Die Nicotinamid-N-Methyltransferase (NNMT) reguliert den Energiehaushalt des Fettgewebes und spielt eine Rolle in der Pathogenese von Adipositas. Während die NNMT-Expression im Fettgewebe und die Konzentrationen von NNMT-Metaboliten in Serum und Urin vielfach beschrieben sind, ist bisher wenig über die spezifische Aktivität des Enzyms im humanen Fettgewebe bekannt. Ziel der vorliegenden Arbeit war es, eine fluoreszenzbasierte Methode zur Bestimmung der NNMT-Aktivität zu entwickeln und die spezifischen Aktivitäten im Fettgewebe von nicht adipösen und adipösen Probanden zu vergleichen. Eingeschlossen wurden 43 Probanden (16 adipöse, 15 nicht adipöse, 12 ehemals Adipöse nach bariatrischer Operation) in einer monozentrischen analytischen Querschnittsanalyse. Es wurde eine Methode entwickelt, optimiert und validiert, mit der die NNMT-Aktivität zahlreicher Proben unter Anfangsbedingungen in überschaubarer Zeit gemessen werden kann. Die Michaelis-Menten-Konstanten für die Kosubstrate Nicotinamid und S-Adenosylmethionin wurden mit 0,19 ± 0,11 mmol/l und 5 ± 2,5 µmol/l (Mittelwert ± Standardabweichung) bestimmt, was mehreren veröffentlichten Werten entspricht. Die spezifischen Aktivitäten im subkutanen Fettgewebe der adipösen, nicht adipösen und ehemals adipösen Probanden wurden mit 34 ± 19, 51 ± 44 und 90 ± 21 pU/mg bestimmt. Entgegen der Annahme war die Aktivität bei adipösen Probanden gegenüber nicht adipösen also nicht erhöht. Überraschend wurden sogar die höchsten Aktivitäten bei den ehemals Adipösen gemessen (p < 0,05 in der Varianzanalyse). Die Aktivitäten im Omentum-majus-Fettgewebe betrugen 57 ± 14 pU/mg (Adipöse) und 85 ± 66 pU/mg (nicht Adipöse). Die Aktivität im Omentum majus war zudem signifikant höher als im subkutanen Fett (p = 0,01).
Wie es zu einer Steigerung der NNMT-Aktivität nach Gewichtsabnahme kommt, konnte in der vorliegenden Arbeit nicht untersucht werden. Vorläufige eigene Experimente mit Mäusen legen aber nahe, dass NNMT während der Entwicklung von Adipositas dynamisch reguliert ist. Dies muss Gegenstand zukünftiger Untersuchungen sein, auch um NNMT als mögliches Ziel therapeutischer Interventionen zur Bekämpfung der sich pandemisch ausbreitenden Adipositas und ihrer Folgeerkrankungen zu definieren.
Fabry Disease (FD) is a genetic lysosomal storage disorder based on mutations in the gene encoding α-Galactosidase A (α-GalA) leading to accumulation of globotriaosylceramide (Gb3). Missense mutations induce an amino acid exchange (AAE) in the α-GalA. Pain is a predominant symptom in FD and the pathophysiology is unclear. Skin punch biopsies were obtained from 40 adult FD patients and ten healthy controls and dermal fibroblast cultures were generated for cell culture experiments to investigate Gb3 load, gene and protein expression patterns and ion channel activity. The 3D-structure of α-GalA was downloaded into Pymol Graphics System and the AAE was depicted and located in order to investigate the correlation between the AAE location type in the α-GalA and the clinical FD phenotype.
FD dermal fibroblasts showed high Gb3 load depending on treatment interval and expressed Kca1.1 channels. Activity was reduced in FD cells at baseline, but increased over-proportionately upon Gb3-cleavage by enzyme replacement therapy. Gene and protein expression of Kca1.1 was increased in FD cells. FD dermal fibroblasts showed higher gene expression of Notch1 and several cytokines. Further, it was shown that three different AAE location types can be differentiated: mutations in the active site (‘active site’), those buried in the core of α-GalA (‘buried’) and those at another location, mostly on the protein surface (‘other’). FD patients carrying active site or buried mutations showed a severe clinical phenotype with multi-organ manifestation and early disease onset. Patients with other mutations were less severely affected with oligo-organ manifestation sparing the nervous system and later disease onset.
These results show that dermal fibroblasts may be involved in FD-associated pain and that stratification of FD patients carrying missense mutations by AAE location type may be an advantageous parameter that can help in the management of FD patients.
For the past 20 years, chronic kidney disease (CKD) has remained one of the major causes of death worldwide. Cardiovascular events account for approximately 50% of deaths in CKD patients, underscoring the clinical relevance of the observed cardiac changes. These changes define uremic cardiomyopathy (UCM) and include left-ventricular hypertrophy (LVH), LV dilatation, and LV systolic and diastolic dysfunction. LVH is seen as the primary manifestation of UCM and is caused by a multitude of different systems including in-creased pre- and afterload and the renin-angiotensin system (RAS). More recent studies found that myocardial dysfunction is apparent before changes in the ventricular geome-try, like hypertrophy, occur to the uremic heart. This leads to the conclusion that LVH is not the cause of cardiac dysfunction, but one of the alterations caused by factors related to the uremic state itself. Among these factors that are independent of pressure and vol-ume overload, are cardiotonic steroids as well as the parathyroid hormone and the endo-thelin (ET-1) system. But we suggest a different substance to play an important role in UCM: Urea. Patients in end-stage renal disease (ESRD) display increased oxidative stress and urea has been found to increase levels of oxidative stress, at least in endothelial cells. Therefore, we investigated the effect that elevated urea levels, as seen in patients undergoing dialysis, have on cardiomyocytes. As the oxidative stress in a cell is regulated by mitochondrial processes, we suspected the mitochondrial orchestrator PGC-1α to play an important role.
The uremic heart is in a state of elevated oxidative stress. This has been presented by multiple authors before. By conducting immunofluorescent staining for reactive oxygen species (ROS), we tried to replicate their findings and illustrate elevated levels of ROS. As the fluorescence analysis did not bear significant results, we approached oxidative stress from a different angle: Via mass spectrometry, we looked at the amino acids methionine, cysteine and betaine which are highly involved in sustaining the oxidative balance in the cell. Our findings in the media of urea-treated HL-1 cells lead us to the conclusion, that these cardiomyocytes were in a state of low antioxidative resources.
Next, to find the intracellular mechanisms that connect uremia to oxidative stress and compromised energetics, we investigated possible downstream effectors of uremia. The urea-treated cardiomyocytes exhibited significant alterations regarding upstream effec-tors of PGC-1α: The protein kinases Akt and Erk were expressed and phosphorylated dif-ferently in a western blot analysis of uremic h9c2 cells and in mice with induced kidney failure. To combine these findings regarding the protein kinases Akt and Erk and oxidative stress, the Erk/p38 pathways seems conclusive (figure 20). This pathway links uremia and oxidative stress to intracellular effectors that have been found to influence the develop-ment of uremic cardiomyopathy.
Another life-threatening alteration in uremic cardiomyopathy is a corrupted cardiac func-tion. The myocardium of uremic patients has shown to be more susceptible to ischemic damage and most patients receiving dialysis experience repeated episodes of intradialytic impairments in cardiac function. The urea-treated cardiomyocytes had a significantly higher oxygen consumption rate due to an inefficiently increased metabolism, most likely caused by an increased level of uncoupling.
Taken together, the results of this study indicate that urea by itself plays a role in the de-velopment of uremic cardiomyopathy. So-called high-physiologic levels of urea have led to a mitochondrial inefficiency and an increase of oxidative stress in cardiomyocytes. The protein kinases Akt and Erk may work as effectors of urea to induce these changes via the Erk/p38 pathway. It seems very likely that the mitochondrial changes are mediated by the mitochondrial orchestrator PGC-1α. These observations might lead to further studies in-vestigating urea levels in dialysis patients. In the future, these might lead to a change of practice regarding tolerated urea levels in dialysis and help reduce the cardiac mortality of patients with chronic kidney disease.
In der Gruppe der Hämodialyse-PatientInnen besteht ein deutlich erhöhtes Risiko an kardiovaskulären Ereignissen zu versterben. Korrespondierend hierzu weisen Hämodia-lyse-PatientInnen eine erhöhte Prävalenz an linksventrikulärer Hypertrophie (LVH) auf. Diese gilt als starker unabhängiger Risikofaktor für kardiovaskuläre Mortalität. Auf-grund der prognostischen Aussagekraft dient die Bewertung des linksventrikulären Massenindex (LVMI) sowie die Diagnose einer LVH vor allem in prospektiven Studien als ein bedeutendes Werkzeug zur Beurteilung des kardiovaskulären Risikos. Die Be-stimmung der LVH kann anhand von bildgebenden Verfahren (u.a. Echokardiographie, CMRI) oder dem EKG erfolgen. Die CMRI-Messung wird gegenwärtig als Goldstan-dard zur Messung der LVH betrachtet.
Die 2D geführte M-mode-Methode der Echokardiographie zur Bestimmung der LVM zeichnet sich durch seine einfache und schnelle Durchführbarkeit aus und wird deshalb trotz präziserer Messverfahren wie dem 3D-Verfahren sowie diverser Einschränkungen weiterhin von der American Society of Echocardiography (ASE) als Screening-Methode und zur Untersuchung großer PatientInnenpopulationen empfohlen. Die empfohlene ASE-Formel überschätzt jedoch den LVMI nachweislich im Vergleich zum CMRI-Messverfahren. Die Überschätzung zeigte sich abhängig von der Höhe des LVMI. Es wird vermutet, dass die zunehmende Überschätzung Folge der geometrischen Grundan-nahmen ist, welche den LV vereinfachend als Ellipsoid mit konstantem L/D-Verhältnis annimmt. Dieses Verhältnis scheint sich jedoch bei zunehmender Herzgröße zu verän-dern, was wiederum zu einer Fehleinschätzung des LVMI führt. Die Teichholz (Th)-Formel korrigiert das L/D-Verhältnis mithilfe einer kurvilinearen Anpassung an den linksventrikulären Durchmesser und zeigte kürzlich in einer PatientInnengruppe mit Aor-tenstenose die geringste Tendenz der Überschätzung bei PatientInnen mit LVH. In der vorliegenden Studie wurden die echokardiographischen Formeln – ASE und Th – mit dem CMRI-Messverfahren verglichen. Beide Formeln zeigten eine deutliche Überschät-zung des LVMI. Die Th-Formel demonstrierte jedoch neben einer besseren Überein-stimmung zum CMRI, eine insgesamt geringere Überschätzung des LVMI sowie eine sukzessive Abnahme der Überschätzung mit zunehmendem LVMI. Zusammenfassend kann festgehalten werden, dass die Th-Formel der ASE-Formel in Bezug auf die Be-rechnung des LVMI bei Hämodialyse-PatientInnen insbesondere bei PatientInnen mit LVH überlegen ist. Weitere Studien sind jedoch erforderlich, um die Th-Formel in grö-ßeren Hämodialyse-PatientInnen-Kohorten mit höheren LVMI-Werten zu testen sowie um den prognostischen Wert der Th-Formel im Vergleich zur ASE-Formel zu ermitteln.
Die klassischen EKG-Indices und -Scores zur Feststellung einer LVH wiesen, wie be-reits in anderen CMRI-Vergleichsstudien gezeigt, eine schlechte Sensitivität bei guter Spezifität auf. Aufgrund dessen verlor das EKG zunehmend an Bedeutung als Scree-ning-Untersuchung. In dieser Studie wurde der Versuch unternommen die Sensitivität durch zwei Lösungsansätze zu verbessern, einerseits durch die Kombination verschiede-ner EKG-Kriterien und andrerseits durch eine Adjustierung der EKG-Kriterien an den mittels Bioimpedanz gemessenen Fettmassenanteil. Die Kombination verschiedener EKG-Kriterien erzielte eine deutlich erhöhte Sensitivität von >70 %. Auch die Anpas-sung der EKG-Kriterien an den individuellen Fettmassenanteil könnte ein hilfreicher Lösungsansatz zur Verbesserung der Sensitivität bei Adipositas darstellen.
Mineralocorticoid-receptor antagonism and its metabolic consequences in haemodialysis patients

Patients on haemodialysis are highly susceptible to different forms of heart failure. To date, the benefit of Mineralocorticoid-receptor antagonist (MRA) administration in haemodialysis patients remains subject to discussion. Biomarkers play an important role in therapy guidance and pose a promising tool to detect pathological processes of heart failure in an earlier stage. The randomised-controlled Mineralocorticoid-Receptor Antagonists in End-Stage Renal Disease (MiREnDa) trial was set up to investigate the effect of 50 mg of spironolactone once daily on left ventricular mass index in haemodialysis patients and several secondary endpoints. This dissertation reports findings from the MiREnDa trial on (a) the efficacy of spironolactone to influence serum levels of biomarkers of heart failure, fibrosis and inflammation and electrolytes and (b) the ability of N-terminal pro-B-type natriuretic peptide (NT-proBNP), Galectin-3 and soluble source of tumorigenicity 2 (sST2) to reflect left ventricular hypertrophy and diastolic dysfunction assessed by imaging characteristics. Treatment of spironolactone over a 40-week period did not alter serum levels of biomarkers of heart failure, fibrosis and inflammation including NT-proBNP, Galectin-3 and sST2. A small but significant effect on serum sodium but not potassium was observed. NT-proBNP was significantly different in the presence or absence of left ventricular hypertrophy (LVH) (normal vs. LVH (median [IQR]): 2,120 [810; 5,040] vs. 6,340 [2,410; 15,360] pg/ml, p<0.01) or moderate and severe diastolic dysfunction (DD) (normal diastolic function and DD grade I vs. DD grade II and DD grade III: 2,300 [850; 6,050] vs. 12,260 [3,340; 34,830] pg/ml, p=0.02). NT-proBNP further showed a significant correlation at baseline with LVMi (Spearman’s rho=0.41, p<0.001), LAVi (Spearman’s rho=0.55, p<0.001) and septal E/e’ (Spearman’s rho=0.45, p<0.001). No correlation was observed between Galectin-3 and the investigated functional and morphological parameters. sST2 was mildly correlated to LVMi at baseline (Spearman’s rho=0.21, p=0.05) and NT-proBNP at baseline (Spearman’s rho=0.37, p<0.001). In conclusion, spironolactone did not affect the investigated parameters but NT-proBNP proved to be significantly correlated to cardiac imaging measurements.
