Background and purpose
The effects of the coronavirus disease 2019 (COVID-19) pandemic on telemedical care have not been described on a national level. Thus, we investigated the medical stroke treatment situation before, during, and after the first lockdown in Germany.
Methods
In this nationwide, multicenter study, data from 14 telemedical networks including 31 network centers and 155 spoke hospitals covering large parts of Germany were analyzed regarding patients' characteristics, stroke type/severity, and acute stroke treatment. A survey focusing on potential shortcomings of in-hospital and (telemedical) stroke care during the pandemic was conducted.
Results
Between January 2018 and June 2020, 67,033 telemedical consultations and 38,895 telemedical stroke consultations were conducted. A significant decline of telemedical (p < 0.001) and telemedical stroke consultations (p < 0.001) during the lockdown in March/April 2020 and a reciprocal increase after relaxation of COVID-19 measures in May/June 2020 were observed. Compared to 2018–2019, neither stroke patients' age (p = 0.38), gender (p = 0.44), nor severity of ischemic stroke (p = 0.32) differed in March/April 2020. Whereas the proportion of ischemic stroke patients for whom endovascular treatment (14.3% vs. 14.6%; p = 0.85) was recommended remained stable, there was a nonsignificant trend toward a lower proportion of recommendation of intravenous thrombolysis during the lockdown (19.0% vs. 22.1%; p = 0.052). Despite the majority of participating network centers treating patients with COVID-19, there were no relevant shortcomings reported regarding in-hospital stroke treatment or telemedical stroke care.
Conclusions
Telemedical stroke care in Germany was able to provide full service despite the COVID-19 pandemic, but telemedical consultations declined abruptly during the lockdown period and normalized after relaxation of COVID-19 measures in Germany.
Die Aufgabe der vorliegenden Arbeit war die Beantwortung der Frage, ob die Verständlichkeit von Sprache, unter experimentellen Bedingungen, mit digitalen Hörhilfen, mit und ohne Mikrofonrichtcharakteristik, differiert. Um möglichst realistische Testbedingungen zu schaffen wurden bei der Untersuchung Umweltgeräusche durch einen Störlärm-Rauschpegel imitiert. Messungen bei 60 und 80 dB wurden miteinander verglichen. Als Nutzsignal diente der HSM-Test, bei dem einfache kurze Sätze zum Einsatz kommen. Nach den Kriterien der Realitätsnähe, Validität und Standartisierbarkeit erschien dieser Test besonders geeignet. Die Messungen fanden in einer schalldichten Kabine der HNO-Klinik Würzburg statt. Den Probanden wurden DAT-Cassetten über Kopfhörer vorgespielt, die vorher in der Camera Silens, im Freifeld, unter Mithilfe des Kunstkopfes von B&K aufgenommen wurden. Dieser Kunstkopf ermöglichte die Aufnahme mit dem Dazwischenschalten unterschiedlicher Hörgeräte und Hörgeräteinstellungen, die das normalhörende Probandenkollektiv in die Situation eines Hörbehinderten versetzten. Nach der Prüfung auf Normalverteilung wurden in der Auswertung die Sprachverständlichkeitsschwellen verglichen, zeichnerisch gegenübergestellt und entsprechend diskutiert. Diese Schwelle entspricht dem Signal-Störlärmabstand bei dem genau 50 % aller Testworte verstanden wurden. Schließlich zeigte sich von der Einstellung „Niere“ zur Einstellung „Kugel“ eine geringe Verbesserung der Wortverständlichkeit. Die Resultate der leichten Verständlichkeitsverbesserung mit Richtmikrofon fielen bei 80 dB deutlicher aus als bei 60 dB. Diese Verbesserungen sind rechnerisch nicht signifikant.
Developmentally regulated features of innate immunity are thought to place preterm and term infants at risk of infection and inflammation-related morbidity. Underlying mechanisms are incompletely understood. Differences in monocyte function including toll-like receptor (TLR) expression and signaling have been discussed. Some studies point to generally impaired TLR signaling, others to differences in individual pathways. In the present study, we assessed mRNA and protein expression of pro- and anti-inflammatory cytokines in preterm and term cord blood (CB) monocytes compared with adult controls stimulated ex vivo with Pam3CSK4, zymosan, polyinosinic:polycytidylic acid, lipopolysaccharide, flagellin, and CpG oligonucleotide, which activate the TLR1/2, TLR2/6, TLR3, TLR4, TLR5, and TLR9 pathways, respectively. In parallel, frequencies of monocyte subsets, stimulus-driven TLR expression, and phosphorylation of TLR-associated signaling molecules were analyzed. Independent of stimulus, pro-inflammatory responses of term CB monocytes equaled adult controls. The same held true for preterm CB monocytes—except for lower IL-1β levels. In contrast, CB monocytes released lower amounts of anti-inflammatory IL-10 and IL-1ra, resulting in higher ratios of pro-inflammatory to anti-inflammatory cytokines. Phosphorylation of p65, p38, and ERK1/2 correlated with adult controls. However, stimulated CB samples stood out with higher frequencies of intermediate monocytes (CD14\(^+\)CD16\(^+\)). Both pro-inflammatory net effect and expansion of the intermediate subset were most pronounced upon stimulation with Pam3CSK4 (TLR1/2), zymosan (TR2/6), and lipopolysaccharide (TLR4). Our data demonstrate robust pro-inflammatory and yet attenuated anti-inflammatory responses in preterm and term CB monocytes, along with imbalanced cytokine ratios. Intermediate monocytes, a subset ascribed pro-inflammatory features, might participate in this inflammatory state.
