  • Die vorliegende Studie untersuchte die Langzeitergebnisse nach operativer Therapie einer Osteochondrosis dissecans des Ellenbogengelenkes bei 28 Patienten. Die Therapie erfolgte in 15 Fällen arthrotomisch und in 13 Fällen arthroskopisch. Hauptfragestellung war die Darstellung von möglichen postoperativen funktionellen und subjektiv empfundenen Unterschieden innerhalb der arthrotomisch bzw. arthroskopisch behandelten Patientengruppen. Daneben sollte der Einfluß verschiedener Parameter auf das Ergebnis untersucht werden. DieDie vorliegende Studie untersuchte die Langzeitergebnisse nach operativer Therapie einer Osteochondrosis dissecans des Ellenbogengelenkes bei 28 Patienten. Die Therapie erfolgte in 15 Fällen arthrotomisch und in 13 Fällen arthroskopisch. Hauptfragestellung war die Darstellung von möglichen postoperativen funktionellen und subjektiv empfundenen Unterschieden innerhalb der arthrotomisch bzw. arthroskopisch behandelten Patientengruppen. Daneben sollte der Einfluß verschiedener Parameter auf das Ergebnis untersucht werden. Die Untersuchungsergebnisse basieren auf Auswertungen der Krankenakten, der Operationsberichte, der klinischen und röntgenologischen Nachuntersuchung sowie auf der Erhebung des Secec -Elbow - Scores und des DASH- Scores. Es wurden 6 Patienten konservativ vorbehandelt, ohne dass dies eine Heilung erzielte. Die Gruppen Arthrotomie vs. Arthroskopie unterschieden sich nicht im postoperativen Arthrosegrad und unwesentlich in den Punktzahlen des DASH- Scores und des Secec - Elbow - Scores. Die offen operierten Patienten erzielten innerhalb der Module des DASH-Scores sowie des Secec - Elbow - Scores einen nicht signifikant aber tendenziell besseren Punktwert. Es lagen präoperative Röntgenbilder von 28 Patienten vor. Hier zeigten 5 Ellenbogen Anzeichen arthrotischer Veränderung, 3 dieser Patienten waren postoperativ in ihrer Beweglichkeit eingeschränkt. Die Herdgröße korrelierte mit der postoperativen Schmerzempfindung, der Bewältigung des täglichen Lebens und dem Gesamtscore des Secec - Elbow - Scores. Der perioperativ erhobene Grad des Knorpelschadens nach Outerbridge stand in Zusammenhang mit den Ergebnissen des Moduls Secec adl (Bewältigung des täglichen Lebens). Die Dissekatentfernung erwies sich mit zunehmender Größe der Gelenkkörper als Steigerungsfaktor für den postoperativen Arthrosegrad, während die Refixation eines Dissekates zu einem besseren Punktwert des Secec Gesamtscores führte. Die Durchführung einer Abrasionschondroplastik führte zu höheren Werten bei der Bewertung des Schmerzempfindens zum Zeitpunkt der Nachuntersuchung. Die Ergebnisse legen nahe, dass funktionelle und subjektiv empfundene Beeinträchtigungen mit präoperativen degenerativen Veränderungen, dem Grad einer Knorpelschädigung, einem großen osteochondralen Herd und den Maßnahmen Gelenkkörperentfernung, Refixation und Abrasionschondroplastik in Verbindung stehen. Die Art des Eingriffes, ob arthrotomisch bzw. arthroskopisch, beeinflußt den Untersuchungen nach nur unwesentlich das Endergebnis.show moreshow less

Main question was the representation of possible postoperative functionally and subjectively felt differences within the two groups. The influence of different parameters should be examined for the result beside it. The results of examination are based on evaluations of the sick person files, the operation reports, the clinical and x-ray follow-up as well as on the rise of the Secec Elbow Score and the DASH - Score. 6 patients were pre-treated conservatively without achieving a healing . The groups arthrotomy and arthroskopy didn't differ in the postoperative stage of arthrosis and unimportantly in the scores of the DASH- Score and the Secec Elbow Score.The arthrotomicly operated patients achieved tendencially a better point value in the modules of the DASH-Scores as well as the Secec Elbow Score. Pre-operative x-rays of 28 patients were available. Here 5 elbows showed signs of arthrotis, 3 of these patients were restricted postoperatively in their mobility. The defect size correlated with the postoperative pain sensation, the daily living and the result of the Secec Elbow Score. The perioperative stage of chondral damage after Outerbridge stood in context with the module Secec adl (activities of daily living). The removal of loose bodies and their size proved to be an increasing factor for the postoperative stage of arthrosis while the refixation of a fragment led to a better point value of the Secec Elboe Score. The transaction of an abrasion chondroplasty led to higher values within the evaluation of the pain felt at the time of the follow-up. The results suggest that functionally and subjectively felt differences are basing on pre-operative degenerative changes, the degree of a cartilage damage, the defect size and the measures removal of loose bodies, refixation and abrasion chondroplasty.According to the examinations the type of the intervention, whether with arthrotomic as well as arthroskopic surgery influences the final result only unimportantly .show moreshow less
