Objectives
Parapneumonic pleural effusions/empyema (PPE/PE) are severe complications of community-acquired pneumonia. We investigated the bacterial aetiology and incidence of paediatric PPE/PE in Germany after the introduction of universal pneumococcal conjugate vaccine (PCV) immunization for infants.
Methods
Children <18 years of age hospitalized with pneumonia-associated PPE/PE necessitating pleural drainage or persisting >7 days were reported to the German Surveillance Unit for Rare Diseases in Childhood between October 2010 and June 2017. All bacteria detected in blood or pleural fluid (by culture/PCR) were included, with serotyping for Streptococcus pneumoniae.
Results
The median age of all 1447 PPE/PE patients was 5 years (interquartile range 3–10). In 488 of the 1447 children with PPE/PE (34%), 541 bacteria (>40 species) were detected. Aerobic gram-positive cocci accounted for 469 of 541 bacteria detected (87%); these were most frequently Streptococcus pneumoniae (41%), Streptococcus pyogenes (19%) and Staphylococcus aureus (6%). Serotype 3 accounted for 45% of 78 serotyped S. pneumoniae strains. Annual PPE/PE incidence varied between 14 (95%CI 12–16) and 18 (95%CI 16–21) PPE/PE per million children. Incidence of S. pneumoniae PPE/PE decreased from 3.5 (95%CI 2.5–4.6) per million children in 2010/11 to 1.5 (95%CI 0.9–2.4) in 2013/14 (p 0.002), followed by a re-increase to 2.2 (95%CI 1.5–3.2) by 2016/17 (p 0.205).
Conclusions
In the era of widespread PCV immunization, cases of paediatric PPE/PE were still caused mainly by S. pneumoniae and, increasingly, by S. pyogenes. The re-increase in the incidence of PPE/PE overall and in S. pneumoniae-associated PPE/PE indicates ongoing changes in the bacterial aetiology and requires further surveillance.
In der Pathogenese der Psoriasis spielen IL 17 und die Plastizität von Tregs zu Th17 Zellen mit Produktion proinflammatorischer Zytokine sowie die möglicherweise reduzierte suppressive Funktion von Tregs eine entscheidende Rolle. Wir versuchten daher in unserer Arbeit einen Überblick über die T Zellverteilung im peripherem Blut bei PSO und HC zu erhalten und die Reaktion der Zellen auf IL 17, anti IL 17 und Secukinumab sowie ein Th 17 induzierendes Milieu im Vergleich von PSO und HC zu evaluieren.
In der Analyse der PBMCs von PSO und HC konnten bei PSO tendenziell weniger inflammatorische Marker, wahrscheinlich aufgrund der niedrigen Krankheitsaktivität und der bereits eingeleiteten medikamentösen Therapie festgestellt werden.
Nach Isolierung der Tregs und Kultivierung konnten bei PSO im Vergleich zu HC erhöhte inflammatorische Marker nachgewiesen werden. Dies kann an der höheren Plastizität von Tregs bei PSO ex vivo ohne den Einfluss einer medikamentösen Therapie hin zu inflammatorischen Zellen.
In den Suppressionsversuchen zeigte sich sowohl bei PSO als auch bei HC unter Th17 Milieu eine verminderte Inhibition der PBMCs durch die autologen Tregs.
Ursächlich hierfür könnte eine Dysregulation der Tregs durch das Th17 Milieu oder eine Auswirkung des Th17-induzierenden Cocktails auf die PBMCs im Sinne einer Effektorresistenz gegenüber den Tregs sein.
Eine Veränderung der Suppression ergab sich für IL 17 oder anti IL 17 nicht. Unter der gleichzeitigen Kultivierung mit Secukinumab und einem Th17 induzierendem Cocktail konnte keine verbesserte Inhibition festgestellt werden.
Insgesamt bestätigt die Arbeit eine Instabilität der Tregs bei PSO mit der Möglichkeit der Plastizität zu Th17 Zellen unter proinflammatorischem Milieu, sowie einen Verlust der Suppressionsfähigkeit durch eine Treg Dysfunktion oder eine erhöhte Effektorresistenz. Für IL 17 oder die Blockade von IL 17 durch monoklonale Antikörper konnte in unserer Studie kein Einfluss festgestellt werden.
The regulation of immune cell migration throughout the body is essential to warrant immunosurveillance and to maintain immune homeostasis. Marking and tracking of these cells has proven important to study mechanisms of immune cell trafficking and cell interaction in vivo. Photoconversion is a well-suited technique for intravital application because it enables contactless time- and location-specific marking of cells in the tissue without surgically manipulating the microenvironment of the cells in question. However, in dividing cells the converted fluorescent protein may decline quickly. Here, we provide a detailed description of the photoconversion technique and its applicability to tracking highly proliferating T cells from the priming site of T cell activation to peripheral target organs of effector function in a preclinical model. Dendra2+ T cells were photoconverted in the Peyer’s patches during the initiation phase of acute graft-versus-host disease (GvHD) and tracked through the mesenteric lymph nodes and the peripheral blood to the small intestine with flow cytometry and intravital two-photon microscopy. Photoconverted alloreactive T cells preserved the full proliferative capacity, homing, and migration of alloreactive T cells in the intestinal lamina propria. We conclusively proved that photoconversion of highly proliferative alloreactive T cells in the Peyer’s patches is an effective tool to study trafficking of alloreactive T cells under physiologic conditions and to GvHD target tissues. This technique can also be applied to the study of immune cell tracking under inflammatory and non-inflammatory conditions.
In dieser Arbeit wurde der Impfstatus der Würzburger Medizinstudierenden von 2004-2020 entsprechend der jeweils im sechsten Semester geltenden STIKO Empfehlungen ausgewertet (im folgenden Impfquote genannt) und mit den Ergebnissen von Studien an Universitäten in Frankfurt, Bochum und Dresden, sowie der Allgemeinbevölkerung und dem medizinischen Personal in Deutschland verglichen. Außerdem wurde ausgewertet, inwiefern das Angebot der Nachimpfungen im Impfkurs angenommen wurde und mögliche Zusammenhänge mit aufgedeckten Impflücken wurden diskutiert.
Bei manchen impfpräventablen Infektionskrankheiten (IPIE) wie Pertussis war von 2004-2020 ein deutlicher Anstieg der Impfquote (von <2% auf knapp 90%) zu beobachten, bei anderen, wie Tetanus war bereits seit 2004 eine Impfquote von etwa 75-90% zu sehen, der über die gesamte Beobachtungszeit auf etwa 85-90% anstieg.
Im Vergleich zu anderen Studien mit Medizinstudierenden anderer Universitäten in Deutschland schnitten die Würzburger Medizinstudierenden in Bezug auf Masern, Mumps, Röteln und Varizellen mit Impfquoten um die 80-90% oder höher im Vergleich zu 73-86% in den anderen Städten durchweg besser ab. Bei Hepatitis B war anfangs eine vergleichbare (65-90%), später eine höhere Impfquote (um die 80%) als in den Vergleichsstudien (um die 40%) zu beobachten. In Bezug auf Tetanus (Impfquote im Schnitt 85,2%), Diphtherie (Impfquote im Schnitt 82,9%), Pertussis (Impfquote im Schnitt 49,3%) und Influenza (Impfung in Vorsaison im Schnitt bei 29,3%) waren die Daten aus Würzburg gut mit den Daten aus Vorstudien in ähnlichen Zeiträumen vergleichbar.
Im Vergleich zu Daten zur Impfquote bei Meningokokken und HPV aus der Allgemeinbevölkerung lagen die Würzburger Medizinstudierenden von 2017-2020 über den dort verzeichneten Werten (48% zu 29% bzw. 63% zu 53). Im Vergleich zu Daten der Impfsurveillance des RKI aus 2020 zeigte sich der Effekt der Impfempfehlung bei Kindern (Meningokokken: 90% der 4-7 Jährigen, HPV: 63,3% der 14 Jährigen). Bei der Pneumokokken Impfung gaben – obwohl die STIKO Empfehlung nicht auf medizinisches Personal zutrifft – 10,8% der Studierenden an, mindestens einmal geimpft zu sein. Dies könnte ein erhöhtes Gesundheitsbewusstsein der Medizinstudierenden widerspiegeln.
Zusammenfassend kann gesagt werden, dass der Anteil der Würzburger Medizinstudierenden, deren Impfstatus für die einzelnen IPIE den STIKO Empfehlungen für medizinisches Personal entsprach, über die Jahre 2004 bis 2020 angestiegen ist. Zum Großteil lag der Anteil der Studierenden mit Impfstatus entsprechend den STIKO Empfehlungen über dem aus den Studien der anderen Universitäten.
Trotzdem blieben noch deutliche Lücken im Impfstatus, bspw. bei Pertussis oder Masern, und Wissen der Würzburger Medizinstudierenden bestehen. Diese Lücken werden sich auf Dauer in die Ärzteschaft und schließlich auch in die Empfehlungen durch das ärztliche Personal fortsetzen. Deshalb sollte ein besonderer Fokus auf die Verbesserung des Impfstatus Medizinstudierender gelegt werden, beispielsweise durch regelmäßige verpflichtende Kontrollen durch Betriebärzt*in, intensivierte Lehre sowie bessere Aufklärung bereits zu Beginn des Studiums.
Das Format des Impfkurses, wie er in Würzburg durchgeführt wird, scheint ein gut gewähltes Format, um den Studierenden die Möglichkeit zu geben, ihren eigenen Impfstatus zu überprüfen und diesen weiter zu verbessern. Die Impfquote der Studierenden lag in den Jahren 2014 bis 2020 – den Jahren, in denen die Nachimpfungen im Kurs erfasst wurden – im Schnitt nach dem Kurs bei fast allen IPIE über 90%. Nur bei Pertussis lag die Impfquote nach dem Kurs bei 83,4% (vgl. vor dem Kurs 68,4%). Durch nationale Vereinheitlichung der Lehre im NKLM zum Thema Impfen, frühe Auseinandersetzung mit dem Thema und regelmäßige Überprüfung des eigenen Impfstatus sowie niederschwellige Impfangebote im Medizinstudium, kann einerseits eine Verbesserung des Impfstatus von Medizinstudierenden erreicht werden. Andererseits können so auch insgesamt bessere Impfquoten in der Bevölkerung durch die verbesserte Ausbildung von Ärztinnen bereits im Medizinstudium erzielt werden.
Introduction: The German PID-NET registry was founded in 2009, serving as the first national registry of patients with primary immunodeficiencies (PID) in Germany. It is part of the European Society for Immunodeficiencies (ESID) registry. The primary purpose of the registry is to gather data on the epidemiology, diagnostic delay, diagnosis, and treatment of PIDs.
Methods: Clinical and laboratory data was collected from 2,453 patients from 36 German PID centres in an online registry. Data was analysed with the software Stata® and Excel.
Results: The minimum prevalence of PID in Germany is 2.72 per 100,000 inhabitants. Among patients aged 1-25, there was a clear predominance of males. The median age of living patients ranged between 7 and 40 years, depending on the respective PID. Predominantly antibody disorders were the most prevalent group with 57% of all 2,453 PID patients (including 728 CVID patients). A gene defect was identified in 36% of patients. Familial cases were observed in 21% of patients. The age of onset for presenting symptoms ranged from birth to late adulthood (range 0-88 years). Presenting symptoms comprised infections (74%) and immune dysregulation (22%). Ninety-three patients were diagnosed without prior clinical symptoms. Regarding the general and clinical diagnostic delay, no PID had undergone a slight decrease within the last decade. However, both, SCID and hyper IgE-syndrome showed a substantial improvement in shortening the time between onset of symptoms and genetic diagnosis. Regarding treatment, 49% of all patients received immunoglobulin G (IgG) substitution (70%-subcutaneous; 29%-intravenous; 1%-unknown). Three-hundred patients underwent at least one hematopoietic stem cell transplantation (HSCT). Five patients had gene therapy.
Conclusion: The German PID-NET registry is a precious tool for physicians, researchers, the pharmaceutical industry, politicians, and ultimately the patients, for whom the outcomes will eventually lead to a more timely diagnosis and better treatment.
Purpose
Biallelic pathogenic NBAS variants manifest as a multisystem disorder with heterogeneous clinical phenotypes such as recurrent acute liver failure, growth retardation, and susceptibility to infections. This study explores how NBAS-associated disease affects cells of the innate and adaptive immune system.
Methods
Clinical and laboratory parameters were combined with functional multi-parametric immunophenotyping methods in fifteen NBAS-deficient patients to discover possible alterations in their immune system.
Results
Our study revealed reduced absolute numbers of mature CD56dim natural killer (NK) cells. Notably, the residual NK cell population in NBAS-deficient patients exerted a lower potential for activation and degranulation in response to K562 target cells, suggesting an NK cell–intrinsic role for NBAS in the release of cytotoxic granules. NBAS-deficient NK cell activation and degranulation was normalized upon pre-activation by IL-2 in vitro, suggesting that functional impairment was reversible. In addition, we observed a reduced number of naïve B cells in the peripheral blood associated with hypogammaglobulinemia.
Conclusion
In summary, we demonstrate that pathogenic biallelic variants in NBAS are associated with dysfunctional NK cells as well as impaired adaptive humoral immunity.
Innerhalb der Juvenilen Idiopathischen Arthritis (JIA) bilden Patienten mit Antinukleären Antikörpern (ANA) Subgruppen-übergreifend eine klinisch homogene Erkrankungsgruppe. Ob diesen klinischen Gemeinsamkeiten jedoch auch eine einheitliche Pathogenese zugrunde liegt, ist bisher unbekannt. Sogenannte periphere T-Helferzellen (TPH) spielen im Kontext zahlreicher Autoimmunerkrankungen eine entscheidende Rolle bei der Aktivierung autoreaktiver B-Zellen. Ziel dieser Arbeit war daher die phänotypische und funktionelle Analyse von PD-1hiCXCR5-CD4+ TPH-Zellen, sowie deren Verteilung in der Synovialflüssigkeit von Patienten unterschiedlicher Subgruppen der JIA.
Hierzu wurden Phänotyp und Zytokinprofil von PD-1hiCD4+ T-Zellen durchflusszytometrisch analysiert. Der funktionelle Einfluss von PD-1hiCD4+ T-Zellen auf die B-Zell-Differenzierung wurde mittels in vitro Kokulturen FACS-sortierter TPH-Zellen der Synovialflüssigkeit untersucht.
IL-21- und IL-17-produzierende T-Gedächtniszellen der Synovialflüssigkeit zeigten eine negative Korrelation zueinander. Die IL-21-Produktion ging besonders von PD-1hiCXCR5-HLA-DR+CD4+ T-Zellen aus, welche besonders in den Gelenken ANA-positiver JIA-Patienten akkumulierten. Diese Population zeigte phänotypische Ähnlichkeit mit TPH-Zellen und leistete in vitro effiziente B-Zell-Hilfe zu Plasmazelldifferenzierung und Immunglobulinsekretion, induzierte jedoch zudem einen CD21lo/-CD11c+T-bet+ Phänotyp in B-Zellen. Passend hierzu bestand auch ex vivo eine signifikante Korrelation zwischen TPH und CD21lo/-CD11c+T-bet+ doppelt-negativen B-Zellen (BDN).
Es konnte also die Expansion einer spezifischen T-Zellpopulation mit phänotypischen und funktionellen Charakteristika von TPH-Zellen beobachtet und deren funktioneller Zusammenhang mit CD21lo/-CD11c+T-bet+ BDN in der Synovialflüssigkeit von JIA-Patienten aufgezeigt werden. Dies könnte die Autoimmunantwort auf ubiquitäre Autoantigene innerhalb betroffener Gelenke ANA-positiver JIA-Patienten widerspiegeln.
B cell maturation and immunoglobulin (Ig) repertoire selection are governed by expression of a functional B cell receptor (BCR). Naïve B cells co-express their BCR as IgM and IgD isotype. However, the role of the additionally expressed IgD on naïve B cells is not known. Here we assessed the impact of IgD on naïve B cell maturation and Ig repertoire selection in 8 individuals from 3 different families with heterozygous loss-of-function or loss-of expression mutations in IGHD. Although naïve B cells from these individuals expressed IgM on their surface, the IGHD variant in heterozygous state entailed a chimeric situation by allelic exclusion with almost half of the naïve B cell population lacking surface IgD expression. Flow cytometric analyses revealed a distinct phenotype of IgD-negative naïve B cells with decreased expression of CD19, CD20 and CD21 as well as lower BAFF-R and integrin-β7 expression. IgD-negative B cells were less responsive in vitro after engaging the IgM-BCR, TLR7/9 or CD40 pathway. Additionally, a selective disadvantage of IgD-negative B cells within the T2 transitional and mature naïve B cell compartment as well as reduced frequencies of IgMlo/- B cells within the mature naïve B cell compartment lacking IgD were evident. RNA-Ig-seq of bulk sorted B cell populations showed an altered selection of distinct VH segments in the IgD-negative mature naïve B cell population. We conclude that IgD expression on human naïve B cells is redundant for generation of naïve B cells in general, but further shapes the naive B cell compartment starting from T2 transitional B cells. Our observations suggest an unexpected role of IgD expression to be critical for selection of distinct Ig VH segments into the pre-immune Ig repertoire and for the survival of IgMlo/- naïve B cells known to be enriched in poly-/autoreactive B cell clones.
Infections with influenza A viruses (IAV) cause seasonal epidemics and global pandemics. The majority of these infections remain asymptomatic, especially among children below five years of age. Importantly, this is a time, when immunological imprinting takes place. Whether early-life infections with IAV affect the development of antimicrobial immunity is unknown. Using a preclinical mouse model, we demonstrate here that silent neonatal influenza infections have a remote beneficial impact on the later control of systemic juvenile-onset and adult-onset infections with an unrelated pathogen, Staphylococcus aureus, due to improved pathogen clearance and clinical resolution. Strategic vaccination with a live attenuated IAV vaccine elicited a similar protection phenotype. Mechanistically, the IAV priming effect primarily targets antimicrobial functions of the developing innate immune system including increased antimicrobial plasma activity and enhanced phagocyte functions and antigen-presenting properties at mucosal sites. Our results suggest a long-term benefit from an exposure to IAV during the neonatal phase, which might be exploited by strategic vaccination against influenza early in life to enforce the host’s resistance to later bacterial infections.
Autophagy is an essential cellular homeostasis pathway initiated by multiple stimuli ranging from nutrient deprivation to viral infection, playing a key role in human health and disease. At present, a growing number of evidence suggests a role of autophagy as a primitive innate immune form of defense for eukaryotic cells, interacting with components of innate immune signaling pathways and regulating thymic selection, antigen presentation, cytokine production and T/NK cell homeostasis. In cancer, autophagy is intimately involved in the immunological control of tumor progression and response to therapy. However, very little is known about the role and impact of autophagy in T and NK cells, the main players in the active fight against infections and tumors. Important questions are emerging: what role does autophagy play on T/NK cells? Could its modulation lead to any advantages? Could specific targeting of autophagy on tumor cells (blocking) and T/NK cells (activation) be a new intervention strategy? In this review, we debate preclinical studies that have identified autophagy as a key regulator of immune responses by modulating the functions of different immune cells and discuss the redundancy or diversity among the subpopulations of both T and NK cells in physiologic context and in cancer.
Im sechsten Semester des Medizinstudiums an der Julius-Maximilians-Universität Würzburg findet das verpflichtende Praktikum „Impfkurs“ statt. Im Rahmen dieses Kurses wurde vom Sommersemester 2020 bis zum Sommersemester 2021 ein standardisierter online Fragebogen erhoben, der unter anderem demographische Daten sowie Expositionsmöglichkeiten gegenüber SARS-CoV-2 im privaten, beruflichen und universitären Umfeld erfragte. Zusätzlich wurde im gleichen Zeitraum der SARS-CoV-2 Serostatus der Medizinstudierenden erhoben und ausgewertet und dieser mit den Daten des Fragebogens zusammengeführt. Dafür wurden Blutproben entnommen, welche im Labor des Instituts für Virologie der Universität Würzburg mittels Western Blot auf IgG/IgM/IgA Antikörper gegen SARS-CoV-2 untersucht wurden.
Immune checkpoint blockade therapy is beneficial and even curative for some cancer patients. However, the majority don’t respond to immune therapy. Across different tumor types, pre-existing T cell infiltrates predict response to checkpoint-based immunotherapy. Based on in vitro pharmacological studies, mouse models and analyses of human melanoma patients, we show that the cytokine GDF-15 impairs LFA-1/β2-integrin-mediated adhesion of T cells to activated endothelial cells, which is a pre-requisite of T cell extravasation. In melanoma patients, GDF-15 serum levels strongly correlate with failure of PD-1-based immune checkpoint blockade therapy. Neutralization of GDF-15 improves both T cell trafficking and therapy efficiency in murine tumor models. Thus GDF-15, beside its known role in cancer-related anorexia and cachexia, emerges as a regulator of T cell extravasation into the tumor microenvironment, which provides an even stronger rationale for therapeutic anti-GDF-15 antibody development.
Expression immunmodulierende Marker in Zusammenhang mit Immuntherapie bei kindlichen Hirntumoren

Atypische teratoide Rhabdoidtumore sind trotz Ausschöpfen der multimodalen Therapieoptionen weiterhin mit einer schlechten Prognose belastet. Gründe hierfür liegen in den oftmals unzureichenden Resektionsmöglichkeiten, dem jungen Erkrankungsalter der PatientInnen und der Resistenz der Tumorzellen gegenüber Chemotherapeutika (Frühwald et al. 2020; Egiz et al. 2022; Richards et al. 2019). Gerade deshalb versucht man durch die aktuelle Forschung zu kindlichen Hirntumoren mit Immuntherapie ein besseres Outcome zu erreichen. Wichtige Grundlagen hierzu sind durch diese Arbeit dargestellt worden. Erstmals wurde gezeigt, dass Tumorzellen der AT/RT sowohl HLA-Klasse I und -Klasse II Antigene präsentieren. Es wurde außerdem die Expression von PD-L1 nachgewiesen. Des Weiteren konnte die Anwesenheit von Immunzellen durch den Nachweis CD 3+ Zellen bewiesen werden. Insgesamt zeigte sich eine große Heterogenität innerhalb des einzelnen und unter den verschiedenen Tumoren. Es zeigte sich eine negative Korrelation zwischen der Expression von MHC I und CD 3+ Zellen, welche insgesamt für einen Tumor Escape Mechanismus sprechen könnte, wie er bereits bei Glioblastomen nachgewiesen wurde (Bagley et al. 2018; Marcu et al. 2021). Es sollte eine Ausweitung der hier begonnen Forschung mit Einbeziehung der personenbezogenen Daten und Vergrößerung der untersuchten Fallzahl erfolgen.
Eine Dysbalance zwischen regulatorischen und proinflammatorischen T-Helferzellen kann zu Autoimmunerkrankungen führen. In dieser methodischen Arbeit wurde die Polarisierbarkeit von peripheren T-Lymphozyten durch verschiedene Zytokin-Stimuli untersucht. Hauptziel war es, CD4+CD25-CD127- Lymphozyten durch Stimulation mit einem IL-2 und TGFβ-beinhaltenden Zytokin-Cocktail (Treg-Cocktail) zu iTregs zu polarisieren und deren Suppressionsfunktion auf autologe Effektor-Leukozyten zu untersuchen.
Es erfolgte eine Phänotypisierung der PBMCs gesunder Probanden, insbesondere im Hinblick auf die Verteilung der T-Lymphozyten-Subpopulationen, deren Zytokinproduktion und FoxP3-Expression. Zudem wurden aus den PBMCs der Probanden Tregs (CD4+CD25+CD127low/-) sowie CD4+CD25-CD127- Zellen isoliert und deren Funktionsfähigkeit durch die Untersuchung ihrer Suppressionsfunktion auf autologe Effektor-Lymphozyten analysiert. Die Zellen wurden mittels verschiedener Zytokin-Cocktails in Richtung Treg sowie in Richtung Th17-Zellen polarisiert; anschließend wurde die Funktionsfähigkeit der polarisierten Zellen in Suppression-Assays gemessen.
Wir konnten zeigen, dass die CD4+CD25+CD127low/- Zellen Tregs mit der Fähigkeit zur Suppression der Proliferation autologer Effektor-Lymphozyten waren. Bei den CD4+CD25-CD127-Zellen handelte es sich um T-Lymphozyten ohne Suppressionsfunktion. Nach Stimulation der CD4+CD25-CD127-Zellen mit dem Treg-Cocktail zeigten die Zellen eine mit den Tregs vergleichbare Suppressionsfunktion.
Mit dieser Studie haben wir eine aktuelle methodische Quelle für die Untersuchung von Phänotyp und Funktion regulatorischer T-Zellen sowie für die Stimulation peripherer T-Lymphozyten hin zu Tregs geschaffen, die als Basis für Folgeversuche dienen soll, in denen Zellen von Patienten mit Autoimmunkrankheiten untersucht werden sollen. Da sich die Inflammation bei Autoimmunerkrankungen insbesondere in den betroffenen Geweben abspielt, wäre eine Studie anzustreben, in der aus dem Blut isolierte T-Lymphozyten den Zellen aus den entzündeten Geweben gegenübergestellt werden. Ergänzend sollte eine Phänotypisierung der Tregs und der CD4+CD25-CD127- Zellen nach der Zytokin-Stimulation erfolgen.
Zusammenfassend konnte die Plastizität peripherer T-Lymphozyten in Richtung Treg gezeigt werden. Besonders hervorzuheben ist die bislang wenig untersuchte Zellpopulation der CD4+CD25-CD127- Zellen, die eine vielversprechende Zellpopulation für die in vitro Induktion von Tregs darstellt.
Background
Juvenile idiopathic arthritis (JIA) is often accompanied by pathomorphological changes to the temporomandibular joint (TMJ). By analyzing orthodontical orthopantomograms of JIA patients the aims of the study were a) classification of condyle changes, b) quantification of bony asymmetries of condylar destruction and c) detection of relationships between disease duration and TMJ-involvement.
Patients/Methods
46 caucasian JIA-patients (28 female; 18 male; < 16.0 years) were enrolled, each joint (n = 92) was morphologically assessed by means of orthopantomogram, quantitatively analysed and compared with duration of general disease. Condyle morphology was assessed using the Billiau scale for severity of destruction [1]. The quantitative analysis was based on ratios of condyle, ramus and mandible height.
Results
Patients were divided into groups (Group I – slightly affected, n = 36; Billiau severity 0–2; condyle findings: X-ray normal, condyle erosions, condylar flattening; Group II – severely affected, N = 10; Billiau severity 3–4; condyle findings: condylar flattenings and erosions, unilateral/bilateral complete loss of condyles), based on morphological analysis of condylar destruction. Duration of disease was significantly longer in Group II (8.9 ± 5.2 years) than in Group I (4.6 ± 4.7 years). Asymmetries of condyle, ramus and mandible height, quantitatively analysed by contralateral comparison, were significantly more marked in patients of Group II than of Group I.
Conclusions
Orthopantomogram imaging can be used in orthodontics clinical routine to detect TMJ-pathologies and is an important reference for monitoring progression of JIA. Classification into severe and slightly affected TMJ is possible by analysis of condylar pathomorphology. An association between degree of destruction, extent of lower jaw asymmetry and disease duration is suggested by the results.
Background
Children with different underlying malignant diseases require long-term central venous access. As for port systems in a pectoral position, peripherally implanted port systems in the forearm revealed high levels of technical and clinical success in adult cohorts.
Objective
To investigate the technical and clinical outcomes of percutaneous central venous port implantation in the forearm in adolescents.
Materials and methods
Between April 2010 and August 2020, 32 children ages 9 to 17 years with underlying malignancy received 35 totally implantable venous access ports (TIVAPs) in the forearm. All venous port systems were peripherally inserted under ultrasound guidance. Correct catheter placement was controlled by fluoroscopy. As primary endpoints, the technical success, rate of complications and catheter maintenance were analyzed. Secondary endpoints were the side of implantation, vein of catheter access, laboratory results on the day of the procedure, procedural radiation exposure, amount of contrast agent and reasons for port device removal.
Results
Percutaneous TIVAP placement under sonographic guidance was technically successful in 34 of 35 procedures (97.1%). Procedure-related complications did not occur. During the follow-up, 13,684 catheter days were analyzed, revealing 11 complications (0.8 per 1,000 catheter-duration days), Of these 11 complications, 7 were major and 10 occurred late. In seven cases, the port device had to be removed; removal-related complications did not occur.
Conclusion
Peripheral TIVAP placement in the forearms of children is a feasible, effective and safe technique with good midterm outcome. As results are comparable with standard access routes, this technique may be offered as an alternative when intermittent venous access is required.
Background
Chronic non-bacterial osteomyelitis (CNO) is an autoinflammatory disorder of the skeletal system of yet unknown etiology. Patients present with local bone pain and inflammation and - to our experience - often suffer from functional impairment with significant disabilities of daily life. The objective of this study was to assess physical activity, fitness and health-related quality of life (HRQOL) in adolescents with established diagnosis of CNO versus healthy controls (HC).
Methods
15 patients with CNO and 15 age and gender matched HC aged 13–18 years, completed questionnaires, performed an incremental exercise test with gas exchange measures up to voluntary fatigue and wore an accelerometer over 7 days at home to assess physical activity behavior.
Results
At the time of assessment, 5 CNO patients were in clinical, one in radiological and 5 in clinical and radiological remission. 7 did not receive any therapy at the time of assessment. The results of the exercise test and of the accelerometry did not show any significant difference between CNO and HC. However, reported sports participation was lower in patients with CNO and PedsQL3.0 and 4.0 showed significant lower values in most of the scores indicating reduced HRQOL.
Conclusion
Although most CNO patients showed a favorable course of disease without any relevant differences in objective measurements of physical activity and fitness versus HC at the time of assessment, questionnaires revealed perceived limitations. Further studies are needed to measure HRQOL and to validate questionnaires in patients with CNO against objective measures including more participants with a higher level of disease activity.
Background
Increasing bacterial resistance to antibiotics is a serious problem worldwide. We sought to record the acquisition of antibiotic-resistant Escherichia coli (E. coli) in healthy infants in Northern Thailand and investigated potential determinants.
Methods
Stool samples from 142 infants after birth, at ages 2wk, 2mo, 4 to 6mo, and 1y, and parent stool samples were screened for E. coli resistance to tetracycline, ampicillin, co-trimoxazole, and cefazoline by culture, and isolates were further investigated for multiresistance by disc diffusion method. Pulsed-field gel electrophoresis was performed to identify persistent and transmitted strains. Genetic comparison of resistant and transmitted strains was done by multilocus sequence typing (MLST) and strains were further investigated for extra- and intra-intestinal virulence factors by multiplex PCR.
Results
Forty-seven (33%) neonatal meconium samples contained resistant E. coli. Prevalence increased continuously: After 1y, resistance proportion (tetracycline 80%, ampicillin 72%, co-trimoxazole 66%, cefazoline 35%) almost matched those in parents. In 8 infants (6%), identical E. coli strains were found in at least 3 sampling time points (suggesting persistence). Transmission of resistant E. coli from parents to child was observed in only 8 families. MLST showed high diversity. We could not identify any virulence genes or factors associated with persistence, or transmission of resistant E. coli. Full-term, vaginal birth and birth in rural hospital were identified as risk factors for early childhood colonization with resistant E. coli.
Conclusion
One third of healthy Thai neonates harboured antibiotic-resistant E. coli in meconium. The proportion of resistant E. coli increased during the first year of life almost reaching the value in adults. We hypothesize that enhancement of infection control measures and cautious use of antibiotics may help to control further increase of resistance.
Die cGvHD ist eine relevante Komplikation der aHSCT. Sie hat Einfluss auf die Morbidität und Mortalität nach der aHSCT. Die genaue Pathogenese ist unbekannt. Ein Einfluss dendritscher Zellen als Schlüsselzellen immunologischer Prozesse auf die Entstehung einer cGvHD ist wahrscheinlich.
In dieser Studie erfassten wir patient:innen und aHSCT-bezogene Daten 61 stammzelltransplantierter Kinder und Jugendlicher sowie deren möglichen Einfluss auf das Auftreten einer cGvHD. Zudem wurde die Rekonstitution der Immunzellen, insbesondere der DCs, nach der aHSCT evaluiert.
Die Rekonstitution der Lymphozyten sowie die Zytokinexpression der T-Zellen während der Rekonstitution verhielt sich ähnlich zu vorherigen Studien. Signifikante Unterschiede zwischen Patient:innen mit oder ohne cGvHD zeigten sich nicht konsistent. Die Rekonstitution der DCs erfolgte innerhalb von 60-100 Tagen. Stabile Werte zeigten sich jedoch frühestens ein Jahr nach der aHSCT. Während des Engraftments war der Anteil CCR7+ DCs bei Patient:innen erhöht, die eine TBI erhalten hatten. Ein hoher Anteil CCR7+ DCs wirkte sich zu jedem Zeitpunkt positiv auf das Gesamtüberleben aus. Trat eine höhergradige aGvHD auf, konnte eine verminderte absolute und relative Zellzahl 60-365 Tage nach der aHSCT für DCs insgesamt und mDCs erfasst werden, für pDCs 60-100 Tage nach aHCST. Zwischen der Rekonstitution der DCs und dem Auftreten einer cGvHD konnten wir vier Korrelationen beobachten. Der absolute und relative Zahlenwert an mDCs war während des Engraftments bis Tag 60-100 vermindert. Der absolute und relative Zahlenwert an DCs insgesamt war ab Tag 101-365 vermindert. Ein erhöhter Anteil an pDCs konnte ab Tag 101-365 sowie zu Beginn einer cGvHD bestimmt werden. Der Anteil monozytärer DCs war ab Tag 60-100 erhöht.
Die Pathogenese der cGvHD bleibt weiter teilweise unklar. Unsere Daten suggerieren einen Einfluss der DCs auf die Entstehung einer cGvHD.
Seltene Erkrankungen (SE) werden durch die im deutschen Gesundheitssystem verwendete Diagnosenklassifikation ICD-10-GM (International Statistical Classification of Diseases and Related Health problems, 10th Revision, German Modification) nur zu einem kleinen Teil eindeutig erfasst. Daher sind Aussagen zur Häufigkeit von SE sowie zum speziellen Versorgungs- und Finanzierungsbedarf nicht möglich, was zu einer lückenhaften Datenlage als Entscheidungsgrundlage für Krankenkassen, Leistungserbringer und Gesundheitspolitik führt. Das Fehlen exakter Informationen behindert auch die wissenschaftliche Arbeit. Daher wird deutschlandweit ab 2023 die Verwendung der Alpha-ID-SE-Datei und der ORPHAcodes für die spezifische Erfassung von SE bei stationären Fällen verpflichtend.
Die Alpha-ID-SE-Datei verknüpft die ICD-10-GM-Kodes mit den international anerkannten ORPHAcodes für die Diagnose von SE. Kommerzielle Anbieter stellen zunehmend die benötigten IT-Tools zur Kodierung von SE zur Verfügung. An mehreren Universitätskliniken mit Zentren für SE wurden Lösungen etabliert, die eine vollständige Kodierung gewährleisten sollen. Hierzu gehören finanzielle Anreize für die kodierenden Bereiche, konkrete Nachfragen nach dem Vorliegen einer SE beim Kodiervorgang und eine semiautomatische Kodierung bei Patient*innen, die schon einmal mit einer SE an der Einrichtung betreut worden waren. Eine Kombination der verschiedenen Ansätze verspricht die höchste Wahrscheinlichkeit einer vollständigen Kodierung.
Für ein umfängliches Bild der SE im Gesundheitssystem und um dem speziellen Versorgungs- und Finanzierungsbedarf besser Rechnung tragen zu können, wäre auch im ambulanten Bereich eine möglichst spezifische und eindeutige Kodierung wünschenswert. Für komplexe SE und bisher undiagnostizierte Patient*innen wird zusätzlich eine strukturierte Erfassung des Phänotyps benötigt.
Hyper-IgM syndrome type 2 (HIGM2) is a B cell intrinsic primary immunodeficiency caused by mutations in AICDA encoding activation-induced cytidine deaminase (AID) which impair immunoglobulin class switch recombination (CSR) and somatic hypermutation (SHM). Whereas autosomal-recessive AID-deficiency (AR-AID) affects both CSR and SHM, the autosomal-dominant form (AD-AID) due to C-terminal heterozygous variants completely abolishes CSR but only partially affects SHM. AR-AID patients display enhanced germinal center (GC) reactions and autoimmune manifestations, which are not present in AD-AID, suggesting that SHM but not CSR regulates GC reactions and peripheral B cell tolerance. Herein, we describe two siblings with HIGM2 due to a novel homozygous AICDA mutation (c.428-1G > T) which disrupts the splice acceptor site of exon 4 and results in the sole expression of a truncated AID variant that lacks 10 highly conserved amino acids encoded by exon 4 (AID-ΔE4a). AID-ΔE4a patients suffered from defective CSR and enhanced GC reactions and were therefore indistinguishable from other AR-AID patients. However, the AID-ΔE4a variant only partially affected SHM as observed in AD-AID patients. In addition, AID-ΔE4a but not AD-AID patients revealed impaired targeting of mutational hotspot motives and distorted mutational patterns. Hence, qualitative defects in AID function and altered SHM rather than global decreased SHM activity may account for the disease phenotype in these patients.
Immunization of preterm infants: current evidence and future strategies to individualized approaches

Preterm infants are at particularly high risk for infectious diseases. As this vulnerability extends beyond the neonatal period into childhood and adolescence, preterm infants benefit greatly from infection-preventive measures such as immunizations. However, there is an ongoing discussion about vaccine safety and efficacy due to preterm infants’ distinct immunological features. A significant proportion of infants remains un- or under-immunized when discharged from primary hospital stay. Educating health care professionals and parents, promoting maternal immunization and evaluating the potential of new vaccination tools are important means to reduce the overall burden from infectious diseases in preterm infants. In this narrative review, we summarize the current knowledge about vaccinations in premature infants. We discuss the specificities of early life immunity and memory function, including the role of polyreactive B cells, restricted B cell receptor diversity and heterologous immunity mediated by a cross-reactive T cell repertoire. Recently, mechanistic studies indicated that tissue-resident memory (Trm) cell populations including T cells, B cells and macrophages are already established in the fetus. Their role in human early life immunity, however, is not yet understood. Tissue-resident memory T cells, for example, are diminished in airway tissues in neonates as compared to older children or adults. Hence, the ability to make specific recall responses after secondary infectious stimulus is hampered, a phenomenon that is transcriptionally regulated by enhanced expression of T-bet. Furthermore, the microbiome establishment is a dominant factor to shape resident immunity at mucosal surfaces, but it is often disturbed in the context of preterm birth. The proposed function of Trm T cells to remember benign interactions with the microbiome might therefore be reduced which would contribute to an increased risk for sustained inflammation. An improved understanding of Trm interactions may determine novel targets of vaccination, e.g., modulation of T-bet responses and facilitate more individualized approaches to protect preterm babies in the future.
Ureaplasma species (spp.) are considered commensals of the adult genitourinary tract, but have been associated with chorioamnionitis, preterm birth, and invasive infections in neonates, including meningitis. Data on mechanisms involved in Ureaplasma-driven neuroinflammation are scarce. The present study addressed brain inflammatory responses in preterm lambs exposed to Ureaplasma parvum (UP) in utero. 7 days after intra-amniotic injection of UP (n = 10) or saline (n = 11), lambs were surgically delivered at gestational day 128–129. Expression of inflammatory markers was assessed in different brain regions using qRT-PCR and in cerebrospinal fluid (CSF) by multiplex immunoassay. CSF was analyzed for UP presence using ureB-based real-time PCR, and MRI scans documented cerebral white matter area and cortical folding. Cerebral tissue levels of atypical chemokine receptor (ACKR) 3, caspases 1-like, 2, 7, and C–X–C chemokine receptor (CXCR) 4 mRNA, as well as CSF interleukin-8 protein concentrations were significantly increased in UP-exposed lambs. UP presence in CSF was confirmed in one animal. Cortical folding and white matter area did not differ among groups. The present study confirms a role of caspases and the transmembrane receptors ACKR3 and CXCR4 in Ureaplasma-driven neuroinflammation. Enhanced caspase 1-like, 2, and 7 expression may reflect cell death. Increased ACKR3 and CXCR4 expression has been associated with inflammatory central nervous system (CNS) diseases and impaired blood–brain barrier function. According to these data and previous in vitro findings from our group, we speculate that Ureaplasma-induced caspase and receptor responses affect CNS barrier properties and thus facilitate neuroinflammation.
Purpose
In selected cases of severe Cushing’s syndrome due to uncontrolled ACTH secretion, bilateral adrenalectomy appears unavoidable. Compared with unilateral adrenalectomy (for adrenal Cushing’s syndrome), bilateral adrenalectomy has a perceived higher perioperative morbidity. The aim of the current study was to compare both interventions in endogenous Cushing’s syndrome regarding postoperative outcomes.
Methods
We report a single-center, retrospective cohort study comparing patients with hypercortisolism undergoing bilateral vs. unilateral adrenalectomy during 2008–2021. Patients with adrenal Cushing’s syndrome due to adenoma were compared with patients with ACTH-dependent Cushing’s syndrome (Cushing’s disease and ectopic ACTH production) focusing on postoperative morbidity and mortality as well as long-term survival.
Results
Of 83 patients with adrenalectomy for hypercortisolism (65.1% female, median age 53 years), the indication for adrenalectomy was due to adrenal Cushing’s syndrome in 60 patients (72.2%; 59 unilateral and one bilateral), and due to hypercortisolism caused by Cushing’s disease (n = 16) or non-pituitary uncontrolled ACTH secretion of unknown origin (n = 7) (27.7% of all adrenalectomies). Compared with unilateral adrenalectomy (n = 59), patients with bilateral adrenalectomy (n = 24) had a higher rate of severe complications (0% vs. 33%; p < 0.001) and delayed recovery (median: 10.2% vs. 79.2%; p < 0.001). Using the MTL30 marker, patients with bilateral adrenalectomy fared worse than patients after unilateral surgery (MTL30 positive: 7.2% vs. 25.0% p < 0.001). Postoperative mortality was increased in patients with bilateral adrenalectomy (0% vs. 8.3%; p = 0.081).
Conclusion
While unilateral adrenalectomy for adrenal Cushing’s syndrome represents a safe and definitive therapeutic option, bilateral adrenalectomy to control ACTH-dependent extra-adrenal Cushing’s syndrome or Cushing’s disease is a more complicated intervention with a mortality of nearly 10%.

Extracellular vesicles (EVs) are a heterogenous population of plasma membrane-surrounded particles that are released in the extracellular milieu by almost all types of living cells. EVs are key players in intercellular crosstalk, both locally and systemically, given that they deliver their cargoes (consisting of proteins, lipids, mRNAs, miRNAs, and DNA fragments) to target cells, crossing biological barriers. Those mechanisms further trigger a wide range of biological responses. Interestingly, EV phenotypes and cargoes and, therefore, their functions, stem from their specific parental cells. For these reasons, EVs have been proposed as promising candidates for EV-based, cell-free therapies. One of the new frontiers of cell-based immunotherapy for the fight against refractory neoplastic diseases is represented by genetically engineered chimeric antigen receptor T (CAR-T) lymphocytes, which in recent years have demonstrated their effectiveness by reaching commercialization and clinical application for some neoplastic diseases. CAR-T-derived EVs represent a recent promising development of CAR-T immunotherapy approaches. This crosscutting innovative strategy is designed to exploit the advantages of genetically engineered cell-based immunotherapy together with those of cell-free EVs, which in principle might be safer and more efficient in crossing biological and tumor-associated barriers. In this review, we underlined the potential of CAR-T-derived EVs as therapeutic agents in tumors.
Preterm infants are susceptible to infection and their defense against pathogens relies largely on innate immunity. The role of the complement system for the immunological vulnerability of preterm infants is less understood. Anaphylatoxin C5a and its receptors C5aR1 and -2 are known to be involved in sepsis pathogenesis, with C5aR1 mainly exerting pro-inflammatory effects. Our explorative study aimed to determine age-dependent changes in the expression of C5aR1 and C5aR2 in neonatal immune cell subsets. Via flow cytometry, we analyzed the expression pattern of C5a receptors on immune cells isolated from peripheral blood of preterm infants (n = 32) compared to those of their mothers (n = 25). Term infants and healthy adults served as controls. Preterm infants had a higher intracellular expression of C5aR1 on neutrophils than control individuals. We also found a higher expression of C5aR1 on NK cells, particularly on the cytotoxic CD56\(^{dim}\) subset and the CD56\(^-\) subset. Immune phenotyping of other leukocyte subpopulations revealed no gestational-age-related differences for the expression of and C5aR2. Elevated expression of C5aR1 on neutrophils and NK cells in preterm infants may contribute to the phenomenon of “immunoparalysis” caused by complement activation or to sustained hyper-inflammatory states. Further functional analyses are needed to elucidate the underlying mechanisms.
Developmentally regulated features of innate immunity are thought to place preterm and term infants at risk of infection and inflammation-related morbidity. Underlying mechanisms are incompletely understood. Differences in monocyte function including toll-like receptor (TLR) expression and signaling have been discussed. Some studies point to generally impaired TLR signaling, others to differences in individual pathways. In the present study, we assessed mRNA and protein expression of pro- and anti-inflammatory cytokines in preterm and term cord blood (CB) monocytes compared with adult controls stimulated ex vivo with Pam3CSK4, zymosan, polyinosinic:polycytidylic acid, lipopolysaccharide, flagellin, and CpG oligonucleotide, which activate the TLR1/2, TLR2/6, TLR3, TLR4, TLR5, and TLR9 pathways, respectively. In parallel, frequencies of monocyte subsets, stimulus-driven TLR expression, and phosphorylation of TLR-associated signaling molecules were analyzed. Independent of stimulus, pro-inflammatory responses of term CB monocytes equaled adult controls. The same held true for preterm CB monocytes—except for lower IL-1β levels. In contrast, CB monocytes released lower amounts of anti-inflammatory IL-10 and IL-1ra, resulting in higher ratios of pro-inflammatory to anti-inflammatory cytokines. Phosphorylation of p65, p38, and ERK1/2 correlated with adult controls. However, stimulated CB samples stood out with higher frequencies of intermediate monocytes (CD14\(^+\)CD16\(^+\)). Both pro-inflammatory net effect and expansion of the intermediate subset were most pronounced upon stimulation with Pam3CSK4 (TLR1/2), zymosan (TR2/6), and lipopolysaccharide (TLR4). Our data demonstrate robust pro-inflammatory and yet attenuated anti-inflammatory responses in preterm and term CB monocytes, along with imbalanced cytokine ratios. Intermediate monocytes, a subset ascribed pro-inflammatory features, might participate in this inflammatory state.
Die Inadäquate Sinustachykardie (IST) ist eine häufige Erkrankung des autonomen Nervensystems bei Kindern und Erwachsenen. Die Diagnose und Therapie einer IST bei Jugendlichen ist bisher nicht genau definiert. In dieser retrospektiven Studie haben wir unsere Hypothese bezüglich autonomer Dysfunktion im Kindesalter durch die Analyse von 24-h Herzfrequenzvariabilität (HRV) bei 479 Kindern mit einem Durchschnittsalter von 13,7 ± 2,1 Jahren, die innerhalb der letzten 15 Jahre an die pädiatrische Ambulanz überwiesen wurden, untersucht.
74 Jugendliche hatten eine mittlere Herzfrequenz ≥95/min (Cut-off Werte für eine IST basieren auf der gesunden Kontrollgruppe) und hatten damit eine IST.
Wir stellten fest, dass das Risiko einer IST bei Jugendlichen mit einer Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) (OR = 3,5, p < 0,001), Prä-Hypertonie (OR = 2,5, p = 0,043) und Hypertonie (OR = 2,1, p = 0,02) hoch ist; nicht signifikant erhöht bei Kindern mit Kleinwuchs (OR = 1,9, p = 0,19), chirurgisch behandelte angeborene Herzkrankheit (OR = 1,4). ,p = 0,51) und Adipositas ohne Bluthochdruck (OR = 1,4; p = 0,25); und unbedeutsam bei Jugendlichen mit Anorexia nervosa (OR = 0,3, p = 0,26) und konstitutioneller Dünnheit (OR = 0,9, p = 0,89).
Eine IST war mit signifikant reduzierten HRV-Werten und erhöhten Blutdrücken assoziiert, was auf ein erhöhtes kardiovaskuläres Risiko hindeutet.
In dieser retrospektiven Analyse litten 15,4 % der Jugendlichen an einer IST mit einer 24h HF ≥ 95 bpm hauptsächlich aufgrund einer ADHS und Hypertonie.
The aim of this research was to characterize cognitive abilities in patients with Glut1-Deficiency syndrome (Glut1DS) following ketogenic diet therapy (KDT). Methods: The cognitive profiles of eight children were assessed using the Wechsler Intelligence Scale (WISC-IV). The effect of ketogenic diet therapy (KDT) on individual subareas of intelligence was analyzed considering the potential influence of speech motor impairments. Results: Patients with Glut1DS showed a wide range of cognitive performance levels. Some participants showed statistically and clinically significant discrepancies between individual subdomains of intelligence. Both variables, KDT initiation as well as duration, had a positive effect on the overall IQ score. Significant correlations were partially found between the time of KDT initiation and the level of IQ scores, depending on the presence of expressive language test demands of the respective subtests of the WISC-IV. Accordingly, the participants benefited les in the linguistic cognitive domain. The discrepancies in cognitive performance profiles of patients with Glut1DS can be attributed to the possibility of a negative distortion of the results due to the influence of speech motor impairments. Conclusions: The individual access skills of test persons should be more strongly considered in test procedures for the assessment of intelligence to reduce the negative influence of motor deficits on test performance. Specific characterization and systematization of the speech disorder are indispensable for determining the severity of speech motor impairment in Glut1DS. Therefore, a stronger focus on dysarthria during diagnosis and therapy is necessary.
Dendritische Zellen (DC) spielen eine Schlüsselrolle im Immunsystem. Sie dienen als professionelle antigenpräsentierende Zellen und können eine antigenspezifische Immunantwort initiieren, indem sie naive T-Zellen primen.
DC können auch verwendet werden, um T-Zellen im Kontext der onkologischen Immuntherapie zu stimulieren. In vitro können sie leicht aus Monozyten differenziert werden. Die daraus resultierenden unreifen DC können bereits Antigene phagozytieren und präsentieren, sie aktivieren jedoch noch keine Immunantwort solange keines der aufgenommenen Antigene als pathogen erkannt wird. Die Ausreifung einer unreifen, tolerogenen DC zu einer immunogenen reifen DC kann, neben anderen Methoden, durch einen Cocktail aus TLR-Liganden oder Zytokinen erreicht werden. Die Auswahl der Substanzen in diesem Cocktail bestimmt den Phänotyp und die funktionellen Eigenschaften der resultierenden reifen DC. Einige der benötigten Fähigkeiten der DC in der Tumorimmuntherapie, wo sie aus Patientenmonozyten generiert, mit Tumorantigen beladen und dem Patienten wieder zugeführt werden sollen, umfassen die Migration zu den T-Zell-Zonen der Lymphknoten, Antigenpräsentation auf sowohl MHC-I- als auch MHC-II-Molekülen, Zytokinproduktion für die Direktion der T-Zell-Antwort wie IL-12p70, und die Expression von Oberflächenmarkern wie der kostimulatorischen Moleküle CD80 und CD86.
In der Vergangenheit wurde gezeigt, dass durch Zugabe von Prostaglandin E2 (PGE2) zu einem Cocktail mit dem synthetischen TLR3-Liganden poly-I:C und dem TLR7/8-Liganden R848 (Resiquimod) sowohl eine gute migratorische Fähigkeit als auch eine erhöhte IL-12p70-Produktion erreicht werden kann, während etwa die Fähigkeit zur Antigen-Kreuzpräsentation reduziert erschien. Anhand von Monozyten anonymer gesunder Spender beleuchtet diese Arbeit daher den Effekt von PGE2 auf monozytenderivierte DC näher, indem seine konzentrationsabhängige Wirkung auf deren Phänotyp untersucht wird. In den durchgeführten Versuchen wurde dabei die Expressionsdichte der Oberflächenmarker CD83, CD80 und CD86, HLA-DR und CCR7 sowie der monozytäre Marker CD14 durchflusszytometrisch analysiert. Die Ergebnisse zeigen bei Exposition mit PGE2 dosisabhängig eine Heraufregulation von CD80, CD83, CD86 und CCR7 in der Population reifer DC, deren Maximum in unteren mikromolaren Konzentrationen erreicht wird. Gleichzeitig induzierte PGE2 dosisabhängig auch die Entstehung einer zweiten Zellpopulation mit anderen Eigenschaften, die stattdessen den monozytären Marker CD14 re-exprimierte. Dies ist für künftige Studien eine interessante Beobachtung, da sie eine differenzierte Betrachtung beider resultierender Subpopulationen anregt.
Impfungen sind essenziell zur Prävention vieler Erkrankungen. Für die Impfentscheidung von Patienten spielt die ärztliche Beratung eine wichtige Rolle. Die universitäre Lehre sollte das notwendige Wissen zu Impfungen vermitteln. Die vorliegende Studie untersuchte die Selbsteinschätzung des Impfwissens von Medizinstudierenden sowie die Struktur der Impfausbildung im Medizinstudium. In einer Umfrage wurden Medizinstudierende an den damaligen fünf medizinischen Fakultäten in Bayern im Sommer- und Wintersemester 2018 zu ihrem Impfwissen und ihrer Impfausbildung befragt.
In der nicht vollständig geklärten Pathogenese der juvenilen idiopathischen Arthritis (JIA) spielen insbesondere Effektor-T-Zellen und regulatorische T-Zellen (Tregs), sowie das Kontinuum ihrer plastischen Zelltypen eine wichtige Rolle. Im arthritischen Milieu, das auch Interleukin-17 (IL-17) beinhaltet, verschiebt sich das Gleichgewicht dieser Zellen hin zur Inflammation. Ziel dieser Arbeit war es, die T-Zellbalance im peripheren Blut von JIA-Patienten mit gesunden Kontrollen (HC) zu vergleichen, sowie den Einfluss von IL-17, anti-IL-17 und eines Th17-stimulierenden, proinflammatorischen Zytokinmilieus auf Phänotyp und Funktion der Tregs zu untersuchen.
Es erfolgte die durchflusszytometrische Analyse des Lymphozytenpools von 16 JIA- und 10 HC-Probanden. Isolierte CD25+CD127-CD4+ Tregs wurden mit den genannten Stimuli kultiviert und danach durchflusszytometrisch phänotypisch sowie in Co-Kultur mit peripheren mononukleären Zellen (PBMCs) auf ihre Suppressionsfunktion untersucht.
Wir stellten erhöhte Proportionen von Th17-Zellen, Tregs und effector-like Tregs in JIA-Patienten fest. Bei Stimulation mit dem Th17-Cocktail zeigte sich eine verminderte Suppression der Effektor-Zellen durch Tregs bei zeitgleich vermehrter FoxP3-Expression insbesondere in JIA-Tregs. Secukinumab bewirkte eine Anpassung der FoxP3-Expression der Tregs in der Patientengruppe auf etwa das Niveau der gesunden Kontrollen.
Insgesamt bestätigt diese Arbeit eine proinflammatorische Dysbalance bei JIA-Patienten mit Shift zu Th17-like Tregs als möglicherweise pathologischen Phänotyp. Ursache für die verminderte Suppression ist vermutlich eine gesteigerte Resistenz der Effektor-Zellen durch das Inflammationsmilieu. Die vermehrte FoxP3-Expression der JIA-Tregs ist am ehesten durch eine gesteigerte Zellaktivierung zu erklären. Diese erhöhte Sensitivität der Tregs für inflammatorische Stimuli mit vermehrter Aktivierung ist ein möglicher Ansatzpunkt für zukünftige Therapien. Die Adjustierung der FoxP3-Expression unter Secukinumab in der JIA-Gruppe auf Niveau der Kontrollen bildet daher einen denkbaren zusätzlichen therapeutischen Effekt der IL-17A Blockade durch potenzielle intrinsische Stabilisierung des Phänotyps der Tregs bei der JIA ab.
Bei der HPP handelt es sich um eine seltene, erblich bedingte Stoffwechselerkrankung, die unter anderem mit einer Störung des Knochen- und Mineralstoffwechsels einhergeht. Ziel dieser Arbeit war es, die objektiv messbare Aktivität und die HRQoL der jungen HPP-Patientinnen und -Patienten zu untersuchen. Dazu sollten die hierbei erhobenen Daten des erkrankten Patientenkollektivs mit den Daten des gesunden Kontrollkollektivs verglichen werden. Dies geschah unter der Verwendung von Accelerometrie, Spiroergometrie und etablierten Fragebögen in 18 Probandinnen und Probanden und 18 Gesundkontrollen.
In den Fragebögen zeigten sich deutliche Defizite, welche sich nur zum Teil in den objektiven Untersuchungen wiederspiegelten. Weitere Untersuchungen mit einer größeren Studienpopulation und Validierung der Untersuchungsmethoden für die HPP werden zukünftig benötigt.
Linear, dimeric, tetrameric, and cyclic peptides derived from lactoferricin B, containing the RRWQWR motif, were designed, synthesized, purified, and characterized using RP-HPLC chromatography and MALDI-TOF mass spectrometry. The antibacterial activity of the designed peptides against E. coli (ATCC 11775 and 25922) and their cytotoxic effect against MDA-MB-468 and MDA-MB-231 breast cancer cell lines were evaluated. Dimeric and tetrameric peptides showed higher antibacterial activity in both bacteria strains than linear peptides. The dimeric peptide (RRWQWR)\(_2\)K-Ahx exhibited the highest antibacterial activity against the tested bacterial strains. Furthermore, the peptides with high antibacterial activity exhibited significant cytotoxic effect against the tested breast cancer cell lines. This cytotoxic effect was fast and dependent on the peptide concentration. The tetrameric molecule containing RRWQWR motif has an optimal cytotoxic effect at a concentration of 22 µM. The evaluated dimeric and tetrameric peptides could be considered as candidates for developing new therapeutic agents against breast cancer. Polyvalence of linear sequences could be considered as a novel and versatile strategy for obtaining molecules with high anticancer activity.
Introduction: Malignant rhabdoid tumors (MRT) predominantly affect infants and young children. Patients below six months of age represent a particularly therapeutically challenging group. Toxicity to developing organ sites limits intensity of treatment. Information on prognostic factors, genetics, toxicity of treatment and long-term outcomes is sparse. Methods: Clinical, genetic, and treatment data of 100 patients (aged below 6 months at diagnosis) from 13 European countries were analyzed (2005–2020). Tumors and matching blood samples were examined for SMARCB1 mutations using FISH, MLPA and Sanger sequencing. DNA methylation subgroups (ATRT-TYR, ATRT-SHH, and ATRT-MYC) were determined using 450 k / 850 k-profiling. Results: A total of 45 patients presented with ATRT, 29 with extracranial, extrarenal (eMRT) and 9 with renal rhabdoid tumors (RTK). Seventeen patients demonstrated synchronous tumors (SYN). Metastases (M+) were present in 27% (26/97) at diagnosis. A germline mutation (GLM) was detected in 55% (47/86). DNA methylation subgrouping was available in 50% (31 / 62) with ATRT or SYN; for eMRT, methylation-based subgrouping was not performed. The 5-year overall (OS) and event free survival (EFS) rates were 23.5 ± 4.6% and 19 ± 4.1%, respectively. Male sex (11 ± 5% vs. 35.8 ± 7.4%), M+ stage (6.1 ± 5.4% vs. 36.2 ± 7.4%), presence of SYN (7.1 ± 6.9% vs. 26.6 ± 5.3%) and GLM (7.7 ± 4.2% vs. 45.7 ± 8.6%) were significant prognostic factors for 5-year OS. Molecular subgrouping and survival analyses confirm a previously described survival advantage for ATRT-TYR. In an adjusted multivariate model, clinical factors that favorably influence the prognosis were female sex, localized stage, absence of a GLM and maintenance therapy. Conclusions: In this cohort of homogenously treated infants with MRT, significant predictors of outcome were sex, M-stage, GLM and maintenance therapy. We confirm the need to stratify which patient groups benefit from multimodal treatment, and which need novel therapeutic strategies. Biomarker-driven tailored trials may be a key option.
Adrenocortical tumors are rare in children. This systematic review summarizes the published evidence on pediatric adrenocortical carcinoma (ACC) to provide a basis for a better understanding of the disease, investigate new molecular biomarkers and therapeutic targets, and define which patients may benefit from a more aggressive therapeutic approach. We included 137 studies with 3680 ACC patients (~65% female) in our analysis. We found no randomized controlled trials, so this review mainly reflects retrospective data. Due to a specific mutation in the TP53 gene in ~80% of Brazilian patients, that cohort was analyzed separately from series from other countries. Hormone analysis was described in 2569 of the 2874 patients (89%). Most patients were diagnosed with localized disease, whereas 23% had metastasis at primary diagnosis. Only 72% of the patients achieved complete resection. In 334 children (23%), recurrent disease was reported: 81% — local recurrence, 19% (n = 65) — distant metastases at relapse. Patients < 4 years old had a different distribution of tumor stages and hormone activity and better overall survival (p < 0.001). Although therapeutic approaches are typically multimodal, no consensus is available on effective standard treatments for advanced ACC. Thus, knowledge regarding pediatric ACC is still scarce and international prospective studies are needed to implement standardized clinical stratifications and risk-adapted therapeutic strategies.
Outpatient antibiotic use is closely related to antimicrobial resistance and in Germany, almost 70% of antibiotic prescriptions in human health are issued by primary care physicians (PCPs). The aim of this study was to explore PCPs, namely General Practitioners' (GPs) and outpatient pediatricians' (PDs) knowledge of guideline recommendations on rational antimicrobial treatment, the determinants of confidence in treatment decisions and the perceived need for training in this topic in a large sample of PCPs from southern Germany. Out of 3753 reachable PCPs, 1311 completed the survey (overall response rate = 34.9%). Knowledge of guideline recommendations and perceived confidence in making treatment decisions were high in both GPs and PDs. The two highest rated influencing factors on prescribing decisions were reported to be guideline recommendations and own clinical experiences, hence patients' demands and expectations were judged as not influencing treatment decisions. The majority of physicians declared to have attended at least one specific training course on antibiotic use, yet almost all the participating PCPs declared to need more training on this topic. More studies are needed to explore how consultation-related and context-specific factors could influence antibiotic prescriptions in general and pediatric primary care in Germany beyond knowledge. Moreover, efforts should be undertaken to explore the training needs of PCPs in Germany, as this would serve the development of evidence-based educational interventions targeted to the improvement of antibiotic prescribing decisions rather than being focused solely on knowledge of guidelines.
The signal modelling framework JimenaE simulates dynamically Boolean networks. In contrast to SQUAD, there is systematic and not just heuristic calculation of all system states. These specific features are not present in CellNetAnalyzer and BoolNet. JimenaE is an expert extension of Jimena, with new optimized code, network conversion into different formats, rapid convergence both for system state calculation as well as for all three network centralities. It allows higher accuracy in determining network states and allows to dissect networks and identification of network control type and amount for each protein with high accuracy. Biological examples demonstrate this: (i) High plasticity of mesenchymal stromal cells for differentiation into chondrocytes, osteoblasts and adipocytes and differentiation-specific network control focusses on wnt-, TGF-beta and PPAR-gamma signaling. JimenaE allows to study individual proteins, removal or adding interactions (or autocrine loops) and accurately quantifies effects as well as number of system states. (ii) Dynamical modelling of cell–cell interactions of plant Arapidopsis thaliana against Pseudomonas syringae DC3000: We analyze for the first time the pathogen perspective and its interaction with the host. We next provide a detailed analysis on how plant hormonal regulation stimulates specific proteins and who and which protein has which type and amount of network control including a detailed heatmap of the A.thaliana response distinguishing between two states of the immune response. (iii) In an immune response network of dendritic cells confronted with Aspergillus fumigatus, JimenaE calculates now accurately the specific values for centralities and protein-specific network control including chemokine and pattern recognition receptors.
Background and purpose
Pediatric adrenocortical carcinoma (pACC) is a rare disease with poor prognosis. Publications on radiotherapy (RT) are scarce. This review summarizes the current data on RT for pACC and possibly provides first evidence to justify its use in this setting.
Materials and methods
We searched the PubMed and Embase database for manuscripts regarding RT for pACC.
Results
We included 17 manuscripts reporting on 76 patients treated with RT, after screening 2961 references and 269 full articles. In addition, we added data of 4 unreported pACC patients treated by co-authors. All reports based on retrospective data. Median age at first diagnosis was 11.1 years (70% female); 78% of patients presented with hormonal activity. RT was mostly performed for curative intent (78%). 88% of RT were administered during primary therapy. The site of RT was predominantly the local tumor bed (76%). Doses of RT ranged from 15 to 62 Gy (median 50 Gy). Information on target volumes or fractionation were lacking. Median follow-up was 6,9 years and 64% of the patients died of disease, with 33% alive without disease. In 16 of 48 patients with available follow-up data after adjuvant RT (33%) no recurrence was reported and in 3 of 9 patients palliative RT seemed to induce some benefit for the patient.
Conclusions
Our first systematic review on RT for pACC provides too few data for any general recommendation, but adjuvant RT in patients with high risk might be considered. International collaborative studies are urgently needed to establish better evidence on the role of RT in this rare malignancy.
Background
Multiple myeloma (MM) is the third most common hematologic malignancy with increasing importance due to improving treatment strategies and long-term outcomes in an aging population. This study aims to analyse influencing factors on health-related quality of life (HRQoL), such as treatment strategies, participation in a clinical trial and patient characteristics like anxiety, depression, gender, and age. A better understanding of the individual factors in context with HRQoL could provide a helpful instrument for clinical decisions.
Methods
In this prospective observational study, the HRQoL of MM patients with different therapies (first-line and relapse) was quantified by standardized questionnaires (EORTC QLQ-C30 and -MY20) in the context of sociodemographic data, individual anxiety and depressiveness (PHQ-4), and a selected number of clinical parameters and symptoms at defined time-points before, during, and after therapy.
Results
In total, 70 patients were included in the study. The median age of the study cohort was 62 years. 44% were female and 56% were male patients. More than half of the patients were fully active with an ECOG 0. Global health status was significantly higher in patients with first-line treatment and even increased after start of therapy, while the pain level decreased. In contrast, patients with relapsed MM reported a decreasing global health status and increasing pain. Additionally, there was a higher global health status in less anxious/depressive patients. HRQoL decreased significantly after start of chemotherapy in the parameters body image, side effects of treatment, and cognitive functioning. Tandem stem-cell transplantation was not found to be a risk factor for higher impairment of HRQoL. Participation in a clinical study led to an improvement of most aspects of HRQoL. Among others, increased anxiety and depression, female gender, older age, impaired performance status, and recurrent disease can be early indicators for a reduced HRQoL.
Conclusion
This study showed the importance of regular longitudinal assessments of patient reported outcomes (PROs) in routine clinical care. For the first time, to our knowledge, we were able to demonstrate a potential impact between participation in clinical trials and HRQoL. However, due to frequently restrictive inclusion criteria for clinical trials, these MM patients might not be directly comparable with patients treated within standard therapy concepts. Further studies are needed to clarify the relevance of this preliminary data in order to develop an individualized, patient-centred, therapy concept.
In large vessel occlusion stroke, recanalization to restore cerebral perfusion is essential but not necessarily sufficient for a favorable outcome. Paradoxically, in some patients, reperfusion carries the risk of increased tissue damage and cerebral hemorrhage. Experimental and clinical data suggest that endothelial cells, representing the interface for detrimental platelet and leukocyte responses, likely play a crucial role in the phenomenon referred to as ischemia/reperfusion (I/R)-injury, but the mechanisms are unknown. We aimed to determine the role of endoglin in cerebral I/R-injury; endoglin is a membrane-bound protein abundantly expressed by endothelial cells that has previously been shown to be involved in the maintenance of vascular homeostasis. We investigated the expression of membranous endoglin (using Western blotting and RT-PCR) and the generation of soluble endoglin (using an enzyme-linked immunosorbent assay of cell culture supernatants) after hypoxia and subsequent reoxygenation in human non-immortalized brain endothelial cells. To validate these in vitro data, we additionally examined endoglin expression in an intraluminal monofilament model of permanent and transient middle cerebral artery occlusion in mice. Subsequently, the effects of recombinant human soluble endoglin were assessed by label-free impedance-based measurement of endothelial monolayer integrity (using the xCELLigence DP system) and immunocytochemistry. Endoglin expression is highly inducible by hypoxia in human brain endothelial monolayers in vitro, and subsequent reoxygenation induced its shedding. These findings were corroborated in mice during MCAO; an upregulation of endoglin was displayed in the infarcted hemispheres under occlusion, whereas endoglin expression was significantly diminished after transient MCAO, which is indicative of shedding. Of note is the finding that soluble endoglin induced an inflammatory phenotype in endothelial monolayers. The treatment of HBMEC with endoglin resulted in a decrease in transendothelial resistance and the downregulation of VE-cadherin. Our data establish a novel mechanism in which hypoxia triggers the initial endothelial upregulation of endoglin and subsequent reoxygenation triggers its release as a vasoactive mediator that, when rinsed into adjacent vascular beds after recanalization, can contribute to cerebral reperfusion injury.
Background
Adrenalectomies are rare procedures especially in childhood. So far, no large cohort study on this topic has been published with data on to age distribution, operative procedures, hospital volume and operative outcome.
Methods
This is a retrospective analysis of anonymized nationwide hospital billing data (DRG data, 2009-2017). All adrenal surgeries (defined by OPS codes) of patients between the age 0 and 21 years in Germany were included.
Results
A total of 523 patient records were identified. The mean age was 8.6 ± 7.7 years and 262 patients were female (50.1%). The majority of patients were between 0 and 5 years old (52% overall), while 11.1% were between 6 and 11 and 38.8% older than 12 years. The most common diagnoses were malignant neoplasms of the adrenal gland (56%, mostly neuroblastoma) with the majority being younger than 5 years. Benign neoplasms in the adrenal gland (D350) account for 29% of all cases with the majority of affected patients being 12 years or older. 15% were not defined regarding tumor behavior. Overall complication rate was 27% with a clear higher complication rate in resection for malignant neoplasia of the adrenal gland. Bleeding occurrence and transfusions are the main complications, followed by the necessary of relaparotomy. There was an uneven patient distribution between hospital tertiles (low volume, medium and high volume tertile). While 164 patients received surgery in 85 different “low volume” hospitals (0.2 cases per hospital per year), 205 patients received surgery in 8 different “high volume” hospitals (2.8 cases per hospital per year; p<0.001). Patients in high volume centers were significant younger, had more extended resections and more often malignant neoplasia. In multivariable analysis younger age, extended resections and open procedures were independent predictors for occurrence of postoperative complications.
Conclusion
Overall complication rate of adrenalectomies in the pediatric population in Germany is low, demonstrating good therapeutic quality. Our analysis revealed a very uneven distribution of patient volume among hospitals.
Background
Mucopolysaccharidosis type III (Sanfilippo syndrome) is a lysosomal storage disorder, caused by a deficiency in the heparan-N-sulfatase enzyme involved in the catabolism of the glycosaminoglycan heparan sulfate. It is characterized by early nonspecific neuropsychiatric symptoms, followed by progressive neurocognitive impairment in combination with only mild somatic features. In this patient group with a broad clinical spectrum a significant genotype-phenotype correlation with some mutations leading to a slower progressive, attenuated course has been demonstrated.
Case presentation
Our patient had complications in the neonatal period and was diagnosed with Mucopolysaccharidosis IIIa only at the age of 28 years. He was compound heterozygous for the variants p.R245H and p.S298P, the latter having been shown to lead to a significantly milder phenotype.
Conclusions
The diagnostic delay is even more prolonged in this patient population with comorbidities and a slowly progressive course of the disease.
Background
Cancer vaccines can effectively establish clinically relevant tumor immunity. Novel sequencing approaches rapidly identify the mutational fingerprint of tumors, thus allowing to generate personalized tumor vaccines within a few weeks from diagnosis. Here, we report the case of a 62-year-old patient receiving a four-peptide-vaccine targeting the two sole mutations of his pancreatic tumor, identified via exome sequencing.
Methods
Vaccination started during chemotherapy in second complete remission and continued monthly thereafter. We tracked IFN-γ+ T cell responses against vaccine peptides in peripheral blood after 12, 17 and 34 vaccinations by analyzing T-cell receptor (TCR) repertoire diversity and epitope-binding regions of peptide-reactive T-cell lines and clones. By restricting analysis to sorted IFN-γ-producing T cells we could assure epitope-specificity, functionality, and TH1 polarization.
Results
A peptide-specific T-cell response against three of the four vaccine peptides could be detected sequentially. Molecular TCR analysis revealed a broad vaccine-reactive TCR repertoire with clones of discernible specificity. Four identical or convergent TCR sequences could be identified at more than one time-point, indicating timely persistence of vaccine-reactive T cells. One dominant TCR expressing a dual TCRVα chain could be found in three T-cell clones. The observed T-cell responses possibly contributed to clinical outcome: The patient is alive 6 years after initial diagnosis and in complete remission for 4 years now.
Conclusions
Therapeutic vaccination with a neoantigen-derived four-peptide vaccine resulted in a diverse and long-lasting immune response against these targets which was associated with prolonged clinical remission. These data warrant confirmation in a larger proof-of concept clinical trial.
Background
The plasticity of T helper-17 (Th17) and regulatory T (Treg) cells may be a clue to pathogenesis of Juvenile Idiopathic Arthritis (JIA). It is still unclear, whether targeted suppression of Interleukin (IL)-17 is able to influence regulatory function of Treg to control pro-inflammatory effectors in JIA. This study aimed to assess the effect of a Th17-stimulating cytokine environment and of IL-17A-inhibition on phenotype plasticity and suppressive function of Treg derived from JIA patients.
Methods
Th17 and Treg characteristics of CD4\(^{+}\) helper T cells were investigated in blood samples of JIA patients with oligo- and polyarticular pattern and healthy controls (HC). Isolated CD4\(^{+}\)CD25\(^{+}\)CD127\(^{-}\) cells defined as Treg were cultivated with Th17-inducing cytokine environment as well as with IL-17A-inhibitors and analyzed for plasticity of phenotype by flow cytometry. Furthermore, inhibitory function of Treg on autologous effectors after cultivation with these stimuli was determined by suppression assays.
Results
Our findings demonstrated significantly elevated proportions of Th17 and Th17-like Treg in JIA compared to HC. After incubation with Th17-inducing stimuli, increased FoxP3 expression in separated Treg in JIA and an impaired suppressive capacity in JIA and HC were found. Blockade of IL-17A resulted in adjustment of FoxP3-expression in JIA to proportions found in controls and in regular suppressive function.
Conclusions
Our results demonstrate an induction of FoxP3 expressing Treg by Th17-inducing cytokines with concomitant mitigated suppressive function. In contrast, specific IL-17A blockade maintains suppressive Treg function and adjusted FoxP3-expression in JIA to levels found in controls. These findings may help to provide experimental evidence for the successful clinical use of IL-17A inhibition in JIA patients.
Background
In individuals suffering from a rare disease the diagnostic process and the confirmation of a final diagnosis often extends over many years. Factors contributing to delayed diagnosis include health care professionals' limited knowledge of rare diseases and frequent (co-)occurrence of mental disorders that may complicate and delay the diagnostic process. The ZSE-DUO study aims to assess the benefits of a combination of a physician focusing on somatic aspects with a mental health expert working side by side as a tandem in the diagnostic process.
Study design
This multi-center, prospective controlled study has a two-phase cohort design.
Methods
Two cohorts of 682 patients each are sequentially recruited from 11 university-based German Centers for Rare Diseases (CRD): the standard care cohort (control, somatic expertise only) and the innovative care cohort (experimental, combined somatic and mental health expertise). Individuals aged 12 years and older presenting with symptoms and signs which are not explained by current diagnoses will be included. Data will be collected prior to the first visit to the CRD’s outpatient clinic (T0), at the first visit (T1) and 12 months thereafter (T2).
Outcomes
Primary outcome is the percentage of patients with one or more confirmed diagnoses covering the symptomatic spectrum presented. Sample size is calculated to detect a 10 percent increase from 30% in standard care to 40% in the innovative dual expert cohort. Secondary outcomes are (a) time to diagnosis/diagnoses explaining the symptomatology; (b) proportion of patients successfully referred from CRD to standard care; (c) costs of diagnosis including incremental cost effectiveness ratios; (d) predictive value of screening instruments administered at T0 to identify patients with mental disorders; (e) patients’ quality of life and evaluation of care; and f) physicians’ satisfaction with the innovative care approach.
Conclusions
This is the first multi-center study to investigate the effects of a mental health specialist working in tandem with a somatic expert physician in CRDs. If this innovative approach proves successful, it will be made available on a larger scale nationally and promoted internationally. In the best case, ZSE-DUO can significantly shorten the time to diagnosis for a suspected rare disease.
Background
Physical activity (PA) and exercise have become an accepted and valued component of cystic fibrosis (CF) care. Regular PA and exercise can positively impact pulmonary function, improve physical fitness, and enhance health-related quality of life (HRQoL). However, motivating people to be more active is challenging. Supervised exercise programs are expensive and labour intensive, and adherence falls off significantly once supervision ends. Unsupervised or partially supervised programs are less costly and more flexible, but compliance can be more problematic. The primary objective of this study is to evaluate the effects of a partially supervised exercise intervention along with regular motivation on forced expiratory volume in 1 s (FEV1) at 6 months in a large international group of CF patients. Secondary endpoints include patient reported HRQoL, as well as levels of anxiety and depression, and control of blood sugar.
Methods/design
It is planned that a total of 292 patients with CF 12 years and older with a FEV1 ≥ 35% predicted shall be randomised. Following baseline assessments (2 visits) patients are randomised into an intervention and a control group. Thereafter, they will be seen every 3 months for assessments in their centre for one year (4 follow-up visits). Along with individual counselling to increase vigorous PA by at least 3 h per week on each clinic visit, the intervention group documents daily PA and inactivity time and receives a step counter to record their progress within a web-based diary. They also receive monthly phone calls from the study staff during the first 6 months of the study. After 6 months, they continue with the step counter and web-based programme for a further 6 months. The control group receives standard care and keeps their PA level constant during the study period. Thereafter, they receive the intervention as well.
Discussion
This is the first large, international multi-centre study to investigate the effects of a PA intervention in CF with motivational feedback on several health outcomes using modern technology. Should this relatively simple programme prove successful, it will be made available on a wider scale internationally.
