N2  - Analyse zur Diagnostik und Therapie der kardialen und pulmonalen Sarkoidose. Die Problematik der Diagnose einer Sarkoidose besteht in dem Fehlen eines einzelnen diagnostischen Bestätigungstests. Einem internationalen Konsens entsprechend sind ein kompatibles klinisches und radiologisches Bild, der histopathologische Nachweis von nicht-verkäsenden Granulomen und der Ausschluss von Differentialdiagnosen für die Diagnose einer pulmonalen Sarkoidose entscheidend. In der Diagnostik der kardialen Sarkoidose gibt es keinen Referenzstandard oder international akzeptierten Standard. Trotz widersprüchlicher Datenlage und teils fehlender Wirkungsnachweise ist die immunsuppressive Therapie, in erster Linie mit Kortikosteroiden, die Standardtherapie sowohl der pulmonalen wie auch der kardialen Sarkoidose.

According to an international consensus, a compatible clinical and radiological picture, the histopathological detection of non-casing granulomas and the exclusion of differential diagnoses are decisive for the diagnosis of pulmonary sarcoidosis. There is no reference or internationally accepted standard in the diagnosis of cardiac involvment. Despite contradicting data and a lack of evidence of effectiveness, immunosuppressive therapy, primarily with corticosteroids, is the standard therapy for both pulmonary and cardiac sarcoidosis.
