Zur Berücksichtigung vermuteter relevanter Einflussfaktoren sollten geschlechts- und altersspezifische Unterschiede sowie Hörleistungen im Neugeborenen-Hörscreening (NHS) berücksichtigt werden. Dazu wurden spontan geäußerte Lautproduktionen von 82 Neugeborenen (2. – 4. Lebenstag) analysiert. Um die Ergebnisse des NHS-Tests zu berücksichtigen, wurden die Probanden in 2 Untergruppen eingeteilt. Diese waren (1) Neugeborene mit unauffälligem Hörscreening-Test und (2) Neugeborene mit auffälligem Hörscreening-Test. Mit Hilfe der Sprachanalysesoftware Praat wurden 1545 Tonaufnahmen der Probanden vermessen. Der Autor vorliegender Arbeit hat ein Praat – Textscript erstellt, um die Segmente des Atemzyklus manuell durch Cursor festzulegen und die Messgrößen automatisch zu berechnen und anschließend in das am ZVES vorliegende Routinesystem einzubetten. Die Ergebnisse der vorliegenden Studie belegen, dass schon in den ersten Schreien Neugeborener ein regelhaft segmentierter Atemrhythmus mit einer kurzen Inspirationsphase und nachfolgender längerer Exspirationsphase, die den Hauptanteil des ganzen Atemzyklus ausmacht, zu beobachten ist. Das Verhältnis zwischen Inspirations- und Exspirationslänge zeigt sich variabel in Abhängigkeit von der Gesamtlänge des Atemzyklus. Signifikante geschlechts- und altersspezifische Unterschiede könnten für eine schnellere Adaptation (Reifung) des Atemsystems bei weiblichen Neugeborenen sprechen und damit für eine reduzierte respiratorische Kontrollfähigkeit bei Jungen. Ursächlich dafür könnten anatomisch-somatische Differenzen sowie nachwirkende intrauterine hormonelle Einflüsse sein. Auch der signifikante Einfluss des Gestationsalters lässt möglicherweise auf Reifungsprozesse des Atemapparats schließen, die mit dem Gestationsalter voranschreiten. Weitere Faktoren wie der Geburtsmodus oder das Ergebnis des Neugeborenen-Hörscreening-Tests wirkten sich hier nicht auf die temporalen Eigenschaften inspiratorischer und exspiratorischer Laute der Neugeborenen aus. Insgesamt konnte die vorliegende Arbeit erstmalig temporale Eigenschaften ingressiver Laute und spontaner egressiver Phonationsleistungen Neugeborener im Alter von 2 bis 4 Lebenstagen objektiv analysieren und entsprechende Referenzwerte für gesunde Neugeborene liefern. Diese können in zukünftigen Studien mit größerer Probandenanzahl und einem längerem Untersuchungszeitraum (Einschluss der Minipubertät) als Vergleichswerte dienen. Ein Beleg für den angeborenen respiratorischen Mechanismus für die Sprech- und Sprachentwicklung konnte mit der vorliegenden Arbeit ebenfalls geliefert werden.   • The present study analyzed temporal characteristics of temporally consecutive inspiratory phonation and expiratory cry utterance in spontaneous crying of neonates. The aim of this study was to objectively determine the breath cycle in neonates spontaneous crying and to acquire reference values for each segment of the breath cycle. Considering supposed relevant influential factors, gender- and age-specific differences as well as hearing capacity in neonatal hearing screening (NHS) were observed. For that spontaneous sound utterances of 82The present study analyzed temporal characteristics of temporally consecutive inspiratory phonation and expiratory cry utterance in spontaneous crying of neonates. For that spontaneous sound utterances of 82 neonates (2. – 4. day of life) were analyzed. To consider the results of the NHS-tests the subjects were divided into 2 subgroups. These were (1) neonates with normal NHS-test and (2) neonates with remarkable NHS-test. By using the speech analysis software PRAAT 1545 sound recordings of the subjects were measured. The author of the present work created a PRAAT-textscript to manually set the segments of the breath cycle with cursors and to automatically compute the measured variables and afterwards embed them in the existing routine of the ZVES. The results of the present study attest already in the first cries of neonates a regular segmented breath group with a short inspiration and a following longer expiration, which constitutes the main part of the whole breath cycle. The ratio of inspiration- and expirationlength exhibits a variability in dependence on the total-length of the breath cycle. Significant gender- and age-specific differences could suggest a faster adaptation (maturation) of the respiratory system in female neonates and therefore a limited respiratory control-capability in male neonates. This could be due to anatomical-somatic differences and intrauterine hormonal influences. Also the significant influence of gestational age potentially implies maturation processes of the respiratory system, which proceed with gestational age. Other factors, such as birth mode or the result of the NHS-test, did not affect the temporal characteristics of inspiratory and expiratory sounds of the neonates. Overall, the present study was the first to objectively analyze the temporal characteristics of inspiratory sounds and spontaneous expiratory phonation of neonates aged 2 to 4 days of life and to provide appropriate reference values for healthy neonates. These can serve as comparative values in future studies with a larger number of subjects and a longer evaluation period (inclusion of minipuberty). Evidence for the innate respiratory mechanism for speech and language development could also be provided with the present work.
