Introduction:
Evidence from a number of open-label, uncontrolled studies has suggested that rituximab may benefit patients with autoimmune diseases who are refractory to standard-of-care. The objective of this study was to evaluate the safety and clinical outcomes of rituximab in several standard-of-care-refractory autoimmune diseases (within rheumatology, nephrology, dermatology and neurology) other than rheumatoid arthritis or non-Hodgkin’s lymphoma in a real-life clinical setting.
Methods:
Patients who received rituximab having shown an inadequate response to standard-of-care had their safety and clinical outcomes data retrospectively analysed as part of the German Registry of Autoimmune Diseases. The main outcome measures were safety and clinical response, as judged at the discretion of the investigators.
Results:
A total of 370 patients (299 patient-years) with various autoimmune diseases (23.0% with systemic lupus erythematosus, 15.7% antineutrophil cytoplasmic antibody-associated granulomatous vasculitides, 15.1% multiple sclerosis and 10.0% pemphigus) from 42 centres received a mean dose of 2,440 mg of rituximab over a median (range) of 194 (180 to 1,407) days. The overall rate of serious infections was 5.3 per 100 patient-years during rituximab therapy. Opportunistic infections were infrequent across the whole study population, and mostly occurred in patients with systemic lupus erythematosus. There were 11 deaths (3.0% of patients) after rituximab treatment (mean 11.6 months after first infusion, range 0.8 to 31.3 months), with most of the deaths caused by infections. Overall (n = 293), 13.3% of patients showed no response, 45.1% showed a partial response and 41.6% showed a complete response. Responses were also reflected by reduced use of glucocorticoids and various immunosuppressives during rituximab therapy and follow-up compared with before rituximab. Rituximab generally had a positive effect on patient well-being (physician’s visual analogue scale; mean improvement from baseline of 12.1 mm)
Testing based on multiple choice questions (MCQ) is one of the most established forms of assessment, not only in the medical field. Extended matching questions (EMQ) represent a specific type of MCQ designed to require higher levels of cognition, such as problem-solving. The purpose of this evaluation was to assess the suitability and efficiency of EMQ as an assessment method. EMQ were incorporated into the end-of-semester examination in internal medicine, in which 154 students participated, and compared with three established MCQ types. Item and examination quality were investigated, as well as readability and processing time. EMQ were slightly more difficult to score; however, both item discrimination and discrimination index were higher when compared to other item types. EMQ were found to be significantly longer and required more processing time, but readability was improved. Students judged EMQ as clearly challenging, but attributed significantly higher clinical relevance when compared to established MCQ formats. Using the Spearman-Brown prediction, only ten EMQ items would be needed to reproduce the Cronbach’s alpha value of 0.75 attained for the overall examination. EMQ proved to be both efficient and suitable when assessing medical students, demonstrating powerful characteristics of reliability. Their expanded use in favor of common MCQ could save examination time without losing out on statistical quality.
Die Blut-Hirn-Schranke (BHS) bildet eine Barriere, die das ZNS vor dem Einfluss der Substanzen aus der Peripherie schützt. Die Aufrechterhaltung dieser wichtigen Struktur ist für die Homöostase und Vermeidung von Schäden im ZNS von besonderer Bedeutung. Eine häufige Ursache für die Schädigung der BHS ist der ischämische Schlaganfall. In der Hypoxie werden zahlreiche Proteine degradiert oder von den Zellkontakten delokalisiert, die für die Integrität der Zell-Zell-Kontakte eine wichtige Rolle spielen. Als Folge resultiert eine Destabilisierung der BHS mit vermehrtem Durchstrom von ZNS-schädigenden Substanzen.
Seit der Entdeckung von miRNAs (Lee et al., 1993) konnte deren Rolle vor allem in der Entstehung von Tumoren und Entzündungen nachgewiesen werden. In der Arbeitsgruppe Förster wurde eine vermehrte Expression von miRNA-132 und miRNA-212 nach OGD festgestellt. Als Zielgene von miRNA-132/212 konnten Cldn1, Tjap1, MMP9 und Jam3 detektiert werden. Die Validierungsversuche wurden an einem etablierten In-vitro-Modell der BHS bestehend aus murinen cerebralen Hirnendothelzellen (cEND-2) durchgeführt.
Zu Beginn wurden die Bedingungen zur Kultivierung der Zellen unter Hypoxie und ohne Glucose (eng. oxygen glucose deprivation, OGD) mit und ohne Astrozyten-konditioniertem Medium etabliert. Expression von diversen Genen, die bekannt sind eine Rolle in der OGD zu spielen, wurden mittels qPCR ermittelt. Dabei kam es zu signifikanten Veränderungen der Genexpression von wichtigen Genen in der BHS, die besonders in Kombination mit Astrozyten-konditioniertem Medium noch verstärkt werden konnten. So wurde die Kultivierung der Zellen für die weiteren OGD-Versuche in Astrozyten-konditioniertem Medium gewählt.
Des Weiteren wurde die Regulation von miRNA-132/212 auf die Zielgene an der BHS untersucht. Die hemmende Wirkung der miRNAs auf die Genexpression der Zielgene konnte durch Transfektion von pre-miR-132/212 sowohl auf mRNA- als auch auf Proteinebene gezeigt werden. In der OGD konnte durch Hemmung der vermehrt exprimierten miRNA-132/212 ein Anstieg der Zielgene beobachtet werden. Damit konnten sowohl auf mRNA-Ebene als auch auf Proteinebenen Cldn1, Tjap1, Jam3 und MMP9 als Zielgene bestätigt werden. Zusätzlich wurden als funktionelle Tests eine TEER-Messung für den Widerstand und eine Permeabilitätsmessung durchgeführt. Beide Tests bestätigten die Destabilisierung der BHS nach Transfektion von miRNA-132/212.
Als therapeutische Implikation könnte durch Inhibition von miRNA-132/212 bei einer Minderperfusion des Gehirns die Barriere stabilisiert werden, was eine neue Möglichkeit in der Therapie des akuten Schlaganfalls bietet.
