Methods: A total of 123 arthroplasties were evaluated utilizing the Harris Hip Score (HHS), the extra short musculoskeletal functional assessment questionnaire (XSFMA), the Short Form 36 (SF-36) health survey, a Stepwatch™ Activity Monitor (SAM), and a timed 25 m foot walk (T25-FW). Postoperative x-ray images after THA were reviewed to determine inclination and stem positioning. Results: At final follow-up, the XSFMA functional index scores were 10.3 (anterior) and 15.08 (lateral) while the bother index summed up to a score of 15.8 (anterior) and 21.66 (lateral) respectively, thus only differing significantly for the functional index (p = 0.040 and p = 0.056). The SF-36 physical component score (PCS) was 47.49 (anterior) and 42.91 (lateral) while the mental component score (MCS) summed up to 55.0 (anterior) and 56.23 (lateral) with a significant difference evident for the PCS (p = 0.017; p = 0.714). Patients undergoing THA through a DAA undertook a mean of 6402 cycles per day while those who had undergone THA through a transgluteal approach undertook a mean of 5340 cycles per day (p = 0.012). Furthermore, the obtained outcome for the T25-FW with 18.4 s (anterior) and 19.75 s (lateral) and the maximum walking distance (5932 m and 5125 m) differed significantly (p = 0.046 and p = 0.045). The average HHS showed no significant difference equaling 92.4 points in the anterior group and 91.43 in the lateral group (p = 0.477). The radiographic analysis revealed an average cup inclination of 38.6° (anterior) and 40.28° (lateral) without signs of migration. Conclusion: In summary, our outcomes show that after 1 year THA through the direct anterior approach results in a higher patient activity compared to THA utilizing a transgluteal lateral approach while no differences regarding hip function are evident. Ecological momentary assessment (EMA) driven smartphone apps can help alleviate such issues, allowing for in situ recordings. Implementing such an app is not trivial, necessitates strict regulatory and legal requirements, and requires short development cycles to appropriately react to abrupt changes in the pandemic. Based on an existing app framework, we developed Corona Health, an app that serves as a platform for deploying questionnaire-based studies in combination with recordings of mobile sensors. In this paper, we present the technical details of Corona Health and provide first insights into the collected data. Through collaborative efforts from experts from public health, medicine, psychology, and computer science, we released Corona Health publicly on Google Play and the Apple App Store (in July 2020) in eight languages and attracted 7290 installations so far. Currently, five studies related to physical and mental well-being are deployed and 17,241 questionnaires have been filled out. Corona Health proves to be a viable tool for conducting research related to the COVID-19 pandemic and can serve as a blueprint for future EMA-based studies. The data we collected will substantially improve our knowledge on mental and physical health states, traits and trajectories as well as its risk and protective factors over the course of the COVID-19 pandemic and its diverse prevention measures. The evolving field of regenerative medicine offers promising treatment strategies using cells, biomaterial scaffolds, and bioactive factors, which might improve clinical outcome. Early stages of AVN with preserved structural integrity of the subchondral plate are accessible to retrograde surgical procedures, such as core decompression to reduce the intraosseous pressure and to induce bone remodeling. The additive application of concentrated bone marrow aspirates, ex vivo expanded mesenchymal stem cells, and osteogenic or angiogenic growth factors (or both) holds great potential to improve bone regeneration. In contrast, advanced stages of AVN with collapsed subchondral bone require an osteochondral reconstruction to preserve the physiological joint function. Analogously to strategies for osteochondral reconstruction in the knee, anterograde surgical techniques, such as osteochondral transplantation (mosaicplasty), matrix-based autologous chondrocyte implantation, or the use of acellular scaffolds alone, might preserve joint function and reduce the need for hip replacement. This review summarizes recent experimental accomplishments and initial clinical findings in the field of regenerative medicine which apply cells, growth factors, and matrices to address the clinical problem of AVN. Methods In the present study, we induced the overexpression of the growth factors bone morphogenetic protein 2 (BMP-2) and/or Indian hedgehog (IHH) in human MSCs by adenoviral transduction to increase their osteogenic potential. GFP and nontransduced MSCs served as controls. The influence of the respective genetic modification on cell metabolic activity, proliferation, alkaline phosphatase (ALP) activity, mineralization in cell culture, and osteogenic marker gene expression was investigated. Results Transduction had no negative influence on cell metabolic activity or proliferation. ALP activity showed a typical rise-and-fall pattern with a maximal activity at day 14 and 21 after osteogenic induction. Enzyme activity was significantly higher in groups cultured with osteogenic media. The overexpression of BMP-2 and especially IHH + BMP-2 resulted in a significantly higher mineralization after 28 days. This was in line with obtained quantitative reverse transcriptase polymerase chain reaction (qRT-PCR) analyses, which showed a significant increase in osteopontin and osteocalcin expression for osteogenically induced BMP-2 and IHH + BMP-2 transduced cells when compared with the other groups. Moreover, an increase in runx2 expression was observed in all osteogenic groups toward day 21. It was again more pronounced for BMP-2 and IHH + BMP-2 transduced cells cultured in osteogenic media. Conclusions In summary, viral transduction did not negatively influence cell metabolic activity and proliferation. The overexpression of BMP-2 in combination with or without IHH resulted in an increased deposition of mineralized extracellular matrix, and expression of osteogenic marker genes. Viral transduction therefore represents a promising means to increase the osteogenic potential of MSCs and the combination of different transgenes may result in synergistic effects. Sie schränken nicht nur die Gesundheit und Lebensqualität des Betroffenen ein sondern bringen bei steigender Inzidenz und kostenintensiver Behandlung auch eine gewaltige sozioökonomische Problematik mit sich. Die bisher zur Verfügung stehenden, therapeutischen Mittel wie die Entnahme von autologer Spongiosa aus dem Beckenkamm bergen das Problem der Morbidität an der Entnahmestelle, von persistierender Schmerzsyndromen, von Hypersensitivität, Instabilität des Beckens und Infektionen. Zudem ist die Menge an Knochen, die gewonnen werden kann, limitiert. Allografts sind von einer bedeutend niedrigeren Zellularität, besitzen eine geringere Revaskularisierungsrate sowie eine höhere Resorptionsrate, führen zu einer niedrigeren Knochenformationsrate und gehen mit der Gefahr einer Abstoßungsreaktion einher. Zukünftig könnte das Tissue Engineering als interdisziplinäres Forschungskonzept hier eine entscheidende Rolle spielen. Insbesondere MSZ wird ein großes therapeutisches Potential für die Rekonstruktion von Knochengewebe zugeschrieben. Die vorliegende Studie beschäftigte sich einerseits mit der Frage, ob unter dem Einfluss entsprechender Wachstumsfaktoren humane MSZ aus dem Knochenmark in Kollagen I Hydrogelen zu einer osteogenen Differenzierung und der Produktion mineralisierter, extrazellulärer Matrix angeregt werden können. Zum andern wurde untersucht, ob es möglich ist, ein Konstrukt aus MSZ, einem Kollagen Gel und einem geeigneten Scaffold herzustellen, das sich zur Rekonstruktion segmentaler Knochendefekte eignet. Zunächst wurden MSZ aus dem Knochenmark isoliert und in Monolayerkulturen osteogen differenziert. Die histochemischen Untersuchungen zeigten, dass in osteogenem Differenzierungsmedium kultivierte MSZ in der primären Zellkultur vermehrt mineralisierte Matrix bildeten und ALP exprimierten. In den RT-PCR Analysen konnte eine deutliche Mehrexpression später osteogener Markergene wie Osteokalzin nachgewiesen werden. MSZ, die leicht zu isolieren und zu 53 kultivieren sind, eignen sich demnach gut als Zellen zur Herstellung eines Konstruktes für die Rekonstruktion von segmentalen Knochendefekten. Eingebracht in Kollagen I Hydrogele zeigten die Zellen unter dem Einfluss verschiedener osteogener Differenzierungsbedingungen unterschiedliche Genexpressionsmuster. Nach 42 Tagen Kultur in SZM (Kontrollgruppe) konnte sowohl die Expression osteogener Markergene als auch eine chondrogene Differenzierung nachgewiesen werden. Es konnte eine deutliche Mehrexpression von AGN, Col II und SOX-9, gleichfalls der osteogenen Marker ALP und Cbfa1 gezeigt werden. Ein entsprechendes Bild hatte auch die histologische Aufarbeitung ergeben. Dies könnte auf die Eigenschaften des Kollagen Hydrogels zurückzuführen sein, dem aufgrund seiner Zusammensetzung und biologisch-induktiven Merkmale eine chondrogene Induktion, sogar ohne Wachstumsfaktoren, zugeschrieben wird. In der mit BMP- 2 differenzierten Gruppe zeigte sich eine deutliche Zunahme der Expression der chondrogenen Markergene, wie Col II und SOX-9. Auch die osteogenen Marker wie ALP, Cbfa1 und OC waren etwas stärker exprimiert. Den Ergebnissen nach zu schließen begünstigte BMP-2 bei der Kultivierung von MSZ in Kollagen I Hydrogelen in vitro eine eher chondrogene Differenzierung. Zumindest in Kombination mit Kollagen I Hydrogelen scheint daher BMP-2 als osteogener Wachstumsfaktor bei der Herstellung stammzellbasierter Konstrukte für den Knochenersatz weniger geeignet. Nach Kultur in osteogenem Medium konnte gegenüber der SZM Gruppe eine deutliche Mehrexpression aller getesteten, osteogenen Marker, insbesondere auch von Cbfa1 und OC, nachgewiesen werden. Allerdings zeigte sich auch bei den chondrogenen Markergenen wie Col II eine geringe Zunahme der Genexpression. Das osteogene Medium induzierte demnach MSZ in Kollagen I Gelen vorwiegend eine osteogene Differenzierung. Entsprechend konnte in den histologischen Untersuchungen die Bildung einer mineralisierten, extrazellulären Matrix nachgewiesen werden. Weiterhin konnte im Rahmen dieser Arbeit in vitro aus einem Kollagen I Gel, MSZ und einem PCL-Scaffold ein Konstrukt hergestellt werden, das die Regeneration segmentaler Knochendefekte positiv beeinflussen könnte. Es zeigte sich ein gutes Bonding an der Grenzfläche zwischen Kollagen Gel und 54 PCL-Scaffold. Das Kollagen Gel hatte die makroporösen und mikroporösen Freiräume des Scaffolds komplett ausgefüllt, wodurch eine homogene Verteilung der MSZ innerhalb des Scaffolds erreicht werden konnte. The mobile application approach of QuestionSys mainly pursues the goal to mitigate existing drawbacks of paper-based collection procedures in mHealth scenarios. Importantly, researchers shall be enabled to gather data in an efficient way. To evaluate the applicability of QuestionSys, several studies have been carried out to measure the efforts when using the framework in practice. In this work, the results of a study that investigated psychological insights on the required mental effort to configure the mobile applications are presented. Specifically, the mental effort for creating data collection instruments is validated in a study with N=80 participants across two sessions. Thereby, participants were categorized into novices and experts based on prior knowledge on process modeling, which is a fundamental pillar of the developed approach. Each participant modeled 10 instruments during the course of the study, while concurrently several performance measures are assessed (e.g., time needed or errors). The results of these measures are then compared to the self-reported mental effort with respect to the tasks that had to be modeled. On one hand, the obtained results reveal a strong correlation between mental effort and performance measures. On the other, the self-reported mental effort decreased significantly over the course of the study, and therefore had a positive impact on measured performance metrics. Altogether, this study indicates that novices with no prior knowledge gain enough experience over the short amount of time to successfully model data collection instruments on their own. Therefore, QuestionSys is a helpful instrument to properly deal with large-scale data collection scenarios like clinical trials. When following a fine-grained monitoring of these machines, challenges emerge pertaining to the (1) feeding procedure of large amounts of sensor data to downstream processing components and the (2) meaningful analysis of the produced data. Regarding the latter aspect, manifold purposes are addressed by practitioners and researchers. Two analyses of real-world datasets that were generated in production settings are discussed in this paper. More specifically, the analyses had the goals (1) to detect sensor data anomalies for further analyses of a pharma packaging scenario and (2) to predict unfavorable temperature values of a 3D printing machine environment. Based on the results of the analyses, it will be shown that a proper management of machines and their components in industrial manufacturing environments can be efficiently supported by the detection of anomalies. The latter shall help to support the technical evangelists of the production companies more properly.
