Beeinträchtigungen des Arbeitsgedächtnisses und der präfrontalen Hirnaktivität (Hypofrontalität) sind Anwärter für Endophänotypen für bipolare Störungen. Diese Studie untersuchte zum ersten Mal manisch-depressive Patienten (n=14) mit einer Arbeitsgedächtnisaufgabe, die verschiedene Prozesse (Speicherung und Abgleich) und verschiedene Komponenten (objektbezogenes und visuell-räumliches) beinhaltete. Mit Hilfe der funktionellen Nah-Infrarot Spektroskopie wurde die neuronale Aktivität über dem präfrontalen Kortex erfasst. Bei der Kontrollgruppe, die der Patientengruppe vergleichbar war, zeigten Veränderungen von oxygeniertem und deoxygeniertem Hämoglobin eine Zunahme der Aktivität im ventero-lateralen, im dorso-lateralen und superioren präfrontalen Kortex in der objektbezogenen und visuell-räumlichen Arbeitsgedächtnisaufgabe im Vergleich zur Kontrollaufgabe. Obwohl sich die kognitive Leistung (Anzahl der korrekten Antworten und Reaktionszeit) zwischen den beiden Gruppen nicht unterschied, zeigte die Patientengruppe eine verminderte Gehirnaktivität während der Speicherphase der Arbeitsgedächtnisaufgaben. Die fNIRS kann als valides, leicht zu handhabendes, billiges und schnelles Mittel verwendet werden um (verminderte) präfrontale kortikale Aktivität zu messen.
Background: The DAOA/G30 (D-amino acid oxidase activator) gene complex at chromosomal region 13q32-33 is one of the most intriguing susceptibility loci for the major psychiatric disorders, although there is no consensus about the specific risk alleles or haplotypes across studies. Methods: In a case-control sample of German descent (affective psychosis: n = 248; controls: n = 188) we examined seven single nucleotide polymorphisms (SNPs) around DAOA/G30 (rs3916966, rs1935058, rs2391191, rs1935062, rs947267, rs3918342, and rs9558575) for genetic association in a polydiagnostic approach (ICD 10; Leonhard’s classification). Results: No single marker showed evidence of overall association with affective disorder neither in ICD10 nor Leonhard’s classification. Haplotype analysis revealed no association with recurrent unipolar depression or bipolar disorder according to ICD10, within Leonhard’s classification manic-depression was associated with a 3-locus haplotype (rs2391191, rs1935062, and rs3916966; P = 0.022) and monopolar depression with a 5-locus combination at the DAOA/G30 core region (P = 0.036). Conclusion: Our data revealed potential evidence for partially overlapping risk haplotypes at the DAOA/G30 locus in Leonhard’s affective psychoses, but do not support a common genetic contribution of the DAOA/G30 gene complex to the pathogenesis of affective disorders.
The Contribution of Common and Rare Variants to the Complex Genetics of Psychiatric Disorders

Attention deficit/hyperactivity disorder (ADHD), one of the most frequent childhood-onset, chronic and lifelong neurodevelopmental diseases, affects 5 - 10% of school – aged children and adolescents, and 4% of adults. The classified basic symptoms are - according to the diagnostic system DSM-VI - inattentiveness, impulsivity and hyperactivity. Also daily life of patients is impaired by learning problems, relationship crises, conflicts with authority and unemployment, but also comorbidities like sleep - and eating problems, mood - and anxiety disorders, depression and substance abuse disorders are frequently observed. Although several twin and family studies have suggested heritability of ADHD, the likely involvement of multiple genes and environmental factors has hampered the elucidation of its etiology and pathogenesis. Due to the successful medication of ADHD with dopaminergic drugs like methylphenidate, up to now, the search for candidate genes has mainly focused on the dopaminergic and - because of strong interactions - the serotonergic system, including the already analyzed candidate genes DAT1, DRD4 and 5, DBH or 5-HTTLPR. Recently, DNA copy number changes have been implicated in the development of a number of neurodevelopmental diseases and the analysis of chromosomal gains and losses by Array Comparative Genomic Hybridization (Array CGH) has turned out a successful strategy to identify disease associated genes. Here we present the first systematic screen for chromosomal imbalances in ADHD using sub-megabase resolution Array CGH. To detect micro-deletions and -duplications which may play a role in the pathogenesis of ADHD, we carried out a genome-wide screen for copy number variations (CNVs) in a cohort of 99 children and adolescents with severe ADHD. Using high-resolution aCGH, a total of 17 potentially syndrome-associated CNVs were identified. The aberrations comprise four deletions and 13 duplications with approximate sizes ranging from 110 kb to 3 Mb. Two CNVs occurred de novo and nine were inherited from a parent with ADHD, whereas five are transmitted by an unaffected parent. Candidates include genes expressing acetylcholine-metabolising butyrylcholinesterase (BCHE), contained in a de novo chromosome 3q26.1 deletion, and a brain-specific pleckstrin homology domain-containing protein (PLEKHB1), with an established function in primary sensory neurons, in two siblings carrying a 11q13.4 duplication inherited from their affected mother. Other genes potentially influencing ADHD-related psychopathology and involved in aberrations inherited from affected parents are the genes for the mitochondrial NADH dehydrogenase 1 alpha subcomplex assembly factor 2 (NDUFAF2), the brain-specific phosphodiesterase 4D isoform 6 (PDE4D6), and the neuronal glucose transporter 3 (SLC2A3). The gene encoding neuropeptide Y (NPY) was included in a ~3 Mb duplication on chromosome 7p15.2-15.3, and investigation of additional family members showed a nominally significant association of this 7p15 duplication with increased NPY plasma concentrations (empirical FBAT, p = 0.023). Lower activation of the left ventral striatum and left posterior insula during anticipation of large rewards or losses elicited by fMRI links gene dose-dependent increases in NPY to reward and emotion processing in duplication carriers. Additionally, further candidate genes were examined via Matrix assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF MS). This method enables the analysis of SNPs directly from human genomic DNA without the need for initial target amplification by PCR. All these findings implicate CNVs of behavior-related genes in the pathogenesis of ADHD and are consistent with the notion that both frequent and rare variants influence the development of this common multifactorial syndrome. The second part of this work concentrates on MLC1, a gene associated with Megalencephalic leukoencephalopathy with subcortical cysts, located on chromosome 22q13.33. To get more insight in the disease itself, a targeting vector for a conditional knockout mouse was constructed using homologous recombination. Furthermore, MLC1 has been suggested as a risk gene for schizophrenia, especially the periodic catatonia subtype. An initially identified missense mutation was found to be extremely rare in other patient cohorts; however, a recent report again argued for an association of two intronic MLC1 SNPs with schizophrenia and bipolar disorder. A case-control study of these polymorphisms as well as SNPs in the transcriptional control region of MLC1 was conducted in 212 chronic schizophrenic patients, 56 of which suffered from periodic catatonia, 106 bipolar patients, and 284 controls. Both intronic and promoter polymorphisms were specifically and significantly associated with periodic catatonia but not schizophrenia or bipolar disorder in general. A haplotype constructed from all polymorphisms was also associated with periodic catatonia. The MLC1 variation is associated with periodic catatonia; whether it constitutes a susceptibility or a modifier gene has to be determined.
22 alkoholabhängige Probanden und 24 gesunde Kontrollpersonen wurden im Rahmen von Expositionssitzungen, sowohl mit neutralen als auch mit alkoholassoziierten Reizen unterschiedlicher Modalitäten (in vivo, visuell, taktil, imaginär) konfrontiert. Ziel der Studie war, die neuralen hämodynamischen Reaktionen der Versuchsteilnehmer während den Expositionssitzungen anhand der NIRS (Nah-Infrarot Spektroskopie) in bestimmten Kortexregionen zu messen und unter Berücksichtigung der unterschiedlichen Suchtreizmodalitäten, der zeitlichen Persistenz, des Cravings und der Gruppenunterschiede in Bezug auf mögliche Suchtreiz-Reaktivitätsphänomene zu prüfen und zu vergleichen. Das Craving vor und nach den Expositionssitzungen wurde anhand der deutschen Version des ACQ (Alcohol Craving Questionnaire) erhoben. Zusammenfassend ist zu bestätigen, dass alkoholabhängige Patienten eine sich von gesunden Normen unterscheidende neurale Reaktion auf suchtbezogene Reize aufweisen – im Sinne einer ROI-bezogenen Cue-Reaktivität. Diese lässt sich jedoch anhand der vorliegenden Ergebnisse nur schwer präzisieren oder quantifizieren. Die bestehende Divergenz der erzielten Ergebnisse deutet auf mögliche reiz- bzw. sinnesspezifische Suchtreiz-Reaktivitätsmechanismen, welche in einem agonisierenden-antagonisierenden Zusammenspiel eine wesentliche Rolle bei der Entstehung von Cue-Clustering-Phänomenen und der Entwicklung von Craving spielen könnten. Es gibt auch Hinweise für eine abnehmende ACR über die Zeit unter abstinenten Bedingungen. Dieses wird durch die Feststellung über die progrediente Reduktion von subjektivem Craving gestützt.
Die Alkoholabhängigkeit ist die häufigste stoffgebundene Suchterkrankung, die immense volkswirtschaftliche Kosten verursacht. Sie führt bei den Betroffenen zu massiven Beeinträchtigungen in allen Lebensbereichen, insbesondere zu gesundheitlichen Problemen. Es existieren bisher mehrere Studien, die sich mit strukturellen Auffälligkeiten des Gehirns bei Alkoholabhängigkeit befassen, jedoch kaum funktionell-bildgebende Untersuchungen. Insbesondere fehlen Studien, welche die Erholung neuronaler Funktionen bei abstinenten alkoholabhängigen Personen untersuchen. In der vorliegenden Arbeit bearbeiteten je 20 entzügige, entgiftete und abstinente Probanden sowie 20 gesunde Kontrollprobanden einen seman-tischen und phonologischen Wortflüssigkeitstest (WFT). Währenddessen wurde ihre präfrontale Oxygenierung mittels funktioneller Nahinfrarotspektroskopie (fNIRS) aufgezeichnet. Die Gruppen waren nach Geschlecht, Alter, Händigkeit und Bildung vergleichbar. Auf Verhaltensebene fanden sich keine signifikanten Gruppenunterschiede. Die Gruppen der Entzügigen und Entgifteten zeigten insbesondere während des phonologischen WFT eine im Vergleich zur Kontrollgruppe signifikant verminderte Aktivierung in frontalen Gehirnbereichen. Die Gruppe der Abstinenten unterschied sich nicht signifikant von der Kontrollgruppe. Darüber hinaus war ein signifikanter linearer Trend mit zunehmender präfrontaler Oxygenierung über die Gruppen hinweg nachweisbar (Kontrast: Entzügige < Entgiftete < Abstinente < Kontrollen). Die Ergebnisse lassen eine Abnahme der pathologischen Durchblutungsmuster bei Probanden mit mindestens dreimonatiger Abstinenz erkennen, was neuronale Regenerationsprozesse vermuten lässt. Ein solcher Befund könnte im Rahmen der Suchttherapie als motivationaler Faktor eingesetzt werden. Allerdings sind aufgrund fehlender Längsschnittstudien und wegen der Komplexität der Alkoholabhängigkeit weitere Untersuchungen notwendig, um genauere Aussagen zu neuronalen Regenerationsprozessen treffen zu können.
Patienten mit Depression zeigen typischerweise eine Beeinträchtigung kognitiver Funktionen, vor allem im Bereich der exekutiven Funktionen. Als neuroanatomisches Korrelat konnte den exekutiven Funktionen der präfrontale Kortex zugeordnet werden. In den bisherigen bildgebenden Untersuchungen bei depressiven Patienten konnte vor allem eine Hypofrontalität festgestellt werden. Durch verschiedene neuropsychologische Tests konnten kognitive Defizite vor allem im visuell-räumlichen Arbeitsgedächtnis gezeigt werden. Als neuroanatomisches Korrelat konnte dem Arbeitsgedächtnis der DLPFC zugeordnet werden. Die bisher durchgeführten kombinierten Untersuchungen bei depressiven Patienten lieferten jedoch keine einheitlichen Ergebnisse. Mittelpunkt unserer Untersuchung war es deshalb mittels NIRS während der Durchführung eines Tests für das visuell-räumliche und das objektbezogene Arbeitsgedächtnis sowohl bei einer Patientengruppe mit unipolarer Depression als auch bei einer gesunden Kontrollgruppe die Aktivierungsmuster des präfrontalen Kortex zu ermitteln. Für den Zusammenhang zwischen der Hirnaktivierung und der Schwere der depressiven Erkrankung konnten keine signifikanten Korrelationen gezeigt werden. Dies spricht gegen den „state“-Charakter und für den „trait“-Charakter der Hypofrontalität bei einer depressiven Erkrankung. Die bezüglich der Verhaltensdaten gerechneten Varianzanalysen zeigten eine deutliche Schwierigkeitsabstufung zwischen den drei Bedingungen (OWM>VWM>KON). Der fehlende Interaktionseffekt Gruppe x Bedingung, also eine höhere Reaktionszeit der Patienten während allen Aufgaben und nicht nur während OWM und VWM deutet auf eine allgemeine Verlangsamung im Sinne einer psychomotorischen Verlangsamung hin und nicht wie erwartet auf ein besonderes Defizit im Bereich kognitiver Funktionen. Interaktionseffekte bei den bildgebenden Daten bei gleichzeitig fehlenden Interaktionsnachweisen bei den Verhaltensdaten deuten an, dass die funktionellen Daten unabhängig von den Verhaltensdaten interpretiert werden können. Ein kognitives Defizit für beide Komponenten des visuell-räumlichen Arbeitsgedächtnisses bei Patienten mit einer depressiven Erkrankung zeigt sich in unserer Untersuchung also weniger über die Verhaltensdaten als vielmehr über die verminderte Hirnaktivierung während OWM und VWM. Im Gruppenvergleich konnte in den ROI-Analysen für OWM und VWM wie erwartet ein spezieller Arbeitsgedächtniseffekt gezeigt werden, also eine höhere Aktivierung der Kontrollgruppe speziell für die Arbeitsgedächtnisaufgaben. Es wurde also insgesamt in unserer Untersuchung eine präfrontale Hypoaktivierung bei Patienten mit einer depressiven Erkrankung festgestellt.
Schizophrenie und die bipolar-affektive Erkrankung sind mit einer Lebenszeitprävalenz von ca. 1% häufige psychiatrische Krankheitsbilder. Die genaue Ätiologie beider Krankheiten ist bisher noch nicht eindeutig geklärt, allerdings nimmt man jeweils eine multifaktorielle Genese an, bei der eine genetische Anfälligkeit im Zusammenspiel mit Umweltfaktoren zur Krankheitsentstehung führt. Es bestehen für beide Krankheiten diverse pathophysiologische Modelle, besonders interessant ist dabei eine Dysregulation der Neurotransmitter. Neben Dopamin und GABA steht auch Glutamat, ein häufiger exzitatorischer Neurotransmitter im ZNS, im Verdacht, eine entscheidende Rolle bei der Entstehung der Schizophrenie zu spielen. Bei der bipolar-affektiven Erkrankung stehen besonders Veränderungen der monoaminergen Neurotransmission im Vordergrund. Eine Beteiligung des Glutamatsystems wird ebenfalls diskutiert. NOS1AP liegt auf Chromosom 1q22, einem aus Kopplungsstudien bekannten Suszeptibilitätslokus für Schizophrenie. Bereits in diversen anderen Studien wurde Assoziation auf Einzelmarker- und Haplotypebene festgestellt. NOS1AP interagiert mit der NOS-I und führt zu einer Translokation dieses Enzyms ins Zytosol, wodurch es dem Calciumeinstrom durch den glutamatergen NMDA-Rezeptor entzogen wird. Auf diese Weise ist es zu einem geringeren Grad aktiv und produziert weniger NO. Aufgrund der funktionellen Verbindung mit dem NMDA-Rezeptor und der NOS-I, die beide im Verdacht stehen, an der Pathogenese der Schizophrenie beteiligt zu sein, ist NOS1AP ein interessantes Kandidatengen. 14 SNPs im Bereich des NOS1AP-Gens und daraus resultierende Haplotypen wurden mittels Primerextension und MALDI-ToF Massenspektrometrie bei 245 Patienten mit Schizophrenie, 90 Patienten mit bipolar-affektiver Erkrankung und 360 Kontrollpersonen analysiert. Dabei konnte für drei SNPs (rs1538018, rs945713 und rs4306106) jeweils eine nominelle Assoziation mit Schizophrenie festgestellt werden. Auch nach Durchführung eines Permutationstests blieb für rs1538018 und rs945713 ein statistischer Trend bestehen. Bei Betrachtung der Haplotypen ließ sich lediglich nominelle Assoziation eines Haplotyps mit Schizophrenie nachweisen. Die geschlechtsspezifische Analyse ergab für die männlichen Patienten im Permutationstest eine grenzwertig signifikante Assoziation von rs1538018 und rs945713, während zwei Haplotypen nur eine nominelle Assoziation zeigten. Bei den weiblichen Patienten ließ sich weder eine allelische noch eine haplotypische Assoziation nachweisen. Für die bipolar-affektive Erkrankung wurden keine Assoziationen, weder auf Einzelmarker- noch auf Haplotyp-Ebene festgestellt. Die grenzwertige Assoziation der SNPs mit Schizophrenie macht eine pathogenetische Beteiligung von NOS1AP an Schizophrenie denkbar. Es sind jedoch noch weitere Replikationsstudien, auch in anderen Kollektiven, notwendig, um besser einschätzen zu können, welchen Einfluss NOS1AP tatsächlich für die Krankheitsentstehung hat.
EEG-Studie mit 34 erwachsenen ADHS-Patienten und 34 Kontrollpersonen mit einem modifizierten Eriksen-Flanker-Test. Dabei zeigten sich bei den ADHS Patienten verglichen mit der Kontrollgruppe verlängerte Reaktionszeiten, aber eine normale Fehleranzahl und ein normales post error slowing. Die ERN und die Pe waren vermindert. Dies weist auf Defizite in der Handlungsüberwachung bei erwachsenen ADHS-Patienten hin.
Integrating neurobiological markers of depression: an fMRI-based pattern classification approach

While depressive disorders are, to date, diagnosed based on behavioral symptoms and course of illness, the interest in neurobiological markers of psychiatric disorders has grown substantially in recent years. However, current classification approaches are mainly based on data from a single biomarker, making it difficult to predict diseases such as depression which are characterized by a complex pattern of symptoms. Accordingly, none of the previously investigated single biomarkers has shown sufficient predictive power for practical application. In this work, we therefore propose an algorithm which integrates neuroimaging data associated with multiple, symptom-related neural processes relevant in depression to improve classification accuracy. First, we identified the core-symptoms of depression from standard classification systems. Then, we designed and conducted three experimental paradigms probing psychological processes known to be related to these symptoms using functional Magnetic Resonance Imaging. In order to integrate the resulting 12 high-dimensional biomarkers, we developed a multi-source pattern recognition algorithm based on a combination of Gaussian Process Classifiers and decision trees. Applying this approach to a group of 30 healthy controls and 30 depressive in-patients who were on a variety of medications and displayed varying degrees of symptom-severity allowed for high-accuracy single-subject classification. Specifically, integrating biomarkers yielded an accuracy of 83% while the best of the 12 single biomarkers alone classified a significantly lower number of subjects (72%) correctly. Thus, integrated biomarker-based classification of a heterogeneous, real-life sample resulted in accuracy comparable to the highest ever achieved in previous single biomarker research. Furthermore, investigation of the final prediction model revealed that neural activation during the processing of neutral facial expressions, large rewards, and safety cues is most relevant for over-all classification. We conclude that combining brain activation related to the core-symptoms of depression using the multi-source pattern classification approach developed in this work substantially increases classification accuracy while providing a sparse relational biomarker-model for future prediction.
Impact of the AHI1 Gene on the Vulnerability to Schizophrenia: A Case-Control Association Study

Background: The Abelson helper integration-1 (AHI1) gene is required for both cerebellar and cortical development in humans. While the accelerated evolution of AHI1 in the human lineage indicates a role in cognitive (dys)function, a linkage scan in large pedigrees identified AHI1 as a positional candidate for schizophrenia. To further investigate the contribution of AHI1 to the susceptibility of schizophrenia, we evaluated the effect of AHI1 variation on the vulnerability to psychosis in two samples from Spain and Germany. Methodology/Principal Findings: 29 single-nucleotide polymorphisms (SNPs) located in a genomic region including the AHI1 gene were genotyped in two samples from Spain (280 patients with psychotic disorders; 348 controls) and Germany (247 patients with schizophrenic disorders; 360 controls). Allelic, genotypic and haplotype frequencies were compared between cases and controls in both samples separately, as well as in the combined sample. The effect of genotype on several psychopathological measures (BPRS, KGV, PANSS) assessed in a Spanish subsample was also evaluated. We found several significant associations in the Spanish sample. Particularly, rs7750586 and rs911507, both located upstream of the AHI1 coding region, were found to be associated with schizophrenia in the analysis of genotypic (p = 0.0033, and 0.031,respectively) and allelic frequencies (p = 0.001 in both cases). Moreover, several other risk and protective haplotypes were detected (0.006,p,0.036). Joint analysis also supported the association of rs7750586 and rs911507 with the risk for schizophrenia. The analysis of clinical measures also revealed an effect on symptom severity (minimum P value = 0.0037). Conclusions/Significance: Our data support, in agreement with previous reports, an effect of AHI1 variation on the susceptibility to schizophrenia in central and southern European populations.
