  • In der vorliegenden Arbeit wurden die Daten von 250 Patienten, die zwischen 1993 und 2004 an der Glandula parotidea operiert worden waren, retrospektiv ausgewertet. Der Nachbeobachtungszeitraum musste mindestens 6 Monate betragen. Bei 146 Patienten wurde im Anschluss an eine Parotidektomie frei transplantiertes, abdominelles Fettgewebe in den Operationssitus eingebracht. Bei weiteren 104 Patienten wurde auf eine solche Augmentation verzichtet. Vereinzelt konnten sehr gute kosmetische Ergebnisse erzielt werden. Insgesamt zeigte sich jedoch kein signifikanter Unterschied in der kosmetischen Zufriedenheit zwischen den beiden Gruppen. Ein Frey-Syndrom trat mit 23% in der Gruppe der augmentierten Patienten signifikant seltener auf als in der nicht-augmentierten Gruppe (31%). Demgegenüber hatten die Patienten der augmentierten Gruppe signifikant häufiger eine Wundheilungsstörung (39%) als jene Patienten, bei welchen auf eine Augmentation verzichtet worden war (18%).   • Parotidectomy has aesthetic and functional sequelae, including Freys Syndrome and soft tissue depression of the auricular region. The purpose of this study was to evaluate the impact of using free subumbilical harvested fatty tissue. Between July 1993 and March 2004 a group of 250 patients undergoing parotidectomy met the study criteria having responded to the questionnaire and were included in this study. The minimum follow-up period was 6 months. Immediate augmentation was performed with a free subumbilical fatty graft in 146 cases. TheParotidectomy has aesthetic and functional sequelae, including Freys Syndrome and soft tissue depression of the auricular region. The purpose of this study was to evaluate the impact of using free subumbilical harvested fatty tissue. Between July 1993 and March 2004 a group of 250 patients undergoing parotidectomy met the study criteria having responded to the questionnaire and were included in this study. The results were compared with the outcomes of 104 patients who had been treated without reconstruction of the parotid bed. In selected cases, aesthetic results were very satisfactory, but no significant improvement was observed with using augmentation with fatty tissue. The majority of both groups were satisfied with the cosmetic outcomes. Frey Syndrome could be reduced significantly by placing a fatty graft into the defect. Patients who underwent conventional parotidectomy (without augmentation) showed increased gustatory sweating (23% to 31%). However, free fatty reconstruction following parotidectomy is associated with increased wound healing, 39% with fatty augmentation, whereas 18% of the control group developed wound healing disorder.show moreshow less
