  • Im Hinblick auf objektive und subjektive Langzeitergebnisse wurde prospektiv ein Kollektiv von 308 Patienten untersucht, die sich einer Antirefluxoperation unterzogen hatten. Anamnese, Lebensqualitätsindex, Endoskopie, Manometrie. pH-Metrie und Bilitec-Messung charakterisieren den individuellen Befund prä- und postoperativ, sowie ein Jahr postoperativ. Die gewählte Operationtechnik im Sinne des "Tailored Concept" richtet sich nach dem zugrunde liegenden Funktionsdefekt. Eine 360°-Nissen-Fundoplikatio bekamen 256 Patienten. 180°-TeilmanschettenIm Hinblick auf objektive und subjektive Langzeitergebnisse wurde prospektiv ein Kollektiv von 308 Patienten untersucht, die sich einer Antirefluxoperation unterzogen hatten. Eine 360°-Nissen-Fundoplikatio bekamen 256 Patienten. 180°-Teilmanschetten wurden bei 21, 270°-Manschetten bei 31 Patienten angelegt.Innerhalb des Kollektivs lag die Geschlechtsverteilung bei ca.75% Männer zu 25% Frauen. Der Altersschwerpunkt lag bei 50-60 Jahren. Präoperativ litten ca.70% an einer ösophagitis, 30% waren endoskopisch unauffällig, wärend Defekte des UÖS 70% des Gesamtkollektivs aufwiesen. Das Therapieregime der GERD ist nach Einführung der Protonenpumpeninhibitoren stärker medikamentös orientiert. Die Entscheidung zu einer operativen Therapie fällt bei progredientem Verlauf, nicht-säureassoziierten Symptomen und als Alternative bei Kontraindikationen zur medikamentösen Therapie. Bezüglich der Dysphagie weist der short-floppy-Nissen keine größeren Probleme als die Teilmanschetten auf, sodass das "Tailored Concept" verlassen wurde und standardisiert laparoskopisch diese Vollmanschette mit posteriorer Hiatoplastik angestrebt wird.show moreshow less

The majority of the patients were about 50-60 years old. Before surgery 70% of the patients suffered from anesophagitis, 30% were endoscpically inconspicious. 70% had a damaged Lower Esophageal Sphincter. The decision in favour of surgery is made in sptie of an increasing disease development, non- acid-associated and optionally if there where contraindications against medical treatment. At the sight of similar good results refer to dysphagia the partitial fundoplication and the tailored concept can be abandoned.
