Urinary, Circulating, and Tissue Biomonitoring Studies Indicate Widespread Exposure to Bisphenol A

Bisphenol A (BPA) is one of the highest-volume chemicals produced worldwide, and human exposure to BPA is thought to be ubiquitous. Thus, there are concerns that the amount of BPA to which humans are exposed may cause adverse health effects. We examined many possibilities for why biomonitoring and toxicokinetic studies could come to seemingly conflicting conclusions. More than 80 published human biomonitoring studies that measured BPA concentrations in human tissues, urine, blood, and other fluids, along with two toxicokinetic studies of human BPA metabolism were examined. Unconjugated BPA was routinely detected in blood (in the nanograms per milliliter range), and conjugated BPA was routinely detected in the vast majority of urine samples (also in the nanograms per milliliter range). In stark contrast, toxicokinetic studies proposed that humans are not internally exposed to BPA. Available data from biomonitoring studies clearly indicate that the general population is exposed to BPA and is at risk from internal exposure to unconjugated BPA. The two toxicokinetic studies that suggested human BPA exposure is negligible have significant deficiencies, are directly contradicted by hypothesis-driven studies, and are therefore not reliable for risk assessment purposes.
In 55 chronic schizophrenics, the occurrence of infectious diseases during their mothers' pregnancies was investigated. Different psychiatrie diagnostic systems were compared. Infections were reported by the mothers of familial and sporadic DSM I1I-R schizophrenics in equal proportion. However, applying Leonhard's classification, the frequency of infections was found to be significantly increased in 'systematic' schizophrenia (mainly exogenously induced in the view of Leonhard) compared to 'unsystematic' schizophrenia (mainly genetically determined according to Leonhard's findings). Most of the infections occurred during the second trimester (nine out of 13). Thus, in the 'systematic' forms of schizophrenia (low genetic loading), maternal infections in this crucial period of neurodevelopment would appear to be important causative factors in the cytoarchitectural deviance detected in the central nervous system of schizophrenics.
Background
The onset of mental illness such as depression and anxiety disorders in pregnancy and postpartum period is common. The coronavirus induced disease 2019 (COVID-19) pandemic and the resulting public policy responses represent an exceptional situation worldwide and there are hints for adverse psychosocial impact, hence, the study of psychological effects of the pandemic in women during hospitalization for delivery and in the postpartum period is highly relevant.
Methods
Patients who gave birth during the first wave of the COVID-19 pandemic in Germany (March to June 2020) at the Department of Obstetrics and Gynecology, University of Würzburg, Germany, were recruited at hospital admission for delivery. Biosamples were collected for analysis of SARS-CoV-2 infection and various stress hormones and interleukin-6 (IL-6). In addition to sociodemographic and medical obstetric data, survey questionnaires in relation to concerns about and fear of COVID-19, depression, stress, anxiety, loneliness, maternal self-efficacy and the mother–child bonding were administered at T1 (delivery stay) and T2 (3–6 months postpartum).
Results
In total, all 94 recruited patients had a moderate concern of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) at T1 with a significant rise at T2. This concern correlated with low to low-medium general psychosocial stress levels and stress symptoms, and the women showed a significant increase of active coping from T1 to T2. Anxiety levels were low and the Edinburgh Postnatal Depression Scale showed a medium score of 5 with a significant (T1), but only week correlation with the concerns about SARS-CoV-2. In contrast to the overall good maternal bonding without correlation to SARS-CoV-2 concern, the maternal self-efficiency correlated negatively with the obstetric impairment caused by the COVID-19 pandemic.
Conclusion
Obstetric patients` concerns regarding SARS-CoV-2 and the accompanying pandemic increased during the course of the pandemic correlating positively with stress and depression. Of note is the increase in active coping over time and the overall good mother–child-bonding. Maternal self-efficacy was affected in part by the restrictions of the pandemic.
Der Erfolg der allogenen hämatopoetischen Stammzelltransplantation (HSZT) als Immuntherapie basiert neben den Minorantigendifferenzen zwischen Spender und Empfänger entscheidend auf einer spendervermittelten Immunität gegen tumorassoziierte Antigene (TAA), über deren Herkunft und Frequenz bei gesunden Blutspendern die derzeitige Studienlage kaum Informationen bietet. Da für viele klinisch relevante TAA eine Expression im fetalen und plazentaren Gewebe bekannt ist, wurde in dieser Arbeit die Schwangerschaft als möglicher Auslöser dieser T-lymphozytären Gedächtnisimmunantworten im Sinne eines Tumor- und Transplantationsmodells untersucht.
Hierfür wurden insgesamt 114 gesunde Blutspender in drei Subgruppen aus 38 Frauen mit negativer Schwangerschaftsanamnese, 38 Frauen mit positiver Geburtenanamnese und 38 Männern in einer Querschnittsstudie betrachtet, daneben wurden 44 Frauen longitudinal während und nach ihrer ersten Schwangerschaft untersucht. Dabei wurden die CD8-positiven T-Lymphozyten der Probanden isoliert, mit Peptiden der vier klinisch relevanten TAA HER2/neu (human epidermal growth factor receptor 2), MUC1 (Mucin 1), PRAME (preferentially expressed antigen of melanoma) und WT1 (Wilms tumor protein 1) stimuliert und die Produktion von IFN-γ-mRNA mittels RT-qPCR gemessen. Daneben wurden zum Vergleich durchflusszytometrische und ELISPOT-basierte Verfahren durchgeführt.
Bei den gesunden Blutspendern konnten CD8-positive Gedächtnisimmunantworten von niedriger und/oder hoher funktioneller Avidität gegen alle vier untersuchten TAA gemessen werden: Die Frequenz der dabei als „positiv“ definierten Immunantworten betrug bei HER2/neu 5 %, bei MUC1 14 %, bei PRAME 7 % und bei WT1 15 %. Männer wiesen insgesamt höhere absolute Level der Immunantworten gegen die untersuchten TAA auf, was auf eine testikuläre Expression dieser Antigene zurückzuführen sein könnte. In der Longitudinalanalyse bei den erstschwangeren Frauen ließen sich die stärksten Immunantworten zu Beginn der Schwangerschaft nachweisen, so dass es hier zu einem „Boost“ präexistenter TAA-spezifischer Autoimmunität zu kommen scheint. Durch das immunsuppressive hormonelle Milieu im Verlauf der Schwangerschaft und den Verlust der Zielantigene der feto-plazentaren Einheit durch die Geburt und Nachgeburt scheint diese Immunität aber nicht zu persistieren. Dadurch erklärt sich auch die Beobachtung, dass Frauen mit einer positiven Geburtenanamnese keine stärkeren Immunantworten gegen die untersuchten TAA aufwiesen als Frauen mit einer negativen Schwangerschaftsanamnese. Die Schwangerschaft hinterlässt diesbezüglich also ohne die Anwesenheit der vermittelnden Antigene keinen regelhaft bleibenden Effekt.
Diese Resultate decken sich mit Beobachtungen aus der Tumorimmuntherapie, bei denen Vakzinierungen gegen TAA zwar eine kurzfristige Immunität generieren konnten, die aber nicht persistierte. Im Rahmen der HSZT kann eine solche TAA-spezifische Immunität vom Spender auf den Empfänger transferiert werden und vermag dann aufgrund des proinflammatorischen Immunmilieus sehr wohl zu expandieren und in einem begrenzten Ausmaß auch zu persistieren.
Dementsprechend ergeben sich aus den in dieser Arbeit gewonnenen Resultaten relevante Implikationen für die allogene und in geringerem Ausmaß die autologe HSZT, daneben aber auch für innovative Tumortherapien wie die Immuncheckpoint-Blockade, da die Persistenz von tumorspezifischer Immunität letztendlich hochrelevant für eine langfristige Tumorkontrolle und damit für ein tumorfreies Überleben ist. Das vorliegende Modell trägt somit zum Verständnis der komplexen immunregulatorischen Vorgänge bei der Tumorkontrolle bei. Ob die hierbei aufgezeigten Immunantworten generell zu einer verbesserten TAA-spezifischen Immunrekonstitution und konsekutiv zu einem besseren klinischen Ergebnis beitragen, bleibt offen und wird in klinischen Studien geklärt werden müssen.
Aim:
This randomized controlled trial assessed the impact of Lactobacillus reuteri on pregnancy gingivitis in healthy women.
Materials and Methods:
Forty-five healthy women (24 test/21 placebo) with pregnancy gingivitis in the third trimester of pregnancy were enrolled. At baseline Gingival Index (GI) and Plaque Index (PlI) were assessed at the Ramfjord teeth and venous blood taken for TNF-alpha analysis. Subsequently participants were randomly provided with lozenges to be consumed 2 9 daily until birth (approx. 7 weeks) containing >= 10(8) CFU L. reuteri ATCC PTA 5289 and >= 10(8) CFU L. reuteri DSM 17938 (test) or being devoid of L. reuteri (placebo). Within 2 days after birth recording of GI, PlI and blood sampling were repeated.
Results:
At baseline, mean GI and mean PlI did not differ significantly between both groups. In the test group mean TNF-alpha serum level was significantly (p < 0.02) lower than in the placebo group. At reevaluation, mean GI and mean PlI of the test group were both significantly (p < 0.0001) lower than in the placebo group. Mean TNF-alpha serum level did no longer differ significantly between the groups.
Conclusions:
The consumption of L. reuteri lozenges may be a useful adjunct in the control of pregnancy gingivitis.
There is still limited knowledge about alterations of blood concentrations of psychotropic drugs during pregnancy, the transfer of psychotropic drugs into breastmilk and the effects on exposed children. We investigated changes in concentrations of psychopharmacological medication during pregnancy and lactation in serum and breastmilk at different time points in a naturalistic sample of 60 mothers and observed the development of the exposed children in the first 12 months. We found a decrease in serum concentrations from the first to the second trimester of amitriptyline, duloxetine, escitalopram, quetiapine and sertraline. Citalopram stayed rather stable during pregnancy, sertraline levels interestingly increased again from the second to the third trimester. High concentration-by-dose ratios in breastmilk were found for venlafaxine as well as lamotrigine, low for quetiapine and clomipramine. Similarly, clomipramine and quetiapine showed low milk/serum–penetration ratios. Regarding the birth outcome measures in children, we found no significant differences between in utero exposed compared to nonexposed newborns. There were no significant differences in the development in the first 12 months. Psychotropic medication in the peripartum needs a balancing of risks and benefits and a continuous therapeutic drug monitoring can be a guidance for clinicians to monitor drug alteration patterns, which are likely to occur due to physiological pregnancy-associated changes in pharmacokinetics. Accordingly, therapeutic drug monitoring can optimize a medication in pregnancy and lactation with the lowest effective dose.
Nachdem im Rahmen einer engen Zusammenarbeit zwischen dem Missionsärztlichen Institut in Würzburg und dem Centre de Santé in Gikonko, Ruanda bereits im Vorfeld dieser Arbeit erhebliche Probleme mit falsch positiven HIV-Schnelltestergebnissen aufgefallen waren, wurden 162 Patienten bezüglich ihres HIV-Status untersucht und bei einer kleineren Gruppe (46 Patienten) eine eingehende serologische Analyse durchgeführt. Dabei waren Ziele dieser Arbeit (1) den HIV-Status aller in Gikonko als HIV-positiv geführten Patienten zu überprüfen, um so das diagnostische Problem genauer einschätzen zu können; (2) die Reaktionsmuster der Tests zu analysieren, um mögliche Schwachpunkte der einzelnen Schnelltests zu finden; (3) nach Ursachen sowie potentiellen Kreuzreaktivitäten für uneindeutige oder falsch positive Testergebnisse zu suchen; (4) Risikogruppen zu definieren, in denen HIV-Schnelltests weniger aussagekräftig sind und (5) die angepasste HIV-Diagnostik in einigen ausgewählten Fällen mit genaueren Verfahren in Deutschland zu überprüfen und nach weiteren serologischen Auffälligkeiten zu suchen. Hierfür wurden Blutproben mithilfe der im ruandischen Testalgorithmus verwendeten Schnelltests (DetermineTM, UnigoldTM, CapillusTM, First ResponseTM) vor Ort auf HIV getestet. Von Patienten mit zu Voruntersuchungen abweichenden bzw. wider-sprüchlichen Ergebnissen wurden Serumproben nach Deutschland zu genaueren serologischen Analyse transportiert. Diese umfasste die virologische Untersuchung auf HIV mittels ELISA, Western Blot (im positiven Fall zur Bestätigung) sowie PCR, eine immunologische und autoimmunologische Untersuchung (Bestimmung von IgG, IgM, Rheumafaktor sowie ANCA), sowie eine parasitologische Untersuchung (Bestimmung von Malaria-Antikörper) und die Messung von Werten der klinischen Chemie (Kreatinin, GOT, GPT). Ergänzt wurde die serologische Analyse durch die Erhebung von Daten aus der Kartei des Krankenhauses sowie der Schwangerenambulanz in Gikonko. Es zeigte sich, dass unter Verwendung von HIV-Schnelltests bei nur 79% der Patienten (n = 112) der HIV-positive Status bestätigt werden konnte, 15% (n = 21) erhielten ein klar negatives Ergebnis und in 6% (n = 8) führten die HIV-Schnelltests zu keinem eindeutigen Ergebnis. Bei der virologischen Untersuchung in Deutschland wurden alle negativen Ergebnisse bestätigt; unter den acht uneindeutig getesteten Seren wurde in zwei Fällen eine HIV-Infektion nachgewiesen. Um Schwachpunkte oder Anfälligkeiten einzelner Tests auf Störfaktoren aufzeigen, wurden die Reaktionsmuster der HIV-Schnelltests entsprechend der genaueren serologischen Untersuchungen dargestellt und analysiert. Es zeigte sich, dass besonders DetermineTM und CapillusTM ursächlich für uneindeutige Testergebnisse waren. Bei der genaueren Analyse von Seren von Patienten mit zu Voruntersuchungen abweichenden bzw. widersprüchlichen Ergebnissen wurden bei 7% (n = 3) Rheumafaktoren und bei 16% (n = 5) ANCAs nachgewiesen. Außerdem zeigten 48% der Proben (n = 22) stark erhöhte IgM-Werte. Es waren wiederum vornehmlich die Tests DetermineTM und CapillusTM, welche Probleme bei der Testung der entsprechenden Seren aufwiesen. Der Einfluss zweier äußerer Faktoren hingegen war evident: Zum einen führte die Interpretationsvorschrift der Hersteller, jegliche Reaktion als positiv zu werten, zu vielen falsch positiven Ergebnissen und zum anderen genügten die zur Verfügung stehenden HIV-Schnelltests im ländlich gelegenen Gikonko, mit einer HIV-Prävalenz von 4%, den biomathematischen Anforderungen nicht. Die erhobenen Daten aus der Kartei des Centre de Santé erhärteten die Annahme, dass die Neutralität der interpretierenden Person durch eine verdächtige Familien- oder Eigenanamnese bzw. eine entsprechende Klinik gefährdet wird. Die Analyse der Kartei der Schwangerenambulanz zeigte eine signifikante Häufung falsch positiver Testergebnisse im letzten Trimenon.
(1) Background: Data on coronavirus 2 infection during pregnancy vary. We aimed to describe maternal characteristics and clinical presentation of SARS-CoV-2 positive women requiring intensive care treatment for COVID-19 during pregnancy and postpartum period based on data of a comprehensive German surveillance system in obstetric patients. (2) Methods: Data from COVID-19 Related Obstetric and Neonatal Outcome Study (CRONOS), a prospective multicenter registry for SARS-CoV-2 positive pregnant women, was analyzed with respect to ICU treatment. All women requiring intensive care treatment for COVID-19 were included and compared regarding maternal characteristics, course of disease, as well as maternal and neonatal outcomes. (3) Results: Of 2650 cases in CRONOS, 101 women (4%) had a documented ICU stay. Median maternal age was 33 (IQR, 30–36) years. COVID-19 was diagnosed at a median gestational age of 33 (IQR, 28–35) weeks. As the most invasive form of COVID-19 treatment interventions, patients received either continuous monitoring of vital signs without further treatment requirement (n = 6), insufflation of oxygen (n = 30), non-invasive ventilation (n = 22), invasive ventilation (n = 28), or escalation to extracorporeal membrane oxygenation (n = 15). No significant clinical differences were identified between patients receiving different forms of ventilatory support for COVID-19. Prevalence of preterm delivery was significantly higher in women receiving invasive respiratory treatments. Four women died of COVID-19 and six fetuses were stillborn. (4) Conclusions: Our cohort shows that progression of COVID-19 is rare in pregnant and postpartum women treated in the ICU. Preterm birth rate is high and COVID-19 requiring respiratory support increases the risk of poor maternal and neonatal outcome.
Multiple Antenatal Dexamethasone Treatment Alters Brain Vessel Differentiation in Newborn Mouse Pups

Antenatal steroid treatment decreases morbidity and mortality in premature infants through the maturation of lung tissue, which enables sufficient breathing performance. However, clinical and animal studies have shown that repeated doses of glucocorticoids such as dexamethasone and betamethasone lead to long-term adverse effects on brain development. Therefore, we established a mouse model for antenatal dexamethasone treatment to investigate the effects of dexamethasone on brain vessel differentiation towards the blood-brain barrier (BBB) phenotype, focusing on molecular marker analysis. The major findings were that in total brains on postnatal day (PN) 4 triple antenatal dexamethasone treatment significantly downregulated the tight junction protein claudin-5, the endothelial marker Pecam-1/CD31, the glucocorticoid receptor, the NR1 subunit of the N-methyl-D-aspartate receptor, and Abc transporters (Abcb1a, Abcg2 Abcc4). Less pronounced effects were found after single antenatal dexamethasone treatment and in PN10 samples. Comparisons of total brain samples with isolated brain endothelial cells together with the stainings for Pecam-1/CD31 and claudin-5 led to the assumption that the morphology of brain vessels is affected by antenatal dexamethasone treatment at PN4. On the mRNA level markers for angiogenesis, the sonic hedgehog and the Wnt pathway were downregulated in PN4 samples, suggesting fundamental changes in brain vascularization and/or differentiation. In conclusion, we provided a first comprehensive molecular basis for the adverse effects of multiple antenatal dexamethasone treatment on brain vessel differentiation.
Maternal asthma is associated with persistent changes in allergic offspring antibody glycosylation

Background
Maternal asthma during pregnancy is considered an environmental risk factor for asthma development in children. Immunoglobulin G (IgG) antibodies that are transferred from the mother to the fetus are known to act in a pro‐ or anti‐inflammatory manner depending on their glycosylation status.
Objective
Using a mouse model, we examined how maternal allergic airway inflammation during pregnancy influenced offspring experimental asthma severity, as well as maternal and offspring serum IgG antibody glycosylation patterns. Additionally, the effects of maternal and offspring exposure to the same or different allergens were investigated.
Methods
Female mice were either sham sensitized or sensitized to casein (CAS) or ovalbumin (OVA) before mating. Subsequently, allergic lung inflammation was induced in pregnant dams via aerosol allergen challenge (sham, CAS or OVA). After weaning, pups were subjected to an experimental asthma protocol using OVA. Asn‐297 IgG glycosylation was analysed in maternal and offspring serum.
Results
When mothers and offspring were sensitized to the same allergen (OVA‐OVA), offspring had more severe experimental asthma. This was evidenced by altered antibody concentrations, increased bronchoalveolar lavage inflammatory cell influx and decreased lung tissue and lung draining lymph node regulatory T cell percentages. When mothers and offspring were sensitized to different allergens (CAS‐OVA), this phenotype was no longer observed. Additionally, maternal serum from allergic mothers had significantly higher levels of pro‐inflammatory IgG1, shown by decreased galactosylation and sialylation at the Asn‐297 glycosylation site. Similar glycosylation patterns were observed in the serum of adult allergic offspring from allergic mothers.
Conclusions and Clinical Relevance
We observed a strong association between maternal experimental asthma during pregnancy, increased offspring airway inflammation and pro‐inflammatory IgG glycosylation patterns in mothers and offspring. IgG glycosylation is not a standard measurement in the clinical setting, and we argue that it may be an important parameter to include in future clinical studies.
