Dies erschwert die klinische Diagnosestellung der Erkrankung. Nahezu alle Patienten berichteten von starken Knochen-, Gelenk,- und Muskelschmerzen, von Karies und Parodontose sowie von vermehrten Frakturen, die zum Teil weitere chronische Schmerzen und Wiederholungsfrakturen erzeugen. Eine deutlich verminderte Leistungsfähigkeit im Vergleich zu Gleichaltrigen wurde ebenso häufig angegeben. Es konnte keine eindeutige Phänotyp -  Genotyp Korrelation gefunden werden, allerdings geben die Daten einen deutlichen Hinweis, dass Patienten mit zwei Mutationen am stärksten symptomatisch betroffen sind. Ebenfalls konnten keine Unterschiede zwischen dominant negativen Mutationen und nicht dominant negativen Mutationen gefunden werden. Almost all patients reported severe bone, joint, and muscle pain, caries and periodontal disease, as well as increased fractures, some of which produced further chronic pain and recurrent fractures. Significantly reduced performance compared to peers was also reported frequently. No definite phenotype - genotype correlation could be found, but the data provide a clear indication that patients with two mutations are most symptomatically affected. Verursacht wird die Erkrankung durch Mutationen im ALPL-Gen, welches für die gewebeunspezifische Alkalische Phosphatase codiert und mit einem Funktionsverlust des Enzyms einhergeht. Die Ausprägung der klinischen Symptomatik ist sehr heterogen und reicht von milden und unspezifischen bis hin zu potenziell lebensbedrohlichen Symptomen, was die korrekte Diagnose zusätzlich erschwert und verzögert. Um das Verständnis der pädiatrischen HPP zu verbessern und die Dauer von Symptombeginn bis zur korrekten Diagnose zu verkürzen, haben wir den Verlauf der Erkrankung anhand einer retrospektiven Aufarbeitung der Daten von 50 pädiatrischen HPP Patienten, die in den letzten 25 Jahren an der Universitäts-Kinderklinik in Würzburg vorstellig waren, untersucht. Diese Ergebnisse bestätigen den klinischen Eindruck der HPP als chronische Systemerkrankung, welche aufgrund ihrer unspezifischen klinischen Präsentation oftmals nur mit zeitlicher Verzögerung diagnostiziert wird. Dieser Verzögerung kommt insbesondere im Hinblick auf die 2015 zur Behandlung der pädiatrischen HPP zugelassenen Enzymersatztherapie mit dem Wirkstoff Asfotase alfa eine besondere Bedeutung zu, da die Patienten von einer frühzeitigen Diagnose und einem damit einhergehenden frühzeitigen Beginn der Behandlung profitieren können. Diese Ergebnisse tragen einen Teil dazu bei, das Bewusstsein und die Kenntnis der Erkrankung zu verbessern, um so die die Zeitspanne zwischen Symptombeginn und Diagnosestellung zu verkürzen und die medizinische Versorgung der Patienten zu verbessern. N2 - Hypophosphatasia (HPP) is a rare, inherited metabolic disorder caused by loss-of-function mutations in the ALPL gene that encodes the tissue-nonspecific alkaline phosphatase TNAP (ORPHA 436). Its clinical presentation is highly heterogeneous with a remarkably wide-ranging severity. HPP affects patients of all age. Therefore, diagnosis is often difficult and delayed. To improve the understanding of HPP in children and in order to shorten the diagnostic time span in the future we studied the natural history of the disease in our large cohort of pediatric patients. In light of the enzyme replacement therapy (Asfotase alfa, a recombinant mineral-targeted TNAP), HPP patients may benefit from early treatment in the course of the disease. This single centre retrospective chart review included longitudinal data from 50 patients with HPP diagnosed and followed at the University Children`s Hospital Wuerzburg, Germany over the last 25 years. Reported findings support our clinical impression of a chronic multi-systemic disease with often delayed diagnosis. Our natural history information provides detailed insights into the prevalence of different symptoms which can help to improve and to shorten diagnostics and thereby lead to an optimised medical care, especially with promising therapeutic options like enzyme-replacement-therapy with Asfotase alfa in mind. Der vorzeitige Verlust von Milchzähnen ist das zahnmedizinische Leitsymptom und in vielen Fällen ein erstes Anzeichen dieser Erkrankung. In dieser Arbeit wurde ein in vitro Modell der HPP etabliert und der Fokus auf die dentalen Aspekte dieser Erkrankung gelegt. Hierzu wurden mesenchymale Stammzellen (MSCs) aus Bereichen analysiert, die bei einer Erkrankung von dieser Mineralisierungsstörung betroffen sind. Es wurden dentale Stammzellen aus der Pulpa (dental pulp stem cells, DPSCs) und dem parodontalen Ligament (periodontal ligament stem cells, PDLSCs) isoliert und im Vergleich zu Stammzellen aus dem Knochenmark (bone marrow mesenchymal stem cells, BMSCs) charakterisiert. Um den Einfluss der Spendervariabilität zu reduzieren, wurden aus dem gesamten dentalen Probenmaterial nur vollständige Probenpaare aus DPSCs und PDLSCs von 5 Spendern für die vergleichenden Analysen verwendet. Die dentalen MSCs konnten somit paarweise direkt miteinander verglichen werden. DPSCs gelten seit ihrer Entdeckung von Gronthos et al. im Jahr 2000 als geeignete Quelle für die Stammzellgewinnung mit vielversprechenden Anwendungsmöglichkeiten im Bereich des Tissue Engineering und der regenerativen muskuloskelettalen Medizin. PDLSCs sind aufgrund der parodontalen Problematik der HPP von besonderem Interesse in dieser Arbeit. Die Isolation von Stammzellen aus Pulpa und PDL konnte mit dem Nachweis der sogenannten Minimalkriterien für MSCs bestätigt werden. In diesem durch Enzyminhibition mit Levamisol induzierten in vitro Modell der HPP wurde die TNAP-abhängige Genexpression, die Enzym-Aktivität und das osteogene Differenzierungspotenzial an diesen drei Mineralisierungs-assoziierten MSCs untersucht. Die erweiterte Genexpressionsanalyse in Kooperation mit der Core Unit Systemmedizin der Universität Würzburg mit einer RNA-Sequenzierung der PDLSCs ergab interessante Einblicke in die differentielle Genexpression nach der TNAP-Inhibition während der osteogenen Differenzierung und Ansatzpunkte für weitere Analysen. Die beobachteten Genregulationen waren nach dem derzeitigen Verständnis pathologischer Zusammenhänge nachvollziehbar und simulierten in vitro HPP-relevante Signalwege repräsentativ. Insbesondere die signifikante Genregulation von P2X7 und DMP1, sowie Zusammenhänge aus dem Wnt-Signalweg zeigen hinsichtlich der dentalen Aspekte der HPP neue Ansatzpunkte auf. Die erhöhte P2X7-Expression in diesem in vitro HPP-Modell scheint mit der Parodontitis-Problematik der HPP zu korrelieren und verdeutlicht unter anderem die multifaktorielle Ätiologie und Pathogenese der Parodontitis. Die Tatsache, dass die experimentellen Beobachtungen in Einklang mit dem klinischen Bild der HPP gebracht werden können, bestätigt die Relevanz des hier etablierten in vitro Modells. Zusammenfassend konnten anhand dieses in vitro Modells der HPP neue Aspekte aufgedeckt werden, die nicht nur im Hinblick auf die dentale Problematik der HPP aufschlussreich sind. N2 - In the rare clinical picture of hypophosphatasia (HPP) skeletal and dental symptoms occur in very variable severity due to a dysfunction of the enzyme tissue-nonspecific alkaline phosphatase (TNAP). The premature loss of deciduous teeth is the main dental symptom and in many cases the first sign of this disease. In this thesis, an in vitro model for HPP was established and the research focus was put on the dental aspects of this disease. Mesenchymal stem cells (MSCs) from areas affected by this mineralization disorder were analyzed. Dental pulp stem cells (DPSCs) and periodontal ligament stem cells (PDLSCs) were isolated and characterized in comparison to bone marrow mesenchymal stem cells (BMSCs). In order to reduce the influence of donor variability, only complete pairs of DPSCs and PDLSCs from five donors were used for comparative analysis. The mesenchymal stem cell character of the isolated cells from the dental pulp and the PDL was confirmed by the verification of the so-called minimal criteria for MSCs. In this in vitro model for HPP, which was induced by inhibition of TNAP enzyme with levamisole, the TNAP-dependent gene expression, the enzyme activity and the osteogenic differentiation potential of these three mineralization-associated MSCs were investigated. Additionally, an extended gene expression analysis was performed in cooperation with the Core Unit System Medicine of the University of Würzburg using the RNA sequencing technique. Analysis of RNAs isolated from PDLSCs provided interesting insights into the differential gene expression due to TNAP inhibition during osteogenic differentiation and consequently starting points for further analyses. The observed gene regulation was in line with the current understanding of pathological relationships and simulated HPP-relevant signaling pathways in vitro. In particular, the significant gene regulation of P2X7 and DMP1, as well as correlations from the Wnt signaling pathway, reveal new considerable approaches for the dental aspects of HPP. The increased P2X7 expression in this in vitro HPP model seems to correlate with the periodontal disease of HPP and illustrates, among other things, the multifactorial etiology and pathogenesis of periodontitis. The fact that the experimental observations can be reconciled with the clinical picture of HPP confirms the relevance of the in vitro model established here. In summary, this in vitro model for HPP has revealed a number of new interesting aspects - not only with regard to the dental problems of HPP. Störungen dieses genau geregelten Prozesses resultieren in schwerwiegenden Erkrankungen, wie z.B. bei der Hypophosphatasie [3]. Dieser meist autosomal rezessiv vererbten Erkrankung liegt eine durch genetische Mutationen beeinträchtigte Funktion der TNSALP zugrunde, wodurch sich die PPi- Konzentration im Microenvironment der Zelle erhöht. Diese kann im Knochengewebe zu schweren Mineralisierungsstörungen führen [1, 2]. Andere Krankheiten, mit erniedrigten PPi- Konzentrationen, werden mit pathologischen Verkalkungen in verschiedensten Geweben in Verbindung gebracht [4, 5]. Diese gehen unter anderem auf genetische Defekte von ENPP1 zurück[4]. Auch der Mevalonat-Pathway trägt zur Komposition des Microenvironments bezüglich der Homöostase von Phosphaten bei [6, 7]. Hier bestehen auch medizinisch relevante Einflussmöglichkeiten, zum Beispiel durch Bisphosphonate, bei der sogenannten Volkskrankheit Osteoporose. In dieser Arbeit wurden die Auswirkungen einer PPi-Belastung auf die in vitro Mineralisierung von Mesenchymalen Stammzellen untersucht, wobei Modulatoren der Enzymaktivität für ALP und ENPP1 und der Aktivität des PPi-Kanals ANKH sowie des Mevalonatstoffwechsels zum Einsatz kamen (PPi, Pyridoxalphosphat (PLP), Probenecid, Vitamin D, PPADS (Pyridoxalphosphat-6-azophenyl-2‘,4‘-disulfid Säure) und ß-γmeATP (ß-γ Methylentriphosphat)). Die Resultate zeigen, dass die Modulation der PPi-Konzentration bei der osteogenen Differenzierung von hMSCs in vitro keine eindeutigen Effekte bewirkt. Geringe Änderungen des Genexpressionsmusters sind letztlich nicht auszuschliessen, blieben jedoch aufgrund der hohen Spendervariabilität durch eine erhöhte Anzahl von Experimenten zu beweisen. Diese Arbeit zeigt insgesamt eine unerwartet geringe Auswirkung einer exogenen und endogenen Modulation der PPi-Konzentration sowohl mit Blick auf die rein physikalischen Phänomene der Mineralisierung, als auch mit Blick auf die untersuchte Genregulation der wichtigsten beteiligten Proteine, was möglicherweise die hohe Kompensationskapazität der Systeme unter physiologischen Bedingungen reflektiert. Untersuchungen auf proteomischer Ebene, besonders mit Blick auf die Prozessierung von Polypeptiden mit Mineralisierungs-modulierender Wirkung würden möglicherweise genaueren Einblick vermitteln. Eine genauere Untersuchung der Einflüsse von ENPP1 erscheint für die Zukunft vielversprechend. Allerdings treten hier, besonders auch durch die verwendeten Hemmstoffe der ENPP1, die Phänomene der Vernetzung des Stoffwechsels der Phosphate (inklusive ATP und seiner Metabolite) mit dem Purinergen Signalling deutlich zutage. Diese Vernetzung generiert durch ihre Komplexität sowohl klinisch als auch zellbiologisch/biochemisch erhebliche Interpretationsprobleme, die zukünftige Arbeiten auflösen müssen. N2 - Together, the enzymes TNSALP (Tissue Non-Specific Alkaline Phosphatase), ENPP1 (Ectonucleotide Pyrophosphatase/Phosphodiesterase 1) and ANKH (Ankylosis, progressive human homolog) form a central regulation entity for the cellular metabolism of pyrophosphate (PPi)[1, 2]. Dysregulation of these coordinated processes result in severe diseases, such as Hypophosphatasia (HPP) [3]. This condition is caused by an autosomal recessive inheritance pattern, which restricts the function of TNSALP, thus resulting in an increased concentration of PPi in the micro-environment of the cell. This can lead to severe disruption of skeletal mineralization [1, 2]. Other diseases with low PPi concentrations are associated with the pathological calcification of different tissues [1, 5] and can be traced back to genetic defects of ENPP1 [1]. The mevalonate pathway contributes to the composition of the micro-environment and hence to the homeostasis of phosphates [6, 7]. This constitutes a medically relevant possibility of influence, for example through bisphosphonates as a treatment for widespread diseases like Osteoporosis. This study analyzed the impact of a PPi exposure on the in vitro mineralization of human mesenchymal stem cells (hMSCs) in the process of osteogenic differentiation. For this purpose, we used enzymatic activity modulators for ALP, ENPP1 as well as for ANKH and the Mevalonate pathway (PPi, Pyridoxalphosphate, Probenecid, Vitamine D, PPADS (Pyridoxalphosphate-6-azophenyl-2‘,4‘-disulfid acid) and ß-γmeATP (ß-γ Methylentriphosphate)). The results show no clear effects due to the modulation of the PPi concentration during osteogenic differentiation of hMSCs in vitro. Minor changes in genetic expression patterns cannot be ruled out due to an elevated variability among the donor cells, said discrepancy would have to be consolidated through an increased number of experiments. Altogether, this study shows unexpectedly low impacts of exogenic an endogenic modulation of the PPi concentration, in regards to the physical effects of mineralization as well as the genetic regulation of the key proteins involved. This could be a reflection of the compensation capacity of these mechanisms under physiological circumstances. In order to provide indepth insight into this matter, further examination on a proteomic level would be necessary, especially with an outlook onto the processing of polypeptides with mineralization-modulating effects. A promising strategy for future studies seems to be a further investigation of the effects of ENPP1. However, this approach will be confronted, especially due to inhibitors of ENPP1, with the complex networking of the phosphate metabolism (included ATP and his metabolites) with purinerg signaling. Due to its complexity, this interconnectedness generates considerable interpretation issues on a clinical as well as a cell biological level, which would have to be investigated further in future studies. The focus here should be put on two results of potential clinical significance for HPP-patients, namely the unfavorable effects on the ALPL-expression of a Probenecid therapy as well as the increased expression of ALPL during ENPP1 inhibition. 1. Die daraus resultierenden Mineralisierungsstörungen betreffen sowohl den Knochen als auch in milderen Ausprägungsformen die Zähne und den Zahnhalteapparat. Das zahnmedizinische Leitsymptom und in vielen Fällen das erste Anzeichen der HPP ist dabei der vorzeitige Verlust der Milchzähne ohne physiologische Wurzelresorption. Im Rahmen dieser Arbeit wurden verschiedene TNAP defiziente immortalisierte Zellen des parodontalen Ligaments (PDL) mittels der CRISPR/Cas9 Methode generiert und anschließend fünf Zelllinien charakterisiert. Die dabei entstandenen Mutationen variierten von einer moderaten heterozygoten Punktmutation zu einer schwerwiegenden homozygoten Deletion eines einzelnen Nukleotids, welche in einem vorzeitigen Stopcodon resultierte. Analysen der ALPL Expression (qPCR), TNAP Aktivitätsmessungen (CSPD Assay) und TNAP Färbungen zeigten einen signifikanten Rückgang in allen TNAP-defizienten Zelllinien mit einer starken Korrelation zwischen der Restaktivität und dem Ausmaß der Mutation, welche in Einklang mit der komplexen Genotyp-Phänotyp Korrelation bei HPP zu bringen ist. Das Potential der osteogenen Differenzierung der hTERT PDL Zellen wurde in der homozygot mutierten Zelllinie komplett unterdrückt. Mögliche Mechanismen des vorzeitigen Zahnverlustes bei HPP Patienten ist die geminderte Formation und Mineralisation des Wurzelzements und die fehlerhafte Insertion der parodontalen Fasern. Die hier erstmalig etablierten Zellkulturmodelle liefern ein valides spenderunabhängiges in vitro Modell der HPP, welches dazu beitragen kann, die molekularbiologischen Zusammenhänge der dentalen Aspekte der Hypophosphatasie zu ergründen und daraus gegebenenfalls neue Therapieansätze abzuleiten. N2 - Hypophosphatasia (HPP) is a rare inherited disorder caused by loss-of-function mutations in the ALPL gene encoding the Tissue Nonspecific Alkaline Phosphatase (TNAP). Besides skeletal symptoms, some patients also present dental abnormalities like for example the premature loss of deciduous teeth. Here we generated and characterized five different TNAP-deficient periodontal ligament (PDL) derived cell lines using the method of CRISPR-Cas9. The mutations varied from a moderate heterozygous point mutation to a severe homozygous deletion leading to a premature stop codon. Analysis of the ALPL expression and TNAP activity measurements in CSPD Assays and TNAP stainings revealed a decrease for all TNAP-deficient cell lines with a strong correlation between the residual activity and the extend of the mutation. The already limited differentiation capacity of immortalized hTERT (human telomerase reverse transcriptase) PDL cells is completely abolished in the homozygously mutated cell line. Putative key mechanisms for the premature exfoliation in HPP are the restricted formation and mineralization of the cementum and the impaired insertion of elastic dental fibers. The newly generated TNAP-deficient cell lines provide a promising and donor independent in vitro model to gain better understanding of the molecular mechanisms of dental problems in HPP.
