Background
Measles virus (MV) causes T cell suppression by interference with phosphatidylinositol-3-kinase (PI3K) activation. We previously found that this interference affected the activity of splice regulatory proteins and a T cell inhibitory protein isoform was produced from an alternatively spliced pre-mRNA.
Hypothesis
Differentially regulated and alternatively splice variant transcripts accumulating in response to PI3K abrogation in T cells potentially encode proteins involved in T cell silencing.
Methods
To test this hypothesis at the cellular level, we performed a Human Exon 1.0 ST Array on RNAs isolated from T cells stimulated only or stimulated after PI3K inhibition. We developed a simple algorithm based on a splicing index to detect genes that undergo alternative splicing (AS) or are differentially regulated (RG) upon T cell suppression.
Results
Applying our algorithm to the data, 9% of the genes were assigned as AS, while only 3% were attributed to RG. Though there are overlaps, AS and RG genes differed with regard to functional regulation, and were found to be enriched in different functional groups. AS genes targeted extracellular matrix (ECM)-receptor interaction and focal adhesion pathways, while RG genes were mainly enriched in cytokine-receptor interaction and Jak-STAT. When combined, AS/RG dependent alterations targeted pathways essential for T cell receptor signaling, cytoskeletal dynamics and cell cycle entry.
Conclusions
PI3K abrogation interferes with key T cell activation processes through both differential expression and alternative splicing, which together actively contribute to T cell suppression.
Knowledge of local bone quality is essential for surgeons to determine operation techniques. A device for intraoperative measurement of local bone quality has been developed by the AO-Research Foundation (DensiProbe®). We used this device to experimentally measure peak breakaway torque of trabecular bone in the proximal femur and correlated this with local bone mineral density (BMD) and failure load. Bone mineral density of 160 cadaver femurs was measured by ex situ dual-energy X-ray absorptiometry. The failure load of all femurs was analyzed by side-impact analysis. Femur fractures were fixed and mechanical peak torque was measured with the DensiProbe® device. Correlation was calculated whereas correlation coefficient and significance was calculated by Fisher’s Z-transformation. Moreover, linear regression analysis was carried out. The unpaired Student’s t-test was used to assess the significance of differences. The Ward triangle region had the lowest BMD with 0.511 g/cm2 (±0.17 g/cm2), followed by the upper neck region with 0.546 g/cm2 (±0.16 g/cm2), trochanteric region with 0.685 g/cm2 (±0.19 g/cm2) and the femoral neck with 0.813 g/cm2 (±0.2 g/cm2). Peak torque of DensiProbe® in the femoral head was 3.48 Nm (±2.34 Nm). Load to failure was 4050.2 N (±1586.7 N). The highest correlation of peak torque measured by Densi Probe® and load to failure was found in the femoral neck (r=0.64, P<0.001). The overall correlation of mechanical peak torque with T-score was r=0.60 (P<0.001). A correlation was found between mechanical peak torque, load to failure of bone and BMD in vitro. Trabecular strength of bone and bone mineral density are different aspects of bone strength, but a correlation was found between them. Mechanical peak torque as measured may contribute additional information about bone strength, especially in the perioperative testing.
Background: Revision in failed shoulder arthroplasty often requires removal of the humeral component with a significant risk of fracture and bone loss. Newer modular systems allow conversion from anatomic to reverse shoulder arthroplasty with retention of a well-fixed humeral stem. We report on a prospectively evaluated series of conversions from hemiarthroplasty to reverse shoulder arthroplasty.
Methods: In 14 cases of failed hemiarthroplasty due to rotator cuff deficiency and painful pseudoparalysis (in 13 women), revision to reverse shoulder arthroplasty was performed between October 2006 and 2010, with retention of the humeral component using modular systems. Mean age at the time of operation was 70 (56-80) years. Pre- and postoperative evaluation followed a standardized protocol including Constant score, range of motion, and radiographic analysis. Mean follow-up time was 2.5 (2-5.5) years.
Results: Mean Constant score improved from 9 (2-16) to 41 (17-74) points. Mean lengthening of the arm was 2.6 (0.9-4.7) cm without any neurological complications. One patient required revision due to infection. Interpretation Modular systems allow retainment of a well-fixed humeral stem with good outcome. There is a risk of excessive humeral lengthening.
Häufig stellt die frühe Phase der innerklinischen Traumaversorgung bei Polytraumapatienten besonders die Schädel-Hirn-Verletzungen betreffend einen sehr dynamischen Zeitraum dar, weshalb zeitlich getriggerte Kontrolluntersuchungen weit verbreitet sind. Andererseits bedeutet der Transport zur Untersuchung für beatmete Patienten eine zusätzliche Gefahr. Untersucht wurde, wie häufig sich in einer frühen Kontroll-Computertomografie des Schädels neue oder verschlechterte Befunde finden und, wie diese neuen Informationen die Intensivtherapie beeinflussen. Methode: Retrospektive Analyse aus Befunddaten und Krankenakten. Eingeschlossen wurden 150 beatmete Patienten mit SHT oder Verdacht auf ein SHT, die eine frühe CCT-Kontrolle bekamen. Ergebnisse: Eine Verschlechterung der Befunde wurde bei 63 Patienten (42%) und neue Befunde bei 18 Patienten (12%) gefunden. Bei 47 Pateinten (31%) wurde die Intensivtherapie aufgrund der frühen Kontroll-CCT geändert. Schlussfolgerung: Bei beatmeten Polytraumapatienten treten in mehr als der Hälfte der Fälle Verschlechterungen oder neue intracranielle Befunde auf. Eine frühe CCT-Kontrolle kann diese erkennen und zu einer rechtzeitigen Therapieoptimierung führen.
Die Phimose ist eine Verengung der Vorhaut, die durch das Unvermögen der Retraktion des Präputiums über die Glans penis gekennzeichnet ist. Da dies zu Komplikationen wie Harnwegsobstruktion, Balanoposthitis und Paraphimose führen kann, ist oftmals eine Therapie indiziert. Diesbezüglich zeigte sich in den letzten Jahren ein Wandel im Therapieregime. Die früher routinemäßig durchgeführte Zirkumzision wurde von der konservativen Therapie mittels topischer Steroidapplikation abgelöst.
Anhand einer retrospektiven Datenanalyse wurden die Therapien, der im Klinikum der Julius- Maximilians- Universität Würzburg als behandlungsbedürftig diagnostizierten Phimosen im Zeitraum vom 01.01.2005 bis 31.12.2010 hinsichtlich der Therapiewahl sowie der jeweiligen Erfolgs-und Komplikationsraten eruiert. Desweiteren wurden Vorhäute von 36 Patienten verblindet lichtmikroskopisch untersucht, um Hinweise auf die Ursachen für ein Versagen der konservativen Therapie mit Betnesol- V® Salbe 0,1% zu finden.
Ab 2009 entwickelte sich der Trend dahingehend, dass die konservative Therapie immer öfter und ab 2010 erstmals auch absolut gesehen häufiger als die operative Therapie durchgeführt wurde. Während die Erfolgsrate der Zirkumzision mit 99% hoch ist, zeigte sich die topische Steroidapplikation bei nur 70% als erfolgreich. Jedoch sind mit der Zirkumzision mehr Nebenwirkungen und Risiken wie Blutungen und Entzündungen sowie das operative Risiko verbunden. In Würzburg wurde kein Patient mit unerwünschten Arzneimittelwirkung der Steroide auffällig. Bei der lichtmikroskopischen Untersuchung stellte sich kein weg-weisender Unterschied zwischen den Vorhäuten der verschiedenen Patientengruppen dar.
Zusammenfassend lässt sich sagen, dass die topische Steroidapplikation, soweit keine Kontraindikationen wie z.B. eine Balanitis xerotica obliterans , eine Paraphimose oder Vernarbungen vorliegen, vor Indikationsstellung zur Operation, angeboten werden sollte, da somit 70% der Operationen vermieden werden können.
