faktoren erweisen sowie bei der Validierung neuer therapeutischer Ansätze im Bereich der autoimmunen Neuroinflammation, insbesondere der Devic-Krankheit im Menschen.
