T1 - Nutritional Psychiatry: Inaugural Meeting in Tokyo N2 - Welcome to the inaugural meeting of the International Society for Nutritional Psychiatry Research (ISNPR). It is a great pleasure to have the opportunity to join with colleagues working in this new and exciting field of research. Although there has long been interest in the links between nutritional deficiencies and psychiatric illness, as well as interest in the role of food allergies in such illnesses, the last five years has seen a significant and notable growth in this nascent field of research, with an accompanying impact on the viewpoints and practices of scientists and clinicians working in mental health. In my particular field of interest – that of the role of overall dietary quality in the common mental disorders, depression and anxiety - there has been an exponential growth in the literature since the end of 2009. It is exciting and gratifying to see concordant results from across the globe, in young children and adolescents through to older adults, and from countries as diverse as Norway and Taiwan. The study of the efficacy of nutritional interventions in psychiatric illness is also developing rapidly, with high quality randomised controlled trials now being conducted in multiple settings and with outcomes that include cognition as well as depression, bipolar disorder, schizophrenia and anxiety disorders. Another important development in this field is the rapidly growing recognition that nutrition is of central importance in the risk for cognitive decline and dementia. As this new recognition filters through to clinical researchers, I look forward to seeing new interventions in this area. Another area of research with significant interest and activity grows from the understanding of the centrality of physical health to mental health and vice versa. There are many nutrition researchers, dietitians and other health practitioners working to address the physical health of patients with mental illness; acting on the recognition that physical and mental health are closely related and mutually reinforcing. There is no doubt that the formation of an international society is timely; we now have the opportunity to join forces to share knowledge and build important collaborations. Building capacity in this field by sharing our knowledge with students and early career researchers will be another important activity of our society, as will building the credibility of nutritional psychiatry research through a clear understanding and implementation of best practice scientific methodology. I welcome each of you to extend the invitation to join our new ISNPR to colleagues and students in your networks. I would also encourage you to contribute to the discussions and sharing of knowledge by contributing short pieces to our newsletter, which will be disseminated by the end of this year. For those who are unable to attend this year’s meeting, we hope that 2014 may present a possibility for attendance. Our aim is conduct our first Annual General Meeting before the end of July 2013 via teleconference and I welcome agenda items from those interested. However, data on the effects of ketosis in the latter group of individuals are scarce. We therefore collected pilot data on the physiological response to a self-prescribed ketogenic diet lasting 5-7 weeks in a small cohort of healthy and physically active individuals. Methods: Twelve subjects (7 males, 5 females, age 24-60 years) who followed moderate to intensive exercise routines underwent blood testing, bioelectrical impedance analysis (BIA) and spiroergometry during an incremental treadmill test. On the next day, they went on a self-prescribed KD for a median of 38 days (range 35-50 days), after which the same tests were performed again. Ketosis was self-monitored by urinary ketone strips. Subjective feeling during the diet was assessed by a questionnaire after the intervention. Due to the small and heterogenous sample, the results are interpreted in the context of the already existing literature. Results: The KDs were tolerated well by the majority of individuals. Impaired recovery from exercise remained the most frequently reported side effect until the end of the study. Most blood parameters remained stable during the intervention. However, there were significant elevations of total and LDL cholesterol concentrations (p<0.01) and a trend towards increased HDL-cholesterol (p=0.05). The drastic reduction of carbohydrates had no statistically significant influence on running performance judged by the time to exhaustion, VO2max and respiratory compensation points. BIA measurements showed significant increases in phase angle (p=0.01) indicating improvements of body composition with an estimated decrease of 3.4 kg of fat mass (p=0.002) and gain of 1.3 kg of fat free mass. We discuss the validity of these estimates taking into account a possibly altered hydration status due to the KD. Conclusions: Active healthy individuals will probably experience no major problems during a short term KD lasting several weeks. The drastically reduced carbohydrate content of the diet seems to be no limiting factor for running performance. In addition, improvements in body composition can be expected. While most biochemical parameters are not influenced by the diet, there seems to be an impact on the blood lipid profile that could be considered problematic with respect to cardiovascular disease risk. However, the predictive role of cholesterol levels alone in individuals undergoing regular physical activity remains to be elucidated. The present review focuses on nutrition-derived bioactive metabolites produced by gut microbiota with potential beneficial effects upon human health. Thereby, the emphasis is on newly generated bacterial metabolites that are not concomitantly present at higher amounts in dietary sources and that have been previously detected in human blood samples. Since a multitude of different substances is generated by gut microbes primarily those metabolites which exert a more pronounced activity than their immediate precursor compound are discussed here. Specifically, the in vitro and in vivo nutridynamics as well as the nutrikinetics of equol, enterolactone / enterodiol, urolithins, 8-prenylnaringenin, 3,4-dihydroxyphenylacetic acid and 5-(3’,4’-dihydroxyphenyl)-g-valerolactone, the short-chain fatty acids butyrate, propionate and acetate, and indole-3-propionic acid are reviewed. Though the metabolites’ mechanism of action and the influence of health conditions on metabolite production are not always fully understood yet, there are many reasons to direct the attention to “gut health”. It could offer new options for preventing or treating a variety of disease states and nutrition-derived microbial products might inspire future drug development. To investigate underlying interactions between food ingredients and genes simple models, such as the red flour beetle Tribolium castaneum, appear especially constructive. Here we show that 1 % of a grape seed extract containing 30 % of procyanidins, significantly increases the survival of T. castaneum at 42 °C when added to flour as a dietary source. The beneficial effects of grape seed extract could not be reproduced by supplementing flour with single catechins of which the oligomeric procyanidins consist. We identified previously stress resistance genes responsible for a survival extension by dietary ingredients and show here by the use of RNA-interference that a knockdown of transcripts encoding homologues of Nrf-2 or Jnk-1 block the effects of grape seed extract on survival. Interestingly, grape seed extract under knockdown of Foxo-1 caused a significant survival reduction, stressing the hormetic response as underlying the survival extension by the dietary interventions. In conclusion, our studies provide evidence that a procyanidin-rich extract is able to extend the survival of the model organism T. castaneum. Catechin monomers, however, appear not to mediate the effects. The active ingredients, moreover, need the presence of stress resistance factors, and here especially of Foxo-1, in order to promote their preventive activities with regard to degenerations.
