Background: The COVID-19 pandemic has led to a flood of — often contradictory — evidence. HCWs had to develop strategies to locate information that supported their work. We investigated the information-seeking of different HCW groups in Germany. Methods: In December 2020, we conducted online surveys on COVID-19 information sources, strategies, assigned trustworthiness, and barriers — and in February 2021, on COVID-19 vaccination information sources. Results were analyzed descriptively; group comparisons were performed using χ\(^2\)-tests. Results: For general COVID-19-related medical information (413 participants), non-physicians most often selected official websites (57%), TV (57%), and e-mail/newsletters (46%) as preferred information sources — physicians chose official websites (63%), e-mail/newsletters (56%), and professional journals (55%). Non-physician HCWs used Facebook/YouTube more frequently. The main barriers were insufficient time and access issues. Non-physicians chose abstracts (66%), videos (45%), and webinars (40%) as preferred information strategy; physicians: overviews with algorithms (66%), abstracts (62%), webinars (48%). Information seeking on COVID-19 vaccination (2700 participants) was quite similar, however, with newspapers being more often used by non-physicians (63%) vs. physician HCWs (70%). Conclusion: Non-physician HCWs more often consulted public information sources. Employers/institutions should ensure the supply of professional, targeted COVID-19 information for different HCW groups.
Propofol is a widely used general anesthetic in clinical practice, but its use is limited by its water-insoluble nature and associated pharmacokinetic and pharmacodynamic limitations. Therefore, researchers have been searching for alternative formulations to lipid emulsion to address the remaining side effects. In this study, novel formulations for propofol and its sodium salt Na-propofolat were designed and tested using the amphiphilic cyclodextrin (CD) derivative hydroxypropyl-β-cyclodextrin (HPβCD). The study found that spectroscopic and calorimetric measurements suggested complex formation between propofol/Na-propofolate and HPβCD, which was confirmed by the absence of an evaporation peak and different glass transition temperatures. Moreover, the formulated compounds showed no cytotoxicity and genotoxicity compared to the reference. The molecular modeling simulations based on molecular docking predicted a higher affinity for propofol/HPβCD than for Na-propofolate/HPβCD, as the former complex was more stable. This finding was further confirmed by high-performance liquid chromatography. In conclusion, the CD-based formulations of propofol and its sodium salt may be a promising option and a plausible alternative to conventional lipid emulsions.
Background
Airway management is crucial and, probably, even the most important key competence in anaesthesiology, which directly influences patient safety and outcome. However, high-quality research is rarely published and studies usually have different primary or secondary endpoints which impedes clear unbiased comparisons between studies. The aim of the present study was to gather and analyse primary and secondary endpoints in video laryngoscopy studies being published over the last ten years and to create a core set of uniform or homogeneous outcomes (COS).
Methods
Retrospective analysis. Data were identified by using MEDLINE® database and the terms “video laryngoscopy” and “video laryngoscope” limited to the years 2007 to 2017. A total of 3351 studies were identified by the applied search strategy in PubMed. Papers were screened by two anaesthesiologists independently to identify study endpoints. The DELPHI method was used for consensus finding.
Results
In the 372 studies analysed and included, 49 different outcome categories/columns were reported. The items “time to intubation” (65.86%), “laryngeal view grade” (44.89%), “successful intubation rate” (36.56%), “number of intubation attempts” (23.39%), “complications” (21.24%), and “successful first-pass intubation rate” (19.09%) were reported most frequently. A total of 19 specific parameters is recommended.
Conclusions
In recent video laryngoscopy studies, many different and inhomogeneous parameters were used as outcome descriptors/endpoints. Based on these findings, we recommend that 19 specific parameters (e.g., “time to intubation” (inserting the laryngoscope to first ventilation), “laryngeal view grade” (C&L and POGO), “successful intubation rate”, etc.) should be used in coming research to facilitate future comparisons of video laryngoscopy studies.
Postoperative pulmonale Komplikationen (PPC) stellen den Hauptgrund für erhöhte Morbidität und Mortalität sowie eine längere stationäre Liegedauer nach chirurgischen Eingriffen dar. Die Elektrische Impedanztomographie (EIT) ermöglicht als strahlungsfreie Methode die bettseitige Visualisierung der regionalen pulmonalen Ventilation in einem thorakalen Querschnittsbereich über den zeitlichen Verlauf.
Die Hauptfragestellung dieser Studie war die perioperativen Veränderungen der regionalen pulmonalen Ventilation bei spontanatmenden Patienten nach abdominalchirurgischen Eingriffen in Allgemeinnarkose bis in die späte postoperative Phase zu untersuchen. Zusätzlich untersuchten wir die Lungenfunktion mittels Spirometrie. Wir nahmen eine Verschiebung der pulmonalen Ventilation in dorso-ventraler Richtung an, sowie eine postoperativ reduzierte Vitalkapazität, z.B. durch Atelektasen oder Pleuraergüsse.
In die prospektive Observationsstudie wurden 36 erwachsene Patienten eingeschlossen, die sich einem elektiven abdominalchirurgischen Eingriff unter Allgemeinanästhesie unterzogen und ein mittleres Risiko gemäß ARISCAT Score für die Entwicklung von PPC aufwiesen. Präoperativ, sowie am 1. und 3. postoperativen Tag erfolgte die Untersuchung der pulmonalen Ventilation mittels EIT in Spontanatmung, Errechnung des Center of Ventilation (COV), sowie eine Lungenfunktionsprüfung mittels Spirometrie.
Nach abdominalchirurgischen Operationen kam es zu einer statistisch signifikanten und bis zum 3. postoperativen Tag anhaltenden Verschiebung der pulmonalen Ventilation nach ventral (COVy präop. 16,5; 1. Tag postop. 17,8; 3. Tag postop. 17,4). Zudem zeigte sich eine anhaltend reduzierte Forcierten Vitalkapazität in % vom Sollwert (FVC%Soll): präop. 93%; 1. Tag postop. 58%; 3. Tag postop. 64%. Am 3. postoperativen Tag bestand unter forcierter Atmung eine negative Assoziation zwischen der Änderung des COVy und der Änderung der FVC%Soll. PPC traten bei 10 Patienten in Form von respiratorischer Insuffizienz, Atelektase und Pleuraerguss auf. Bei diesen Patienten zeigte die EIT keine komplikationsspezifischen Bilder.
Abdominalchirurgische Operationen hatten hat einen relevanten Einfluss auf die postoperative regionale Lungenventilation und somit auf die Entstehung von PPC. Die EIT hilft die Entstehung von PPC besser zu verstehen und Strategien zur Vermeidung solcher im klinischen Alltag zu implementieren.
Die vorliegende Arbeit befasst sich mit der Beschreibung des Status quo der Versorgungsrealität von BARMER Patient*innen, welche nach operativem inguinalen- oder femoralen Hernienverschluss an Schmerzen litten und geht in weiterer Folge dessen Hinweisen auf CPIP nach. Es fand die Sekundärdatenanlyse von Routinedaten der BARMER Krankenkasse Anwendung. Die Stichprobe umfasste 11221 Patient*innen, von denen 77.7% unter keinen Leistenschmerzen im prä- oder postoperativen Zusammenhang mit dem Eingriff litten, bezeichnet als Gruppe „Pain 0“. 4.2% litten sowohl innerhalb von 365 Tagen vor- als auch nach dem Krankenhausaufenthalt an Schmerzen, was als chronisch zu bezeichnen war und unter Gruppe „Pain 2“ geführt wurde. 8.5% der Patient*innen litten nur innerhalb von 365 Tagen nach Entlassung an Schmerzen, was nur im erweiterten Sinne auf CPIP hinwies, da der Ausschluss der ersten 90 Tage postoperativ nicht in der Definition der Gruppe enthalten war. Diese Patient*innen gehörten der Gruppe „Pain 1“ an. Die Gruppe „Pain 3“ umfasste diejenigen 9.6% der Patient*innen, welche innerhalb von 365 Tagen präoperativ an Schmerzen litten. Obwohl keine postoperativen Leistenschmerzen für diese Patient*innen codiert worden sind, stellte sich eine bessere Versorgung als die der Gruppe „Pain 0“ dar.
Patient*innen der Gruppe „Pain 2“ mit der längsten Schmerzerfahrung wurden signifikant besser versorgt. Diese Gruppe, welche an chronischen, postoperativen, inguinalen Schmerzen litt, zeichnete sich durch eine signifikant jüngere Patient*innenklientel aus. Der Anteil an Frauen war signifikant höher. Begleitende psychiatrische Komorbiditäten traten signifikant häufiger auf. Die Versorgung dieser Patient*innengruppe war signifikant besser, allerdings vor allem hinsichtlich der psychologischen und psychiatrischen Betreuung nicht ausreichend gut. Die Mehrzahl der Analysen war hochsignifikant, deren Effektstärke fiel klein aus.
Interpreting blood gas analysis results can be challenging for the clinician, especially in stressful situations under time pressure. To foster fast and correct interpretation of blood gas results, we developed Visual Blood. This computer-based, multicentre, noninferiority study compared Visual Blood and conventional arterial blood gas (ABG) printouts. We presented six scenarios to anaesthesiologists, once with Visual Blood and once with the conventional ABG printout. The primary outcome was ABG parameter perception. The secondary outcomes included correct clinical diagnoses, perceived diagnostic confidence, and perceived workload. To analyse the results, we used mixed models and matched odds ratios. Analysing 300 within-subject cases, we showed noninferiority of Visual Blood compared to ABG printouts concerning the rate of correctly perceived ABG parameters (rate ratio, 0.96; 95% CI, 0.92–1.00; p = 0.06). Additionally, the study revealed two times higher odds of making the correct clinical diagnosis using Visual Blood (OR, 2.16; 95% CI, 1.42–3.29; p < 0.001) than using ABG printouts. There was no or, respectively, weak evidence for a difference in diagnostic confidence (OR, 0.84; 95% CI, 0.58–1.21; p = 0.34) and perceived workload (Coefficient, 2.44; 95% CI, −0.09–4.98; p = 0.06). This study showed that participants did not perceive the ABG parameters better, but using Visual Blood resulted in more correct clinical diagnoses than using conventional ABG printouts. This suggests that Visual Blood allows for a higher level of situation awareness beyond individual parameters’ perception. However, the study also highlighted the limitations of today’s virtual reality headsets and Visual Blood.
Visual Blood, a 3D animated computer model to optimize the interpretation of blood gas analysis

Acid–base homeostasis is crucial for all physiological processes in the body and is evaluated using arterial blood gas (ABG) analysis. Screens or printouts of ABG results require the interpretation of many textual elements and numbers, which may delay intuitive comprehension. To optimise the presentation of the results for the specific strengths of human perception, we developed Visual Blood, an animated virtual model of ABG results. In this study, we compared its performance with a conventional result printout. Seventy physicians from three European university hospitals participated in a computer-based simulation study. Initially, after an educational video, we tested the participants’ ability to assign individual Visual Blood visualisations to their corresponding ABG parameters. As the primary outcome, we tested caregivers’ ability to correctly diagnose simulated clinical ABG scenarios with Visual Blood or conventional ABG printouts. For user feedback, participants rated their agreement with statements at the end of the study. Physicians correctly assigned 90% of the individual Visual Blood visualisations. Regarding the primary outcome, the participants made the correct diagnosis 86% of the time when using Visual Blood, compared to 68% when using the conventional ABG printout. A mixed logistic regression model showed an odds ratio for correct diagnosis of 3.4 (95%CI 2.00–5.79, p < 0.001) and an odds ratio for perceived diagnostic confidence of 1.88 (95%CI 1.67–2.11, p < 0.001) in favour of Visual Blood. A linear mixed model showed a coefficient for perceived workload of −3.2 (95%CI −3.77 to −2.64) in favour of Visual Blood. Fifty-one of seventy (73%) participants agreed or strongly agreed that Visual Blood was easy to use, and fifty-five of seventy (79%) agreed that it was fun to use. In conclusion, Visual Blood improved physicians’ ability to diagnose ABG results. It also increased perceived diagnostic confidence and reduced perceived workload. This study adds to the growing body of research showing that decision-support tools developed around human cognitive abilities can streamline caregivers’ decision-making and may improve patient care.
The relevance of user experience in safety–critical domains has been questioned and lacks empirical investigation. Based on previous studies examining user experience in consumer technology, we conducted an online survey on positive experiences with interactive technology in acute care. The participants of the study consisted of anaesthesiologists, nurses, and paramedics (N = 55) from three German cities. We report qualitative and quantitative data examining (1) the relevance and notion of user experience, (2) motivational orientations and psychological need satisfaction, and (3) potential correlates of hedonic, eudaimonic, and extrinsic motivations such as affect or meaning. Our findings reveal that eudaimonia was the most salient aspect in these experiences and that the relevance of psychological needs is differently ranked than in experiences with interactive consumer technology. We conclude that user experience should be considered in safety–critical domains, but research needs to develop further tools and methods to address the domain-specific requirements.
Postoperative Übelkeit und postoperatives Erbrechen (PONV) sind eine der häufigsten und für Patient*innen unangenehmsten Nebenwirkungen einer Allgemeinanästhesie. Trotz jahrzehntelanger Forschung und der Vielfalt an mittlerweile bekannten Maßnahmen und Substanzen zur PONV-Prophylaxe und -Therapie gibt es noch keine Strategie, die eine sichere Vermeidung oder stets wirksame Therapie von PONV garantieren kann. In vorangegangenen Studien zeigte Amisulprid als Dopaminantagonist an den Rezeptortypen D2 und D3 vielversprechende Ergebnisse zur PONV-Prophylaxe und -Therapie.
Die dieser Arbeit zugrunde liegende prospektive, randomisierte, Placebo-kontrollierte Doppelblindstudie untersuchte die Wirksamkeit einer Einzeldosis APD421 5 mg bzw. 10 mg zur Therapie von manifestem PONV nach fehlgeschlagener PONV-Prophylaxe. „Complete Response“ (CR) wurde definiert als das Ausbleiben jeglicher weiterer emetischer Episoden im Zeitraum von 30 Minuten bis 24 Stunden nach Applikation des Studienmedikaments sowie keine Gabe von antiemetischer Rescue-Medikation im gesamten Zeitraum bis 24 Stunden nach Applikation des Studienmedikaments. Die CR-Raten lagen bei 41,7% für APD421 10 mg, 33,8% für APD421 5 mg und 28,5% für Placebo, wobei die Studienarme jeweils 230, 237 bzw. 235 Patient*innen umfassten. Eine Dosis APD421 10 mg zeigte somit statistisch signifikante Überlegenheit in der PONV-Therapie gegenüber Placebo. Auch hinsichtlich sekundärer Studienendpunkte wie Auftreten bzw. Stärke von Übelkeit, Würgen oder Erbrechen und Bedarf an Rescue-Medikation war APD421 10 mg gegenüber Placebo überlegen. Eine Dosis von 5 mg APD421 zeigte für die meisten Endpunkte hingegen keine statistisch signifikante Überlegenheit gegenüber Placebo.
Limitationen der Studie liegen im Ausschluss von Patientengruppen wie beispielsweise Kindern oder bestimmten Vorerkrankungen und dem mit über 90% sehr hohen Anteil weiblicher Patient*innen. Es bleiben weitere Studien abzuwarten, die APD421 einem direkten Vergleich mit bislang etablierten Substanzen zur PONV-Therapie unterziehen, um den künftigen Stellenwert der Substanz im klinischen Alltag einschätzen zu können.
Trotz des breiten, klinischen Einsatzes des Propofols haben sich aufgrund bestehender Nebenwirkungen zunehmend Bestrebung entwickelt, alternative Formulierungen zur Lipidemulsion zu finden. In dieser Arbeit wurden die Substanzen Propofol/HPβCD und Na-Propofolat/HPβCD als alternative Formulierungen des Propofols mittels 1H-NMR-Spektroskopie, molekularer Modellierungen, DSC-Messungen, Zytotoxizitätstests (MTT-Test, LDH-Assay, EZ4U-Test) und dem Comet-Assay als Genotoxizitätstest auf biologische, chemische und physikalische Eigenschaften untersucht. Die Frage nach der Komplexbildung zwischen Propofol und HPβCD lässt sich nicht allein auf Basis des 1D-1H-NMR-Spektrums beantworten. Allerdings lässt sich mit Hilfe der 1H-NMR-Spektroskopie für Na-Propofolat/HPβCD das Vorliegen als deprotonierte Form nachweisen. Anderseits legen aber die DSC-Messungen eine Komplexbildung zwischen Propofol und HPβCD nahe. Zum einen ergibt sich das aus den unterschiedlichen Glasübergangstemperaturen der Substanzen. Zum anderen zeigt der Kurvenverlauf für HPβCD als auch für Na-Propofolat/HPβCD und Propofol/HPβCD keinen Verdampfungspeak bis zu der untersuchten maximalen Temperatur von 280 °C trotz des Propofol-Siedepunktes von 256 °C. Die molekularen Modellierungen legen nahe, dass die Bindung zwischen Propofol und Cyclodextrin über die Isopropylgruppen des Propofols erfolgt, wobei der aromatische Ring nicht in die Cavität des Cyclodextrinringes eindringt. Aus den molekularen Modellierungen ergeben sich Enthalpiewerte, die in ihrer Größenordnung schwachen Wasserstoffbrückenbindungen entsprechen. Für das Na-Propofolat/HPβCD lässt sich eine leichtere Abspaltung des Propofols aus dem Komplex voraussagen, wohingegen für Propofol/HPβCD die Abspaltung langsamer, aber über einen längeren Zeitraum erfolgen dürfte. Im Vergleich zu der hohen Zytotoxizität des Propofols als Lipidemulsion ergibt sich aus den an der CerebEND-Zelllinie durchgeführten Zyotoxizitätstests für HPβCD, Propofol/HPβCD und Na-Propofolat/HPβCD kein Nachweis einer Zytotoxizität nach 24-stündiger Exposition. Die Resultate zeigen für die drei Testverfahren die jeweils gleiche Reihenfolge mit der höchsten Zellvitalität für die mit Na-Propofolat/HPβCD behandelten Zellen und der niedrigsten Zellvitalität für HPβCD. In dem Comet-Assay zeigt sich nach 24-stündiger Exposition bei 37 °C für HPβCD, Propofol/HPβCD und Na-Propofolat/HPβCD keine genotoxische Wirkung an der HL-60-Zelllinie. Die Ergebnisse dieser Arbeit sprechen für die Fortsetzung der Untersuchungen von Propofol/HPβCD und Na-Propofolat/HPβCD als aussichtsreiche Option der Propofol-Formulierungen.
Artificial intelligence (AI) is predicted to play an increasingly important role in perioperative medicine in the very near future. However, little is known about what anesthesiologists know and think about AI in this context. This is important because the successful introduction of new technologies depends on the understanding and cooperation of end users. We sought to investigate how much anesthesiologists know about AI and what they think about the introduction of AI-based technologies into the clinical setting. In order to better understand what anesthesiologists think of AI, we recruited 21 anesthesiologists from 2 university hospitals for face-to-face structured interviews. The interview transcripts were subdivided sentence-by-sentence into discrete statements, and statements were then grouped into key themes. Subsequently, a survey of closed questions based on these themes was sent to 70 anesthesiologists from 3 university hospitals for rating. In the interviews, the base level of knowledge of AI was good at 86 of 90 statements (96%), although awareness of the potential applications of AI in anesthesia was poor at only 7 of 42 statements (17%). Regarding the implementation of AI in anesthesia, statements were split roughly evenly between pros (46 of 105, 44%) and cons (59 of 105, 56%). Interviewees considered that AI could usefully be used in diverse tasks such as risk stratification, the prediction of vital sign changes, or as a treatment guide. The validity of these themes was probed in a follow-up survey of 70 anesthesiologists with a response rate of 70%, which confirmed an overall positive view of AI in this group. Anesthesiologists hold a range of opinions, both positive and negative, regarding the application of AI in their field of work. Survey-based studies do not always uncover the full breadth of nuance of opinion amongst clinicians. Engagement with specific concerns, both technical and ethical, will prove important as this technology moves from research to the clinic.
Das zeitgleiche Auftreten eines ischämischen Schlaganfalls sowie eines Takotsubo-Syndroms (TTS) scheint eine relevante, bisher nicht ausreichend verstandene klinische Konstellation zu sein. Die Pathologien können als über die Hirn-Herz-Achse gekoppelt verstanden werden, in die die Blut-Hirn-Schranke (BHS) als funktionale Komponente integriert ist. Das klinisch-neurologische Outcome dieses Patient:innen-Kollektivs scheint signifikant schlechter zu sein als nach solitärem ischämischen Insult. Es wurde hypothetisiert, dass die BHS in besonderem Maße kompromittiert sein könnte. Das vorwiegend weibliche, postmenopausale Patient:innenkollektiv präsentierte laborchemisch elevierte Katecholaminspiegel sowie Entzündungsparameter. Diese Konditionen wurden unter Sauerstoff-Glucose-Entzug (OGD) in vitro simuliert und resultierende Alterationen eines etablierten BHS-Modells aus murinen cEND-Zellen der cerebralen Mikrozirkulation untersucht. Die Evaluation der BHS-Integrität erfolgte anhand von spezifischen Junktionsproteinen sowie Integrinuntereinheiten. Alle Versuche wurden parallel unter Östrogen-Applikation (E2) durchgeführt, um die mögliche BHS-Protektion durch das weibliche Sexualhormon zu untersuchen. Die getrennte Applikation von Katecholaminen (KAT) sowie Entzündungsmediatoren (INF) führte gegenüber der simultanen Applikation zu einem geringeren BHS-Schaden. Dieser erschien zeitgebunden, wobei sich das Ausmaß gewissermaßen proportional zur Einwirkdauer verhielt. Auswirkungen von OGD sowie einer Reoxygenierung, im Sinne einer simulierten Reperfusion, potenzierten sich mit den Effekten von KAT/INF. Überwiegend kompromittierten OGD und KAT/INF die BHS-Integrität, wobei nach Reoxygenierung eine „Erholung“ oder ein „Reperfusionsschaden“ vorlag. Eine Protektion durch E2 war morphologisch nachweisbar, speziell gegenüber OGD, KAT/INF sowie einem „Reperfusionsschaden“. Auf Ebene der Gen- sowie Proteinexpression konnte dies nicht gezeigt werden. Die Homöostase des ZNS würde in vivo beeinträchtigt, Katecholamine sowie Entzündungsmediatoren könnten ungehindert das bereits durch die Ischämie geschädigte neuronale Gewebe erreichen. Insgesamt trägt diese Arbeit zu einem Verständnis der molekularen BHS-Veränderungen im Kontext des zeitgleichen Auftretens von TTS und einem ischämischem Insult bei. Es wurde eine experimentelle Grundlage geschaffen, um zukünftig pathogenetische Hintergründe weiter erforschen zu können. Darauf aufbauend könnten, nach weiterer in vitro- sowie in vivo-Forschung, klinische Therapiekonzepte optimiert werden.
Purpose
Therapeutic options for breast cancer (BC) treatment are constantly evolving. The Human Epidermal Growth Factor 2 (HER2)-low BC entity is a new subgroup, representing about 55% of all BC patients. New antibody–drug conjugates demonstrated promising results for this BC subgroup. Currently, there is limited information about the conversion of HER2 subtypes between primary tumor and recurrent disease.
Methods
This retrospective study included women with BC at the University Medical Centre Wuerzburg from 1998 to 2021. Data were retrieved from patients' records. HER2 evolution from primary diagnosis to the first relapse and the development of secondary metastases was investigated.
Results
In the HR-positive subgroup without HER2 overexpression, HER2-low expression in primary BC was 56.7 vs. 14.6% in the triple-negative subgroup (p < 0.000). In the cohort of the first relapse, HER2-low represented 64.1% of HR-positive vs. 48.2% of the triple-negative cohort (p = 0.03). In patients with secondary metastases, HER2-low was 75.6% vs. 50% in the triple negative subgroup (p = 0.10). The subgroup of HER2-positive breast cancer patients numerically increased in the course of disease; the HER2-negative overall cohort decreased. A loss of HER2 expression from primary BC to the first relapse correlated with a better OS (p = 0.018). No clinicopathological or therapeutic features could be identified as potential risk factors for HER2 conversion.
Conclusion
HER2 expression is rising during the progression of BC disease. In view of upcoming therapeutical options, the re-analysis of newly developed metastasis will become increasingly important.
Hintergrund und Fragestellung
Im Rahmen der überörtlichen Katastrophenhilfe war die Medizinische Task Force 47 (Unterfranken) im Juli 2021 bei der Hochwasserkatastrophe in Rheinland-Pfalz (Ahrtal) im Einsatz. Mit dem Ziel, mögliche Verbesserungen im Katastrophenschutz aus Sicht einer überregionalen Einheit aus dem Einsatz ableiten zu können, wurde dieser wissenschaftlich evaluiert und die Erkenntnisse in einen übergeordneten Kontext gesetzt.
Material und Methoden
Nach Definition eines konkreten Auswerteprozesses wurden durch ein interdisziplinäres Expertengremium Einsatzunterlagen und relevante Konzepte des Bayerischen Roten Kreuzes gesichtet. Auf dieser Basis wurden Strategien und Taktiken entwickelt, um die vordefinierten Ziele zu erreichen.
Ergebnisse
Die Leistungsfähigkeit der Einsatzeinheiten könnte durch moderne Einsatzmittel (E-Bike, Drohnen, hochgeländegängige Fahrzeuge) gesteigert werden. Zur Erhöhung der Reaktionsfähigkeit könnten neue Schnell-Einsatz-Gruppen (SEG) erforderlich sein, die als Teil von BOS-übergreifenden Erkundungseinheiten agieren und in unwegsamen Geländen autark Einsatzaufträge abarbeiten können (SEG Erkundung und SEG Gelände-Infrastruktur-Logistik-Transport). Die taktischen Einheiten könnten in die regionale und überregionale Katastrophenhilfe eingebunden und synchronisiert werden.
Diskussion
Für den weitestgehend ehrenamtlich organisierten Katastrophenschutz in Deutschland könnte es in Zukunft erschwert sein, die Vielzahl möglicher modernster Einsatzmittel im hochkomplexen Umfeld einer Katastrophe sicher einsetzen zu können. Eine Teilprofessionalisierung durch hauptamtliche Führungs- und Einsatzkräfte könnte eine Lösung hierfür sein.
Purpose
Anaemia is common in patients presenting with aneurysmal subarachnoid (aSAH) and intracerebral haemorrhage (ICH). In surgical patients, anaemia was identified as an idenpendent risk factor for postoperative mortality, prolonged hospital length of stay (LOS) and increased risk of red blood cell (RBC) transfusion. This multicentre cohort observation study describes the incidence and effects of preoperative anaemia in this critical patient collective for a 10-year period.
Methods
This multicentre observational study included adult in-hospital surgical patients diagnosed with aSAH or ICH of 21 German hospitals (discharged from 1 January 2010 to 30 September 2020). Descriptive, univariate and multivariate analyses were performed to investigate the incidence and association of preoperative anaemia with RBC transfusion, in-hospital mortality and postoperative complications in patients with aSAH and ICH.
Results
A total of n = 9081 patients were analysed (aSAH n = 5008; ICH n = 4073). Preoperative anaemia was present at 28.3% in aSAH and 40.9% in ICH. RBC transfusion rates were 29.9% in aSAH and 29.3% in ICH. Multivariate analysis revealed that preoperative anaemia is associated with a higher risk for RBC transfusion (OR = 3.25 in aSAH, OR = 4.16 in ICH, p < 0.001), for in-hospital mortality (OR = 1.48 in aSAH, OR = 1.53 in ICH, p < 0.001) and for several postoperative complications.
Conclusions
Preoperative anaemia is associated with increased RBC transfusion rates, in-hospital mortality and postoperative complications in patients with aSAH and ICH.
Hintergrund und Fragestellung
Die Entwöhnung von Beatmungsgeräten wird nicht immer auf der primär behandelnden Intensivstation abgeschlossen. Die Weiterverlegung in andere Behandlungseinrichtungen stellt einen sensiblen Abschnitt in der Behandlung und Rehabilitation des Weaningpatienten dar. Ziel der vorliegenden Studie war die Untersuchung des Überleitungsmanagements und des Interhospitaltransfers von Weaningpatienten unter besonderer Berücksichtigung der Dokumentationsqualität.
Methodik
Es erfolge eine retrospektive Datenanalyse eines Jahrs (2018) auf 2 Intensivstationen eines Universitätsklinikums. Eingeschlossen wurden alle beatmeten Patienten mit folgenden Tracerdiagnosen: COPD, Asthma, Polytrauma, Pneumonie, Sepsis, ARDS und Reanimation (Beatmung > 24 h).
Ergebnisse
Insgesamt konnten 750 Patienten in die Untersuchung eingeschlossen werden (Alter 64 [52, 8–76; Median, IQR]; 32 % weiblich). Davon waren 48 (6,4 %) Patienten zum Zeitpunkt der Verlegung nicht entwöhnt (v. a. Sepsis und ARDS). Die Routinedokumentation war bei den Abschnitten „Spontaneous Breathing Trial“, „Bewertung der Entwöhungsbereitschaft“ und „vermutete Entwöhnbarkeit“ ausreichend, um die Erfüllung der Parameter der S2k-Leitlinie „Prolongiertes Weaning“ adäquat zu beurteilen. Vorwiegend wurden diese Patienten mit Tracheostoma (76 %) in Rehabilitationskliniken (44 %) mittels spezialisierten Rettungsmitteln des arztbegleiteten Patiententransports verlegt (75 %).
Diskussion
Die Verlegung nicht entwöhnter Patienten nach initialem Intensivaufenthalt ist ein relevantes Thema für den Interhospitaltransfer. Die Routinedokumentation eines strukturierten Weaningprozesses ist in Kernelementen ausreichend, um den Weaningprozess lückenlos zu beschreiben. Dies ist für die Kontinuität in der Weiterbehandlung dieser Patienten von großer Bedeutung.

Hintergrund
Internationale Studien belegen negative Auswirkungen der COVID-19-Pandemie auf Stimmung und Stresslevel befragter Personen. Auch konnten Zusammenhänge zwischen der Pandemie und höheren Schmerzstärken sowie stärkerer schmerzbedingter Beeinträchtigung nachgewiesen werden. Die Studienlage dazu, ob ältere Menschen besser oder schlechter mit der Pandemie und ihren Auswirkungen umgehen können als jüngere Personen, ist aber uneindeutig.
Methodik
Seit einigen Jahren bietet das Universitätsklinikum Würzburg ein multimodales Schmerztherapieprogramm für SeniorInnen an. Für die vorliegende Arbeit wurden retrospektiv klinische Routinedaten zum Zeitpunkt des interdisziplinären multimodalen Assessments von n = 75 TeilnehmerInnen in den Jahren 2018 und 2019 mit denen von n = 42 Patientinnen während der COVID-19-Pandemie 2020–2021 verglichen. Wir untersuchten Schmerz, psychische Belastung und körperliches Funktionsniveau mithilfe des Deutschen Schmerzfragebogens, klinischer Diagnostik und geriatrischer Funktionstests.
Ergebnisse
Die beiden Teilstichproben unterschieden sich nicht in demografischen Merkmalen. Bezüglich Schmerzintensität und Beeinträchtigung sowie der psychischen Belastung fanden sich ebenfalls keine signifikanten Unterschiede. Lediglich die Anzahl der schmerzbedingt beeinträchtigten Tage war vor Corona signifikant höher. Die geriatrischen Funktionstests zeigten signifikant bessere Werte während der Pandemie an.
Diskussion
Die vorliegenden Daten zeigen keine Verschlechterung von Schmerz und körperlichem sowie psychischem Wohlbefinden bei SeniorInnen vor dem Hintergrund der Pandemie. Weitere Studien sollten die möglichen Gründe dafür untersuchen. Diese könnten in einer höheren Resilienz der SeniorInnen basierend auf ihrer Lebenserfahrung, finanziellen Sicherheit oder einer geringeren Veränderung des Lebensalltags liegen.
PURPOSE
The threat of national and international terrorism remains high. Preparation is the key requirement for the resilience of hospitals and out-of-hospital rescue forces. The scientific evidence for defining medical and tactical strategies often feeds on the analysis of real incidents and the lessons learned derived from them. This systematic review of the literature aims to identify and systematically report lessons learned from terrorist attacks since 2001.
METHODS
PubMed was used as a database using predefined search strategies and eligibility criteria. All countries that are part of the Organization for Economic Cooperation and Development (OECD) were included. The time frame was set between 2001 and 2018.
RESULTS
Finally 68 articles were included in the review. From these, 616 lessons learned were extracted and summarized into 15 categories. The data shows that despite the difference in attacks, countries, and casualties involved, many of the lessons learned are similar. We also found that the pattern of lessons learned is repeated continuously over the time period studied.
CONCLUSIONS
The lessons from terrorist attacks since 2001 follow a certain pattern and remained constant over time. Therefore, it seems to be more accurate to talk about lessons identified rather than lessons learned. To save as many victims as possible, protect rescue forces from harm, and to prepare hospitals at the best possible level it is important to implement the lessons identified in training and preparation.
Effect of Tjap1 knock-down on blood-brain barrier properties under normal and hypoxic conditions

Stroke is one of the leading causes of mortality and disability worldwide. The blood-brain barrier (BBB) plays an important role in maintaining brain homeostasis by tightly regulating the exchange of substances between circulating blood and brain parenchyma. BBB disruption is a common pathologic feature of stroke and traumatic brain injury. Understanding the cellular and molecular events that affect the BBB after ischaemic brain injury is important to improve patient prognosis.
We have previously shown that microRNA-212/132 is elevated in hypoxic brain microvascular endothelial cells and acts through suppressing the expression of direct microRNA-212/132 target genes with function at the BBB: claudin-1, junctional adhesion molecule 3 (Jam3) and tight-junction associated protein 1 (Tjap1). While the role of claudin-1 and Jam3 at the BBB is well known, the role of Tjap1 is still unclear. The aim of this work was therefore to characterize the role of Tjap1 in brain endothelial cells using a knock-down (KD) approach in established murine in vitro BBB models cEND and cerebEND. Tjap1 KD was established by stable transfection of a plasmid expressing shRNA against Tjap1. The successful downregulation of Tjap1 mRNA and protein was demonstrated by qPCR and Western blot. Tjap1 KD resulted in impaired barrier properties of endothelial cells as shown by lower TEER values and higher paracellular permeability. Interestingly, the Tjap1 KD cells showed lower cell viability and proliferation but migrated faster in a wound healing assay. In the tube formation assay, Tjap1 KD cell lines showed a lower angiogenic potential due to a significantly lower tube length and number as well as a lower amount of branching points in formed capillaries. Tjap1 KD cells showed changes in gene and protein expression. The TJ proteins claudin-5, Jam3 and ZO-1 were significantly increased in Tjap1 KD cell lines, while occludin was strongly decreased. In addition, efflux pump P-glycoprotein was downregulated in Tjap1 KD cells. Oxygen-glucose deprivation (OGD) is a method to mimic stroke in vitro. Brain endothelial cell lines treated with OGD showed lower barrier properties compared to cells cultured under normal condition. These effects were more severe in Tjap1 KD cells, indicating active Tjap1 involvement in the OGD response in brain microvascular endothelial cells.
We thus have shown that Tjap1 contributes to a tight barrier of the BBB, regulates cell viability and proliferation of endothelial cells, suppresses their migration and promotes new vessel formation. This means that Tjap1 function is important for mature BBB structure in health and disease.
In Vitro Toxizität der Nanopartikel Graphen und Siliciumdioxid für die Medikamentenapplikation

Graphen und Siliciumdioxid Nanopartikel sind als Trägersubstanz für Medikamente beim Drug Targeting von Interesse. Diese Arbeit ist eine toxikologische Untersuchung der Nanopartikel Graphen und Siliciumdioxid im Zellmodell. Dabei wurden Graphen Nanopartikel mit einer Dicke von 6 bis 8 nm und einer Breite von 15 µm verwendet. Die verwendeten Siliciumdioxid Nanopartikel waren kugelförmig und porös mit einer Partikel-Größe von 5 bis 20 nm. Die dosisabhängige Toxizität (Konzentrationen 0,01 mg/ml, 0,1 mg/ml und 1 mg/ml, Inkubation über 24 Stunden) gegenüber 5 verschiedenen Zelllinien (cerebEND, Caco-2, Hep G2, HEK-293, H441) wurde geprüft. Dabei kamen Zellviabilitätstests (CellTiter-Glo Assay, EZ4U-Test) zum Einsatz. Zudem wurde mit den Apoptose-Markern Bax und Caspase-3 auf Gen- und Proteinebene (Polymerasekettenreaktion und Western Blot) überprüft, ob eine Apoptose eingeleitet wurde.
Zur Untersuchung der Zellviabilität wurde der CellTiter-Glo Assay verwendet. Für Graphen Nanopartikel zeigte sich ab einer Konzentration von 1 mg/ml bei den Zelllinien HEK-293 und H441 ein statistisch signifikanter Abfall der Zellviabilität. CerebEND und Hep G2 Zellen reagierten auf Graphen Nanopartikel ab einer Konzentration von 1 mg/ml ebenfalls mit einem deutlichen Abfall der Zellviabilität, diese Ergebnisse waren jedoch nicht statistisch signifikant. Die Zelllinie Caco-2 zeigte sich von den Graphen Nanopartikeln unbeeindruckt, es kam zu keiner statistisch signifikanten Veränderung der Zellviabilität. Siliciumdioxid Nanopartikel bewirkten ab einer Konzentration von 1 mg/ml einen statistisch signifikanten Abfall der Zellviabilität bei den Zelllinien cerebEND, HEK-293 und H441. HepG2 Zellen zeigten bei 1 mg/ml Siliciumdioxid einen deutlichen aber statistisch nicht signifikanten Abfall der Zellviabilität. Die Zelllinie Caco-2 erwies sich auch bei Siliciumdioxid Nanopartikel als äußerst robust und zeigte keine statistisch signifikanten Veränderungen der Zellviabilität.
Messungen der Zellviabilität auf Grundlage von Adsorptionsmessung, wie beim EZ4U-Test, hatten sich als ungeeignet erwiesen, da die Eigenfarbe der Nanopartikel Graphen und Siliciumdioxid mit dieser Messung interferierte.
Zudem wurde geprüft, ob die bei einem Teil der Zelllinien eingetretene toxische Wirkung der Nanopartikel ab einer Konzentration von 1 mg/ml durch Nekrose oder durch Apoptose zustande kam. Die Polymerasekettenreaktion zeigte mit einer einzigen Ausnahme keine statistisch signifikante Erhöhung der Genexpression für Bax und Caspase-3 und gab somit auch keine Hinweise auf die Einleitung einer Apoptose. Im Western Blot zeigte sich keine statistisch signifikante Erhöhung der Proteinexpression von Bax und Caspase-3. Zudem konnte im Western Blot auch keine aktivierte Caspase-3 nachgewiesen werden. Somit lagen auf Grundlage von Polymerasekettenreaktion und Western Blot keine Hinweise auf das Eintreten einer Apoptose vor. Die toxische Wirkung der Nanopartikel Graphen und Siliciumdioxid, die bei einem Teil der Zelllinien ab einer Konzentration von 1 mg/ml nachgewiesen werden konnte, beruhte demnach auf Nekrose.
PONV ist eine häufige und für Patient*innen belastende Nebenwirkung nach einer Allgemeinanästhesie. Trotz der Vielzahl an Studien zu den zahlreichen antiemetischen Medikamenten gibt es bisher keinen Überblick über die Effizienz und Sicherheit all dieser Medikamente. Im Rahmen des Cochrane-Reviews „Drugs for preventing postoperative nausea and vomiting in adults after general anaesthesia: a network meta-analysis“ wurden RCTs zur Prävention von PONV nach Allgemeinanästhesie bei Erwachsenen gesucht. Zu den primären Endpunkten gehörten Erbrechen 0-24 Stunden, schwere unerwünschte Ereignisse und unerwünschte Ereignisse, zu den sekundären Endpunkten Substanz-spezifische Nebenwirkungen, frühes und spätes postoperatives Erbrechen, Übelkeit und vollständiger Behandlungserfolg. In dieser Dissertation wurden die Vergleiche mit mindestens zehn Studien auf das Vorliegen eines Publikationsbias überprüft. Die Beurteilung des Publikationsbias erfolgte unter Anwendung verschiedener Tests (Funnel Plots, contour-enhanced Funnel Plot, Arcsine Test, Trim-and-Fill-Methode). Bei sieben von den 64 analysierten Vergleichen wurde der Verdacht auf einen Publikationsbias gestellt. Für den primären Endpunkt Erbrechen 0-24 Stunden wurde bei zwei Vergleichen (Droperidol vs. Placebo und Metoclopramid vs. Ondansetron) ein Publikationsbias vermutet, für die sekundären Endpunkte Übelkeit bei drei Vergleichen (Tropisetron vs. Placebo, Dexamethason-Ondansetron vs. Dexamethason, Dexamethason-Ondansetron vs. Ondansetron) und für den vollständigen Behandlungserfolg bei zwei Vergleichen (Droperidol vs. Placebo, Ondansetron vs. Placebo). Die Effektschätzer der restlichen 54 Vergleiche sind hinsichtlich der klinischen Relevanz robust und eine Verzerrung durch einen Publikationsbias wurde nicht vermutet.
Hintergrund
Die Versorgung Schwerverletzter setzt das zügige Erkennen
lebensbedrohlicher Verletzungen und deren Priorisierung voraus. Hierzu verfügt
das Universitätsklinikum Würzburg seit 2018 über ein Doppelschockraumkonzept mit
Ganzkörper-CT, fahrbarer CT-Gantry und einer mobilen Schutzwand, wodurch zwei
Personen nahezu simultan behandelt werden können. Das Ziel der vorliegenden Arbeit
war zum Einen mögliche Spezifika von simultan versorgten Patienten und Patientinnen
zu identifizieren und zum Anderen die Evaluation möglicher Unterschiede in der Qualität
der Versorgung in einem Doppel- und einem Einzelschockraum, insbesondere der Zeit
bis zur CT-Bildgebung und bis zum Beginn operativer Maßnahmen.
Methodik Im Rahmen einer retrospektiven Untersuchung wurden die Patientendaten
aller Schockraumaufnahmen des Universitätsklinikums Würzburg vom 1. Mai 2019 bis
zum 29. April 2020 analysiert. Die Datensätze wurden bei einer simultanen Versorgung
im Schockraum der Doppelschockraumgruppe (Gruppe 1) und bei einer alleinigen
Versorgung der Einzelschockraumgruppe (Gruppe 2) zugeordnet.
Ergebnisse 10,9 % aller Schockraumaufnahmen wurden simultan im Schockraum
versorgt (46 von 423). Personen aus Gruppe 1 verunglückten häufiger bei PKW-Unfällen
(47,8 % vs. 19,6 %; p < 0,05). Keine Unterschiede fanden sich bezüglich Alter,
Geschlecht, ASA, ISS und präklinischer Versorgung. Die Klinik bei der Aufnahme
unterschied sich nicht bezüglich A-, B-, C- und D-Problemen, allerdings litten Personen
aus Gruppe 1 häufiger unter Schmerzen (hier als E-Problem nach ATLS klassifiziert)
(45,7 % vs. 29,2 %; p < 0,05). Die Versorgung im Schockraum (Instrumentierung,
Medikamentenapplikation, Transfusion) ergab keine relevanten Unterschiede.
Insbesondere zeigte sich keine klinisch relevante Verzögerung bei simultan versorgten
Patienten und Patientinnen bis zur CT-Bildgebung oder dem Beginn operativer
Maßnahmen (tCT: 8 vs. 6 min (Gruppe 1 vs. Gruppe 2), p < 0,05; tOP: 99 vs. 90 min
(Gruppe 1 vs. Gruppe 2), p < 0,05). Auch das Outcome in beiden Gruppen war
vergleichbar.
Diskussion Die simultane Versorgung zweier Schwerverletzter stellt hohe
Anforderungen an Personal, Ausstattung und Organisation. Das in dieser Arbeit
untersuchte Doppelschockraumkonzept kann auch bei simultan versorgten Patienten
und Patientinnen eine bestmögliche Versorgung auf individualmedizinischem Niveau
gewährleistet werden.
Background: Phenprocoumon has been used as an oral anticoagulant in patients with thromboembolic disease for more than 40 years. So far its pharmacokinetics have not been analyzed in emergency situations. Methods: Phenprocoumon-treated patients with major bleeding or urgent surgery were included in a prospective, observational registry. Phenprocoumon drug concentrations were analyzed in samples, collected as part of routine care using ultraperformance liquid chromatography tandem mass spectrometry. Moreover, anticoagulant intensity and drug half-life (t1/2) were calculated. Results: 115 patients were included. Phenprocoumon levels declined over time with a half-life of 5.27 and 5.29 days in patients with major bleedings (n = 82) and with urgent surgery (n = 33). Baseline phenprocoumon levels were 2.2 times higher in the bleeding group compared to the surgery group (1.92 vs. 0.87 ng/mL, p < 0.0001). International normalized ratio (INR) values decreased rapidly during the first 24 h. In 27.6% of patients a rebound of INR (recurrent increase > 1.5) was observed which was associated with significantly increased bleeding rates (22% vs. 4.2% in patients with or without INR rebound, p = 0.012). Conclusions: In emergency situations, the long half-life of phenprocoumon may cause INR rebound and associated recurrent bleedings. Optimal management may need to include repeated vitamin K supplementation over days.
Background: Extracorporeal hemadsorption eliminates proinflammatory mediators in critically ill patients with hyperinflammation. The use of a pumpless extracorporeal hemadsorption technique allows its early usage prior to organ failure and the need for an additional medical device. In our animal model, we investigated the feasibility of pumpless extracorporeal hemadsorption over a wide range of mean arterial pressures (MAP). Methods: An arteriovenous shunt between the femoral artery and femoral vein was established in eight pigs. The hemadsorption devices were inserted into the shunt circulation; four pigs received CytoSorb\(^®\) and four Oxiris\(^®\) hemadsorbers. Extracorporeal blood flow was measured in a range between mean arterial pressures of 45–85 mmHg. Mean arterial pressures were preset using intravenous infusions of noradrenaline, urapidil, or increased sedatives. Results: Extracorporeal blood flows remained well above the minimum flows recommended by the manufacturers throughout all MAP steps for both devices. Linear regression resulted in CytoSorb\(^®\) blood flow [mL/min] = 4.226 × MAP [mmHg] − 3.496 (R-square 0.8133) and Oxiris\(^®\) blood flow [mL/min] = 3.267 × MAP [mmHg] + 57.63 (R-square 0.8708), respectively. Conclusion: Arteriovenous pumpless extracorporeal hemadsorption resulted in sufficient blood flows through both the CytoSorb\(^®\) and Oxiris\(^®\) devices over a wide range of mean arterial blood pressures and is likely an intriguing therapeutic option in the early phase of septic shock or hyperinflammatory syndromes.
Alzheimer’s disease (AD), the most common cause of dementia in the elderly, is a neurodegenerative disorder associated with neurovascular dysfunction, cognitive decline, and the accumulation of amyloid β peptide (Aβ) in the brain and tau-related lesions in neurons termed neurofibrillary tangles (NFTs). Aβ deposits and NFT formation are the central pathological hallmarks in AD brains, and the majority of AD cases have been shown to exhibit a complex combination of systemic comorbidities. While AD is the foremost common cause of dementia in the elderly, age-related hearing loss (ARHL) is the most predominant sensory deficit in the elderly. During aging, chronic inflammation and resulting endothelial dysfunction have been described and might be key contributors to AD; we discuss an intriguing possible link between inner ear strial microvascular pathology and blood–brain barrier pathology and present ARHL as a potentially modifiable and treatable risk factor for AD development. We present compelling evidence that ARHL might well be seen as an important risk factor in AD development: progressive hearing impairment, leading to social isolation, and its comorbidities, such as frailty, falls, and late-onset depression, link ARHL with cognitive decline and increased risk of dementia, rendering it tempting to speculate that ARHL might be a potential common molecular and pathological trigger for AD. Additionally, one could speculate that amyloid-beta might damage the blood–labyrinth barrier as it does to the blood–brain barrier, leading to ARHL pathology. Finally, there are options for the treatment of ARHL by targeted neurotrophic factor supplementation to the cochlea to improve cognitive outcomes; they can also prevent AD development and AD-related comorbidity in the future.
Alzheimer’s disease (AD) is considered a chronic and debilitating neurological illness that is increasingly impacting older-age populations. Some proteins, including clusterin (CLU or apolipoprotein J) transporter, can be linked to AD, causing oxidative stress. Therefore, its activity can affect various functions involving complement system inactivation, lipid transport, chaperone activity, neuronal transmission, and cellular survival pathways. This transporter is known to bind to the amyloid beta (Aβ) peptide, which is the major pathogenic factor of AD. On the other hand, this transporter is also active at the blood–brain barrier (BBB), a barrier that prevents harmful substances from entering and exiting the brain. Therefore, in this review, we discuss and emphasize the role of the CLU transporter and CLU-linked molecular mechanisms at the BBB interface in the pathogenesis of AD.
Cyclodextrins (CDs) are cyclic oligosaccharide structures that could be used for theranostic applications in personalized medicine. These compounds have been widely utilized not only for enhancing drug solubility, stability, and bioavailability but also for controlled and targeted delivery of small molecules. These compounds can be complexed with various biomolecules, such as peptides or proteins, via host-guest interactions. CDs are amphiphilic compounds with water-hating holes and water-absorbing surfaces. Architectures of CDs allow the drawing and preparation of CD-based polymers (CDbPs) with optimal pharmacokinetic and pharmacodynamic properties. These polymers can be cloaked with protein corona consisting of adsorbed plasma or extracellular proteins to improve nanoparticle biodistribution and half-life. Besides, CDs have become famous in applications ranging from biomedicine to environmental sciences. In this review, we emphasize ongoing research in biomedical fields using CD-based centered, pendant, and terminated polymers and their interactions with protein corona for theranostic applications. Overall, a perusal of information concerning this novel approach in biomedicine will help to implement this methodology based on host-guest interaction to improve therapeutic and diagnostic strategies.
Chronic stress, even stress of a moderate intensity related to daily life, is widely acknowledged to be a predisposing or precipitating factor in neuropsychiatric diseases. There is a clear relationship between disturbances induced by stressful stimuli, especially long-lasting stimuli, and cognitive deficits in rodent models of affective disorders. Regular physical activity has a positive effect on the central nervous system (CNS) functions, contributes to an improvement in mood and of cognitive abilities (including memory and learning), and is correlated with an increase in the expression of the neurotrophic factors and markers of synaptic plasticity as well as a reduction in the inflammatory factors. Studies published so far show that the energy challenge caused by physical exercise can affect the CNS by improving cellular bioenergetics, stimulating the processes responsible for the removal of damaged organelles and molecules, and attenuating inflammation processes. Regular physical activity brings another important benefit: increased stress robustness. The evidence from animal studies is that a sedentary lifestyle is associated with stress vulnerability, whereas a physically active lifestyle is associated with stress resilience. Here, we have performed a comprehensive PubMed Search Strategy for accomplishing an exhaustive literature review. In this review, we discuss the findings from experimental studies on the molecular and neurobiological mechanisms underlying the impact of exercise on brain resilience. A thorough understanding of the mechanisms underlying the neuroprotective potential of preconditioning exercise and of the role of exercise in stress resilience, among other things, may open further options for prevention and therapy in the treatment of CNS diseases.
In früheren Studien wurde gezeigt, dass durch eine mit FCA-induzierte Pfotenentzündung die Permeabilität für hydrophile Analgetika der kleinen Nerven am Entzündungsort zunimmt. In der vorliegenden Arbeit wurden spezifische Veränderungen von Barriereproteinen des Perineuriums und der Schwannschen Zellen und ihren Regulatoren nach intraplantarer Injektion von FCA lokal in die Hinterpfote und proximal am N. ischiadicus untersucht. Aus früheren Studien ist bekannt, dass vor allem Claudin-1 das Perineurium abdichtet. Daher konzentrierte sich die Arbeit auf Claudin-1 und einen möglichen Einfluss von Claudin-19 aus Schwannschen Zellen. Alle Untersuchungen erfolgten an Wister-Ratten.
Zwei Stunden bis 96 Stunden nach der FCA-Injektion in die Hinterpfote waren die Expression sowie die Immunreaktivität von Claudin-1 und die Expression von Claudin-19 im ipsilateralen proximalen Ischiasnerv unverändert. Zudem wurde keine Penetration des Farbstoffes EBA in das Endoneurium und in den Ischiasnerv nach ex vivo Applikation nachgewiesen, was auf eine gute Abdichtung des Perineuriums hinweist. In der entzündeten Pfote selbst allerdings nahm die Expression von Claudin-1 und Claudin-19 ab. Parallel dazu kam es zu einer starken Abnahme des Co-Transkriptionsfaktors β-Catenin in der Pfote, aber nicht im Nerven. β-Catenin steuert die Expression von Claudin-1. Die Behandlung mit einem GSK3 β-Inhibitor bremste die Herunterregulation von Claudin-1 24 Stunden nach der intraplantaren Injektion von FCA ins Hinterpfotengewebe und führte zu einem Wiederanstieg der Konzentration.
Daher kann abschließend festgehalten werden, dass eine periphere Entzündung zwar wie erwartet lokal die Barriere öffnet, es aber proximal nicht zu einer Barrierestörung kommt. Dies ist bei der Blut-Hirn-Schranke anders. Diese wird vermutlich über lösliche Faktoren bei Entzündung oder bei Nervenschäden, bei denen sich auch die Barriere im Spinalganglion verändert, durchlässiger.
Background: Prehabilitation is gaining increasing interest and shows promising effects on short- and long-term outcomes among patients undergoing major surgery. The effect of multimodal, interdisciplinary prehabilitation has not yet been studied in patients with severe heart failure scheduled for the implantation of a left-ventricular assist device (LVAD). Methods: This randomized controlled multi-center study evaluates the effect of preoperative combined optimization of nutritional and functional status. Patients in the intervention group are prescribed daily in-bed cycling and oral nutrition supplements (ONS) from study inclusion until the day before LVAD-implantation. Patients in the control group receive standard of care treatment. The primary outcomes for the pilot study that involves 48 patients are safety (occurrence of adverse events), efficacy (group separation regarding the intake of macronutrients), feasibility of the trial protocol (compliance (percentage of received interventions) and confirmation of recruitment rates. Secondary outcomes include longitudinal measurements of muscle mass, muscle strength, physical function and quality of life, next to traditional clinical outcomes (30-day mortality, hospital and ICU length of stay, duration of mechanical ventilation and number of complications and infections). If the pilot study is successful, a larger confirmatory, international multicenter study is warranted.
Early treatment with glucocorticoids could help reduce both cytotoxic and vasogenic edema, leading to improved clinical outcome after stroke. In our previous study, isosteviol sodium (STVNA) demonstrated neuroprotective effects in an in vitro stroke model, which utilizes oxygen-glucose deprivation (OGD). Herein, we tested the hypothesis that STVNA can activate glucocorticoid receptor (GR) transcriptional activity in brain microvascular endothelial cells (BMECs) as previously published for T cells. STVNA exhibited no effects on transcriptional activation of the glucocorticoid receptor, contrary to previous reports in Jurkat cells. However, similar to dexamethasone, STVNA inhibited inflammatory marker IL-6 as well as granulocyte-macrophage colony-stimulating factor (GM-CSF) secretion. Based on these results, STVNA proves to be beneficial as a possible prevention and treatment modality for brain ischemia-reperfusion injury-induced blood–brain barrier (BBB) dysfunction.
Intracranial hemorrhage results in devastating forms of cerebral damage. Frequently, these results also present with cardiac dysfunction ranging from ECG changes to Takotsubo syndrome (TTS). This suggests that intracranial bleeding due to subarachnoid hemorrhage (SAH) disrupts the neuro–cardiac axis leading to neurogenic stress cardiomyopathy (NSC) of different degrees. Following this notion, SAH and secondary TTS could be directly linked, thus contributing to poor outcomes. We set out to test if blood circulation is the driver of the brain–heart axis by investigating serum samples of TTS patients. We present a novel in vitro model combining SAH and secondary TTS to mimic the effects of blood or serum, respectively, on blood–brain barrier (BBB) integrity using in vitro monolayers of an established murine model. We consistently demonstrated decreased monolayer integrity and confirmed reduced Claudin-5 and Occludin levels by RT-qPCR and Western blot and morphological reorganization of actin filaments in endothelial cells. Both tight junction proteins show a time-dependent reduction. Our findings highlight a faster and more prominent disintegration of BBB in the presence of TTS and support the importance of the bloodstream as a causal link between intracerebral bleeding and cardiac dysfunction. This may represent potential targets for future therapeutic inventions in SAH and TTS.
The biomedical consequences of allogeneic blood transfusions and the possible pathomechanisms of transfusion-related morbidity and mortality are still not entirely understood. In retrospective studies, allogeneic transfusion was associated with increased rates of cancer recurrence, metastasis and death in patients with colorectal cancer. However, correlation does not imply causation. The purpose of this study was to elucidate this empirical observation further in order to address insecurity among patients and clinicians. We focused on the in vitro effect of microparticles derived from red blood cell units (RMPs). We incubated different colon carcinoma cells with RMPs and analyzed their effects on growth, invasion, migration and tumor marker expression. Furthermore, effects on Wnt, Akt and ERK signaling were explored. Our results show RMPs do not seem to affect functional and phenotypic characteristics of different colon carcinoma cells and did not induce or inhibit Wnt, Akt or ERK signaling, albeit in cell culture models lacking tumor microenvironment. Allogeneic blood transfusions are associated with poor prognosis, but RMPs do not seem to convey tumor-enhancing effects. Most likely, the circumstances that necessitate the transfusion, such as preoperative anemia, tumor stage, perioperative blood loss and extension of surgery, take center stage.
Diabetes mellitus is a common disease affecting more than 537 million adults worldwide. The microvascular complications that occur during the course of the disease are widespread and affect a variety of organ systems in the body. Diabetic retinopathy is one of the most common long-term complications, which include, amongst others, endothelial dysfunction, and thus, alterations in the blood-retinal barrier (BRB). This particularly restrictive physiological barrier is important for maintaining the neuroretina as a privileged site in the body by controlling the inflow and outflow of fluid, nutrients, metabolic end products, ions, and proteins. In addition, people with diabetic retinopathy (DR) have been shown to be at increased risk for systemic vascular complications, including subclinical and clinical stroke, coronary heart disease, heart failure, and nephropathy. DR is, therefore, considered an independent predictor of heart failure. In the present review, the effects of diabetes on the retina, heart, and kidneys are described. In addition, a putative common microRNA signature in diabetic retinopathy, nephropathy, and heart failure is discussed, which may be used in the future as a biomarker to better monitor disease progression. Finally, the use of miRNA, targeted neurotrophin delivery, and nanoparticles as novel therapeutic strategies is highlighted.
Despite the availability of numerous therapeutic substances that could potentially target CNS disorders, an inability of these agents to cross the restrictive blood–brain barrier (BBB) limits their clinical utility. Novel strategies to overcome the BBB are therefore needed to improve drug delivery. We report, for the first time, how Tumor Treating Fields (TTFields), approved for glioblastoma (GBM), affect the BBB’s integrity and permeability. Here, we treated murine microvascular cerebellar endothelial cells (cerebEND) with 100–300 kHz TTFields for up to 72 h and analyzed the expression of barrier proteins by immunofluorescence staining and Western blot. In vivo, compounds normally unable to cross the BBB were traced in healthy rat brain following TTFields administration at 100 kHz. The effects were analyzed via MRI and immunohistochemical staining of tight-junction proteins. Furthermore, GBM tumor-bearing rats were treated with paclitaxel (PTX), a chemotherapeutic normally restricted by the BBB combined with TTFields at 100 kHz. The tumor volume was reduced with TTFields plus PTX, relative to either treatment alone. In vitro, we demonstrate that TTFields transiently disrupted BBB function at 100 kHz through a Rho kinase-mediated tight junction claudin-5 phosphorylation pathway. Altogether, if translated into clinical use, TTFields could represent a novel CNS drug delivery strategy.
Mit dem Auftreten des SARS-CoV-2 Virus im Jahr 2020 war der Informationsgewinn für vulnerable Patientengruppen essentiell. Ziel dieser Arbeit war es maternale Charakteristika und das klinische Bild SARS-CoV-2 positiver Frauen mit Notwendigkeit einer intensivmedizinischen Behandlung während der Schwangerschaft und postpartal darzustellen, und diese Kohorte mit den SARS-CoV-2 positiven Schwangeren ohne intensivmedizinischen Handlungsbedarf zu vergleichen. Die Daten stammten aus dem deutschen CRONOS-Register, einem prospektiven, multizentrischen Register für SARS-CoV-2 positive schwangere Frauen. Eingeschlossen wurden alle schwangeren und postpartalen Frauen, die während ihrer SARS-CoV-2 Infektion auf eine ITS aufgenommen wurden. Diese wurden hinsichtlich maternaler Charakteristika, Krankheitsverlauf, sowie Outcomes verglichen.
In 101 von 2650 Fällen (4%) der Patientinnen des CRONOS-Registers, kam es zu einer Aufnahme auf die ITS. Als invasivste Form der COVID-19 Behandlung war bei 6 Patientinnen nur eine Überwachung notwendig, 30 Patientinnen benötigten eine Sauerstoffinsufflation, 22 wurden nicht-invasiv beatmet, 28 erhielten eine invasive Beatmung und bei 15 Frauen wurde die Behandlung zur ECMO-Therapie eskaliert. Es wurden keine klinisch signifikanten Unterschiede zwischen Patientinnen gefunden, die unterschiedliche Behandlungsformen benötigten. Die Gruppe der ITS und Non-ITS Patientinnen unterschied sich statistisch signifikant beim Einfluss von Alter, BMI bei Einschluss und der Herkunft. Die Prävalenz der Frühgeburtlichkeit war unter den invasiv behandelten Patientinnen signifikant höher und auch im Vergleich der ITS mit den Non-ITS Patientinnen zeigte sich ein signifikanter Unterschied. Vier Frauen verstarben an COVID-19 und sechs Feten der ITS-Gruppe waren Totgeburten.
Diese Kohorte zeigt, dass schwere COVID-19 Erkrankung bei schwangeren Frauen und Wöchnerinnen selten sind. Die Frühgeburtenrate ist hoch und COVID-19 mit Notwendigkeit einer Atemunterstützung erhöht das Risiko für ein schlechtes maternales und neonatales Outcome. Unter anderem ein höheres Alter und BMI sind mit einem höheren Risiko für eine ITS-Aufnahme verbunden.
Hypersensitivity to mechanical stimuli is a cardinal symptom of neuropathic and inflammatory pain. A reduction in spinal inhibition is generally considered a causal factor in the development of mechanical hypersensitivity after injury. However, the extent to which presynaptic inhibition contributes to altered spinal inhibition is less well established. Here, we used conditional deletion of GABA\(_A\) in NaV1.8-positive sensory neurons (Scn10a\(^{Cre}\);Gabrb3\(^{fl/fl}\)) to manipulate selectively presynaptic GABAergic inhibition. Behavioral testing showed that the development of inflammatory punctate allodynia was mitigated in mice lacking pre-synaptic GABA\(_A\). Dorsal horn cellular circuits were visualized in single slices using stimulus-tractable dual-labelling of c-fos mRNA for punctate and the cognate c-Fos protein for dynamic mechanical stimulation. This revealed a substantial reduction in the number of cells activated by punctate stimulation in mice lacking presynaptic GABA\(_A\) and an approximate 50% overlap of the punctate with the dynamic circuit, the relative percentage of which did not change following inflammation. The reduction in dorsal horn cells activated by punctate stimuli was equally prevalent in parvalbumin- and calretinin-positive cells and across all laminae I–V, indicating a generalized reduction in spinal input. In peripheral DRG neurons, inflammation following complete Freund’s adjuvant (CFA) led to an increase in axonal excitability responses to GABA, suggesting that presynaptic GABA effects in NaV1.8\(^+\) afferents switch from inhibition to excitation after CFA. In the days after inflammation, presynaptic GABA\(_A\) in NaV1.8\(^+\) nociceptors constitutes an “open gate” pathway allowing mechanoreceptors responding to punctate mechanical stimulation access to nociceptive dorsal horn circuits.
Background: The close monitoring of blood pressure during a caesarean section performed under central neuraxial anaesthesia should be the standard of safe anaesthesia. As classical oscillometric and invasive blood pressure measuring have intrinsic disadvantages, we investigated a novel, non-invasive technique for continuous blood pressure measuring. Methods: In this monocentric, retrospective data analysis, the reliability of continuous non-invasive blood pressure measuring using ClearSight\(^®\) (Edwards Lifesciences Corporation) is validated in 31 women undergoing central neuraxial anaesthesia for caesarean section. In addition, patients and professionals evaluated ClearSight\(^®\) through questioning. Results: 139 measurements from 11 patients were included in the final analysis. Employing Bland–Altman analyses, we identified a bias of −10.8 mmHg for systolic, of −0.45 mmHg for diastolic and of +0.68 mmHg for mean arterial blood pressure measurements. Pooling all paired measurements resulted in a Pearson correlation coefficient of 0.7 for systolic, of 0.67 for diastolic and of 0.75 for mean arterial blood pressure. Compensating the interindividual differences in linear regressions of the paired measurements provided improved correlation coefficients of 0.73 for systolic, of 0.9 for diastolic and of 0.89 for mean arterial blood pressure measurements. Discussion: Diastolic and mean arterial blood pressure are within an acceptable range of deviation from the reference method, according to the Association for the Advancement of Medical Instrumentation (AAMI) in the patient collective under study. Both patients and professionals prefer ClearSight\(^®\) to oscillometric blood pressure measurement in regard of comfort and handling.
Systematische Übersichtsarbeiten und Metaanalysen zielen darauf ab, die gesamte verfügbare Evidenz zu einer Fragestellung zu erfassen. Die aktuelle Cochrane Netzwerk-Metaanalyse (NMA) verglich Antiemetika zur Prävention von postoperativer Übelkeit und Erbrechen. Die vorliegende Untersuchung ergänzte den Studienpool retrospektiv um die ursprünglich exkludierten Kongressabstracts und analysierte deren Mehrwert hinsichtlich Berichterstattungsqualität und Auswirkung auf die NMA.
Kongressabstracts wurden bezüglich Studiencharakteristika, Biasrisiko und Berichter-stattung analysiert. Die Auswirkung des Einschlusses von Abstracts wurde mittels Sensitivitäts-Netzwerk-Metaanalysen analysiert. Anhand CONSORT (Consolidated Standards of Reporting Trials) für Abstracts wurde ein Beispielabstract verfasst.
Von 90 identifizierten Abstracts waren 14 % (13/90) inkludierbar und bei 86 % (77/90) ist die Klassifikation im Hinblick auf Ein- oder Ausschlussfähigkeit aufgrund insuffizienter Berichterstattung der engen Einschlusskriterien der NMA ausstehend. Dabei fehlten bei 71 % (55/77) der Abstracts Informationen zur Standardisierung der Anästhesie, bei 56 % (43/77) zum Alter der Teilnehmer und bei 46 % (36/77) zu den Endpunktdetails. In 73 % (66/90) der Abstracts fehlten suffiziente Informationen zu 15 von 25 Datenextraktions-merkmalen. Die berichteten Studiencharakteristika der Abstracts waren vergleichbar mit den inkludierten Studien der NMA. 62 % (56/90) der Abstractstudien hatten aufgrund mangelhafter Berichterstattung ein hohes Gesamt-Biasrisiko. Die Adhärenz zu CONSORT für Abstracts betrug im Median 24 % (IQR: 18 % - 29 %). Eine Verbesserung der Berichterstattung nach der Publikation von CONSORT für Abstracts war nicht zu beobachten. Nur sechs der inkludierbaren Abstracts berichteten relevante Endpunktdaten suffizient und konnten in Sensitivitäts-NMAs inkludiert werden. Deren Inklusion führte weder zu einer relevanten Änderung der Netzwerkstruktur oder Effektschätzungen noch des Rankings der Wirksamkeit der Antiemetika bzw. der klinischen Konklusion. Eine Empfehlung für zukünftige Abstracts mit Vorlage entsprechend der Kriterien der Fachgesellschaften sowie CONSORT für Abstracts konnte entwickelt werden.
Die Mehrheit der identifizierten Abstracts waren insuffizient berichtet und konnten daher nicht eingeschlossen werden. Kongressorganisatoren sollten ihre Autoren zur Umsetzung von CONSORT für Abstracts verpflichten. Die ressourcenintensive Einbeziehung von Abstracts brachte keinen relevanten Evidenzgewinn. Somit kann die Inklusion von Kongressabstracts in systematische Übersichtsarbeiten mit engen Inklusionskriterien und solider Volltext-Evidenzbasis nicht empfohlen werden.
Advances in breast cancer management and extracellular vesicle research, a bibliometric analysis

Extracellular vesicles transport variable content and have crucial functions in cell–cell communication. The role of extracellular vesicles in cancer is a current hot topic, and no bibliometric study has ever analyzed research production regarding their role in breast cancer and indicated the trends in the field. In this way, we aimed to investigate the trends in breast cancer management involved with extracellular vesicle research. Articles were retrieved from Scopus, including all the documents published concerning breast cancer and extracellular vesicles. We analyzed authors, journals, citations, affiliations, and keywords, besides other bibliometric analyses, using R Studio version 3.6.2. and VOSviewer version 1.6.0. A total of 1151 articles were retrieved, and as the main result, our analysis revealed trending topics on biomarkers of liquid biopsy, drug delivery, chemotherapy, autophagy, and microRNA. Additionally, research related to extracellular vesicles in breast cancer has been focused on diagnosis, treatment, and mechanisms of action of breast tumor-derived vesicles. Future studies are expected to explore the role of extracellular vesicles on autophagy and microRNA, besides investigating the application of extracellular vesicles from liquid biopsies for biomarkers and drug delivery, enabling the development and validation of therapeutic strategies for specific cancers.
Anaemia is a risk factor for several adverse postoperative outcomes. Detailed data about the prevalence of anaemia are not available over a long time-period in Germany. In this retrospective, observational, multicentre study, patients undergoing surgery in March in 2007, 2012, 2015, 2017 and 2019 were studied. The primary objective was the prevalence of anaemia at hospital admission. The secondary objectives were the association between anaemia and the number of units of red blood cells transfused, length of hospital stay and in-hospital mortality. A total of 23,836 patients were included from eight centres. The prevalence of pre-operative anaemia in patients aged ≥ 18 years decreased slightly from 37% in 2007 to 32.5% in 2019 (p = 0.01) and increased in patients aged ≤ 18 years from 18.8% in 2007 to 26.4% in 2019 (p > 0.001). The total amount of blood administered per 1000 patients decreased from 671.2 units in 2007 to 289.0 units in 2019. Transfusion rates in anaemic patients declined from 33.8% in 2007 to 19.1% in 2019 (p < 0.001) and in non-anaemic patients from 8.4% in 2007 to 3.4% in 2019 (p < 0.001). Overall, the mortality rate remained constant over the years: 2.9% in 2007, 2.1% in 2012, 2.5% in 2015, 1.9% in 2017 and 2.5% in 2019. In the presence of anaemia, mortality was significantly increased compared with patients without anaemia (OR 5.27 (95%CI 4.13–6.77); p < 0.001). Red blood cell transfusion was associated with an increased risk of mortality (OR 14.98 (95%CI 11.83–19.03); p < 0.001). Using multivariable linear regression analysis with fixed effects, we found that pre-operative anaemia (OR 2.08 (95%CI 1.42–3.05); p < 0.001) and red blood cell transfusion (OR 4.29 (95%CI 3.09–5.94); p < 0.001) were predictors of mortality but not length of stay (0.99 (95%CI 0.98–1.00) days; p = 0.12) and analysed years (2007 vs. 2019: OR 1.49 (95%CI 0.86–2.69); p = 0.07). Pre-operative anaemia affects more than 30% of surgical patients in Germany and multidisciplinary action is urgently required to reduce adverse outcomes.
Evidence synthesis findings depend on the assumption that the included studies follow good clinical practice and results are not fabricated or false. Studies which are problematic due to scientific misconduct, poor research practice, or honest error may distort evidence synthesis findings. Authors of evidence synthesis need transparent mechanisms to identify and manage problematic studies to avoid misleading findings. As evidence synthesis authors of the Cochrane COVID-19 review on ivermectin, we identified many problematic studies in terms of research integrity and regulatory compliance. Through iterative discussion, we developed a research integrity assessment (RIA) tool for randomized controlled trials for the update of this Cochrane review. In this paper, we explain the rationale and application of the RIA tool in this case study. RIA assesses six study criteria: study retraction, prospective trial registration, adequate ethics approval, author group, plausibility of methods (e.g., randomization), and plausibility of study results. RIA was used in the Cochrane review as part of the eligibility check during screening of potentially eligible studies. Problematic studies were excluded and studies with open questions were held in awaiting classification until clarified. RIA decisions were made independently by two authors and reported transparently. Using the RIA tool resulted in the exclusion of >40% of studies in the first update of the review. RIA is a complementary tool prior to assessing “Risk of Bias” aiming to establish the integrity and authenticity of studies. RIA provides a platform for urgent development of a standard approach to identifying and managing problematic studies.
In 1747, an important milestone in the history of clinical research was set, as the Scottish surgeon James Lind conducted the first randomized controlled trial. Lind was interested in scurvy, a severe vitamin C deficiency which caused the death of thousands of British seamen. He found that a dietary intervention with oranges and lemons, which are rich in vitamin C by nature, was effective to recover from scurvy. Because of its antioxidative properties and involvement in many biochemical processes, the essential micronutrient vitamin C plays a key role in the human biology. Moreover, the use of vitamin C in critical illness—a condition also resulting in death of thousands in the 21st century—has gained increasing interest, as it may restore vascular responsiveness to vasoactive agents, ameliorate microcirculatory blood flow, preserve endothelial barriers, augment bacterial defense, and prevent apoptosis. Because of its redox potential and powerful antioxidant capacity, vitamin C represents an inexpensive and safe antioxidant, with the potential to modify the inflammatory cascade and improve clinical outcomes of critically ill patients. This narrative review aims to update and provide an overview on the role of vitamin C in the human biology and in critically ill patients, and to summarize current evidence on the use of vitamin C in diverse populations of critically ill patients, in specific focusing on patients with sepsis and coronavirus disease 2019.
Inflammation and oxidative stress represent physiological response mechanisms to different types of stimuli and injury during critical illness. Its proper regulation is fundamental to cellular and organismal survival and are paramount to outcomes and recovery from critical illness. A proper maintenance of the delicate balance between inflammation, oxidative stress, and immune response is crucial for resolution from critical illness with important implications for patient outcome. The extent of inflammation and oxidative stress under normal conditions is limited by the antioxidant defense system of the human body, whereas the antioxidant capacity is commonly significantly compromised, and serum levels of micronutrients and vitamins significantly depleted in patients who are critically ill. Hence, the provision of antioxidants and anti-inflammatory nutrients may help to reduce the extent of oxidative stress and therefore improve clinical outcomes in patients who are critically ill. As existing evidence of the beneficial effects of antioxidant supplementation in patients who are critically ill is still unclear, actual findings about the most promising anti-inflammatory and antioxidative candidates selenium, vitamin C, zinc, and vitamin D will be discussed in this narrative review. The existing evidence provided so far demonstrates that several factors need to be considered to determine the efficacy of an antioxidant supplementation strategy in patients who are critically ill and indicates the need for adequately designed multicenter prospective randomized control trials to evaluate the clinical significance of different types and doses of micronutrients and vitamins in selected groups of patients with different types of critical illness.
Both nerve injury and complex regional pain syndrome (CRPS) can result in chronic pain. In traumatic neuropathy, the blood nerve barrier (BNB) shielding the nerve is impaired—partly due to dysregulated microRNAs (miRNAs). Upregulation of microRNA-21-5p (miR-21) has previously been documented in neuropathic pain, predominantly due to its proinflammatory features. However, little is known about other functions. Here, we characterized miR-21 in neuropathic pain and its impact on the BNB in a human-murine back translational approach. MiR-21 expression was elevated in plasma of patients with CRPS as well as in nerves of mice after transient and persistent nerve injury. Mice presented with BNB leakage, as well as loss of claudin-1 in both injured and spared nerves. Moreover, the putative miR-21 target RECK was decreased and downstream Mmp9 upregulated, as was Tgfb. In vitro experiments in human epithelial cells confirmed a downregulation of CLDN1 by miR-21 mimics via inhibition of the RECK/MMP9 pathway but not TGFB. Perineurial miR-21 mimic application in mice elicited mechanical hypersensitivity, while local inhibition of miR-21 after nerve injury reversed it. In summary, the data support a novel role for miR-21, independent of prior inflammation, in elicitation of pain and impairment of the BNB via RECK/MMP9.
Background
Complex regional pain syndrome (CRPS) is an orphan disease occurring as a complication after trauma. Due to its acute onset and the typical clinical presentation of the inflammatory and autonomous signs, it is an eye-catching chronic pain disease affecting also young and working people. In social media and the internet, high pain severity and the unfavourable prognosis are often empathized.
Methods
Here, we compared epidemiological, pain and lifestyle factors of 223 CPRS patients from the “ncRNAPain” cohort with 255 patients with chronic musculoskeletal pain (MSK). MSK patients were recruited at the beginning of a multimodal pain therapy programme. We searched for factors predicting pain intensity.
Results
Both chronic pain diseases affected women in middle age. Patients with MSK were more obese, drank more alcohol, and were less educated (Pearson chi-square Test or Mann–Whitney/U-Test). Both groups smoked more than healthy people in the OECD (Organization for Economic Cooperation and Development). Mann–Whitney/U-Test confirmed that CRPS patients did not have more severe pain and did not suffer more from pain-related disability than patients with MSK. CRPS patients also had less psychiatric comorbidities. Multiple linear regression analysis revealed that group assignment, depressive characteristics, body mass index, average alcohol consumption and smoking predicted higher pain ratings, while disease duration, anxiety symptoms or gender had no influence on pain intensity.
Conclusion
In summary, our study supports a more optimistic view on pain in CRPS patients in comparison to MSK and identifies lifestyle factors that might contribute to the pathophysiology like smoking and drinking. Important next steps are the identification of CRPS patients at risk for chronification or—vice versa—with protective factors for pain resolution.
Significance
This study compares complex regional pain syndrome (CRPS) and chronic musculoskeletal pain and questions previously reported pain, disability and lifestyle factors associated with CRPS.
Pharmacologic cardiac conditioning increases the intrinsic resistance against ischemia and reperfusion (I/R) injury. The cardiac conditioning response is mediated via complex signaling networks. These networks have been an intriguing research field for decades, largely advancing our knowledge on cardiac signaling beyond the conditioning response. The centerpieces of this system are the mitochondria, a dynamic organelle, almost acting as a cell within the cell. Mitochondria comprise a plethora of functions at the crossroads of cell death or survival. These include the maintenance of aerobic ATP production and redox signaling, closely entwined with mitochondrial calcium handling and mitochondrial permeability transition. Moreover, mitochondria host pathways of programmed cell death impact the inflammatory response and contain their own mechanisms of fusion and fission (division). These act as quality control mechanisms in cellular ageing, release of pro-apoptotic factors and mitophagy. Furthermore, recently identified mechanisms of mitochondrial regeneration can increase the capacity for oxidative phosphorylation, decrease oxidative stress and might help to beneficially impact myocardial remodeling, as well as invigorate the heart against subsequent ischemic insults. The current review highlights different pathways and unresolved questions surrounding mitochondria in myocardial I/R injury and pharmacological cardiac conditioning.
The nervous system is shielded by special barriers. Nerve injury results in blood–nerve barrier breakdown with downregulation of certain tight junction proteins accompanying the painful neuropathic phenotype. The dorsal root ganglion (DRG) consists of a neuron-rich region (NRR, somata of somatosensory and nociceptive neurons) and a fibre-rich region (FRR), and their putative epi-/perineurium (EPN). Here, we analysed blood–DRG barrier (BDB) properties in these physiologically distinct regions in Wistar rats after chronic constriction injury (CCI). Cldn5, Cldn12, and Tjp1 (rats) mRNA were downregulated 1 week after traumatic nerve injury. Claudin-1 immunoreactivity (IR) found in the EPN, claudin-19-IR in the FRR, and ZO-1-IR in FRR-EPN were unaltered after CCI. However, laser-assisted, vessel specific qPCR, and IR studies confirmed a significant loss of claudin-5 in the NRR. The NRR was three-times more permeable compared to the FRR for high and low molecular weight markers. NRR permeability was not further increased 1-week after CCI, but significantly more CD68\(^+\) macrophages had migrated into the NRR. In summary, NRR and FRR are different in naïve rats. Short-term traumatic nerve injury leaves the already highly permeable BDB in the NRR unaltered for small and large molecules. Claudin-5 is downregulated in the NRR. This could facilitate macrophage invasion, and thereby neuronal sensitisation and hyperalgesia. Targeting the stabilisation of claudin-5 in microvessels and the BDB barrier could be a future approach for neuropathic pain therapy.
Microvascular endothelial cells are an essential part of many biological barriers, such as the blood–brain barrier (BBB) and the endothelium of the arteries and veins. A reversible opening strategy to increase the permeability of drugs across the BBB could lead to improved therapies due to enhanced drug bioavailability. Vanilloids, such as capsaicin, are known to reversibly open tight junctions of epithelial and endothelial cells. In this study, we used several in vitro assays with the murine endothelial capillary brain cells (line cEND) as a BBB model to characterize the interaction between capsaicin and endothelial tight junctions.
Das Mammakarzinom ist weltweit die häufigste krebsbedingte Todesursache bei Frauen. Fortschritte in der Therapie ermöglichen zwar eine Verlängerung der Lebens- dauer, jedoch kommt es dadurch vermehrt zur Bildung von Metastasen im zentralen Nervensystem (ZNS). Die Diagnostik und Behandlung von ZNS-Metastasen sind be- grenzt und die Lebensqualität sowie Lebensdauer der Betroffenen nimmt bei zerebraler Metastasierung rapide ab. Ziel aktueller Forschungsprojekte ist daher, Biomarker zu identifizieren, die Hinweise auf eine Brustkrebserkrankung oder Metastasierung liefern. So soll eine kostengünstige, risikoarme und minimalinvasive Methode etabliert werden, die zuverlässige Daten über die Prognose und dementsprechende Therapien erbringt. Diese Arbeit hatte daher die Absicht, mithilfe von qPCR Expressionsprofile von miRNAs aus Serumproben von Brustkrebspatientinnen zu erstellen und deren Funktion als prog- nostische Biomarker für eine Metastasierung ins ZNS zu erweisen. Anhand von Metas- tasierung und Rezeptorstatus wurden die Proben in Untergruppen eingeteilt und statis- tisch mit einer gesunden Kontrollgruppe verglichen.
Insgesamt zeigte sich bei 26 miRNAs eine signifikante Dysregulation der Expression bei mindestens einer der Untergruppen. Insbesondere bei ZNS-Metastasen war das Expres- sionsmuster bei miRNA-122-5p, miRNA-296-5p, miRNA-490-3p und miRNA-576-3p sig- nifikant erhöht, während die Expression von miRNA-130a-3p, miRNA-148b-3p und miRNA-326 signifikant reduziert war. Basierend auf den Übereinstimmungen unserer Er- gebnisse mit den Daten bisheriger Forschungsprojekten wiesen vier miRNAs eine po- tenzielle Funktion als Biomarker für Metastasen auf: miRNA-122-5p, miRNA-490-3p und miRNA-130a-3p, miRNA-326. Bei ZNS-Metastasen zeigten besonders miRNA-122-5p und miRNA-490-3p statistisch relevante Veränderungen.
Um den Einfluss von miRNAs auf den gesamten Körper darzustellen, wurde mithilfe ver- schiedener Datenbanken nach entsprechenden Zielgenen und Signalwegen für die 26 identifizierten miRNAs recherchiert. Neben dem Einfluss auf Stoffwechselwege und Er- krankungen, zeigte sich bei acht Targets ein Zusammenhang mit der Entstehung von Krebs.
Ergänzend zur Identifikation von miRNA-Expressionsprofilen wurden Zellkulturversuche mit zerebralen Endothel- (cerebEND) und Brustkrebszellen (4T1) durchgeführt. Verwendet wurden zwei cerebEND- und eine 4T1-Zellreihe von Mäusen, von denen eine ce- rebEND-Kultur zuvor in der Arbeitsgruppe Burek mit einem miRNA-210-Vektor trans- fiziert wurde.
Studien belegen den Einfluss von miRNA-210 auf den mitochondrialen Stoffwechsel, Angiogenese, Reaktionen auf DNA-Schäden, Apoptose und Zellüberleben sowie auf die Proteine BRCA1, PARP1 und E-Cadherin und schreiben ihr damit eine Funktion in der Krebsentstehung und Metastasierung zu.
Zur Bestimmung der Proliferation und Aktivität der transfizierten cerebEND-210-Zellen im Verhältnis zur unbehandelten Kontrolle, wurden BrdU-Proliferationsassays und MTT- Assays mit verschiedenen Zellzahlen durchgeführt. Bei der Untersuchung der Prolifera- tion zeigte sich in beiden Versuchen eine erhöhte Aktivität der cerebEND-210-Zellen, da miRNA-210 vermutlich auch hier das Zellüberleben gesichert hat. Zudem wurde die An- heftung der Brustkrebszellen an den zerebralen Endothelzellen im Adhäsionsversuchs überprüft. Hierbei wurde eine Abnahme der Adhäsion der cerebEND-210-Zellen beo- bachtet. Vermutet wird eine Veränderung des Phänotyps der Rezeptorbindungen der cerebEND-210-Zellen.
Die Ergebnisse der Zellkulturversuche dienen als Grundlage für weitere Experimente.
(1) Background: Health care workers (HCWs) play a key role in increasing anti-COVID vaccination rates. Fear of potential side effects is one of the main reasons for vaccine hesitancy. We investigated which side effects are of concern to HCWs and how these are associated with vaccine hesitancy. (2) Methods: Data were collected in an online survey in February 2021 among HCWs from across Germany with 4500 included participants. Free-text comments on previously experienced vaccination side effects, and fear of short- and long-term side effects of the COVID-19 vaccination were categorized and analyzed. (3) Results: Most feared short-term side effects were vaccination reactions, allergic reactions, and limitations in daily life. Most feared long-term side effects were (auto-) immune reactions, neurological side effects, and currently unknown long-term consequences. Concerns about serious vaccination side effects were associated with vaccination refusal. There was a clear association between refusal of COVID-19 vaccination in one's personal environment and fear of side effects. (4) Conclusions: Transparent information about vaccine side effects is needed, especially for HCW. Especially when the participants' acquaintances advised against vaccination, they were significantly more likely to fear side effects. Thus, further education of HCW is necessary to achieve good information transfer in clusters as well.
Backround: In February 2021, the first formal evidence and consensus-based (S3) guidelines for the inpatient treatment of patients with COVID-19 were published in Germany and have been updated twice during 2021. The aim of the present study is to re-evaluate the dissemination pathways and strategies for ICU staff (first evaluation in December 2020 when previous versions of consensus-based guidelines (S2k) were published) and question selected aspects of guideline adherence of standard care for patients with COVID-19 in the ICU. Methods: We conducted an anonymous online survey among German intensive care staff from 11 October 2021 to 11 November 2021. We distributed the survey via e-mail in intensive care facilities and requested redirection to additional intensive care staff (snowball sampling). Results: There was a difference between the professional groups in the number, selection and qualitative assessment of information sources about COVID-19. Standard operating procedures were most frequently used by all occupational groups and received a high quality rating. Physicians preferred sources for active information search (e.g., medical journals), while nurses predominantly used passive consumable sources (e.g., every-day media). Despite differences in usage behaviour, the sources were rated similarly in terms of the quality of the information on COVID-19. The trusted organizations have not changed over time. The use of guidelines was frequently stated and highly recommended. The majority of the participants reported guideline-compliant treatment. Nevertheless, there were certain variations in the use of medication as well as the criteria chosen for discontinuing non-invasive ventilation (NIV) compared to guideline recommendations. Conclusions: An adequate external source of information for nursing staff is lacking, the usual sources of physicians are only appropriate for the minority of nursing staff. The self-reported use of guidelines is high.
Background: The adequate choice of perioperative antibiotic prophylaxis (PAP) could influence the risk of surgical site infections (SSIs) in general surgery. A new local PAP guideline was implemented in May 2017 and set the first-generation cefazolin (CFZ) instead the second-generation cefuroxime (CXM) as the new standard prophylactic antibiotic. The aim of this study was to compare the risk of SSIs after this implementation in intra-abdominal infections (IAIs) without sepsis. Methods: We performed a single center-quality improvement study at a 1500 bed sized university hospital in Germany analyzing patients after emergency surgery during 2016 to 2019 (n = 985), of which patients receiving CXM or CFZ were selected (n = 587). Propensity score matching was performed to ensure a comparable risk of SSIs in both groups. None-inferiority margin for SSIs was defined as 8% vs. 4%. Results: Two matched cohorts with respectively 196 patients were compared. The rate of SSIs was higher in the CFZ group (7.1% vs. 3.6%, p = 0.117) below the non-inferiority margin. The rate of other postoperative infections was significantly higher in the CFZ group (2.0% vs. 8.7%, p = 0.004). No other differences including postoperative morbidity, mortality or length-of-stay were observed. Conclusion: Perioperative antibiotic prophylaxis might be safely maintained by CFZ even in the treatment of intra-abdominal infections.
In a recent study, we showed in an in vitro murine cerebellar microvascular endothelial cell (cerebEND) model as well as in vivo in rats that Tumor-Treating Fields (TTFields) reversibly open the blood–brain barrier (BBB). This process is facilitated by delocalizing tight junction proteins such as claudin-5 from the membrane to the cytoplasm. In investigating the possibility that the same effects could be observed in human-derived cells, a 3D co-culture model of the BBB was established consisting of primary microvascular brain endothelial cells (HBMVEC) and immortalized pericytes, both of human origin. The TTFields at a frequency of 100 kHz administered for 72 h increased the permeability of our human-derived BBB model. The integrity of the BBB had already recovered 48 h post-TTFields, which is earlier than that observed in cerebEND. The data presented herein validate the previously observed effects of TTFields in murine models. Moreover, due to the fact that human cell-based in vitro models more closely resemble patient-derived entities, our findings are highly relevant for pre-clinical studies.
Simple Summary
Anti-hormonal therapie regimes are well established in oncological treatments in breast cancer. In contrast there is limited knowledge of their effects on metastatic brain metastases in advanced breast cancer and their ability to cross the blood brain-barrier. In this review, we point out the usual antihormonal therapy options in the primary disease, but also in metastatic breast cancer. In addition, we explain the epidemiological facts of brain metastases, as well as the basics of the blood-brain barrier and how this is overcome by metastase. Last but not least, we deal with the known anti-hormonal therapy options and present clinical studies on their intracerebral effect, as well as the known basics of their blood-brain barrier penetration. Not all common anti-hormonal therapeutics are able to penetrate the CNS. It is therefore important for the treating oncologists to use substances that have been proven to cross the BBB, despite the limited data available. Aromataseinhibitors, especially letrozole, probably also tamoxifen, everolimus and CDK4/6 inhibitors, especially abemaciclib, appear to act intracerebrally by overcoming the blood-brain barrier. Nevertheless, further data must be obtained in basic research, but also health care research in relation to patients with brain metastases.
Abstract
The molecular receptor status of breast cancer has implications for prognosis and long-term metastasis. Although metastatic luminal B-like, hormone-receptor-positive, HER2−negative, breast cancer causes brain metastases less frequently than other subtypes, though tumor metastases in the brain are increasingly being detected of this patient group. Despite the many years of tried and tested use of a wide variety of anti-hormonal therapeutic agents, there is insufficient data on their intracerebral effectiveness and their ability to cross the blood-brain barrier. In this review, we therefore summarize the current state of knowledge on anti-hormonal therapy and its intracerebral impact and effects on the blood-brain barrier in breast cancer.
The blood-brain barrier (BBB) is a highly specialized structure that separates the brain from the blood and allows the exchange of molecules between these two compartments through selective channels. The breakdown of the BBB is implicated in the development of severe neurological diseases, especially stroke and traumatic brain injury. Oxygen-glucose deprivation is used to mimic stroke and traumatic brain injury in vitro. Pathways that trigger BBB dysfunction include an imbalance of oxidative stress, excitotoxicity, iron metabolism, cytokine release, cell injury, and cell death. MicroRNAs are small non-coding RNA molecules that regulate gene expression and are emerging as biomarkers for the diagnosis of central nervous system (CNS) injuries. In this review, the regulatory role of potential microRNA biomarkers and related therapeutic targets on the BBB is discussed. A thorough understanding of the potential role of various cellular and linker proteins, among others, in the BBB will open further therapeutic options for the treatment of neurological diseases.
Protocadherins (PCDHs) belong to the cadherin superfamily and represent the largest subgroup of calcium-dependent adhesion molecules. In the genome, most PCDHs are arranged in three clusters, α, β, and γ on chromosome 5q31. PCDHs are highly expressed in the central nervous system (CNS). Several PCDHs have tumor suppressor functions, but their individual role in primary brain tumors has not yet been elucidated. Here, we examined the mRNA expression of PCDHGC3, a member of the PCDHγ cluster, in non-cancerous brain tissue and in gliomas of different World Health Organization (WHO) grades and correlated it with the clinical data of the patients. We generated a PCDHGC3 knockout U343 cell line and examined its growth rate and migration in a wound healing assay. We showed that PCDHGC3 mRNA and protein were significantly overexpressed in glioma tissue compared to a non-cancerous brain specimen. This could be confirmed in glioma cell lines. High PCDHGC3 mRNA expression correlated with longer progression-free survival (PFS) in glioma patients. PCDHGC3 knockout in U343 resulted in a slower growth rate but a significantly faster migration rate in the wound healing assay and decreased the expression of several genes involved in WNT signaling. PCDHGC3 expression should therefore be further investigated as a PFS-marker in gliomas. However, more studies are needed to elucidate the molecular mechanisms underlying the PCDHGC3 effects.
At any moment in time, cells coordinate and balance their calcium ion (Ca\(^{2+}\)) fluxes. The term ‘Ca\(^{2+}\) homeostasis’ suggests that balancing resting Ca2+ levels is a rather static process. However, direct ER Ca\(^{2+}\) imaging shows that resting Ca\(^{2+}\) levels are maintained by surprisingly dynamic Ca\(^{2+}\) fluxes between the ER Ca\(^{2+}\) store, the cytosol, and the extracellular space. The data show that the ER Ca\(^{2+}\) leak, continuously fed by the high-energy consuming SERCA, is a fundamental driver of resting Ca\(^{2+}\) dynamics. Based on simplistic Ca\(^{2+}\) toolkit models, we discuss how the ER Ca\(^{2+}\) leak could contribute to evolutionarily conserved Ca\(^{2+}\) phenomena such as Ca\(^{2+}\) entry, ER Ca\(^{2+}\) release, and Ca\(^{2+}\) oscillations.
Brain metastases are the most severe tumorous spread during breast cancer disease. They are associated with a limited quality of life and a very poor overall survival. A subtype of extracellular vesicles, exosomes, are sequestered by all kinds of cells, including tumor cells, and play a role in cell-cell communication. Exosomes contain, among others, microRNAs (miRs). Exosomes can be taken up by other cells in the body, and their active molecules can affect the cellular process in target cells. Tumor-secreted exosomes can affect the integrity of the blood-brain barrier (BBB) and have an impact on brain metastases forming. Serum samples from healthy donors, breast cancer patients with primary tumors, or with brain, bone, or visceral metastases were used to isolate exosomes and exosomal miRs. Exosomes expressed exosomal markers CD63 and CD9, and their amount did not vary significantly between groups, as shown by Western blot and ELISA. The selected 48 miRs were detected using real-time PCR. Area under the receiver-operating characteristic curve (AUC) was used to evaluate the diagnostic accuracy. We identified two miRs with the potential to serve as prognostic markers for brain metastases. Hsa-miR-576-3p was significantly upregulated, and hsa-miR-130a-3p was significantly downregulated in exosomes from breast cancer patients with cerebral metastases with AUC: 0.705 and 0.699, respectively. Furthermore, correlation of miR levels with tumor markers revealed that hsa-miR-340-5p levels were significantly correlated with the percentage of Ki67-positive tumor cells, while hsa-miR-342-3p levels were inversely correlated with tumor staging. Analysis of the expression levels of miRs in serum exosomes from breast cancer patients has the potential to identify new, non-invasive, blood-borne prognostic molecular markers to predict the potential for brain metastasis in breast cancer. Additional functional analyzes and careful validation of the identified markers are required before their potential future diagnostic use.
Ziel dieser Arbeit war der Nachweis eines neuroprotektiven Effektes von STVNA auf cerebEND Zellen der Maus in einem in vitro Modell des Schlaganfalls. Mit dem Verfahren zur Herstellung von STVNA konnte ein reines und im Vergleich zu Isosteviol in Wasser gut lösliches Produkt hergestellt werden, das die Anforderungen an eine Versuchssubstanz in einem in vitro Modell voll erfüllen konnte. Als in vitro Modell wurde das bereits bewährte Verfahren der OGD gewähl. CerebEND Zellen der Maus wurden für 4 h OGD ausgesetzt und anschließend für 4 h und 24 h mit 0, 1, 5, 10 und 20 mg/l STVNA behandelt. Direkt, 4 h und 24 h nach 4 h OGD wurden die jeweiligen Zellen geerntet und mittels Western Blot und qRT-PCR ausgewertet. Es wurden eine erhöhte Expression der Tight-Junction-Proteine Claudin-5 und Occludin, sowie ein stabilisierendes Expressionsverhalten der Transmembranproteine Integrin a 1 und Integrin a v nach Behandlung mit STVNA nachgewiesen. Ebenso wurde eine verminderte Expression des Glukosetransporters GLUT 1 beobachtet. Eine Volumenreduktion der cerebEND Zellen durch STVNA, während 4h OGD und gleichzeitiger Behandlung mit STVNA konnte ebenfalls festgestellt werden. Die Ergebnisse dieser Arbeit stützen die Thesen und Ergebnisse der aktuellen Literatur, dass STVNA neuroprotektive Eigenschaften hat.
Background
Parenteral lipid emulsions in critical care are traditionally based on soybean oil (SO) and rich in pro-inflammatory omega-6 fatty acids (FAs). Parenteral nutrition (PN) strategies with the aim of reducing omega-6 FAs may potentially decrease the morbidity and mortality in critically ill patients.
Methods
A systematic search of MEDLINE, EMBASE, CINAHL and CENTRAL was conducted to identify all randomized controlled trials in critically ill patients published from inception to June 2021, which investigated clinical omega-6 sparing effects. Two independent reviewers extracted bias risk, treatment details, patient characteristics and clinical outcomes. Random effect meta-analysis was performed.
Results
1054 studies were identified in our electronic search, 136 trials were assessed for eligibility and 26 trials with 1733 critically ill patients were included. The median methodologic score was 9 out of 14 points (95% confidence interval [CI] 7, 10). Omega-6 FA sparing PN in comparison with traditional lipid emulsions did not decrease overall mortality (20 studies; risk ratio [RR] 0.91; 95% CI 0.76, 1.10; p = 0.34) but hospital length of stay was substantially reduced (6 studies; weighted mean difference [WMD] − 6.88; 95% CI − 11.27, − 2.49; p = 0.002). Among the different lipid emulsions, fish oil (FO) containing PN reduced the length of intensive care (8 studies; WMD − 3.53; 95% CI − 6.16, − 0.90; p = 0.009) and rate of infectious complications (4 studies; RR 0.65; 95% CI 0.44, 0.95; p = 0.03). When FO was administered as a stand-alone medication outside PN, potential mortality benefits were observed compared to standard care.
Conclusion
Overall, these findings highlight distinctive omega-6 sparing effects attributed to PN. Among the different lipid emulsions, FO in combination with PN or as a stand-alone treatment may have the greatest clinical impact.
Background
The clinical significance of vitamin D administration in critically ill patients remains inconclusive. The purpose of this systematic review with meta-analysis was to investigate the effect of vitamin D and its metabolites on major clinical outcomes in critically ill patients, including a subgroup analysis based on vitamin D status and route of vitamin D administration.
Methods
Major databases were searched through February 9, 2022. Randomized controlled trials of adult critically ill patients with an intervention group receiving vitamin D or its metabolites were included. Random-effect meta-analyses were performed to estimate the pooled risk ratio (dichotomized outcomes) or mean difference (continuous outcomes). Risk of bias assessment included the Cochrane tool for assessing risk of bias in randomized trials.
Results
Sixteen randomized clinical trials with 2449 patients were included. Vitamin D administration was associated with lower overall mortality (16 studies: risk ratio 0.78, 95% confidence interval 0.62–0.97, p = 0.03; I2 = 30%), reduced intensive care unit length of stay (12 studies: mean difference − 3.13 days, 95% CI − 5.36 to − 0.89, n = 1250, p = 0.006; I2 = 70%), and shorter duration of mechanical ventilation (9 studies: mean difference − 5.07 days, 95% CI − 7.42 to − 2.73, n = 572, p < 0.0001; I2 = 54%). Parenteral administration was associated with a greater effect on overall mortality than enteral administration (test of subgroup differences, p = 0.04), whereas studies of parenteral subgroups had lower quality. There were no subgroup differences based on baseline vitamin D levels.
Conclusions
Vitamin D supplementation in critically ill patients may reduce mortality. Parenteral administration might be associated with a greater impact on mortality. Heterogeneity and assessed certainty among the studies limits the generalizability of the results.
The trauma center of the University Hospital Wuerzburg has developed an advanced trauma pathway based on a dual-room trauma suite with an integrated movable sliding gantry CT-system. This enables simultaneous CT-diagnostics and treatment of two trauma patients. The focus of this study was to investigate the quality of the concept based on defined outcome criteria in this specific setting (time from arrival to initiation of CT scan: tCT; time from arrival to initiation of emergency surgery: tES). We analyzed all trauma patients admitted to the hospital’s trauma suite from 1st May 2019 through 29th April 2020. Two subgroups were defined: trauma patients, who were treated without a second trauma patient present (group 1) and patients, who were treated simultaneously with another trauma patient (group 2). Simultaneous treatment was defined as parallel arrival within a period of 20 min. Of 423 included trauma patients, 46 patients (10.9%) were treated simultaneously. Car accidents were the predominant trauma mechanism in this group (19.6% vs. 47.8%, p < 0.05). Prehospital life-saving procedures were performed with comparable frequency in both groups (intubation 43.5% vs. 39%, p = 0.572); pleural drainage 3.2% vs. 2.2%, p = 0.708; cardiopulmonary resuscitation 5% vs. 2.2%, p = 0.387). At hospital admission, patients in group 2 suffered significantly more pain (E-problem according to Advanced Trauma Life Support principles©; 29.2% vs. 45.7%, p < 0.05). There were no significant differences in the clinical treatment (emergency procedures, vasopressor and coagulant therapy, and transfusion of red blood cells). tCT was 6 (4–10) minutes (median and IQR) in group 1 and 8 (5–15.5) minutes in group 2 (p = 0.280). tES was 90 (78–106) minutes in group 1 and 99 (97–108) minutes in group 2 (p = 0.081). The simultaneous treatment of two trauma patients in a dual-room trauma suite with an integrated movable sliding gantry CT-system requires a medical, organizational, and technical concept adapted to this special setting. Despite the oftentimes serious and life-threatening injuries, optimal diagnostic and therapeutic procedures can be guaranteed for two simultaneous trauma patients at an individual medical level in consistent quality.
Background
Severe COVID-19 induced acute respiratory distress syndrome (ARDS) often requires extracorporeal membrane oxygenation (ECMO). Recent German health insurance data revealed low ICU survival rates. Patient characteristics and experience of the ECMO center may determine intensive care unit (ICU) survival. The current study aimed to identify factors affecting ICU survival of COVID-19 ECMO patients.
Methods
673 COVID-19 ARDS ECMO patients treated in 26 centers between January 1st 2020 and March 22nd 2021 were included. Data on clinical characteristics, adjunct therapies, complications, and outcome were documented. Block wise logistic regression analysis was applied to identify variables associated with ICU-survival.
Results
Most patients were between 50 and 70 years of age. PaO\(_{2}\)/FiO\(_{2}\) ratio prior to ECMO was 72 mmHg (IQR: 58–99). ICU survival was 31.4%. Survival was significantly lower during the 2nd wave of the COVID-19 pandemic. A subgroup of 284 (42%) patients fulfilling modified EOLIA criteria had a higher survival (38%) (p = 0.0014, OR 0.64 (CI 0.41–0.99)). Survival differed between low, intermediate, and high-volume centers with 20%, 30%, and 38%, respectively (p = 0.0024). Treatment in high volume centers resulted in an odds ratio of 0.55 (CI 0.28–1.02) compared to low volume centers. Additional factors associated with survival were younger age, shorter time between intubation and ECMO initiation, BMI > 35 (compared to < 25), absence of renal replacement therapy or major bleeding/thromboembolic events.
Conclusions
Structural and patient-related factors, including age, comorbidities and ECMO case volume, determined the survival of COVID-19 ECMO. These factors combined with a more liberal ECMO indication during the 2nd wave may explain the reasonably overall low survival rate. Careful selection of patients and treatment in high volume ECMO centers was associated with higher odds of ICU survival.
Background: Over the recent years, technological advances of wrist-worn fitness trackers heralded a new era in the continuous monitoring of vital signs. So far, these devices have primarily been used for sports.
Objective: However, for using these technologies in health care, further validations of the measurement accuracy in hospitalized patients are essential but lacking to date.
Methods: We conducted a prospective validation study with 201 patients after moderate to major surgery in a controlled setting to benchmark the accuracy of heart rate measurements in 4 consumer-grade fitness trackers (Apple Watch 7, Garmin Fenix 6 Pro, Withings ScanWatch, and Fitbit Sense) against the clinical gold standard (electrocardiography).
Results: All devices exhibited high correlation (r≥0.95; P<.001) and concordance (rc≥0.94) coefficients, with a relative error as low as mean absolute percentage error <5% based on 1630 valid measurements. We identified confounders significantly biasing the measurement accuracy, although not at clinically relevant levels (mean absolute error<5 beats per minute).
Conclusions: Consumer-grade fitness trackers appear promising in hospitalized patients for monitoring heart rate.
Background
Systematic reviews attempt to gather all available evidence. Controversy exists regarding effort and benefit of including study results presented at conferences only. We recently published a Cochrane network meta-analysis (NMA) including 585 randomized controlled trials comparing drugs for prevention of postoperative nausea and vomiting (PONV). Studies published as conference abstracts only were excluded. This study aimed to include all eligible studies published as abstracts only, assessing their added value regarding reporting quality and effect on the review’s interpretation.
Methods
Conference abstracts were searched in the review’s excluded studies and conference proceedings of anaesthesiologic societies. We assessed their reporting quality regarding review’s eligibility criteria, Cochrane ‘risk of bias’ assessment tool 1.0, and adherence to CONSORT (Consolidated Standards of Reporting Trials) for abstracts. Abstracts were included in sensitivity NMA, and impact on the NMA structure was investigated.
Results
We identified 90 abstracts. A total of 14% (13/90) were eligible. A total of 86% (77/90) are awaiting classification due to insufficient reporting of review’s eligibility criteria. In abstracts awaiting classification, sufficient information was missing on standardization of anaesthesia in 71% (55/77), age of participants in 56% (43/77), and outcome details in 46% (36/77). A total of 73% (66/90) of abstracts lacked sufficient information on 15/25 data extraction items. Reported study characteristics of abstracts were comparable to included studies of the review. A total of 62% (56/90) of abstract trials were assessed as overall high risk of bias due to poor reporting. Median adherence to CONSORT for abstracts was 24% (IQR, 18 to 29%). Six of the 13 eligible abstracts reported relevant outcome data in sufficient detail for NMA on seven outcomes of the Cochrane review. Inclusion of abstracts did not substantially change the network structure, network effect estimates, ranking of treatments, or the conclusion. Certainty of evidence for headache on palonosetron use was upgraded from very low to low.
Conclusions
Most conference abstracts on PONV were insufficiently reported regarding review’s narrow inclusion criteria and could not be included in NMA. The resource-intensive search and evaluation of abstracts did not substantially extent the full-text evidence base of the review, given the few adequately reported abstracts. Conferences should oblige authors to adhere to CONSORT for abstracts.
Background:
Studies have reported on the incidence of sedation-related adverse events (AEs), but little is known about their impact on health care costs and resource use.
Methods: Health care providers and payers in five countries were recruited for an online survey by independent administrators to ensure that investigators and respondents were blinded to each other. Surveys were conducted in the local language and began with a "screener" to ensure that respondents had relevant expertise and experience. Responses were analyzed using Excel and R, with the Dixon's Q statistic used to identify and remove outliers. Global and country-specific average treatment patterns were calculated via bootstrapping; costs were mean values. The sum product of costs and intervention probability gave a cost per AE.
Results: Responses were received from 101 providers and 26 payers, the majority having. 5 years of experience. At a minimum, the respondents performed a total of 3,430 procedural sedations per month. All AEs detailed occurred in clinical practice in the last year and were reported to cause procedural delays and cancellations in some patients. Standard procedural sedation costs ranged from (sic)74 (Germany) to $2,300 (US). Respondents estimated that AEs would increase costs by between 16% (Italy) and 179% (US). Hypotension was reported as the most commonly observed AE with an associated global mean cost (interquartile range) of $43 ($27-$68). Other frequent AEs, including mild hypotension, bradycardia, tachycardia, mild oxygen desaturation, hypertension, and brief apnea, were estimated to increase health care spending on procedural sedation by $2.2 billion annually in the US.
Conclusion: All sedation-related AEs can increase health care costs and result in substantial delays or cancellations of subsequent procedures. The prevention of even minor AEs during procedural sedation may be crucial to ensuring its value as a health care service.
Therapeutic drug monitoring (TDM) is increasingly relevant for an individualized antibiotic therapy and subsequently a necessary tool to reduce multidrug-resistant pathogens, especially in light of diminishing antimicrobial capabilities. Critical illness is associated with profound pharmacokinetic and pharmacodynamic alterations, which challenge dose finding and the application of particularly hydrophilic drugs such as β-lactam antibiotics. Methods: Implementation strategy, potential benefit, and practicability of the developed standard operating procedures were retrospectively analyzed from January to December 2020. Furthermore, the efficacy of the proposed dosing target of piperacillin in critically ill patients was evaluated. Results: In total, 160 patients received piperacillin/tazobactam therapy and were subsequently included in the study. Of them, 114 patients received piperacillin/tazobactam by continuous infusion and had at least one measurement of piperacillin serum level according to the standard operating procedure. In total, 271 measurements were performed with an average level of 79.0 ± 46.0 mg/L. Seventy-one piperacillin levels exceeded 100 mg/L and six levels were lower than 22.5 mg/L. The high-level and the low-level group differed significantly in infection laboratory parameters (CRP (mg/dL) 20.18 ± 11.71 vs. 5.75 ± 5.33) and renal function [glomerular filtration rate (mL/min/1.75 m2) 40.85 ± 26.74 vs. 120.50 ± 70.48]. Conclusions: Piperacillin levels are unpredictable in critically ill patients. TDM during piperacillin/tazobactam therapy is highly recommended for all patients. Although our implementation strategy was effective, further strategies implemented into the daily clinical workflow might support the health care staff and increase the clinicians' alertness.
Takotsubo syndrome (TTS), also known as the transient left ventricular apical ballooning syndrome, is in contemporary times known as novel acute cardiac syndrome. It is characterized by transient left ventricular apical akinesis and hyperkinesis of the basal left ventricular portions. Although the precise etiology of TTS is unknown, events like the sudden release of stress hormones, such as the catecholamines and the increased inflammatory status might be plausible causes leading to the cardiovascular pathologies. Recent studies have highlighted that an imbalance in lipid accumulation might promote a deviant immune response as observed in TTS. However, there is no information on comprehensive profiling of serum lipids of TTS patients. Therefore, we investigated a detailed quantitative lipid analysis of TTS patients using ES-MSI. Our results showed significant differences in the majority of lipid species composition in the TTS patients compared to the control group. Furthermore, the computational analyses presented was able to link the altered lipids to the pro-inflammatory cytokines and disseminate possible mechanistic pathways involving TNFα and IL-6. Taken together, our study provides an extensive quantitative lipidome of TTS patients, which may provide a valuable Pre-diagnostic tool. This would facilitate the elucidation of the underlying mechanisms of the disease and to prevent the development of TTS in the future.
Peripheral neuropathy is accompanied by changes in the neuronal environment. The blood-nerve barrier (BNB) is crucial in protecting the neural homeostasis: Tight junctions (TJ) seal paracellular spaces and thus prevent external stimuli from entering. In different models of neuropathic pain, the BNB is impaired, thus contributing to local damage, immune cell invasion and, ultimately, the development of neuropathy with its symptoms. In this study, we examined changes in expression and microstructural localization of two key tight junction proteins (TJP), claudin-1 and the cytoplasmic anchoring ZO-1, in the sciatic nerve of mice subjected to chronic constriction injury (CCI). Via qPCR and analysis of fluorescence immunohistochemistry, a marked downregulation of mRNA as well as decreased fluorescence intensity were observed in the nerve for both proteins. Moreover, a distinct zig-zag structure for both proteins located at cell-cell contacts, indicative of the localization of TJs, was observed in the perineurial compartment of sham-operated animals. This microstructural location in cell-cell-contacts was lost in neuropathy as semiquantified via computational analysis, based on a novel algorithm. In summary, we provide evidence that peripheral neuropathy is not only associated with decrease in relevant TJPs but also exhibits alterations in TJP arrangement and loss in barrier tightness, presumably due to internalization. Specifically, semiquantification of TJP in cell-cell-contacts of microcompartments could be used in the future for routine clinical samples of patients with neuropathy.
We present an exceptional case of recurrent cycling‐induced spontaneous pneumomediastinum and pneumopericardium in a female patient without any trauma. Radiological and endoscopic examinations were carried out to exclude other differential diagnoses. Decision for in‐hospital observation and conservative treatment was made. No symptoms were reported 12 months after return to sports activity.
Der Zusammenhang von neuropathischem Schmerz mit einer gestörten Blut-Nerven-
Schranke (BNS) ist bekannt. Die BNS wird durch Tight Junction Proteine (TJP) gebildet.
Netrin-1 (Ntn1) hat je nach Rezeptorbindung verschiedene Effekte auf TJP und somit auf
die Barriereeigenschaften.
In dieser Arbeit wurde im Tiermodell (Chronic Constriction Injury-CCI) untersucht, ob
Netrin-1 einen Einfluss auf die BNS hat und die Wirkung der Rezeptoren Unc5b und
Neogenin-1 beleuchtet. Es wurde untersucht, ob der barrierestabilisierende Netrin-1-
Spiegel auch von neuropathischen Schmerzen, im Speziellen durch „Chronic Regional
Pain Syndrom“ (CRPS), beeinflusst wird.
Männl. Wistar-Ratten wurde lokal Unc5b Antikörper injeziert oder nach Netrin-1 Gabe
der Neogeninrezeptor durch lokale Neogenin-1-siRNA Injektion geblockt. Die mRNA
Expression von Ntn1, seine Rezeptoren sowie der TJP (Claudine-Cldn) wurde mittels q-
PCR untersucht. Netrin-1 wurde im Rattennerven mittels Western Blot bestimmt. Die
Netrin-1-Spiegel im Plasma von CRPS Patient*innen und Kontrollen wurde mittels ELISA
bestimmt. Im Rattenmodell war die Ntn1 vermehrt exprimiert, die Proteinexpression
mittels Western Blot tendenziell vermindert. Die Claudinexpression war nach CCI
herabreguliert. Netrin-1-Injektion steigerte die Expression von Cldn5 und 19. Der
Netrin-1-Rezeptor UNC5B wird bei Neuropathie verstärkt und Neogenin-1 vermindert
exprimiert. Die Expression von Cldn 12 und Cldn19 war bei Blockade des Unc5b
Rezeptors gesteigert und bei Blockade des Neogenin-1 Rezeptors tendenziell
vermindert. Im Plasma von CRPS Patient*innen zeigte sich ein verminderter Netrin-1-
Spiegel.
Die Ergebnisse der vorliegenden Experimente legen nahe, dass Netrin-1 über die
Stabilisierung der Blut-Nerven-Schranke einen lindernden Effekt auf neuropathische
Schmerzen hat und sich auch die Expression dieses Proteins durch CRPS verändert.
Patients operated for infective endocarditis (IE) are at high risk of developing an excessive systemic hyperinflammatory state, resulting in systemic inflammatory response syndrome and septic shock. Hemoadsorption (HA) by cytokine adsorbers has been successfully applied to remove inflammatory mediators. This randomized controlled trial investigates the effect of perioperative HA therapy on inflammatory parameters and hemodynamic status in patients operated for IE. A total of 20 patients were randomly assigned to either HA therapy or the control group. HA therapy was initiated intraoperatively and continued for 24 hours postoperatively. Cytokine levels (IL‐6, IL‐1b, TNF‐α), leukocytes, C‐reactive protein (CRP), and Procalcitonin (PCT) as well as catecholamine support, and volume requirement were compared between both groups. Operative procedures included aortic (n = 7), mitral (n = 6), and multiple valve surgery (n = 7). All patients survived to discharge. No significant differences concerning median cytokine levels (IL‐6 and TNF‐α) were observed between both groups. CRP and PCT baseline levels were significantly higher in the HA group (59.5 vs. 26.3 mg/dL, P = .029 and 0.17 vs. 0.05 µg/L, P = .015) equalizing after surgery. Patients in the HA group required significantly higher doses of vasopressors (0.093 vs. 0.025 µg/kg/min norepinephrine, P = .029) at 12 hours postoperatively as well as significantly more overall volume replacement (7217 vs. 4185 mL at 12 hours, P = .015; 12 021 vs. 4850 mL at 48 hours, P = .015). HA therapy did neither result in a reduction of inflammatory parameters nor result in an improvement of hemodynamic parameters in patients operated for IE. For a more targeted use of HA therapy, appropriate selection criteria are required.

Eine adäquate Schmerztherapie ist eine zentrale Aufgabe der geburtsbegleitenden Anästhesie.
Ziel der Sekundäranalyse der prospektiven Längsschnittstudie war es, herauszufinden, ob es Variablen gibt, in denen sich Frauen unterscheiden, die mit der peripartalen Schmerztherapie zufrieden bzw. weniger zufrieden sind bzw. den Wunsch nach mehr Schmerzmitteln äußern oder nicht äußern.
Methodik:
Um dies herauszufinden wurden 210 Frauen vor Geburt (T1), kurz nach Geburt (T2) sowie drei (T3) und sechs Monate (T) postpartal bezüglich ihrer Zufriedenheit/ ihres Wunschs nach mehr Schmerzmitteln befragt. Des Weiteren wurden Daten zu Demographie, Schmerzmitteleinnahme, Geburtsablauf u.v.m. sowie Daten aus verschiedenen, psychologischen Fragebögen ermittelt. Die Auswertung der Daten konzentrierte sich auf die Erhebungszeitpunkte T1 und T2.
Ergebnisse:
Am ersten postpartalen Tag nach Kaiserschnitt zeigten sich sehr hohe Schmerzintensitäten (mediane Schmerzintensität bei Belastung: 8). Auffallend war, dass sowohl zu T1 als auch zu T2 eine erfolgreiche Schmerzlinderung beide Variablen signifikant beeinflusste, unabhängig davon, ob die Frauen trotz Schmerzlinderung noch starke Schmerzen hatten. 28% der Frauen erhielten nach Sectio retardierte Opioide jedoch blieb die Zufriedenheit und der Wunsch nach mehr Schmerzmitteln davon unbeeinflusst. Der Grund für die Ergebnisse bleibt unklar, könnte aber in möglichen Nebenwirkungen oder Vorbehalten gegenüber pharmakologischen Analgetika liegen. Beim präpartal durchgeführten PCS-Fragebogen waren Frauen mit Kaiserschnitt, die eine erhöhte Punktezahl aufwiesen, zum Zeitpunkt T2 signifikant unzufriedener und äußerten häufiger den Wunsch nach mehr Schmerzmittel.
Diskussion:
Die Ergebnisse weisen darauf hin, dass eine präpartale Erhebung von Risikofaktoren, wie z.B. erhöhte Werte auf der PCS-Skala ein Instrument sein könnte, um die Zufriedenheit mit der peripartalen Schmerztherapie zu verbessern. Patientinnen mit Risikofaktoren könnten so intensiver betreut werden, durch eine multimodale Therapie in Form von intensivierter Analgesie, aber z.B. auch durch eine psychologische Begleitung oder alternative Methoden zur Schmerzreduktion. Die erhaltene Schmerzlinderung präsentierte sich als wesentliche Einflussgröße auf die Zufriedenheit und den Wunsch nach mehr Schmerzmittel und ist somit womöglich zur Kontrolle des Therapieerfolgs besser geeignet als die alleinige Angabe der aktuellen Schmerzen.
Takotsubo syndrome (TTS), a transient form of dysfunction in the heart's left ventricle, occurs predominantly in postmenopausal women who have emotional stress. Earlier studies support the concept that the human circulatory system is modulated by a cortical network (consisting of the anterior cingulate gyrus, amygdala, and insular cortex (Ic)) that plays a pivotal role in the central autonomic nervous system in relation to emotional stressors. The Ic plays a crucial role in the sympathovagal balance, and decreased levels of female sex hormones have been speculated to change functional cerebral asymmetry, with a possible link to autonomic instability. In this review, we focus on the Ic as an important moderator of the human brain–heart axis in association with sex hormones. We also summarize the current knowledge regarding the sex-specific neuroanatomy in TTS.
Background: Anemia remains one of the most common comorbidities in intensive care patients worldwide. The cause of anemia is often multifactorial and triggered by underlying disease, comorbidities, and iatrogenic factors, such as diagnostic phlebotomies. As anemia is associated with a worse outcome, especially in intensive care patients, unnecessary iatrogenic blood loss must be avoided. Therefore, this scoping review addresses the amount of blood loss during routine phlebotomies in adult (>17 years) intensive care patients and whether there are factors that need to be improved in terms of patient blood management (PBM). Methods: A systematic search of the Medline Database via PubMed was conducted according to PRISMA guidelines. The reported daily blood volume for diagnostics and other relevant information from eligible studies were charted. Results: A total of 2167 studies were identified in our search, of which 38 studies met the inclusion criteria (9 interventional studies and 29 observational studies). The majority of the studies were conducted in the US (37%) and Canada (13%). An increasing interest to reduce iatrogenic blood loss has been observed since 2015. Phlebotomized blood volume per patient per day was up to 377 mL. All interventional trials showed that the use of pediatric-sized blood collection tubes can significantly reduce the daily amount of blood drawn. Conclusion: Iatrogenic blood loss for diagnostic purposes contributes significantly to the development and exacerbation of hospital-acquired anemia. Therefore, a comprehensive PBM in intensive care is urgently needed to reduce avoidable blood loss, including blood-sparing techniques, regular advanced training, and small-volume blood collection tubes.
Im Jahr 2017, 13 Jahre nachdem das ICMJE die prospektive Protokollregistrierung zur Notwendigkeit für zuverlässige klinische Studien erklärt hat, ist die Häufigkeit und Qualität der Studienregistrierung im Bereich der PONV-Forschung sehr gering. Für nur ein Fünftel der seit dem Jahr 2004 publizierten klinischen Studien, die in den 2020 veröffentlichten PONV Cochrane Review aufgenommen wurden, wurden Studienprotokolle registriert. Von diesen waren fast zwei Drittel retrospektiv registriert. Schlussendlich konnten weniger als 50% der prospektiv registrierten Studien als frei von Bias bei der selektiven Endpunktberichterstattung identifiziert werden. Dies ist ein alarmierendes Defizit. Diese Arbeit zeigt auch, dass registrierte Studien im Allgemeinen häufiger mit einem niedrigen Gesamtbiasrisiko beurteilt wurden. Dies legt die Studienregistrierung als Qualitätskriterium für RCTs in der klinischen PONV-Forschung nahe. Bias durch selektive Endpunktberichterstattung verringert die Vertrauenswürdigkeit von Studienergebnissen. Wissenschaftler*innen und Kliniker*innen sollten sich darüber im Klaren sein, dass nur die Adhärenz bezüglich einer adäquaten Protokollregistrierung und die transparente Berichterstattung über vordefinierte Endpunkte, unabhängig von Richtung und Bedeutung deren Ergebnisse, letztlich die Evidenz der PONV-Forschung in der Zukunft stärken kann.
Im Zeitraum vom 30.01.2019 bis zum 26.05.2020 wurden aus dem Universitätsklinikum Würzburg alle Patienten, die sowohl eine Charmi-Erstellung als auch eine Anlage des ASD-Dokuments erhalten haben, in das zu untersuchende Patientenkollektiv aufgenommen. Von diesen, so ermittelten, 1458 Patienten werden die stattgefundenen Operationen der Patienten eruiert. Die Bewegungswerte werden mithilfe des hierarchisch angeordneten Charmi-Systems von 0 bis 10 von den Physiotherapeuten erhoben. Dabei stellt der Charité Mobilitätsindex (Charmi-Index) ein neuartiges Diagnostikum dar, welches als Bewertungssystem zu Beginn 2019 erstmalig von den Physiotherapeuten an der Universität Würzburg eingesetzt wurde.
Der Schmerzzustand und Schmerzverlauf wird im Rahmen der Schmerzvisiten durch den Akutschmerzdienst (ASD) mithilfe der NRS-Skala festgehalten. Auch die NRS-Skala stellt eine hierarchisch aufgebaute Skala von 0 bis 10 dar. Schmerzwerte werden täglich im Verlauf der postoperativen Visiten erhoben. Charmi-Werte werden zu Beginn der postoperativen Phase, bei Zwischenereignissen und am Ende des stationären Aufenthalts oder bei Verlegung in einen anderen Fachbereich erhoben. Bei zeitgleich erhobenen Charmi-Werten und Schmerzwerten können diese zueinander in Bezug gesetzt werden.
Dabei kommt heraus, dass das Schmerzniveau in Ruhe und bei Belastung in der frühen postoperativen Phase negativ mit der Mobilität assoziiert ist. Zusätzlich zeigt sich, dass bei zunehmender Medikamentendosis die Mobilität sinkt. Je größer die Schmerzen der Patienten sind, desto schlechter die Mobilität. Wegen fehlender Charmi-Zwischenbefunde und zeitlich versetzt erhobener Charmi-Endbefunde ist kaum ein Zusammenhang zur Mobilität ersichtlich. Bei zeitlich passenden Charmi-Endwerten zeigt sich dennoch, dass bei steigenden Belastungsschmerzen die Mobilität abnimmt.
Eine weitere Erkenntnis ist, dass der Belastungsschmerz bei niedrigen Schmerzwerten durchschnittlich um 1,5-1,6 höher ist als der Ruheschmerz, bei mittleren Schmerzwerten um 1,5-1,4 höher und bei hohen Schmerzwerten um 1,4-1,2 höher.
Bei steigendem Ruheschmerz wird von den Patienten mehr Medikament angefordert als bei steigendem Belastungsschmerz, was die Vermutung zulässt, dass Patienten eher auf die Bewegung verzichten. Ein Therapieregime vermehrt mit Blickrichtung auf den Belastungsschmerz scheint daher an Bedeutung zuzunehmen.
Background: Acute respiratory failure is the most important organ dysfunction of COVID-19 patients. While non-invasive ventilation (NIV) and high-flow nasal cannula (HFNC) oxygen are frequently used, efficacy and safety remain uncertain. Benefits and harms of awake prone positioning (APP) in COVID-19 patients are unknown. Methods: We searched for randomized controlled trials (RCTs) comparing HFNC vs. NIV and APP vs. standard care. We meta-analyzed data for mortality, intubation rate, and safety. Results: Five RCTs (2182 patients) were identified. While it remains uncertain whether HFNC compared to NIV alters mortality (RR: 0.92, 95% CI 0.65–1.33), HFNC may increase rate of intubation or death (composite endpoint; RR 1.22, 1.03–1.45). We do not know if HFNC alters risk for harm. APP compared to standard care probably decreases intubation rate (RR 0.83, 0.71–0.96) but may have little or no effect on mortality (RR: 1.08, 0.51–2.31). Conclusions: Certainty of evidence is moderate to very low. There is no compelling evidence for either HFNC or NIV, but both carry substantial risk for harm. The use of APP probably has benefits although mortality appears unaffected.
(1) Background: Data on coronavirus 2 infection during pregnancy vary. We aimed to describe maternal characteristics and clinical presentation of SARS-CoV-2 positive women requiring intensive care treatment for COVID-19 during pregnancy and postpartum period based on data of a comprehensive German surveillance system in obstetric patients. (2) Methods: Data from COVID-19 Related Obstetric and Neonatal Outcome Study (CRONOS), a prospective multicenter registry for SARS-CoV-2 positive pregnant women, was analyzed with respect to ICU treatment. All women requiring intensive care treatment for COVID-19 were included and compared regarding maternal characteristics, course of disease, as well as maternal and neonatal outcomes. (3) Results: Of 2650 cases in CRONOS, 101 women (4%) had a documented ICU stay. Median maternal age was 33 (IQR, 30–36) years. COVID-19 was diagnosed at a median gestational age of 33 (IQR, 28–35) weeks. As the most invasive form of COVID-19 treatment interventions, patients received either continuous monitoring of vital signs without further treatment requirement (n = 6), insufflation of oxygen (n = 30), non-invasive ventilation (n = 22), invasive ventilation (n = 28), or escalation to extracorporeal membrane oxygenation (n = 15). No significant clinical differences were identified between patients receiving different forms of ventilatory support for COVID-19. Prevalence of preterm delivery was significantly higher in women receiving invasive respiratory treatments. Four women died of COVID-19 and six fetuses were stillborn. (4) Conclusions: Our cohort shows that progression of COVID-19 is rare in pregnant and postpartum women treated in the ICU. Preterm birth rate is high and COVID-19 requiring respiratory support increases the risk of poor maternal and neonatal outcome.
Background: COVID-19 patients are at high thrombotic risk. The safety and efficacy of different anticoagulation regimens in COVID-19 patients remain unclear. Methods: We searched for randomised controlled trials (RCTs) comparing intermediate- or therapeutic-dose anticoagulation to standard thromboprophylaxis in hospitalised patients with COVID-19 irrespective of disease severity. To assess efficacy and safety, we meta-analysed data for all-cause mortality, clinical status, thrombotic event or death, and major bleedings. Results: Eight RCTs, including 5580 patients, were identified, with two comparing intermediate- and six therapeutic-dose anticoagulation to standard thromboprophylaxis. Intermediate-dose anticoagulation may have little or no effect on any thrombotic event or death (RR 1.03, 95% CI 0.86–1.24), but may increase major bleedings (RR 1.48, 95% CI 0.53–4.15) in moderate to severe COVID-19 patients. Therapeutic-dose anticoagulation may decrease any thrombotic event or death in patients with moderate COVID-19 (RR 0.64, 95% CI 0.38–1.07), but may have little or no effect in patients with severe disease (RR 0.98, 95% CI 0.86–1.12). The risk of major bleedings may increase independent of disease severity (RR 1.78, 95% CI 1.15–2.74). Conclusions: Certainty of evidence is still low. Moderately affected COVID-19 patients may benefit from therapeutic-dose anticoagulation, but the risk for bleeding is increased.
Vaccination hesitancy is a threat to herd immunity. Healthcare workers (HCWs) play a key role in promoting Coronavirus disease 2019 (COVID-19) vaccination in the general population. We therefore aimed to provide data on COVID-19 vaccination acceptance/hesitancy among German HCWs. For this exploratory, cross-sectional study, an online survey was conducted in February 2021. The survey included 54 items on demographics; previous vaccination behavior; trust in vaccines, physicians, the pharmaceutical industry and health politics; fear of adverse effects; assumptions regarding the consequences of COVID-19; knowledge about vaccines; and information seeking behavior. Odds ratios with 95% confidence intervals were calculated and chi-square tests were performed. Four thousand five hundred surveys were analyzed. The overall vaccination acceptance was 91.7%. The age group ≤20 years showed the lowest vaccination acceptance. Factors associated with vaccination hesitancy were lack of trust in authorities and pharmaceutical companies. Attitudes among acquaintances were associated with vaccination hesitancy too. Participants with vaccination hesitancy more often obtained information about COVID-19 vaccines via messenger services or online video platforms and underperformed in the knowledge test. We found high acceptance amongst German HCWs. Several factors associated with vaccination hesitancy were identified which could be targeted in HCW vaccination campaigns.
(1) Background: Nowadays, the use of microsurgical free flaps is a standard operative procedure in reconstructive surgery. Still, thrombosis of the microanastomosis is one of the most fatal postoperative complications. Clinical evaluation, different technical devices and laboratory markers are used to monitor critical flap perfusion. Macrophage migration inhibitory factor (MIF), a structurally unique cytokine with chemokine-like characteristics, could play a role in predicting vascular problems and the failure of flap perfusion. (2) Methods: In this prospective observational study, 26 subjects that underwent microsurgical reconstruction were observed. Besides clinical data, the number of blood leukocytes, CRP and MIF were monitored. (3) Results: Blood levels of MIF, C-reactive protein (CRP) and leukocytes increased directly after surgery. Subjects that needed surgical revision due to thrombosis of the microanastomosis showed significantly higher blood levels of MIF than subjects without revision. (4) Conclusion: We conclude that MIF is a potential and innovative indicator for thrombosis of the microanastomosis after free flap surgery. Since it is easy to obtain diagnostically, MIF could be an additional tool to monitor flap perfusion besides clinical and technical assessments.
In der Volumentherapie galt das Kolloid Hydroxyethylstärke (HES) lange Zeit als ideale Infusionslösung und war der in Deutschland am häufigsten eingesetzte Plasmaexpander. In den letzten Jahren mehrten sich jedoch die Hinweise, dass HES insbesondere bei kritisch Kranken zu einer akuten Nierenfunktionsverschlechterung beitragen könnte, welche das klinische Ergebnis wesentlich beeinflusst. Der genaue Pathomechanismus ist bis heute nicht geklärt. Bekannt ist, dass sich HES nach intravenöser Applikation in vielen verschiedenen Geweben ablagert, wobei eine renale Anreicherung bevorzugt in proximalen Tubuluszellen stattfindet. Histopathologisch finden sich große Mengen intrazellulärer Vesikel im Zytoplasma, welche zu einer Zellschwellung führen, die auch als osmotische Nephrose bezeichnet und als prinzipiell reversibel erachtet wird. Der zelluläre Abbau soll dabei im Allgemeinen über das lysosomale System stattfinden. Dieses ist fester Bestandteil der Autophagie, welche ein evolutionär in allen Eukaryonten konservierter stress-induzierter kataboler Prozess ist, der der zellulären Homöostase und energieeffizienten Selbstreinigung dient. Hierbei werden defekte Makromoleküle durch Lysosomen in ihre Grundbestandteile zerlegt und der Zelle als Bausteine wieder zur Verfügung gestellt.
In dieser Arbeit wurde in einem ersten Schritt der Einfluss von HES der 3. Generation auf die Viabilität von HK-2-Zellen mit zwei unabhängigen in vitro Assays überprüft. Diese beruhen auf einem Substratumsatz durch zytosolische bzw. mitochondriale Dehydrogenasen. Für beide Assays konnte zu verschiedenen Inkubationszeitpunkten bis 21 Stunden jeweils eine konzentrationsabhängige Viabilitätsreduktion durch HES festgestellt werden, welche auch nach einer „Regenerationsphase“ noch verringert nachweisbar und somit partiell reversibel war.
Im nächsten Schritt wurde die Hypothese überprüft, ob eine medikamentöse Induktion der Autophagie die beobachtete Viabilitätsreduktion abschwächen oder sogar aufheben kann. Hierbei wurde analog zu einer Arbeit von Liu et al. (2014) eine Inkubationszeit von insgesamt acht Stunden gewählt, da nach diesem Zeitraum eine perinukleäre Cluster-Bildung der Lysosomen beobachtet werden konnte, welche für eine erhöhte Autophagierate spricht. Es kamen im Folgenden HES-Lösungen von 0,75% zum Einsatz, da diese aufgrund einer Viabilitätsreduktion um zirka ein Drittel als am besten geeignet betrachtet wurden. Der Autophagieinduktor Everolimus zeigte hierbei in dem auf mitochondrialen Dehydrogenasen basierenden EZ4U-Assay eine fast vollständige Aufhebung der HES-vermittelten Viabilitätsreduktion. Dieser Effekt konnte durch den MAP-Kinase-Kinase-Blocker U0126 aufgehoben werden. Andere Autophagiemodulatoren hingegen bewirkten zumeist nur geringe Änderungen der Zellviabilität.
Zuletzt wurde auf Proteinebene untersucht, ob zentrale Moleküle bzw. Komplexe der Autophagie unter HES zum einen im zeitlichen Verlauf und zum anderen unter zusätzlichem Einfluss der Modulatoren eine Expressionsänderung aufwiesen. HES alleine bewirkte im zeitlichen Verlauf weitestgehend keine signifikante Expressionsänderung. Auch im Vergleich zu einer 0%-HES-Kontrolllösung konnten keine relevanten Unterschiede festgestellt werden. Die Koinkubation mit Everolimus führte zu einer erhöhten Expression der Quotienten ppERK/pERK und LC3BII/LC3BI, U0126 konnte dies jeweils weitestgehend wieder aufheben. Perifosin bewirkte ebenso wie 3-Methyladenin eine verringerte Expression von Akt, Chloroquin führte zu keiner signifikanten Expressionsänderung aller bestimmter Proteine. Darüber hinaus verursachten alle Modulatoren keine signifikante Expressionsänderung des zentralen Autophagiekomplexes Beclin1 sowie von LAMP2 und SQSTM1.
Insgesamt sprechen die Ergebnisse dieser Arbeit gegen eine allgemeine, direkte Beeinflussung von klassischer Autophagie durch HES. Der Autophagieinduktor Everolimus zeigte jedoch einen protektiven Effekt auf die Zellviabilität, welcher vermutlich über einen Autophagie-vermittelten Weg verursacht wird. Unsere Arbeitsgruppe konnte darüber hinaus eine HES-vermittelte Reduktion von ROS beobachten. Das Autophagienetzwerk ist eng mit der zellulären Redox-Homöostase verknüpft. Eine verminderte ROS-Bildung könnte zu einer verminderten Autophagierate und somit auch verringerten Zellviabilität führen, welche durch Everolimus kompensiert wird.
Bisher ungeklärt ist auch, auf welchem Weg HES in die Zelle aufgenommen wird. Denkbar wäre, dass größere HES-Moleküle via Endozytose in die Zelle gelangen. Hierbei ist der mTORC1-Komplex ein wichtiger Regulator, der durch Everolimus gehemmt wird und somit über eine verringerte HES-Aufnahme zu einer Viabilitätssteigerung führen könnte. Kleinere HES-Moleküle dagegen könnten über Glukose-Transporter aufgenommen werden, die möglicherweise über AMPK reguliert werden.
Mortality in critically ill coronavirus disease 2019 (COVID-19) patients is high and pharmacological treatment strategies remain limited. Early-stage predictive biomarkers are needed to identify patients with a high risk of severe clinical courses and to stratify treatment strategies. Macrophage migration inhibitory factor (MIF) was previously described as a potential predictor for the outcome of critically ill patients and for acute respiratory distress syndrome (ARDS), a hallmark of severe COVID-19 disease. This prospective observational study evaluates the predictive potential of MIF for the clinical outcome after severe COVID-19 infection. Plasma MIF concentrations were measured in 36 mechanically ventilated COVID-19 patients over three days after intensive care unit (ICU) admission. Increased compared to decreased MIF was significantly associated with aggravated organ function and a significantly lower 28-day survival (sequential organ failure assessment (SOFA) score; 8.2 ± 4.5 to 14.3 ± 3, p = 0.009 vs. 8.9 ± 1.9 to 12 ± 2, p = 0.296; survival: 56% vs. 93%; p = 0.003). Arterial hypertension was the predominant comorbidity in 85% of patients with increasing MIF concentrations (vs. decreasing MIF: 39%; p = 0.015). Without reaching significance, more patients with decreasing MIF were able to improve their ARDS status (p = 0.142). The identified association between an early MIF response, aggravation of organ function and 28-day survival may open future perspectives for biomarker-based diagnostic approaches for ICU management of COVID-19 patients.
Purpose: The threat of national and international terrorism remains high. Preparation is the key requirement for the resilience of hospitals and out-of-hospital rescue forces. The scientific evidence for defining medical and tactical strategies often feeds on the analysis of real incidents and the lessons learned derived from them. This systematic review of the literature aims to identify and systematically report lessons learned from terrorist attacks since 2001.
Methods: PubMed was used as a database using predefined search strategies and eligibility criteria. All countries that are part of the Organization for Economic Cooperation and Development (OECD) were included. The time frame was set between 2001 and 2018.
Results: 68 articles were included in the review. From these, 616 lessons learned were extracted and summarized into 15 categories. The data shows that despite the difference in attacks, countries, and casualties involved, many of the lessons learned are similar. We also found that the pattern of lessons learned is repeated continuously over the time period studied.
Conclusions: The lessons from terrorist attacks since 2001 follow a certain pattern and remained constant over time. Therefore, it seems to be more accurate to talk about lessons identified rather than lessons learned. To save as many victims as possible, protect rescue forces from harm, and to prepare hospitals at the best possible level it is important to implement the lessons identified in training and preparation.
D-4F, ein ApoA-I-mimetisches Peptid, lindert mechanische Hyperalgesie nach Verabreichung an Nagetiere, die an entzündlichen und neuropathischen Schmerzen leiden. D-4F fängt proalgetische oxidierte Lipide ab – als ein solches entzündungshemmendes Medikament – aktiviert aber auch ATP-bindende Kassettentransporter (Abca1 und Abcg1) – als ein solches anti-atherosklerotisches Medikament. Ziel dieser Arbeit war es, den Einfluss von Neuropathie und deren Behandlung mit D-4F auf die Expression von Abca1/Abcg1 sowie Zytokinen zu untersuchen.
Hintergrund
Im Rahmen der Pandemie des SARS-CoV-2-Virus erlangte das Patientenkollektiv der Schwangeren früh Aufmerksamkeit. Initial wurde angesichts sich früh abzeichnender Krankheitsfälle bei jüngeren Patienten mit einem erheblichen Aufkommen peripartal zu betreuender, COVID-19-positiver Schwangerer gerechnet.
Ziel der Arbeit
Diese Arbeit vermittelt einen Einblick in die SARS-CoV-2-Infektionszahlen im Rahmen der geburtshilflichen Anästhesie zu Beginn der Pandemie sowie während der zweiten Infektionswelle in Deutschland.
Methoden
Über das COALA-Register (COVID-19 related Obstetric Anaesthesia Longitudinal Assessment-Registry) wurden sowohl von März bis Mai 2020 als auch von Oktober 2020 bis Februar 2021 in Deutschland und der Schweiz wöchentlich prospektiv Daten zu Verdachts- und bestätigten SARS-CoV-2-Fällen bei Schwangeren zum Zeitpunkt der Geburt erhoben. Betrachtet wurden die Verteilung dieser auf die Anzahl der Geburten, Zentren und Erhebungswochen sowie mütterliche Charakteristika und Krankheitsverläufe.
Ergebnisse
Neun Zentren haben im Verlauf 44 SARS-CoV-2-positive Schwangere zum Zeitpunkt der Geburt bei 7167 Geburten (0,6 %) gemeldet (3 Fälle auf 2270 Geburten (0,4 %) und 41 Fälle auf 4897 Geburten (0,8 %)). Berichtet wurden 2 schwere COVID-19-Verläufe (n = 1 mit Todesfolge nach ECMO, n = 1 mit ECMO überlebt). Bei 28 (68 %) Patientinnen verlief die Infektion asymptomatisch. Ein Neugeborenes wurde im Verlauf positiv auf SARS-CoV‑2 getestet.
Schlussfolgerung
Mithilfe des Registers konnte das Auftreten von Fällen zu Beginn der Pandemie zeitnah eingeschätzt werden. Es traten sporadisch Verdachtsfälle bzw. bestätigte Fälle auf. Aufgrund fehlender flächendeckender Testung muss aber von einer Dunkelziffer asymptomatischer Fälle ausgegangen werden. Während der zweiten Infektionswelle wurden 68 % asymptomatische Fälle gemeldet. Jedoch kann es bei jungen, gesunden Patientinnen ohne das Vorliegen typischer Risikofaktoren zu schwerwiegenden Verläufen kommen.
Cardiac surgery (CSX) can be lifesaving in elderly patients (age ≥ 80 years) but may still be associated with complications and functional decline. Frailty represents a determinant to outcomes in critically ill patients, but little is known about its influence on elderly CSX-patients. This is a secondary exploratory analysis of a multi-center, prospective observational cohort study of 610 elderly patients admitted to the ICU and followed for one year to document long-term outcomes. CSX-ICU-patients (n = 49) were compared to surgical ICU patients (n = 184) with regard to demographics, frailty, and outcomes. Of all surgical patients, 102 (43%) were considered vulnerable or frail. The subdistribution hazard ratio (SHR) of time to discharge home (TTDH) for vulnerable/frail vs. fit/well patients was 0.54 (95% confidence interval (CI), 0.34, 0.86, p = 0.007). The p-value for effect modification between surgery group (CSX vs. surgical ICU patients) and Clinical Frailty Scale (CFS) group was not significant (p = 0.37) suggesting that the observed difference in the CFS effect between the CSX and surgical ICU patients is consistent with random error. A further subgroup analysis shows that among surgical ICU patients, the SHR of time to discharge home (TTDH) for vulnerable/frail vs. fit/well patients was 0.49 (95% CI, 0.29, 0.83) while the corresponding SHR for CSX patients was 0.77 (0.32–1.88). In conclusion, preoperative frailty reduced the rate of discharge to home in both surgical and CSX patients, but a larger sample of CSX patients is needed to adequately address this question in this patient group.
Laparoscopic techniques have established themselves as a major part of modern surgery. Their implementation in every surgical discipline has played a vital part in the reduction of perioperative morbidity and mortality. Precise robotic surgery, as an evolution of this, is shaping the present and future operating theatre that an anesthetist is facing. While incisions get smaller and the impact on the organism seems to dwindle, challenges for anesthetists do not lessen and could even become more demanding than in open procedures. This review focuses on the pathophysiological effects of contemporary laparoscopic and robotic procedures and summarizes anesthetic challenges and strategies for perioperative management.
Background: Macrophage Migration Inhibitory Factor (MIF) is highly elevated after cardiac surgery and impacts the postoperative inflammation. The aim of this study was to analyze whether the polymorphisms CATT\(_{5–7}\) (rs5844572/rs3063368,“-794”) and G>C single-nucleotide polymorphism (rs755622,-173) in the MIF gene promoter are related to postoperative outcome. Methods: In 1116 patients undergoing cardiac surgery, the MIF gene polymorphisms were analyzed and serum MIF was measured by ELISA in 100 patients. Results: Patients with at least one extended repeat allele (CATT\(_7\)) had a significantly higher risk of acute kidney injury (AKI) compared to others (23% vs. 13%; OR 2.01 (1.40–2.88), p = 0.0001). Carriers of CATT\(_7\) were also at higher risk of death (1.8% vs. 0.4%; OR 5.12 (0.99–33.14), p = 0.026). The GC genotype was associated with AKI (20% vs. GG/CC:13%, OR 1.71 (1.20–2.43), p = 0.003). Multivariate analyses identified CATT\(_7\) predictive for AKI (OR 2.13 (1.46–3.09), p < 0.001) and death (OR 5.58 (1.29–24.04), p = 0.021). CATT\(_7\) was associated with higher serum MIF before surgery (79.2 vs. 50.4 ng/mL, p = 0.008). Conclusion: The CATT\(_7\) allele associates with a higher risk of AKI and death after cardiac surgery, which might be related to chronically elevated serum MIF. Polymorphisms in the MIF gene may constitute a predisposition for postoperative complications and the assessment may improve risk stratification and therapeutic guidance.
Alzheimer's disease (AD), the most common cause of dementia in the elderly, is a neurodegenerative disorder associated with neurovascular dysfunction and cognitive decline. While the deposition of amyloid β peptide (Aβ) and the formation of neurofibrillary tangles (NFTs) are the pathological hallmarks of AD-affected brains, the majority of cases exhibits a combination of comorbidities that ultimately lead to multi-organ failure. Of particular interest, it can be demonstrated that Aβ pathology is present in the hearts of patients with AD, while the formation of NFT in the auditory system can be detected much earlier than the onset of symptoms. Progressive hearing impairment may beget social isolation and accelerate cognitive decline and increase the risk of developing dementia. The current review discusses the concept of a brain–ear–heart axis by which Aβ and NFT inhibition could be achieved through targeted supplementation of neurotrophic factors to the cochlea and the brain. Such amyloid inhibition might also indirectly affect amyloid accumulation in the heart, thus reducing the risk of developing AD-associated amyloid cardiomyopathy and cardiovascular disease.
Progressive deterioration of the central nervous system (CNS) is commonly associated with aging. An important component of the neurovasculature is the blood-brain barrier (BBB), majorly made up of endothelial cells joined together by intercellular junctions. The relationship between senescence and changes in the BBB has not yet been thoroughly explored. Moreover, the lack of in vitro models for the study of the mechanisms involved in those changes impede further and more in-depth investigations in the field. For this reason, we herein present an in vitro model of the senescent BBB and an initial attempt to identify senescence-associated alterations within.
Background: Selective outcome reporting in clinical trials introduces bias in the body of evidence distorting clinical decision making. Trial registration aims to prevent this bias and is suggested by the International Committee of Medical Journal Editors (ICMJE) since 2004.
Methods: The 585 randomized controlled trials (RCTs) published between 1965 and 2017 that were included in a recently published Cochrane review on antiemetic drugs for prevention of postoperative nausea and vomiting were selected. In a retrospective study, we assessed trial registration and selective outcome reporting by comparing study publications with their registered protocols according to the ‘Cochrane Risk of bias’ assessment tool 1.0.
Results: In the Cochrane review, the first study which referred to a registered trial protocol was published in 2004. Of all 585 trials included in the Cochrane review, 334 RCTs were published in 2004 or later, of which only 22% (75/334) were registered. Among the registered trials, 36% (27/75) were pro- and 64% (48/75) were retrospectively registered. 41% (11/27) of the prospectively registered trials were free of selective outcome reporting bias, 22% (6/27) were incompletely registered and assessed as unclear risk, and 37% (10/27) were assessed as high risk. Major outcome discrepancies between registered and published high risk trials were a change from the registered primary to a published secondary outcome (32%), a new primary outcome (26%), and different outcome assessment times (26%). Among trials with high risk of selective outcome reporting 80% favoured at least one statistically significant result. Registered trials were assessed more often as ‘overall low risk of bias’ compared to non-registered trials (64% vs 28%).
Conclusions: In 2017, 13 years after the ICMJE declared prospective protocol registration a necessity for reliable clinical studies, the frequency and quality of trial registration in the field of PONV is very poor. Selective outcome reporting reduces trustworthiness in findings of clinical trials. Investigators and clinicians should be aware that only following a properly registered protocol and transparently reporting of predefined outcomes, regardless of the direction and significance of the result, will ultimately strengthen the body of evidence in the field of PONV research in the future.
The blood–nerve barrier and myelin barrier normally shield peripheral nerves from potentially harmful insults. They are broken down during nerve injury, which contributes to neuronal damage. Netrin-1 is a neuronal guidance protein with various established functions in the peripheral and central nervous systems; however, its role in regulating barrier integrity and pain processing after nerve injury is poorly understood. Here, we show that chronic constriction injury (CCI) in Wistar rats reduced netrin-1 protein and the netrin-1 receptor neogenin-1 (Neo1) in the sciatic nerve. Replacement of netrin-1 via systemic or local administration of the recombinant protein rescued injury-induced nociceptive hypersensitivity. This was prevented by siRNA-mediated knockdown of Neo1 in the sciatic nerve. Mechanistically, netrin-1 restored endothelial and myelin, but not perineural, barrier function as measured by fluorescent dye or fibrinogen penetration. Netrin-1 also reversed the decline in the tight junction proteins claudin-5 and claudin-19 in the sciatic nerve caused by CCI. Our findings emphasize the role of the endothelial and myelin barriers in pain processing after nerve damage and reveal that exogenous netrin-1 restores their function to mitigate CCI-induced hypersensitivity via Neo1. The netrin-1-neogenin-1 signaling pathway may thus represent a multi-target barrier protector for the treatment of neuropathic pain.

Intensiv- und Beatmungskapazitäten sind für die Behandlung COVID-19-erkrankter Patienten essenziell. Unabhängig davon beanspruchen auch schwer verletzte Patienten häufig Intensiv- und Beatmungskapazitäten. Daraus ergibt sich folgende Fragestellung: Führt eine Ausgangsbeschränkung zu einer Reduktion schwer verletzter Patienten, und kann hierdurch mit frei werdenden Intensivkapazitäten gerechnet werden?
Material und Methoden
Es erfolgte eine retrospektive Auswertung schwer verletzter Patienten mit einem Injury Severity Score (ISS) ≥16 zwischen dem 17.03.2020 und 30.04.2020 (landesweiter Shutdown) an einem überregionalen Traumazentrum. Erfasst wurden der Unfallmechanismus, ISS, Versicherungsträger (BG vs. GKV/PKV), ob es sich um einen dokumentierten Suizidversuch handelte, und ob eine operative Intervention innerhalb der ersten 24 h erforderlich war. Als Kontrollgruppe wurden die Daten des gleichen Zeitraums der Jahre 2018 und 2019 ausgewertet.
Ergebnisse
Es konnte keine wesentliche Veränderung bezüglich der Anzahl an schwer verletzten Patienten festgestellt werden (2018 n = 30, 2019 n = 23, 2020 n = 27). Es zeigten sich insgesamt keine deutlichen Veränderungen der absoluten Zahlen bezüglich der Intensivpflichtigkeit in den ersten 24 h und der Beatmungspflichtigkeit beim Verlassen des Schockraums. Die Anzahl an Patienten, die eine Operation innerhalb der ersten 24 h nach Eintreffen im Schockraum benötigten, war 2020 sogar leicht erhöht, jedoch nicht statistisch signifikant. Der durchschnittliche ISS blieb konstant. Bezüglich der Unfallursache zeigte sich 2020 kein Motorradfahrer, der einen nicht berufsgenossenschaftlich versicherten Unfall erlitt (2018 n = 5, 2019 n = 4, 2020 n = 0). Es wurde 2020 ein erhöhter Anteil an Arbeitsunfällen mit einem ISS ≥16 festgestellt (2018: 10 %, 2019: 26,1 %, 2020: 44,4 %).
Diskussion
Eine Ausgangsbeschränkung führte zu keiner Reduktion verletzter- und intensivpflichtiger Patienten am untersuchten Zentrum. Auch unter einer landesweiten Ausgangsbeschränkung muss für dieses Patientenkollektiv eine ausreichende Menge an Intensiv- und OP-Kapazitäten vorgehalten werden. Die Bestätigung dieser Ergebnisse durch Auswertung nationaler Register steht noch aus.
Diabetic polyneuropathy (DPN) is the most common complication in diabetes and can be painful in up to 26% of all diabetic patients. Peripheral nerves are shielded by the blood-nerve barrier (BNB) consisting of the perineurium and endoneurial vessels. So far, there are conflicting results regarding the role and function of the BNB in the pathophysiology of DPN. In this study, we analyzed the spatiotemporal tight junction protein profile, barrier permeability, and vessel-associated macrophages in Wistar rats with streptozotocin-induced DPN. In these rats, mechanical hypersensitivity developed after 2 weeks and loss of motor function after 8 weeks, while the BNB and the blood-DRG barrier were leakier for small, but not for large molecules after 8 weeks only. The blood-spinal cord barrier remained sealed throughout the observation period. No gross changes in tight junction protein or cytokine expression were observed in all barriers to blood. However, expression of Cldn1 mRNA in perineurium was specifically downregulated in conjunction with weaker vessel-associated macrophage shielding of the BNB. Our results underline the role of specific tight junction proteins and BNB breakdown in DPN maintenance and differentiate DPN from traumatic nerve injury. Targeting claudins and sealing the BNB could stabilize pain and prevent further nerve damage.
