Background: The sympathetic nervous system is considered to modulate the endotoxin-induced activation of immune cells. Here we investigate whether thoracic epidural anesthesia with its regional symapathetic blocking effect alters endotoxin-induced leukocyte-endothelium activation and interaction with subsequent endothelial injury. Methods: Sprague Dawley rats were anesthetized, cannulated and hemodynamically monitored. E. coli lipopolysaccharide (Serotype 0127: B8, 1.5 mg x kg(-1) x h(-1)) or isotonic saline (controls) was infused for 300 minutes. An epidural catheter was inserted for continuous application of lidocaine or normal saline in endotoxemic animals and saline in controls. After 300 minutes we measured catecholamine and cytokine plasma concentrations, adhesion molecule expression, leukocyte adhesion, and intestinal tissue edema. Results: In endotoxemic animals with epidural saline, LPS significantly increased the interleukin-1 beta plasma concentration (48%), the expression of endothelial adhesion molecules E-selectin (34%) and ICAM-1 (42%), and the number of adherent leukocytes (40%) with an increase in intestinal myeloperoxidase activity (26%) and tissue edema (75%) when compared to healthy controls. In endotoxemic animals with epidural infusion of lidocaine the values were similar to those in control animals, while epinephrine plasma concentration was 32% lower compared to endotoxemic animals with epidural saline. Conclusions: Thoracic epidural anesthesia attenuated the endotoxin-induced increase of IL-1 beta concentration, adhesion molecule expression and leukocyte-adhesion with subsequent endothelial injury. A potential mechanism is the reduction in the plasma concentration of epinephrine.
Der vorliegende Beitrag behandelt inhaltliche und methodische Probleme einer Längsschnittstudie, die derzeit am MPI für Psychiatrie in München an depressiven Patienten durchgeführt wird. Diese Untersuchung hat einen diagnostischen und einen kommunikationstheoretischen Aspekt. Von diagnostischem Interesse sind die depressionsspezifischen Veränderungen des kommunikativen Verhaltens. Kommunikationstheoretisch gewendet lautet die Frage, wie sich der interne Zustand einer Person in ihrem Verhalten reflektiert. In diesem Beitrag wird die Fragestellung konzentriert auf tonische Eigenschaften des Blickverhaltens im Interaktionsprozeß und deren Stabilität im Krankheitsverlauf. Zunächst wird die theoretische Grundlage referiert, die wir für das Blickverhalten entwickelt haben. Im zweiten Abschnitt wird auf Datenerhebung und Reliabilitätskontrolle eingegangen. Die Struktur der Daten bestimmt die Auswertungsmethoden. Einige der Methoden, mit denen wir derzeit arbeiten, werden im Zusammenhang mit umschriebenen Problemstellungen erläutert: Deskription und Interpretation des tonischen Blickverhaltens depressiver Patienten im Krankheitsverlauf (Abschnitt 3). Zusammenhang der individuellen Blickniveaus von Interaktionspartnern (Abschnitt 4) Deskription eines Dialogs (Interviews) als Sequenz von distinkten Zuständen. In den Zuständen werden Sprech- und Blickverhalten beider Interaktionspartner miteinander kombiniert (Abschnitt 5) Analyse der intraindividuellen Wechselwirkung zwischen Blick- und Sprechverhalten (Abschnitt 6). Die Abschnitte 3 mit 6 werden durch vorläufige Ergebnisse illustriert.
Validität und Reliabilität der Sprechkodierung einer Beobachtergruppe wurden in Abhängigkeit von der zeitlichen Auflösung untersucht. Die Validität wurde anhand der Übereinstimmung der Beobachter mit einem automatischen Sprachdetektor berechnet. Die Reliabilitätswerte für die Kodierung von Sprechen und Blikken ergaben sich aus der Übereinstimmung der Beobachter untereinander. Im wesentlichen zeigten sich folgende Ergebnisse: 1. Die Validität/Reliabilität der Sprechkodierung ist eine monotone, nichtlineare Funktion der gewählten Auflösung. Die systematischen Fehler, die auf Latenz und Trägheit der menschlichen Beobachter zurückgehen, werden bei einer Auflösung von 400 msec nahezu vollständig unterdrückt. 2. Weder bei der Erfassung des Sprech- noch des Blickverhaltens lassen sich Anzeichen für Observer-Drift feststellen. Trainierte und untrainierte Beobachter unterscheiden sich nicht signifikant. 3. Die Kodierung des Sprechverhaltens ist geringfügig reliabler als die des Blickverhaltens. Dieser Unterschied kann in der Praxis vernachlässigt werden.
Tuberculosis (TB) is one of the leading causes of death by an infectious disease. It remains a major health burden worldwide, in part due to misdiagnosis. Therefore, improved diagnostic tests allowing the faster and more reliable diagnosis of patients with active TB are urgently needed. This prospective study examined the performance of the new molecular whole-blood test T-Track\(^®\) TB, which relies on the combined evaluation of IFNG and CXCL10 mRNA levels, and compared it to that of the QuantiFERON\(^®\)-TB Gold Plus (QFT-Plus) enzyme-linked immunosorbent assay (ELISA). Diagnostic accuracy and agreement analyses were conducted on the whole blood of 181 active TB patients and 163 non-TB controls. T-Track\(^®\) TB presented sensitivity of 94.9% and specificity of 93.8% for the detection of active TB vs. non-TB controls. In comparison, the QFT-Plus ELISA showed sensitivity of 84.3%. The sensitivity of T-Track\(^®\) TB was significantly higher (p < 0.001) than that of QFT-Plus. The overall agreement of T-Track\(^®\) TB with QFT-Plus to diagnose active TB was 87.9%. Out of 21 samples with discordant results, 19 were correctly classified by T-Track\(^®\) TB while misclassified by QFT-Plus (T-Track\(^®\) TB-positive/QFT-Plus-negative), and two samples were misclassified by T-Track\(^®\) TB while correctly classified by QFT-Plus (T-Track\(^®\) TB-negative/QFT-Plus-positive). Our results demonstrate the excellent performance of the T-Track\(^®\) TB molecular assay and its suitability to accurately detect TB infection and discriminate active TB patients from non-infected controls.
Introduction: The German PID-NET registry was founded in 2009, serving as the first national registry of patients with primary immunodeficiencies (PID) in Germany. It is part of the European Society for Immunodeficiencies (ESID) registry. The primary purpose of the registry is to gather data on the epidemiology, diagnostic delay, diagnosis, and treatment of PIDs.
Methods: Clinical and laboratory data was collected from 2,453 patients from 36 German PID centres in an online registry. Data was analysed with the software Stata® and Excel.
Results: The minimum prevalence of PID in Germany is 2.72 per 100,000 inhabitants. Among patients aged 1-25, there was a clear predominance of males. The median age of living patients ranged between 7 and 40 years, depending on the respective PID. Predominantly antibody disorders were the most prevalent group with 57% of all 2,453 PID patients (including 728 CVID patients). A gene defect was identified in 36% of patients. Familial cases were observed in 21% of patients. The age of onset for presenting symptoms ranged from birth to late adulthood (range 0-88 years). Presenting symptoms comprised infections (74%) and immune dysregulation (22%). Ninety-three patients were diagnosed without prior clinical symptoms. Regarding the general and clinical diagnostic delay, no PID had undergone a slight decrease within the last decade. However, both, SCID and hyper IgE-syndrome showed a substantial improvement in shortening the time between onset of symptoms and genetic diagnosis. Regarding treatment, 49% of all patients received immunoglobulin G (IgG) substitution (70%-subcutaneous; 29%-intravenous; 1%-unknown). Three-hundred patients underwent at least one hematopoietic stem cell transplantation (HSCT). Five patients had gene therapy.
Conclusion: The German PID-NET registry is a precious tool for physicians, researchers, the pharmaceutical industry, politicians, and ultimately the patients, for whom the outcomes will eventually lead to a more timely diagnosis and better treatment.
