Die Daten wurden zwischen HHP-Patienten mit einer und zwei Mutationen auf dem für die alkalische Phosphatase kodierenden Genabschnitt (ALPL-Gen), sowie mit den Ergebnissen der DMS V verglichen, welche die Mundgesundheit der deutschen Allgemeinbevölkerung repräsentiert. 80 Patienten wurden untersucht; 64 von ihnen waren weiblich, 18 männlich. Das Mittlere Alter lag bei 46,4 Jahren. 55 Studienteilnehmer wiesen eine Mutation und 18 zwei Mutationen des ALPL-Gens auf (n=7 keine Testung). Patienten, die von zwei Mutationen des für die alkalische Phosphatase kodierenden Genabschnitts betroffen waren, zeigten ein höheres Risiko für Parodontitis und Zahnverlust als HPP-Patienten mit nur einer Mutation, sowie im Vergleich mit der deutschen Allgemeinbevölkerung. Allerdings stiegen auch bei Patienten mit nur einer Mutation mit zunehmendem Lebensalter die Ausprägungen parodontaler Erkrankungen, sowie die Zahnverlustrate im Vergleich zur Durchschnittsbevölkerung in Deutschland. N2 - This study evaluated the oral health status of adult hypophosphatasia (HPP) patients including the age of first deciduous tooth loss, number of lost permanent teeth, Decayed/Missing/Filled Teeth (DMF/T) Index, Bleeding on Probing (BOP), Probing Pocket Depth (PPD), Clinical Attachment Level (CAL), agomphiasis and periodontal health status according to CCD/AAP criteria. Data were compared inside HHP-patients with one and two ALPL gene mutations and to the findings of the DMS V, which represents the oral health status of the general German population. 80 patients were examined (64 female / 18 male). The mean age were 46.4 years. 55 attendees of the study had one mutation of the ALPL gene and 18 two mutations (7 no testing). HPP-patients affected by two mutations of the ALPL gene showed a higher risk for periodontitis and tooth loss than patients with one mutation and general German population. With increasing age HPP-patients with one mutation also suffer from something more pronounced periodontal diseases and a higher tooth-lost rate than general German population. Findings were compared with national reference data (DMS V survey) reporting oral health status in age‐related controls. Within‐group comparisons were made between the HPP patients harbouring one versus two alkaline phosphatase liver/bone/kidney type (ALPL) gene variants. Results Of 80 HPP patients (64 female) with a mean age of 46.4 years (range 24–78) and one (n = 55) or two (n = 18) variants (n = 7 lacking testing) within the ALPL gene, those with two variants displayed substantially higher tooth loss rate (14.0 ± 9.3) than those affected by only one ALPL variant (4.1 ± 5.4), who did not differ substantially from healthy DMS V controls. While DMFT score and severe periodontal diseases (PDs) of HPP patients with one variant only increased with progressing age, the two‐variant sub‐cohort age independently exhibited increased DMFT scores and a higher rate of severe PDs. Conclusions HPP patients affected by two variants of the ALPL gene exhibited a higher risk of periodontitis and tooth loss than the general population, while patients with one variant developed clinically relevant oral disease symptoms with progressing ageing. Materials and methods Eighty bovine teeth cut to a length of 18 mm (crown 8 mm, root 10 mm) were randomly assigned to 10 groups (n = 8), receiving orthograde apical plug treatment (APT). Apical plugs were 4 mm in length and made of ProRoot MTA (Dentsply), Medcem MTA (Medcem), TotalFill BC RRM Fast Set Putty (Brasseler), or Medcem Medical Portland Cement (Medcem) plus bismuth oxide (Bi2O3) with and without bovine blood. Further, orthograde (with or without preoperative adhesive coronal dentin sealing) and retrograde APT were compared. Teeth were obturated with gutta-percha and sealer, sealed with composite and stored in distilled water. Tooth color was measured on apical plug, gutta-percha/sealer, and crown surface before treatment versus 24 h, 1, 3, 6, 12, and 24 months after treatment by spectrophotometry. Color difference (ΔE) values were calculated and analyzed by Shapiro-Wilk test, ANOVA with post hoc tests, Friedman test, t test, and post hoc tests with Bonferroni correction (α = .05). Results Tooth discoloration occurred in all groups with no significant differences between HCSCs (p > .05). After 24 months, color changes were prominent on roots but insignificant on crowns. Blood contamination induced a significantly decreased luminescence (p < .05). Blood had a stronger impact on tooth color than Bi2O3. No relevant effects of retrograde placement (p > .05) or preoperative dentin sealing (p > .05) were detected. Conclusions Apical plugs of the tested HCSCs cause discoloration of bovine roots, but not discoloration of bovine tooth crowns within a 24-month period. Clinical relevance APT should be performed carefully while avoiding direct contact with the coronal dentin, and in that case no aesthetic impairments occur. The aim of this explorative study was to investigate the role of Interleukin-(IL)-1, IL-4, GATA-3 and Cyclooxygenase-(COX)-2 polymorphisms after non-surgical periodontal therapy with adjunctive systemic antibiotics (amoxicillin/metronidazole) and subsequent maintenance in a Caucasian population. Analyses were performed using blood samples from periodontitis patients of a multi-center trial (ClinicalTrials.gov NCT00707369=ABPARO-study). Polymorphisms were analyzed using quantitative real-time PCR. Clinical attachment levels (CAL), percentage of sites showing further attachment loss (PSAL) ≥1.3 mm, bleeding on probing (BOP) and plaque score were assessed. Exploratory statistical analysis was performed. A total of 209 samples were genotyped. Patients carrying heterozygous genotypes and single-nucleotide-polymorphisms (SNP) on the GATA-3-IVS4 +1468 gene locus showed less CAL loss than patients carrying wild type. Heterozygous genotypes and SNPs on the IL-1A-889, IL-1B +3954, IL-4-34, IL-4-590, GATA-3-IVS4 +1468 and COX-2-1195 gene loci did not influence CAL. In multivariate analysis, CAL was lower in patients carrying GATA-3 heterozygous genotypes and SNPs than those carrying wild-types. For the first time, effects of different genotypes were analyzed in periodontitis progression after periodontal therapy and during supportive treatment using systemic antibiotics demonstrating a slight association of GATA-3 gene locus with CAL. This result suggests that GATA-3 genotypes are a contributory but non-essential risk factor for periodontal disease progression. While for a long time, a diet as low in nitrates as possible was advocated on the basis of epidemiological data suggesting a cancer-promoting effect of nitrate-rich diets, more recent findings show that dietary nitrate, after its conversion to nitrite by nitrate-reducing bacteria of the oral microbiota, is an indispensable alternative source for the formation of nitric oxide (NO), which comprises a key element in the physiology of a variety of central body functions such as blood pressure control, defense against invading bacteria and maintenance of a eubiotic microbiota in the gut and oral cavity. This compact narrative review aims to present the evidence supported by clinical and in vitro studies on the ambivalent nature of dietary nitrates for general and oral health and to explain how the targeted adjuvant use of nitrate-rich diets could open new opportunities for a more cause-related control of caries and periodontal disease. Methods: A total of 39 study patients were enrolled. At baseline, bleeding on probing (BoP; primary outcome), gingival index (GI), plaque control record (PCR), recession (REC) and probing pocket depth (PPD) for the calculation of the periodontal inflamed surface area (PISA) were documented. After subsequent professional mechanical plaque removal (PMPR), participants were randomly provided with a supply of sachets containing either a specific collagen peptide preparation (test group; n = 20) or a placebo (placebo group; n = 19) to be consumed dissolved in liquid once daily until reevaluation at day 90. Results: PMPR supplemented with the consumption of the specific collagen peptides resulted in a significantly lower mean percentage of persisting BoP-positive sites than PMPR plus placebo (test: 10.4% baseline vs. 3.0% reevaluation; placebo: 14.2% baseline vs. 9.4% reevaluation; effect size: 0.86). Mean PISA and GI values were also reduced compared to baseline, with a significant difference in favor of the test group (PISA test: 170.6 mm\(^2\) baseline vs. 53.7 mm\(^2\) reevaluation; PISA placebo: 229.4 mm\(^2\) baseline vs. 184.3 mm\(^2\) reevaluation; GI test: 0.5 baseline vs. 0.1 reevaluation; GI placebo: 0.4 baseline vs. 0.3 reevaluation). PCR was also significantly decreased in both experimental groups at revaluation, but the difference between the groups did not reach the level of significance. Conclusions: The supplementary intake of specific collagen peptides may further enhance the anti-inflammatory effect of PMPR in periodontal recall patients. Weiterhin wurde der Einfluss der Oberflächenbehandlung sowie des Zwischenmediums zwischen Lichtleiter und Keramikoberfläche untersucht. Dazu wurden plane Proben mit einer Schichtstärke von 1,25 mm, 2,5 mm, 5 mm, 7,5 mm der Keramik IPS e.max CAD und mit einem Sägemikrotom gesägt und anschließend kristallisiert. Die Messung des Transmissionsgrades des Lichtes eines LED-Lichtgerätes erfolgte mit einem faseroptischen Spektralfotometer. Je dicker, opaker und dunkler eine Keramikprobe ist, desto geringer ist ihr Transmissionsgrad. Die Abnahme des Transmissionsgrades konnte durch das Lambert-Beer‘sche-Gesetz beschrieben werden. Der Einfluss der Reflexion und / oder Streuung an der Oberfläche muss als Absorptionsgrad α miteinbezogen werden. Der Absorptionsgrad α ist von der Oberflächenbehandlung und dem Medium zwischen Lichtleiter und Keramikoberfläche abhängig. Die Durchführung einer multiplen linearen Regressionsanalyse zeigt, dass die Schichtstärke, Transluzenz, Helligkeitsstufen und die Farben einen signifikanten Einfluss auf den Transmissionsgrad haben. Den stärksten Einfluss hat die Keramikschichtstärke. Der Transmissionsgrad der Farben B und D unterscheidet sich nicht signifikant vom Transmissionsgrad der Farbe A. Dagegen weist die Keramik der Farbe Bleach einen signifikant höheren und die Keramik der Farbe C einen signifikant niedrigeren Transmissionsgrad auf. Der Absorptionsgrad der glasierten Oberfläche und der Absorptionsgrad bei Belichtung durch die Zwischenmedien ist im Vergleich zum Absorptionsgrad einer gesägten Oberfläche, bzw. bei Belichtung ohne Zwischenmedium signifikant niedriger. Dagegen ist der Unterschied des Absorptionsgrades der polierten Oberfläche zum Absorptionsgrad der gesägten Oberfläche bei Mittelung der Versuche ohne bzw. mit Zwischenmedien knapp nicht signifikant. N2 - This study examines the transmittance of visible blue light through lithium disilicate reinforced ceramics as a function of the layer thickness, color and translucency. Furthermore, the influence of the surface treatment and the intermediate medium between the light guide and the ceramic surface was investigated. For this purpose, flat samples with a layer thickness of 1.25 mm, 2.5 mm, 5 mm, 7.5 mm of the ceramic IPS e.max CAD were sawn with a saw microtome and then crystallized. The transmittance of the light from an LED light curing unit was measured using a fiber optic spectrophotometer. The thicker, more opaque and darker a ceramic sample, the lower its transmittance. The decrease in the degree of transmission could be described by the Lambert-Beer law. The influence of the reflection and / or scattering on the surface must be included as the degree of absorption α. The degree of absorption α depends on the surface treatment and the medium between the light guide and the ceramic surface. A multiple linear regression analysis shows that the layer thickness, translucency, brightness levels and the colors have a significant influence on the degree of transmission. The thickness of the ceramic layer has the greatest influence. The transmittance of colors B and D does not differ significantly from the transmittance of color A. In contrast, the ceramics of the bleach color have a significantly higher and the ceramic of the color C a significantly lower transmittance. The degree of absorption of the glazed surface and the degree of absorption when exposed to light through the intermediate media is significantly lower compared to the degree of absorption of a sawn surface or when exposed to light without an intermediate medium. In contrast, the difference between the degree of absorption of the polished surface and the degree of absorption of the sawn surface is barely significant when averaging the tests with or without intermediate media. Material and Methods We identified three relevant PICO questions related to soft tissue augmentation procedures and conducted a systematic search of four major electronic databases for clinical studies in systemically healthy patients receiving at least one dental implant and a minimum follow-up of one year after implant placement. The primary outcome was mean difference in marginal bone levels, and secondary outcomes were clinical and patient-related outcomes such as thickness of peri-implant mucosa, bleeding indices, and Pink Esthetic Score. Results We identified 20 publications reporting on 16 relevant comparisons. Studies varied considerably and thus only two meta-analyses could be performed. This systematic review showed that: Soft tissue augmentation either for augmentation of keratinized mucosa or soft tissue volume inconsistently had an effect on marginal bone level changes when compared to no soft tissue augmentation, but consistently improved secondary outcomes. The combination soft and hard tissue augmentation showed no statistically significant difference in terms of marginal bone level changes when compared to hard tissue augmentation alone, but resulted in less marginal soft tissue recession as shown by a meta-analysis. Soft or hard tissue augmentation performed as contour augmentations resulted in comparable marginal bone level changes. Conclusions Peri-implant soft and hard tissues seem to have a bidirectional relationship: “Bone stands hard, but soft tissue is the guard”. A popular combination is Deproteinized Bovine Bone Mineral (DBBM) placed with a native collagen membrane (NCM) tacked to native bone. In this study, we compared the outcome of this treatment option to those achieved with three different graft/membrane combinations with respect to total newly occupied area and the mineralized compound inside. After bi-lateral extraction of two mandibular premolars in five adult beagles L-shaped alveolar defects were created. A total of 20 defects healed for 6 weeks resulting in chronic type bone defects. At baseline, four options were randomly allocated to five defects each: a. DBBM + NCM with a four-pin fixation across the ridge; b. DBBM + RCLC (ribose cross-linked collagen membrane); c. DBBM + NPPM (native porcine pericardium membrane); and d. Ca-sulfate (CS) + RCLC membrane. Membranes in b/c/d were not fixed; complete tensionless wound closure was achieved by CAF. Termination after 3 months and sampling followed, and non-decalcified processing and toluidine blue staining were applied. Microscopic images obtained at standardized magnification were histomorphometrically assessed by ImageJ software (NIH). An ANOVA post hoc test was applied; histomorphometric data are presented in this paper as medians and interquartile ranges (IRs). All sites healed uneventfully, all sites were sampled and block separation followed before Technovit embedding. Two central sections per block for each group were included. Two of five specimen were lost due to processing error and were excluded from group b. New bone area was significantly greater for option b. compared to a. (p = 0.001), c. (p = 0.002), and d. (p = 0.046). Residual non-bone graft area was significantly less for option d. compared to a. (p = 0.026) or c. (p = 0.021). We conclude that collagen membranes with a prolonged resorption/barrier profile combined with bone substitutes featuring different degradation profiles sufficiently support new bone formation. Tacking strategy/membrane fixation appears redundant when using these biomaterials.
