Gemäß den Zielen der Intersektionalitätsforschung untersucht der Beitrag die gesteigerte Diskriminierung derjenigen in Literatur und Film, die mehreren minderprivilegierten Gruppen angehören: die Frauen von Bechelaren, deren narrative Existenz im ‚Nibelungenlied‘ nur gesichert ist, solange sie in einer Relation zu ihrem Ehemann und Vormund stehen. Der Vergleich des mittelhochdeutschen Epos mit den modernen Adaptationen von Thea von Harbou und Fritz Lang zeigt, wie sich die Bedeutung der Ungleichheitskategorien fundamental verschiebt. Am negativsten wirkt sich die Verstärkung der Machtdifferenzen in Thea von Harbous ‚Nibelungenbuch‘ und Fritz Langs Film ‚Die Nibelungen‘ auf Rüdigers Frau Gotelind aus. Für die Markgräfin bleibt in den modernen Adaptationen aufgrund der rassisierenden Überformung der hunnischen Welt kein Platz mehr. Während sie im Epos Repräsentationsaufgaben übernimmt, Herrschaftspflichten erfüllt und großzügige Geschenke verteilt, tritt sie im Buch als Figur nicht in Erscheinung und gerät im Film nicht einmal in der Utopie von Bechelaren in den Blick. Gotelind wird nicht nur marginalisiert, sondern vollständig eliminiert.
Background: Scientific guidelines have been developed to update and harmonize exercise based cardiac rehabilitation (ebCR) in German speaking countries. Key recommendations for ebCR indications have recently been published in part 1 of this journal. The present part 2 updates the evidence with respect to contents and delivery of ebCR in clinical practice, focusing on exercise training (ET), psychological interventions (PI), patient education (PE). In addition, special patients' groups and new developments, such as telemedical (Tele) or home-based ebCR, are discussed as well. Methods: Generation of evidence and search of literature have been described in part 1. Results: Well documented evidence confirms the prognostic significance of ET in patients with coronary artery disease. Positive clinical effects of ET are described in patients with congestive heart failure, heart valve surgery or intervention, adults with congenital heart disease, and peripheral arterial disease. Specific recommendations for risk stratification and adequate exercise prescription for continuous-, interval-, and strength training are given in detail. PI when added to ebCR did not show significant positive effects in general. There was a positive trend towards reduction in depressive symptoms for “distress management” and “lifestyle changes”. PE is able to increase patients’ knowledge and motivation, as well as behavior changes, regarding physical activity, dietary habits, and smoking cessation. The evidence for distinct ebCR programs in special patients’ groups is less clear. Studies on Tele-CR predominantly included low-risk patients. Hence, it is questionable, whether clinical results derived from studies in conventional ebCR may be transferred to Tele-CR. Conclusions: ET is the cornerstone of ebCR. Additional PI should be included, adjusted to the needs of the individual patient. PE is able to promote patients self-management, empowerment, and motivation. Diversity-sensitive structures should be established to interact with the needs of special patient groups and gender issues. Tele-CR should be further investigated as a valuable tool to implement ebCR more widely and effectively.
In vorhandenen Arbeiten wurden geschlechtsspezifische Unterschiede hinsichtlich prä-, peri- und postoperativer Parameter in Bezug auf die isolierte koronare Bypass-Operation nachgewiesen und das weibliche Geschlecht unter anderem mit einer signifikant erhöhten 30-Tage-Mortalität in Verbindung gebracht. Unsere monozentrische, retrospektive Analyse umfasste alle 4655 Patienten und Patientinnen, die sich in der Klinik und Poliklinik für Thorax-, Herz- und thorakale Gefäßchirurgie der Universität Würzburg vom 01.01.2007 bis einschließlich 31.12.2015 einer isolierten Bypass-Operation unterzogen haben. Anhand der Daten sollte eruiert werden, ob signifikante Unterschiede bezüglich prä-, peri- und postoperativer Parameter zwischen den Geschlechtern vorliegen. Des Weiteren erfolgte eine Analyse, ob signifikante Unterschiede zwischen den Altersdekaden auszumachen sind. Die 30-Tage-Mortalität zeigte sich beim weiblichen Geschlecht im Vergleich zum männlichen Kollektiv signifikant erhöht. Sowohl bei den Männern als auch bei den Frauen stieg die 30-Tage-Mortalität mit dem Alter signifikant an. Die 30-Tage-Mortalität wurde durch das Alter, die OP-Dringlichkeit sowie durch die intraoperativen Zeiten (OP-Zeit, HLM-Zeit, Aortenabklemmzeit) signifikant beeinflusst. Das Geschlecht korrelierte signifikant mit der OP-Dringlichkeit. Unter Berücksichtigung der OP-Dringlichkeit konnten wir zeigen, dass sich die Mortalität bei Frauen im Vergleich zu den Männern allein durch die Dringlichkeitsstufe Notfall nahezu verdoppelt. In unserer Studie waren Frauen im Vergleich zu den Männern zum OP-Zeitpunkt signifikant älter, wurden signifikant häufiger als Notfall eingestuft, erhielten weniger Grafts mit signifikant niedrigeren IMA-Verwendungsraten – wobei deren Verwendung für beide Geschlechter mit steigendem Alter signifikant abnahm – erhielten signifikant mehr EK und hatten signifikant kürzere Operationszeiten (OP-Zeit, HLM-Zeit und Aortenabklemmzeit). Neben einer im Gesamtkollektiv längeren Krankenhausaufenthaltsdauer zeigte sich beim weiblichen Geschlecht im Vergleich zu den Männern eine signifikant längere Intensivdauer. Das weibliche Geschlecht und das Alter korrelierten hierbei signifikant mit der Liegedauer. Des Weiteren zeigten sich geschlechtervergleichend höhere Mediastinitisraten bei Frauen, die in der Altersgruppe der 60- bis 69-Jährigen am deutlichsten und signifikant ausfielen. Diese Tatsache und das Ergebnis, dass Frauen zum Zeitpunkt der Operation signifikant älter waren, häufiger als Notfall stationär aufgenommen wurden und mehr Blutprodukte als die männliche Vergleichsgruppe benötigten, erklärte unter anderem die signifikant längere Intensivliegedauer von Frauen gegenüber Männern in unserer Studie. Ursächlich für die sinkende arterielle Bypassverwendung bei den älteren Patienten konnte ein stärkeres Nutzen venöser statt arterieller Grafts im Alter angeführt werden. Vor allem bei jüngeren Patientinnen und Patienten wurden arterielle Grafts mit in Studien beschriebenen längeren Offenheitsraten favorisiert, um Reinterventionen nach Möglichkeit zu vermeiden. Gleichzeitig ermöglichte die signifikant höhere Rate an elektiven Aufnahmen beim männlichen Geschlecht eine längere therapeutische Planung und somit zielgerichtetere Operationen im Gegensatz zu den beim weiblichen Geschlecht dominierenden notfallmäßigen Eingriffen.
Unsere Studie konnte eine signifikant höhere 30-Tage-Mortalität bei Frauen im Vergleich zu den Männern aufzeigen und darauf hinweisen, dass Therapiemöglichkeiten und Behandlungskonzepte einer genderspezifischen Betrachtung bedürfen. Besonders die Gruppe der unter 50-jährigen Frauen, bei denen in unserer Analyse eine hohe Mortalitätsrate, die häufigsten Ultima Ratio-Operationen, die längsten intraoperativen Zeiten gepaart mit der geringsten Anzahl an genutzten Grafts und den meisten Gaben von FFP zu verzeichnen waren, waren hervorzuheben. Eine frühzeitigere Zuführung zu diagnostischen sowie therapeutischen Schritten gerade beim weiblichen Patientenkollektiv im ambulanten wie im stationären Verlauf und damit verbunden eine Sensibilisierung für atypische Symptome und Verlaufsformen bei Frauen könnte die Rate an notfallmäßigen Hospitalisierungen und Komplikationen in Zukunft maßgeblich senken.
In forensic science determination of the origin and sex of skeletal remains is an important task for identification purposes. In this study we investigated the krotaphion-sphenion distance (K‑S distance) in the pterion region of German, Euro-American, African-American and Rwandan skulls of modern individuals from the nineteenth to the twenty-first century to look for statistically significant differences in sex and ancestry. We found a statistically significant sex-specific difference in the K‑S distance, which was greater in male skulls than in female skulls for both sides of the skull. Our study also showed that there is a statistically significant difference in the K‑S distance between the four populations studied. Landmarks and morphometric parameters measured in our investigations, which were not used for the present examination were provided to the software program Fordisc for its reference data to enhance the range of its usability for identification of unknown skulls or partial skulls of European individuals.
The present study focuses on Rulin waishi 儒林外史 (The Unofficial History of the Scholars), a well-known Qing novel, from the perspective of gender. It attempts to contribute to the discussion about Chinese masculinity by identifying the representation of the scholars' masculinity in Rulin waishi and offer a better understanding of the novel's position regarding femininity and women.
This project shows that the novel nevertheless reflects rather than challenges gender ideologies of its time. The ideal manhood showed in the novel comprises real virtues and authentic learning. It goes against the traditional, orthodox Confucian masculinity which advocates that officialdom is the glorious path to fulfill a learned man's masculinity. It is mainly due to Wu Jingzi's own failure in the civil service examinations and official careers. Regarding the relation of masculinity and sexuality, the novel reveals that a masculine man is not tempted by female charm but can enjoy a harmonious and companionate marriage. Besides, scholars show great anxiety about their masculinity since they are in a marginal position in society. Their manliness is challenged by officials, merchants, and even commoners, as well as their colleagues.
Through a careful examination of stories of Pinniang, Miss Lu, and Mrs. Wang, it reveals that the novel holds a conventional opinion on women although it criticizes widow suicide and shows an egalitarian husband-wife relationship. It praises Confucian womanly virtues, such as following and serving the husband, managing the household, and keeping chastity. Female sexuality is blamed as an evil temptation to lead men to go astray. Women’s learning gains legitimacy when serving to fulfill domestic responsibilities. It carries the Confucian message that men should take the lead and maintain order in the household and reinforces the rightful patriarchy.
In a word, rather than go ahead of its time, Rulin waishi holds a conservative attitude towards gender issues.
Objective:
Over the past decade, myocardial triglyceride content has become an accepted biomarker for chronic metabolic and cardiac disease. The purpose of this study was to use proton (hydrogen 1)-magnetic resonance spectroscopy (\(^{1}\)H-MRS) at 3Tesla (3 T) field strength to assess potential gender-related differences in myocardial triglyceride content in healthy individuals.
Methods:
Cardiac MR imaging was performed to enable accurate voxel placement and obtain functional and morphological information. Double triggered (i.e., ECG and respiratory motion gating) \(^{1}\)H-MRS was used to quantify myocardial triglyceride levels for each gender. Two-sample t-test and Mann-Whitney U-test were used for statistical analyses.
Results:
In total, 40 healthy volunteers (22 male, 18 female; aged >18 years and age matched) were included in the study. Median myocardial triglyceride content was 0.28% (interquartile range [IQR] 0.17–0.42%) in male and 0.24% (IQR 0.14–0.45%) in female participants, and no statistically significant difference was observed between the genders. Furthermore, no gender-specific difference in ejection fraction was observed, although on average, male participants presented with a higher mean ± SD left ventricular mass (136.3 ± 25.2 g) than female participants (103.9 ± 16.1 g).
Conclusions:
The study showed that \(^{1}\)H-MRS is a capable, noninvasive tool for acquisition of myocardial triglyceride metabolites. Myocardial triglyceride concentration was shown to be unrelated to gender in this group of healthy volunteers.
Background
The spontaneously hypertensive rat (SHR) has been used to model changes in the central nervous system associated with cognitive-related disorders. Recent human and animal studies indicate a possible relationship between cognitive deficits, insulin resistance and hypertension. We aimed to investigate whether cognitively impaired SHRs develop central and/or peripheral insulin resistance and how their cognitive performance is influenced by the animal’s sex and age as well as strains used for comparison (Wistar and Wistar-Kyoto/WKY).
Methods
Three and seven-month-old SHR, Wistar, and WKY rats were studied for their cognitive performance using Morris Water Maze (MWM) and Passive Avoidance tests (PAT). Plasma glucose and insulin were obtained after oral glucose tolerance tests. Cerebral cortex, hippocampus, and striatum status of insulin-receptor (IR) β-subunit and glycogen synthase kinase-3β (GSK3β) and their phosphorylated forms were obtained via ELISA.
Results
SHRs performed poorly in MWM and PAT in comparison to both control strains but more pronouncedly compared to WKY. Females performed poorer than males and 7-month-old SHRs had poorer MWM performance than 3-month-old ones. Although plasma glucose levels remained unchanged, plasma insulin levels were significantly increased in the glucose tolerance test in 7-month-old SHRs. SHRs demonstrated reduced expression and increased activity of IRβ-subunit in cerebral cortex, hippocampus, and striatum with different regional changes in phospho/total GSK3β ratio, as compared to WKYs.
Conclusion
Results indicate that cognitive deficits in SHRs are accompanied by both central and peripheral insulin dysfunction, thus allowing for the speculation that SHRs might additionally be considered as a model of insulin resistance-induced type of dementia.
Background: Despite pleiotropic immunomodulatory effects of apolipoprotein E (apoE) in vitro, its effects on the clinical course of experimental autoimmune encephalomyelitis (EAE) and multiple sclerosis (MS) are still controversial. As sex hormones modify immunomodulatory apoE functions, they may explain contentious findings. This study aimed to investigate sex-specific effects of apoE on disease course of EAE and MS.
Methods: MOG\(_{35-55}\) induced EAE in female and male apoE-deficient mice was assessed clinically and histopathologically. apoE expression was investigated by qPCR. The association of the MS severity score (MSSS) and APOE rs429358 and rs7412 was assessed across 3237 MS patients using linear regression analyses.
Results: EAE disease course was slightly attenuated in male apoE-deficient (apoE\(^{-/-}\)) mice compared to wildtype mice (cumulative median score: apoE\(^{-/-}\) = 2 [IQR 0.0-4.5]; wildtype = 4 [IQR 1.0-5.0]; n = 10 each group, p = 0.0002). In contrast, EAE was more severe in female apoE\(^{-/-}\) mice compared to wildtype mice (cumulative median score: apoE\(^{-/-}\) = 3 [IQR 2.0-4.5]; wildtype = 3 [IQR 0.0-4.0]; n = 10, p = 0.003). In wildtype animals, apoE expression during the chronic EAE phase was increased in both females and males (in comparison to naive animals; p < 0.001). However, in MS, we did not observe a significant association between MSSS and rs429358 or rs7412, neither in the overall analyses nor upon stratification for sex.
Conclusions: apoE exerts moderate sex-specific effects on EAE severity. However, the results in the apoE knock-out model are not comparable to effects of polymorphic variants in the human APOE gene, thus pinpointing the challenge of translating findings from the EAE model to the human disease.
