Zudem hat die Immunogenität des p33ING1b Proteins in Patientinnen mit einem Mammakarzinom ihre potentielle Bedeutung für die Diagnose und eine auf dem Prinzip der Vakzinierung basierende Behandlung demonstriert. Häufig wird das Nierenzellkarzinom von einer großen Anzahl mononukleärer Zellen einschließlich T-Lymphozyten, NK-Zellen und Makrophagen infiltriert. Dabei wurde für frühe Tumorstadien in der vorliegenden Arbeit eine erhöhte Serumkonzentration an IL-2 und dessen Rezeptor (IL-2R) gezeigt, die auf eine erhöhte T-Zellproliferation und Aktivierung der Immunantwort im Tumorpatienten hindeutet. Eine verstärkte Expression von Zytokinen wie IFN-γ und TNF-α neben IL-2 im Serum weist, wie nachgewiesen, auf eine verstärkte Aktivierung von Th1 und zytotoxischen Tc1 Zellen im Tumorpatienten hin. Im Tumorgewebe selber akkumulieren T-Zellen in den Anfangsstadien des Nierenzellkarzinoms. Dies lässt eine lokale Tumorimmunantwort vermuten. Im peripheren Blut von Patienten später Stadien weisen die Ergebnisse dieser Arbeit jedoch auf eine stadienabhängige Abschwächung der gegen den Tumor gerichteten Th1-Antwort und eine Stärkung der Th2/Treg-Antwort mit supprimierender Auswirkung auf die Tumor-Immunabwehr hin. In fortgeschrittenen Tumorstadien bedeutet dies eine zunehmend inaktive Tumorimmunantwort. Intratumoral überwogen sowohl IL-10 positive CD4+ und CD8+ als auch IL-2 positive CD8+ T-Zellen, von denen immunsuppressive Effekte auf die Tumorimmunantwort vermutet werden. Möglicherweise entsteht durch eine Verschiebung im Zellverhältnis von zytotoxischen und T-Helferzellen gegenüber supprimierenden und regulatorischen Zellen ein Selektionsvorteil für das weitere Tumorwachstum. So wäre eine zytotoxische Immunantwort gegen den Tumor zunehmend ineffektiv. Immunologische Therapieansätze stellen wegen der Möglichkeit eines selektiven Angriffs auf Tumorzellen ein attraktives Konzept dar. Diese Arbeit zeigt, dass etwa 40% der untersuchten Patienten früher wie auch später Stadien eine signifikante Überexpression des ING1b Gens und über 30% eine signifikante Überexpression des ING4 Gens aufwiesen. Eine zytotoxische CD8+ sowie CD4+ T-Zell-Tumorimmunantwort gegen diese im Tumor selektiv überexprimierten Proteine könnte sich somit als ein attraktives Therapieziel erweisen. In der in begrenztem Umfang, da nicht thematischer Schwerpunkt der Arbeit, durchgeführten HLA-Analyse zeigte sich für die untersuchten HLA-A, -B und -C sowie HLA-DRA und -DRB Antigene im untersuchten Patientenkollektiv eine heterogene Genexpression. Der für verschiedenen Tumoren beschriebene Verlust von HLA Klasse I Molekülen auf der Tumorzelloberfläche stellt offensichtlich keine Allgemeingültigkeit dar. Beim Nierenzellkarzinom ist diese Beobachtung nach der eigenen Datenlage nicht unbedingt mit einem oft diskutierten Escape-Mechanismus gleichzusetzen. Dagegen zeigten sämtliche Patienten früher Stadien und 75% fortgeschrittener Tumoren eine signifikant herabregulierte HLA-G Expression auf. Die Bedeutung des HLA-G Moleküls für das Nierenzellkarzinom bleibt unklar, für andere Tumorentitäten wird es derzeit kontrovers diskutiert. Dessen Überexpression könnte sich als ein Selektionsvorteil für den Tumor herausstellen. Die Tatsache, dass sowohl im Tumor als auch im peripheren Blut p29ING4-positive T-Zellen nachweisbar sind, und dass diese Zellen in fortgeschrittenen Tumorstadien abnehmen, wurde in dieser Arbeit eindrücklich gezeigt. Dabei war der Anteil p29ING4-positiver CD4+ T-Zellen in frühen Tumoren größer als der spezifischer CD8+ T-Zellen. Dabei könnte es sich vermutlich um CD4+ T-Helferzellen und CD8+ zytotoxische T-Zellen handeln. Die Sequenz p29ING4 aa 149-158 bewirkte eine IL-2-Antwort and war für eine verstärkte Expression des IFN-γ in PBLs von Patienten früher Stadien bedeutsam. Sie löste dazu nur eine geringe IL-10-Antwort aus. Dies deutet auf eine stimulierende Rolle dieses p29ING4 Epitops für die Induktion einer zytotoxischen Tc1- aber auch einer Th1-vermittelten T-Zellantwort in den PBLs der Tumorpatienten hin. Schlussfolgernd wäre die therapeutische Effizienz des identifizierten p29ING4 Epitops für eine Immuntherapie klinisch zu überprüfen. Möglicherweise stellt eine auf p29ING4-basierte Immuntherapie in Kombination mit CTLs, die durch eine begleitende Zytokintherapie (z.B. INF-α und IL-2) aktiviert werden, bei Patienten mit p29ING4-überexprimierenden Nierentumoren ein wirksames immunologisches Therapiekonzept für die Zukunft dar.show moreshow less

ING1b and ING4 genes has been of interest in cancer research after the demonstration of high expression levels in various tumors. In addition, p33ING1b immunogenicity in patients with breast cancer has raised the potential value of p33ING1b in diagnosis and vaccine based treatment. Recent data show that the RCC is often infiltrated by a variable amount of cellular infiltrates composed of T cells, NK cells and macrophages. An increased expression of cytokines like IL-2 and its receptor IL-2R in the serum of the patients as demonstrated in this thesis points to an enhanced level of immunological activation during early stages of the tumor development. A significant increase in the expression of further cytokines like IFN-γ and TNF-α together with IL-2 may support this hypothesis of early activation of T cells like T helper 1 (Th1) and cytotoxic Tc1 cells at early stages of the tumor development. As demonstrated in this thesis an increasing amount of tumor infiltrating T cells in early stages show significant in vitro reactivity against tumor antigen indicating a peripheral tumor immune response. However, conclusive data also show that in advanced tumor stages the type-1 response, indicative for tumor destruction, decreased whereas the type-2 response, indicative for regulatory or suppressive immune responses, increased. This suggests an increasingly ineffective tumor immune response during later stages of the disease. Indeed, the data presented in this thesis show a further diminished tumor immune response in RCC patients in advanced stages. CD4+ and CD8+ tumor-infiltrating cells, double positive for IL-10, as well as IL-2 expressing CD8+ T cells increased in later stages. This may be indicative for a shift from infiltrating cytotoxic and T helper cells in early stages to an increasing population of suppressive CD8+ T cells infiltrating the tumor in later stages. The change in the proportion of cytotoxic and helper T cells towards suppressive and/or regulatory T cells in the tumor may be responsible for the polarization from a Th1- to a Th2 type response. This may contribute to tumor growth and thus resemble an increasingly ineffective cytotoxic tumor immune response. The data presented in this thesis demonstrate for the first time heterogenic expression of the tumor suppressor genes ING1b and ING4 in patients at different stages of the RCC. Approximately 40% of renal cancers showed significant overexpression of the ING1b gene and more than 30% overexpressed the ING4 gene. Thus, a CD4+ and CD8+ T cell mediated tumor immune response against defined proteins may be an attractive therapeutical strategy. To characterize a common pattern of HLA class I and II gene expression for epidemiological reason it requires a significant population of tumor patients to compare with healthy subjects. The limited patient study group, which was investigated, demonstrated heterogenic HLA-A, -B, -C, -DRA and -DRB gene expression. The loss of HLA class I molecule expression on the surface of the tumor cell does not exhibit a common abnormality for the RCC and does not display an obligatory tumor escape mechanism. In addition, all patients in early stages and 75% of patients in advanced stages showed significant overexpression of the HLA-G gene. The specific role of the HLA-G molecule in the RCC as controversially also discussed for other malignancies, seems to be unknown and could turn out to be beneficial for tumor growth. This thesis demonstrates for the first time significant numbers of p29ING1b specific T cells in tumor-infiltrating lymphocytes and in PBLs. However, TILs show a decreased ratio of p29ING1b specific T cells in later stages of the RCC. Interestingly, in early stages there were more p29ING4 positive CD4+ lymphocytes than such CD8+ T cells within the tumor tissue. Moreover the p29ING4 sequence aa 149-158 was shown to induce a significant IFN-γ response but only a low IL-10 response in PBLs from RCC patients in early stages but not from healthy controls. The sequence p29ING4 aa 149-158 exclusively induced a significant IL-2 response. These data indicate a stimulating role of the sequence p29ING4 aa 149-158 for inducing a cytotoxic Tc1 and Th1 T cell mediated immune response in PBLs from RCC patients. In conclusion, the therapeutic efficiency of the epitope p29ING4 aa 149-158 should be further examined in in vivo studies. The results of this study may provide the basis for a combined immunotherapeutical strategy in particular with actually clinical relevant protocols with lymphocyte - (CTL) activating cytokines such as IFN-α and IL-2 in patients with upregulated ING4 expression within their tumor.show moreshow less
