  • Für den Bispektral Index (BIS) ist eine gute Korrelation zum Grad der Sedierung, hervorgerufen durch unterschiedliche Sedativa wie auch zum Teil in Verbindung mit Opioiden, nachgewiesen [16]. Ziel dieser prospektiven Studie war es, die Eignung des BIS für das Sedationsmonitoring auf der Intensivstation zu überprüfen, da hier nach wie vor mangels valider Überwachungssysteme hauptsächlich klinische Scoringverfahren zum Einsatz kommen. Nach Zustimmung der örtlichen Ethikkommission wurde bei 19 beatmeten, operativen Intensivpatienten ohne zentralneurologische Vorerkrankungen zweimal täglich bis zur Extubation der BIS Vs. 3.11 (Aspect Medical Systems, Natick, MA, USA) soweit der entsprechende Score der modifizierten Ramsay-Scale, Cook-Scale, Glasgow-Coma-Scale, Sedation-/Agitation-Scale und der Obeserver´s Assessment of Altertness Scale erhoben und miteinander korreliert. Erfasst wurden ferner alle Labor- und Kreislaufparameter wie auch die Medikation. Insgesamt wurden 247 Messungen durchgeführt. Nur bei 11 von 19 Patienten (58%) lässt sich eine positive Korrelation zwischen BIS und allen Scores zeigen (0.64 &#61603; &#61556; &#61603; 0,7, p < 0,0001). Verlässliche Hinweise zur Unterscheidung zwischen Patienten mit und ohne Korrelation zeigten sich nach 4–15 Messungen (Mittel 8,4). Bei Patienten mit Korrelation waren die Steigungen der jeweiligen Regressionsgeraden signifikant verschieden voneinander (p < 0,05).show moreshow less

The goal of this prospective study was to review the suitability of BIS for the monitoring of sedation in the intensive care unit, since here only mainly clinical scoring systems are being used due to missing valid monitoring systems. In line with the agreement of the local ethics committee 19 intubated, operated, intensive care patients without any central neurological disorder,For the bispectral index (BIS) a good correlation with the degree of sedation, which is caused by different sedatives as well as in connection with opoids, is proven. The goal of this prospective study was to review the suitability of BIS for the monitoring of sedation in the intensive care unit, since here only mainly clinical scoring systems are being used due to missing valid monitoring systems. In line with the agreement of the local ethics committee 19 intubated, operated, intensive care patients without any central neurological disorder, were surveyed twice a day until extubation of BIS Vs. 3.11 (Aspect Medical Systems, Natick, MA, USA) as far as the equivalent score of the modified Ramsay-Scale, Cook-Scale, Glasgow-Coma-Scale, Sedation-/Agitation-Scale and the Obeserver´s Assessment of Altertness Scale and correlated with each other. Furthermore all laboratory and cardiovascular system parameters as well as medication were recorded. In total 247 readings were performed. Only with 11 of the 19 patients (58%) a positive correlation between BIS and all scores could be shown (0.64 &#61603; &#61556; &#61603; 0,7, p < 0,0001). Reliable indicators to differentiate between patients with and without correlation were shown after 4 -15 readings (middle 8,4) . For patients with correlation the increase of the respective degree of regression was significantly different of each other (p < 0,05).show moreshow less
