  • Der endorektale Ultraschall (ERUS) ist eine bewährte Methode, um präoperativ Rektumtumoren zu klassifizieren. Viele Untersucher glauben, dass eine vorhergehende Tumorbiopsie einen negativen Einfluss auf die Staginggenauigkeit habe, meist im Sinne eines Overstaging. Jedoch gab es bisher in der aktuellen Literatur keine validen Daten um diese Hypothese zu stützen. Die Doktorarbeit handelt um eine prospektive Studie, die belegen soll, ob die Tumorbiopsie einen Einfluss auf das endosonographische Staging von Rektumtumoren hat. METHODE: InsgesamtDer endorektale Ultraschall (ERUS) ist eine bewährte Methode, um präoperativ Rektumtumoren zu klassifizieren. Viele Untersucher glauben, dass eine vorhergehende Tumorbiopsie einen negativen Einfluss auf die Staginggenauigkeit habe, meist im Sinne eines Overstaging. Jedoch gab es bisher in der aktuellen Literatur keine validen Daten um diese Hypothese zu stützen. Die Doktorarbeit handelt um eine prospektive Studie, die belegen soll, ob die Tumorbiopsie einen Einfluss auf das endosonographische Staging von Rektumtumoren hat. METHODE: Insgesamt 333 Rektumneoplasien wurden endosonographisch untersucht, dann operiert und histologisch aufgearbeitet. 21 Tumoren waren nicht biopsiert worden und dienten als sogenannte Goldstandardgruppe. Die übrigen Tumoren wurden in verschiedenen zeitlichen Abständen vor Endosonographie biopsiert. Der Chi-Quadrat-Test wurde zur statistischen Auswertung genutzt. ERGEBNISSE: Die Gesamtgenauigkeit des Tumorstagings betrug 71,2%. Bei der Analyse der Abhängigkeit des endosonographischen T-Stagings vom Zeitabstand in Wochen zum Biopsie-Tag findet man in der ersten Woche eine Genauigkeit von 78,5%. In der zweiten Woche beträgt die Genauigkeit 65,2%. Ein Minimum der Genauigkeit von 52,9% liegt in der dritten Woche mit einem Overstaging-Anteil von 37,3% vor. Im Vergleich zur Kontrollgruppe mit 84,9% sind die Unterschiede bei der Genauigkeit mit p=0,008 und bei dem Overstaging mit p=0,001 statistisch signifikant. SCHLUSSFOLGERUNG: Der ERUS ist eine zuverlässige und genaue Methode für das präoperative Staging von Rektumtumoren. Allerdings leidet die Genauigkeit des T-Stagings unter einer vorher durchgeführten Biopsie. Daher sollte idealerweise die Biopsie nach der Ultraschalluntersuchung durchgeführt werden, um bestmögliche Staging- Ergebnisse zu erzielen. Eine weitere Möglichkeit wäre ein frühes Staging innerhalb der ersten Woche nach Biopsie. In der zweiten und dritten Woche nach Tumorbiopsie sollte eine endosonographische Klassifikation vermieden werden.show moreshow less

However, no data validating this hypothesis are to be found in the current literature. This prospective study was carried out to establish whether obtaining a tumour biopsy prior to ERUS examination has an influence on the reliability of ERUS staging. METHODS: A total of 333 rectal tumoursBACKGROUND: Endorectal ultrasound (ERUS) is a well-established method for the preoperative staging of rectal tumours. Many investigators believe that pre-ERUS biopsy of the tumour has a negative impact on staging accuracy, mostly as a result of overstaging. This prospective study was carried out to establish whether obtaining a tumour biopsy prior to ERUS examination has an influence on the reliability of ERUS staging. METHODS: A total of 333 rectal tumours were examined preoperatively using ERUS. All patients underwent rectal resection, and the specimens were sent for histological assessment. Twenty-one tumours were not biopsied prior to ERUS and served as the gold standard group. The remaining tumours were biopsied at various times prior to ERUS. The Chi-square test was used for statistical analysis. RESULTS: The overall staging accuracy was 71%., but differed significantly (p=0.001) between the groups as a function of time elapsed since biopsy. The best results were seen in tumours that were not biopsied prior to ERUS (“gold standard group”), which were correctly staged in 85%. The least accurate staging (53%) was noted when ERUS was performed in the third week following biopsy, mostly as a result of overstaging. CONCLUSION: ERUS is a reliable method for the preoperative staging of rectal tumours, but prior biopsy significantly affects its accuracy. To achieve the most accurate staging results biopsy should be performed after ERUS. ERUS done in the first week following biopsy is the second best option, but should be avoided in the second or third week.show moreshow less
