Gegenstand aktueller Forschung ist die Aufklärung der Signaltransduktionskette der Anästhetika-induzierten (APC) und ischämischen Präkonditionierung (IPC). Bisherige Studien konnten die Beteiligung verschiedenster Mediatoren und Effektoren wie Proteinkinase C (PKC), Adenosintriphosphat-regulierte Kaliumkanäle, freie Sauerstoffradikale (ROS) und Stickstoffmonoxid (NO) am Signaltransduktionsweg der APC und IPC nachweisen. Eine Beteiligung des beta-1-adrenergen Signaltransduktionsweges ist noch weitgehend ungeklärt und wird in der Literatur widersprüchlich diskutiert. Die vorliegende Arbeit untersuchte im In-vivo-Herzinfarktmodell des Kaninchens die Beteiligung des beta-1-adrenergen Signaltransduktionsweges an der APC und IPC. Durch die 30-minütige Gabe der volatilen Anästhetika Sevofluran und Desfluran wurde die APC hervorgerufen, durch eine einmalige 5-minütige Koronararterienokklusion die IPC. Um eine Beteiligung des beta-1-adrenergen Signaltransduktionsweges in der APC und IPC zu prüfen, wurde zum einen auf Rezeptorebene mit Esmolol, einem ultrakurzwirksamen beta-1-selektiven Betablocker, zum anderen intrazellulär mit H-89, einem selektiven PKA-Inhibitor, gearbeitet. APC und IPC wurden jeweils mit Esmolol oder H-89 kombiniert. Die Zielgröße der vorliegenden Arbeit war die prozentuale Berechnung des infarzierten Myokards am ischämischen Areal (IS/AAR in %). In Übereinstimmung mit vielen bisherigen Studien konnte eine myokardiale Präkonditionierung sowohl mit Desfluran und Sevofluran als auch durch eine kurze Koronararterienischämie hervorgerufen werden. Insgesamt war die IPC hinsichtlich der Infarktgrößenreduktion effektiver als die APC. Die APC wurde sowohl durch die beta-1-Adrenozeptor-Blockade als auch durch die PKA-Inhibition vollständig unterdrückt. Die IPC hingegen wurde zwar durch die beta-1-Adrenozeptor-Blockade komplett, durch die PKA-Inhibition jedoch nur teilweise aufgehoben. Unter Berücksichtigung der vorliegenden Ergebnisse kann somit festgehalten werden, dass der beta-1-adrenerge Signaltransduktionsweg eine wesentliche Rolle in der Vermittlung der APC und IPC spielt.   • Myocardial preconditioning is an endogenous protective mechanism, which increases the cellular resistance to prolonged ischemia and delays the appearance of myocardial infarction. This “program of protection” can be activated by different stimuli like short coronary artery occlusions (termed ischemic preconditioning, IPC) or by the application of volatile anesthetics (termed anesthetic-induced preconditioning, APC). Focus of current research is the examination of signal transduction mechanisms of IPC and APC. Former studies showed an involvement of different mediators and effectors in IPC and APC like protein kinase C (PKC), adenosine-triphosphate–regulated potassium channels, reactive oxygen species (ROS) and nitric oxide (NO). The role of the beta-1-adrenergic receptor in the signaling cascade of preconditioning is widely unclear in current literature. The present study examined the involvement of the beta-1-adrenergic signal transduction pathway in IPC and APC using an in-vivo-model of acute myocardial infarction in rabbits. The volatile anesthetics sevoflurane or desflurane given for 30 minutes were used to initiate APC, while IPC was evoked by administration of a single 5-min cycle of coronary artery occlusion. The involvement of the beta-1-adrenergic signal transduction pathway was tested by using two blocking agents in combination with APC and IPC. On the one hand esmolol was used, which is known to be an ultrashort-acting and selective beta-1-adrenergic receptor-blocker. On the other hand H-89 was given to inhibit protein kinase A (PKA) selectively, a downstream target of the betabe1-adrenergic receptor. APC and IPC were combined with esmolol or H-89, respectively. Endpoint of the current study was the examination of myocardial infarct size (IS) expressed as a percentage of the area-at-risk (IS/AAR in %). In accordance with former studies it could be shown that desflurane, sevoflurane and a single 5-min cycle of coronary artery occlusion are able to initiate myocardial preconditioning by reducing myocardial IS. Thereby IPC was a more potent agent to reduce infarct size than APC. The latter was completely abolished by blocking the beta-1-adrenergic receptor and inhibiting PKA, whereas IPC was totally inhibited by blocking the beta-1-adrenergic receptor and only attenuated by inhibition of PKA. In conclusion, beta-1-adrenergic signaling cascade plays an important role in IPC and APC.
