In der vorliegenden Arbeit sollte die Frage beantwortet werden, ob sich die morphologische Variante der multilobulierten centroblastischen Lymphome (CB multilobuliert) von anderen in der Kiel-Klassifikation definierten morphologischen Untergruppen großzelliger B-Zell Lymphome hinsichtlich ihres Immunphänotyps und des klinischen Verlaufs unterscheiden. Insbesondere ergab sich die Frage nach einem möglichen Keimzentrumsursprung derDiffuse großzellige B-Zell Non-Hodgkin Lymphome (DLBCL) stellen eine heterogene Kategorie maligner Lymphome dar. In der vorliegenden Arbeit sollte die Frage beantwortet werden, ob sich die morphologische Variante der multilobulierten centroblastischen Lymphome (CB multilobuliert) von anderen in der Kiel-Klassifikation definierten morphologischen Untergruppen großzelliger B-Zell Lymphome hinsichtlich ihres Immunphänotyps und des klinischen Verlaufs unterscheiden. Insbesondere ergab sich die Frage nach einem möglichen Keimzentrumsursprung der Tumoren. Insgesamt wurden 72 Fälle aus dem Referenzzentrum für Lymphknotenpathologie untersucht. 46 Lymphome zeigten ein rein diffuses Wachstumsmuster und 26 CB multilobuliert wiesen ein partiell follikuläres Wachstumsmuster auf. Bemerkenswert war die Tatsache, dass häufig nur partielles Infiltrationsmuster vorlag, oft in Form eines subkapsulären Randsaums. Dieser Befund legt nahe, dass im Rahmen der Tumorausbreitung reaktive Follikel kolonisiert werden, die in der Weiterentwicklung des Tumors konfluieren. Die Auswertung der in der Technik der „Tissue Microarray“ -Stanzen (TMA) hergestellten Schnittpräparate zeigte, dass CB multilobuliert weitaus häufiger als andere Entitäten von DLBCL den (prognostisch günstigeren) Keimzentrums-ähnlichen (GC-like) Lymphomen zugeordnet werden können. Klinische Daten konnten von insgesamt 46 Patienten mit CB multilobuliert erhoben werden. Sie wurden mit einer Kontrollgruppe von 54 nodalen DLBCL, 14 FL 3B und 47 FL 1-3A korreliert. Die Analysen der Überlebensdaten der Patienten zeigten ein signifikant besseres Gesamtüberleben der Patienten mit CB multilobuliert im Vergleich zu nodalen DLBCL anderer Differenzierung. Bemerkenswert war auch das signifikant bessere Überleben der Patienten mit CB ohne Immunoblasten (CB multilobuliert/monomorph) vs. DLBCL mit Immunoblasten (CB polymorph und IB/PB). In der Zusammenschau der Daten legen diese Befunde nahe, dass CB multilobuliert eine dem Keimzentrum nahe stehende Differenzierungsstufe großzelliger B-Zell Lymphome repräsentieren. Die relativ gesehen bessere Prognose dieser Tumoren gegenüber insbesondere den centroblastischen Lymphomen vom polymorphen Subtyp ist offenbar in ihrer sehr häufigen Keimzentrums-ähnlichen Differenzierung begründet. 75% der in der vorliegenden Serie untersuchten Tumoren ließen ein „GCB-like“ Proteinexpressionsmuster nachweisen. Aus Genexpressionsuntersuchungen größerer Serien von DLBCL ist die „Keimzentrums-Signatur“ in der Regel mit einer besseren Prognose assoziiert.show moreshow less

The aim of this theses was to find out if the morphological variant of the multilobulated centroblastic lymphoma ( CB multilobulated) is different to other defined morphological subgroups of B-cell lymphoma in the Kiel-classification concerning the immuno phenotype and the clinical end. Some special interest focused on potential germ center origin of the tumor. We examined 72 cases from the reference center of lymph node pathology. 46 of thoseDiffuse large B-cell lymphomas (DLBCL) are a heterogeneous group of non- hodgkin lymphomas (NHL). The aim of this theses was to find out if the morphological variant of the multilobulated centroblastic lymphoma ( CB multilobulated) is different to other defined morphological subgroups of B-cell lymphoma in the Kiel-classification concerning the immuno phenotype and the clinical end. Some special interest focused on potential germ center origin of the tumor. We examined 72 cases from the reference center of lymph node pathology. 46 of those showed lymphoma with a strictly diffuse kind of growth whereas 26 CB multilobulated showed a partial folliculated kind of growth. We considered it as a remarkable fact that 37 cases showed only a partial infiltration, frequently with a subcapsular border. These findings suggest the colonisation of reactive follicels due to the spreading of the tumor and further on their confluency during tumor development. In the analysis of the “tissue microarray”-prepared slides could be shown, that CB multilobulated are much more often than other DLBCL entities associated to the germ center similar lymphomas including a better prognosis. Clinical data could be observed from 46 patient suffering from CB multilobulated. They were compared with a control group consisting of 54 nodal DLBCL, 14 FL 3B and 47 FL 1-3A. The analysis of patient survival showed a signifanct advantage for patients suffering from CB multilobulated compared with nodal DLBCL of other specification. We could also observe a remarkable better rate of survival for patients affected by CB without immunoblasts (CB multilobulated/monomorph) compared to patients with immunoblasts (CB polymorph and IB/PB). In the conclusion all these data suggest that the CB multilobulated represent a certain type of large cell B-cell lymphoma familiar to the germ center. The better clinical prognosis compared to the centroblastic lymphomas of polymorphic subspecification is obviously due to to the frequently shown germ cell similar appearance. 75 percent of the examinated tumors showed a “GCB-like” pattern of protein expression. However, large series concerning examinations of DLBCL gene expression patterns showed a better prognosis for the “germ center signature”.show moreshow less
