Before the introduction of erythropoiesis-stimulating agents (ESAs) in 1989, repeated transfusions given to patients with end-stage renal disease caused iron overload, and the need for supplemental iron was rare. However, with the widespread introduction of ESAs, it was recognized that supplemental iron was necessary to optimize hemoglobin response and allow reduction of the ESA dose for economic reasons and recent concerns about ESA safety. Iron supplementation was also found to be more efficacious via intravenous compared to oral administration, and the use of intravenous iron has escalated in recent years. The safety of various iron compounds has been of theoretical concern due to their potential to induce iron overload, oxidative stress, hypersensitivity reactions, and a permissive environment for infectious processes. Therefore, an expert group was convened to assess the benefits and risks of parenteral iron, and to provide strategies for its optimal use while mitigating the risk for acute reactions and other adverse effects.
Background
Providing adequate healthcare to newly arrived refugees is considered one of the significant challenges for the German healthcare system. These refugees can be classified mainly into two groups: asylum seekers (who have applied for asylum after arrival in Germany and are waiting for the refugee-status decision) and resettlement refugees (who have already been granted asylum status before arriving in Germany). Whereas earlier studies have explored the health status of asylum seekers especially in terms of mental and behavioural disorders and infectious diseases without distinguishing between these two groups, our study aims to evaluate possible relationships of asylum status and medical needs of these two groups with a special focus on mental and behavioural disorders and infectious diseases.
Methods
In this retrospective observational study, collected data on all asylum-seeker and resettlement-refugee patients (N = 2252) of a German reception centre (August 2017 to August 2018) is analysed by absolute and relative frequencies and medians. Patient data, collected by chart review, include age, gender, country of origin, asylum status, and diagnoses (ICD-10). To describe the relationship between sociodemographic factors (including asylum status) and diagnoses, we used tests of significance and bivariate correlations with Spearman correlation coefficients. All collected data are pseudonymised.
Results
Of all 2252 patients, 43% were resettlement refugees. In almost all ICD-10 categories, asylum seekers received significantly more diagnoses than resettlement refugees. According to our data, asylum seekers presented with mental and behavioural disorders nine times more often (9%) than resettlement refugees (1%). In the case of infectious diseases, the results are mixed: asylum seekers were twice as frequently (11%) diagnosed with certain infectious and parasitic diseases than resettlement refugees (5%), but resettlement refugees were treated twice as often (22% of the asylum seekers and 41% of the resettlement refugees) for diseases of the respiratory system, of which 84% were acute respiratory infections (in both groups).
Conclusion
This study indicates that patients with unregulated migration more frequently present symptoms of psychiatric diseases and somatoform symptoms than resettlement refugees. A health policy approach within migration policy should aim to enable persecuted persons to migrate under regulated and safe conditions.

Die Angaben über harnableitungsassoziierte Harnwegsinfektionen bei operativen Intensivpatienten sind in der Literatur nicht einheitlich. Daher wurden 259 Patienten im Studienzeitraum auf eine harnableitungsassoziierte Harnwegskolonisation und - infektion untersucht. 58 Patienten (22,4%) wurden mit einer harnableitungsassoziierten Harnwegsinfektion und 44 Patienten (17%) mit einer harnableitungsassoziierten Harnwegskolonisation ermittelt. Die Inzidenz der harnableitungsassoziierten Harnwegsinfektion lag bei 18,6/1000 Tagen und die der harnableitungsassoziierten Harnwegskolonisation bei 20/1000 Tagen. Dies liegt im Bereich der Literaturangaben für harnableitungsassoziierte Harnwegsinfektionen bei Patienten auf der operativen Intensivstation. Die Patientenliegedauer konnte unter den patientenbezogenen Risikofaktoren als signifikant unabhängiger Risikofaktor für eine Infektion ermittelt werden. Als behandlungsbezogene Risikofaktoren für eine harnableitungsassoziierte Harnwegsinfektion fanden sich der Verzicht auf Antibiotikagabe, die transurethrale Harnableitung und eine Liegedauer der Harnableitung von über 5 Tagen. E.Coli mit 41,2% gefolgt von Enterokokken mit 21,6% und Proteus mirabilis mit 10,2% waren die am häufigsten nachgewiesene Keime bei harnableitungsassoziierten Harnwegsinfektionen auf der operativen Intensivstation. Diese Ergebnisse sind im Einklang mit den Literaturangaben. Die Craniotomie als Eingriffsart war ein signifikant abhängiger Risikofaktor für eine Harnwegsinfektion, da hier signifikant häufiger eine transurethrale Harnableitung verwendet wurde, und der Einsatz einer gleichzeitigen Antibiotikagabe signifikant geringer war. Bei gleicher Kolonisationsrate zwischen der transurethralen und suprapubischen Harnableitung ist die Zahl der harnableitungsassoziierten Harnwegsinfektion bei transurethraler Harnableitung signifikant deutlich höher, was durch die Irritation der Harnröhre durch die transurethrale Harnableitung zu erklären sein dürfte. Bei einer längerfristigen Harndrainage ( > 5 Tage) sollten mittels bakteriologischer Urinuntersuchungen, Harnwegsinfektionen frühzeitig erkannt werden. Zur Prävention einer Harnwegsinfektion sollte auch die Anlage einer suprapubischen Harnableitung in Betracht gezogen werden. Eine routinemäßige Gabe von Antibiotika zur Prävention harnableitungsassoziierter Harnwegsinfektionen ist jedoch, angesichts einer drohenden Resistenzentwicklung, nicht indiziert.
Invasive Pilzinfektionen haben in den letzten Jahren stetig zugenommen und bedrohen insbesondere immunsupprimierte Patienten. Nach der Zulassung von neuen antimykotisch wirksamen Substanzen hat sich das verfügbare Spektrum an therapeutischen Optionen deutlich erweitert. Demgegenüber steckt die verfügbare Technik zur Resistenztestung humanpathogener Pilze noch weitgehend in den Kinderschuhen. Die Analyse zur Verfügung stehender Techniken ist Gegenstand der vorliegenden Arbeit. Zu diesem Zweck werden zunächst die Daten von 728 klinischen Isolaten von Candida spp., die zwischen April 2000 und November 2002 am Institut für Hygiene und Mikrobiologie der Universität Würzburg unter Anwendung des Mikrodilutionsverfahrens nach DIN getestet wurden, unter epidemiologischen Aspekten ausgewertet. Es zeigt sich ein deutliches Überwiegen von Candida albicans (53%), gefolgt von Candida glabrata (26%). Candida parapsilosis und Candida tropicalis hatten jeweils einen Anteil von ca. 7%. Sowohl Candida albicans wie auch Candida glabrata verhielten sich gegen Fluconazol, Amphotericin B und 5-Flucytosin in hohem Maße sensibel. Candida krusei und Candida tropicalis wurden zu einem nennenswerten Anteil resistent, vor allem gegen Fluconazol und 5-Flucytosin, getestet. Im zweiten Teil der Arbeit wird anhand von 56 Candida spp. ein Vergleich des Mikrodilutionsverfahrens nach DIN und des internationalen Standardverfahrens nach NCCLS durchgeführt. Es zeigen sich sowohl hinsichtlich Reproduzierbarkeit, Ablesezeitpunkt der minimalen Hemmkonzentration sowie Stabilität der Testergebnisse über den angestrebten Ablesezeitpunkt hinaus deutliche Vorteile des NCCLS- gegenüber dem DIN-Verfahren. Der wesentliche Unterschied zwischen beiden Protokollen liegt im verwendeten Wachstumsmedium. Ein Problem beider Verfahren besteht im großen Zeitaufwand, weshalb das YeastOne-Testverfahren, ein kommerziell erhältliches Mikrodilutionsverfahren, das den Farbindikator Alamar Blue zur Erleichterung der visuellen Endpunktbestimmung beinhaltet, im dritten Teil der Arbeit für die Anwendung am Institut in Würzburg gegenüber der NCCLS-Methode evaluiert wird. Es zeigt sich eine hohe Reproduzierbarkeit mit ca. 96% bei Fluconazol, Amphotericin B und 5-Flucytosin und eine gute Korrelation zum Standardverfahren nach NCCLS. Entscheidende Vorteile gegenüber dem NCCLS-Protokoll sind eine frühere Endpunktbestimmung für Azole, die bereits nach 24 Stunden erfolgen kann, sowie in der Zeitersparnis zwischen 64% und 94%. Ergänzend werden einige Schimmelpilze sowohl mit dem NCCLS- als auch mit dem YeastOne-Verfahren getestet, wobei sich eine hohe Korrelation der YeastOne- mit den NCCLS-Ergebnissen zeigt und sich für beide Vorgehensweisen der Ablesezeitpunkt bei gutem Wachstum nach 48 Stunden festlegen lässt. Die Resistenztestung klinischer Isolate wurde daraufhin im Institut für Hygiene und Mikrobiologie auf das YeastOne-System umgestellt. In ersten Anwendungen wurden dabei gute Ergebnisse erzielt.
Nasal colonization is a major risk factor for S. aureus infections. The mechanisms responsible for colonization are still not well understood and involve several factors on the host and the bacterial side. One key factor is the cell wall teichoic acid (WTA) of S. aureus, which governs direct interactions with nasal epithelial surfaces. We report here the first receptor for the cell wall glycopolymer WTA on nasal epithelial cells. In several assay systems this type F-scavenger receptor, termed SREC-I, bound WTA in a charge dependent manner and mediated adhesion to nasal epithelial cells in vitro. The impact of WTA and SREC-I interaction on epithelial adhesion was especially pronounced under shear stress, which resembles the conditions found in the nasal cavity. Most importantly, we demonstrate here a key role of the WTA-receptor interaction in a cotton rat model of nasal colonization. When we inhibited WTA mediated adhesion with a SREC-I antibody, nasal colonization in the animal model was strongly reduced at the early onset of colonization. More importantly, colonization stayed low over an extended period of 6 days. Therefore we propose targeting of this glycopolymer-receptor interaction as a novel strategy to prevent or control S. aureus nasal colonization.
