Biofilm production is an important step in the pathogenesis of S. epidermidis polymer-associated infections and depends on the expression of the icaADBC operon leading to the synthesis of a polysaccharide intercellular adhesin (PIA). The PIA represents a sugar polymer consisting of ß-1,6 linked N-acetyl glucosaminoglycans and mediates the intercellular adherence of the bacteria to each other and the accumulation of a multilayered biofilm. Epidemiological and experimental studies strongly suggest that PIA-production and subsequently biofilm formation contributes significantly to the virulence of specific S. epidermidis strains. This work aimed on the investigation of external factors regulating the ica expression in S. epidermidis. For this purpose, a reporter gene fusion between the ica promoter and the beta-galactosidase gene lacZ from E. coli was constructed and integrated into the chromosome of an ica positive S. epidermidis clinical isolate. The reporter gene fusion was used to investigate the influence of external factors and of sub-MICs of different antibiotics on the ica expression. It was shown that the S. epidermidis biofilm formation is growth phase dependent with a maximum expression in the late logarithmic and early stationary growth phase. The optimal expression was recorded at 42 °C at a neutral pH ranging from 7.0 to 7.5. The glucose content of the medium was found to be essential for biofilm formation, since concentrations of 1.5 to 2 per cent glucose induced the ica expression. In addition, external stress factors as high osmolarity (mediated by 3 to 5 per cent sodium chloride), and sub-lethal concentrations of detergents, ethanol, hydrogene peroxide, and urea significantly enhanced the biofilm production. Subinhibitory concentrations of tetracyline, the semisynthetic streptogramin quinupristin/dalfopristin and the streptogramin growth promoter virginiamycin were found to enhance the ica expression 8 to 11-fold, respectively, whereas penicillin, oxacillin, gentamicin, clindamycin, vancomycin, teicoplanin, ofloxacin, and chloramphenicol had no effects. A weak induction was recorded for sub-MICs of erythromycin. Both quinupristin/ dalfopristin and tetracyline exhibited a strong postexposure effect on the S. epidermidis ica expression, respectively, even when the substances were immediately removed from the growth medium. The results were confirmed by Northern blot analysis of the ica transcription and quantitative analysis of biofilm formation in a colorimetric assay. Expression of the icaprom::lacZ reporter gene plasmid in Bacillus subtilis and S. epidermidis revealed that the ica induction by sub-MICs of streptogramins and tetracycline might depend on unidentified regulatory elements which are specific for the staphylococcal cell. In contrast, the activation by external stress signals seems to be mediated by factors which are present both in Staphylococci and in Bacillus subtilis. Construction and analysis of an agr-mutant in a biofilm-forming S. epidermidis strain excluded the possibility that the Agr-quorum-sensing system significantly contributes to the ica expression in the stationary growth phase. However, clear evidence was provided that in S. aureus the ica transcription depends on the expression of the alternative transcription factor sigmaB, which represents a global regulator of the stress response in S. aureus as well as in B. subtilis. For this purpose, a sigB knockout mutant had been constructed in a biofilm-forming S. aureus. This mutant showed a markedly decrease of the ica transcription and biofilm-production, whereas a complement strain carrying the sigB gene on an expression vector completely restored the biofilm-forming phenotype of the S. aureus wild type. Southern blot analysis indicated that the the sigB gene is also present in S. epidermidis and Northern analyses of the sigB and the ica transcription revealed that both genes are activated under identical conditions (i. e. in the stationary growth phase and by external stress factors) suggesting a similar regulatory pathway as in S. aureus. However, since neither in S. aureus nor in S. epidermidis the ica promoter has obvious similiarities to known SigB-dependent promotoer sequences it is tempting to speculate that the ica activation is not directely mediated by SigB, but might be indirectely controlled by other SigB-dependent regulatory elements which remain to be elucidated.
Bislang inhibieren antibakterielle Substanzen in erster Linie die Zellwandsynthese, den DNA- und RNA-Stoffwechsel sowie die Proteinsynthese. In dieser Arbeit wurde ein erster Versuch unternommen, neue Zielstrukturen für die Antibiotika-Therapie in S. aureus zu finden. Insgesamt wurden 10 Gene untersucht, die Funktion von 7 dieser Gene war in S. aureus unbekannt. Zunächst sollte herausgefunden werden, ob diese 10 Zielgene: topB, polA, nusG, SA1857, SA1444, SA1063, SA0453, SA0241, SA0245, SA0367, für S. aureus essentiell sind. Es wurde versucht, jedes dieser Gene in S. aureus zu deletieren. Außer den Genen SA1444, SA0245 und nusG konnten alle Zielgene deletiert werden und waren daher für S. aureus nicht essentiell. Die Deletionsmutanten ΔtopB, ΔpolA, ΔSA1857, ΔSA1063, ΔSA0453, ΔSA0241, ΔSA0367 wurden außerdem in einem Sepsismodell in Mäusen untersucht und waren nicht attenuiert. Gene, die weder in vitro noch in vivo essentiell sind, sind nur von geringem Interesse für die Entwicklung neuer Antibiotika. Zur Untersuchung essentieller Gene in S. aureus wurden vier verschiedene konditionale Expressionssystem untersucht. Bei drei dieser Systeme wurde der wildtypische Promotor gegen einen regulierbaren Promotor ausgetauscht. Bei einem weiteren System handelte es sich um einen Antisense-RNA-Ansatz. Zur Überprüfung der konditionalen Expressionssysteme („proof-of-principle“) wurden die bekannten essentiellen Gene ligA und dnaE in S. aureus mutiert. Eines dieser konditionalen Expressionssysteme, das pLL30/Pspac/pMJ8426-System, konnte erfolgreich in S. aureus angewandt werden. Die konditionale Expression von ligA und von SA0245 wurde mit diesem System erzielt. Der Temperatur-sensitive Shuttle-Vektor pLL30 dieses konditionalen Expressionssystems enthält eine Insertionskassette, die das Antibiotikaresistenzgen cat (Chloramphenicol-Acetyltransferase) sowie den regulierbaren Promotor Pspac enthält. Ein wesentliches Merkmal des pLL30/Pspac/pMJ8426-Systems ist die stabile Integration des Resistenzmarkers cat und des Pspac-Promotors durch ein doppeltes Crossover Ereignis vor dem Zielgen. Der Temperatur-sensitive Shuttle-Vektor pLL30 kann anschließend durch mehrfaches Subkultivieren der Bakterien bei nicht permissiver Temperatur eliminiert werden. Der Repressor LacI wird auf einem Extra-Plasmid pMJ8426 kodiert und wird nach erfolgter Integration des Pspac-Promotors vor dem Zielgen in die Bakterien transformiert. Mehrere Kopien des Repressorplasmids ermöglichen eine gute Repression an Pspac. Durch die Zugabe des Induktors IPTG kann der Repressor LacI inaktiviert und die Gen-Expression induziert werden. Insbesondere konnte dieses konditionale Expressionssystem erfolgreich im Tiermodell angewandt werden. Ein weiterer Schwerpunkt dieser Arbeit bildete die genetische und funktionelle Charakterisierung von SA0367. Durch biochemische Untersuchungen wurde gezeigt, dass dieses Gen für eine FMN-enthaltende Oxidoreduktase codiert. Auf Grundlage der in dieser Arbeit gewonnenen Erkenntnisse zur Funktion des Proteins wurde das Gen SA0367, in Analogie zum orthologen Gen nfrA aus B. subtilis, in nfrA umbenannt. Die Oxidoreduktase NfrA oxidiert NADPH in Gegenwart von FMN. In Gegenwart von NADPH und FMN zeigt das Enzym NfrA außerdem Nitroreduktase- und eine leichte Disulfidreduktase-Aktivität. Induktionsexperimente im Wildtyp zeigten, dass nfrA durch oxidativen Stress, verursacht von Diamide oder Nitrofurantoin, und durch Ethanol induziert wird. Ethanol und oxidativer Stress führt zu Denaturierung von Proteinen. NfrA könnte an der Reparatur geschädigter Proteine beteiligt sein. Die Induktion von nfrA in S. aureus erfolgt unabhängig vom alternativen Sigmafaktor SigB. Durch Primer Extension-Experimente wurde eine putative PerR-Box vor dem nfrA-Gen identifiziert. Diese könnte für die Regulation von nfrA wichtig sein. Außerdem wird nfrA während des gesamten Wachstumszyklus von einem SigAabhängigen Promotor exprimiert. Die Deletionsmutante ΔnfrA zeigt nur in Gegenwart hoher Ethanolkonzentrationen von 6,5 % einen leichten Wachstumsnachteil im Vergleich zum Wildtyp. Außerdem weist die ΔnfrA-Mutante eine höhere Resistenz gegenüber Nitrofurantoin auf. In weiteren Untersuchungen wurde begonnen die Funktion der Ser/Thr-Kinase SA1063 durch Microarray-Experimente aufzuklären. Hierbei wurde deutlich, dass dieses Gen eine regulatorische Rolle bei der Zellwandsynthese in S. aureus spielen könnte.
Staphylococcus aureus ist ein bedeutender opportunistischer Krankheitserreger, der eine Vielzahl von Infektionen in Menschen und Tieren hervorrufen kann. Das Krankheitsbild reicht von leichten Hautinfektionen bis hin zu lebensbedrohlichen Infektionen wie Endokarditis, Sepsis oder Pneumonien. S. aureus ist ein Haupterreger nosokomialer Infektionen. Besonders die Antibiotikaresistenzentwicklung von S. aureus–Stämmen ist problematisch. Als wirksame Antibiotika können zur Zeit oft nur noch Vancomycin, Synercid oder Linezolid zur Therapie eingesetzt werden. Die alarmierende Resistenzentwicklung in S. aureus verdeutlicht, dass die Entwicklung neuer Antibiotika und die Identifizierung neuer bakterieller Angriffsstrukturen dringend erforderlich ist. Gängige antiinfektive Therapeutika sind gegen die bakterielle Zellwandsynthese, den DNA- und RNA-Stoffwechsel oder die Proteinbiosynthese gerichtet. In dieser Arbeit sollten Virulenz-relevante Zielstrukturen für die Entwicklung neuer Antibiotika untersucht werden. Insgesamt wurden sieben Gene analysiert, von denen vier zu Anfang dieser Arbeit in S. aureus noch nicht charakterisiert waren. Die Zielgene (clpP, purH, ssrA und smpB) in S. aureus sollten deletiert werden, um ihre Überlebensnotwendigkeit in vitro- und in vivo zu überprüfen. Eine Deletion gelang bei den Genen clpP und purH, die somit als nicht essenziell in S. aureus zu betrachten sind. Die bereits zuvor als nicht-essenziell charakterisierten Gene arlR, arlS und putP wurden deletiert und die Mutanten dclpP, darlR, darlS, dpurH und dputP wurden phänotypisch in Hinsicht auf ihren Einfluss auf die Pathogenität in S. aureus analysiert. Die differenzielle Genexpression der Mutanten dclpP und darlR wurde mit Hilfe von Microarray-Hybridisierungsexperimenten untersucht. Die ∆clpP-Mutante zeigte einen starken Wachstumsdefekt bei verschiedenen Temperaturen (30, 37, 42°C) und war nicht mehr in der Lage bei 20°C zu wachsen. Ebenso war das Wachstum unter anaeroben Bedingungen stark beeinträchtigt. Der Stamm dclpP wies eine verringerte hämolytische Aktivität sowie eine verminderte Adhärenz an Polystyren auf. Außerdem konnte eine stark erhöhte autolytische Aktivität in einem Triton X-100-Assay beobachtet werden. In einem Invasions-Zellkulturassay mit 293T-Epithelzellen konnte eine ~10-fach erhöhte Invasivität im Vergleich zu dem isogenen Wildtyp festgestellt werden. Die Komplementierung der ∆clpP-Mutante durch Einführung eines clpP-Expressionsvektors führte nahezu bei allen getesteten Bedingungen zur Wiederherstellung des wildtypischen Phänotyps. Die Transkriptomanalyse der dclpP-Mutante ergab eine deutliche Veränderung in der Genexpression (15 % aller Gene). Eine computerunterstützte Analyse der Upstreambereiche der deregulierten Gene führte zu der Identifizierung verschiedener Regulons, die bei der bakteriellen Antwort auf verschiedene Stressbedingungen eine Rolle spielen. Die clpP-Deletion betrifft besonders Regulatoren, deren Aktivität in Abhängigkeit zu veränderten Redox-Bedingungen reguliert wird, wie z. B. verschiedenen Stressbedingungen und Anaerobiose. Die Konstruktion der darlR- und darlS-Mutanten führte zu einer gesteigerten hämolytischen Aktivität, einer erhöhten Adhärenz an Polystyren sowie einer erhöhten autolytischen Aktivität in Triton X-100-Assays. Die Internalisierungsrate durch 293T-Epithelzellen war vermindert. Die darlR-Mutante wurde in einem Katheter-assoziierten Infektionsmodell in Ratten eingesetzt. Die kompetitive Infektion mit Mutante und Wildtyp ergab einen deutlichen Nachteil bei der Etablierung einer Infektion durch die Mutante. Die Transkriptomanalyse der 8325darlR-Mutante in der exponenziellen und in der stationären Phase unterstreicht den großen Einfluss des ArlRS-Zwei-Komponenten-Systems auf die Regulation der Genexpression in S. aureus. In der exponenziellen Phase wurden insgesamt 5 % und in der stationären Phase 15 % der Gene differenziell exprimiert. dpurH- und dputP-Mutanten wiesen in vitro keine Veränderungen im Wachstums-verhalten, der Biofilmbildung oder hämolytischen Aktivität auf. In einem Infektionsmodell in Ratten führte die Deletion von purH in dem S. aureus-Stamm MA12 zu einer signifikanten Verminderung der Virulenz. Die Herstellung von smpB- und ssrA-Deletionsmutanten verlief ohne Erfolg. Es wurde versucht, einen direkten Nachweis für den essenziellen Charakter dieser Gene durch den Einsatz konditional letaler Expressionssysteme zu erbringen. Weder der Austausch des wildtypischen durch einen regulierbaren Promotor noch eine Antisense-RNA-Strategie war für eine eindeutige Klärung dieser Frage ausreichend. Es konnte durch diese Arbeit jedoch gezeigt werden, dass die Antisense-RNA-Strategie eine Beeinträchtigung des Wachstums von S. aureus bewirkt.
Die Therapie von bakteriellen Infektionen beruht heutzutage zum Großteil auf dem Einsatz von Antibiotika. Die schnelle Entwicklung und rasche Verbreitung von resistenten Stämmen mancher Erreger gegen diese Antibiotika stellt ein enormes Problem für das Gesundheitswesen dar. Da momentan zur Antibiotikatherapie keine Alternativen bestehen, kommt der Erforschung neuer potenzieller Wirkstoffe eine sehr große Bedeutung zu. In einem Screening-Verfahren lagen die minimalen Hemmkonzentrationen einiger bisquartärer Bisnaphthalimide gegen Staphylococcus aureus und S. epidermidis im Bereich von 0,6 bis 2,5 µg/ml. Die Substanz mit den geringsten minimalen Hemmkonzentrationen war MT02. Daraufhin wurde das Wirkungsspektrum von MT02 gegen Bakterien detaillierter untersucht und gefunden, dass die Substanz vorwiegend gegen Gram-positive Erreger und nicht gegen Gram-negative Bakterien wirksam ist. Zytotoxizitätstests ergaben eine geringe bis nicht nachweisbare Toxizität gegen verschiedene Zelllinien im Bereich von 73 bis mehr als 150 µg/ml. Um die Wirkungsweise von MT02 genauer zu untersuchen wurden zunächst DNA-Microarray-Untersuchungen an S. aureus durchgeführt. Deren Ergebnisse ließen einen Einfluss der Substanz auf viele Gene des DNA-Metabolismus erkennen. Inkorporationsstudien mittels radioaktiver Ganzzellmarkierung bestätigten die Auswirkung von MT02 auf den DNA-Stoffwechsel. Durch kompetitive Inkubation wurde festgestellt, dass MT02 in der Lage ist Ethidiumbromid von DNA zu verdrängen bzw. dessen Bindung zu verhindern. Genauere Untersuchungen mittels Oberflächen-Plasmon-Resonanz ergaben, dass MT02 konzentrationsabhängig, reversibel und sequenzunspezifisch an DNA bindet. Die thermodynamischen Dissoziationskonstanten lagen im Mittel bei ca. 4 x 10-8 mol/l und beschrieben somit eine relativ starke Bindung von MT02 an DNA. Neben diesem primären Wirkungsmechanismus der DNA-Bindung gaben mehrere Befunde Hinweise auf einen sekundären Wirkmechanismus, der die Zellwand-Struktur bzw. Zellwand-Biosynthese beinhaltet. Eine MT02-resistente Mutante von S. aureus HG001 konnte durch vielfaches Passagieren in MT02-haltigem Medium generiert werden. Diese erzeugte bei Wachstum mit hohen Konzentrationen an MT02 einen roten Phänotyp. Die Natur dieses roten Farbstoffes konnte bislang nicht aufgeklärt werden, jedoch gibt es Hinweise, dass dieser auf Abbauprodukte von MT02 zurückzuführen ist. In einem weiteren Projekt wurde mittels Transkriptionsstudien die Auswirkung von verschiedenen bekannten Antibiotika sowie von neuen Wirkstoffen auf das Transkriptom von S. epidermidis untersucht. Die Ergebnisse dieser Studien können durch vergleichende Analysen als Grundlage für die Einordnung des Wirkmechanismus neuer Substanzen dienen.
Die vorliegende Arbeit, die im Rahmen des SFB 630 „Erkennung, Gewinnung und funktiona-le Analyse von Wirkstoffen gegen Infektionskrankheiten“ erstellt worden ist, beschäftigt sich mit der Entwicklung und Synthese der bisquartären Bisnaphthalimide und deren antimikro-biellen Eigenschaften, speziell gegen Erreger tropischer Infektionskrankheiten, wie Plasmo-dien und Trypanosomen aber auch Bakterien wie Staphylococcus aureus. Erste Testungen einer kleinen Bibliothek verschiedener mono- und bisquartärer Phthal- und Naphthalimide im SFB 630 offenbarten deren antimikrobielles Potenzial. Daher war es das Hauptziel dieser Arbeit, durch systematische Variation der verschiedenen Strukturbestandteile diese Bibliothek zu erweitern. Dazu mussten zuerst die entsprechenden Naphthalin-1,8-dicarbonsäure-Anhydride hergestellt werden. Im nächsten Schritt wurden diese mit einem N,N-Dimethylaminopropylamin-Derivat zum Imid kondensiert und abschließend mit einem Alkyl-Linker zur bisquartären Verbindung alkyliert. So konnte die Bibliothek um 25 Verbin-dungen erweitert werden. Dabei umfassten die Variationen die Alkylkettenlänge zwischen den quartären Stickstoffen mit 3–14 Methylen-Einheiten, das aromatische Substitutionsmuster mit Amino- bzw. Nitrogruppen und symmetrische, wie asymmetrische Bisnaphthalimide. Durch anschließende antimikrobielle Testung und qualitativen Vergleich der ermittelten IC50-Werte konnten verschiedene strukturelle Merkmale der Bisnaphthalimide identifiziert werden, die einen positiven Einfluss auf die Aktivität gegen den untersuchten Mikroorganismus haben.
Staphylococcus aureus is a major threat to public health systems all over the globe. This second most cause of nosocomial infections is able to provoke a wide variety of different types of infection in humans and animals, ranging from superficial skin and skin structure infections to invasive disease like sepsis or pneumonia. But not enough, this pathogen is also notorious in acquiring and/or developing resistance to antimicrobial compounds, thus limiting available treatment options severely. Therefore, development of new compounds and strategies to fight S. aureus is of paramount importance. But since only 1 out of 5 compounds, which entered clinical trials, becomes a drug, the preclinical evaluation of promising compounds has to be reconsidered, too. The aim of this thesis was to address both sides of this problem: first, to improve preclinical testing by incorporating in vivo imaging technologies to the preclinical testing procedure in order to acquire additional and clearer data about efficacy of promising compounds and second, by evaluating lysostaphin, which is a promising, new option to fight S. aureus infections.
The first aim of this thesis focused on the establishment of a dual modality in vivo imaging platform, consisting of Bioluminescence Imaging (BLI) and Magnetic Resonance Imaging (MRI), to offer detailed insights into the course and gravity of S. aureus infection in the murine thigh infection model. Since luciferase-expressing S. aureus strains were generated in former studies and enabled thus bioluminescence imaging of bacterial infection, this technology should be implemented into the compound evaluation platform in order to non-invasively track the bacterial burden over time. MRI, in contrast, was only rarely used in earlier studies to visualize and measure the course of infection or efficacy of anti-bacterial therapy. Thus, the first set of experiments was performed to identify benefits and drawbacks of visualizing S. aureus infections in the mouse model by different MR methods. Native, proton-based MR imaging showed in this regard increased T2 relaxation times in the infected thigh muscles, but it was not possible to define a clear border between infected and uninfected tissue. Iron oxide nanoparticles and perfluorocarbon emulsions, two MR contrast agents or tracer, in contrast, offered this distinction. Iron oxide particles were detected in this regard by their distortion of 1H signal in proton-based MRI, while perfluorocarbon emulsion was identified by 19F MRI. Mammals do not harbor sufficient intrinsic amounts of 19F to deliver specific signal and therefore, 19F MR imaging visualizes only the signal of administered perfluorocarbon emulsion. The in vivo accumulation of perfluorocarbon emulsion can be imaged by 19F MRI and overlayed on a simultaneously acquired 1H MR image, which shows the anatomical context in clear detail. Since this is advantageous compared to contrast agent based MR methods like iron oxide particle-based MRI, further experiments were performed with perfluorocarbon emulsions and 19F MRI.
Experimental studies to elucidate the accumulation of perfluorocarbon emulsion at the site of infection showed robust 19F MR signals after administration between day 2 and at least day 8 p.i.. Perfluorocarbon emulsion accumulated in all investigated mice in the shape of a ‘hollow sphere’ at the rim of the abscess area and the signal remained stable as long as the infection prevailed. In order to identify the mechanism of accumulation, flow cytometry, cell sorting and histology studies were performed. Flow cytometry and cell sorting analysis of immune cells at the site of infection showed that neutrophils, monocytes, macrophages and dendritic cells carried contrast media at the site of infection with neutrophils accounting for the overwhelming portion of perfluorocarbon signal. In general, most of the signal was associated with immune cells, thus indicating specific immune cell dependent accumulation. Histology supported this observation since perfluorocarbon emulsion related fluorescence could only be visualized in close proximity to immune cell nuclei.
After establishing and testing of 19F MRI with perfluorocarbon emulsions as infection imaging modality, the effects of antibiotic therapy upon MR signal was investigated in order to evaluate the capability of this modality for preclinical testing procedure. Thus, the efficacy of vancomycin and linezolid, two clinically highly relevant anti - S. aureus compounds, were tested in the murine thigh infection model. Both of them showed reduction of the colony forming units and bioluminescence signal, but also of perfluorocarbon emulsion accumulation strength and volume at the site of infection, which was visualized and quantified by 19F MRI. The efficacy pattern with linezolid being more efficient in clearing bacterial infection was shown similarly by all three methods. In consequence, 19F MRI with perfluorocarbon emulsion as MR tracer proved to be capable to visualize antibacterial therapy in preclinical testing models.
The next step was consequently to evaluate a promising new compound against S. aureus infections. Thus, lysostaphin, an endo-peptidase that cleaves the cell wall of S. aureus, was tested in different concentrations alone or in combination with oxacillin for efficacy in murine thigh and catheter associated infection models. Lysostaphin only in the concentration of 5 mg/kg body weight or combined with oxacillin in the concentration of 2 mg/kg showed strong reduction of bacterial burden by colony forming unit determination and bioluminescence imaging in both models. The perfluorocarbon accumulation was investigated in the thigh infection model by 19F MRI and was strongly reduced in terms of volume and signal strength in both above-mentioned groups. In general, lysostaphin showed comparable or superior efficacy than vancomycin or oxacillin alone. Therefore, further development of lysostaphin for the treatment of S. aureus infections is recommended by these experiments. Overall, the antibiotic efficacy pattern of all applied antibiotic regimens was similar with all three applied methods, demonstrating the usefulness of MRI for antibiotic efficacy testing. Importantly, treatment with oxacillin either alone or in combination with lysostaphin resulted in stronger perfluorocarbon emulsion accumulation at the site of infection than expected compared to the results from bioluminescence imaging and colony forming unit determination. This might be an indication for immunomodulatory properties of oxacillin.
Further murine infection experiments demonstrated in this context a differential release of cytokine and chemokines in the infected thigh muscle in dependence of the applied antibacterial therapy. Especially treatment with oxacillin, but to a less degree with minocycline or linezolid, too, exhibited high levels of various cytokines and chemokines, although they reduced the bacterial burden efficiently. In consequence, possible immunomodulatory effects of antibacterial compounds have to be taken into account for future applications of imaging platforms relying on the visualization of the immune response. However, this observation opens a new field for these imaging modalities since it might be extraordinary interesting to study the immunomodulatory effects of compounds or even bacterial factors in vivo. And finally, a two modality imaging platform which combines methods to visualize on the one hand the bacterial burden and on the other hand the immune response offers an innovative, new platform to study host-pathogen interaction in vivo in a non-invasive fashion.
In summary, it could be shown that perfluorocarbon emulsions accumulate in immune cells at the site of infection in the murine S. aureus thigh infection model. The accumulation pattern shapes a ‘hollow sphere’ at the rim of the abscess area and its size and perfluorocarbon content is dependent on the severity of disease and/or efficacy of antibiotic therapy. Thus, 19F MRI with perfluorocarbon emulsions is a useful imaging modality to visualize sites and course of infection as well as to evaluate promising antibacterial drug candidates. Furthermore, since the accumulation of tracer depends on immune cells, it might be additionally interesting for studies regarding the immune response to infections, auto-immune diseases or cancer, but also to investigate the efficacy of immunomodulatory compounds and immunization.
EINLEITUNG: Die frühzeitige Erregeridentifikation bei Sepsis-Patienten ist essentiell zur Therapieoptimierung und Senkung der Letalität. Molekularbiologische Detektionsmethoden mit direktem Nachweis bakterieller oder fungaler DNA aus Vollblut stellen einen vielversprechenden Ansatz dar, mit kürzerer Zeitdauer bis zum Resultat und potentiell erhöhter Sensitivität. Beim Vergleich dieser PCR-basierten, kulturunabhängigen Verfahren mit der konventionellen Blutkultur muss streng zwischen antibiotisch vorbehandelten und antibiotisch nicht vorbehandelten Patienten unterschieden werden.
METHODIK: Bei Patienten, die sich von Mai 2010 bis Dezember 2011 mit V.a. Sepsis im Zentrum für Innere Medizin einer Universitätsklinik vorstellten, wurden im Rahmen der IMPACT Sepsis Studie zusätzlich zum routinemäßigen Vorgehen 2 x 5 ml EDTA Blut für die VYOO®-PCR entnommen. In der vorliegenden Arbeit wurden die Erregernachweise der PCR mit den Ergebnissen der Blutkultur für alle antibiotisch nicht vorbehandelten Patienten hinsichtlich Detektionsrate, Time to Result und Plausibilität verglichen. Außerdem wurde die antibiotische Therapie dieser Patienten analysiert und potentielle Therapieoptimierungen durch die PCR-Ergebnisse evaluiert.
ERGEBNISSE: 126 der 200 in die IMPACT Sepsis Studie eingeschlossenen Patienten waren nicht antibiotisch vorbehandelt. Ihr Durchschnittsalter betrug 66,0 ± 16,4 (MW ± SD) Jahre, der Anteil männlicher Patienten 60% und der Anteil immunsupprimierter Patienten 33%. Die durchschnittliche Krankenhaus-Liegedauer lag bei 11,9 ± 10,5 (MW ± SD) Tagen, der Anteil der Patienten mit schwerer Sepsis oder septischem Schock bei 47% und die Letalitätsrate bei 9,7%. Die durchschnittliche Latenzzeit bis zur ersten Antibiotika-Gabe betrug 4,13 ± 6,75 (MW ± SD) h bei einem Median von 2,16 h.
Insgesamt wurden 26 Erreger identifiziert. In 6 Fällen wurde der Erreger von beiden Methoden identifiziert, in 15 nur von der Blutkultur und in 5 nur von der PCR. Die Detektionsraten betrugen 8,7% für die PCR und 16,7% für die Blutkultur (Fisher-Yates-Test; p=0,087; korrigiertes p=1). Die Zeitdauer bis zum Erregerresultat war bei der PCR signifikant kürzer (8,0h bzw. 40,0h; korrigiertes p<0,001). Die PCR versagte vor allem beim Nachweis von Streptokokken, während die Blutkultur mehrere, teilweise gramnegative Problemkeime nicht erfasste. Bei mindestens 4% aller Patienten, 9% der Patienten mit schweren Verlaufsformen und 45% der Patienten mit positivem PCR-Resultat hätte eine Berücksichtigung des PCR-Ergebnisses höchstwahrscheinlich zu einer Therapieoptimierung beigetragen.
SCHLUSSFOLGERUNG: Die beiden untersuchten Verfahren zur Erregerdiagnostik unterschieden sich hinsichtlich der Detektionsrate nicht signifikant, eine diagnostische Überlegenheit der VYOO®-PCR gegenüber der Blutkultur konnte also nicht festgestellt werden. Als komplementäres Verfahren zusätzlich zur Blutkultur bei ausgewählten Patientengruppen eingesetzt, kann durch die PCR eine Verbesserung des therapeutischen Managements von Sepsis-Patienten erzielt werden.
Diese Arbeit ist eine epidemiologische, deskriptive Beobachtungsstudie mit retrospektiver Datenerhebung. Sie stellt das bakterielle Erreger- und Resistenzspektrum von pädiatrischen Patienten mit Blutkultur-positiver Sepsis in Innsbruck dar.
Dazu wurden 797 positive Blutkulturen von pädiatrischen Patienten des Departments für Pädiatrie der Medizinischen Universität Innsbruck, bei denen gemäß Patientenakte eine Sepsis diagnostiziert wurde, im Zeitraum von Januar 2000 bis Dezember 2008 an der Sektion für Hygiene und Medizinische Mikrobiologie der Medizinischen Universität Innsbruck bezüglich der Erregerverteilung und des Resistenzspektrums untersucht. In einem weiteren Schritt wurde im Rahmen einer statistischen Auswertung überprüft, inwieweit patientenbezogene Daten (Geschlecht, Alter, Begleiterkrankung) sowie der Infektionsort (nosokomial oder ambulant erworben), die Entnahmestelle der Blutkultur und der Zeitraum das Erreger- und Resistenzspektrum beeinflussen.
Assay and impurity profiling of the pharmaceuticals are the key routine quality control methods employed worldwide for which High Performance Liquid Chromatography (HPLC) is the most widely used technique. The ability to carry out these routine laboratory procedures in low- and middle- income countries (LMICs) need the methods to be based upon simple instruments manageable with moderate levels of personnel skill and costs involved.
Simple, convenient, and cost effective reverse phase HPLC methods were developed using phosphate buffer and methanol as mobile phase with C18 column as stationary phase for the impurity profiling and assay of beta lactam antibiotics. Isocratic elution and UV detection was employed in these methods. Impurity profiling method was developed for coamoxiclav tablets and ceftriaxone bulk drug. The method for ceftriaxone included a supplementary method to quantify one of its known impurity (Impurity D of ceftriaxone). This method involved use of acetonitrile where as the two main methods were achieved on the targeted method design, described above. With the exception of impurity A of ceftriaxone, the methods developed can successfully quantify impurities to the concentration as low as ≤0.05%, which is in accordance with the current guidelines for the impurity profiling of antibiotics issued by European Medicines Agency.
As ensuring cost reduction was one of the key objectives of carrying out the method development exercise, in situ methods for the preparation of impurities were also identified and some new methods were introduced. The stability of beta lactam antibiotics and the choice of solvent were given due attention during the process of method development revealing information on the presence of new impurities. Deacetyl cefotaxime and 2-mercaptobenzathiazole were identified in this process as new impurities of ceftriaxone currently not listed under known impurities by United States Pharmacopoeia and European Pharmacopoeia. However, deacetyl cefotaxime is a known impurity of cefotaxime whereas the latter molecule is a degradation product of one of the synthesis impurities of ceftriaxone. This substance is reported to be carcinogenic and is resolved using the supplementary method developed for ceftriaxone, hence making its detection and quantification possible. A known inactive impurity of ceftriaxone (Impurity A, E-isomer of ceftriaxone) was` also shown to be produced by exposure to day light, thus warranting the light protection of the ceftriaxone solution, an information that is of critical importance in the clinical settings.
A series of experimentation was carried out on the finished products of beta lactam antibiotics sampled from Pakistan and few other countries, to identify key quality issues in the samples. Though the limited sample size and convenient sampling did not provide results that could yield a decisive figure for the country status for prevalence of substandard and falsified medical products, but the experiments have clearly indicated that the problems in drug quality do exist and beta lactam antibiotics form a class of high-risk medicine with respect to surveillance for poor-quality medicines. Isolation of unknown impurities was also carried out along with the introduction of new and modified methods for preparation of impurities of beta-lactam antibiotics.
In addition, detailed literature survey was carried out for understanding the complex problem of the poor-quality medicine, impact of poor quality antimicrobials on health care system and the magnitude of the problem at the global level. The country status of Pakistan regarding quality of medicines was recorded based upon the available documentary evidence. The current technologies and strategic options available for low- and middle-income countries in aiding fight for combating poor quality medicines was also laid down to design recommendations for Pakistan. A comprehensive review of the information technology tools used for identification and control of substandard and falsified medicines was also conducted.
