TY - THES A1 - Rauchfuß, Steffen T1 - Der Einfluss des Insulins auf die Thrombozyten und auf die Wechselwirkung zwischen den Blutplättchen und dem Endothel T1 - The Effect of Insulin on Platelets and on Platelet-Endothelium Interactions N2 - Unter physiologischen Bedingungen spielen die Thrombozyten oder Blutplättchen eine zentrale Rolle bei der Erhaltung der Hämostase. Indem sie in Blutgefäßen beschädigte Bereiche erkennen und sich dort gezielt anheften können, verhindern sie das Austreten von Blut in subendotheliale Bereiche und halten eine Blutung gering. Intrazelluläre Signalmoleküle kontrollieren das Zusammenspiel der Plättchenagonisten und der dazugehörigen Rezeptoren und regulieren somit die Aktivierung der Blutplättchen. Verschiedene vorangegangene Publikationen demonstrierten sowohl aktivierende als auch inhibierende Effekte des Insulins auf die Aktivierung, Adhäsion und Aggregation der Blutplättchen. Diese durch das Insulin hervorgerufenen Effekte sollen hauptsächlich über den cGMP und cAMP Signalweg sowie über die Aktivierung von eNOS durch Insulin in den Blutplättchen wirken. Unsere Forschungsergebnisse weisen darauf hin, daß ein akuter, durch das Insulin hervorgerufener Effekt auf die Blutplättchen sowohl unter physiologischen wie auch unter pathologischen Glukosekonzentrationen nicht nachweisbar ist. Insulin zeigte keine Wirkung auf die intrazellulären Signalmoleküle PKB, VASP, P38 und ERK, welche in der Aktivierung/Hemmung der Blutplättchen eine wichtige Rolle spielen. Gleichfalls blieb eine Wirkung auf die Aggregation der Blutplättchen und Aktivierung des Oberflächenrezeptors Integrin αIIbβ3 sowie die Expression von P-Selektin auf der Oberfläche der Thrombozyten nach der Stimulation durch Insulin aus. Auch konnte der Insulinrezeptor durch uns auf der Oberfläche der Thrombozyten weder in seiner unphosphorylierten, noch in seiner phosphorylierten Form nach der Stimulation mit Insulin nachgewiesen werden. Zusammen mit dem Fehlen eines direkten, akuten Insulin-abhängigen Effekts auf die Thrombozyten läßt dies auf das Fehlen eines funktionell aktiven Insulinrezeptors auf der Oberfläche der Thrombozyten schließen. Wir konnten zeigen, daß eNOS in den Blutplättchen nicht vorhanden und damit seine in anderen Publikationen beschriebene Aktivierung durch Insulin in den Thrombozyten nicht gegeben ist. Der von uns und in anderen Publikationen verwendete Antikörper gegen phospho-eNOS erkennt vielmehr ein anderes Protein gleicher Größe, wie wir im Kontrollexperiment mit eNOS-/- knockout Mäusen zeigen konnten. Im Flußkammerexperiment konnte ein indirekter insulinabhängiger Effekt auf die Adhäsion der Thrombozyten an das Endothel und ihre Bildung von Aggregaten beobachtet werden. Die Gabe von pathologischen Insulinmengen führte zu einem Anstieg der NO Sekretion durch das Endothel, welches hemmend auf die Adhäsion und Aggregation der Thrombozyten wirkte. N2 - Under physiologic conditions platelets play a central role in upholding the haemostasis. By recognizing lesions in the vessel walls and attachment to those areas they prevent an influx of blood into subendothelial areas. Intracellular molecules mediate the interaction of platelet agonists and their receptors and therefore control platelet activation. Several previous publications demonstrated enhancing as well as inhibiting effects by insulin on platelet activation, adhesion and aggregation. These insulin- mediated effects are reported to act via the cGMP and cAMP pathway as well as the activation of eNOS by insulin in platelets. Our findings show that there is no acute insulin dependent effect on platelet function under physiologic and pathologic glucose levels. Insulin showed no effect on the phosphorylation of intracellular markers of platelet activation like PKB, VASP, P38 und ERK. Furthermore an effect on platelet aggregation and the activation of the platelet surface receptor integrin αIIbβ3 and the expression of P-Selectin on the platelet surface after insulin stimulation was also not detectable. Expression of the IR was not detectable in platelets which agrees with our other data that insulin-stimulation did not trigger IR phosphorylation or activation of any insulin-induced downstream signalling pathways in platelets including eNOS, PKB, ERK, and p-38 MAPK. These data indicate that either there is no functionally active IR expressed in platelets or that the copy number of IR proteins is too low to activate downstream signalling systems like PKB and MAPK. We could also show that eNOS is not present in platelets and that its activation by insulin, postulated in previous publications, is therefore not given. The antibody which we employed and which was also used in other publications against phospho-eNOS recognizes another protein of the same size, which we could demonstrate in a control experiment with eNOS-/- knock out mice. However, we could detect an indirect insulin dependent effect on platelet endothel adhesion and aggregation in our flow chamber experiments. The insulin induced increase of endothelial NO caused a significant decrease of platelet adhesion to the endothelial cells.
