  • Ein Kind, das mit einer orofazialen Spalte geboren wird, ist in seiner Sprech- und Sprachentwicklung einer Reihe von erschwerten Bedingungen ausgesetzt. Neben einer gestörten Muskelbalance der orofazialen Muskulatur, können die typischen Zungenbewegungen beim Trinken vom Säugling nicht ausgeführt und damit wichtige feinmotorische Trainingsschritte nicht absolviert werden. Nicht selten kommt es auch zu Störungen des Schluckvorgangs und zu einer, durch pathologische Druck-Volumenverhältnisse auftretenden, hypernasalen Resonanz. Um dieEin Kind, das mit einer orofazialen Spalte geboren wird, ist in seiner Sprech- und Sprachentwicklung einer Reihe von erschwerten Bedingungen ausgesetzt. Neben einer gestörten Muskelbalance der orofazialen Muskulatur, können die typischen Zungenbewegungen beim Trinken vom Säugling nicht ausgeführt und damit wichtige feinmotorische Trainingsschritte nicht absolviert werden. Nicht selten kommt es auch zu Störungen des Schluckvorgangs und zu einer, durch pathologische Druck-Volumenverhältnisse auftretenden, hypernasalen Resonanz. Um die Auswirkungen der offenen Mund-Nasenraum-Verbindung zu mindern, sehen viele Therapiekonzepte den Einsatz einer Gaumenplatte vor, die den Spaltbereich abdeckt. Auch die vorsprachliche Lautproduktion wird durch den fehlenden oder mangelhaften Verschluss zwischen Velum und Rachenhinterwand bereits erheblich eingeschränkt. Um die im direkten oder indirekten Zusammenhang mit der Spalte entstehenden Entwicklungsstörungen in Bezug auf den Sprech- und Spracherwerb zukünftig bei Kindern mit Lippen-Kiefer-Gaumen-Segelspalten früher als bisher zu diagnostizieren und geeignete Therapiekonzepte entwickeln zu können, sind prospektive Längsschnittstudien, die engmaschig vorsprachliche und sprachliche Entwicklungsschritte dokumentieren, unerlässlich. Vorliegende Arbeit versucht einen Mosaikstein dazu beitzutragen, zukünftig geeignete Entwicklungsmarker durch eine objektive vorsprachliche Entwicklungsdiagnostik zu identifizieren. Würde es zukünftig gelingen, neben bekannten Risikomarkern für neurophysiologische Störungen am Schrei auch Risikomarker für potentielle spätere Sprech- und Sprachstörungen zu identifizieren, könnte dies grundlegend für neue Frühförderkonzepte in der Behandlung von Kindern mit orofazialen Spalten sein. Im Rahmen dieser Longitudinalstudie wurde die vorsprachliche Lautentwicklung von sechs Säuglingen mit einer orofazielen Spalte im zweiten Lebenshalbjahr aufgezeichnet und quantitativ analysiert. Um dabei u.a. auch einen Zusammenhang zwischen ausgewählten Schreieigenschaften und der Gaumenplatte zu untersuchen, wurden zu jedem Termin Aufnahmen mit und ohne eingesetzte Platte gemacht. Zur objektiven Beurteilung des Lauterwerbs und des Platteneinflusses wurden so in zeitlich dichten Intervallen insgesamt 2752 Signale mittels qualitativ hochwertiger Aufnahmetechnik und standardisierter Analysen aufgezeichnet und ausgewertet und mit 1940 Signalen einer altersentsprechenden Kontrollgruppe ohne orofaziale Spalte verglichen. Die Signal-analyse erfolgte unter Verwendung des CSL-4300 Sprachanalysesystems der Firma KAY Elemetrics/ USA. Anschließend wurden eine quantitative und eine qualitative Strukturanalyse nach Wermke (2004) durchgeführt. Diese Art der Analyse ermöglichte einen direkten und objektiven Vergleich prosodierelevanter Eigenschaften vorsprachlicher Laute der Gruppe von Säuglingen mit orofazialen Spalten zur Kontrollgruppe. Es wurde gefunden, daß die untersuchte Gruppe von Säuglingen mit orofazialen Spalten im Vergleich zur Kontrollgruppe eine deutliche Entwicklungsverzögerung in bestimmten Aspekten der Zeitorganisation der Phonation sowohl von Schreien als auch von Übergangs- und Babbellauten aufweist. Diese Unterschiede bestanden unabhängig vom Tragen einer Gaumenplatte. Dies bedeutet, daß der potentielle positive Effekt, den die Gaumenplatte auf die früheste Sprech- und Sprachentwicklung hat im untersuchten Altersbereich bereits stabil verankert ist. Die gefundenen Unterschiede bestehen vor allem in einer verspäteten, bzw. deutlich selteneren spontanen Produktion von intentional segmentierten Lauten verschiedener Vokalisationstypen und in stark verlängerten Lauten bei den Säuglingen mit orofazialen Spalten. Derartige Besonderheiten sind bisher bei Säuglingen mit orofazialen Spalten nicht beschrieben worden, da vergleichbare quantitative Analysen nach unserer Kenntnis nicht vorliegen. Die Arbeit vergleicht eigene Befunde, soweit sinnvoll und möglich, mit bisherigen Erkenntnissen und versucht anhand der Daten Erklärungsansätze für die teilweise sehr widersprüchlichen Befunde anderer Autoren zu zeigen. Die Ergebnisse der vorliegenden Untersuchung liefern erstmalig wissenschaftliche Argumente für die Annahme, daß neben den rein morphologischen Besonderheiten des Vokaltrakts der Säuglinge mit orofazialen Spalten auch neurophysiologische Besonderheiten bei diesen Patienten auftreten. Weiterführende Studien an einer größeren Stichprobe unter strengerer Standardisierung sind erforderlich, um die Befunde zu evaluieren. Sollte sich dabei zeigen, daß die Zeitorganisation der Phonation tatsächlich, wie hier vermutet, eine essentielle Komponente zur Charakterisierung abweichender vorsprachlicher Entwicklungsverläufe ist, hätte man die Möglichkeit, daraus einen klinisch relevanten Indikator zu entwickeln.show moreshow less

Apart from a disturbed muscle balance of the orofacial muscles, the typical tongue movements during drinking cannot be accomplished by the baby, thus important fine-motoric training steps can not be achieved. It often comes to interferences of the sip procedure and to a hypernasal resonance, because of a pathological pressure volume relation. Resonance conditions in the vocal area during the generation ofA child, who is born with an orofacial cleft, is exposed to a set of more difficult conditions during its speech and language development. Apart from a disturbed muscle balance of the orofacial muscles, the typical tongue movements during drinking cannot be accomplished by the baby, thus important fine-motoric training steps can not be achieved. It often comes to interferences of the sip procedure and to a hypernasal resonance, because of a pathological pressure volume relation. Resonance conditions in the vocal area during the generation of pre-speech of a baby with an orofacial cleft differ substantially from a baby without these anatomical anomalies. In order to reduce the effects of the open mouth-to-nose passage, many therapy concepts plan the implementation of a palate plate, which covers the gap range. Also pre-linguistic sound production is already substantially limited by the missing or unsatisfactory connection between velum and pharynx. Concerning new diagnostic possibilities and suitable future therapy-concepts more prospective longitudinal studies are necessary. This thesis tries to add a mosaic stone to identify suitable development markers for an objective pre-linguistic-development diagnosis. If it would be possible to identify besides well-known risk markers for neurophysiological disturbances of the cry also risk markers for potential later speech and language disturbances, this could be fundamental to develop new early furtherance concepts in the treatment of children with orofacial clefts. In the context of this longitudinal study the pre-linguistic sound-development of six babies with an orofacial cleft were recorded in their second half-year of life and the sounds were analysed quantitatively. In order to examine a connection between selected cry characteristics and the palate plate, recordings were taken with and without the palate plate on each date. For the objective evaluation of the pre-speech development and the influence of infant orthopedic appliances we compared 2752 voice-signals, taken in close time-intervals at a high-standard recording technique in standardized analyses, and compared them with1940 signals of an age-appropriate control-group without orofacial clefts. For analysing the sound-signals we used the CSL-4300 speech-analysis-system of KAY Elemetrics/ USA. Subsequently, a quantitative and a qualitative structure-analysis after Wermke (2004) was accomplished. This kind of analysis enabled us to make a direct and objective comparison of prosodic relevant characteristics of pre-linguistic sounds between the group of babies with orofacial clefts and the control group. It was found that, in comparison to the control group, the examined group of babies with orofacial clefts exhibits a clear development delay in certain aspects of the time organization of the phonation both from cries and from babbling. These differences existed independently carrying a palate plate or not. This means that the potential positive effect, which the palate plate has on the earliest speech and language development is already embodied stably in the examined age range. The differences we found exist particularly in a late, and/or clearly rarer spontaneous production of intentional segmented sounds of different vocalisation types and in strongly extended sounds of the babies with orofacial clefts. Such characteristics were not described for babies with orofacial clefts so far, since comparable quantitative analyses to our knowledge are not present. As far as possible, the study compares own results on the basis of our data with existing knowledge and tries to find an explanation for the partly very contradictory results of other authors. The results of this investigation are giving for the first time scientific arguments for the assumption that apart from the purely morphologic characteristics of the vocal area of the babies with orofacial clefts also neurophysiological peculiarities appear on these patients. To evaluate the findings, more studies under stricter standardisation and of larger samples are necessary. If it shows that the time organization of phonation, as assumed here, is an essential component for the characterisation of pre-linguistic development processes, one would have the possibility to develop a clinically relevant indicator.show moreshow less
