T1  - Indications and clinical results for flaps in the head and neck area based on the example of pectoralis major flaps and deltopectoral flaps
N2  - Im Zeitraum zwischen 1982 bis 2004 wurden an der Klinik und Poliklinik für Hals-, Nasen- und Ohrenkranke des Universitätsklinikums Würzburg 41 Patienten mit einem Pectoralis-Major-Lappen (PM) und/oder Deltopectorallappen (DP) versorgt. Insgesamt wurden 28 PM und 22 DP transplantiert, um einen Defekt nach ausgiebiger Tumorentfernung im Kopf-Hals-Bereich zu decken, eine persistierende Fistel zu verschließen oder eine Rekonstruktion von Teilen des Pharynx, des Hypopharynx, des Mundbodens oder der Zunge vorzunehmen. Die hier vorliegende Arbeit gibt Aussage über die präoperative und periopertive Behandlungen sowie den postoperativen Verlauf wie Komplikationsrate und Einheilungsrate. Es wurde zwei Kollektive gebildet. Das zu untersuchende Kollektiv A bestand aus 34 Patienten, die entweder nur einen DP (15 Patienten) oder nur einen PM (19 Patienten) erhalten hatten. Kollektiv B bestand aus 7 Patienten, die mit mehr als einem Lappen versorgt wurden, dabei traten der PM und der DP auch nebeneinander auf. Der PM und DP sind im Kopf-Hals-Bereich mit ihren hohen Einheilungsraten sehr gut geeignet, auch wenn die Anzahl der Komplikationsarten als auch die Komplikationsraten beim DP signifikant höher liegen als beim PM.
N2  - In the time period between 1982 to 2004 41 patients were provided with a pectoralis major (PM) flap or a deltopectoral (DP) flap by the Otolaryngology Department of the Würzburg University Hospital. A total of 28 PM and 22 DP were engrafted, either to occlude a head and neck cavity after radical tumor surgery, to occlude a fistula, or for reconstructing parts of the pharynx, the hypopharynx, the mouth base or the tongue. This thesis deals with the preoperative and perioperative treatment as well as the postoperative process like complication rate and flap healing. Two groups were formed: Group A consisted of 34 patients who either got only one DP flap (15 patients) or only one PM flap (19 Patients). Group B consisted of 7 patients, who were provided with more than one flap. In this case the PM and the DP can coexist. The PM and DP with their high flap healing rate are appropriate for reconstructions in the head and neck area, even though the diversity of complications and the complication rate are significantly higher in the case of the DP than in the case of the PM.
