Background
There is growing evidence that endothelial failure and subsequent blood brain barrier (BBB) breakdown initiate cerebral small vessel disease (CSVD) pathology. In spontaneously hypertensive stroke-prone rats (SHRSP) endothelial damage is indicated by intraluminal accumulations of erythrocytes (erythrocyte thrombi) that are not observed with current magnetic resonance imaging techniques. Two-photon microscopy (2 PM) offers the potential for real-time direct detection of the small vasculature. Thus, within this pilot study we investigated the sensitivity of 2 PM to detect erythrocyte thrombi expressing initiating CSVD phenomena in vivo.
Methods
Eight SHRSP and 13 Wistar controls were used for in vivo imaging and subsequent histology with haematoxylin-eosin (HE). For 2 PM, cerebral blood vessels were labeled by fluorescent Dextran (70 kDa) applied intraorbitally. The correlation between vascular erythrocyte thrombi observed by 2 PM and HE-staining was assessed. Artificial surgical damage and parenchymal Dextran distribution were analyzed postmortem.
Results
Dextran was distributed within the small vessel walls and co-localized with IgG.
Artificial surgical damage was comparable between SHRSP and Wistar controls and mainly affected the small vasculature. In fewer than 20% of animals there was correlation between erythrocyte thrombi as observed with 2 PM and histologically with HE.
Conclusions
Contrary to our initial expectations, there was little agreement between intravital 2 PM imaging and histology for the detection of erythrocyte thrombi. Two-photon microscopy is a valuable technique that complements but does not replace the value of conventional histology.
Traumatic brain injury (TBI) induces a strong inflammatory response which includes blood-brain barrier damage, edema formation and infiltration of different immune cell subsets. More recently, microvascular thrombosis has been identified as another pathophysiological feature of TBI. The contact-kinin system represents an interface between inflammatory and thrombotic circuits and is activated in different neurological diseases. C1-Inhibitor counteracts activation of the contact-kinin system at multiple levels. We investigated the therapeutic potential of C1-Inhibitor in a model of TBI. Male and female C57BL/6 mice were subjected to cortical cryolesion and treated with C1-Inhibitor after 1 h. Lesion volumes were assessed between day 1 and day 5 and blood-brain barrier damage, thrombus formation as well as the local inflammatory response were determined post TBI. Treatment of male mice with 15.0 IU C1-Inhibitor, but not 7.5 IU, 1 h after cryolesion reduced lesion volumes by ~75% on day 1. This protective effect was preserved in female mice and at later stages of trauma. Mechanistically, C1-Inhibitor stabilized the blood-brain barrier and decreased the invasion of immune cells into the brain parenchyma. Moreover, C1-Inhibitor had strong antithrombotic effects. C1-Inhibitor represents a multifaceted anti-inflammatory and antithrombotic compound that prevents traumatic neurodegeneration in clinically meaningful settings.
Background: Depression and anxiety are common and independent outcome predictors in patients with chronic heart failure (CHF). However, it is unclear whether CHF causes depression. Thus, we investigated whether mice develop anxiety- and depression-like behavior after induction of ischemic CHF by myocardial infarction (MI).
Methods and Results: In order to assess depression-like behavior, anhedonia was investigated by repeatedly testing sucrose preference for 8 weeks after coronary artery ligation or sham operation. Mice with large MI and increased left ventricular dimensions on echocardiography (termed CHF mice) showed reduced preference for sucrose, indicating depression-like behavior. 6 weeks after MI, mice were tested for exploratory activity, anxiety-like behavior and cognitive function using the elevated plus maze (EPM), light-dark box (LDB), open field (OF), and object recognition (OR) tests. In the EPM and OF, CHF mice exhibited diminished exploratory behavior and motivation despite similar movement capability. In the OR, CHF mice had reduced preference for novelty and impaired short-term memory. On histology, CHF mice had unaltered overall cerebral morphology. However, analysis of gene expression by RNA-sequencing in prefrontal cortical, hippocampal, and left ventricular tissue revealed changes in genes related to inflammation and cofactors of neuronal signal transduction in CHF mice, with Nr4a1 being dysregulated both in prefrontal cortex and myocardium after MI.
Conclusions: After induction of ischemic CHF, mice exhibited anhedonic behavior, decreased exploratory activity and interest in novelty, and cognitive impairment. Thus, ischemic CHF leads to distinct behavioral changes in mice analogous to symptoms observed in humans with CHF and comorbid depression.
Traumatic brain injury (TBI) is a major cause of mortality and morbidity worldwide. Despite improvements in acute intensive care, there are currently no specific therapies to ameliorate the effects of TBI. Successful therapeutic strategies for TBI should target multiple pathophysiologic mechanisms that occur at different stages of brain injury. The kallikrein-kinin system is a promising therapeutic target for TBI as it mediates key pathologic events of traumatic brain damage, such as edema formation, inflammation, and thrombosis. Selective and specific kinin receptor antagonists and inhibitors of plasma kallikrein and coagulation factor XII have been developed, and have already shown therapeutic efficacy in animal models of stroke and TBI. However, conflicting preclinical evaluation, as well as limited and inconclusive data from clinical trials in TBI, suggests that caution should be taken before transferring observations made in animals to humans. This review summarizes current evidence on the pathologic significance of the kallikrein-kinin system during TBI in animal models and, where available, the experimental findings are compared with human data.
Background
Glioblastoma multiforme located in the posterior fossa is extremely rare with a frequency up to 3.4%. Compared with glioblastoma of the hemispheres the prognosis of infratentorial glioblastoma seems to be slightly better. Absence of brainstem invasion and low expression rates of epidermal growth factor receptor are described as factors for long-time survival due to the higher radiosensitivity of these tumors.
Case presentation
In this case study, we report a German female patient with an exophytic glioblastoma multiforme arising from the cerebellar tonsil and a secondary spinal manifestation. Furthermore, the tumor showed no O (6)-Methylguanine-DNA methyltransferase promotor-hypermethylation and no isocitrate dehydrogenase 1 mutations. All these signs are accompanied by significantly shorter median overall survival. A long-term tumor control of the spinal metastases was achieved by a combined temozolomide/bevacizumab and irradiation therapy, as part of a standard care administered by the treating physician team.
Conclusion
To our knowledge this is the first published case of a combined cerebellar exophytic glioblastoma with a subsequent solid spinal manifestation. Furthermore this case demonstrates a benefit undergoing this special adjuvant therapy regime in terms of overall survival. Due to the limited overall prognosis of the disease, spinal manifestations of glioma are rarely clinically relevant. The results of our instructive case, however, with a positive effect on both life quality and survival warrant treating future patients in the frame of a prospective clinical study.
Background
The human endogenous retrovirus K (HERV-K) has been acquired by the genome of human ancestors million years ago. It is the most complete of the HERVs with transcriptionally active gag, pol and env genes. Splice variants of env, which are rec, 1.5 kb transcript and Np9 have been suggested to be tumorigenic. Transcripts of HERV-K have been detected in a multitude of human cancers. However, no such reports are available concerning glioblastomas (GBM), the most common malignant brain tumor in adults. Patients have a limited prognosis of 14.6 months in median, despite standard treatment. Therefore, we elucidated whether HERV-K transcripts could be detected in these tumors and serve as new molecular target for treatment.
Findings
We analyzed human GBM cell lines, tissue samples from patients and primary cell cultures of different passages for HERV-K full length mRNA and env, rec and 1.5 kb transcripts. While the GBM cell lines U138, U251, U343 and GaMG displayed weak and U87 strong expression of the full length HERV-K, the splice products could not be detected, despite a weak expression of env mRNA in U87 cells. Very few tissue samples from patients showed weak expression of env mRNA, but none of the rec or 1.5 kb transcripts. Primary cells expressed the 1.5 kb transcript weakly in early passages, but lost HERV-K expression with extended culture time.
Conclusions
These data suggest that HERV-K splice products do not play a role in human malignant gliomas and therefore, are not suitable as targets for new therapy regimen.
Neurophysiologische Veränderungen der Hörbahn während und nach Vestibularisschwannom-Operationen

Aufgrund von Fallberichten bei Patienten mit operativer Resektion eines Vestibularisschwannoms über verzögerte Ertaubung bei endoperativ erhaltenem Resthörvermögen, war es das Ziel dieser Arbeit die intraoperative und frühe postoperative Phase mittels ABR-Monitoring näher zu charakterisieren und prädiktive Faktoren für die postoperative Entwicklung und die resultierende Hörfunktion zu untersuchen.
Dabei zeigte sich, dass der ABR-Befund sowohl zu Beginn der Operation als auch postoperativ mit dem jeweiligen Hörbefund korreliert. Fälle mit erhaltenem ABR und sekundärer Ertaubung müssen am ehesten auf das Vorhandensein weniger noch leitender Fasern ohne funktionellen Nutzen oder eine Veränderung der Elektrophysiologie zurückgeführt werden.
Die detaillierte Betrachtung des intraoperativen Verlaufes ergab eine Untergliederung in sechs Gruppen:
- Gruppe 1: ABR-Verbesserung
- Gruppe 2: ABR kontinuierlich erhalten
- Gruppe 3: ABR temporär abweichend, letztlich erhalten
- Gruppe 4: ABR verschlechtert im Vergleich zum Ausgangsniveau
- Gruppe 5: ABR-Verlust intraoperativ bei fluktuierendem Verlauf
- Gruppe 6: klarer intraoperativer ABR-Verlust,
welche eine sinnvolle Korrelation mit dem Hörergebnis und der postoperativen Entwicklung zeigten. Im Gegensatz zur bisher verbreiteten Auffassung, dass die endoperative ABR-Qualität einen bleibenden Zustand nach den intraoperativen Veränderungen darstellt, wurde in dieser Studie erstmals nachgewiesen, dass in den ersten Stunden und mit weiteren Fluktuationen in der ersten Woche erhebliche Veränderungen auftreten mit erheblichen Auswirkungen auf das Endergebnis. Darüber hinaus konnten verschiedene Typen dieser veränderlichen physiologischen Funktionszustände identifiziert und hier erstmals beschrieben werden:
- Gruppe mit stabilem und erhaltenem ABR
- Gruppe mit fluktuierendem und erhaltenem ABR
- Gruppe mit fluktuierendem und verlorenem ABR
- Gruppe mit stabilem ABR-Verlust.
In der Analyse und Korrelation der intra- mit den postoperativen Entwicklungen wurde die ABR-Qualität nach 60% Tumorresektion als prognostisch signifikantes Merkmal erkannt. Ferner konnte nachgewiesen werden, dass Patienten mit postoperativem Erholungspotential als typisches Merkmal Tumore der Klasse T3A oder kleiner nach der Hannover Klassifikation zeigen.
Diese Studie deutet darauf hin, dass sich der intraoperative Verlauf postoperativ fortsetzt. Während bei intraoperativ instabilem Verlauf (Gruppen 3-5) sich postoperativ eine fluktuierende Entwicklung anschließt oder die ABR-Qualität sich sogar dauerhaft im Vergleich zum endoperativen Status ändert, behalten Patienten mit intraoperativ stabilem Verlauf (Gruppen 1 und 2) oder mit verlorenem ABR (Gruppe 6) diesen Status auch in der frühen postoperativen Phase in der Regel bei.
Im Hinblick auf mögliche Empfehlungen zur mikrochirurgischen Strategie stellten sich die Phase der knöchernen Eröffnung des IAC sowie die direkte Präparation der Tumor-Nerven-Grenze als besonders gefahrenträchtig heraus. Unter Kenntnis der zwar gegebenen aber begrenzten postoperativen Erholungschancen verschlechterter ABRs muss das chirurgische Vorgehen also in dieser Phase auf schonendste Präparation und rasche Reaktion auf kritisches Monitoring-Feedback ausgerichtet sein.
Die Ergebnisse dieser Studie legen nahe bei Patienten mit auffälligen intraoperativen ABR-Fluktuationen (intraoperative Gruppen 3, 4 und 5) postoperativ ein ABR-Monitoring bis zum 5. Tag nach Duraschluss durchzuführen. Auf der Grundlage der hier vorgelegten Studie ist ein postoperatives ABR-Monitoring gleichermaßen für Patienten mit erhaltenem oder erloschenem ABR jeweils nach Fluktuationen zu empfehlen, da sich intraoperative Fluktuationen fortsetzen und postoperativ sowohl ein ABR-Verlust als auch eine ABR-Erholung erfolgen kann. Dies ist besonders indiziert, wenn in Zukunft die medikamentösen Interventionsmöglichkeiten näher evaluiert und weiterentwickelt sind, aber auch mit den aktuellen Standardmedikamenten HAES, Nimodipin und Kortison ist eine Unterstützung der Regeneration bei Feststellung einer postoperativen Verschlechterung zu empfehlen.
Der M. Fabry ist eine X-chromosomal rezessiv vererbte lysosomale Lipid-speicherkrankheit, die sich am peripheren Nervensystem meist mit einer small fiber Neuropathie manifestiert, die mit typischen Fabry-assoziierten neuropathischen Schmerzen einhergeht. Diese können schon in der frühen Kindheit auftreten und sind ein wichtiges Erstsymptom der Erkrankung.
In dieser retrospektiven Analyse wurde eine detaillierte Charakterisierung von Schmerz und Schmerzmedikation in einer monozentrischen Fabry-Kohorte von 132 erwachsenen Patienten und sieben Kindern vorgenommen. Unsere Ergebnisse zeigen, dass Schmerz sowohl bei Männern als auch bei Frauen ein sehr häufiges Erstsymptoms ist, das bereits in früher Kindheit einsetzt. Fabry-assoziierte Schmerzen sind meist episodisch und manifestieren sich mit vier Schmerzformen: evozierte Schmerzen, Schmerzattacken, chronische Schmerzen und Schmerzkrisen. Die Schmerzen sind typischerweise durch körperliche Belastung, Wärme und Fieber auslösbar und sind meist an den Akren lokalisiert. Der Schmerzcharakter ist in der Regel brennend und die episodischen Schmerzen halten mehrere Stunden an. Diese Schmerzcharakteristika decken sich überwiegend mit den Ergebnissen bei Kindern mit M. Fabry. Die ERT scheint nur in seltenen Fällen analgetisch wirksam zu sein, während die meiste Linderung bei akut einsetzenden Schmerzen durch nicht-steroidale Antirheumatika bzw. Antikonvulsiva der älteren Generation erreicht wird. Aufgrund der Besonderheiten von Fabry-assoziierten Schmerzen sind die verfügbaren validierten Schmerzfragebögen für ihre Erfassung ungeeignet. Zur Verbesserung der Datenerfassung wäre in künftigen Studien ein spezifischer Schmerzfragebogen nützlich. Des Weiteren könnte durch den Einsatz von Schmerztagebüchern die Charakterisierung von Fabry-assoziierten Schmerzen ebenfalls verbessert werden.
