The self-assembly of a bowl-shaped naphthalimide-annulated corannulene of high solubility has been studied in a variety of solvents by NMR and UV/Vis spectroscopy. Evaluation by the anti-cooperative K\(_2\)-K model revealed the formation of supramolecular dimers of outstanding thermodynamic stability. Further structural proof for the almost exclusive formation of dimers over extended aggregates is demonstrated by atomic force microscopy (AFM) and diffusion ordered spectroscopy (DOSY) measurements as well as by theoretical calculations. Thus, herein we present the first report of a supramolecular dimer of an annulated corannulene derivative in solution and discuss its extraordinarily high thermodynamic stability with association constants up to > 10\(^6\)M\(^-\) \(^1\) in methylcyclohexane, which is comparable to the association constants given for planar phthalocyanine and perylene bisimide dyes.
Erstellung eines genregulatorischen Netzwerkes zur Simulation der Entstehung von Zahnhartsubstanz

In dieser Dissertation beschreibt der Autor die Erstellung eines grundlegenden bioinformatischen Modelles der menschlichen Zahnschmelzreifung. Mithilfe der KEGG Pathway-Datenbank wurde ein genregulatorisches Netzwerk (GRN) erstellt, welches maßgeblich auf den Signaltransduktionswegen Apoptose, Zellzyklus, Hedgehog-Signalweg, MAP-Kinase-Weg, mTOR-Signalweg Notch-Signalweg Signalweg, TGF-β-Signalweg und Wnt-Signalweg basiert. Im Weiteren wurde dieses Netzwerk durch zahlreiche verifizierte Wechselwirkungen erweitert und die zahnspezifischen Gene AMELX, AMELY, AMBN, ENAM und DSPP implementiert. In der anschließenden Simulation des Netzwerks mit dem Simulations-Tool Jimena konnten sechs stabile Zustände identifiziert werden. Diese wurden genauer untersucht und den Erkenntnissen eines GEO-Datensatzes gegenübergestellt. Langfristiges Ziel ist es, durch konsequente Optimierung des bioinformatischen Netzwerks Rückschlüsse auf die Odontogenese des Menschen zu ziehen.
Hier wird über den objektiven Vergleich der Kurz- und Langzeitergebnisse, die mit der laparoskopischen Heller-Myotomie plus Dor-Fundoplikatio (LHM + Dor) und der per-oralen endoskopischen Myotomie (POEM) an einer einzigen Institution erzielt wurden und über die Entwicklung eines "maßgeschneiderten Ansatzes" berichtet. Die laparosko-pischen Heller Myotomie (LHM) gilt aktuell als das Standardverfahren zur Therapie der Achalasie. Die perorale endoskopische Myotomie (POEM) bietet eine neue Alternative zur Durchführung der Heller-Myotomie auf endoskopischem und natural orifice translu-minal endoscopic surgery (NOTES) Wege.
Wir berichten über unsere unselektierte konsekutive Patientenserien (n = 238), die zwi-schen 2009 und 2017 entweder einer LHM + Dor Prozedur (n = 100) oder einer POEM Operation (n = 138) unterzogen wurden.
Es gab keine Unterschiede hinsichtlich des Behandlungserfolgs (Eckardt-Score ≤ 3) nach 3 - 6 Monaten (LHM: 90,1% vs. POEM: 90,5%; p = 0,735), 12 Monate (LHM 84,4% vs. POEM 84,0%; p = 0,766 und> 2 Jahre (LHM 81,1% vs. POEM 79,2%; p = 0,432). Die Verbesserung der Lebensqualität (GIQLI-Score) und die Patientenzufriedenheit waren in beiden Gruppen ebenfalls ver-gleichbar gut (~ 90%) und zu jedem Zeitpunkt der Nachbeobachtung. Refluxraten sind ebenfalls ähnlich (Sodbrennen LHM: 32,9% vs. POEM: 30,1%; Ösophagitis LHM: 16,3% vs. POEM: 14,6%; PPI-Aufnahme 21,2% vs. 32,5%, pathologischer pH-Wert -Meter-LHM: 8,8% vs. POEM: 23,3%) ohne Signifikanz der Unterschiede.
Die „maßgeschneiderte Verfahrenswahl POEM vs. LHM“ könnte geeignet sein und sollte in weiteren Studien weiter untersucht werden.
Einleitung:
TRPC-Kanäle spielen eine wichtige Rolle in der Pathologie der Herzinsuffizienz und kardialen Hypertrophie. Diese Effekte werden unter anderem über den Calcineurin-NFAT-Signalweg vermittelt. Ein wichtiger Interaktionspartner und Regulator von TRPC-Kanälen ist das Protein FKBP52. Mittels eines Yeast Two-Hybrid Systems wurde in einer kardialen cDNA library eine Interaktion zwischen einem C-terminalen Fragment von TRPC3 (AS 742-848), welches außerhalb der bekannten FKBP-Bindungsdomäne (AS 703-714) liegt, und FKBP52 beobachtet. Da dies eine weitere Bindungsstelle in FKBP52 vermuten ließ, erzeugten wir ein Fragment von FKBP52, welches FKBP52s genannt wurde und dem die funktionell relevante PPIase I-Domäne mit der bekannten Bindungsstelle fehlt. Eine erste Co-IP zwischen diesem Fragment und TRPC3 war erfolgreich.
Ziel:
Die Bestimmung, ob die Anwesenheit des verkürzten FKBP52 in vivo die Komplexbildung aus TRPC3 bzw. TRPC4 und dem Wildtyp-FKBP52 unterdrückt. Zusätzlich, ob FKBP52s die Interaktion zwischen TRPC3 bzw. TRPC4 und Calcineurin in vivo unterbricht und damit die Aktivierung des Calcineurin-NFAT-Signalweges hemmt.
Methoden:
Co-Immunopräzipitationen (Co-IP) wurden mit HEK-293-Zellen durchgeführt, die mit cDNA transfiziert wurden, welche Gene für TRPC3, TRPC4, Calcineurin A und FKBP52s enthielt. Zur Bestimmung der nukleären Translokation von NFATc1 mittels Fluoreszenzmikroskopie wurden HEK-293-Zellen mit TRPC3, TRPC4, GFP-NFATc1 ± FKBP52s transfiziert. Die statistische Analyse erfolgte mit einer One-Way ANOVA.
Ergebnisse:
In dieser Arbeit konnte gezeigt werden, dass FKBP52 sowohl mit TRPC3 als auch mit TRPC4 interagiert. Ebenso wurde festgestellt, dass FKBP52 auch ohne seine katalytische PPIase I-Domäne Bindungen mit TRPC3 bzw. TRPC4 eingeht. Dieses FKBP52-Konstrukt nimmt ebenso an der Komplexbildung mit TRPC3 bzw. TRPC4 und Calcineurin teil. Des Weiteren ließ sich für TRPC3 zeigen, dass unter Stimulation mit Carbachol (GPCR-Agonist) bei Anwesenheit dieses gekürzten FKBP52 eine signifikant geringere Aktivierung und Wanderung des Transkriptionsfaktors NFAT in den Nucleus erfolgte.
Schlussfolgerung:
FKBP52 spielt daher eine wichtige Rolle in dieser Signalkaskade, indem es entscheidend an der Aktivierung von Calcineurin und dessen Rekrutierung zum TRPC-Kanalkomplex beteiligt ist und damit auch an der Aktivierung des Calcineurin-NFAT-Signalweges.
Magnetic Resonance Imaging at field strengths up to 3 T, has become a default diagnostic modality for a variety of disorders and injuries, due to multiple reasons ranging from its non-invasive nature to the possibility of obtaining high resolution images of internal organs and soft tissues. Despite tremendous advances, MR imaging of certain anatomical regions and applications present specific challenges to be overcome. One such application is MR Musculo-Skeletal Imaging. This work addresses a few difficult areas within MSK imaging from the hardware perspective, with coil solutions for dynamic imaging of knee and high field imaging of hand.
Starting with a brief introduction to MR physics, different types of RF coils are introduced in chapter 1, followed by sections on design of birdcage coils, phased arrays and their characterization in chapter 2. Measurements, calculations and simulations, done during the course of this work, have been added to this chapter to give a quantitative feel of the concepts explained.
Chapter 3 deals with the construction of a phased array receiver for dynamic imaging of knee of a large animal model, i.e. minipig, at 1.5 T. Starting with details on the various aspects of an application that need to be considered when an MR RF array is designed, the chapter details the complex geometry of the region of interest in a minipig and reasons that necessitate a high density array. The sizes of the individual elements that constitute the array have been arrived at by studying the ratio of unloaded to loaded Q factors and choosing a size that provides the best ratio but still maintains a uniform SNR throughout the movement of the knee. To have a minimum weight and to allow mechanical movement of the knee, the Preamplifiers were located in a separate box. A movement device was constructed to achieve adjustable periodic movement of the knee of the anesthetized animal. The constructed array has been characterized for its SNR and compared with an existing product coil to show the improvement. The movement device was also characterized for its reproducibility. High resolution static images with anatomical details marked have been presented. The 1/g maps show the accelerations possible with the array. Snapshots of obtained dynamic images trace the cruciate ligaments through a cycle of movement of the animal's knee.
The hardware combination of a high density phased array and a movement device designed for a minipig's knee was used as a 'reference' and extended in chapter 4 for a human knee. In principle the challenges are similar for dynamic imaging of a human knee with regards to optimization of the elements, the associated electronics and the construction of the movement device. The size of the elements were optimized considering the field penetration / sensitivity required for the internal tissues. They were distributed around the curvature of the knee keeping in mind the acceleration required for dynamic imaging and the direction of the movement. The constructed movement device allows a periodic motion of the lower half of the leg, with the knee placed within the coil, enabling visualization of the tissues inside, while the leg is in motion. Imaging has been performed using dynamic interleaved acquisition sequence where higher effective TR and flip angles are achieved due to a combination of interleaving and segmentation of the sequence. The movement device has been characterized for its reproducibility while the SNR distribution of the constructed RF array has been compared with that of a commercially available standard 8 channel array. The results show the improvement in SNR and acceleration with the constructed geometry. High resolution static images, dynamic snapshots and the 3D segmentation of the obtained images prove the usefulness of the complete package provided in the design, for performing dynamic imaging at a clinically relevant field strength.
A simple study is performed in chapter 5 to understand the effects of changes in overlap for coil configurations with different loads and at different frequencies. The noise levels of individual channels and the correlation between them are plotted against subtle changes in overlap, at 64 and 123 MHz. SNR for every overlap setup is also measured and plotted. Results show that achieving critical overlap is crucial to obtain the best possible SNR in those coil setups where the load offered by the sample is low.
Chapter 6 of the thesis work deals with coil design for high field imaging of hand and wrists at 7 T, with an aim to achieve ultra high resolution imaging. At this field strength due to the increase in dielectric effects and the resulting decrease in homogeneity, whole body transmit coils are impractical and this has led engineers to design local transmit coils, for specific anatomies. While transmit or transceive arrays are usually preferred, to mitigate SAR effects, the spatial resolution obtained is limited. It is shown that a solution to this, with regards to hand imaging, can be a single volume transmit coil, along with high density receive arrays optimized for different regions of the hand. The use of a phased array for reception provides an increased SNR / penetration under high resolution. A volume transmit coil could pose issues in homogeneity at 7 T, but the specific anatomy of hand and wrist, with comparatively less water content, limits dielectric effects to have homogeneous B_1+ profile over the hand. To this effect, a bandpass birdcage and a 12 channel receive array are designed and characterized. Images of very high spatial resolution (0.16 x 0.16 x 0.16 mm3) with internal tissues marked are presented. In vivo 1/g maps show that an acceleration of up to 3 is possible and the EM simulation results presented show the uniform field along with SAR hotspots in the hand. To reduce the stress created due to the 'superman' position of imaging, provisions in the form of a holder and a hand rest have been designed and presented. Factors that contributed to the stability of the presented design are also listed, which would help future designs of receive arrays at high field strengths.
In conclusion, the coils and related hardware presented in this thesis address the following two aspects of MSK imaging: Dynamic imaging of knee and High resolution imaging of hand / wrist. The presented hardware addresses specific challenges and provides solutions. It is hoped that these designs are steps in the direction of improving the existing coils to get a better knowledge and understanding of MSK diseases such as Rheumatoid Arthritis and Osteoarthritis. The hardware can aid our study of ligament reconstruction and development. The high density array and transmit coil design for hand / wrist also demonstrates the benefits of the obtained SNR at 7 T while maintaining SAR within limits. This design is a contribution towards optimizing hardware at high field strength, to make it clinically acceptable and approved by regulatory bodies.
Teil 1: Koordination und Funktionalisierung von Dihydroboranen an Übergangsmetallkomplexen
Im Rahmen der vorliegenden Arbeit wurden Untersuchungen zur Koordination und Funktionalisierung von Dihydroboranen an Übergangsmetallkomplexen durchgeführt. Aufgrund der möglichen Anwendung in Dehydrokupplungsreaktionen wurde die Umwandlung von Dihydroboranen in Borylenkomplexe genauer untersucht.
Teil 2: Darstellung neuer Carbodiphosphorane und deren Koordination an ausgewählte Substrate
Durch Anwendung einfacher Synthesemethoden konnten in der vorliegenden Arbeit neuartige Carbodiphosphorane dargestellt werden. Diese wurden im weiteren Verlauf der Untersuchungen auf ihre Reaktivität gegenüber ausgewählten Substraten untersucht.
Das Non-Hodgkin-Lymphom (NHL) steht an siebter Stelle der Inzidenzen aller Krebserkrankungen, mit jährlich steigender Tendenz. Wie kann einer so gefährlichen und heterogenen Krankheitsentität in der heutigen Medizin angemessen begegnet werden? Neben etablierten Therapien, die geraden bei rezidivierten oder refraktären NHL an ihre Grenzen stoßen, bieten experimentelle Therapieansätze neue Hoffnung: Blinatumomab ist ein bispezifischer Antikörper, der durch seine beiden Domänen als Adapter für die T-Zelle und die Tumor-Zelle fungiert und eine Zytolyse der malignen B-Zelle induziert. Bei der ALL fand Blinatumomab schon Anwendung in mehreren klinischen Studien und wurde im Dezember 2014 von der FDA in den USA zur Behandlung von Philadelphia-Chromosom-negativer rezidivierten/ refraktären B-Zell Vorläufer-ALL zugelassen. Als erste klinische Studie an NHL-Patienten wurde von 2004-2011 die MT103/104-Studie veranlasst. Im Zuge dieser unverblindeten, multizentrischen Phase I/II Studie wurden 76 Patienten mit refraktärem und rezidiviertem NHL vier bis acht Wochen mit Blinatumomab als Dauerinfusion behandelt und hierbei Informationen zu Toxizität und Tolerabilität gesammelt. Mit der Langzeitbeobachtung der Würzburger Kohorte aus dieser Studie befasst sich die vorliegende Arbeit. Ziel ist es zunächst, festzustellen, wie lange die Patienten nach Blinatumomab-Therapie im Zuge der MT103/104 Studie gesamt, rezidiv- oder therapiefrei überlebten und ob bei einem bestimmten Patientensubkollektiv ein besonders vorteilhaftes Langzeitüberleben gezeigt werden kann. Die Frage nach der Sicherheit von Blinatumomab beantwortet die Erfassung des Langzeitnebenwirkungsspektrums: Somit werden als zweiter Endpunkt die häufigsten Gründe für Krankenhausaufenthalte nach Blinatumomabtherapie, eventuelle Häufungen einer spezifischen Nebenwirkungsentität und die Reversibilität der unter der Therapie aufgetretenen Nebenwirkungen mit einem selbst entwickelten Fragebogen erfasst. Der MoCA-Test soll neurokognitive Langzeittoxizitäten ausschließen. Die Arbeit konnte nicht nur zeigen, dass Patienten, die auf Blinatumomab ansprachen gegenüber den Patienten ohne Ansprechen ein deutlich längeres Überleben zeigten, sie bestätigte die Wichtigkeit des Erhalts der effektiven Dosis von 60 µg/m²/24h für das Erreichen und den Erhalt der Progressionsfreiheit. Sechs Patienten waren bei Beobachtungsende noch in Remission. Die unterschiedlichen Eindosierungsmodi hatten keinen Effekt auf das Langzeitüberleben, können aber nebenwirkungsbedingte Therapieabbrüche während der Therapie minimieren. Alle während der Therapie aufgetretenen Nebenwirkungen waren in der Langzeitnachbeobachtung vollständig reversibel. Am häufigsten mussten Patienten auf Grund von Infektionen im Verlauf hospitalisiert werden, bei zwei Patienten traten zusätzliche Tumorerkrankungen auf, die allerdings nicht mit der Blinatumomab-Therapie assoziiert waren. Die Rate der Transformationen von indolenten in aggressive NHL war nicht erhöht. Im MoCA-Test lassen sich keine Häufungen von neurokognitiven Defiziten finden. Blinatumomab zeigt sich auch in der Langzeitbeobachtung als ein für die Behandlung von rezidivierten und refraktären NHLs effektives und sicheres Medikament.
Hintergrund:
Eine Panikattacke beginnt typischerweise mit der Wahrnehmung einer physiologischen oder psychischen Veränderung, die von der Person als bedrohlich eingestuft wird. Während in klassischen neuroanatomischen Modellen der Panikstörung die Amygdala in der sich anschließenden aufschaukelnden Symptomatik in den Mittelpunkt gestellt wurde, erweitern aktuelle Studien dieses amygdalozentrische Bild und lenken die Aufmerksamkeit auf extratemporale neuronale Netzwerke. Dysfunktionen im neuronalen Aufmerksamkeitsnetzwerk, relevant für die Wahrnehmung und Regulierung exterozeptiver und interozeptiver Prozesse, könnten zur Entstehung einer Panikstörung beitragen. Weiterhin scheinen bestimmte Risikogenotypen für die Panikstörung wie z.B. im Adenosin Rezeptor 2A (ADORA2A) oder dem Neuropeptid S Rezeptor (NPSR1) Gen und die entsprechenden Neurotransmittersysteme in der Regulierung der Aufmerksamkeitsnetzwerke involviert zu sein.
Fragestellung:
Dysfunktionen im noradrenergen bottom-up Alertingnetzwerk und in der dopaminergen exekutiven top-down Aufmerksamkeitskontrolle könnten in einem neurokognitiven Entstehungsmodell der Panikstörung eine wichtige Rolle spielen. Mit Hilfe funktioneller Bildgebung soll die Funktion des neuronalen Aufmerksamkeitsnetzwerkes in einer nichtklinischen Stichprobe abhängig von genetischen Risikofaktoren und einer klinischen Stichprobe von und nach einer kognitiven Verhaltenstherapie untersucht werden.
Methoden:
Im nichtklinischen Teil der Untersuchung wurden in Studie 1 47 gesunde Versuchspersonen für die NPSR1 rs324981 Variante stratifiziert rekrutiert. Mittels fMRT wurde die Aktivität des Alertingnetzwerks und des Executive Control Netzwerks auf neuronaler Ebene mit dem Attentional Networt Test (ANT) untersucht. In Studie 2 wurde bei N=65 Versuchspersonen stratifiziert für die ADORA2A rs5751876 Variante als zusätzliches Verhaltensmaß die Fähigkeit zur interozeptiven Wahrnehmung in Bezug zur Konnektivität im insulären Ruhenetzwerk untersucht. Im klinischen Teil der Untersuchung (Studie 3) wurden 44 Patienten mit Panikstörung sowie eine entsprechend große und gematchte Kontrollgruppe rekrutiert. Es wurden fMRT Ruhemessungen vor und nach Abschluss einer kognitiven Verhaltenstherapie erhoben. Als zusätzliches Verhaltensmaß wurde die selbstberichtete Aufmerksamkeitskontrolle zwischen der Patienten- und der Kontrollgruppe verglichen.
Ergebnisse:
Träger des NPSR1 TT und des ADORA2A TT Risikogenotyps für Angst und Angsterkrankungen zeigten eine erhöhte Aktivität in Teilen des Alertingnetzwerks. Die Aktivität im Executive Control Netzwerk war arealabhängig teilweise erhöht, teilweise reduziert. Innerhalb eines interozeptiven Netzwerks zeigten Träger des ADORA2A TT Genotyps Hinweise auf eine dysfunktionale fronto-striatale-insuläre Interaktion. Im klinischen Teil der Studie zeigten Patienten mit Panikstörung eine reduzierte Konnektivität des dorsolateralen Präfrontalkortex (dlPFC) im fronto-parietalen Aufmerksamkeitsnetzwerk. Die Konnektivität innerhalb dieses Netzwerks korrelierte mit Defiziten in selbstberichteter Aufmerksamkeitskontrolle bei Patienten mit Panikstörung. Nach Abschluss der Therapie zeigte sich bei Patienten, die von der Therapie profitiert hatten, wieder eine Zunahme oder Verbesserung der Konnektivität mit dem dlPFC.
Schlussfolgerung:
Die Ergebnisse dieser Untersuchung betonen die Rolle dysfunktionaler interozeptiver und exterozeptiver Aufmerksamkeitsnetzwerke in der Entstehung von Angsterkrankungen. Bei Patienten mit Panikstörung sowie gesunden Versuchspersonen mit bestimmten prädisponierenden genetischen Variationen scheint eine Dysbalance des neuronalen Aufmerksamkeitsnetzwerks bzgl. der Abstimmung von bottom-up und top-down Netzwerken vorzuliegen.
Alzheimer's disease (AD) is a multifactorial disease and the most common form of dementia. There are no treatments to cure, prevent or slow down the progression of the disease. Natural products hold considerable interest for the development of preventive neuroprotectants to treat neurodegenerative disorders like AD, due to their low toxicity and general beneficial effects on human health with their anti-inflammatory and antioxidant features. In this work we describe regioselective synthesis of 7-O-ester hybrids of the flavonoid taxifolin with the phenolic acids cinnamic and ferulic acid, namely 7-O-cinnamoyltaxifolin and 7-O-feruloyltaxifolin. The compounds show pronounced overadditive neuroprotective effects against oxytosis, ferroptosis and ATP depletion in the murine hippocampal neuron HT22 cell model. Furthermore, 7-O-cinnamoyltaxifolin and 7-O-feruloyltaxifolin reduced LPS-induced neuroinflammation in BV-2 microglia cells as assessed by effects on the levels of NO, IL6 and TNFα. In all in vitro assays the 7-O-esters of taxifolin and ferulic or cinnamic acid showed strong overadditive activity, significantly exceeding the effects of the individual components and the equimolar mixtures thereof, which were almost inactive in all of the assays at the tested concentrations. In vivo studies confirmed this overadditive effect. Treatment of an AD mouse model based on the injection of oligomerized Aβ\(_{25-35}\) peptide into the brain to cause neurotoxicity and subsequently memory deficits with 7-O-cinnamoyltaxifolin or 7-O-feruloyltaxifolin resulted in improved performance in an assay for short-term memory as compared to vehicle and mice treated with the respective equimolar mixtures. These results highlight the benefits of natural product hybrids as a novel compound class with potential use for drug discovery in neurodegenerative diseases due to their pharmacological profile that is distinct from the individual natural components.
Prostate-specific membrane antigen (PSMA)-targeted PET imaging for prostate cancer with \(^{68}\)Ga-labeled compounds has rapidly become adopted as part of routine clinical care in many parts of the world. However, recent years have witnessed the start of a shift from \(^{68}\)Ga- to \(^{18}\)F-labeled PSMA-targeted compounds. The latter imaging agents have several key advantages, which may lay the groundwork for an even more widespread adoption into the clinic. First, facilitated delivery from distant suppliers expands the availability of PET radiopharmaceuticals in smaller hospitals operating a PET center but lacking the patient volume to justify an onsite \(^{68}\)Ge/\(^{68}\)Ga generator. Thus, such an approach meets the increasing demand for PSMA-targeted PET imaging in areas with lower population density and may even lead to cost-savings compared to in-house production. Moreover, \(^{18}\)F-labeled radiotracers have a higher positron yield and lower positron energy, which in turn decreases image noise, improves contrast resolution, and maximizes the likelihood of detecting subtle lesions. In addition, the longer half-life of 110 min allows for improved delayed imaging protocols and flexibility in study design, which may further increase diagnostic accuracy. Moreover, such compounds can be distributed to sites which are not allowed to produce radiotracers on-site due to regulatory issues or to centers without access to a cyclotron. In light of these advantageous characteristics, \(^{18}\)F-labeled PSMA-targeted PET radiotracers may play an important role in both optimizing this transformative imaging modality and making it widely available. We have aimed to provide a concise overview of emerging \(^{18}\)F-labeled PSMA-targeted radiotracers undergoing active clinical development. Given the wide array of available radiotracers, comparative studies are needed to firmly establish the role of the available \(^{18}\)F-labeled compounds in the field of molecular PCa imaging, preferably in different clinical scenarios.
In dynamic CE MR perfusion imaging the passage of an intravenously injected CA bolus through tissue is monitored to assess the myocardial pefusion state.
To enable this, knowledge of the shape of CA wash-in through upstream epicardial vessels is required, the so-called AIF.
For technical reasons this cannot be quantified directly in the supplying vessels and is thus measured in the left ventricle, which introduces the risk of systematic errors in quantification of MBF due to bolus dispersion in coronary vessels.
This means occuring CA dispersion must be accounted in the quantification process in order to produce reliable and reproducible results.
In order to do this, CFD simulations are performed to analyze and approximate these errors and deepen insights and knowledge gained from previous CFD analyses on both idealized as well as realistic and pathologically altered 3D geometries.
In a first step, several different procedures and approaches are undertaken in order to accelerate the performed workflow, however, maintaining a sufficient degree of numerical accuracy.
In the end, the implementation of these steps makes the analysis of the cardiovascular 3D model of unprecedented detail including vessels at pre-arteriolar level feasible at all.
The findings of the Navier-Stokes simulations are thus validated with regard to different aspects of cardiac blood flow.
These include the distribution of VBF into the different myocardial regions, the areals, which can be associated to the large coronary arteries as well as the fragmentation of VBF into vessels of different diameters.
The subsequently performed CA transport simulations yield results on the one hand confirming previous studies.
On the other hand, interesting additional knowledge about the behavior of CA dispersion in coronary arteries is obtained both regarding travelled distance as well as vessel diameters.
The relative dispersion of the so-called vascular transport function, a characterizing feature of vascular networks, shows a linear decrease with vessel diameter.
This results in asymptotically decreased additional dispersion of the CA time curve towards smaller and more distal vessels.
Nonetheless, perfusion quantification errors are subject to strong regional variability and reach an average value of $(-28\pm16)$ \% at rest across the whole myocardium.
Depending on the distance from the inlet and the considered coronary tree, MBF errors up to 62 \% are observed.
Background:
Diese prospektiv randomisierte kontrollierte Single-Center-Studie untersucht das klinische Outcome 3 bis 12 Monate nach Implantation einer Hüfttotalendoprothese über den transglutealen lateralen Hüftzugang (Bauer).
Methods:
77 Patienten wurden in die transgluteale laterale Zugangsweg Kohorte randomisiert, wovon 50 Patienten die Studie beendeten. Alle Patienten wurden mit dem Harris Hip Score (HHS), dem Xtra Short Musculosceletal Functional Assesment Questionaire (XSFMA), dem 36-Item Short Form Survey (SF-36), einem Stepwatch™-Aktivitätsmonitor (SAM) und einem zeitabhängigen 25m-Gehtest bewertet. Mit Hilfe der postoperativen Standard- röntgenaufnahmen wurde die Inklination, Schaftpositionierung und das Offset bewertet.
Results:
Unsere Patienten verbesserten sich nach drei Monaten in Bezug auf den 25-m-Gehtest (prä-operativ 24 Sekunden, SD ± 6,07, 3 Monate 19,36 Sekunden, SD ± 3,80) und die vom Patienten geschätzte maximale Gehstrecke nicht signifikant (präoperativ 1187, SD ± 1809m und 12 Monate 5125m, SD ± 3867m). Mit der Stepwatch konnte ein postoperativer Anstieg der mittleren Gehzyklen pro Tag gemessen werden (präoperativ 4964, SD ± 2687, 3 Monate 5238, SD ± 2309, 12 Monate 5340, SD ± 1887). Der Bewegungsumfang verbesserte sich insbesondere für die Flexion (präoperativ 89°, 3 Monate 97,34°) und die Abduktion (präoperativ 25,31°, 3 Monate 37,45°) und blieb im weiteren Verlauf nahezu unverändert. Der HHS zeigte einen signifikanten Unterschied zwischen dem präoperativen Score (52,95, SD ± 15,69) und den Scores aller postoperativen Messzeitpunkte (3 Monate 88,40, SD ± 9,89, 6 Monate 89,05, SD ± 10,01, 12 Monate 91,43, SD ± 9,05, p <0,001). Gleiches gilt für den XSFMA-Funktionsindex, der sich zwischen dem präoperativen Score (40,50, SD ± 15,78) und den postoperativen Scores signifikant verbesserte (3 Monate 18,80, SD ± 16,12, 6 Monate 15,84, SD ± 15,39, 12 Monate 15,08, SD ± 16,33, p<0.001). Der SF-36-Score mit der Subgruppe der körperlichen Verfassung betrug präoperativ 25,62, SD ± 8,74 und war postoperativ ebenfalls signifikant höher (3 Monate 40,67, SD ± 10,01, 6 Monate 42,76, SD ± 9,59, 12 Monate 42,91, SD ± 11,86, p < 0,05), während der Subscore der psychischen Beurteilung sich stets auf konstantem Niveau befand (präoperativ 56,26, SD ± 9,23, 3 Monate 56,68, SD ± 8,23, 6 Monate 55,81, SD ± 7,22, 12 Monate 56,23, SD ± 6,92, p <0,05). Die Bewertung der postoperativen Röntgenbildgebung ergab eine durchschnittliche Pfanneninklination von 40,28 ° und einen femorales Offset von 38,1mm ohne Anzeichen von Migration.
Conclusion:
Zusammenfassend zeigen unsere Ergebnisse, dass sich bereits drei Monate nach Implantation einer Totalhüftendoprothese über den transglutealen lateralen Zugang die Lebensqualität, das Aktivitätsniveau, die Patientenzufriedenheit und der postoperative Bewegungsumfang signifikant verbessert haben.

The aim of the thesis was to develop water soluble poly(2-oxazoline) (POx) copolymers with new side group functionalities, which can be used for the formation of hydrogels in biomedical applications and for the development of peptide-polymer conjugates.
First, random copolymers of the monomer MeOx or EtOx with ButEnOx and EtOx with DecEnOx were synthesized and characterized. The vinyl functionality brought into the copolymer by the monomers ButEnOx and DecEnOx would later serve for post-polymerization functionalization. The synthesized copolymers were further functionalized with thiols via post-polymerization functionalization using a newly developed synthesis protocol or with a protected catechol molecule for hydrogel formation. For the formation of peptide-polymer conjugates, a cyclic thioester, namely thiolactone acrylamide and an azlactone precursor, whose synthesis was newly developed, were attached to the side chain of P(EtOx-co-ButEnOx) copolymers.
The application of the functionalized thiol copolymers as hydrogels using thiol-ene chemistry for cross-linking was demonstrated. The swelling behavior and mechanical properties were characterized. The hydrophilicity of the network as well as the cross-linking density strongly influenced the swelling behavior and the mechanical strength of the hydrogels. All hydrogels showed good cell viability results.
The hydrogel networks based on MeOx and EtOx were loaded with two dyes, fluorescein and methylene blue. It was observed that the uptake of the more hydrophilic dye fluorescein depended more on the ability of the hydrogel to swell. In contrast, the uptake of the more hydrophobic dye methylene blue was less dependent on the swelling degree, but much more on the hydrophilicity of the network.
For the potential application as cartilage glue, (biohybrid) hydrogels were synthesized based on the catechol-functionalized copolymers, with and without additional fibrinogen, using sodium periodate as the oxidizing agent. The system allowed for degradation due to the incorporated ester linkages at the cross-linking points. The swelling behavior as well as the mechanical properties were characterized. As expected, hydrogels with higher degrees of cross-linking showed less swelling and higher elastic modulus. The addition of fibrinogen however increased the elasticity of the network, which can be favorable for the intended application as a cartilage glue. Biological evaluation clearly demonstrated the advantage of degradable ester links in the hydrogel network, where chondrocytes were able to bridge the artificial gap in contrast to hydrogels without any ester motifs.
Lastly, different ways to form peptide-polymer conjugates were presented. Peptides were attached with the thiol of the terminal cysteine group to the vinyl side chain of P(EtOx-co-ButEnOx) copolymers by radical thiol-ene chemistry. Another approach was to use a cyclic thioester, thiolactone, or an azlactone functionality to bind a model peptide via native chemical ligation. The two latter named strategies to bind peptides to POx side chains are especially interesting as one and in the case of thiolactone two free thiols are still present at the binding site after the reaction, which can, for example, be used for further thiol-ene cross-linking to form POx hydrogels.
In summary, side functional poly(oxazoline) copolymers show great potential for numerous biomedical applications. The various side chain functionalities can be introduced by an appropriate monomer or by post-polymerization functionalization, as demonstrated. By their multi-functionality, hydrogel characteristics, such as cross-linking degree and mechanical strength, can be fine-tuned and adjusted depending on the application in the human body. In addition, the presented chemoselective and orthogonal reaction strategies can be used in the future to synthesize polymer conjugates, which can, for example, be used in drug delivery or in tissue regeneration.
Häufigkeit und Bedeutung eines Apnoe-Hypopnoe-Index ≥ 5/h bei Patienten mit pulmonaler Hypertonie

Ein gleichzeitiges Auftreten von schlafbezogenen Atmungsstörungen(SBAS)und pulmonaler Hypertonie(PH) wurde bereits in mehreren kleineren Studien beobachtet. Die hier vorgestellte Studie soll die Prävalenz von SBAS bei verschiedenen Ätiologien der PH in einem größeren Patientenkollektiv untersuchen. Es sollen Kausalitäten zwischen den nach ICSD-3 klassifizierten SBAS-Formen und den nach Nizza klassifizierten PH-Gruppen festgestellt werden. Bisher gibt es keine Studien zur funktionellen Leistungsfähigkeit und zur Mortalität bei SBAS und PH.
Es handelt sich um eine retrospektive Studie, in die 111 Patienten eingeschlossen werden konnten, bei denen eine pulmonale Hypertonie nach dem Vorgehen gemäß den ESC/ERS Guidelines gesichert wurde. Alle Patienten erhielten ein ambulantes oder stationäres Schlafscreening in Form einer Polygraphie und/oder Polysomnographie, unabhängig davon, ob eine Symptomatik einer schlafbezogenen Atmungsstörung vorlag.
In dieser Studie konnte in einem größeren Patientenkollektiv eine hohe Prävalenz von SBAS bei PH-Patienten festgestellt werden. Bei dem Vergleich von PH-Patienten mit und ohne SBAS zeigten sich keine signifikanten Unterschiede bzgl. funktioneller Leistungsfähigkeit oder Mortalität. In der hier vorgestellten Studie kommen die zentrale und die gemischte schlafbezogene Atmungsstörung vor allem in der PH-Gruppe I und IV vor.
Zusammenfassung
Der Beitrag stellt vor, wie die Universitätsbibliothek Würzburg im Rahmen ihrer Veranstaltungen rund um das 400-jährige Jubiläum für die Studierenden im Lesesaal ein Escape Game konzipierte und veranstaltete. Nach einem kurzen Abriss, woher das Konzept des Live-Action Escape Game stammt, liegt der Fokus auf den konkreten Arbeitsschritten, die mit der Umsetzung dieses an wissenschaftlichen Bibliotheken bisher wenig vertretenen Formats verbunden waren.
This thesis explores the development of monoaminergic systems in the central nervous system (CNS) of zebrafish. The serotonergic cells of the hypothalamus pose the main focus of the present work. Most vertebrates except for mammals possess serotonin (5-HT) synthesising cells in more than one region of the CNS. In zebrafish such regions are, e.g. the hypothalamus, the raphe nuclei and the spinal cord. Serotonin functions as a neurotransmitter and neuromodulator in the CNS. Presumably due to its neuromodulatory tasks hypothalamic serotonergic cells are in contact with the cerebrospinal fluid (CSF), which expands the field of potential serotonergic targets tremendously. This highlights that serotonergic CSF-contacting (CSF-c) cells are vital for the execution of many functions and behaviours. Further, the hypothalamic serotonergic clusters constitute the largest population of serotonergic cells in the CNS of zebrafish. Together, these facts emphasise the need to understand the development and function of serotonergic CSF-c cells in the hypothalamus. Few studies have dealt with this subject, hence, information about the development of these cells is scarce. The zinc-finger transcription factor fezf2, and Fibroblast growth factor (Fgf)-signalling via the ETS-domain transcription factor etv5b are known to regulate serotonergic cell development in the hypothalamus (Bosco et al., 2013; Rink and Guo, 2004). However, the main Fgf ligand responsible for this mediation has not been determined prior to this work. The present thesis identifies Fgf3 as a crucial Fgf ligand. To achieve this result three independent strategies to impair Fgf3 activity have been applied to zebrafish embryos: the fgf3t24152 mutant, an fgf3 morpholino-based knock-down and the CRISPR/Cas9 technique. The investigations show that Fgf3 regulates the development of monoaminergic CSF-c cells in the hypothalamus. Additionally, Fgf3 impacts on cells expressing the peptide hormone arginine vasopressin (avp). Most interestingly, the requirement for Fgf3 by these cells follows a caudo-rostral gradient with a higher dependence on Fgf3 by caudal cells. This also seems to be the case for dopaminergic CSF-c cells in the hypothalamus (Koch et al., 2014). Moreover, etv5b a downstream target of Fgf-signalling is demonstrated to be under the control of Fgf3. With regard to serotonergic CSF-c cell development, it is shown that fgf3 is expressed several hours before tph1a and 5-HT (Bellipanni et al., 2002; Bosco et al., 2013). Together with the result that the hypothalamus is already smaller before mature serotonergic CSF-c cells appear, this argues for an early impact of Fgf3 on serotonergic specification. This hypothesis is supported by several findings in this study: the universal decrease of proliferating cells in the hypothalamus and simultaneous increase of cell death after fgf3 impairment. Complementary cell fate experiments confirm that proliferating serotonergic progenitors need Fgf3 to commit serotonergic specification. Further, these results corroborate findings of an earlier study stating that hypothalamic serotonergic progenitors require Fgf-signalling via etv5b to maintain the progenitor pool (Bosco et al., 2013). Additionally, the transcriptome of the hypothalamus has been analysed and 13 previously overlooked transcripts of Fgf ligands are expressed at developmental stages. The transcriptome analysis provides evidence for a self-compensatory mechanism of fgf3 since expression of fgf3 is upregulated as a consequence of its own impairment. Moreover, the Fgf-signalling pathway appears to be mildly affected by fgf3 manipulation. Together, Fgf-signalling and especially Fgf3 are established to be of critical importance during hypothalamic development with effects on serotonergic, dopaminergic CSF-c and avp expressing cells. Furthermore, this thesis provides two strategies to impair the tph1a gene. Both strategies will facilitate investigations regarding the function of hypothalamic serotonergic CSF-c cells. Finally, the presented findings in this study provide insights into the emergence of the posterior recess region of the hypothalamus, thereby, contributing to the understanding of the evolution of the vertebrate hypothalamus.
Die Bedeutung des seriell bestimmten Spender-Chimärismus nach allogener Stammzelltransplantation beim Multiplen Myelom wurde in der vorliegenden Arbeit analysiert. Die Analyse der Spenderchimärismen nach einer allogenen Stammzelltransplantation zur Messung der Krankheitslast und zur Detektion oder Prognose eines Rezidives wurde bereits in mehreren Studien für Leukämien und Lymphome untersucht, nun wurde erstmalig eine Patientengruppe mit Multiplen Myelom isoliert untersucht.
Insgesamt waren von 155 Patienten aus drei Transplantationszentren (Uniklinikum Würzburg, Uniklinikum Dresden und HELIOS Klinik Wiesbaden) 2324 Chimärismusproben (2198 aus dem peripheren Blut und 126 aus dem Knochenmark) zur Auswertung verfügbar. Die Chimärismusanalyse wurde durch die AgenDix GmbH – Applied Genetic Diagnostics – in Dresden durchgeführt und freundlicherweise bereitgestellt.
Abgesehen von der Überprüfung des Engraftments scheint die Analyse des seriell bestimmten Spender-Chimärismus nur einen begrenzten Aussagewert für das Krankheitsmanagement beim Multiplen Myelom zu haben. Durch die Analyse konnte nur in etwas mehr als einem Drittel der Fälle der Krankheitsprogress detektiert werden, obwohl knapp die Hälfte der Patienten einen Krankheitsprogress nach der allogenen Stammzelltransplantation aufwiesen. Insbesondere die Patienten mit einer extramedullären Manifestation als Rezidiv und einer geringen Knochenmarkfiltration wurden nicht ausreichend erfasst.
Objectives
Virtual reality exposure therapy (VRET) is a promising treatment for patients with fear of driving. The present pilot study is the first one focusing on behavioral effects of VRET on patients with fear of driving as measured by a post-treatment driving test in real traffic.
Methods
The therapy followed a standardized manual including psychotherapeutic and medical examination, two preparative psychotherapy sessions, five virtual reality exposure sessions, a final behavioral avoidance test (BAT) in real traffic, a closing session, and two follow-up phone assessments after six and twelve weeks. VRE was conducted in a driving simulator with a fully equipped mockup. The exposure scenarios were individually tailored to the patients’ anxiety hierarchy. A total of 14 patients were treated. Parameters on the verbal, behavioral and physiological level were assessed.
Results
The treatment was helpful to overcome driving fear and avoidance. In the final BAT, all patients mastered driving tasks they had avoided before, 71% showed an adequate driving behavior as assessed by the driving instructor, and 93% could maintain their treatment success until the second follow-up phone call. Further analyses suggest that treatment reduces avoidance behavior as well as symptoms of posttraumatic stress disorder as measured by standardized questionnaires (Avoidance and Fusion Questionnaire: p < .10, PTSD Symptom Scale–Self Report: p < .05).
Conclusions
VRET in driving simulation is very promising to treat driving fear. Further research with randomized controlled trials is needed to verify efficacy. Moreover, simulators with lower configuration stages should be tested for a broad availability in psychotherapy.
The eukaryotic parasite Trypanosoma brucei has evolved sophisticated strategies to persist within its mammalian host. Trypanosomes evade the hosts' immune system by antigenic variation of their surface coat, consisting of variant surface glycoproteins (VSGs). Out of a repertoire of thousands of VSG genes, only one is expressed at any given time from one of the 15 telomeric expression sites (ES). The VSG is stochastically exchanged either by a transcriptional switch of the active ES (in situ switch) or by a recombinational exchange of the VSG within the active ES. However, for infections to persist, the parasite burden has to be limited. The slender (sl) bloodstream form secretes the stumpy induction factor (SIF), which accumulates with rising parasitemia. SIF induces the irreversible developmental transition from the proliferative sl to the cell cycle-arrested but fly-infective stumpy (st) stage once a concentration threshold is reached. Thus, antigenic variation and st development ensure persistent infections and transmissibility. A previous study in monomorphic cells indicated that the attenuation of the active ES could be relevant for the development of trypanosomes. The present thesis investigated this hypothesis using the inducible overexpression of an ectopic VSG in pleomorphic trypanosomes, which possess full developmental competence. These studies revealed a surprising phenotypic plasticity: while the endogenous VSG was always down-regulated upon induction, the ESactivity determined whether the VSG overexpressors arrested in growth or kept proliferating. Full ES-attenuation induced the differentiation of bona fide st parasites independent of the cell density and thus represents the sole natural SIF-independent differentiation trigger to date. A milder decrease of the ES-activity did not induce phenotypic changes, but appeared to prime the parasites for SIF-induced differentiation. These results demonstrate that antigenic variation and development are linked and indicated that the ES and the VSG are independently regulated. Therefore, I investigated in the second part of my thesis how ES-attenuation and VSG-silencing can be mediated. Integration of reporters with a functional or defective VSG 3'UTR into different genomic loci showed that the maintenance of the active state of the ES depends on a conserved motif within the VSG 3'UTR. In situ switching was only triggered when the telomere-proximal motif was partially deleted, suggesting that it serves as a DNA-binding motif for a telomere-associated protein. The VSG levels seem to be additionally regulated in trans based on the VSG 3'UTR independent of the genomic context, which was reinforced by the regulation of a constitutively expressed reporter with VSG 3' UTR upon ectopic VSG overexpression.
Cellular therapies using chimeric antigen receptor (CAR) modified T cells to eradicate tumor cells have been a major breakthrough in the treatment of hematologic malignancies. However, there are no measures to control CAR T cell activity after infusion, which is mostly required in cases of CAR T cell overreaction, e.g. cytokine release syndrome, or in the case of T cell failure, e.g. caused by exhaustion.
In our study, we identified the tyrosine kinase inhibitor (TKI) dasatinib (© Sprycel) as a suitable agent to steer CAR T cells in vitro and in vivo. We show that single treatment of CD4+ and CD8+ CAR T cells with dasatinib conferred either partial or complete inhibition, depending on the applied concentration. The blockade was immediate and encompassed spe-cific lysis, cytokine secretion and proliferation following antigen encounter. The mechanism relied on reduced phosphorylation of key kinases in the CAR signaling cascade, which led to abrogation of nuclear factor of activated T-cells (NFAT) signaling. Importantly, inhibition was fully reversible by dasatinib withdrawal. In vivo, dasatinib blocked CAR T cell function without impairing the engraftment of CAR T cells or their subsequent anti tumor function once dasatinib administration was discontinued. We therefore introduce dasatinib as a new tool to efficiently block CAR T cells in vitro and in vivo, with data suggesting that dasatinib can be used in a clinical setting to mitigate toxicity after adaptive transfer of CAR modified T cells and other forms of T cell based immunotherapy.
Additionally we show that intermittent inhibition of CAR T cells by dasatinib im-proves the efficacy of CAR T cell therapy. By pausing T cells for short periods of time in vi-vo, upregulation of programmed death protein 1 (PD-1) and subsequent induction of exhaus-tion was prevented, which increased the expansion of T cells and the rate of tumor eradica-tion. Our data therefore suggest that dasatinib can additionally be used to overcome T cell exhaustion that is induced by massive tumor burden and upregulation of inhibitory receptors.
