Die Rezeptsammlung "Freywillig auffgesprungener Granat-Apffel des Christlichen Samaritans" wurde erstmals 1695 in Wien publiziert. Verfasserin dieses in über zwanzig Auflagen erschienenen Werkes ist die Fürstin Eleonora Maria Rosalia von Eggenberg (1647–1703), geborene Fürstin von Liechtenstein und seinerzeitige Herzogin zu Troppau und Jaegerndorff. Mit der vorliegenden Arbeit wurde das medizinische Werk der Eleonora von Eggenberg systematisch erfasst und mit modernem Wissen verglichen.
Background
Increasing bacterial resistance to antibiotics is a serious problem worldwide. We sought to record the acquisition of antibiotic-resistant Escherichia coli (E. coli) in healthy infants in Northern Thailand and investigated potential determinants.
Methods
Stool samples from 142 infants after birth, at ages 2wk, 2mo, 4 to 6mo, and 1y, and parent stool samples were screened for E. coli resistance to tetracycline, ampicillin, co-trimoxazole, and cefazoline by culture, and isolates were further investigated for multiresistance by disc diffusion method. Pulsed-field gel electrophoresis was performed to identify persistent and transmitted strains. Genetic comparison of resistant and transmitted strains was done by multilocus sequence typing (MLST) and strains were further investigated for extra- and intra-intestinal virulence factors by multiplex PCR.
Results
Forty-seven (33%) neonatal meconium samples contained resistant E. coli. Prevalence increased continuously: After 1y, resistance proportion (tetracycline 80%, ampicillin 72%, co-trimoxazole 66%, cefazoline 35%) almost matched those in parents. In 8 infants (6%), identical E. coli strains were found in at least 3 sampling time points (suggesting persistence). Transmission of resistant E. coli from parents to child was observed in only 8 families. MLST showed high diversity. We could not identify any virulence genes or factors associated with persistence, or transmission of resistant E. coli. Full-term, vaginal birth and birth in rural hospital were identified as risk factors for early childhood colonization with resistant E. coli.
Conclusion
One third of healthy Thai neonates harboured antibiotic-resistant E. coli in meconium. The proportion of resistant E. coli increased during the first year of life almost reaching the value in adults. We hypothesize that enhancement of infection control measures and cautious use of antibiotics may help to control further increase of resistance.
WUEMoCA — научный инструмент веб-кар¬тографирования для мониторинга эф¬фек¬тивности земле- и водопользования на территориях орошаемого земледелия стран трансграничного бассейна Араль¬ского моря (Казахстана, Кыргызстана, Таджикистана, Туркменистана, Узбеки¬стана и Афганистана). Путём интеграции спутниковых данных по землепользованию, растениеводству и потреблению воды с гидрологическими и экономическими данными создаётся целый набор показателей. Инструмент полезен для выработки масштабных решений в вопросах распределения воды и землепользования, а также может применяться во многих практических сферах, в которых требуются независимые данные о конкретных обширных территориях.
In der vorliegenden Studie wurden Häufigkeit, Charakteristika und Einflussfaktoren von Überleben, funktioneller Kapazität, Atempumpenstörung und pulmonaler Hypertonie bei Sarkoidose analysiert. Insgesamt wurden die Daten von 182 mitteleuropäischen Patienten mit gesicherter Sarkoidose retrospektiv erfasst und der Überlebensstatus erhoben. Von 127 Patienten lagen die kompletten Ergebnisse der Diagnostik in der Missionsärztlichen Klinik Würzburg vor. Mit den Daten aller dieser Patienten wurden Überlebensanalysen, Prädiktionsanalysen und Korrelationsanalysen durchgeführt. Damit war die Studie eine der umfangreichsten, die sich mit mitteleuropäischen Sarkoidosepatienten und ihrem Krankheitsverlauf retrospektiv auseinandersetzte.
Von allen 179 im Follow-up erfassten Patienten sind 19 (11 männlich) im mittleren Alter von 68 Jahren verstorben (10%). Eine pulmonale Hypertonie wurde in 6 Fällen mittels Rechtsherzkatheter wenige Monate bis 3 Jahre vor dem Tod diagnostiziert und 7 verstorbene Patienten erfüllten die echokardiographischen Kriterien einer pulmonalen Hypertonie. Die Prävalenz der SAPH war insgesamt 10% (12 Patienten mit mPAP 39±10 mmHg), der echokardiographische Verdacht auf pulmonale Hypertonie bestand bei 22 (PASP > 36 mmHg) bzw. 15 (TRVmax > 2,8 m/s) Patienten. Funktionell relevant waren Rechtsherzfunktion (RA, TAPSE, NT-proBNP), Hämodynamik (PASP, TRVmax, mPAP), Ventilation (FEV1, VC) und Transferfaktor (r bzw. ß > 0,4 bzw. < -0,4, p < 0.05). Mortalitätsprädiktoren waren u.a. eine Gehstrecke < 338 m und eine V’O2peak ≤ 63,5 %/Soll (AUC > 0,8, p < 0.05). Bei 13 von 17 mit Mundverschlussdruckmessung untersuchten Patienten wurde eine Atempumpenstörung diagnostiziert. 7 der 13 Patienten hatten eine invasiv-diagnostizierte SAPH. Die 13 Patienten zeigten eine erhöhte Atempumpenlast (P0.1 0,56[0,38-0,66] kPa) und 12 der 13 Patienten auch eine erhöhte respiratorische Kapazität (P0.1/PImax 12,0[7,5-19,5]%) und Atemfrequenz (BF 23[19-24]/min). Die Atemmuskelkraft war nur bei einer Patientin pathologisch gemäß den Grenzwerten von Koch et al. Die Atempumpenlast bzw. die respiratorische Kapazität hing stark vom pulmonalarteriellen Druck ab (mPAP, PVR, PASP, TRVmax korrelierten sehr gut mit P0.1 bzw. P0.1/PImax). SAPH-Patienten und Patienten ohne pulmonale Hypertonie zeigten signifikante Unterschiede in Bezug auf Atemmuskelkraft und Atempumpenlast. Die echokardiographischen Lungenhochdruck-Parameter hatten starke prädiktive Eigenschaften (100% Sensitivität, Spezifität) bezüglich der Entwicklung einer Atempumpenstörung (PASP ≥ 40 mmHg, TRVmax ≥ 2,9 m/s). Innerhalb des Lungenhochdruck-Kollektivs war der Zusammenhang zwischen Atempumpenlast und Parametern der pulmonalen Hypertonie und der Einfluss auf funktionelle Kapazität und Ventilation zudem wesentlich stärker (r > 0,8 bzw. < -0,8, p < 0.05).
Die Prävalenz der SAPH ist hoch, das Auftreten mit eingeschränktem Überleben und funktioneller sowie Atempumpeneinschränkung assoziiert. Der Verlauf der Sarkoidose wird von dem Auftreten einer pulmonale Hypertonie, einer Atemlasterhöhung und einer Einschränkung der funktionellen Kapazität beeinflusst. Die bisher gefundenen Daten sollten an größeren Populationen bestätigt oder korrigiert werden. Sarkoidose-Mortalitäts- sowie SAPH-Prädiktoren könnten im klinischen Alltag in Form von Therapiekontroll- und Screeningparametern von Nutzen sein und das Management der Sarkoidosepatienten erleichtern.
Die vorliegende Arbeit erörtert rechtliche Besonderheiten des Abrechnungsbetrugs im kassenärztlichen Vergütungssystem der gesetzlichen Krankenversicherung. Das Wirtschaftsstrafrecht, in dem der thematische Schwerpunkt der Arbeit liegt, verknüpft Fragestellungen der klassischen Strafrechtsdogmatik mit solchen von gesellschaftspolitischer Relevanz, wie es beispielsweise bei der Begehung von Delikten im Gesundheitswesen der Fall ist.
Zu Beginn der Arbeit wird das kassenärztliche Abrechnungssystem anhand des Fünften Buches Sozialgesetzbuch (SGB V) erörtert, um sozialrechtliche Besonderheiten der strafrechtlichen Beurteilung eines Abrechnungsbetrugs im Rahmen des § 263 StGB rechtlich zu würdigen. Dogmatische Probleme im Betrugstatbestand werden anhand der typischen Fallgruppen täterschaftlicher Begehung durch Vertragsärzte im ambulanten Sektor beleuchtet. Schwerpunktmäßig setzt sich die Arbeit kritisch mit der Begründung eines Vermögensschadens auseinander, insbesondere wird die von der Rechtsprechung vertretene streng formale Betrachtungsweise diskutiert.
Während des Zweiten Weltkrieges waren immer mehr Männer im Kriegseinsatz und Frauen blieben in der Heimat zurück. Dieses führte dazu, dass Frauen immer wichtiger für die Arbeitswelt wurden. So wurde es für die Gesellschaft immer selbstverständli-cher, dass Frauen studierten und auch Berufe übernahmen, welche vorher vorwiegend Männern zugeschrieben wurden. Gerade im Studienfach Medizin wurde zu Beginn der Zeit des Nationalsozialismus versucht, Frauen den Zugang zu erschweren. Als jedoch durch den Krieg ein immer größerer Mangel an Ärzten entstand und immer weniger Männer ein Studium beginnen konnten, wurden diese Zugangsbeschränkungen wieder gelockert. So stieg die Zahl der Studentinnen an den Universitäten immer weiter an und es wurden im Verlauf immer mehr Frauen als Ärztinnen tätig. In der Nachkriegszeit und den folgenden 1950er Jahren wurde durch die Politik und die Kirchen in Westdeutsch-land versucht, diese Entwicklung aufzuhalten. So wurde die Normalfamilie propagiert und die Frau wieder in ihrer Rolle als Hausfrau und Mutter gesehen. In Westdeutsch-land fiel durch diese Tendenzen der Anteil der Medizinstudentinnen und Studentinnen in anderen Studienfächern wieder ab. In der Sowjetischen Besatzungszone und in der im Verlauf entstehenden Deutschen Demokratischen Republik wurden schon mit Be-ginn der 1950er Jahre vermehrt Frauenkarrieren gefördert, hier konnte ein Anstieg der Zahlen der Studentinnen an den Universitäten verzeichnet werden und schließlich auch im Arztberuf. Auch wenn in der DDR vermehrt Einrichtungen zur Unterstützung der berufstätigen Frau errichtet wurden, wie etwa Kindergärten, war es trotz der Förderung durch den Staat für Frauen nicht einfach, Beruf und Karriere zu vereinen, auch wenn es in der DDR für Frauen weitaus einfacher war als in Westdeutschland. Dort war dies ohne Hilfe durch die Familie oder Kindermädchen noch weniger möglich. Um über-haupt zum Medizinstudium zugelassen zu werden, war eine Hochschulzugangsberechti-gung nötig, welche auf dem Gymnasium erworben werden musste. In dieser Zeit war es für Frauen durchaus unüblich, ein Gymnasium zu besuchen und Medizin zu studieren, so zumindest war die Annahme im Vorfeld dieser Arbeit.
Stehen die Biographien der Ärztinnen in einem Verhältnis zu den zeithistorischen Ent-wicklungen, wie ist ihre subjektive Wahrnehmung zu dieser Zeit, wie haben sie es emp-funden Ärztin und Frau zu sein und welchen Beeinträchtigungen, das Medizinstudium zu bestreiten und den Arztberuf auszuüben, waren sie ausgesetzt. Um diese Fragen be-antworten zu können wurde die Geschlechterordnung in der Nachkriegszeit und den 1950er Jahre in West- und Ostdeutschland beleuchtet.

In einer Studie wurde der Zusammenhang pathologischer Werte von C-Reaktivem Protein (295 Fälle)- - sowie Leukozyten-Anzahlen (292 Fälle) einerseits und dosiskorrigierter Serumkonzentration der Antipsychotika Haloperidol, Risperidon, Olanzapin, Quetiapin und Aripiprazol andererseits mittels therapeutischem Drug Monitoring bestimmter Serumkonzentrationen sowie pathologische CRP (295 Fälle)- und Leukozyten (292 Fälle)-Werte ausgewertet.
Zielsetzung der vorliegenden Arbeit war es, den Einfluss von Entzündung, gemessen durch CRP-Wert und Leukozyten, auf die dosiskorrigierten Antipsychotika-Serumkonzentrationen zu untersuchen.
In der Quetiapin-Stichprobe konnte ein signifikanter Zusammenhang von pathologischen CRP-Werten und der dosiskorrigierten Serumkonzentration berechnet werden. In der Olanzapin-Stichprobe ergab sich ein trendmäßiger Zusammenhang von pathologischen CRP-Werten und der dosiskorrigierten Serumkonzentration. Dosiskorrigierte Serumkonzentrationen über der therapeutischen Obergrenze waren in der Quetiapin-Stichprobe mit pathologischen CRP-Werten assoziiert.
In keiner Stichprobe konnte ein signifikanter Zusammenhang von Leukozyten-Anzahl und dosiskorrigierter Serumkonzentration aufgezeigt werden.
Wir konnten damit erstmals einen signifikanten Zusammenhang von Entzündung und Serumkonzentration für Quetiapin (und partiell auch Olanzapin) zeigen. Klinische Konsequenz sollte - vor allem bei älteren Patienten - eine Quetiapin (und auch Olanzapin-)-Dosisanpassung unter inflammatorischen Bedingungen sein, um das Risiko verstärkter bzw. sogar toxischer Nebenwirkungen durch einen Anstieg der Serumkonzentration zu minimieren.
Weitere Studien mit einer größeren Fallzahl für die anderen hier untersuchten und nicht signifikant assoziiert gefundenen Antipsychotika sind erforderlich, um das Risiko erhöhter Serumkonzentrationen unter inflammatorischen Bedingungen für diese ausschließen zu können. Darüber hinaus sind weitere Untersuchungen mit Berücksichtigung anderer auf CYP-Ebene interagierender Faktoren wie Komedikation, körperlichen Begleiterkrankungen, Raucherstatus oder Polymorphismen sinnvoll, um Risikogruppen noch genauer definieren zu können.
Zusammenhang zwischen der Serumkonzentration serotonerger Antidepressiva und der Blutgerinnung

Das Verordnungsvolumen von Antidepressiva in Deutschland hat sich in den letzten zehn Jahren etwa verdoppelt. Gleichzeitig liegen zahlreiche Untersuchungen über erhöhte Blutungstendenzen unter der Therapie mit serotonergen Antidepressiva vor. Die aktuelle Studienlage deutet darauf hin, dass es unter anderem über das serotonerge System zu Beeinflussungen der Thrombozyteneigenschaften und in Folge dessen zu Veränderungen der Blutgerinnung kommen könnte.
Ziel der vorliegenden Arbeit war es, den Zusammenhang zwischen der Serumkonzentration serotonerger Antidepressiva und der Blutgerinnung zu untersuchen. Im Gegensatz zur Dosis bietet die Serumkonzentration exakte Informationen über die tatsächlich wirkende Antidepressivamenge und berücksichtigt neben der Patientenadhärenz die interindividuelle Variabilität der pharmakokinetischen Eigenschaften. Die Beurteilung der Blutgerinnung erfolgte unter Zuhilfenahme von Gerinnungsparametern (Thrombozytenzahl, mittleres Plättchenvolumen, Quick, INR, partielle Thromboplastinzeit). Es wurde die Hypothese aufgestellt, dass mit steigender Serumkonzentration Veränderungen der Blutgerinnung und in Folge dessen auch der Gerinnungsparameter entstehen können. Darüber hinaus sollte untersucht werden unter welchen Antidepressiva potentielle Veränderungen auftreten. Es wurden Antidepressiva unterschiedlicher Wirkungsgruppen analysiert: Amitriptylin, Doxepin, Es‑Citalopram, Mirtazapin und Venlafaxin. Besonders selektive Serotonin-Wiederaufnahmehemmer standen auf Grund der aktuellen Studienlage im Verdacht Einfluss auf die Gerinnung zu nehmen. Um Antidepressiva spezifische Aussagen treffen zu können, war das Vorliegen einer antidepressiven Monotherapie grundlegendes Selektionskriterium. Alle potenziell gerinnungsbeeinflussenden sowie serotonerg wirkenden Arzneimittel wurden ausgeschlossen. Die Daten wurden retrospektiv erhoben und stammten von stationär therapierten Patienten der Klinik für Psychiatrie, Psychosomatik und Psychotherapie am Universitätsklinikum Würzburg.
Die Untersuchungen ergaben für das trizyklische Antidepressivum Amitriptylin signifikante Ergebnisse. Die interindividuelle Analyse zeigte signifikant positive Korrelationen zwischen der partiellen Thromboplastinzeit (PTT) und dem Metabolitenspiegel (Nortriptylin‑Konzentration, rs=0,564; p=0,010, N=20) sowie dem Summenspiegel von Amitriptylin (Amitriptylin- und Nortriptylin‑Konzentration, rs=0,477; p=0,033, N=20). Darüber hinaus stellten sich im Rahmen der intraindividuellen Analyse signifikante Unterschiede zwischen der Thrombozytenzahl unter niedriger und hoher Amitriptylin‑Konzentration dar (Z= ‑2,867; p=0,004, N=45). Ergänzend wurde im Rahmen von explorativen Untersuchungen der Zusammenhang zwischen der verabreichten Dosis und der Serumkonzentration der Antidepressiva analysiert. Die Ergebnisse zeigten Schwankungen um den Faktor 3 bis 11, die im Vergleich zu anderen Studien geringer ausfielen.
Der Verdacht, dass besonders selektive Serotonin-Wiederaufnahmehemmer einen erhöhten Einfluss auf die Gerinnungsparameter haben, wurde in der aktuellen Arbeit nicht bestätigt. Ebenso waren unter Doxepin, Mirtazapin und Venlafaxin keine Zusammenhänge zur Serumkonzentration zu beobachten. Die signifikanten Ergebnisse unter Amitriptylin lassen vermuten, dass nicht nur die Inhibition von Serotonintransportern, wie bei selektiven Serotonin-Wiederaufnahmehemmern, sondern zusätzlich auch die Hemmung von Serotoninrezeptoren, wie dem 5‑HT2A‑Rezeptor, eine Rolle im Hinblick auf Veränderungen von Thrombozyteneigenschaften spielen. Dennoch lagen im Rahmen dieser Untersuchung 98% der Gerinnungsparameter aller analysierten Antidepressiva im Normbereich.
Die Ergebnisse legen die Vermutung nahe, dass das Risiko immer wieder berichteter Blutungskomplikationen unter der Behandlung mit Antidepressiva trotz zunehmender Verordnungszahlen überschaubar scheint. Entsprechend aktueller Publikationen ist vermutlich erst bei zusätzlicher Einnahme von nichtsteroidalen Antirheumatika sowie antikoagulativen Arzneimitteln von einem erhöhten Blutungsrisiko auszugehen. Besonders gastrointestinale Blutungen spielen bei Kombination dieser Medikamente auf Grund der gesteigerten Magensäuresekretion eine Rolle. Ob die Serumkonzentration der Antidepressiva bei entsprechender Komedikation ebenfalls eine Rolle im Hinblick auf Veränderungen der Gerinnungsparameter spielt, sollte im Rahmen weiterführender Längsschnittstudien genauer untersucht werden. Ergänzend wären Untersuchungen zur Klärung des Kausalzusammenhangs wünschenswert, um das Blutungsrisiko im Zusammenhang mit Antidepressiva in Zukunft weiter minimieren zu können.
Zn\(^{2+}\) deficiency in the human population is frequent in underdeveloped countries. Worldwide, approximatively 2 billion people consume Zn\(^{2+}\)-deficient diets, accounting for 1–4% of deaths each year, mainly in infants with a compromised immune system. Depending on the severity of Zn\(^{2+}\) deficiency, clinical symptoms are associated with impaired wound healing, alopecia, diarrhea, poor growth, dysfunction of the immune and nervous system with congenital abnormalities and bleeding disorders. Poor nutritional Zn\(^{2+}\) status in patients with metastatic squamous cell carcinoma or with advanced non-Hodgkin lymphoma, was accompanied by cutaneous bleeding and platelet dysfunction. Forcing Zn\(^{2+}\) uptake in the gut using different nutritional supplementation of Zn\(^{2+}\) could ameliorate many of these pathological symptoms in humans. Feeding adult rodents with a low Zn\(^{2+}\) diet caused poor platelet aggregation and increased bleeding tendency, thereby attracting great scientific interest in investigating the role of Zn\(^{2+}\) in hemostasis. Storage protein metallothionein maintains or releases Zn\(^{2+}\) in the cytoplasm, and the dynamic change of this cytoplasmic Zn\(^{2+}\) pool is regulated by the redox status of the cell. An increase of labile Zn\(^{2+}\) pool can be toxic for the cells, and therefore cytoplasmic Zn\(^{2+}\) levels are tightly regulated by several Zn\(^{2+}\) transporters located on the cell surface and also on the intracellular membrane of Zn\(^{2+}\) storage organelles, such as secretory vesicles, endoplasmic reticulum or Golgi apparatus. Although Zn\(^{2+}\) is a critical cofactor for more than 2000 transcription factors and 300 enzymes, regulating cell differentiation, proliferation, and basic metabolic functions of the cells, the molecular mechanisms of Zn\(^{2+}\) transport and the physiological role of Zn\(^{2+}\) store in megakaryocyte and platelet function remain elusive. In this review, we summarize the contribution of extracellular or intracellular Zn\(^{2+}\) to megakaryocyte and platelet function and discuss the consequences of dysregulated Zn\(^{2+}\) homeostasis in platelet-related diseases by focusing on thrombosis, ischemic stroke and storage pool diseases.
Die Ratifizierung der Behindertenrechtskonvention der Vereinten Nationen (UN-BRK) sowie die Empfehlung der Hochschulrektorenkonferenz (HRK) „Eine Hochschule für Alle“ im Jahre 2009 forderten die deutschen Hochschulen dazu auf, eine gleichberechtigte und diskriminierungsfreie Teilhabe von Studierenden mit studienerschwerender Beeinträch-tigung und chronischer Erkrankung (SmB) an der Hochschulbildung durch angemessene Vorkehrungen und geeignete Maßnahmen zur Herstellung von Barrierefreiheit sicherzustellen. In diesem Kontext wurde 2017 der Forschungs- und Praxisverbund „Inklusion an Hochschulen und barrierefreies Bayern“ von sechs bayerischen Hochschulen gegründet, in dessen Rahmen Einzelinitiativen, in Form von Forschungs- und Praxisprojekten, verfolgt wurden, die zur Entwicklung einer inklusionsorientierten Hochschule beitragen sollen. Die Julius-Maximilians-Universität widmete sich als eines der sechs Verbundmitglieder in einem Teilprojekt der Identifizierung zentraler Gelingensbedingungen inklusiver Hochschulbildung für SmB in Bayern. Hierzu wurden neben einer ausführlichen Analyse der nationalen und internationalen Befundlage ein empirischer Mixed-Methods-Ansatz genutzt. Dieser verknüpfte problemzentrierte qualitative Interviews mit bayerischen Beauftragten und Berater/innen für Studierende mit Behinderung und chronischer Erkrankung (n=13) sowie Lehrenden (n=20) unterschiedlicher Fakultäten zu deren themenspezifischen Qualifikationen und Einstellungen mit einer anschließenden quantitativen Erhebung bayerischer Hochschullehrender (n=807) an fünf Hochschulen, welche mit Hilfe einer dafür validierten deutschen Übersetzung eines etablierten themenspezifischen internationalen Befragungs-instruments durchgeführt wurde. Es ließen sich übergeordnet vier zentrale Gelingens-bedingungen für eine inklusionsorientierte Hochschulbildung identifizieren: (1) Ein adäquater Wissensstand in Bezug auf themenrelevante gesetzliche und beeinträchtigungs-bezogene Aspekte, (2) ein hinreichendes Maß an Sensibilität für und eine positive Einstellung zu SmB, (3) eine nachhaltige Förderung von themen- und adressatenspezifischen Weiterbildungen und (4) Netzwerkförderung. Als konkrete zugehörige Handlungs-empfehlungen lassen sich daraus ableiten: Die Stärkung von Beauftragten- und themen-relevanten Netzwerken, die nachhaltige Weiterbildungsförderung, die Berücksichtigung der besonderen Bedürfnisse von SmB im Rahmen des Qualitätsmanagements der Lehre, die besondere Sensibilisierung aller Akteur/innen für die Studieneingangsphase sowie die Schaffung themenspezifischer Informations- und Anreizsysteme für Professor/innen.
Die deutsche Hochschullandschaft ist spätestens seit 2009 aufgefordert eine gleichberechtigte und diskriminierungsfreie Teilhabe von Studierenden mit studienerschwerender Beeinträchtigung sicherzustellen. Die hiesige Monographie fasst diesbezüglich wesentliche Erkenntnisse aus einer im Rahmen des von 2017 bis 2019 vom bayerischen Landtag geförderten Forschungs- und Praxisverbundes „Inklusion an Hochschulen und barrierefreies Bayern“ durchgeführten Untersuchung an fünf bayerischen Hochschulen zusammen. Dabei werden zunächst zentrale Gelingensbedingungen inklusiver Hochschulbildung anhand eines empirischen Mixed-Methods-Ansatzes mit bayerischen Beauftragten und Berater/innen für Studierende mit Behinderung und chronischer Erkrankung sowie Lehrenden unterschiedlicher Fakultäten herausgearbeitet und mit vorwiegend internationalen themenspezifischen Befunden abgeglichen. Abschließend werden konkrete Handlungsempfehlungen abgeleitet, welche die Gestaltung einer inklusionsorientierten Hochschule zukunftsweisend unterstützen sollen.
Background
Almost 90% of cancer patients suffer from symptoms of fatigue during treatment. Supporting treatments are increasingly used to alleviate the burden of fatigue. This study examines the short-term and long-term effects of yoga on fatigue and the effect of weekly reminder e-mails on exercise frequency and fatigue symptoms.
Methods
The aim of the first part of the study will evaluate the effectiveness of yoga for cancer patients with mixed diagnoses reporting fatigue. We will randomly allocate 128 patients to an intervention group (N = 64) receiving yoga and a wait-list control group (N = 64) receiving yoga 9 weeks later. The yoga therapy will be performed in weekly sessions of 60 min each for 8 weeks. The primary outcome will be self-reported fatigue symptoms. In the second part of the study, the effectiveness of reminder e-mails with regard to the exercise frequency and self-reported fatigue symptoms will be evaluated. A randomized allocated group of the participants (“email”) receives weekly reminder e-mails, the other group does not. Data will be assessed using questionnaires the beginning and after yoga therapy as well as after 6 months.
Discussion
Support of patients suffering from fatigue is an important goal in cancer patients care. If yoga therapy will reduce fatigue, this type of therapy may be introduced into routine practice. If the reminder e-mails prove to be helpful, new offers for patients may also develop from this.
WUEMoCA is an operational scientific webmapping tool for the regional monitoring of land and water use efficiency in the irrigated croplands of the transboundary Aral Sea Basin that is shared by Kazakhstan, Kyrgyzstan, Tajikistan, Turkmenistan, Uzbekistan, and Afghanistan. Satellite data on land use, crop pro-duction and water consumption is integrated with hydrological and economic information to provide of a set indicators. The tool is useful for large-scale decisions on water distribution or land use, and may be seen as demonstrator for numerous applications in practice, that require independent area-wide spatial information.

Gemeinsam mit den Alumni der Universität Würzburg führte die Universitätsbibliothek Würzburg 2018 erfolgreich eine Fundraising-Aktion zur Rettung einzigartiger Handschriften aus den Sammlungen der Universitätsbibliothek durch. Mit der eingeworbenen Spendensumme konnten insgesamt 40 wertvolle Handschriften und Drucke restauriert und digitalisiert werden.
Mutationen und Expressionsänderungen im Bereich der Rezeptortyrosinkinasen (RTK) sind Bestandteil der Pathogenese des oralen Plattenepithelkarzinoms (PEC). Insbesondere die Überexpression des epidermal growth factor receptor (EGFR) ist mit 90 % die am häufigsten auftretende Veränderung, jedoch zeigen auf diese Mutationen zielgerichtete Therapie-Formen nicht die gewünschten Erfolge. Angesichts der sich schnell entwickelnden Platinresistenzen und der schlechten 5-Jahre-Überlebensrate von 55–60 %, scheint die Identifikation innovativer Therapieregime durch Kombinationstherapien mittels Tyrosinkinaseinhibitoren (TKI) ein möglicher Ansatz.
Die vorliegende Untersuchung sollte die Wirksamkeit von Lapatinib, Pazopanib, Trametinib, Dabrafenib und in der Monotherapie sowie in Kombination mit herkömmlichen Zytostatika 5-Fluorouracil (5-FU) und Cisplatin auf das orale PEC untersuchen. Alle Versuche wurden dabei an humanen Plattenepithelkarzinomzelllinien (PC1-1, PC13-1, PC52, SCC9 und SCC68) durchgeführt.
Die einzelnen Zelllinien zeigten differentielle Ansprechraten bezüglich der applizierten Mono- und Kombinationstherapien, sowie unterschiedliche RTK Expressionen. Ein direktes Verhältnis zwischen Wirkpotential der untersuchten TKI und Rezeptorexpression scheint nicht zu bestehen. Die TKI konnten in allen Zelllinien eine Zellreduktion hervorrufen. Besonders sensibel reagierten die Zellen auf EGF-Inhibitoren (Lapatinib und Trametinib). Lapatinib ist mit einer kumulativen IC50 von 15,69 µM der wirkungsstärkste TKI in der Monotherapie, gefolgt von Trametinib (17,71 µM), Pazopanib (108,47 µM) und Dabrafenib (152,18 µM). Eine Überlegenheit von Pazopanib als multi-targeted TKI gegenüber der single-targeted therapy konnte nicht dargestellt werden. Dabrafenib wurde primär aufgrund des targets als Kontrolle in die Arbeit mit einbezogen und zeigte eine erwartete mäßige Wirkung bei allen Tumorzelllinien. Zusammenfassend liegen die ermittelten inhibitorischen Konzentrationen der Monotherapie teilweise massiv über denen der herkömmlichen Chemotherapeutika. Dieser Sachverhalt könnte auf die Aktivität alternativer Signalkaskaden und/oder die wesentliche Heterogenität des Tumors zurückzuführen sein.
Ähnliche Ergebnisse zeigten sich für die Kombinationstherapie mit Cisplatin. Auch hier scheinen die EGFR-Inhibitoren Lapatinib und Trametinib aufgrund einer niedrigen IC50 überlegen zu sein. In der Kombinationstherapie mit Cisplatin zeigt Trametinib sogar mit einer kumulativen IC50 von 18,66 µM ein stärkeres inhibitorisches Potential als Lapatinib (38,45 µM). Jedoch wird bei näherer Betrachtung deutlich, dass Kombinationen mit Cisplatin ausschließlich in den Versuchsreihen mit Dabrafenib und Lapatinib eine Verbesserung bezüglich der max. Zellzahlreduktion ergaben. Ursache könnte unter anderem der Einfluss von ATP-binding cassette (ABC)-Transportern sein, welche Cisplatin als Substrat haben. Das gleiche Bild zeigt sich für die Kombination mit 5-FU, sodass sich an der Reihenfolge der TKI nichts ändert, allerdings Dabrafenib von der Kombination erneut am meisten profitiert. Additive Effekte mit 5-FU konnten für zwei von fünf Zelllinien mit Trametinib und für eine von fünf Zelllinien mit Dabrafenib und Lapatinib festgestellt werden.
Die hohen Erwartungen an die targeted therapy durch die Inhibition verschiedener Tyrosinkinasen im oralen PEC konnte in der vorliegenden Arbeit nicht bestätigt werden. Vielmehr scheinen diese ihre Limitation in der Komplexität des Tumors zu finden, was sich auch in nur leichten additiven Effekten in den untersuchten Kombinationstherapien mit herkömmlichen Chemotherapeutika wiederspiegelt. Weitere Studien scheinen nur wenig sinnvoll. Jedoch zeigen andere Vertreter der zielgerichteten Therapie, die Substanzklassen der second mitochondria-derived activator of caspase (SMAC)-mimetics vielversprechende Ergebnisse in ersten eigenen Zelllinienversuchen, so dass eine Fortführung dieser sinnvoller erscheint.
Die Logik der bisher erforschten und beschriebenen Management- und Führungstheorien müssen sich im Zeitalter der Digitalisierung weiterentwickeln. Die ursprüngliche Forschungsfrage nach einer wirksamen Implementierung von strategischen Entscheidungen passt nicht mehr zur Realität von disruptiven Veränderungen in der sogenannten VUCA Welt (Volatile, uncertain, complex, ambiguous).
Die Arbeit ist mutig und wertvoll, weil sie die Lücke zwischen neuen Entwicklungen in der Praxis und fehlenden umfassenden Theoriekonzepten in den Management-, Führungs- und Organisationswissenschaften offenlegt und zu schließen hilft.
Der erste Teil der Arbeit fasst die aktuellen Erkenntnisse rund um strategische Entscheidungsfindung in Unternehmen, globale Megatrends als Rahmenbedingung und Change-Management als Umsetzungshilfe zusammen. Die Schlussfolgerung aus dieser holistischen Betrachtung ist, dass die Forschungsfrage rückwärts gerichtet die Realität des 20. Jahrhunderts adressiert und für das Zeitalter der Digitalisierung keine hilfreiche Antwort bietet.
Vielmehr geht es um die weiter entwickelte Forschungsfrage, wie anpassungsfähige Organisationen entwickelt und gepflegt werden können. Solche Organisationen überleben disruptive Veränderungen nicht nur irgendwie, sondern sind so gestaltet, dass sie diese nutzen, um immer wieder neue Antworten auf sich entwickelnde Kundenbedürfnisse und in der internen Organisation zu finden.
Diese anpassungsfähige oder adaptive Organisation hat fünf wesentliche Dimensionen, die im zentralen Teil der Arbeit beleuchtet werden. Alle Themen entwickeln sich derzeit laufend weiter, so dass es noch keine letztgültige Antwort gibt welche Methoden sich durchsetzen werden.
Im Sinne eines holistischen Transformationsmanagements gibt das letzte Kapitel Hinweise auf die Herangehensweise, um die eigene Organisation in ihrer Anpassungsfähigkeit weiter zu entwickeln.
Die gründliche Diskussion einer Fülle von konzeptionellen Ansätzen in Verbindung mit einer bemerkenswerten Erfahrung der Autorin erlaubt es, die auftretende Problemstellung profunder anzugehen als bei einer rein akademischen Herangehensweise.
Bis weit ins lange 19. Jahrhundert hinein führten Studenten an deutschsprachigen Universitäten bisweilen eine Art „Poesiealbum“ welches “Stammbuch“ genannt wird. In diesem Buch verewigten sich die Kommilitonen des Besitzers mit einem mehr oder minder launigen Eintrag. Der Jurastudent Wilhelm Schmiedeberg erstellte in den 1830er Jahren ein solches Buch, allerdings mit einer Modifikation: Schmiedeberg selbst porträtierte nämlich seine Studienfreunde in meisterhafter Aquarelltechnik.
Die sogenannte „Rückkehr der Wölfe" stellt viele Menschen in Europa und in Deutschland vor besondere Herausforderungen. Viele Menschen in ländlichen Milieus, die in besonderem Maße von den Effekten dieser Rückkehr betroffen sind, reagieren ablehnend auf die tierlichen Rückkehrer*innen. Dies hängt nicht zuletzt mit traumatischen Erlebnissen zusammen, die manche Tierhaltende durch das Beutegreifverhalten mancher Wölfe auf ihre Tiere erleiden. Der vorliegende Beitrag liest die Rückkehr der Wölfe als Teil der dramatischen Veränderungen der Welt im beginnenden 21. Jahrhundert. Mit diesen Veränderungen verbunden sind im gesamten Globalen Norden tiefgreifende Verhaltens- und Einstellungsänderungen, auch gegenüber anderen als menschlichen Lebewesen. Die Wölfe sind Teil dieses Prozesses. Sie zeigen in besonderem Maße, wie die Fluidität gewohnter Ordnungen und Grenzziehungen, etwa die der Zugehörigkeit zu menschlichen Haushalten oder die Einteilung der Lebensräume in rurale und urbane, wilde und zivilisierte, zur ethischen Herausforderung wird. Obgleich der Prozess der Aushandlung des Zusammenlebens offen und höchst widersprüchlich verläuft, zeigen sich in der Diskussion auch neue Einsichten in die Verbundenheit menschlicher Akteur*innen mit anderen Lebewesen. Damit sind wesentliche Voraussetzungen gegeben für die Entwicklung neuer Haltungen und Wertsetzungen, die dem engen Austausch zwischen Menschen und anderen Lebewesen gerecht werden.
A series of photoactivatable CO‐releasing molecules (PhotoCORMs) was prepared from manganese pentacarbonyl bromide and 1H‐benzimidazol‐2‐ylmethyl‐(N‐phenyl)amine ligands (L) bearing different electron‐donating and electron‐withdrawing groups R = H, 4‐CH\(_3\), 4‐OCH\(_3\), 4‐Cl, 4‐NO\(_2\), 2‐, 3‐, and 4‐COOCH\(_3\) on the phenyl substituent to give octahedral manganese(I) complexes of the general formula [MnBr(CO)\(_3\)(L)]. Aerated DMSO solutions of the compounds are stable in the dark for 16 h with no CO release. However, the compounds rapidly release CO upon illumination at 412–525 nm, depending on the substitution pattern. Its influence on the photophysical and photochemical properties was systematically explored using UV/Vis spectroscopy and CO release measurements with a commercial gas sensor system. In the nitro‐substituted compound, the electronically excited state switched from benzimidazole‐ to phenyl‐centered, leading to a markedly different photochemical behavior of this visible‐light activated PhotoCORM.
Summary
Introduction. Rapid and uncontrolled industrialisation and urbanisation in most developing countries are resulting in land, air and water pollution at rates that the natural environment cannot fully renew. These contemporary environmental issues have attracted local, national and international attention. The problem of urban garbage management is associated with rapid population growth in developing countries. These are pertinent environmental crises of sustainability and sanitation in Sub-Saharan Africa and other Third World countries. Despite efforts of the various tiers of government (the case of Nigeria with three tiers: Federal, State and Local governments) in managing solid waste in urban centres, it is still overflowing open dumpsites, litters streets and encroaches into water bodies. These affect the quality of urban living conditions and the natural environment.
Sub-Saharan and other developing countries are experiencing an upsurge in the accumulation and the diversity of waste including E-waste, waste agricultural biomass and waste plastics. The need for effective, sustainable and efficient management of waste through the application of 3Rs principle (Reduce, Reuse, and Recycle) is an essential element for promoting sustainable patterns of consumption and production. This study examined waste management in Imo State, Nigeria as an aspect correlated to the sustainability of its environment.
Materials and methods. To analyse waste management as a correlate of environmental sustainability in Sub-Saharan Africa, Imo State, in eastern Nigeria was chosen as a study area. Issues about waste handling and its impact on the environment in Imo have been reported since its creation in 1976; passing through the State with the cleanest State capital in 1980 to a ‘dunghill’ in 2013 and a ‘garbage capital’ on October 1, 2016. Within this State, three study sites were selected – Owerri metropolis (the State capital) Orlu and Okigwe towns. At these sites, households, commercial areas, accommodation and recreational establishments and schools, as well as dumpsites were investigated to ascertain the composition, quantity, distribution, handling patterns of waste in relation to the sustainability of the State’s environment. This was done conveniently but randomly through questionnaires, interviews, focus group discussions and non-participant observation; these were all heralded by a detailed deskwork. Data were entered using Microsoft Office Excel and were explored and analysed using the Statistical Package for Social Sciences - SPSS.
Data were made essentially of categorical variables and were analysed using descriptive statistics. The association between categorical variables was measured using Cramer’s V the Chi-Square that makes the power and the reliability of the test. Cramer’s V is a measure of association tests directly integrated with cross-tabulation. The Chi-Square test of equal proportions was used to compare proportions for significant differences at 0.05 levels. The statistical package - the Epi Info 6.04d was also used since a contingency table had to be created from several sub-outputs and determine the extent of association between the row and column categories.
The scale variable ‘quantity of waste generated’ was described using measures of central tendency. It was screened for normality using the Kolmogorov-Smirnov and Shapiro-Wilk tests for normality; in all context, the normality assumption was violated (P<0.05). Five null hypotheses were tested using Logistic Regression model. The explanatory power of individual conceptual component was calculated using the Cox & Snell R2 and that of individual indicators was also appraised using the Likelihood Ratio test.
In the context of this work, the significance of the variability explained by the model (baseline model) was appraised using the Omnibus Tests of Model Coefficients, the magnitude of this variability explained by the model using the Cox & Snell R2 and the effects of individual predictors using the Likelihood Ratio test.
Qualitatively, data from open-ended items, observations and interviews were analysed using the process of thematic analysis whereby concepts or ideas were grouped under umbrella terms or keywords. The results were presented using tables, charts, graphs, photos and maps.
Findings and discussions. The total findings and analyses indicated that proper waste handling in Imo State, Nigeria has a positive impact on the environment. This was assessed by the community’s awareness of waste management via sources like the radio and the TV, their education on waste management and schools’ integration of environmental education in their program. Although most community members perceived the State’s environment as compared to it about 10 years’ back has worsened, where they were conscious of proper waste handling measures, the environment was described to be better. This influence of environmental awareness and education on environmental sustainability appraised using Logistic Regression Model, portrayed a significant variability (Omnibus Tests of Model Coefficients: χ2=42.742; P=0.014), inferring that environmental awareness and education significantly predict environmental sustainability.
The findings also revealed that organic waste generation spearheaded amongst other waste types like paper, plastic, E-waste, metal, textile and glass. While waste pickers always sorted paper, plastics, aluminium and metal, some of them also sorted out textile and glass. Statistically (P<0.05), in situations where waste was least generated (i.e., 1-2kg per day), community members maintained that the environmental quality was better in comparison to 10 years’ back. Waste items like broken glass and textile as well as the remains of E-waste after the extraction of copper and brass were not sorted for and these contributed more to environmental degradation.
Similarly, the influence of wealth on environmental sustainability was appraised using Logistic Regression Model including development index related indicators like education, occupation, income and the ability to pay for waste disposal. Harmonising the outcome, farmers, who were mostly the least educated claimed to notice more environmental improvement. In addition, those who did not agree to pay for waste disposal who were mostly those with low income (less than 200,000 Naira, i.e. about 620 Euros monthly) perceived environmental improvement more than those with income above 200,000 Naira. This irony can be attributed to the fact that those with low educational backing lack the capacity to appreciate environmental sustainability pointers well as compared to those with a broader educational background with critical thinking.
The employment and poverty reduction opportunities pertaining to waste management on environmental sustainability was appraised using qualitative thematic analysis. All community members involved in sorting, buying and selling of waste items had no second job. They attested that the money earned from their activities sustained their livelihood and families. Some expressed love for the job, especially as they were their own masters. Waste picking and trading in waste items are offering employment opportunities to many communities around the world. For instance, in the waste recycling, waste composting, waste-to-energy plants and die Stadtreiniger in Würzburg city. The workers in these enterprises have jobs as a result of waste.
Waste disposal influence on environmental sustainability was appraised using the Binary Logistic Regression Model and the variability explained by the model was significant. The validity was also supported by the Wald statistics (P<0.05), which indicates the effect of the predictors is significant. Environmental sustainability was greatly reliant on indicators like the frequency at which community members emptied their waste containers; how/where waste is disposed of, availability of disposal site or public bin near the house, etc. Imolites who asserted to have public waste bins or disposal sites near their houses maintained that the quality of the State’s environment had worsened as such containers/disposal sites were always stinking as well as had animals and smoke around them. Imolites around disposal sites complained of traits like diarrhoea, catarrh, insect bites, malaria, smoke and polluted air.
Conclusions. The liaison between poor waste management strategies and the sustainability of the Imo State environment was considered likely as statistically significant ineffectiveness, lack of awareness, poverty, insufficient and unrealistic waste management measures were found in this study area. In these situations, the environment was said to have not improved. Such inadequacies in the handling of generated waste did not only expose the citizenry to health dangers but also gave rise to streets and roads characterized by filth and many unattended disposal sites unleashing horrible odour to the environment and attracting wild animals. This situation is not only prevalent in Imo State, Nigeria but in many Sub-Saharan cities.
Future Perspectives. To improve the environment in Sub-Saharan Africa, it is imperative to practice an inclusive and integrated sustainable waste management system. The waste quantity in this region is fast growing, especially food/organic waste. The region should aim at waste management laws and waste reduction strategies, which will help save and produce more food that it really needs. Waste management should be dissociated from epidemic outbreaks like cholera, typhoid, Lassa fever and malaria, whose vectors thrive in filthy environments. Water channels and water bodies should not be waste disposal channels or waste disposal sites.
Climate warming has the potential to disrupt plant-pollinator interactions or to increase competition of co-flowering plants for pollinators, due to species-specific phenological responses to temperature. However, studies focusing on the effect of temperature on solitary bee emergence and the flowering onset of their food plants under natural conditions are still rare. We studied the effect of temperature on the phenology of the two spring bees Osmia cornuta and Osmia bicornis, by placing bee cocoons on eleven grasslands differing in mean site temperature. On seven grasslands, we additionally studied the effect of temperature on the phenology of the red-list plant Pulsatilla vulgaris, which was the first flowering plant, and of co-flowering plants with later flowering. With a warming of 0.1°C, the abundance-weighted mean emergence of O. cornuta males advanced by 0.4 days. Females of both species did not shift their emergence. Warmer temperatures advanced the abundance-weighted mean flowering of P. vulgaris by 1.3 days per 0.1°C increase, but did not shift flowering onset of co-flowering plants. Competition for pollinators between P. vulgaris and co-flowering plants does not increase within the studied temperature range. We demonstrate that temperature advances plant flowering more strongly than bee emergence suggesting an increased risk of pollinator limitation for the first flowers of P. vulgaris.
In contrast to the plasma membrane, the vacuole membrane has not yet been associated with electrical excitation of plants. Here, we show that mesophyll vacuoles from Arabidopsis sense and control the membrane potential essentially via the K\(^+\)-permeable TPC1 and TPK channels. Electrical stimuli elicit transient depolarization of the vacuole membrane that can last for seconds. Electrical excitability is suppressed by increased vacuolar Ca\(^{2+}\) levels. In comparison to wild type, vacuoles from the fou2 mutant, harboring TPC1 channels insensitive to luminal Ca\(^{2+}\), can be excited fully by even weak electrical stimuli. The TPC1-loss-of-function mutant tpc1-2 does not respond to electrical stimulation at all, and the loss of TPK1/TPK3-mediated K\(^{+}\) transport affects the duration of TPC1-dependent membrane depolarization. In combination with mathematical modeling, these results show that the vacuolar K\(^+\)-conducting TPC1 and TPK1/TPK3 channels act in concert to provide for Ca\(^{2+}\)- and voltage-induced electrical excitability to the central organelle of plant cells.
Previous research showed that full body ownership illusions in virtual reality (VR) can be robustly induced by providing congruent visual stimulation, and that congruent tactile experiences provide a dispensable extension to an already established phenomenon. Here we show that visuo-tactile congruency indeed does not add to already high measures for body ownership on explicit measures, but does modulate movement behavior when walking in the laboratory. Specifically, participants who took ownership over a more corpulent virtual body with intact visuo-tactile congruency increased safety distances towards the laboratory's walls compared to participants who experienced the same illusion with deteriorated visuo-tactile congruency. This effect is in line with the body schema more readily adapting to a more corpulent body after receiving congruent tactile information. We conclude that the action-oriented, unconscious body schema relies more heavily on tactile information compared to more explicit aspects of body ownership.
It is the aim of this thesis to present a visual body weight estimation, which is suitable for medical applications. A typical scenario where the estimation of the body weight is essential, is the emergency treatment of stroke patients: In case of an ischemic stroke, the patient has to receive a body weight adapted drug, to solve a blood clot in a vessel. The accuracy of the estimated weight influences the outcome of the therapy directly. However, the treatment has to start as early as possible after the arrival at a trauma room, to provide sufficient treatment. Weighing a patient takes time, and the patient has to be moved. Furthermore, patients are often not able to communicate a value for their body weight due to their stroke symptoms. Therefore, it is state of the art that physicians guess the body weight. A patient receiving a too low dose has an increased risk that the blood clot does not dissolve and brain tissue is permanently damaged. Today, about one-third gets an insufficient dosage. In contrast to that, an overdose can cause bleedings and further complications. Physicians are aware of this issue, but a reliable alternative is missing.
The thesis presents state-of-the-art principles and devices for the measurement and estimation of body weight in the context of medical applications. While scales are common and available at a hospital, the process of weighing takes too long and can hardly be integrated into the process of stroke treatment. Sensor systems and algorithms are presented in the section for related work and provide an overview of different approaches.
The here presented system -- called Libra3D -- consists of a computer installed in a real trauma room, as well as visual sensors integrated into the ceiling. For the estimation of the body weight, the patient is on a stretcher which is placed in the field of view of the sensors. The three sensors -- two RGB-D and a thermal camera -- are calibrated intrinsically and extrinsically. Also, algorithms for sensor fusion are presented to align the data from all sensors which is the base for a reliable segmentation of the patient.
A combination of state-of-the-art image and point cloud algorithms is used to localize the patient on the stretcher. The challenges in the scenario with the patient on the bed is the dynamic environment, including other people or medical devices in the field of view.
After the successful segmentation, a set of hand-crafted features is extracted from the patient's point cloud. These features rely on geometric and statistical values and provide a robust input to a subsequent machine learning approach. The final estimation is done with a previously trained artificial neural network.
The experiment section offers different configurations of the previously extracted feature vector. Additionally, the here presented approach is compared to state-of-the-art methods; the patient's own assessment, the physician's guess, and an anthropometric estimation. Besides the patient's own estimation, Libra3D outperforms all state-of-the-art estimation methods: 95 percent of all patients are estimated with a relative error of less than 10 percent to ground truth body weight. It takes only a minimal amount of time for the measurement, and the approach can easily be integrated into the treatment of stroke patients, while physicians are not hindered.
Furthermore, the section for experiments demonstrates two additional applications: The extracted features can also be used to estimate the body weight of people standing, or even walking in front of a 3D camera. Also, it is possible to determine or classify the BMI of a subject on a stretcher. A potential application for this approach is the reduction of the radiation dose of patients being exposed to X-rays during a CT examination.
During the time of this thesis, several data sets were recorded. These data sets contain the ground truth body weight, as well as the data from the sensors. They are available for the collaboration in the field of body weight estimation for medical applications.
Im Rahmen des BUKA-Projektes (Beratung und Unterstützung für Patienten mit Krebs und ihren Angehörigen) wurden im Universitätsklinikum Würzburg Krebspatienten auf Palliativbedarf gescreent. Ziel war es, bei einem positiven Screening die spezialisierte Palliativmedizin möglichst frühzeitig in onkologische Entscheidungsprozesse miteinzubeziehen.
Konzentriert wurde sich in diesem Projekt auf Patienten, die aufgrund eines metastasierten Primärfalles oder aufgrund eines Rezidives/ Progresses im Tumorboard der Universität Würzburg vorgestellt wurden. Als Tumordiagnose lag ein Bronchialkarzinom, ein gastrointestinaler Tumor oder ein malignes Melanom vor. Für die Auswertung unserer Studie wurden ausschließlich Patienten einbezogen, die innerhalb von 12 Monaten nach Vorstellung im Tumorboard verstorben sind.
Um die Wirkung des Screenings beurteilen zu können, wurden Patientendaten zweier Beobachtungszeiträume verglichen. Die erste Kohorte aus dem Jahr 2011 umfasste 143 Patienten mit einem mittleren Alter von 65,9 Jahren. 67,8% der Patienten waren Männer und 32,2% Frauen. Bei dieser Kohorte erfolgte kein Screening auf Palliativbedarf. Die zweite, palliativmedizinisch gescreente Kohorte aus dem Jahr 2014/15 bestand aus 263 Patienten mit einem mittleren Alter von 65,5 Jahren. Das Patientenkollektiv setzte sich zu 65,0% aus Männern und 35,0% Frauen zusammen.
Anhand eines Datenvergleiches des Jahres 2011 mit 2014/15, sollte untersucht werden, ob die Versorgung von Tumorpatienten am Lebensende durch eine Screening gestützte palliativmedizinische Mitbetreuung verbessert werden konnte.
Im Folgenden werden die Ergebnisse zusammengefasst.
Im Jahr 2014/15 konnte eine Erhöhung des Anteils an Patienten mit Kontakt zur Palliativmedizin um knapp 20% erreicht werden. 72,2% des Kollektivs nahmen in unserer Studie palliativmedizinische Angebote in Anspruch, 2011 waren dies 56,6%.
Der Anteil an Patienten mit Erstkontakt ≤ 3 Tage vor dem Tod reduzierte sich um 15% (23,5% vs. 8,9%).
Eine signifikante Erhöhung an dokumentierten Patientenverfügungen wurde nicht verzeichnet (19,6% vs. 26,6%).
Ebenso fand kein Rückgang an tumorspezifischen Therapiemaßnahmen in den letzten 14 Lebenstagen statt. Hier kam es im Vergleich zum Jahr 2011 zu einer leichten Steigerung um 3% (24,5% vs. 27,8%). Die Ursache scheint in der Zunahme an Patienten mit einer zielgerichteten Therapie zu liegen.
Der Vergleich beider Studien ergab, dass eine Verbesserung der Versorgungsqualität von Tumorpatienten am Lebensende durch die Screening gestützte palliativmedizinische Mitbetreuung in Teilpunkten erreicht werden konnte.
Unsere Ergebnisse zeigen aber auch auf, dass es im Vergleich zur Kohorte des Jahres 2011 weiterhin zu einer Überversorgung am Lebensende bei Tumorpatienten kam. Deutlich wird dies im Hinblick auf die Konstanz bzw. leichte Zunahme an tumorspezifischer Therapie in den letzten 14 Lebenstagen. Vor allem neue Therapieansätze und Medikamente bei der zielgerichteten Therapie scheinen einen Rückgang zu verhindern.
Es ist daher entscheidend, die Palliativmedizin möglichst frühzeitig in die Behandlung von Krebspatienten miteinzubeziehen, um so überambitionierte Therapiemaßnahmen am Lebensende zu unterbinden und rückläufige Prozentzahlen bei dem QI „Durchführen einer tumorspezifischen Therapie in den letzten 14 Lebenstagen“ zu erhalten.
Ob die Screening-gestützte palliativmedizinische Mitbetreuung eine Möglichkeit dafür darstellen könnte, konnte mit unserer Studie nicht eindeutig geklärt werden. Es sind weitere Studien bezüglich dieser Thematik nötig.
Die MRD Diagnostik ist von erheblicher Bedeutung für die Risikostratifizierung kindlicher Leukämien. Um aber gesunde, sich regenerierende Vorstufen von blastären Zellen unterscheiden zu können ist die genaue Kenntnis des Antigenverlaufs sowohl der Vorstufen der B-Zellreihe als auch der Blasten notwendig. In dieser Arbeit wird eine Vergleichende Analyse von B-Zellvorstufen und Blasten mittels Durchflusszytometrie durchgeführt. Von besonders diskriminativem Wert waren die Vorläufermarker CD10, CD34, sowie die lymphatischen Marker CD19, CD20, CD22, CD45, cyCD79a und cyTdT.
Zur Beschreibung des individuellen LAIP eigneten sich vor allem die Marker CD11b, CD38, CD58, CD123 und CD133, sowie die myeloischen Marker CD13 und CD33.
Die Bessere Unterscheidung zwischen gesunden und kranken Zellen zusammen mit neuen Entwicklungen in Diagnostik und Therapie muss in Zukunft zur weiteren Verbesserung der Überlebensraten, auch im Rezidiv führen.
In der vorliegenden Arbeit wurden mehrere konventionelle und 3D-konformale Radiotherapie ("3D-CRT")-Techniken in der Bestrahlung von Wirbelsäulenmetastasen miteinander verglichen. Dafür wurde, basierend auf reellen Planungs-CT-Datensätzen, die Bestrahlung von 41 Wirbelsäulen-Zielvolumina mit verschiedenen Techniken simuliert.
Es konnte gezeigt werden, dass im Vergleich zu konventionellen Techniken bereits sehr einfache 3D-CRT-Techniken eine homogenere Abdeckung von Wirbelsäulen-Zielvolumina bei gleichzeitig zuverlässiger Limitierung der Rückenmarksdosis ermöglichen. Angesichts steigender Lebenserwartungen und dem zunehmenden Bedarf an Re-Bestrahlungen kann dies einen entscheidenden Vorteil darstellen.
In dieser Dissertation wurden Unterschiede hinsichtlich der Fähigkeit zur Erfassung depressiver Symp¬to¬matik der drei Screeninginstrumente PHQ-2, ESAS-Dpr und DT im palliativ-onkologischen Kontext für den deutschsprachigen Raum untersucht. Ziel war es eine Empfehlung abzugeben, ob für das Screening nach depressiver Symptomatik, die Empfehlungen der kanadischen Guideline von Cancer Care Ontario oder die Empfehlungen der S3-Leitlinie Palliativmedizin anzuwenden sind. Weiterhin sollte die Frage geklärt werden, ob im deutschsprachigen Raum die Instrumente ESAS-Dpr und DT als äquivalente Instrumente verwendet werden können.
Die Ergebnisse der Hauptfragestellung dieser Dissertation demonstrieren die schwache Übereinstimmung von ESAS-Dpr mit den anderen Ultra-Kurz-Screening-Instrumenten PHQ-2 und DT. Dabei wurde zum ersten Mal ein Vergleich zwischen ESAS-Dpr und PHQ-2 durchgeführt und eine limitierte Screening-Fähigkeit von ESAS-Dpr bei palliativ erkrankten Patienten gemessen. Des Weiteren konnte in dieser Arbeit gezeigt werden, dass im vorliegenden Patientenkollektiv das DT und ESAS-Dpr keine ausreichende Übereinstimmung besitzen um im deutschen Raum synonym verwendet werden zu können. Die zugrundeliegende deutsche Übersetzung der englischen Begrifflichkeiten 'distress' als Belastung und 'depression' als Depression wurde als ausschlaggebend für dieses Ergebnis vermutet.
In der Zusammenschau der Ergebnisse dieser Studie entstand ein Algorithmus für das Erfassen von Depressivität bei palliativ-onkologisch erkrankten Erwachsenen im alltäglichen und praktischen Gebrauch.
Investigations focusing on the social criticism in La Regenta (1884-1885) by Leopoldo Alas «Clarín» have constantly referred to the unmasking of society’s hypocrisy and provincialism through the implementation of satire and irony in the novel. This observation, though, has to be defined more clearly. Vetustan society, specifically the bourgeoisie, is characterized primarily by the incessant exhibition of supposed wisdom and intelligentsia in public to generate social prestige and power. By analyzing the narrative strategies which are related to the composition of the secondary characters, the role of two specific public venues (Casino and Theatre of Vetusta) and the (de)construction of P. Ronzal, P. Guimarán and S. Bermúdez in the novel, this article illustrates how false wisdom and pseudo-intelligentsia become central motifs regarding social criticism in La Regenta.
Die komplette Resektion mittels Pankreatoduodenektomie (Whipple-Operation) ist die einzig kurative Therapieoption bei periampullären Adenokarzinomen. Bei fortgeschrittenem Tumorleiden kann hierzu eine venöse Gefäßresektion mit anschließender Rekonstruktion notwendig sein. In dieser Arbeit erfolgte die retrospektive Auswertung der von Januar 2003 bis Februar 2013 am Universitätsklinikum Würzburg durchgeführten Eingriffe dieser Art.
We aimed to determine a detailed regional ventricular distribution pattern of the novel cardiac nerve PET radiotracer \(^{18}\)F-LMI1195 in healthy rabbits. Ex-vivo high resolution autoradiographic imaging was conducted to identify accurate ventricular distribution of \(^{18}\)F-LMI1195. In healthy rabbits, \(^{18}\)F-LMI1195 was administered followed by the reference perfusion marker \(^{201}\)Tl for a dual-radiotracer analysis. After 20 min of \(^{18}\)F-LMI1195 distribution time, the rabbits were euthanized, the hearts were extracted, frozen, and cut into 20-μm short axis slices. Subsequently, the short axis sections were exposed to a phosphor imaging plate to determine \(^{18}\)F-LMI1195 distribution (exposure for 3 h). After complete \(^{18}\)F decay, sections were re-exposed to determine 201Tl distribution (exposure for 7 days). For quantitative analysis, segmental regions of Interest (ROIs) were divided into four left ventricular (LV) and a right ventricular (RV) segment on mid-ventricular short axis sections. Subendocardial, mid-portion, and subepicardial ROIs were placed on the LV lateral wall. \(^{18}\)F-LMI1195 distribution was almost homogeneous throughout the LV wall without any significant differences in all four LV ROIs (anterior, posterior, septal and lateral wall, 99 ± 2, 94 ± 5, 94 ± 4 and 97 ± 3%LV, respectively, n.s.). Subepicardial \(^{201}\)Tl uptake was significantly lower compared to the subendocardial portion (subendocardial, mid-portion, and subepicardial activity: 90 ± 3, 96 ± 2 and *80 ± 5%LV, respectively, *p < 0.01 vs. mid-portion). This was in contradistinction to the transmural wall profile of \(^{18}\)F-LMI1195 (90 ± 4, 96 ± 5 and 84 ± 4%LV, n.s.). A slight but significant discrepant transmural radiotracer distribution pattern of \(^{201}\)Tl in comparison to \(^{18}\)F-LMI1195 may be a reflection of physiological sympathetic innervation and perfusion in rabbit hearts.
Detecting whether a suspect possesses incriminating (e.g., crime-related) information can provide valuable decision aids in court. To this means, the Concealed Information Test (CIT) has been developed and is currently applied on a regular basis in Japan. But whereas research has revealed a high validity of the CIT in student and normal populations, research investigating its validity in forensic samples in scarce. This applies even more to the reaction time-based CIT (RT-CIT), where no such research is available so far. The current study tested the application of the RT-CIT for an imaginary mock crime scenario both in a sample of prisoners (n = 27) and a matched control group (n = 25). Results revealed a high validity of the RT-CIT for discriminating between crime-related and crime-unrelated information, visible in medium to very high effect sizes for error rates and reaction times. Interestingly, in accordance with theories that criminal offenders may have worse response inhibition capacities and that response inhibition plays a crucial role in the RT-CIT, CIT-effects in the error rates were even elevated in the prisoners compared to the control group. No support for this hypothesis could, however, be found in reaction time CIT-effects. Also, performance in a standard Stroop task, that was conducted to measure executive functioning, did not differ between both groups and no correlation was found between Stroop task performance and performance in the RT-CIT. Despite frequently raised concerns that the RT-CIT may not be applicable in non-student and forensic populations, our results thereby do suggest that such a use may be possible and that effects seem to be quite large. Future research should build up on these findings by increasing the realism of the crime and interrogation situation and by further investigating the replicability and the theoretical substantiation of increased effects in non-student and forensic samples.
Diese Promotion befasst sich mit der Entwicklung eines neuartigen High-Fidelity, Full-Procedural Simulationsmodell für die Durchführung einer offenen Nabelhernienreparation mit präperitonealen Netzimplantation in Underlay-Position (NANEP-Modell). Das Übungsmodell setzten wir in einem eigens konstruierten Operationskurs in der Allgemein- und Viszeralchirugie der Universitätsklinik Würzburg ein. Ziel war die Validierung des Modells durch Untersuchung der Inhalts-Validität, der Konstrukt-Validität und der Differentiellen Validität. Die auf Video aufgezeichneten Operationen der Probanden wurden auf der Internetplattform Catlive mithilfe des Competency Assessment Tools bewertet. Der Lernzuwachs wurde gemessen und untersucht. Die operierten Modelle wurden zur Prüfung der Kriteriums-Validität autopsiert.
In dieser retrospektiven Datenanalyse wurde ein ungefiltertes Patientenkollektiv von 188 Patienten mit Erstdiagnose eines hepatozellulären Karzinoms im Zeitraum von 2005 bis 2012 am Universitätsklinikum Würzburg anhand verschiedener demografische, pathomechanistischer, diagnostischer, therapeutischer oder prognostischer Parameter untersucht. Durch die Datenanalyse konnten verschiedene prognostische Faktoren eruiert werden, die sich auf das mediane Überleben auswirken, wie zum Beispiel das Child-Pugh-Stadium, der AFP-Spiegel, die Pfortaderthrombose und das Geschlecht des Patienten. Darüber hinaus konnte gezeigt werden, dass Patienten von einem multimodalen Therapiekonzept profitieren.
Des Weiteren wurden Besonderheiten bei den Grunderkrankungen evaluiert. Die allermeisten HCCs sind im Würzburger Patientenkollektiv auf einen chronischen Alkoholabusus zurückzuführen. Abgesehen davon wird die Bedeutung der non-alkoholischen Fettlebererkrankung in den kommenden Jahren mehr und mehr zunehmen.
Large-area remote sensing time-series offer unique features for the extensive investigation of our environment. Since various error sources in the acquisition chain of datasets exist, only properly validated results can be of value for research and downstream decision processes. This review presents an overview of validation approaches concerning temporally dense time-series of land surface geo-information products that cover the continental to global scale. Categorization according to utilized validation data revealed that product intercomparisons and comparison to reference data are the conventional validation methods. The reviewed studies are mainly based on optical sensors and orientated towards global coverage, with vegetation-related variables as the focus. Trends indicate an increase in remote sensing-based studies that feature long-term datasets of land surface variables. The hereby corresponding validation efforts show only minor methodological diversification in the past two decades. To sustain comprehensive and standardized validation efforts, the provision of spatiotemporally dense validation data in order to estimate actual differences between measurement and the true state has to be maintained. The promotion of novel approaches can, on the other hand, prove beneficial for various downstream applications, although typically only theoretical uncertainties are provided.
Die Ableitung Vagus-somatosensibel evozierter Potentiale (VSEP) ist eine vielversprechende, kostengünstige und nicht-invasive Methode zur Frühdiagnostik von Alzheimer-Demenz. In der vorliegenden Arbeit wurde die Retest-Reliabilität der VSEP untersucht. Des Weiteren wurden alternative Stimulationsbedingungen (Stimulation in der Cymba conchae und im anterioren äußeren Gehörgang) und alternative Ableitbedingungen (referentielle Ableitungen mit Elektroden an Mastoid, Spina scapulae, Vertebra prominens und Handrücken) getestet und mit der herkömmlichen Methode verglichen. Die Reliabilitätsuntersuchungen wurden an 24 gesunden Probanden im Abstand von sechs Monaten durchgeführt. Die alternativen Stimulations- und Ableitbedingungen wurden an je zehn bis zwölf Probanden angewandt und die Ergebnisse mit denen von Messungen mit der herkömmlichen Methode verglichen.
Es zeigte sich eine besonders gute Reliabilität in den Ableitungen T3/O1 und T4/O2. Außerdem liegen Hinweise darauf vor, dass das Alter der Probanden die Reliabilität beeinflusst.
Als beste alternative Ableitposition erwies sich das Mastoid. Die Messungen mit alternativen Stimulationspositionen ergaben stark von der herkömmlichen Methode abweichende Ergebnisse, deren Ursache die vorliegende Studie nicht sicher klären kann.
Virotherapy on the basis of oncolytic vaccinia virus (VACV) strains is a promising approach for cancer therapy. Recently, we showed that the oncolytic vaccinia virus GLV-1h68 has a therapeutic potential in treating human prostate and hepatocellular carcinomas in xenografted mice. In this study, we describe the use of dynamic boolean modeling for tumor growth prediction of vaccinia virus-injected human tumors. Antigen profiling data of vaccinia virus GLV-1h68-injected human xenografted mice were obtained, analyzed and used to calculate differences in the tumor growth signaling network by tumor type and gender. Our model combines networks for apoptosis, MAPK, p53, WNT, Hedgehog, the T-killer cell mediated cell death, Interferon and Interleukin signaling networks. The in silico findings conform very well with in vivo findings of tumor growth. Similar to a previously published analysis of vaccinia virus-injected canine tumors, we were able to confirm the suitability of our boolean modeling for prediction of human tumor growth after virus infection in the current study as well. In summary, these findings indicate that our boolean models could be a useful tool for testing of the efficacy of VACV-mediated cancer therapy already before its use in human patients.
Almost all life forms on earth have adapted to the most impactful and most predictable recurring change in environmental condition, the cycle of day and night, caused by the axial rotation of the planet. As a result many animals have evolved intricate endogenous clocks, which adapt and synchronize the organisms’ physiology, metabolism and behaviour to the daily change in environmental conditions. The scientific field researching these endogenous clocks is called chronobiology and has steadily grown in size, scope and relevance since the works of the earliest pioneers in the 1960s.
The number one model organism for the research of circadian clocks is the fruit fly, Drosophila melanogaster, whose clock serves as the entry point to understanding the basic inner workings of such an intricately constructed endogenous timekeeping system. In this thesis it was attempted to combine the research on the circadian clock with the techniques of optogenetics, a fairly new scientific field, launched by the discovery of Channelrhodopsin 2 just over 15 years ago. Channelrhodopsin 2 is a light-gated ion channel found in the green alga Chlamydomonas reinhardtii. In optogenetics, researches use these light-gated ion channels like Channelrhodopsin 2 by heterologously expressing them in cells and tissues of other organisms, which can then be stimulated by the application of light. This is most useful when studying neurons, as these channels provide an almost non-invasive tool to depolarize the neuronal plasma membranes at will. The goal of this thesis was to develop an optogenetic tool, which would be able to influence and phase shift the circadian clock of Drosophila melanogaster upon illumination. A phase shift is the adaptive response of the circadian clock to an outside stimulus that signals a change in the environmental light cycle. An optogenetic tool, able to influence and phase shift the circadian clock predictably and reliably, would open up many new ways and methods of researching the neuronal network of the clock and which neurons communicate to what extent, ultimately synchronizing the network.
The first optogenetic tool to be tested in the circadian clock of Drosophila melanogaster was ChR2-XXL, a channelrhodopsin variant with dramatically increased expression levels and photocurrents combined with a prolonged open state. The specific expression of ChR2-XXL and of later constructs was facilitated by deploying the three different clock-specific GAL4-driver lines, clk856-gal4, pdf-gal4 and mai179-gal4. Although ChR2-XXL was shown to be highly effective at depolarizing neurons, these stimulations proved to be unable to significantly phase shift the circadian clock of Drosophila. The second series of experiments was conducted with the conceptually novel optogenetic tools Olf-bPAC and SthK-bPAC, which respectively combine a cyclic nucleotide-gated ion channel (Olf and SthK) with the light-activated adenylyl-cyclase bPAC. These tools proved to be quite useful when expressed in the motor neurons of instar-3 larvae of Drosophila, paralyzing the larvae upon illumination, as well as affecting body length. This way, these new tools could be precisely characterized, spawning a successfully published research paper, centered around their electrophysiological characterization and their applicability in model organisms like Drosophila. In the circadian clock however, these tools caused substantial damage, producing severe arrhythmicity and anomalies in neuronal development. Using a temperature-sensitive GAL80-line to delay the expression until after the flies had eclosed, yielded no positive results either. The last series of experiments saw the use of another new series of optogenetic tools, modelled after the Olf-bPAC, with bPAC swapped out for CyclOp, a membrane-bound guanylyl-cyclase, coupled with less potent versions of the Olf. This final attempt however also ended up being unsuccessful. While these tools could efficiently depolarize neuronal membranes upon illumination, they were ultimately unable to stimulate the circadian clock in way that would cause it to phase shift.
Taken together, these mostly negative results indicate that an optogenetic manipulation of the circadian clock of Drosophila melanogaster is an extremely challenging subject. As light already constitutes the most impactful environmental factor on the circadian clock, the combination of chronobiology with optogenetics demands the parameters of the conducted experiments to be tuned with an extremely high degree of precision, if one hopes to receive positive results from these types of experiments at all.
As structural membrane components and signaling effector molecules sphingolipids influence a plethora of host cell functions, and by doing so also the replication of viruses. Investigating the effects of various inhibitors of sphingolipid metabolism in primary human peripheral blood lymphocytes (PBL) and the human B cell line BJAB we found that not only the sphingosine kinase (SphK) inhibitor SKI-II, but also the acid ceramidase inhibitor ceranib-2 efficiently inhibited measles virus (MV) replication. Virus uptake into the target cells was not grossly altered by the two inhibitors, while titers of newly synthesized MV were reduced by approximately 1 log (90%) in PBL and 70–80% in BJAB cells. Lipidomic analyses revealed that in PBL SKI-II led to increased ceramide levels, whereas in BJAB cells ceranib-2 increased ceramides. SKI-II treatment decreased sphingosine-1-phosphate (S1P) levels in PBL and BJAB cells. Furthermore, we found that MV infection of lymphocytes induced a transient (0.5–6 h) increase in S1P, which was prevented by SKI-II. Investigating the effect of the inhibitors on the metabolic (mTORC1) activity we found that ceranib-2 reduced the phosphorylation of p70 S6K in PBL, and that both inhibitors, ceranib-2 and SKI-II, reduced the phosphorylation of p70 S6K in BJAB cells. As mTORC1 activity is required for efficient MV replication, this effect of the inhibitors is one possible antiviral mechanism. In addition, reduced intracellular S1P levels affect a number of signaling pathways and functions including Hsp90 activity, which was reported to be required for MV replication. Accordingly, we found that pharmacological inhibition of Hsp90 with the inhibitor 17-AAG strongly impaired MV replication in primary PBL. Thus, our data suggest that treatment of lymphocytes with both, acid ceramidase and SphK inhibitors, impair MV replication by affecting a number of cellular activities including mTORC1 and Hsp90, which alter the metabolic state of the cells causing a hostile environment for the virus.
Ureaplasma species are common colonizers of the adult genitourinary tract and often considered as low-virulence commensals. Intraamniotic Ureaplasma infections, however, facilitate chorioamnionitis and preterm birth, and cases of Ureaplasma-induced neonatal sepsis, pneumonia, and meningitis raise a growing awareness of their clinical relevance. In vitro studies are scarce but demonstrate distinct Ureaplasma-driven impacts on immune mechanisms. The current study addressed cytokine and chemokine responses upon exposure of native or lipopolysaccharide (LPS) co-stimulated human brain microvascular endothelial cells (HBMEC) to Ureaplasma urealyticum or U. parvum, using qRT-PCR, RNA sequencing, multi-analyte immunoassay, and flow cytometry. Ureaplasma exposure in native HBMEC reduced monocyte chemoattractant protein (MCP)-3 mRNA expression (p < 0.01, vs. broth). In co-stimulated HBMEC, Ureaplasma spp. attenuated LPS-evoked mRNA responses for C-X-C chemokine ligand 5, MCP-1, and MCP-3 (p < 0.05, vs. LPS) and mitigated LPS-driven interleukin (IL)-1α protein secretion, as well as IL-8 mRNA and protein responses (p < 0.05). Furthermore, Ureaplasma isolates increased C-X-C chemokine receptor 4 mRNA levels in native and LPS co-stimulated HBMEC (p < 0.05). The presented results may imply immunomodulatory capacities of Ureaplasma spp. which may ultimately promote chronic colonization and long-term neuroinflammation.
Fungi of the order Mucorales colonize all kinds of wet, organic materials and represent a permanent part of the human environment. They are economically important as fermenting agents of soybean products and producers of enzymes, but also as plant parasites and spoilage organisms. Several taxa cause life-threatening infections, predominantly in patients with impaired immunity. The order Mucorales has now been assigned to the phylum Mucoromycota and is comprised of 261 species in 55 genera. Of these accepted species, 38 have been reported to cause infections in humans, as a clinical entity known as mucormycosis. Due to molecular phylogenetic studies, the taxonomy of the order has changed widely during the last years. Characteristics such as homothallism, the shape of the suspensors, or the formation of sporangiola are shown to be not taxonomically relevant. Several genera including Absidia, Backusella, Circinella, Mucor, and Rhizomucor have been amended and their revisions are summarized in this review. Medically important species that have been affected by recent changes include Lichtheimia corymbifera, Mucor circinelloides, and Rhizopus microsporus. The species concept of Rhizopus arrhizus (syn. R. oryzae) is still a matter of debate. Currently, species identification of the Mucorales is best performed by sequencing of the internal transcribed spacer (ITS) region. Ecologically, the Mucorales represent a diverse group but for the majority of taxa, the ecological role and the geographic distribution remain unknown. Understanding the biology of these opportunistic fungal pathogens is a prerequisite for the prevention of infections, and, consequently, studies on the ecology of the Mucorales are urgently needed.
Einige psychiatrische Erkrankungen gehen mit einer Veränderung der Riechfunktion einher. In aktuellen Tiermodellen wurde durch Stimulation der Raphe Kerne, die unter anderem zum Bulbus Olfaktorius projizieren, das serotonerge System als Einflussfaktor der Riechfunktion erkannt. Unsere Hypothese geht davon aus, dass das serotonerge System die Riechleistung beeinflussen könnte. Um diese Hypothese zu überprüfen, wurden drei Gene des serotonergen Systems und deren psychiatrisch relevante Genpolymorphismen ausgewählt und untersucht: TPH2 (Tryptophanhydroxylase 2, rs4570625), 5-HTTLPR (Serotonintransporter-Längenpolymorphismus) und 5-HT2C (Serotonintransporter, rs3813929).
Insgesamt wurden 173 gesunde Kinder eingeschlossen und auf deren Riechleistung (Riechschwelle und Diskrimination) hin mittels des Sniffin’Sticks Tests untersucht. Träger des T-Allels des TPH2-Polymorphismus sowie Träger des s-Allels, des 5-HTTLPR wiesen eine signifikant bessere Diskrimination von Gerüchen auf. Der Effekt des 5-HTTLPR ließ sich auf die Mädchen in der Stichprobe zurückführen. Der Genpolymorphismus des 5-HT2C Rezeptors wirkte sich nicht signifikant auf die Riechleistung aus. In Bezug auf die Riechsensitivität zeigten sich keine signifikanten Modulationen durch die untersuchten Genvarianten.
Gene des serotonergen Systems üben bei gesunden Kindern einen modulierenden Einfluss auf die Riechfunktion aus, insbesondere auf die Riechdiskrimination. Von besonderer Bedeutung scheinen in diesem Zusammenhang der Serotonintransporter 5-HTTLPR und das Syntheseenzym TPH2 zu sein.
Zahlreiche humanpathogene bakterielle Erreger können ihre Fähigkeit zur Kolonisation epithelialer Barrieren optimieren, indem sie mit dem Zellzyklus der infizierten Wirtszelle in Wechselwirkung treten und so die Abschilferung und Erneuerung des Epithels verzögern. Die hierbei wirksamen bakteriellen Effektoren sind als „Cyclomoduline“ bekannt und gelten als neue Klasse bakterieller Pathogenitätsfaktoren. Ziel der vorliegenden Promotionsarbeit war es zu untersuchen, ob durch die Infektion menschlicher pharyngealer Epithelzellen mit N. meningitidis der Zellzyklus der Wirtszelle beeinflusst wird. Mit zwei verschiedenen Untersuchungsmethoden konnte übereinstimmend gezeigt werden, dass die Infektion der Epithelzelllinie Detroit 562 mit verschiedenen Meningokokkenisolaten zu einer signifikanten Akkumulation von Epithelzellen in der G1-Phase führte. Dieser Effekt wurde sowohl von pathogenen Meningokokkenstämmen als auch von Trägerstämmen ausgelöst, jedoch nur durch Isolate, die fähig zur Adhärenz und zur Invasion in die Epithelzelle waren. Durch Hitzebehandlung der Bakterien konnte der Zellzyklusarrest vollständig aufgehoben werden. Ebenso konnte der Effekt durch Inkubation der Epithelzellen mit bakteriellen Kulturüberständen und durch Infektion der Zellen mit E. coli-Stämmen, welche die Meningokokkenadhäsine Opa und Opc überexprimieren, nicht ausgelöst werden.
Es konnte weiterhin nachgewiesen werden, dass die Infektion mit N. meningitidis in der Zielzelle zu einer signifikant gesteigerten Expression des CDK-Inhibitors p21WAF1/Cip1 führte, begleitet von einer vermehrten Lokalisation im Zellkern. Auch zeigte sich eine veränderte Proteinexpression der für die G1-Phase relevanten Cycline D und E. Diese scheint sich erst posttranslational zu ereignen, da die unterschiedliche Expression auf mRNA-Ebene nicht festgestellt werden konnte.
Zusammenfassend konnte dargestellt werden, dass die Infektion von Pharynxepithelzellen mit lebenden, zur Adhärenz und Invasion fähigen Meningokokkenstämmen in der menschlichen Zielzelle einen Zellzyklusarrest in der G1-Phase verursacht, vermutlich durch veränderte Expression der Zellzyklusregulatoren p21WAF1/Cip1, Cyclin D und Cyclin E. Möglicherweise stellt die Induktion dieses Zellzyklusarrestes einen wichtigen Schritt in der Pathogenese der bakteriellen Kolonisation des oberen Atemwegsepithels durch N. meningitidis dar.
Kostimulatorische Signalwege spielen beim Zustandekommen einer T-Zell-gebundenen Effektor-Immunantwort eine entscheidende Rolle. In dieser Arbeit wurde die Expression der Signalwege PD-1/PD-L1 und CD137/CD137L im kolorektalen Karzinom untersucht. Hierzu wurde die Expression in den Karzinomen SW480, SW620 und HT-29 mittels qRT-PCR, Western Blot und FACS analysiert.
Es konnte gezeigt werden, dass PD-1 und CD137 sowie deren Rezeptoren PD-L1 und CD137L im Kolonkarzinom auf Gen- und Proteinebene exprimiert werden. Zunehmendes Tumorzellwachstum sowie mangelnde Nährstoffversorgung führten zu deutlichen Veränderungen im Expressionsmuster, wobei sich zwischen den Kolonkarzinomen SW480/SW620 und dem Kolonkarzinom HT-29 Unterschiede aufzeigen ließen.
Durch die Untersuchungen für diese Arbeit konnten wertvolle Informationen über das Expressionsverhalten der untersuchten kostimulatorischen Signalwege gewonnen werden. Eine mögliche Schlussfolgerung ist, dass eine inhibierende PD-1/PD-L1- als auch eine CD137/CD137L-Tumorzell-vermittelte Therapie die Tumorimmunantwort gegen das kolorektale Karzinom stärken und damit das Überleben betroffener Patienten verbessern könnte.
Einhergehend mit einer steigenden Lebenserwartung nimmt auch die Zahl der am Multiplen Myelom Erkrankten zu. Bis dato gibt es nur wenige Therapieansätze dieser selten vorkommenden Blutkrebserkrankung. Im Zusammenhang mit der Entstehung des Multiplen Myeloms stehen vor allem zwei bedeutende Hitzeschockproteine: Hsp90 und Hsp70. Beide haben die Aufgabe, Zellen vor Apoptose zu schützen. In proliferierenden Plasmazellen ist eine Überexpression an Hsp90 zu beobachten. Entwickelte Inhibitoren führten zwar zu einer verminderten Hsp90-Aktivität, allerdings wurde diese durch eine vermehrte Expression von Hsp70 kompensiert, weshalb Myelomzellen weiterhin proliferierten. Aus diesem Grund bietet sich Hsp70 als weiterer Angriffspunkt in der Therapierung des Multiplen Myeloms an. Die bislang entwickelten Inhibitoren binden entweder an die Nukleotid- oder Substratbindedomäne. Da beide Stellen unspezifisch sind, wurden durch virtuelles Screening potenzielle Inhibitoren für Hsp70 identifiziert, welche in vitro und in vivo tatsächlich Effekte hinsichtlich der Herunterregulierung von Hsp70 zeigten. Ob die entwickelten Substanzen jedoch direkt an Hsp70 binden, war die Fragestellung der vorliegenden Arbeit.
In dieser Arbeit wurde untersucht, inwiefern die entwickelten Inhibitoren an Hsp70 binden und dieses inhibieren. Die humane Hsp70-Familie besitzt sechzehn Mitglieder, die alle ähnliche Aufgaben und Strukturmerkmale aufweisen. Für die durchgeführten Versuche wurde die Hsp70-Isoform Hsc70 verwendet. In einem Protein-Ligand-Assay konnte gezeigt werden, dass die meisten Verbindungen durch Aggregatbildung zu einer Inhibition von Hsc70 führten. Durch Zugabe von Detergenz konnten die gebildeten Aggregate aufgebrochen und so der Inhibitionseffekt aufgehoben bzw. deutlich reduziert werden. Damit konnte gezeigt werden, dass die in Zell- und Mausversuchen beobachteten Effekte vermutlich nicht auf eine direkte Inhibition von Hsc70 zurückzuführen sind. Ob diese Effekte nun ebenfalls auf Aggregatbildung beruhen oder aber ein anderes Protein als das vermutete Hsc70 inhibiert wird, was über eine Signalkaskade zur Inhibition von Hsc70 führt, wäre eine interessante Fragestellung für weitere Untersuchungen.
Da sowohl in NMR-Versuchen als auch dem durchgeführten Protein-Ligand-Assay gezeigt werden konnte, dass die vormals als potenzielle Inhibitoren entwickelten Verbindungen nur schwach aktiv sind, wurde durch Fragment-basierte Ansätze eine andere Bindestelle für mögliche Inhibitoren identifiziert. Hierbei konnte N-Acetyl-D-Glucosamin in der Nukleotidbindedomäne von Hsc70 detektiert werden. Hieraus könnten sich neue Ansätze zur Entwicklung neuartiger in silico entwickelter Hsc70-Inhibitoren ergeben.
Ausgangspunkt für die Docking-Studien zur Entwicklung neuer Hsp70-Inhibitoren war die Kristallstruktur von bHsc70 ED 1-554, einer trunkierten Doppelmutante des nativen Hsc70. Bis dato ist diese 554 Aminosäuren umfassende Mutante die einzige Hsc70-Variante von der die Zweidomänenstruktur kristallisiert werden konnte. Für dieses Konstrukt wurde zunächst ein optimiertes Aufreinigungsprotokoll entwickelt, um dann Kristallisationsversuche mit ausgewählten AH-Verbindungen, die in den Docking-Studien entwickelt wurden, durchzuführen. Hierbei konnte jedoch keine Bindung festgestellt werden. Die Kristallisation mit Ver-155008, einem bekannten Hsc70-Inhibitor, führte jedoch zur ersten Zweidomänenstruktur von Hsc70 mit gebundenem Ver-155008.
Neben der obigen Fragestellung wurde außerdem untersucht, wie funktional aktiv das trunkierte Hsc70-Konstrukts ist. Hier zeigte sich, dass aufgrund des fehlenden C-Terminus zwar eine geringe Aktivität von 30 % im Vergleich zur Volllänge zu beobachten war. Für eine nahezu vollständige Rückfaltungsaktivität ist aber der C-Terminus essentiell. Weiterhin konnte in ITC-Versuchen der Kd-Wert von Ver-155008 an die verwendete Mutante ermittelt werden, der dem bereits bekannten Kd von Ver-155008 an das native Hsc70 ähnlich ist.
Die Implantation von Kniegelenks-Totalendoprothesen stellt heutzutage einen Routineeingriff dar, ist allerdings mit postoperativen Schmerzen und einer langwierigen Rehabilitation verbunden. In den letzten Jahren haben alternative Heilmethoden wie etwa die Traditionelle Chinesische Medizin eine zunehmende Verbreitung erfahren und speziell für die Akupunktur konnte auch eine schmerzlindernde Wirkung gezeigt werden. Inwieweit eine Akupunktur-behandlung in der postoperativen Situation das funktionelle Ergebnis verbessern kann ist unklar. Im Rahmen dieser Studie wurde deshalb der Einfluss einer Akupunktur auf das funktionelle Ergebnis zwischen dem zweiten postoperativen Tag und drei Monate nach der Operation untersucht. Dazu wurde eine randomisierte, kontrollierte und dreifach verblindete Monozenterstudie mit 60 Teilnehmern durchgeführt. Die Kontrollgruppe erhielt dabei die postoperative Standardtherapie, während die Interventionsgruppe vom 2. bis 6. Tag nach der Operation einmal täglich zusätzlich akupunktiert wurde. Im gleichen Zeitraum wurde eine dritte Studiengruppe, die Placebogruppe, scheinakupunktiert. Vom 2. bis 6. Tag nach der Operation sowie drei Monate später wurden die postoperativen Schmerzintensitäten abgefragt. Weiterhin wurden die maximal mögliche, passive Kniebeugung und die maximal mögliche Kniebelastung drei Monate nach der Operation ermittelt und das funktionelle Ergebnis der Operation mittels Fragebögen durch die Patienten bewertet. Dabei zeigten sich keine statistisch signifikanten Unterschiede zwischen den Studiengruppen. Aufgrund der Limitationen dieser sowie vergleichbarer Studien und angesichts uneinheitlicher Studienergebnisse bei insgesamt unzureichender Vergleichbarkeit der verschiedenen Akupunkturstudien ist eine abschließende Bewertung zur Wirksamkeit der Akupunktur nach Implantation einer Kniegelenks-Totalendoprothese jedoch nicht möglich. Dazu bedarf es weiterer, qualitativ hochwertiger und größerer Studien zu diesem Thema.
Die Infektion mit dem Masernvirus (MV) stellt weltweit immer noch ein großes Problem dar. Trotz des vorhandenen Lebendimpfstoffs, der eine Erkrankung sicher zu verhindern vermag, haben nicht nur die Entwicklungsländer, in denen ein flächendeckender Impfschutz schwieriger zu erreichen ist, mit der Erkrankung und ihren Komplikationen zu kämpfen. Hat sich die Erkrankung klinisch manifestiert gibt es keine kausalen Therapiemöglichkeiten und es kann nur noch symptomatisch behandelt werden. Dies ist v.a. auch in Hinblick auf die schweren Komplikationen der Maserninfektion von Bedeutung. Bei Erstkontakt mit dem Masernvirus ist die Suszeptibilität nicht geimpfter Menschen sehr hoch. Das bedeutet, dass es in 95-98 % der Fälle nach einer Infektion mit dem Masernvirus auch zum klinischen Bild der Masern kommt, unabhängig von Alter und Geschlecht. Das Ziel dieser Arbeit war es daher, potentielle Hemmstoffe der Maserninfektion auf ihre Wirkung zu testen und zu verstehen, wo im Infektions- und Replikationszyklus des MV sie eingreifen. Es wurden eine Reihe Substanzen mit potentiell-inhibitorischen Eigenschaften in Infektions-Hemmtests und im Zytotoxizitätstest untersucht, von denen im Anschluss die drei besten Inhibitoren (JK80, QD6-8 und Droseron) weiter untersucht wurden. JK80 und QD6-8 waren beide mit IC50-Werten um 30 µM und SI-Werten von über 2 nur mäßig spezifisch antiviral wirksam. Während JK80 vermutlich den Eintritt des MV in die Zellen verhindert, hemmt QD6-8 die intrazelluläre Virusreplikation und wäre im Hinblick auf die Entwicklung neuartiger, spezifischer Medikamente gegen die Maserninfektion von grossem Interesse. Eine Zielmolekülanalyse der Substanz und die Testung anderer Derivate könnten Aufschluss darüber geben, wie Substanzen aussehen müssten, die eine spezifische Hemmung der intrazellulären Replikation bewirken können. Der Naturstoff Droseron könnte mit einer spezifischen Hemmung (IC50 ca. 10 µM; SIWert 6 im Fluoreszenzreader, bzw. IC50 ca. 2 µM; SI-Wert 30 in der Titration) eine mögliche Leitsubstanz für einen neuen MV-Inhibitor darstellen. Allerdings waren alle bisher getesteten Droseron-Derivate entweder weniger inhibitorisch wirksam oder deutlich zytotoxischer als Droseron selbst. Die Ergebnisse der Infektionshemmversuche mit Zugabe von Droseron vor, während oder nach der Infektion mit MV sprechen dafür, dass Droseron den Eintritt des Virus in die Zelle stört.
ADHS-Patienten im Alter von 8-12 Jahren wurde ein Ruhe-EEG von 10 Minuten jeweils mediziert sowie medikamentennüchtern abgeleitet und mittels des Vigilanzalgorithmus Leipzig von Hegerl und Hensch (2012) ausgewertet und den bei gesunden Kontrollkindern gleichen Alters gemessenen EEG-Frequenzen nach Auswertung durch die gleiche Methode gegenübergestellt.
Polyneuropathien (PNP) können zu einer Reorganisation der nodalen und paranodalen Membranproteine mit in der Folge fehlerhafter Axon-Schwann-Zell-Interaktionen führen. Im Rahmen der Basisdiagnostik einer Polyneuropathie haben sich Hautbiopsien als weniger invasive Ergänzung zur Suralisbiopsie mit einem geringen Nebenwirkungsrisiko entwickelt. Die Morphologie dermaler Nervenfasern lässt sich mittels Immunohistochemie in der Haut gezielt untersuchen. In der vorliegenden Studie wurde die Hypothese überprüft, ob pathologisch auffällige Ranvier-Schnürringe Hinweise auf Unterschiede bei PNP-Subgruppen und Schädigungsmuster liefern. Daneben wurden die Hypothesen überprüft, ob Entzündungszellen an myelinisierten Nervenfasern kolokalisiert nachweisbar sind und ob Hautbiopsien einen zusätzlichen Nutzen zur PNP-Basisdiagnostik liefern. Von 92 Patienten wurden Hautbiopsien von Finger, Ober-und Unterschenkel wurden entnommen, daraus gewonnene myelinisierte Nervenfasern der Haut wurden mittels immunohistochemischer Antikörper-Doppelfärbungen analysiert. Neuropathische Schädigungsformen vom axonalen und demyelinisierenden Typ zeigten keine signifikante Korrelation mit dem Auftreten von verlängerten Ranvier-Schnürringen und der Dispersion charakteristischer paranodaler und nodaler Membranproteine (Neurofascin, Caspr, Pan-Natrium-Kanäle). Kolokalisierte Entzündungszellen an myelinisierten Nervenfasern bei entzündlichen PNP ließen sich nicht nachweisen. PNP-Subgruppen zeigten keine signifikanten Unterschiede in Hinblick auf eine pathologische nodale oder paranodale Organisation. Der Zusatznutzen von Hautbiopsien in der PNP-Basisdiagnostik kann in Bezug auf die vorliegende Arbeit nur eingeschränkt bestätigt werden. Da Fingerbiopsien im Vergleich zu Proben aus Ober- und Unterschenkel eine signifikant höhere Dichte myelinisierter Nervenbündel pro Fläche Dermis aufweisen, wäre es durchaus denkbar, zukünftig primär Fingerbiopsien zu entnehmen um diese auf etwaige pathologische Veränderungen infolge neuropathischer Erkrankungen zu untersuchen. Anamnese, Basisdiagnostik und klinischer Befund erbringen nach wie vor den wichtigsten Beitrag zur PNP-Diagnostik.
Die Arbeit zeigt, dass die Symptome der HPP sehr variabel und unterschiedlich stark auftreten können. Dies erschwert die klinische Diagnosestellung der Erkrankung. Nahezu alle Patienten berichteten von starken Knochen-, Gelenk,- und Muskelschmerzen, von Karies und Parodontose sowie von vermehrten Frakturen, die zum Teil weitere chronische Schmerzen und Wiederholungsfrakturen erzeugen. Eine deutlich verminderte Leistungsfähigkeit im Vergleich zu Gleichaltrigen wurde ebenso häufig angegeben. Es konnte keine eindeutige Phänotyp - Genotyp Korrelation gefunden werden, allerdings geben die Daten einen deutlichen Hinweis, dass Patienten mit zwei Mutationen am stärksten symptomatisch betroffen sind.
Ebenfalls konnten keine Unterschiede zwischen dominant negativen Mutationen und nicht dominant negativen Mutationen gefunden werden.
Die Studienergebnisse stützen das Konzept, dass das periphere Nervensystem zu Schmerzen beim Fibromyalgie-Syndrom (FMS) beiträgt. An der Neurologischen Universitätsklinik Würzburg wurden 53 FMS Patientinnen und 35 gesunde Kontrollen rekrutiert, ausführlich anamnestiziert inklusive spezieller Schmerzfragebögen, neurologisch und mittels spezieller Tests auf eine Störung der kleinkalibrigen A-delta- und C-Nervenfasern untersucht. Hierzu gehörte eine quantitative sensorische Testung mit Pleasant touch Untersuchung und die schmerz-assoziierten elektrisch-evoziierten Potentiale für die Kleinfaserfunktion und die corneale confocale Mikroskopie, sowie die Analyse von Hautstanbiopsien für die Kleinfasermorphologie.
Im Unterschied zu gesunden Kontrollen wiesen die FMS Patientinnen eine Reduktion, als auch eine Funktionsänderung der kleinkalibrigen Nervenfasern auf. Des Weiteren konnten wir aus der heterogenen Patientenpopulation anhand von unterschiedlichen Nervenfaserdichten der Haut eine Subgruppe mit generalisierter Reduktion der Hautinnervation identifizieren, die besonders schwer betroffen ist.
Diese Subgruppenanalysen können künftig von großer Bedeutung für die
Therapiewahl sein.
Bei Erwachsenen ist ein Zusammenhang zwischen Depressionen und Herzerkrankungen bekannt. Als möglicher Mechanismus hierfür gilt eine Veränderung der kardialen autonomen Funktion, messbar über eine verminderte Herzfrequenzvariabilität (HRV) und eine höhere Herzfrequenz. Es finden sich in der Literatur erste Hinweise, dass auch bei Kindern und Jugendlichen mit Depressionen Veränderungen der kardialen autonomen Regulation zu beobachten sind. In der vorliegenden Studie an der Klinik und Poliklinik für Kinder- und Jugendpsychiatrie, Psychosomatik und Psychotherapie Würzburg wurde erstmals die kardiale autonome Funktion bei depressiven Kindern und Jugendlichen mit Hilfe von Langzeit-EKGs untersucht. Geprüft wurde hierbei, ob depressive Störungen im Kindes- und Jugendalter mit einer gestörten kardialen autonomen Regulation in Form einer verminderten HRV und einer erhöhten Herzfrequenz vergesellschaftet sind. Zudem wurde der Einfluss einer antidepressiven Therapie untersucht. Die Ergebnisse der Studie zeigen, dass depressive Kinder und Jugendliche im Vergleich zu gesunden eine signifikant höhere mittlere Herzfrequenz im Langzeit-EKG aufweisen. Zudem hatten sie leicht verminderte HRV-Parameter, wobei dieser Unterschied nicht statistisch signifikant war. Eine Veränderung der HRV oder der Herzfrequenz im Therapieverlauf konnte nicht belegt werden. Weitere größere Studien sind nötig, um die Zusammenhänge zwischen Depressionen und Veränderungen der kardialen autonomen Funktion im Kindes- und Jugendalter zu erforschen.
In der vorliegenden Studie wurden der Effekt und die Haltbarkeit der Rekonstruktion der Trikuspidalklappe mittels Ringen und Bändern sowie die postoperative Lebensqualität der Patienten untersucht. Als Ausgangspunkt dieser Arbeit dienten die ausführlichen Krankenakten der 141 Patienten, welche vom 01.01.2009 bis zum 31.12.2011 operiert wurden sowie die von den Patienten ausgefüllten Fragebögen. Anhand der Fragebögen konnte die Lebensqualität der Patienten beurteilt werden. Beim gesamten Patientenkollektiv wurden überwiegend neue Ringmodelle, wie steife CE MC3 Ringe und flexible SJM Tailor Bänder verwendet. Bei postoperativen echokardiographischen Kontrollen sowie telefonischen Follow-ups wurden keine reoperationsnotwendigen TK-Pathologien festgestellt. Anhand der Kaplan-Meier-Analyse betrug das 5-Jahres-Überleben in dieser Studie 70%. Die Beurteilung der Lebensqualität anhand COOP-Wonca-Fragebogen zeigte eine statistisch relevante postoperative Besserung im Bereich der somatischen, psychologischen und sozialen Funktion. Diese Arbeit bestätigt, dass die Anwendung von modernen Bändern und Ringen zu einer dauerhaft kompetenten TK führt, und dass sich der Allgemeinzustand der Patienten gegenüber dem Zustand vor der Operation verbessert.
In unseren Untersuchungen zogen wir einen Vergleich zwischen dem Verlauf der Immunkonstitution bei Kindern nach CD3/19 depletierter Stammzell-Transplantation und historischen Daten von Kindern nach CD34+ selektionierter Stammzell-Transplantation.
In der Frühphase nach Transplantation zeigen sich in unseren Ergebnissen signifikante Vorteile nach CD3/19 Depletion, insbesondere hinsichtlich des numerischen NK- sowie der T-Zell-Rekonstruktion. Auch qualitativ sahen wir vor Tag +100 anhand unserer Ergebnisse der CDR3-Längen-Analyse des TZR-Repertoires eine höhere Komplexizität nach CD3/19-Depletion. Zu einem späteren Zeitpunkt (>200d) zeigte sich auf Seite der CD34+-Selektionierten eine etwas bessere Repertoirekomplexizität.
Zusammenfassend sehen wir die CD3/19-Depletion als eine positive Weiterentwicklung der T-Zell-Depletion bei haploidenter Stammzelltransplantation.
Unterschiede in Frontaler Kortex Oxygenierung in zweierlei Risikogruppen der Alzheimer Demenz

Die verbesserte medizinische Versorgung führt zu einer zunehmenden Lebenserwartung unserer Gesellschaft. Damit steigt auch die sozioökonomische Relevanz neurodegenerativer Erkrankungen kontinuierlich. Für die Alzheimer Demenz (AD), die dabei die häufigste Ursache darstellt, stehen bisher keine krankheitsmodifizierenden Behandlungsoptionen zur Verfügung. Die lange präklinische Phase der Erkrankung birgt jedoch großes Potential für die Entwicklung neuer Behandlungsoptionen. Das Untersuchen von Risikogruppen ist für die Identifikation von Prädiktoren einer späteren AD Manifestation von besonderem Interesse. In diesem Zusammenhang werden insbesondere das Vorliegen genetischer Risikokonstellationen, wie dem Apolipoprotein E (APOE) Ɛ4-Allel, sowie kognitiver Risikofaktoren, wie der „leichten kognitiven Beeinträchtigung“ (MCI), diskutiert. Die Identifikation präklinischer Aktivierungsunterschiede in relevanten Gehirnregionen von Risikogruppen kann als Basis für die Entwicklung neurofunktioneller Früherkennungs-Marker dienen. Der präfrontale Kortex (PFC), welcher mit der Steuerung von Exekutivfunktionen assoziiert wird, hat sich in diesem Zusammenhang in bisherigen Studien als eine relevante Schlüsselregion manifestiert. Aufgrund der aufwendigen und kostenintensiven bildgebenden Untersuchungsmethoden, sind die genauen Prozesse jedoch noch unklar.
Ziel der vorliegenden Arbeit war es daher, Unterschiede in der PFC Oxygenierung in zweierlei Risikogruppen der AD mit einer kostengünstigeren Bildgebungsmethode, der funktionellen Nahinfrarot Spektroskopie (fNIRS), zu untersuchen. Dafür wurde in einem ersten Schritt, der Trailmaking Test (TMT), ein weitverbreiteter neuropsychologischer Test zur Erfassung exekutiver Funktionen, für fNIRS implementiert. Als Grundlage für die Untersuchung frühpathologischer Prozesse, wurden zunächst gesunde Alterungsprozesse betrachtet. Der Vergleich von jungen und älteren Probanden (n = 20 pro Gruppe) wies neben der Eignung der Testimplementierung für fNIRS auf eine spezifische bilaterale PFC Oxygenierung hin, welche bei jungen Probanden rechtshemisphärisch lateralisiert war. Ältere Probanden hingegen zeigten bei vergleichbaren Verhaltensdaten insgesamt mehr signifikante Kanäle sowie eine Abnahme der Lateralisierung. Dies kann als zusätzlicher Bedarf an Ressourcen in gesunden Alterungsprozessen interpretiert werden.
Im Rahmen der Hauptstudie wurden anschließend insgesamt 604 ältere Probanden im Alter von 70 bis 76 Jahren untersucht. Zunächst wurde die genetische Risikogruppe der Ɛ4-Allel-Träger (n = 78) mit den neutralen Ɛ3-Allel-Trägern (n = 216) und den Trägern des als protektiv geltenden Ɛ2-Allels (n = 50) verglichen. Hierbei zeigte sich eine geringere Oxygenierung der Risikogruppe bei geringer Aufgabenschwierigkeit, während sich ein erhöhter Oxygenierungsanstieg im medialen PFC mit steigender Aufgabenschwierigkeit zeigte. Dies deutet auf einen erhöhten Bedarf an neuronalen Kontrollmechanismen der Risikogruppe zur Bewältigung der steigenden Aufgabenschwierigkeit hin. Die protektive Gruppe zeigte hingegen eine erhöhte Oxygenierung im ventralen PFC mit steigender Aufgabenschwierigkeit, was möglicherweise auf einen präventiven Effekt hindeuten könnte.
Weiterführend wurden MCI-Patienten mit gesunden Probanden (n = 57 pro Gruppe) hinsichtlich des kognitiven Risikofaktors verglichen. Hierbei zeigte sich ein punktuell reduzierter Oxygenierunganstieg der MCI Patienten mit steigender Aufgabenschwierigkeit vor allem im ventralen PFC bei ebenfalls stabiler Verhaltensleistung. Die gefundene Reduktion könnte ein Zeichen für eine aufgebrauchte kognitive Reserve sein, welche Einbußen auf Verhaltensebene voranzugehen scheint.
Diese charakteristischen Unterschiede in den frontalen Oxygenierungsmustern von Risikogruppen (APOE, MCI) könnten als Biomarker zur Früherkennung von AD noch vor dem Auftreten kognitiver Einbußen dienen. Die fNIRS-Untersuchung während der Durchführung des TMT hat sich in diesem Zusammenhang als potentielles Instrument zur Frühdiagnose der präklinischen Phase der AD als geeignet erwiesen. Die Ergebnisse werden unter Einbezug des wissenschaftlichen Kontexts interpretiert und Implikationen für weitere notwendige Studien sowie die klinische Anwendbarkeit diskutiert.
Forward Collision Alarms (FCA) intend to signal hazardous traffic situations and the need for an immediate corrective driver response. However, data of naturalistic driving studies revealed that approximately the half of all alarms activated by conventional FCA systems represented unnecessary alarms. In these situations, the alarm activation was correct according to the implemented algorithm, whereas the alarms led to no or only minimal driver responses. Psychological research can make an important contribution to understand drivers’ needs when interacting with driver assistance systems.
The overarching objective of this thesis was to gain a systematic understanding of psychological factors and processes that influence drivers’ perceived need for assistance in potential collision situations. To elucidate under which conditions drivers perceive alarms as unnecessary, a theoretical framework of drivers’ subjective alarm evaluation was developed. A further goal was to investigate the impact of unnecessary alarms on drivers’ responses and acceptance. Four driving simulator studies were carried out to examine the outlined research questions.
In line with the hypotheses derived from the theoretical framework, the results suggest that drivers’ perceived need for assistance is determined by their retrospective subjective hazard perception. While predictions of conventional FCA systems are exclusively based on physical measurements resulting in a time to collision, human drivers additionally consider their own manoeuvre intentions and those attributed to other road users to anticipate the further course of a potentially critical situation. When drivers anticipate a dissolving outcome of a potential conflict, they perceive the situation as less hazardous than the system. Based on this discrepancy, the system would activate an alarm, while drivers’ perceived need for assistance is low. To sum up, the described factors and processes cause drivers to perceive certain alarms as unnecessary. Although drivers accept unnecessary alarms less than useful alarms, unnecessary alarms do not reduce their overall system acceptance. While unnecessary alarms cause moderate driver responses in the short term, the intensity of responses decrease with multiple exposures to unnecessary alarms. However, overall, effects of unnecessary alarms on drivers’ alarm responses and acceptance seem to be rather uncritical.
This thesis provides insights into human factors that explain when FCAs are perceived as unnecessary. These factors might contribute to design FCA systems tailored to drivers’ needs.
The work at hand discusses various universality results for locally univalent and conformal metrics.
In Chapter 2 several interesting approximation results are discussed. Runge-type Theorems for holomorphic and meromorphic locally univalent functions are shown. A well-known local approximation theorem for harmonic functions due to Keldysh is generalized to solutions of the curvature equation.
In Chapter 3 and 4 these approximation theorems are used to establish universality results for locally univalent functions and conformal metrics. In particular locally univalent analogues for well-known universality results due Birkhoff, Seidel & Walsh and Heins are shown.
Understanding extinction debts: spatio-temporal scales, mechanisms and a roadmap for future research

Extinction debt refers to delayed species extinctions expected as a consequence of ecosystem perturbation. Quantifying such extinctions and investigating long‐term consequences of perturbations has proven challenging, because perturbations are not isolated and occur across various spatial and temporal scales, from local habitat losses to global warming. Additionally, the relative importance of eco‐evolutionary processes varies across scales, because levels of ecological organization, i.e. individuals, (meta)populations and (meta)communities, respond hierarchically to perturbations. To summarize our current knowledge of the scales and mechanisms influencing extinction debts, we reviewed recent empirical, theoretical and methodological studies addressing either the spatio–temporal scales of extinction debts or the eco‐evolutionary mechanisms delaying extinctions. Extinction debts were detected across a range of ecosystems and taxonomic groups, with estimates ranging from 9 to 90% of current species richness. The duration over which debts have been sustained varies from 5 to 570 yr, and projections of the total period required to settle a debt can extend to 1000 yr. Reported causes of delayed extinctions are 1) life‐history traits that prolong individual survival, and 2) population and metapopulation dynamics that maintain populations under deteriorated conditions. Other potential factors that may extend survival time such as microevolutionary dynamics, or delayed extinctions of interaction partners, have rarely been analyzed. Therefore, we propose a roadmap for future research with three key avenues: 1) the microevolutionary dynamics of extinction processes, 2) the disjunctive loss of interacting species and 3) the impact of multiple regimes of perturbation on the payment of debts. For their ability to integrate processes occurring at different levels of ecological organization, we highlight mechanistic simulation models as tools to address these knowledge gaps and to deepen our understanding of extinction dynamics.
Despite its history of more than one hundred years, the phenomenon of
superconductivity has not lost any of its allure. During that time the concept
and perception of the superconducting state - both from an experimental and
theoretical point of view - has evolved in way that has
triggered increasing interest. What was initially believed to simply be the
disappearance of electrical resistivity, turned out to be a universal and
inevitable result of quantum statistics, characterized by many more
aspects apart from its zero resistivity. The insights of
BCS-theory eventually helped to uncover its deep connection to particle physics
and consequently led to the formulation of the Anderson-Higgs-mechanism. The
very core of this theory is the concept of gauge symmetry (breaking). Within the
framework of condensed-matter theory, gauge invariance is only one of several
symmetry groups which are crucial for the description and classification of
superconducting states. \\
In this thesis, we employ time-reversal, inversion, point group and spin
symmetries to investigate and derive possible Hamiltonians featuring spin-orbit
interaction in two and three spatial dimensions.
In particular, this thesis aims at a generalization of existing numerical
concepts to open up the path to spin-orbit coupled (non)centrosymmetric
superconductors in multi-orbital models.
This is done in a two-fold way: On the one hand, we formulate - based on the
Kohn-Luttinger effect - the perturbative renormalization group in the
weak-coupling limit. On the other hand, we define the spinful flow equations of
the effective action in the framework of functional renormalization, which is
valid for finite interaction strength as well. Both perturbative and functional
renormalization groups produce a low-energy effective (spinful) theory that
eventually gives rise to a particular superconducting state, which is investigated
on the level of the irreducible two-particle vertex. The symbiotic relationship
between both perturbative and functional renormalization can be traced back to
the fact that, while the perturbative renormalization at infinitesimal coupling
is only capable of dealing with the Cooper instability, the functional
renormalization can investigate a plethora of instabilities both in the
particle-particle and particle-hole channels. \\
Time-reversal and inversion are the two key symmetries, which are being used to
discriminate between two scenarios. If both time-reversal and inversion symmetry
are present, the Fermi surface will be two-fold degenerate and characterized by a
pseudospin degree of freedom. In contrast, if inversion symmetry is broken, the
Fermi surface will be spin-split and labeled by helicity. In both cases, we
construct the symmetry allowed states in the particle-particle as well as the
particle-hole channel. The methods presented are formally unified and implemented
in a modern object-oriented reusable and extendable C++ code.
This methodological implementation is employed to one member of both families of
pseudospin and helicity characterized systems. For the pseudospin case, we choose
the intriguing matter of strontium ruthenate, which has been heavily
investigated for already twenty-four years, but still keeps puzzling researchers.
Finally, as the helicity based application, we consider the oxide heterostructure
LaAlO$_{3}$/SrTiO$_{3}$, which became famous for its highly mobile two-
dimensional electron gas and is suspected to host topological superconductivity.
For the understanding of the variable, transient and non-thermal universe, unbiased long-term monitoring is crucial. To constrain the emission mechanisms at the highest energies, it is important to characterize the very high energy emission and its correlation with observations at other wavelengths. At very high energies, only a limited number of instruments is available. This article reviews the current status of monitoring of the extra-galactic sky at TeV energies.
Background
The vast majority of cases with Beckwith-Wiedemann syndrome (BWS) are caused by a molecular defect in the imprinted chromosome region 11p15.5. The underlying mechanisms include epimutations, uniparental disomy, copy number variations, and structural rearrangements. In addition, maternal loss-of-function mutations in CDKN1C are found. Despite growing knowledge on BWS pathogenesis, up to 20% of patients with BWS phenotype remain without molecular diagnosis.
Case presentation
Herein, we report an Iranian family with two females affected with BWS in different generations. Bisulfite pyrosequencing revealed hypermethylation of the H19/IGF2: intergenic differentially methylated region (IG DMR), also known as imprinting center 1 (IC1) and hypomethylation of the KCNQ1OT1: transcriptional start site (TSS) DMR (IC2). Array CGH demonstrated an 8 Mb duplication on chromosome 11p15.5p15.4 (205,827-8,150,933) and a 1 Mb deletion on chromosome 9p24.3 (209,020-1,288,114). Chromosome painting revealed that this duplication-deficiency in both patients is due to unbalanced segregation of a paternal reciprocal t(9;11)(p24.3;p15.4) translocation.
Conclusions
This is the first report of a paternally inherited unbalanced translocation between the chromosome 9 and 11 short arms underlying familial BWS. Copy number variations involving the 11p15.5 region are detected by the consensus diagnostic algorithm. However, in complex cases which do not only affect the BWS region itself, characterization of submicroscopic chromosome rearrangements can assist to estimate the recurrence risk and possible phenotypic outcomes.
Polymer micelles are an attractive means to solubilize water insoluble compounds such as drugs. Drug loading, formulations stability and control over drug release are crucial factors for drug‐loaded polymer micelles. The interactions between the polymeric host and the guest molecules are considered critical to control these factors but typically barely understood. Here, we compare two isomeric polymer micelles, one of which enables ultra‐high curcumin loading exceeding 50 wt.%, while the other allows a drug loading of only 25 wt.%. In the low capacity micelles, steady‐state fluorescence revealed a very unusual feature of curcumin fluorescence, a high energy emission at 510 nm. Time‐resolved fluorescence upconversion showed that the fluorescence life time of the corresponding species is too short in the high‐capacity micelles, preventing an observable emission in steady‐state. Therefore, contrary to common perception, stronger interactions between host and guest can be detrimental to the drug loading in polymer micelles.
The nuclear envelope serves as important messenger RNA (mRNA) surveillance system. In yeast and human, several control systems act in parallel to prevent nuclear export of unprocessed mRNAs. Trypanosomes lack homologues to most of the involved proteins and their nuclear mRNA metabolism is non-conventional exemplified by polycistronic transcription and mRNA processing by trans-splicing. We here visualized nuclear export in trypanosomes by intra- and intermolecular multi-colour single molecule FISH. We found that, in striking contrast to other eukaryotes, the initiation of nuclear export requires neither the completion of transcription nor splicing. Nevertheless, we show that unspliced mRNAs are mostly prevented from reaching the nucleus-distant cytoplasm and instead accumulate at the nuclear periphery in cytoplasmic nuclear periphery granules (NPGs). Further characterization of NPGs by electron microscopy and proteomics revealed that the granules are located at the cytoplasmic site of the nuclear pores and contain most cytoplasmic RNA-binding proteins but none of the major translation initiation factors, consistent with a function in preventing faulty mRNAs from reaching translation. Our data indicate that trypanosomes regulate the completion of nuclear export, rather than the initiation. Nuclear export control remains poorly understood, in any organism, and the described way of control may not be restricted to trypanosomes.
Despite advances in cartilage repair strategies, treatment of focal chondral lesions remains an important challenge to prevent osteoarthritis. Articular cartilage is organized into several layers and lack of zonal organization of current grafts is held responsible for insufficient biomechanical and biochemical quality of repair-tissue. The aim was to develop a zonal approach for cartilage regeneration to determine whether the outcome can be improved compared to a non-zonal strategy. Hydrogel-filled polycaprolactone (PCL)-constructs with a chondrocyte-seeded upper-layer deemed to induce hyaline cartilage and a mesenchymal stromal cell (MSC)-containing bottom-layer deemed to induce calcified cartilage were compared to chondrocyte-based non-zonal grafts in a minipig model. Grafts showed comparable hardness at implantation and did not cause visible signs of inflammation. After 6 months, X-ray microtomography (µCT)-analysis revealed significant bone-loss in both treatment groups compared to empty controls. PCL-enforcement and some hydrogel-remnants were retained in all defects, but most implants were pressed into the subchondral bone. Despite important heterogeneities, both treatments reached a significantly lower modified O’Driscoll-score compared to empty controls. Thus, PCL may have induced bone-erosion during joint loading and misplacement of grafts in vivo precluding adequate permanent orientation of zones compared to surrounding native cartilage.
In T cells, as in all other cells of the body, sphingolipids form important structural components of membranes. Due to metabolic modifications, sphingolipids additionally play an active part in the signaling of cell surface receptors of T cells like the T cell receptor or the co-stimulatory molecule CD28. Moreover, the sphingolipid composition of their membranes crucially affects the integrity and function of subcellular compartments such as the lysosome. Previously, studying sphingolipid metabolism has been severely hampered by the limited number of analytical methods/model systems available. Besides well-established high resolution mass spectrometry new tools are now available like novel minimally modified sphingolipid subspecies for click chemistry as well as recently generated mouse mutants with deficiencies/overexpression of sphingolipid-modifying enzymes. Making use of these tools we and others discovered that the sphingolipid sphingomyelin is metabolized to ceramide to different degrees in distinct T cell subpopulations of mice and humans. This knowledge has already been translated into novel immunomodulatory approaches in mice and will in the future hopefully also be applicable to humans. In this paper we are, thus, summarizing the most recent findings on the impact of sphingolipid metabolism on T cell activation, differentiation, and effector functions. Moreover, we are discussing the therapeutic concepts arising from these insights and drugs or drug candidates which are already in clinical use or could be developed for clinical use in patients with diseases as distant as major depression and chronic viral infection.
In Brassicaceae werden bei einer Gewebszerstörung unreaktive Glukosinolate durch das Enzym Myrosinase hydrolysiert. Es entstehen reaktive Substanzen wie Isothiocyanate (ITCs). Da diese Reaktion sehr schnell erfolgt wird sie auch als Senföl-Glukosid-Bombe bezeichnet. In Arabidopsis thaliana erfolgt nach Verwundung und Pathogeninfektion eine massive Akkumulation des ITCs Sulforaphan (SF), welches eine reaktive elektophile Spezies (RES) darstellt. Zu der Gruppe der RES zählen auch einige Oxylipine mit einer α,β-ungesättigten Carbonylgruppen wie 12-oxo-Phytodiensäure (OPDA) oder Phytoprostan A1 (PPA1). Die Fähigkeit der kovalenten Modifikation von Peptiden und Proteinen gilt als essentiell sowohl für die toxischen als auch die Gen-induzierenden Eigenschaften der RES. Neben ihrer Reaktivität spielt auch die Lipophilie eine Rolle für die Fähigkeit über Membranen zu diffundieren und unspezifisch an Proteine zu binden.
Die in der vorliegenden Arbeit durchgeführten Transkriptomanalysen an Arabidopsis-Keimlingen mit sub-toxischen Konzentrationen von SF, Benzylisothiocyanat (BITC) und dem Oxylipin Prostaglandin A1 (PGA1) zeigten, dass strukturell sehr verschiedene RES einen gemeinsamen Satz von 55 Genen induzieren. Unter diesen befanden sich verschiedene Hitzeschock-, Stressassoziierte- und Detoxifizierungsgene. Diese Ergebnisse deuten darauf hin, dass die Aktivierung über eine Muster-spezifische Erkennung der RES erfolgt. Als einen möglichen Mechanismus der RES-vermittelten Geninduktion wird die Regulation durch die Veränderung des zellulären Redox-Potentials als Folge kovalenter Modifikation von GSH durch RES diskutiert. Die Untersuchung der GSH-Gehalte sowie des Redox-Potential nach Behandlung mit sub-toxischen RES-Konzentrationen in Arabidopsis-Keimlingen zeigte jedoch unter den getesteten Bedingungen keine Veränderung.
Neben dem Erkennungs- und Signaltransduktionsmechanismus ist auch die biologische Bedeutung von RES für die Vermittlung einer Stresstoleranz noch weitgehend unklar. Durch die Untersuchung der Genexpression in Arabidopsis-Pflanzen nach Verwundung konnte gezeigt werden, dass eine wundinduzierte Akkumulation von SF zur Induktion einiger Gene der Hitzeschockreaktion (HSR) im Wildtyp, jedoch nicht in der myrosinase-defiziten tgg1tgg2-Mutante führte. Auch in der Transkriptomanalyse war nach RES-Gabe ebenfalls eine starke Induktion hitze-responsiver Gene, deren Regulation über den Masterregulator dem Hitzeschock-TF A1 vermittelt wird, zu beobachten. Besonders die Induktion der HSPs, welche als Chaperone fungieren und damit Thiolgruppen von Proteinen vor Modifikation schützen können, haben vermutlich bei chemischer Intoxikation protektive Eigenschaften für die Zellen. Tatsächlich zeigte sich unter den gewählten Bedingungen die hsfa1a,b,d,e-Mutante empfindlicher gegenüber ITCs als der Wildtyp. Die Fähigkeit, eine HSR ausbilden zu können, scheint in Arabidopsis bei chemischer Intoxikation eine bedeutende Rolle zu spielen. Eine Vorbehandlung mit RES wie SF, BITC oder dem HSP90-Inhibitor Radicicol in Arabidopsis-Keimlingen konnte eine Schutzwirkung vor chemischer Intoxikation vermitteln. Dies erfolgte jedoch nicht nach Behandlung mit moderater Hitze (zwei Stunden, 37 °C). Somit scheint die HSR alleine nicht ausreichend für den Aufbau eines effektiven Schutzes vor BITC-Intoxikation zu sein.
Als metabolische Antwort von Arabidopsis-Keimlingen auf Intoxikation mit RES konnte eine konzentrationsabhängige Senkung der maximalen Quantenausbeute am Photosystem II (PSII), sowie gleichzeitig eine Akkumulation an TAG-Spezies beobachtet werden. Diese metabolische Reaktion ist in der Literatur bereits als Schutz gegen Hitzestress beschrieben. Die Bedeutung der TAG-Akkumulation nach chemischem ITC-Stress ist noch unklar.
The transport of glucose across the cell plasma membrane is vital to most mammalian cells. The glucose transporter (GLUT; also called SLC2A) family of transmembrane solute carriers is responsible for this function in vivo. GLUT proteins encompass 14 different isoforms in humans with different cell type-specific expression patterns and activities. Central to glucose utilization and delivery in the brain is the neuronally expressed GLUT3. Recent research has shown an involvement of GLUT3 genetic variation or altered expression in several different brain disorders, including Huntington’s and Alzheimer’s diseases. Furthermore, GLUT3 was identified as a potential risk gene for multiple psychiatric disorders. To study the role of GLUT3 in brain function and disease a more detailed knowledge of its expression in model organisms is needed. Zebrafish (Danio rerio) has in recent years gained popularity as a model organism for brain research and is now well-established for modeling psychiatric disorders. Here, we have analyzed the sequence of GLUT3 orthologs and identified two paralogous genes in the zebrafish, slc2a3a and slc2a3b. Interestingly, the Glut3b protein sequence contains a unique stretch of amino acids, which may be important for functional regulation. The slc2a3a transcript is detectable in the central nervous system including distinct cellular populations in telencephalon, diencephalon, mesencephalon and rhombencephalon at embryonic and larval stages. Conversely, the slc2a3b transcript shows a rather diffuse expression pattern at different embryonic stages and brain regions. Expression of slc2a3a is maintained in the adult brain and is found in the telencephalon, diencephalon, mesencephalon, cerebellum and medulla oblongata. The slc2a3b transcripts are present in overlapping as well as distinct regions compared to slc2a3a. Double in situ hybridizations were used to demonstrate that slc2a3a is expressed by some GABAergic neurons at embryonic stages. This detailed description of zebrafish slc2a3a and slc2a3b expression at developmental and adult stages paves the way for further investigations of normal GLUT3 function and its role in brain disorders.
TRAF2 controls death receptor-induced caspase-8 processing and facilitates proinflammatory signaling

Tumor necrosis factor (TNF) receptor associated factor-2 (TRAF2) knockout (KO) cells were generated to investigate the role of TRAF2 in signaling by TNFR1 and the CD95-type death receptors (DRs) TRAILR1/2 and CD95. To prevent negative selection effects arising from the increased cell death sensitivity of TRAF2-deficient cells, cell lines were used for the generation of the TRAF2 KO variants that were protected from DR-induced apoptosis downstream of caspase-8 activation. As already described in the literature, TRAF2 KO cells displayed enhanced constitutive alternative NFκB signaling and reduced TNFR1-induced activation of the classical NFκB pathway. There was furthermore a significant but only partial reduction in CD95-type DR-induced upregulation of the proinflammatory NFκB-regulated cytokine interleukin-8 (IL8), which could be reversed by reexpression of TRAF2. In contrast, expression of the TRAF2-related TRAF1 protein failed to functionally restore TRAF2 deficiency. TRAF2 deficiency resulted furthermore in enhanced procaspase-8 processing by DRs, but this surprisingly came along with a reduction in net caspase-8 activity. In sum, our data argue for (i) a non-obligate promoting function of TRAF2 in proinflammatory DR signaling and (ii) a yet unrecognized stabilizing effect of TRAF2 on caspase-8 activity.
Das Polyether-Antibiotikum Salinomycin stammt ursprünglich aus der Tierzucht und wurde kürzlich als Inhibitor epithelialer Tumorstammzellen identifiziert. Vor einem möglichen Einsatz in der Onkologie müssen zunächst toxische Effekte von Salinomycin in nicht malignen humanen Zellen evaluiert werden. In dieser Arbeit sollte daher die geno- und zytotoxische Wirkung von Salinomycin auf Zellen humaner nasaler Mukosa und Lymphozyten untersucht werden.
Dazu wurden Zellen humaner nasaler Mukosa (Einzelzellkulturen und Miniorgankulturen) und Lymphozyten von 10 Patienten für 24h mit Salinomycin (0,1 bis 175 μM) inkubiert. Zur Untersuchung der Genotoxizität wurde die Einzelzellmikrogelelektrophorese angewendet. Zur Untersuchung der Zytotoxizität wurden der MTT-Test sowie die Annexin-Propidiumjodid-Durchflusszytometrie durchgeführt. Zusätzlich wurde mit einem Sandwich-ELISA die IL-8-Konzentration in den Überständen der Miniorgankulturen gemessen, um die proinflammatorische Wirkung von Salinomycin bewerten zu können.
Es zeigte sich kein signifikanter Anstieg der DNA-Schädigung der behandelten Zellen im Vergleich zur Negativkontrolle. Im MTT-Test und in der Annexin-Propidiumjodid-Durchflusszytometrie ließ sich ab Konzentrationen von 10-20 μM eine signifikante Reduktion der Vitalität der behandelten Zellen nachweisen. Der Sandwich-ELISA zeigte einen Anstieg der IL-8-Konzentration bei einer Salinomycin-Konzentration von 5 μM und 10 μM.
Diese Ergebnisse lassen darauf schließen, dass Salinomycin in den verwendeten Konzentrationen zytotoxisch und proinflammatorisch aber nicht genotoxisch wirkt. Eine toxische Wirkung auf Tumorstammzellen konnte schon ab 0,5 μM beobachtet werden. Dennoch sollten weitere Untersuchungen folgen, um den genauen Wirkmechanismus von Salinomycin zu klären und dessen zytotoxische Wirkung auf nicht maligne Zellen reduzieren zu können.
Current preclinical models used to evaluate novel therapies for improved healing include both in vitro and in vivo methods. However, ethical concerns related to the use of animals as well as the poor physiological translation between animal and human skin wound healing designate in vitro models as a highly relevant and promising platforms for healing investigation. While current in vitro 3D skin models recapitulate a mature tissue with healing properties, they still represent a simplification of the in vivo conditions, where for example the inflammatory response originating after wound formation involves the contribution of immune cells. Macrophages are among the main contributors to the inflammatory response and regulate its course thanks to their plasticity. Therefore, their implementation into in vitro skin could greatly increase the physiological relevance of the models. As no full-thickness immunocompetent skin model containing macrophages has been reported so far, the parameters necessary for a successful triple co-culture of fibroblasts, keratinocytes and macrophages were here investigated. At first, cell source and culture timed but also an implementation strategy for macrophages were deter-mined. The implementation of macrophages into the skin model focused on the minimization of the culture time to preserve immune cell viability and phenotype, as the environment has a major influence on cell polarization and cytokine production. To this end, incorporation of macrophages in 3D gels prior to the combination with skin models was selected to better mimic the in vivo environment. Em-bedded in collagen hydrogels, macrophages displayed a homogeneous cell distribution within the gel, preserving cell viability, their ability to respond to stimuli and their capability to migrate through the matrix, which are all needed during the involvement of macrophages in the inflammatory response. Once established how to introduce macrophages into skin models, different culture media were evaluated for their effects on primary fibroblasts, keratinocytes and macrophages, to identify a suitable medium composition for the culture of immunocompetent skin. The present work confirmed that each cell type requires a different supplement combination for maintaining functional features and showed for the first time that media that promote and maintain a mature skin structure have negative effects on primary macrophages. Skin differentiation media negatively affected macrophages in terms of viability, morphology, ability to respond to pro- and anti-inflammatory stimuli and to migrate through a collagen gel. The combination of wounded skin equivalents and macrophage-containing gels con-firmed that culture medium inhibits macrophage participation in the inflammatory response that oc-curs after wounding. The described macrophage inclusion method for immunocompetent skin creation is a promising approach for generating more relevant skin models. Further optimization of the co-cul-ture medium will potentially allow mimicking a physiological inflammatory response, enabling to eval-uate the effects novel drugs designed for improved healing on improved in vitro models.
The reactivity of a diruthenium tetrahydride complex towards three selected dihydroboranes was investigated. The use of [DurBH\(_{2}\)] (Dur=2,3,5,6‐Me\(_{4}\)C\(_{6}\)H) and [(Me\(_{3}\)Si)\(_{2}\)NBH\(_{2}\)] led to the formation of bridging borylene complexes of the form [(Cp\(^{*}\)RuH)\(_{2}\)BR] (Cp\(^{*}\)=C\(_{5}\)Me\(_{5}\); 1 a: R=Dur; 1 b: R=N(SiMe\(_{3}\))\(_{2}\)) through oxidative addition of the B−H bonds with concomitant hydrogen liberation. Employing the more electron‐deficient dihydroborane [3,5‐(CF\(_{3}\))\(_{2}\)‐C\(_{6}\)H\(_{3}\)BH\(_{2}\)] led to the formation of an anionic complex bearing a tetraarylated chain of four boron atoms, namely Li(THF)\(_{4}\)[(Cp\(^{*}\)Ru)\(_{2}\)B\(_{4}\)H\(_{5}\)(3,5‐(CF\(_{3}\))\(_{2}\)C\(_{6}\)H\(_{3}\))\(_{4}\)] (4), through an unusual, incomplete threefold dehydrocoupling process. A comparative theoretical investigation of the bonding in a simplified model of 4 and the analogous complex nido‐[1,2(Cp\(^{*}\)Ru)\(_{2}\)(μ‐H)B\(_{4}\)H\(_{9}\)] (I) indicates that there appear to be no classical σ‐bonds between the boron atoms in complex I, whereas in the case of 4 the B\(_{4}\) chain better resembles a network of three B−B σ bonds, the central bond being significantly weaker than the other two.
Dual setting cements composed of an in situ forming hydrogel and a reactive mineral phase combine high compressive strength of the cement with sufficient ductility and bending strength of the polymeric network. Previous studies were focused on the modification with non-degradable hydrogels based on 2-hydroxyethyl methacrylate (HEMA). Here, we describe the synthesis of suitable triblock degradable poly(ethylene glycol)-poly(lactide) (PEG-PLLA) cross-linker to improve the resorption capacity of such composites. A study with four different formulations was established. As reference, pure hydroxyapatite (HA) cements and composites with 40 wt% HEMA in the liquid cement phase were produced. Furthermore, HEMA was modified with 10 wt% of PEG-PLLA cross-linker or a test series containing only 25% cross-linker was chosen for composites with a fully degradable polymeric phase. Hence, we developed suitable systems with increased elasticity and 5-6 times higher toughn ess values in comparison to pure inorganic cement matrix. Furthermore, conversion rate from alpha-tricalcium phosphate (alpha-TCP) to HA was still about 90% for all composite formulations, whereas crystal size decreased. Based on this material development and advancement for a dual setting system, we managed to overcome the drawback of brittleness for pure calcium phosphate cements.
Tropische Infektionskrankheiten sind noch immer die Haupttodesurache in vielen Ländern der Dritten Welt. Unter ihnen ist Malaria neben der Immunschwächekrankheit AIDS und Tuberkulose am weitesten verbreitet. Laut WHO erkrankten allein im Jahr 2016 rund 216 Millionen Menschen an Malaria und weltweit verstarben 445.000 Menschen an den Folgen dieser Infektion. Solange die Wirksamkeit des Impfstoffs RTS,S/AS01 gegen Malaria noch Schwachstellen aufweist und andere Impfstoff-Kanditaten sich noch in präklinischen Testphasen befinden, ist vor allem die Entwicklung neuer Wirkstoffe, auch im Hinblick auf die rasante Ausbreitung von Resistenzen gegen herkömmliche Medikamente, weiterhin eine dringende Aufgabe.
Eine vielversprechende Wirkstoffklasse mit interessanten strukturellen Eigenschaften und einer ungewöhnlichen Biosynthese aus Acetat-Einheiten sind die Naphthylisochinolin-Alkaloide aus den beiden paläotropischen Pflanzenfamilien der Dioncophyllaceae und Ancistrocladaceae. Die Naphthylisochinolin-Alkaloide sind hervozuheben aufgrund ihrer exzellenten Aktivität gegen Plasmodium falciparum, den Erreger der Malaria tropica, sowie wegen ihrer Wirksamkeit gegen Erreger weiterer Krankheiten wie beispielsweise AIDS, Leishmaniose und Afrikanische Trypanosomiasis. Auch im Kampf gegen maligne Erkrankungen wie Leukämie und pankreatischen Krebs werden sie aufgrund ihrer cytotoxischen Eigenschaften als vielversprechende Leitstrukturen betrachtet.
Die strukturell beeindruckendsten Naphthylisochinolin-Alkaloide sind die dimeren Mbandakamine, die von unserer Arbeitsgruppe vor einiger Zeit aus einer kongolesischen Ancistrocladus-Liane isoliert wurden. Sie besitzen sieben stereogene Elemente und sind die ersten natürlich vorkommenden Dimere mit einer höchst unsymmetrischen 6',1''-gekuppelten zentralen Biarylachse. Diese impliziert eine außergewöhnlich hohe sterische Hinderung an der zentralen Achse, wie sie noch in keinem anderen dimeren Naphthylisochinolin-Alkaloid gefunden wurde. Verbunden mit ihren bemerkenswerten und vielseitigen pharmakologischen Wirkeigenschaften sind sie ausgesprochen interessante Moleküle für eine synthetische Erschließung.
Ziel dieser Arbeit war die erstmalige Totalsynthese von Mbandakamin A und B sowie die Synthese ihrer monomeren Hälften 5-epi-Korupensamin E und 8-O-Methylkorupensamin A. Zudem sollten weitere Naphthylisochinolin-Dimere, die bei der Synthese der Mbandakamine anfallen, isoliert und charakterisiert werden. Alle neuen mono- und dimeren Naphthylisochinoline sollten abschließend am Schweizerischen Tropen- und Public-Health-Institut auf ihre biologische Aktivität getestet werden.
Zusätzlich gelang im Rahmen eines Kooperationsprojekts erstmals die stereochemische Charakterisierung des strukturell ganz neuartigen, inhärent chiralen Wolframbiscorrols durch online HPLC-ECD-Analyse in Kombination mit quantenchemischen Rechnungen.
This contribution deals with the phonetic heterogeneity of spoken Spanish in Andalusia in the sector of public auditory media, specifically in the program ¡Anda Levanta! of Canal Fiesta Radio. First, we take into consideration Article 10 of the Statute of the Autonomy of Andalusia, which enhances the protection, promotion, study, and prestige of the Andalusian modalities and its respective variety (cf. Parlamento de Andalucía 2007: 13). Second, we refer to the Libro de Estilo, a mandatory guide for presenters of public audiovisual media in Andalusia since 2014. The results of the qualitative analysis indicate divergences between the presenters and their audience with regard to their use of phonetic characteristics typical of the Andalusian varieties: where the presenters tend to avoid the salient aspects of the varieties, the audience employs a range of phonetic characteristics typical for Andalusian varieties, including some of the characteristics that are considered less prestigious.
Macrophages stand in the first line of defense against a variety of pathogens but are also involved in the maintenance of tissue homeostasis. To fulfill their functions macrophages sense a broad range of pathogen- and damage-associated molecular patterns (PAMPs/DAMPs) by plasma membrane and intracellular pattern recognition receptors (PRRs). Intriguingly, the overwhelming majority of PPRs trigger the production of the pleiotropic cytokine tumor necrosis factor-alpha (TNF). TNF affects almost any type of cell including macrophages themselves. TNF promotes the inflammatory activity of macrophages but also controls macrophage survival and death. TNF exerts its activities by stimulation of two different types of receptors, TNF receptor-1 (TNFR1) and TNFR2, which are both expressed by macrophages. The two TNF receptor types trigger distinct and common signaling pathways that can work in an interconnected manner. Based on a brief general description of major TNF receptor-associated signaling pathways, we focus in this review on research of recent years that revealed insights into the molecular mechanisms how the TNFR1-TNFR2 signaling network controls the life and death balance of macrophages. In particular, we discuss how the TNFR1-TNFR2 signaling network is integrated into PRR signaling.
The knee joint is a complex composite joint containing the C-shaped wedge-like menisci composed of fibrocartilage. Due to their complex composition and structure, they provide mechanical resilience to the knee joint protecting the articular cartilage. Because of the limited repair potential, meniscal injuries do not only affect the meniscus itself but also lead to altered joint homeostasis and inevitably to secondary osteoarthritis.
The meniscus was characterized focusing on its anatomy, structure and meniscal markers such as aggrecan, collagen type I (Col I) and Col II. The components relevant for meniscus tissue engineering, namely cells, Col I scaffolds, biochemical and biomechanical stimuli were studied. Meniscal cells (MCs) were isolated from meniscus, mesenchymal stem cells (MSCs) from bone marrow and dermal microvascular endothelial cells (d-mvECs) from foreskin biopsies. For the human (h) meniscus model, wedge-shape compression of a hMSC-laden Col I gel was successfully established. During three weeks of static culture, the biochemical stimulus transforming growth factor beta-3 (TGF beta-3) led to a compact collagen structure. On day 21, this meniscus model showed high metabolic activity and matrix remodeling as confirmed by matrix metalloproteinases detection. The fibrochondrogenic properties were illustrated by immunohistochemical detection of meniscal markers, significant GAG/DNA increase and increased compressive properties. For further improvement, biomechanical stimulation systems by compression and hydrostatic pressure were designed. As one vascularization approach, direct stimulation with ciclopirox olamine (CPX) significantly increased sprouting of hd-mvEC spheroids even in absence of auxiliary cells such as MSCs. Second, a cell sheet composed of hMSCs and hd-mvECs was fabricated by temperature triggered cell sheet engineering and transferred onto the wedge-shaped meniscus model. Third, a biological vascularized scaffold (BioVaSc-TERM) was re-endothelialized with hd-mvECs providing a viable vascularized network. The vascularized BioVaSc-TERM was suggested as wrapping scaffold of the meniscus model by using two suture techniques, the all-inside-repair (AIR) for the posterior horn, and the outside-in-refixation (OIR) for the anterior horn and the middle part.
This meniscus model for replacing torn menisci is a promising approach to be further optimized regarding vascularization, biochemical and biomechanical stimuli.
Zusammenfassung
Einleitung: Die Inzidenz von Narbenhernien (operativ erworbene Schwachstellen der Bauchwand) ist abhängig von der Art der vorhergegangen Operation, nach Laparaskopien ist sie um einiges niedriger als nach Laparotomien, wird aber mit 2-20% in der Literatur angegeben. Aufgrund der möglichen Komplikationen (Platzbauch, Darminkarzeration, Schmerzen, Funktionseinschränkung, …) stellen Narbenhernien oftmals große Belastungen für die Patienten dar. Die operative Sanierung, in Abhängigkeit von Größe und Lage, wird zumeist durch einbringen eines Netzgewebes erreicht. Dieser Fremdkörper kann seinerseits wieder Komplikationen hervorrufen (Infektionen, Funktionsverlust, Schmerzen, Fisteln), die bis zur Explantation des Netzgewebes führen können. Das Risiko für das Auftreten von Narbenhernien bzw. deren Rezidiven hängt von vielen Faktoren ab, als Risikofaktoren wurden unter anderem Rauchen, männliches Geschlecht, Alter >45 Jahre und ein BMI >25 kg/cm² ausgemacht. Ein Teilbereich des Tissue Engineerings ist die Entwicklung von Modellen, anhand derer in vitro Prozesse des menschlichen Körpers nachvollzogen werden können. Mit dieser Arbeit soll ein Modell etabliert werden Anhand dessen die Untersuchung der Kollagenproduktion und der Netzinkorporation bzw. die Auswirkungen verschiedener Risikofaktoren auf diese Prozesse in vitro ermöglicht werden soll. Weiterhin wurden Studienfragen formuliert, die sich sowohl mit der Durchführbarkeit dieser Methode abzielten, als auch gezielt nach der Stützung der These der „guten und schlechten Heiler“ durch diese Arbeit abzielten. Sowie nach der Vergleichbarkeit der Ergebnisse mit bekannten Kollagenmustern die aus Netzexplantaten bekannt sind. Material und Methode: Für die vorliegende Arbeit wurden Biopsien von Faszien bzw. Narbenhernien im Rahmen einer Operation gewonnen, aus diesen wurden die Fibroblasten isoliert und anschliessend entweder eingefroren bzw. expandiert, um sie in einer Rattenkollagenmatrix mit und ohne synthetischem Netz im dynamisch mechanischen Bioreaktor zu kultivieren. Die Biopsien wurden Anhand der Kollagen I/III Ratio in „gute und schlechte Heiler“ eingruppiert. Anschließend wurden die so gezüchteten Neofaszien HE und Pikrosiriusrot gefärbt um zum einen einen Eindruck von der Verteilung der Fibroblasten innerhalb der Neofaszie zu gewinnen, als auch Aussagen zum Kollagenmuster, der Kollagen I/III Ratio und zur Kollagendensität treffen zu können. Die Dicke der kultivierten Neofaszien wurde sowohl in Sirius als auch in HE Färbung untersucht. Weiterhin wurden RT-PCR und Gene Arrays von Nativgeweben und von Neofaszien mit unterschiedlichen Netztypen durchgeführt. Ergebnisse: Bei gesunden Probanden konnten oftmals nicht genügend Zellen aus den Faszienbiopsaten gewonnen werden, deshalb wurde im Verlauf der Arbeit auf die Gewinnung von gesundem Fasziengewebe als Vergleichsgruppe verzichtet. Fibroblasten von als „schlechten Heilern“ klassifizierten Patienten zeigten meist ein langsameres Wachstum in der Expansionsphase. Der Bioreaktor bereitete kaum Probleme (ein paar Faszien trockneten anfänglich aus, dieses Problem lies sich durch bei Bedarf verkürzten Medienwechselintervallen in den Griff bekommen. Probleme mit Kontaminationen traten nicht auf. Bei den Histologischen Untersuchungen der Neofaszien waren Fibroblasten über den gesamten Bereich der Neofaszie zu sehen, auch in unmittelbarer Umgebung der Netzstrukturen. Die Kollagenmuster stimmten in Ansätzen mit den aus klinischen Netzexplantaten bekannten Mustern überein (Polydirektional bei Polyesternetz, Konzentrisch um die Netzstrukturen bei Polypropylen). Weiterhin war eine verstärkte Kollagenbildung quer zur Druckrichtung des Bioreaktors zu erkennen. Bei der Betrachtung der Dicke der Neofaszien zeigte sich (unter Vorbehalt, aufgrund der geringen Probenanzahl) eine Tendenz zu meist dünneren Faszien bei „schlechten Heilern“ während die Neofaszien von „guten Heilern“ meist eine kleinere Streuung um den Mittelwert zeigten (einheitlicher waren). Die Kollagendensität und auch die Kollagen I/III Ratio lieferten Ergebnisse Anhand derer Gesagt werden kann, dass je höher die Ausgangswerte im Nativgewebe waren, diese mit höherer Wahrscheinlichkeit von den Neofaszien nicht erreicht werden konnten. qRT-PCR und Gene Array zeigten in der Rangkorrelation nach Spearman große Übereinstimmungen. Beantwortung der Studienfragen: Es konnte gezeigt werden, dass es möglich ist Neofaszien mit synthetischen Netzen zu züchten, die über den gesamten Bereich mit Fibroblasten besiedelt waren. Die Ergebnisse der Kollagenmorphologie zeigten in Ansätzen die aus Netzexplantaten bekannten Muster. Bei Kollagen I/III Ratio und Densität war lediglich erkennbar, dass je höher die Ausgangswerte waren, diese mit zunehmender Wahrscheinlichkeit nicht reproduziert werden konnten. Es ließ sich keine Verbindung zwischen der Kollagen I/III Ratio der Histologischen Gewebeproben und den Molekularbiologischen Ergebnissen feststellen. Weiterhin konnte die Theorie der „guten und schlechten Heiler“ molekularbiologisch nicht gestützt werden, da die Proben der als „schlechte Heiler“ Klassifizierten Biopsien stärkere Gemeinsamkeiten mit als „gute Heiler“ Klassifizierten Biopsien aufwiesen als untereinander. Es konnte gezeigt werden dass die Kultur auf die MMP-8 und Elastinproduktion keinen Einfluss zu haben scheint. Diskussion: Im Verlauf der Diskussion wurde darauf hingewiesen, dass die Kollagensynthese, und Sekretion ein komplexes und höchst aktives System darstellt, welches im Rahmen der Wundheilung durch Co-Signalling, und der Interaktion zwischen Fibroblasten und Immunzellen (Makrophagen…) nochmals verändert wird, auch dadurch bedingt, dass Fibroblasten im Verlauf der Wundheilung selbst als immunmodulierende Zellen in Erscheinung treten können. So können weiterhin die Kollagen kodierenden Gene (Col1A1, Col1A2, Col3A1) als Marker für die Kollagenaktivität herangezogen werden, da aber zwischen Synthese und Sekretion des Kollagens ein nicht zu vernachlässigender Teil bereits intrazellulär wieder abgebaut wird kann nur durch Betrachtung dieser Gene die Theorie der „guten und schlechten Heiler“ nicht gestützt werden. Durch die hohe Korrelation der Ergebnisse aus gene-Array und qRT-PCR könnte für die Zukunft vorläufig auf die Durchführung von qRT-PCR verzichtet werden, um eventuell unterschiedliche Pathways mit dem Gene-Array zu identifizieren. Offene Fragen Ausblick und Perspektiven: Da das System der Wundheilung und Kollagensynthese und –Sekretion sehr komplex ist sollte für die Zukunft durch eine Kokultur mit Makrophagen bzw. durch die Zugabe von TNF-α, IL-6, PDGF, G-CSF, GM-CSF, Vitamin C oder Lysyloxidase zum Kulturmedium, geprüft werden ob sich eine Aktivitätsveränderung der Fibroblasten und damit eine andere Neofaszienstruktur erreichen lässt. Weiterhin sollte um einer Verfälschung der Ergebnisse durch das für die Gele verwendete Rattenkollagen vorzubeugen, entweder die Kulturdauer verlängert werden (mit dem Gedanken dass dann das gesamte Rattenkollagen durch humanes ersetzt wurde) bzw. ein Kollagenfreies Gel als Trägerstruktur entwickelt und verwendet werden. Um eine bessere Vergleichbarkeit der Ergebnisse des Gene-Arrays aus Spenderbiopsie und Neofaszie zu erreichen sollten die zur RNA-Gewinnung verwendeten Anteile der Biopsie noch innerhalb des OP in RNA-later bzw. in flüssigen Stickstoff gegeben werden, um einer verstärkten Degradation vorzubeugen.
Objectives
To investigate the feasibility, diagnostic image quality and radiation dose of 3\(^{rd}\) generation dual-source computed tomography (CT) using a tin-filtered 100 kV protocol in patients with suspected acute inflammatory sinus disease.
Methods
We retrospectively evaluated 109 consecutive patients who underwent CT (Siemens SOMATOM Force, Erlangen, Germany) of the paranasal sinus with a new tin-filtered scanprotocol (Sn100 kV; tube current 35 mAs) using iterative reconstruction. Two readers independently assessed subjective image quality using a five-point Likert scale (1 = excellent, 5 = non-diagnostic). Inter-observer agreement was calculated and expressed as percentage of agreement. Noise was determined for calculation of signal-to-noise-ratio (SNR). Effective radiation dose (ED) was calculated from the dose-length-product (DLP).
Results
All examinations showed diagnostic image quality regarding evaluation of inflammatory sinus disease. On average, subjective general image quality was rated moderate (= 3) with a percentage of agreement between the observers of 81%. The mean image noise was 14.3 HU. The calculated median SNR was 6.0 for intraorbital fat, and 3.6 for the vitreous body, respectively. The median DLP was 2.1 mGy*cm, resulting in a median ED of 0.012 mSv.
Conclusions
Taking the study limitations into account, ultra-low-dose tin-filtered CT of the paranasal sinus at a tube voltage of 100 kV utilizing an iterative reconstruction algorithm provides for reliable exclusion of suspected acute inflammatory sinus disease in 100% of the cases.
Die Öffnung der Blut-Nerven-Schranke ist ein wichtiger Baustein in der Pathogenese neuropathischer Schmerzen. Die Blut-Nerven-Schranke schützt das periphere Nervensystem vor externen Einflüssen, wahrt die endoneurale Homöostase und trägt zur Aufrechterhaltung der neuronalen Signalweiterleitung bei. Sie wird durch die Pars epitheloidea des Perineuriums und endoneurale Gefäßzellen gebildet. Essentieller Bestandteil der Blut-Nerven-Schranke sind zwischen Perineural- und Gefäßzellen exprimierte Tight Junctions. Im Rahmen dieser Dissertation wurden die Tight Junction Proteine ZO-1, Claudin-1, -5, -19 und Occludin sowohl in einem Tiermodell neuropathischer Schmerzen, der Chronic Constriction Injury, als auch in Nervenbiopsien (N. suralis) von an Polyneuropathien erkrankten Patienten mittels Immunfluoreszenzfärbungen qualitativ und quantitativ untersucht.
Iodine oxides appear as reactive intermediates in atmospheric chemistry. Here, we investigate IO and HOI by mass‐selective threshold photoelectron spectroscopy (ms‐TPES), using synchrotron radiation. IO and HOI are generated by photolyzing iodine in the presence of ozone. For both molecules, accurate ionization energies are determined, 9.71±0.02 eV for IO and 9.79±0.02 eV for HOI. The strong spin‐spin interaction in the 3Σ− ground state of IO+ leads to an energy splitting into the Ω=0 and Ω=±1 sublevels. Upon ionization, the I−O bond shortens significantly in both molecules; thus, a vibrational progression, assigned to the I−O stretch, is apparent in both spectra.
The thesis provides insights in reconstruction and analysis pipelines for processing of
three-dimensional cell and vessel images of megakaryopoiesis in intact murine bone.
The images were captured in a Light Sheet Fluorescence Microscope. The work
presented here is part of Collaborative Research Centre (CRC) 688 (project B07) of
the University of Würzburg, performed at the Rudolf-Virchow Center. Despite ongoing
research within the field of megakaryopoiesis, its spatio-temporal pattern of
megakaryopoiesis is largely unknown. Deeper insight to this field is highly desirable to
promote development of new therapeutic strategies for conditions related to
thrombocytopathy as well as thrombocytopenia. The current concept of
megakaryopoiesis is largely based on data from cryosectioning or in vitro studies
indicating the existence of spatial niches within the bone marrow where specific stages
of megakaryopoiesis take place. Since classic imaging of bone sections is typically
limited to selective two-dimensional views and prone to cutting artefacts, imaging of
intact murine bone is highly desired. However, this has its own challenges to meet,
particularly in image reconstruction. Here, I worked on processing pipelines to account
for irregular specimen staining or attenuation as well as the extreme heterogeneity of
megakaryocyte morphology. Specific challenges for imaging and image reconstruction
are tackled and solution strategies as well as remaining limitations are presented and
discussed. Fortunately, modern image processing and segmentation strongly benefits
from continuous advances in hardware as well as software-development. This thesis
exemplifies how a combined effort in biomedicine, computer vision, data processing
and image technology leads to deeper understanding of megakaryopoiesis. Tailored
imaging pipelines significantly helped elucidating that the large megakaryocytes are
broadly distributed throughout the bone marrow facing a surprisingly dense vessel
network. No evidence was found for spatial niches in the bone marrow, eventually
resulting in a revised model of megakaryopoiesis.
The aim of the work was the development of thiol-ene cross-linked hydrogels based on functionalized poly(glycidol)s (PG) and hyaluronic acid (HA) for extrusion based 3D bioprinting. Additionally, the functionalization of the synthesized PG with peptides and the suitability of these polymers for physically cross-linked gels were investigated, in a proof of principle study in order to demonstrate the versatile use of PG polymers in hydrogel development.
First, the precursor polymers of the different hydrogel systems were synthesized. For thiol-ene cross-linked hydogels, linear allyl-functionalized PG (P(AGE-co-G)) and three different thiol-(SH-)functionalized polymers, ester-containing PG-SH (PG SHec), ester-free PG-SH (PG-SHef) and HA-SH were synthesized and analysed, The degree of functionalization of these polymers was adjustable.
For physically cross-linked hydrogels, peptide-functionalized PG (P(peptide-co-G)), was synthesized through polymer analogue thiol-ene modification of P(AGE-co-G).
Subsequently, thiol-ene cross-linked hydrogels were prepared with the synthesized thiol- and allyl-functionalized polymers. Depending on the origin of the used polymers, two different systems were obtained: on the one hand synthetic hydrogels consisting of PG-SHec/ef and P(AGE-co-G) and on the other hand hybrid gels, consisting of HA-SH and P(AGE-co-G). In synthetic gels, the degradability of the gels was determined by the applied PG-SH. The use of PG-SHec resulted in hydrolytically degradable hydrogels, whereas the cross-linking with PG-SHef resulted in non-degradable gels.
The physical properties of these different hydrogel systems were determined by swelling, mechanical and diffusion studies and subsequently compared among each other. In swelling studies the differences of degradable and non-degradable synthetic hydrogels as well as the differences of synthetic compared to hybrid hydrogels were demonstrated.
Next, the stiffness and the swelling ratios (SR) of the established hydrogel systems were examined in dependency of different parameters, such as incubation time, polymer concentration and UV irradiation. In general, these measurements revealed the same trends for synthetic and hybrid hydrogels: an increased polymer concentration as well as prolonged UV irradiation led to an increased network density. Moreover, it was demonstrated that the incorporation of additional non-bound HMW HA hampered the hydrogel cross-linking resulting in gels with decreased stiffness and increased SR. This effect was strongly dependent on the amount of additional HMW HA.
The diffusion of different molecular weight fluorescein isothiocyanate-dextran (FITC-dextran) through hybrid hydrogels (with/without HMW HA) gave information about the mesh size of these gels. The smallest FITC-dextran (4 kDa) completely diffused through both hydrogel systems within the first week, whereas only 55 % of 40 kDa and 5-10 % HMW FITC-dextrans (500 kDa and 2 MDa) could diffuse through the networks.
The applicability of synthetic and hybrid hydrogels for cartilage regeneration purpose was investigated through by biological examinations. It was proven that both gels support the survival of embedded human mesenchymal stromal cells (hMSCs) (21/28 d in vitro culture), however, the chondrogenic differentiation was significantly improved in hybrid hydrogels compared to synthetic gels. The addition of non-bound HMW HA resulted in a slightly less distinct chondrogenesis.
Lastly the printability of the established hydrogel systems was examined. Therefore, the viscoelastic properties of the hydrogel solutions were adjusted by incorporation of non-bound HMW HA. Both systems could be successfully printed with high resolution and high shape fidelity.
The introduction of the double printing approach with reinforcing PCL allowed printing of hydrogel solutions with lower viscosities. As a consequence, the amount of additional HMW HA necessary for printing could be reduced allowing successful printing of hybrid hydrogel solutions with embedded cells. It was demonstrated that the integrated cells survived the printing process with high viability measured after 21 d. Moreover, by this reinforcing technique, robust hydrogel-containing constructs were fabricated.
In addition to thiol-ene cross-linked hydrogels, hydrogel cross-linking via ionic interactions was investigated with a hybrid hydrogel based on HMW HA and peptide-functionalized PG. Rheological measurements revealed an increase in the viscosity of a 2 wt.% HMW HA solution by the addition of peptide-functionalized PG. The increase in viscosity could be attributed to the ionic interactions between the positively charge PG and the negatively charge HMW HA.
In conclusion, throughout this thesis thiol-ene chemistry and PG were introduced as promising cross-linking reaction and polymer precursor for the field of biofabrication. Furthermore, the differences of hybrid and synthetic hydrogels as well as chemically and physically cross-linked hydrogels were demonstrated.
Moreover, the double printing approach was demonstrated to be a promising tool for the fabrication of robust hydrogel-containing constructs. It opens the possibility of printing hydrogels that were not printable yet, due to too low viscosities.
This thesis describes the growth and characterization of both the all-oxide heterostructure
Fe3O4/ZnO and the spin-orbit coupling driven layered perovskite iridates.
As for Fe3O4/ZnO, the 100% spin-polarized Fe3O4 is a promising spin electrode candidate
for spintronic devices. However, the single crystalline ZnO substrates exhibit different polar surface termination which, together with substrate preparation method, can drastically affect the physical properties of Fe3O4/ZnO heterostructures. In this thesis two different methods of substrate preparation were investigated: a previously used in situ method involving sputtering and annealing treatments and a recent ex situ method containing only the annealing procedure. For the latter, the annealing treatment was performed in dry and humid O2 gas flow for the O- and Zn-terminated substrates, respectively, to produce atomically at surfaces as verified by atomic force microscopy(AFM). With these methods, four different ZnO substrates were fabricated and used further for Fe3O4 film growth. Fe3O4 films of 20 nm thickness were successfully grown by reactive molecular beam epitaxy. AFM measurements reveal a higher film surface roughness for the samples with in situ prepared substrates. Moreover, X-ray photoelectron spectroscopy (XPS) measurements indicate significant Zn substitution within the Fe3O4 film for these samples, whereas the samples with ex situ prepared substrates show stoichiometric Fe3O4 films. X-ray diffraction measurements confirm the observations from XPS, revealing additional peaks due to Zn substitution in Fe3O4 films grown on in situ prepared ZnO substrates. Conductivity, as well as magnetometry, measurements show the presence of Zn-doped ferrites in films grown on in situ prepared substrates. Such unintentionally intercalated Zn-doped ferrites dramatically change the electrical and magnetic properties of the films and, therefore, are not preferred in a high-quality heterostructure.
X-ray reflectivity (XRR) measurements show for the film grown on ex situ prepared Zn-terminated substrate a variation of film density close to the interface which is also confirmed by transmission electron microscopy (TEM). Using polarized neutron reflectometry, magnetic depth profiles of the films grown on ex situ prepared substrates clearly indicate Fe3O4 layers with reduced magnetization at the interfaces. This result is consistent with earlier observations made by resonant magnetic X-ray reflectometry (RMXR), but in contrast to the findings from XRR and TEM of this thesis. A detailed TEM study of all four samples shows that the sample with ex situ prepared O-terminated substrate has the sharpest interface, whereas those with ex situ prepared Zn-terminated as well as in situ prepared substrates indicate rougher interfaces. STEM-EELS composition profiles of the samples reveal the Zn substitution in the films with in situ prepared substrates and therefore confirm the presence of Zn-doped ferrites. Moreover, a change of the Fe oxidation state of the first Fe layer at the interface which was observed in previous studies done by RMXR, was not verified for the samples with in situ prepared substrates thus leaving the question of a possible presence of the magnetically dead layer open. Furthermore, density functional theory calculations were performed to determine the termination dependent layer sequences which are ...-Zn-O-(interface)-[Fe(octa)-O-Fe(tetra)-Fe(octa)-Fe(tetra)-O]-[...]-... and ...-O-Zn-(interface)-[O-Fe(octa)-O-Fe(tetra)-Fe(octa)-Fe(tetra)]-[...]-... for the samples with O- and Zn-terminated substrates, respectively. Spin density calculations show that in case of O-termination the topmost substrate layers imitate the spin polarization of film layers close to the interface. Here, the first O layer is affected much stronger than the first Zn layer. Due to the strong decrease of this effect toward deeper substrate layers, the substrate surface is supposed to be sensitive to the contiguous spin polarization of the film. Thus, the topmost O layer of the O-terminated substrate could play the most essential role for effective spin injection into ZnO.
The 5d transition metal oxides Ba2IrO4 (BIO) and Sr2IrO4 (SIO) are associated with the Ruddlesden-Popper iridate series with phase type "214" (RP{214), and due to the strong spin-orbit coupling belong to the class of Mott insulators. Moreover, they show many similarities of the isostructural high Tc-cuprate superconductors, e.g. crystal structure, magnetism and electronic band structure. Therefore, it is of great interest to activate a potential superconducting phase in (RP{214) iridates. However, only a small number of publications on PLD grown (RP{214) iridates in the literature exists. Furthermore, published data of soft X-ray angle resolved photoemission spectroscopy (SX-ARPES) experiments mainly originate from measurements which were performed on single crystals or MBE grown films of SIO and BIO. In this thesis La-doped SIO films (La0:2Sr1:8IrO4, further referred as LSIO) were used to pursue a potential superconducting phase.
A set of characterization methods was used to analyze the quality of the PLD grown BIO, SIO and LSIO films. AFM measurements demonstrate that thick PLD grown(RP{214) iridate films have rougher surfaces, indicating a transition from a 2D layer-bylayer growth (which is demonstrated by RHEED oscillations) to a 3D island-like growth mode. In addition, chemical depth profiling XPS measurements indicate an increase of the O and Ir relative concentrations in the topmost film layers. Constant energy k-space maps and energy distribution curves (EDCs) measured by SX-ARPES show for every grown film only weak energy band dispersions, which are in strong contrast to the results obtained on the MBE grown films and single crystals from the literature. In this thesis,
a subsequent TEM study reveals missing SrO layers within the grown films which occur mainly in the topmost layers, confirming the results and suggestions from XPS and SX-ARPES data: the PLD grown films have defects and, therefore, incoherently scatter photoelectrons. Nevertheless, the LSIO film shows small additional spectral weight between the highsymmetry M points close to the Fermi level which can be attributed to quasiparticle states which, in turn, indicates the formation of a Fermi-arc. However, neither conductivity measurements nor valence band analysis via XPS confirm an activation of a superconducting phase or presence of spectral weight of quasiparticle states at the Fermi level in this LSIO film.
It is possible that these discovered difficulties in growth are responsible for the low number of SX-ARPES publications on PLD grown (RP{214) iridate films. For further investigations of (RP{214) iridate films by SX-ARPES, their PLD growth recipes have to be improved to create high quality single crystalline films without imperfections.
Derzeit gibt es nur wenige Informationen zu konzentrationsabhängigen klinischen Effekten von Clozapin und Olanzapin in der Behandlung von Kindern und Jugendlichen mit schizophrenen Störungen. Es existieren keine altersspezifisch-definierte therapeutische Zielbereiche für die Höhe der Serumkonzentration in dieser Altersklasse.
Das Ziel dieser retrospektiven, naturalistischen Studie ist die Untersuchung der Zusammenhänge zwischen Dosis, Serumkonzentration und klinischen Effekten (Therapieeffekt und unerwünschte Arzneimittelwirkungen) sowie die Untersuchung möglicher Einflussfaktoren darauf. Des Weiteren sollen Erkenntnisse zu therapeutischen Konzentrationsbereichen von Clozapin und Olanzapin bei Kindern und Jugendlichen gewonnen werden.
Ausgewertet wurden multizentrische Daten von 32 (Clozapin) bzw. 17 (Olanzapin) Patienten, bei denen routinemäßig Therapeutisches Drug Monitoring im Zeitraum von Februar 2004 bis Dezember 2007 durchgeführt wurde. Die psychopathologische Befundeinschätzung erfolgte mittels der Clinical Global Impression Scale und der Brief Psychiatric Rating Scale, die der unerwünschten Arzneimittelwirkungen mithilfe der Dose Record and Treatment Emergent Symptom Scale bzw. der Udvalg for Kliniske Undersøgelser Side Effect Rating Scale.
Bei beiden untersuchten Wirkstoffen zeigte sich eine signifikant positive Korrelation zwischen der (gewichtskorrigierten) Tagesdosis und der Serumkonzentration sowie eine hohe interindividuelle Variabilität der Serumkonzentrationen bei gleicher Dosierung. Als weiterer möglicher Einflussfaktor auf die Höhe der Serumkonzentration konnte in der Olanzapin-Stichprobe eine signifikante Assoziation zwischen dem Geschlecht und der Serumkonzentration nachgewiesen werden: Mädchen scheinen unter gleicher klinischer Dosierung höhere Serumkonzentrationen aufzubauen als Jungen. In beiden Stichproben gab es eine hohe Rate dokumentierter unerwünschter Arzneimittelwirkungen. Ein Zusammenhang zwischen der Höhe der Serumkonzentration und dem Auftreten unerwünschter Arzneimittelwirkungen ließ sich nicht nachweisen. In der Clozapin-Stichprobe zeigte sich ein signifikanter Zusammenhang zwischen der Serumkonzentration und dem Therapieeffekt: Im untersuchten Sample war der Therapieeffekt besser bei niedrigeren (< 350 ng/ml) Serumkonzentrationen. Zudem zeigte sich eine Tendenz zu einem niedrigeren unteren Schwellenwert für einen empfohlenen therapeutischen Bereich der Serumkonzentration verglichen mit dem Bereich der für Erwachsene definiert wurde. In der Olanzapin-Stichprobe ließ sich mit dem gewählten Studiendesign keine signifikante Korrelation zwischen der Serumkonzentration und dem Therapieeffekt nachweisen. Die Mehrheit der pädiatrischen Patienten hatte eine Serumkonzentration innerhalb des empfohlenen Zielbereichs für Erwachsene. Dieses Ergebnis könnte auf eine Übereinstimmung des zu empfehlenden Zielbereichs der Serumkonzentration von Olanzapin in beiden Altersklassen hinweisen.
Aufgrund der Limitationen des naturalistischen Studiendesigns sind weitere Studien mit kontrolliertem Design und größerer Stichprobe notwendig, um die Ergebnisse zu replizieren.
Two chiral chemical molecules being mirror images of each other, also referred to as enantiomers, may have different pharmacokinetic, pharmacodynamic, and toxicological effects. Thus, pharmaceutical manufacturers and authorities are increasingly interested in the approval of enantiopure drugs. However, the isomeric purity and the limits for isomeric impurities have to be specified applying enantioselective analytical methods, such as capillary electrophoresis.
The separation of enantiomers in capillary electrophoresis may be improved by the addition of ionic liquids to the background electrolyte. The aim of this work was to investigate the influence of different separation conditions on the enantioseparation of phenethylamines in background electrolytes containing ionic liquids based on tetrabutylammonium cations.
Best chiral separations were achieved at acidic pH values using phosphate buffers containing 125 mmol/L tetrabutylammonium based salts. Different reasons explaining enhanced enantioseparations in buffers containing ionic liquids were found. First, due to an improvement of the cyclodextrin solubility, the addition of ionic liquids to the background electrolyte enables the use of higher concentrations of these chiral selector. Furthermore, the adsorption of tetrabutylammonium cations to the negatively charged capillary surface results in a reduction of the electroosmotic flow. Hence, the resulting prolongation of migration times leads to a longer period of time for the separation of temporarily formed diastereomeric analyte cyclodextrin complexes, which yields improved enantioseparation. Additionally, due to a decrease of the adsorption of positively charged phenethylamine analyte molecules to capillary surface silanol groups, the adsorption of ionic liquid cations inhibits peak broadening. A further reason explaining an enhanced enantioseparation by the addition of ionic liquids to the background electrolyte is a competition between tetrabutylammonium cations and analyte enantiomers for the inclusion into cyclodextrin cavities.
Furthermore, the influence of different chiral counterions, combined with tetrabutylammonium cations, on the enantioseparation of phenethylamines was investigated. Solely anions based on the basic proteinogenic amino acids L lysine and L arginine yielded chiral separation results superior to those achieved using achiral tetrabutylammonium chloride as background electrolyte additive. Especially the application of tetrabutylammonium L argininate gave very good enantioseparations of all investigated ephedrine derivatives, which might be explained by the ability of L arginine to affect the formation of complexes between analytes and cyclodextrins.
Besides the investigation of the influence of ionic liquids on the enantioseparation, complexes between phenethylamine enantiomers and β cyclodextrin derivatives were characterized by affinity capillary electrophoresis. The binding constants between analyte enantiomers and cyclodextrins and the electrophoretic mobilities of the temporarily formed complexes were determined and compared to the observed chiral resolution values. While neither the calculated binding constants nor their differences correlated with the quality of the enantioseparation, a strong correlation between the differences of the electrophoretic mobilities of the complexes and the chiral resolution values was found.
This dissertation highlights various aspects of basic social attention by choosing versatile approaches to disentangle the precise mechanisms underlying the preference to focus on other human beings. The progressive examination of different social processes contrasted with aspects of previously adopted principles of general attention. Recent research investigating eye movements during free exploration revealed a clear and robust social bias, especially for the faces of depicted human beings in a naturalistic scene. However, free viewing implies a combination of mechanisms, namely automatic attention (bottom-up), goal-driven allocation (top-down), or contextual cues and inquires consideration of overt (open exploration using the eyes) as well as covert orienting (peripheral attention without eye movement). Within the scope of this dissertation, all of these aspects have been disentangled in three studies to provide a thorough investigation of different influences on social attention mechanisms.
In the first study (section 2.1), we implemented top-down manipulations targeting non-social features in a social scene to test competing resources. Interestingly, attention towards social aspects prevailed, even though this was detrimental to completing the requirements. Furthermore, the tendency of this bias was evident for overall fixation patterns, as well as fixations occurring directly after stimulus onset, suggesting sustained as well as early preferential processing of social features. Although the introduction of tasks generally changes gaze patterns, our results imply only subtle variance when stimuli are social. Concluding, this experiment indicates that attention towards social aspects remains preferential even in light of top-down demands.
The second study (section 2.2) comprised of two separate experiments, one in which we investigated reflexive covert attention and another in which we tested reflexive as well as sustained overt attention for images in which a human being was unilaterally located on either the left or right half of the scene. The first experiment consisted of a modified dot-probe paradigm, in which peripheral probes were presented either congruently on the side of the social aspect, or incongruently on the non-social side. This was based on the assumption that social features would act similar to cues in traditional spatial cueing paradigms, thereby facilitating reaction times for probes presented on the social half as opposed to the non-social half. Indeed, results reflected such congruency effect. The second experiment investigated these reflexive mechanisms by monitoring eye movements and specifying the location of saccades and fixations for short as well as long presentation times. Again, we found the majority of initial saccades to be congruently directed to the social side of the stimulus. Furthermore, we replicated findings for sustained attention processes with highest fixation densities for the head region of the displayed human being.
The third study (section 2.3), tackled the other mechanism proposed in the attention dichotomy, the bottom-up influence. Specifically, we reduced the available contextual information of a scene by using a gaze-contingent display, in which only the currently fixated regions would be visible to the viewer, while the remaining image would remain masked. Thereby, participants had to voluntarily change their gaze in order to explore the stimulus. First, results revealed a replication of a social bias in free-viewing displays. Second, the preference to select social features was also evident in gaze-contingent displays. Third, we find higher recurrent gaze patterns for social images compared to non-social ones for both viewing modalities. Taken together, these findings imply a top-down driven preference for social features largely independent of contextual information.
Importantly, for all experiments, we took saliency predictions of different computational algorithms into consideration to ensure that the observed social bias was not a result of high physical saliency within these areas. For our second experiment, we even reduced the stimulus set to those images, which yielded lower mean and peak saliency for the side of the stimulus containing the social information, while considering algorithms based on low-level features, as well as pre-trained high-level features incorporated in deep learning algorithms.
Our experiments offer new insights into single attentional mechanisms with regard to static social naturalistic scenes and enable a further understanding of basic social processing, contrasting from that of non-social attention. The replicability and consistency of our findings across experiments speaks for a robust effect, attributing social attention an exceptional role within the general attention construct, not only behaviorally, but potentially also on a neuronal level and further allowing implications for clinical populations with impaired social functioning.
Nodular lymphocyte predominant Hodgkin lymphoma (NLPHL) can present with different histopathological growth patterns. The impact of these histopathological growth patterns on relapse characteristics is unknown. We therefore analyzed paired biopsies obtained at initial diagnosis and relapse from 33 NLPHL patients who had received first‐line treatment within German Hodgkin Study Group (GHSG) trial protocols, and from a second cohort of 41 relapsed NLPHL patients who had been treated outside GHSG studies. Among the 33 GHSG patients, 21 patients presented with a typical growth pattern at initial diagnosis, whereas 12 patients had a variant histology. The histopathological growth patterns at initial diagnosis and at relapse were consistent in 67% of cases. A variant histology at initial diagnosis was associated with a shorter median time to lymphoma recurrence (2.8 vs 5.2 years; P = .0219). A similar tendency towards a shorter median time to lymphoma recurrence was observed for patients presenting with a variant histology at relapse, irrespective of the growth pattern at initial diagnosis. Results obtained from the 41 NLPHL patients who had been treated outside GHSG studies were comparable (median time to lymphoma recurrence for variant histology vs typical growth pattern at initial diagnosis: 1.5 vs 7.0 years). In conclusion, the histopathological growth pattern remains consistent at relapse in the majority of NLPHL cases, and has major impact on the time of relapse.
Upon approval of a drug, the stability of the API and the FPP has to be studied intensively because it determines the shelf-life. If a drug is found to be stable, the expiry date is arbitrary set to five years at the maximum, if a drug tends to undergo degradation, the expiry date is set shorter. The drug product must comply with predefined specifications in accordance with the ICH guidelines Q6A and Q6B during its entire market life. The content of the active substance is required to be within a specification of 95–105% of its labeled claim until expiry corresponding to the ICH guideline Q1A(R2). However, there is little or scattered literature information addressing the stability of drug products beyond their expiry dates. The objective of this thesis was to study and assess the long-term stability of a collection involving numerous pure drug substances and ampoules manufactured in the 20th century. The content and the impurity profile were examined by means of appropriate analytical methods, mainly using liquid chromatography. The results were compared to data being available in the literature. Assessing the stability regarding the dosage form and the affiliation of the drug class was conducted.
The experimental studies comprise the examination of 50 drug substances manufactured 20–30 years ago and 14 long expired ampoules which were older than 40 years in the time of analysis, exceeding many times the maximum shelf life of five years.
For investigation of the solid drug substances, pharmacopoeial methods were applied as far as possible. Indeed, results of the study showed that 44 tested substances still complied with the specification of the Ph. Eur. with regard to the content and impurity profile, even after more than two decades of storage.
For analysis of the injection solutions, HPLC-UV and HPLC-ESI/MS techniques were applied, commonly based on liquid chromatography methods of the Ph. Eur. for determination of related substances. Each method was further validated for its application to ensure accurate API quantification corresponding to ICH Q2(R1). Quite a few ampoules were identified to show surprisingly high stability. In spite of their age of 53–72 years, APIs such as caffeine, etilefrine, synephrine, metamizole sodium, furosemide, and sodium salicylate complied with the specified content that is valid nowadays, respectively. Nevertheless, typical degradation reaction, e.g. hydrolysis, oxidation, or isomerization, was observed in all remaining ampoules. Various degrees of hydrolysis were revealed for scopolamine, procaine, and adenosine triphosphate, the contents were decreased to 71%, 70%, and 15% of the declared concentrations, respectively. In the epinephrine and dipyridamole ampoules, oxidative degradation has been occurred, finding respective API contents of more or less 70%. For dihydroergotamine, excessive decomposition by epimerization was observed, resulting in an API content of 21% and degradation by isomerization was found in lobeline, still containing 64% of the labeled claim.
In conclusion, supported by the data of the present studies and the literature, defining and authorizing a longer shelf-life may be applicable to numerous pharmaceuticals which should be considered by pharmaceutical manufacturers and regulatory authorities, if justified based on stability studies. A general extension of the shelf-lives of drug products and the abolishment or extension of the maximum shelf-life limit of five years would prevent disposing of still potent medications and save a lot of money to the entire health care system.
Inflammation is a central aspect of tumour biology and can contribute significantly to both the origination and progression of tumours. The NFκB pathway is one of the most important signal transduction pathways in inflammation and is, therefore, an excellent target for cancer therapy. In this work, we examined the influence of four NFκB inhibitors — Cortisol, MLN4924, QNZ and TPCA1 — on proliferation, inflammation and sensitisation to apoptosis mediated by the death ligand FasL in the HNSCC cell lines PCI1, PCI9, PCI13, PCI52 and SCC25 and in the human dermal keratinocyte cell line HaCaT. We found that the selection of the inhibitor is critical to ensure that cells do not respond by inducing counteracting activities in the context of cancer therapy, e.g., the extreme IL-8 induction mediated by MLN4924 or FasL resistance mediated by Cortisol. However, TPCA1 was qualified by this in vitro study as an excellent therapeutic mediator in HNSCC by four positive qualities: (1) proliferation was inhibited at low μM-range concentrations; (2) TNFα-induced IL-8 secretion was blocked; (3) HNSCC cells were sensitized to TNFα-induced cell death; and (4) FasL-mediated apoptosis was not disrupted.
Abscission marks the last step of cytokinesis and gives rise to two physically separated daughter cells and a midbody remnant. This work studies abscission by examining the extent of the abscission failure in C. elegans septin and ESCRT mutants with the help of the ZF1-degradation technique. The ZF1 technique is also applied to discern a possible role for PI3K during abscission. Lastly, we test the role of proteins required for macroautophagy but not for LC3-associated phagocytosis (LAP) and show that after release into the extracellular space, the midbody is resolved via LAP.
Adipocytes are specialized cells found in vertebrates to ensure survival in terms of adaption to food deficit and abundance. However, their dysfunction accounts for the pathophysiology of metabolic diseases such as T2DM. Preliminary data generated by Mona Löffler suggested that PKD1 is involved in adipocyte function. Here, I show that PKD1 expression and activity is linked to lipid metabolism of murine adipocytes. PKD1 gene expression and activity was reduced in murine white adipose tissue upon fasting, a physiological condition which induces lipolysis. Isoproterenol-stimulated lipolysis in adipose tissue and 3T3-L1 adipocytes reduced PKD1 gene expression. Silencing ATGL in adipocytes inhibited isoproterenol-stimulated lipolysis, however, the β-adrenergic
stimulation of ATGL-silenced adipocytes lowered PKD1 expression levels as well. Adipose tissue of obese mice exhibited high PKD1 RNA levels but paradoxically lower protein levels of phosphorylated PKD1-Ser916. However, HFD generated a second
PKD1 protein product of low molecular weight in mouse adipose tissue. Furthermore, constitutively active PKD1 predominantly displayed nuclear localization in 3T3-L1 adipocytes containing many fat vacuoles. However, adipocytes
overexpressing non-functional PKD1 contained fewer lipid droplets and PKD1-KD was distributed in cytoplasm. Most importantly, deficiency of PKD1 in mouse adipose tissue caused expression of genes involved in adaptive thermogenesis such as UCP-1 and thus generated brown-like phenotype adipocytes. Thus, PKD1 is implicated in adipose tissue function and presents an interesting target for therapeutic approaches in the prevention of obesity and associated diseases.
Background
Colonization with Staphylococcus aureus has been identified as a risk for subsequent occurrence of infection. This study investigated the relationship between S. aureus colonization of patients and healthcare workers (HCWs), and subsequent surgical site infections (SSI).
Methods
Between December 2014 and September 2015, a total of 930 patients and 143 HCWs were enrolled from the Bugando Medical Centre and Sekou Toure hospital in Mwanza, Tanzania. On admission and discharge nasal swabs, with an additional of wound swab for those who developed SSI were collected from patients whereas HCWs were swabbed once. Identification and antimicrobial susceptibility testing were done by VITEK-MS and VITEK-2, respectively. Detection of Panton Valentine leukocidin (PVL) and mecA genes was done by PCR. S. aureus isolates were further characterized by spa typing and Multi-Locus Sequence Typing (MLST).
Results
Among 930 patients screened for S. aureus on admission, 129 (13.9%) were positive of which 5.4% (7/129) were methicillin-resistant S. aureus (MRSA). Amongst 363 patients rescreened on discharge, 301 patients had been tested negative on admission of whom 29 (9.6%) turned positive after their hospital stay. Three (10.3%) of the 29 acquired S. aureus were MRSA. Inducible Clindamycin resistance occurred more often among acquired S. aureus isolates than among isolates from admission [34.5% (10/29) vs. 17.1% (22/129), P = 0.018]. S. aureus contributed to 21.1% (n = 12) of the 57 cases of investigated SSIs among 536 patients followed. Seven out of eight S. aureus carriage/infection pairs had the same spa and sequence types. The previously reported dominant PVL-positive ST88 MRSA strain with spa type t690 was detected in patients and HCW.
Conclusion
A significant proportion of patients acquired S. aureus during hospitalization. The finding of more than 90% of S. aureus SSI to be of endogenous source underscores the need of improving infection prevention and control measures including screening and decolonization of high risk patients.
