Die vorliegende Arbeit ist eine retrospektive Analyse der Schusstodesfälle aus dem Sektionsgut des Instituts für Rechtsmedizin der Universität Würzburg aus dem Zeitraum 2002-2017.
Bei 173 Schusstodesfällen kam es zu 209 registrierten Einschüssen. Hierbei entfielen 83,2 % der Schusstoten auf Suizide, 10,4 % auf Homizide und 3,5 % auf Unfälle. Rund 80 % der Opfer waren 30-79 Jahre alt und zu 91,9 % männlich, zu 8,1 % weiblich, ein Verhältnis von 11:1. Schusstreffer fanden sich zu 75,1 % in der Kopfregion gefolgt von Brust (12,9 %) und Extremitäten (4,8 %). Zu über ¾ ist die Todesursache ein zentrales Regulationsversagen infolge eines Kopfschusses.
Der Schusswaffensuizident ist zu 98,6 % männlichen Geschlechts. Der Schusstod wird zu 95,9 % durch einen Nahschuss (meist absoluter Nahschuss) herbeigeführt. In 91,3 % ist die Einschussregion der Kopf in 8,7 % die Brust. Der Suizident ist zu 70,8 % 40-79 Jahre alt. Psychische Erkrankungen stellen das führende Tatmotiv dar. Die Tat findet zumeist im Frühjahr oder Sommer statt. Wenn die Blutalkoholkonzentration des Opfers erhoben wurde, war diese zu rund 50 % positiv.
In 61,1 % aller Homizide waren Frauen das Opfer. Die Schussabgabe erfolgte in 90,9 % seitens eines Familienmitgliedes, meist dem Partner bzw. Ex-Partner. Klammert man die Tötungen im Polizeieinsatz (4 Fälle) aus, waren 92,9 % aller Homizide Beziehungstaten, auf die eine schusswaffenbedingte Selbsttötung des Täters folgte. Das Homizidopfer war zu 77,8 % 5-49 Jahre alt. Bei den weiblichen Homiziden überwog zu 63,6 % der Nahschuss, bei den männlichen Opfern der Tötung von fremder Hand lag in den meisten Fällen ein Fernschuss vor.
In 26,1 % bestand nachweislich ein beruflich- oder freizeitbedingter Waffenbezug. Zu 64,2 % fand die Schussabgabe im häuslichen Umfeld statt. In 68,2 % aller Fälle wurde eine Kurzwaffe, meist Pistole (59,3 %) benutzt, in 32,8 % war das verwendete Munitionskaliber 9 mm/.357. 85,0 % aller abgegebenen Schüsse waren Nahschüsse, 63,2 % aller Schüsse waren Durchschüsse. In 89,0 % wurde ein einzelner Schuss abgegeben.
Die erhobenen Ergebnisse dieser Arbeit weißen viele Parallelen mit bereits bestehenden Publikationen, insbesondere aus dem deutschsprachigen Raum und dem nordeuropäischen Ausland auf, grenzen sich aber vor allem in Hinsicht auf den Todesumstand von anderen Ländern teils deutlich ab.
The aim of this study was to provide a comprehensive overview of the frontal bone in the forensic context with special emphasis on its shape. Analyses on 19th and 20th century Euro-American and German crania were performed in terms of population differences, sexual dimorphism, secular change, and metopism.
It could clearly be seen that the frontal bone on its own already provides a lot of information toward the biological profile of an individual. Overall, using size and shape combined for analyses would always produce the best results, followed by shape only and then size only. Nevertheless, Log_Centroid_Size was the best sex-discriminating variable in the size-shape combined analyses for both populations. Population differences as well as sexual dimorphism could both be assessed (with varying accuracy) using size only and shape only respectively.
Very little secular change between the 19th and 20th century was found for the frontal in both groups respectively, with the secular change that could be seen mostly being shape variation.
Metopism analysis was only performed on German crania, because the presence or absence of a metopic suture was not documented for the Euro-American crania. Unfortunately, the results of these analyses were very limited due to too small sample sizes for the overall low percentage of metopism. The metopic frontal was once again found to be short in relation to its width and presenting a more rounded frontal curvature. The attempt of creating a formula to morphometrically assess the presence of metopism was not successful.
The results of this thesis suggest that forensic case work on skeletal remains would greatly benefit from a broader application of Geometric Morphometrics and consequently from larger databases containing shape data as well as more advanced and user-friendly software for this type of analyses.
„Black esophagus“ oder „akute Ösophagusnekrose“ (AÖN) ist eine seltene Erkrankung, die sich makroskopisch durch eine zirkumferente Schwarzverfärbung der Ösophagusmukosa mit abruptem Ende am gastroösophagealen Übergang auszeichnet. Die genaue Pathogenese ist unbekannt; es werden multifaktorielle Einflüsse wie z. B. Säurereflux, Ischämie und verringerte Schutzmechanismen der Mukosa als mögliche Ursachen diskutiert.
Vorgestellt werden 2 Obduktionsfälle, die typische Befunde einer AÖN aufwiesen. Zusätzlich hatten Fall 1 eine Candida-Infektion und Fall 2 eine Appendizitis, sodass eine infektiöse Genese in beiden Fällen eine Rolle gespielt haben könnte.
Die dieser Arbeit zugrundliegenden Untersuchungen am postmortalen Hirngewebe und an den korrespondierenden Proben postmortalen Liquor cerebrospinalis (CSF) konnten einen Zusammenhang der Dichte der parenchymalen TMEM119-positiven Mikroglia und der der CSF belegen. Innerhalb der analysierten Kompartimente bestehend aus Kortex, Marklager und CSF ergaben sich weit gefächerte Messwerte zur jeweiligen Dichte der immuno-positiven Mikroglia. Die Ergebnisse implizierten eine schnelle Reaktion der Mikroglia im Hirngewebe und einen zeitverzögerten Nachweis von immuno-positiven Mikroglia in der CSF. Signifikante Effekte von Alter, Geschlecht, Hirngewicht und insbesondere einem steigenden Postmortalintervall konnten als potenzielle Einflussfaktoren hinsichtlich der CSF-Intensität ausgeschlossen werden. Eine positive Korrelation ergab sich hingegen zwischen der Mikroglia-Dichte der CSF und den Angaben bezüglich erfolgter Reanimationsmaßnahmen der eingeschlossenen Sterbefälle als Hinweis auf einen relevanten Zusammenhang mit dem zerebralen Blutfluss.
Neben dem ursprünglich angestrebten isolierten Vergleich zwischen der TMEM119-positiven Mikroglia-Profildichte der CSF, des Kortex und der des Markraums ergaben sich nach Analyse weiterhin morphologische Auffälligkeiten der identifizierten Mikroglia und teils spezifische Verteilungsmuster. Die abschnittsweise laminäre Anordnung der Zellen in den kortikalen Gewebeanteilen wies insbesondere in den supragranulären Schichten nahe der Hirnoberfläche strukturell auffällige Mikroglia-Profile mit annähernd rundem Zellkörper und wenigen bis keinen Zellfortsätzen auf. Ein annähernd identisches Bild konnte im perivaskulären Marklager festgestellt werden und wies auf einen Zusammenhang zum Übertritt der Mikroglia in die CSF sowie eine Assoziation zu den medullären Gefäßen hin. Der erstmalige Nachweis des aktiven Übertritts der TMEM119-positiven Mikroglia durch die weiche Hirnhaut implizierte einen aktiven Zugangsweg der Zellen in die CSF unter Ausbildung eines amöboid erscheinenden Phänotyps neben einem lediglich diffusen und passiven Übertritt der Zellen unter pathologischen Bedingungen.
Die durchgeführten Untersuchungen belegen das enorme Potenzial der postmortalen CSF als Untersuchungsmedium insbesondere im Hinblick auf die Erhebung der Mikroglia-Dichte und die Analyse der Mikroglia-Morphologie in Bezug auf neuropathologische Beteiligung im ZNS und damit verbundenen Fragestellungen.
Intramuscular hemorrhages in the pathway of an electric current through the body - two case reports

Intramuscular hemorrhages at autopsy can have a variety of traumatic as well as non-traumatic causes, but their recognition in electrical deaths is almost a rarity. We report on two autopsy cases of electrical fatalities, the first relating to a portion of the right upper human extremity, consisting (only) of the forearm and hand, while the other case relates to a female child who died after a high voltage electrical shock. In both cases, layered dissection of the upper limb revealed fresh intramuscular hemorrhages in the skeletal muscles that could be topographically related to the path taken by the current through the body. Externally visible electric marks were present in both cases. The hemorrhages were most likely caused by current-induced tetanic muscle contractions, producing an internal muscle trauma with rupture of fibers and bleedings. In complex situations, such as inconspicuous marks or a complete lack of visible signs on the body, the finding may be helpful in solving the case in consideration of the case history and circumstances. The vitality, topography, and pattern of the hemorrhages are discussed in the light of the available literature.
Targeting molecular alterations as an effective treatment for isocitrate dehydrogenase-wildtype glioblastoma (GBM) patients has not yet been established. Sterol-O-Acyl Transferase 1 (SOAT1), a key enzyme in the conversion of endoplasmic reticulum cholesterol to esters for storage in lipid droplets (LD), serves as a target for the orphan drug mitotane to treat adrenocortical carcinoma. Inhibition of SOAT1 also suppresses GBM growth. Here, we refined SOAT1-expression in GBM and IDH-mutant astrocytoma, CNS WHO grade 4 (HGA), and assessed the distribution of LD in these tumors. Twenty-seven GBM and three HGA specimens were evaluated by multiple GFAP, Iba1, IDH1 R132H, and SOAT1 immunofluorescence labeling as well as Oil Red O staining. To a small extent SOAT1 was expressed by tumor cells in both tumor entities. In contrast, strong expression was observed in glioma-associated macrophages. Triple immunofluorescence labeling revealed, for the first time, evidence for SOAT1 colocalization with Iba1 and IDH1 R132H, respectively. Furthermore, a notable difference in the amount of LD between GBM and HGA was observed. Therefore, SOAT1 suppression might be a therapeutic option to target GBM and HGA growth and invasiveness. In addition, the high expression in cells related to neuroinflammation could be beneficial for a concomitant suppression of protumoral microglia/macrophages.
Die geringe DNA-Menge von telogenen Haaren ist ein in der Literatur bekanntes Problem. Aufgrund des bekannten Wachstumszyklus menschlicher Haare ist eine mögliche Sicherung an Tatorten sehr wahrscheinlich.
Das Ziel dieser vergleichenden, experimentellen Studie war die Evaluation sechs gängiger Extraktionsverfahren im Vergleich zu einer noch nicht etablierten chinesischen Methode.
Der originäre, chinesische Lysepuffer setzt sich aus einer optimalen Kombination und Konzentration folgender Einzelkomponenten zusammen: 10mmol (Tris)-HCl (Tris(hydroxymethyl)aminomethan) (pH 8,0), 2mmol EDTA (Ethylendiamintetraessigsäure) (pH 8,0), 0,2mol NaCl und 200µg/ml Proteinase K. Dem Ansatz wurde hoc loco das nicht-ionische Tensid Triton X-100 in einer 3%-igen Konzentration zugefügt. Darüber hinaus wurde im Rahmen einer initialen Screeninguntersuchung (Harris Hämatoxylin – und DAPI-Fluoreszenzfärbung) versucht, eine möglichst verlässliche Aussage über den Typisierungserfolg eines einzelnen telogenen Haars zu geben.
Durch mehrere Versuchsreihen ergaben sich keine signifikanten Unterschiede durch den Zusatz des Oberflächentensids, noch durch eine Verlängerung der originären 30-minütigen Inkubationszeit. Die Untersuchungen der fluoreszenzgefärbten, telogenen Haare ergaben eine Korrelation zwischen der Menge der sichtbaren Zellkerne und dem Typisierungserfolg.
Decreased oligodendrocyte number in hippocampal subfield CA4 in schizophrenia: a replication study

Hippocampus-related cognitive deficits in working and verbal memory are frequent in schizophrenia, and hippocampal volume loss, particularly in the cornu ammonis (CA) subregions, was shown by magnetic resonance imaging studies. However, the underlying cellular alterations remain elusive. By using unbiased design-based stereology, we reported a reduction in oligodendrocyte number in CA4 in schizophrenia and of granular neurons in the dentate gyrus (DG). Here, we aimed to replicate these findings in an independent sample. We used a stereological approach to investigate the numbers and densities of neurons, oligodendrocytes, and astrocytes in CA4 and of granular neurons in the DG of left and right hemispheres in 11 brains from men with schizophrenia and 11 brains from age- and sex-matched healthy controls. In schizophrenia, a decreased number and density of oligodendrocytes was detected in the left and right CA4, whereas mean volumes of CA4 and the DG and the numbers and density of neurons, astrocytes, and granular neurons were not different in patients and controls, even after adjustment of variables because of positive correlations with postmortem interval and age. Our results replicate the previously described decrease in oligodendrocytes bilaterally in CA4 in schizophrenia and point to a deficit in oligodendrocyte maturation or a loss of mature oligodendrocytes. These changes result in impaired myelination and neuronal decoupling, both of which are linked to altered functional connectivity and subsequent cognitive dysfunction in schizophrenia.
Routine coronal paraffin-sections through the dorsal frontal and parieto-occipital cortex of a total of sixty cases with divergent causes of death were immunohistochemically (IHC) stained with an antibody against TMEM119. Samples of cerebrospinal fluid (CSF) of the same cases were collected by suboccipital needle-puncture, subjected to centrifugation and processed as cytospin preparations stained with TMEM119. Both, cytospin preparations and sections were subjected to computer-assisted density measurements. The density of microglial TMEM119-positive cortical profiles correlated with that of cytospin results and with the density of TMEM119-positive microglial profiles in the medullary layer. There was no statistically significant correlation between the density of medullary TMEM119-positive profiles and the cytospin data. Cortical microglial cells were primarily encountered in supragranular layers I, II, and IIIa and in infragranular layers V and VI, the region of U-fibers and in circumscribed foci or spread in a diffuse manner and high density over the white matter. We have evidence that cortical microglia directly migrate into CSF without using the glympathic pathway. Microglia in the medullary layer shows a strong affinity to the adventitia of deep vessels in the myelin layer. Selected rapidly fatal cases including myocardial infarcts and drowning let us conclude that microglia in cortex and myelin layer can react rapidly and its reaction and migration is subject to pre-existing external and internal factors. Cytospin preparations proved to be a simple tool to analyze and assess complex changes in the CNS after rapid fatal damage. There is no statistically significant correlation between cytospin and postmortem interval. Therefore, the quantitative analyses of postmortem cytospins obviously reflect the neuropathology of the complete central nervous system. Cytospins provide forensic pathologists a rather simple and easy to perform method for the global assessment of CNS affliction.
A single, specific, sensitive biochemical biomarker that can reliably diagnose a traumatic brain injury (TBI) has not yet been found, but combining different biomarkers would be the most promising approach in clinical and postmortem settings. In addition, identifying new biomarkers and developing laboratory tests can be time-consuming and economically challenging. As such, it would be efficient to use established clinical diagnostic assays for postmortem biochemistry. In this study, postmortem cerebrospinal fluid samples from 45 lethal TBI cases and 47 controls were analyzed using commercially available blood-validated assays for creatine kinase (CK) activity and its heart-type isoenzyme (CK–MB). TBI cases with a survival time of up to two hours showed an increase in both CK and CK–MB with moderate (CK–MB: AUC = 0.788, p < 0.001) to high (CK: AUC = 0.811, p < 0.001) diagnostic accuracy. This reflected the excessive increase of the brain-type CK isoenzyme (CK–BB) following a TBI. The results provide evidence that CK immunoassays can be used as an adjunct quantitative test aid in diagnosing acute TBI-related fatalities.
The aim of this pilot study was to investigate the diagnostic potential of TMEM119 as a useful microglia-specific marker in combination with immunostainings for phagocytic function and infiltrating capacity of monocytes in cases of lethal monosubstance intoxications by morphine (MOR), methamphetamine (METH), and of ethanol-associated death (ETH) respectively. Human brain tissue samples were obtained from forensic autopsies of cases with single substance abuse (MOR, n = 8; ETH, n = 10; METH, n = 9) and then compared to a cohort of cardiovascular fatalities as controls (n = 9). Brain tissue samples of cortex, white matter, and hippocampus were collected and stained immunohistochemically with antibodies against TMEM119, CD68KiM1P, and CCR2. We could document the lowest density of TMEM119-positive cells in MOR deaths with highly significant differences to the control densities in all three regions investigated. In ETH and METH deaths, the expression of TMEM119 was comparable to cell densities in controls. The results indicate that the immunoreaction in brain tissue is different in these groups depending on the drug type used for abuse.
In the last few years, quantitative analysis of metabolites in body fluids using LC/MS has become an established method in laboratory medicine and toxicology. By preparing metabolite profiles in biological specimens, we are able to understand pathophysiological mechanisms at the biochemical and thus the functional level. An innovative investigative method, which has not yet been used widely in the forensic context, is to use the clinical application of metabolomics. In a metabolomic analysis of 41 samples of postmortem cerebrospinal fluid (CSF) samples divided into cohorts of four different causes of death, namely, cardiovascular fatalities, isoIated torso trauma, traumatic brain injury, and multi-organ failure, we were able to identify relevant differences in the metabolite profile between these individual groups. According to this preliminary assessment, we assume that information on biochemical processes is not gained by differences in the concentration of individual metabolites in CSF, but by a combination of differently distributed metabolites forming the perspective of a new generation of biomarkers for diagnosing (fatal) TBI and associated neuropathological changes in the CNS using CSF samples.
The aim of this study was to investigate if the biomarkers myelin basic protein (MBP) and neurofilament-H (NF-H) yielded informative value in forensic diagnostics when examining cadaveric cerebrospinal fluid (CSF) biochemically via an enzyme-linked immunosorbent assay (ELISA) and comparing the corresponding brain tissue in fatal traumatic brain injury (TBI) autopsy cases by immunocytochemistry versus immunohistochemistry. In 21 trauma and 19 control cases, CSF was collected semi-sterile after suboccipital puncture and brain specimens after preparation. The CSF MBP (p = 0.006) and NF-H (p = 0.0002) levels after TBI were significantly higher than those in cardiovascular controls. Immunohistochemical staining against MBP and against NF-H was performed on cortical and subcortical samples from also biochemically investigated cases (5 TBI cases/5 controls). Compared to the controls, the TBI cases showed a visually reduced staining reaction against MBP or repeatedly ruptured neurofilaments against NF-H. Immunocytochemical tests showed MBP-positive phagocytizing macrophages in CSF with a survival time of > 24 h. In addition, numerous TMEM119-positive microglia could be detected with different degrees of staining intensity in the CSF of trauma cases. As a result, we were able to document that elevated levels of MBP and NF-H in the CSF should be considered as useful neuroinjury biomarkers of traumatic brain injury.
Assessing protein biomarkers to detect lethal acute traumatic brain injuries in cerebrospinal fluid

Diagnosing traumatic brain injury (TBI) from body fluids in cases where there are no obvious external signs of impact would be useful for emergency physicians and forensic pathologists alike. None of the previous attempts has so far succeeded in establishing a single biomarker to reliably detect TBI with regards to the sensitivity: specificity ratio in a post mortem setting. This study investigated a combination of body fluid biomarkers (obtained post mortem), which may be a step towards increasing the accuracy of biochemical TBI detection. In this study, serum and cerebrospinal fluid (CSF) samples from 30 acute lethal TBI cases and 70 controls without a TBI-related cause of death were evaluated for the following eight TBI-related biomarkers: brain-derived neurotrophic factor (BDNF), ferritin, glial fibrillary acidic protein (GFAP), interleukin 6 (IL-6), lactate dehydrogenase, neutrophil gelatinase-associated lipocalin (NGAL), neuron-specific enolase and S100 calcium-binding protein B. Correlations among the individual TBI biomarkers were assessed, and a specificity-accentuated threshold value analysis was conducted for all biomarkers. Based on these values, a decision tree modelling approach was performed to assess the most accurate biomarker combination to detect acute lethal TBIs. The results showed that 92.45% of acute lethal TBIs were able to be diagnosed using a combination of IL-6 and GFAP in CSF. The probability of detecting an acute lethal TBI was moderately increased by GFAP alone and considerably increased by the remaining biomarkers. BDNF and NGAL were almost perfectly correlated (p = 0.002; R\(^2\) = 0.944). This study provides evidence that acute lethal TBIs can be detected to a high degree of statistical accuracy using forensic biochemistry. The high inter-individual correlations of biomarkers may help to estimate the CSF concentration of an unknown biomarker, using extrapolation techniques.
Die vorliegende Arbeit befasste sich mit der Landmarke Pterion am menschlichen Schädel, morphometrisch untersucht wurden populationsspezifische und geschlechtsspezifische Unterschiede von vier verschiedenen Populationen.
Hierfür wurden digital vermessene Krotaphion-Sphenion-Distanzen (K-S-Distanz) für beide Seiten des Schädels von 124 afroamerikanischen Schädeln (männlich: 73, weiblich: 51), 664 euroamerikanischen Schädeln (männlich: 439, weiblich: 225), 89 deutschen Schädeln (männlich: 61, weiblich: 28) und 98 ruandischen Schädeln (männlich: 61, weiblich: 37) miteinander verglichen, um zu untersuchen, ob es einen statistisch signifikanten Unterschied bei der K-S-Distanz gibt. Dabei stellte sich heraus, dass es zwischen den Geschlechtern und den verschiedenen untersuchten Ethnien einen statistisch signifikanten Unterschied bei der K-S-Distanz gibt. Bei den weiblichen Schädeln zeigte die linke Seite mit einem Mittelwert von 13,28 mm (SD 5,25) und die rechte Seite mit einem Mittelwert von 12,5 mm (SD 4.80) eine kürzere Distanz als bei den männlichen Schädeln (links: 14,17 mm, SD 5,23, rechts: 13,53 mm, SD 5,22 mm). Im Independent T-Test zeigte sich auf beiden Seiten mit einem p-Wert < 0,05 ein statisch signifikanter geschlechtsspezifischer Unterschied.
Bei dem Vergleich zwischen den Ethnien zeigten die Schädel der deutschen Population die größte Asymmetrie, der Mittelwert für die K-S Distanz lag rechts bei 14,5 mm und links bei 15,6 mm. Somit ergibt sich eine Differenz von 1,1 mm. Die ruandische Population wies die kleinste mittlere K-S-Distanz bei einem Mittelwert für rechts und links von jeweils 8,7 mm auf und zeigten somit die stärkste Symmetrie in Bezug auf die K-S-Länge. Die ANOVA Analyse, die in dieser Studie durchgeführt wurde, ergab einen statistisch signifikanten Unterschied zwischen den einzelnen Populationen, sowohl auf der linken als auch auf der rechten Seite des Schädels. Überprüfte man in weiteren Analysen (Turkey HSD und Games-Howell-Test), wie die Gruppen abschneiden, wenn sie untereinander verglichen, so sah man, dass die afroamerikanische und die ruandische Population sich entweder von allen Gruppen oder nur von der euroamerikanischen und der deutschen Population unterscheiden. Genauso unterscheiden sich die euroamerikanische und deutsche Population entweder von allen Gruppen oder nur von der afroamerikanischen und ruandischen Population. Dieser Trend zeigt sich sowohl auf der rechten als auch auf der linken Seite.
Die durch diese Studie analysierten Unterschiede bei der K-S-Distanz bezüglich des Geschlechts und der Herkunft könnten in der weiteren Forschung hilfreich sein, um die Aussagekraft und Praktikabilität der Geschlechts- und Herkunftsdifferenzierung am Pterion zu verbessern. Gestützt von weiterer Forschung wäre ein Einsatz der K-S-Distanz für die Geschlechts- und Herkunftsdifferenzierung in Zweifelsfällen indiziert oder dann, wenn nur noch Reste des Schädels vorhanden sind. Auch wäre ein Einsatz möglich, wenn nur die Schläfe eines Schädels und damit das Pterion als Ganzes vorhanden wären.
Intrakranielle Blutungen sind im Säuglingsalter seltene, aber lebensbedrohende Ereignisse. Neben Gefäßmissbildungen, Stoffwechseldefekten sowie Störungen der Blutgerinnung kommen v. a. nichtakzidentielle Traumata, Schütteltrauma in Betracht. Die klinische Diagnostik umfasst hinsichtlich der Blutungsgenese neben Sonographie und MRT als apparatives Verfahren auch eine Fundoskopie sowie laborchemische Analysen, insbesondere der Gerinnungsparameter. Für die Blutgerinnung ist das fettlösliche Vitamin K essenziell: Frühe, klassische und späte Vitamin-K-Mangel-Blutungen werden dabei unterschieden. Um ein gehäuftes Wiederauftreten von Vitamin-K-Mangel-Blutungen bei Neugeborenen und jungen Säuglingen zu verhindern, bedarf es einer hinreichenden Aufklärung der Eltern. Eine Verweigerung der Prophylaxe scheint Folge einer weltanschaulich begründeten Ablehnung der Schulmedizin und ein zunehmendes Phänomen in wohlhabenden Industrieländern zu sein.
In this casuistry, two accidents from Germany and Switzerland are presented that happened during the shot of recoilless anti-tankweapons. In both cases, the injuries led to the death of two soldiers: A 22-year-old soldier in Germany was struck by the countermass of a so-called Davis gun which had been fired by a comrade during a firing exercise; he died from his severe injuries,especially in the abdominal part of the body. As a peculiarity of the wound morphology, it was found to be a thick-layered,metallic, gray material in the wound cavity, which corresponded to the material of the counter mass that was ejected opposite tothe shooting direction. The other case took place in Switzerland, where a 24-year-old soldier was seriously injured during anexercise with portable anti-tank rockets. At the time the shot was fired, he stood behind the launcher and was hit by the propulsionjet of the rocket motor. He died as well from his severe injuries, which were located at the chest done by the gas jet and by the veryhigh pressure. In both cases, two different causes of death were present: massive blunt violence in the first case versus a jet of hotgases of very high speed and temperature in the second case.
The aim of the present study was a refined analysis of neuroinflammation including TMEM119 as a useful microglia-specific marker in forensic assessments of traumatic causes of death, e.g., traumatic brain injury (TBI). Human brain tissue samples were obtained from autopsies and divided into cases with lethal TBI (n = 25) and subdivided into three groups according to their trauma survival time and compared with an age-, gender-, and postmortem interval-matched cohort of sudden cardiovascular fatalities as controls (n = 23). Brain tissue samples next to cortex contusions and surrounding white matter as well as samples of the ipsilateral uninjured brain stem and cerebellum were collected and stained immunohistochemically with antibodies against TMEM119, CD206, and CCR2. We could document the highest number of TMEM119-positive cells in acute TBI death with highly significant differences to the control numbers. CCR2-positive monocytes showed a significantly higher cell count in the cortex samples of TBI cases than in the controls with an increasing number of immunopositive cells over time. The number of CD206-positive M2 microglial cells increased survival time-dependent. After 3 days of survival, the cell number increased significantly in all four regions investigated compared with controls. In sum, we validate a specific and robustly expressed as well as fast reacting microglia marker, TMEM119, which distinguishes microglia from resident and infiltrating macrophages and thus offers a great potential for the estimation of the minimum survival time after TBI.
In forensic science determination of the origin and sex of skeletal remains is an important task for identification purposes. In this study we investigated the krotaphion-sphenion distance (K‑S distance) in the pterion region of German, Euro-American, African-American and Rwandan skulls of modern individuals from the nineteenth to the twenty-first century to look for statistically significant differences in sex and ancestry. We found a statistically significant sex-specific difference in the K‑S distance, which was greater in male skulls than in female skulls for both sides of the skull. Our study also showed that there is a statistically significant difference in the K‑S distance between the four populations studied. Landmarks and morphometric parameters measured in our investigations, which were not used for the present examination were provided to the software program Fordisc for its reference data to enhance the range of its usability for identification of unknown skulls or partial skulls of European individuals.
Glukosetransporter spielen eine wichtige Rolle in der Versorgung des Gehirns mit Nährstoffen und somit für den Erhalt der physiologischen Zellintegrität. Glukose wird über die Blut-Hirn-Schranke (BHS) mittels spezifischen transmembranen Transportproteinen der SLC-Genfamilie (GLUT, SGLT) befördert. Dabei scheint während physiologischen Bedingungen hauptsächlich der Glukosetransporter GLUT1 (SLC2A1) für die Energieversorgung des Gehirns zuständig zu sein.
Die Erforschung der SGLT-Expression ist in den letzten Jahren ein wichtiger Ansatzpunkt für neue Behandlungsstrategien vieler Erkrankungen, wie Diabetes Mellitus, maligne Neoplasien oder eines Herzinfarkts, geworden. Jedoch ist über deren Expression und Funktion im menschlichen Gehirn nur wenig bekannt. Besonders die Lokalisation entlang der BHS bleibt fraglich. Ein Großteil bisheriger Forschungsarbeiten beschäftigt sich hauptsächlich mit der Expressionsanalyse des Transporters SGLT1 im tierischen Gehirn in vivo (Poppe et al. 1997; Balen et al. 2008; Yu et al. 2013). Es konnte aufgezeigt werden, dass SGLT1 mRNA exklusiv in Neuronen und nicht an der BHS exprimiert wird. Dies wird durch in vitro Analysen einer humanen Hirnendothelzelllinie bestätigt. Demnach kann kein SGLT1 unter physiologischen Bedingungen nachgewiesen werden (Sajja et al. 2014). Im menschlichen Hirngewebe besitzen SGLTs somit keine zentrale Funktion für den Glukosetransport an der BHS. Im Gegensatz dazu konnte eine Expression von SGLT sowohl in vivo als auch in vitro während hypoglykämischen Bedingungen belegt werden (Vemula et al. 2009; Sajja et al. 2014). Die Expression der SGLT-Transporter während einer ischämischen Hypoglykämie führt zu der Annahme, dass diese Transporter für die Aufrechterhaltung der Energieversorgung des geschädigten Hirngewebes notwendig sind. Um die physiologischen Mechanismen nach einem Glukosemangel zu untersuchen, wurden SHT-Modelle etabliert (Salvador et al. 2013). In einem experimentellen Modell des Schädel-Hirn-Traumas im Rahmen eines DFG-gefördertes Projekts ist ein Expressionsverlauf von Glukosetransportern im Maushirn und in Hirnendothelzellen erarbeitet worden (Wais 2012; Salvador et al. 2015). Somit könnten SGLTs als Ansatzpunkt für den Nachweis der Überlebenszeit nach einem SHT fungieren.
Die vorliegende Arbeit fokussiert sich auf die Expression der Natrium-abhängigen Glukosetransporter SGLT1 und SGLT2 im menschlichen Gehirn. Hierbei liegt das Hauptaugenmerk auf der Lokalisation dieser Transporter an der menschlichen BHS von post mortalem Hirngewebe. Weiterhin wird untersucht ob die Expressionsstärke von SGLT1 und SGLT2 eine Aussage über die Überlebenszeit von Verstorbenen nach einer traumatisch bedingten Hirnveränderung zulässt.
Die Lokalisation von SGLT1 und SGLT2 an der menschlichen BHS konnte durch die Etablierung eines Protokolls zur Isolation von Hirnkapillaren erfolgen. Vorab wurden alle verwendeten Antikörper auf ihre Spezifität mittels siRNA Transfektion und Blockierung der Immunfluoreszenzsignale mittels immunisierten Peptids getestet. Somit ist die Spezifität der detektierten SGLT1- und SGLT2-Expression in menschlichen Hirnkapillaren gewährleistet. Anschließend wird untersucht, in welchen zeitlichem Verlauf nach einer traumatisch bedingten Hirnveränderung die verschiedenen Formen der Glukosetransporter exprimiert werden und ob ggf. der Umfang und die Verteilung von SGLT1, SGLT2 und GLUT1 sowie das Verhältnis zueinander Auskünfte über eine vitale bzw. postmortale Entstehung eines Traumas bzw. dessen Überlebenszeit zulässt. Hierfür wird ein Expressionsschema der Glukosetransporter generiert, abhängig von Todeszeitpunkt und Todesursache. Es konnte festgestellt werden, dass GLUT1 nicht als Target für die Ermittlung der Überlebenszeit nach einem Trauma geeignet ist. Dahingegen zeigen SGLT1 und SGLT2 eine signifikante Änderung der Expressionsstärke im contusionalen Gewebe in Abhängigkeit von der Überlebenszeit. Obwohl diese vorläufigen Daten einen neuen Ansatzpunkt für die forensische Fragestellung aufzeigen, müssen weitere Experimente mit einem erhöhten Umfang der Probenanzahl und kürzere Zeitspannen der Überlebenszeiträume durchgeführt werden.
