Methods: 77 Patienten wurden in die transgluteale laterale Zugangsweg Kohorte randomisiert, wovon 50 Patienten die Studie beendeten. Alle Patienten wurden mit dem Harris Hip Score (HHS), dem Xtra Short Musculosceletal Functional Assesment Questionaire (XSFMA), dem 36-Item Short Form Survey (SF-36), einem Stepwatch™-Aktivitätsmonitor (SAM) und einem zeitabhängigen 25m-Gehtest bewertet. Mit Hilfe der postoperativen Standard- röntgenaufnahmen wurde die Inklination, Schaftpositionierung und das Offset bewertet. Results: Unsere Patienten verbesserten sich nach drei Monaten in Bezug auf den 25-m-Gehtest (prä-operativ 24 Sekunden, SD ± 6,07, 3 Monate 19,36 Sekunden, SD ± 3,80) und die vom Patienten geschätzte maximale Gehstrecke nicht signifikant (präoperativ 1187, SD ± 1809m und 12 Monate 5125m, SD ± 3867m). Mit der Stepwatch konnte ein postoperativer Anstieg der mittleren Gehzyklen pro Tag gemessen werden (präoperativ 4964, SD ± 2687, 3 Monate 5238, SD ± 2309, 12 Monate 5340, SD ± 1887). Der Bewegungsumfang verbesserte sich insbesondere für die Flexion (präoperativ 89°, 3 Monate 97,34°) und die Abduktion (präoperativ 25,31°, 3 Monate 37,45°) und blieb im weiteren Verlauf nahezu unverändert. Der HHS zeigte einen signifikanten Unterschied zwischen dem präoperativen Score (52,95, SD ± 15,69) und den Scores aller postoperativen Messzeitpunkte (3 Monate 88,40, SD ± 9,89, 6 Monate 89,05, SD ± 10,01, 12 Monate 91,43, SD ± 9,05, p <0,001). Gleiches gilt für den XSFMA-Funktionsindex, der sich zwischen dem präoperativen Score (40,50, SD ± 15,78) und den postoperativen Scores signifikant verbesserte (3 Monate 18,80, SD ± 16,12, 6 Monate 15,84, SD ± 15,39, 12 Monate 15,08, SD ± 16,33, p<0.001). Der SF-36-Score mit der Subgruppe der körperlichen Verfassung betrug präoperativ 25,62, SD ± 8,74 und war postoperativ ebenfalls signifikant höher (3 Monate 40,67, SD ± 10,01, 6 Monate 42,76, SD ± 9,59, 12 Monate 42,91, SD ± 11,86, p < 0,05), während der Subscore der psychischen Beurteilung sich stets auf konstantem Niveau befand (präoperativ 56,26, SD ± 9,23, 3 Monate 56,68, SD ± 8,23, 6 Monate 55,81, SD ± 7,22, 12 Monate 56,23, SD ± 6,92, p <0,05). Die Bewertung der postoperativen Röntgenbildgebung ergab eine durchschnittliche Pfanneninklination von 40,28 ° und einen femorales Offset von 38,1mm ohne Anzeichen von Migration. Conclusion: Zusammenfassend zeigen unsere Ergebnisse, dass sich bereits drei Monate nach Implantation einer Totalhüftendoprothese über den transglutealen lateralen Zugang die Lebensqualität, das Aktivitätsniveau, die Patientenzufriedenheit und der postoperative Bewegungsumfang signifikant verbessert haben. N2 - Background: This prospective randomized controlled single-centre study investigates the clinical outcome 3 up to 12 months after total hip arthroplasty using a transgluteal lateral approach (Bauer). Methods: 77 patients were randomized to the transgluteal lateral approach cohort and 50 of them completed the trial. All patients were evaluated using the Harris Hip Score (HHS), the extra short musculoskeletal functional assessment questionnaire (XSFMA), the Short Form 36 (SF-36) health survey, a Stepwatch™ Activity Monitor (SAM), and a timed 25 m walking test. On standard postoperative radiological x-rays the inclination and stem-positioning was evaluated. Results: Our patients improved non significantly after three months regarding to the 25 m walking test (pre-op 24 sec., SD ± 6.07, 3-months 19,36 sec., SD ± 3.80) and to the expected maximal walking distance, guessed by the patient (pre-op 1187, SD ± 1809m and 12-month 5125m, SD ± 3867m). Using the Stepwatch a postoperative increase of the mean walking cycles per day was measured (pre-op 4964, SD ± 2687, 3-month 5238, SD ± 2309, 12-month 5340, SD ± 1887). Range of motion improved especially in relation to flexion (pre-op 89°, 3-month 97,34°) and abduction (pre-op 25.31°, 3-month 37.45°) and showed just slight improvement during the 6- and 12-month follow up. The HHS showed a significant difference between the preoperative score (52.95, SD ± 15.69) and the scores to all postoperative measurement times (3-month 88.40, SD ± 9.89, 6-month 89.05, SD ± 10.01, 12-month 91.43, SD ± 9.05, p<0.001). Same did the XSFMA functional index score which improved significantly between the preoperative score (40.50, SD ± 15.78) and the postoperative scores (3-month 18.80, SD ± 16.12, 6-month 15.84, SD ± 15.39, 12-month 15.08, SD ± 16.33, p<0.001). The SF-36 physical component score was 25.62, SD ± 8.74 pre-operatively and rised significantly (3-month 40.67, SD ± 10.01, 6-month 42.76, SD ± 9.59, 12-month 42.91, SD ±11.86, p<0.05 ) while the mental component score was almost equal during the whole trial (pre-op 56.26, SD ± 9.23, 3-month 56.68, SD ± 8.23, 6-month 55.81, SD ± 7.22, 12-month 56.23, SD ± 6.92, p<0.05). The radiographic analysis revealed an average cup inclination of 40.28° and a femoral offset of 38.1mm without signs of migration. Conclusion: In summary, our outcomes show that already three months after the implantation of a total hip arthroplasty via a transgluteal lateral approach the patients improved significantly regarding to the quality of life, level of activity, satisfaction and postoperative range of motion. Methods: A total of 123 arthroplasties were evaluated utilizing the Harris Hip Score (HHS), the extra short musculoskeletal functional assessment questionnaire (XSFMA), the Short Form 36 (SF-36) health survey, a Stepwatch™ Activity Monitor (SAM), and a timed 25 m foot walk (T25-FW). Postoperative x-ray images after THA were reviewed to determine inclination and stem positioning. Results: At final follow-up, the XSFMA functional index scores were 10.3 (anterior) and 15.08 (lateral) while the bother index summed up to a score of 15.8 (anterior) and 21.66 (lateral) respectively, thus only differing significantly for the functional index (p = 0.040 and p = 0.056). The SF-36 physical component score (PCS) was 47.49 (anterior) and 42.91 (lateral) while the mental component score (MCS) summed up to 55.0 (anterior) and 56.23 (lateral) with a significant difference evident for the PCS (p = 0.017; p = 0.714). Patients undergoing THA through a DAA undertook a mean of 6402 cycles per day while those who had undergone THA through a transgluteal approach undertook a mean of 5340 cycles per day (p = 0.012). Furthermore, the obtained outcome for the T25-FW with 18.4 s (anterior) and 19.75 s (lateral) and the maximum walking distance (5932 m and 5125 m) differed significantly (p = 0.046 and p = 0.045). The average HHS showed no significant difference equaling 92.4 points in the anterior group and 91.43 in the lateral group (p = 0.477). The radiographic analysis revealed an average cup inclination of 38.6° (anterior) and 40.28° (lateral) without signs of migration. Conclusion: In summary, our outcomes show that after 1 year THA through the direct anterior approach results in a higher patient activity compared to THA utilizing a transgluteal lateral approach while no differences regarding hip function are evident.
