Development of predictable in vitro tumor models is a challenging task due to the enormous complexity of tumors in vivo. The closer the resemblance of these models to human tumor characteristics, the more suitable they are for drug-development and –testing. In the present study, we generated a complex 3D lung tumor test system based on acellular rat lungs. A decellularization protocol was established preserving the architecture, important ECM components and the basement membrane of the lung. Human lung tumor cells cultured on the scaffold formed cluster and exhibited an up-regulation of the carcinoma-associated marker mucin1 as well as a reduced proliferation rate compared to respective 2D culture. Additionally, employing functional imaging with 2-deoxy-2-[\(^{18}\)F]fluoro-D-glucose positron emission tomography (FDG-PET) these tumor cell cluster could be detected and tracked over time. This approach allowed monitoring of a targeted tyrosine kinase inhibitor treatment in the in vitro lung tumor model non-destructively. Surprisingly, FDG-PET assessment of single tumor cell cluster on the same scaffold exhibited differences in their response to therapy, indicating heterogeneity in the lung tumor model. In conclusion, our complex lung tumor test system features important characteristics of tumors and its microenvironment and allows monitoring of tumor growth and -metabolism in combination with functional imaging. In longitudinal studies, new therapeutic approaches and their long-term effects can be evaluated to adapt treatment regimes in future.
Es wird angenommen, dass das linke Vorhofohr vor allem bei Patienten, die unter Vorhofflimmern leiden, durch kardiale Embolisation schädliche oder gar tödliche Ereignisse bedingen kann und eine Amputation dieser anatomischen Struktur zu einer Reduktion der thromboembolisch bedingten Schlaganfallinzidenz des Patienten führen könnte. Es existiert aber zum aktuellen Zeitpunkt keine prospektiv randomisierte Studie, die eine chirurgische LAA-Amputation zum Zeitpunkt einer kardiochirurgischen Operation im Hinblick auf das postoperative Langzeit-Outcome mit ausreichend Studien-Power untersucht.
Diese retrospektive Studie befasst sich mit dem postoperativen Komplikationsrisiko der Amputation des linken Vorhofohrs im Rahmen kardiochirurgischen Operationen. Das Studiendesign wurde entworfen, um postoperativen Komplikationen, die durch die LAA-Amputation generiert werden, zu erfassen und die sichere Durchführbarkeit dieses Eingriffs zu veranschaulichen.
Es wurden Daten von insgesamt 234 Patienten des Universtitätsklinikums Würzburgs erfasst. Eingeschlossen wurden Patienten, die in der Klinik und Poliklinik für Herz, Thorax- und thorakale Gefäßchirurgie des Universitätsklinikums Würzburg im Zeitraum von Februar 2010 bis Mai 2013 eine Bypassoperation mit begleitender LAA-Amputation erhielten. Zu diesem Zweck wurden prä-, intra- und postoperative Daten der Patienten ausgewertet. Besonderes Augenmerk galt der Notwendigkeit einer postoperativen Rethorakotomie aufgrund einer Nachblutung, POAF, der peri-/postoperativen Schlaganfallinzidenz und der Letalität der Patienten. Der Großteil der untersuchten Patienten erhielt eine isolierte Bypassoperation (73,5%, 172/234), bei 26,5% (62/234) der Patienten wurde eine Kombinationsoperation durchgeführt. Es handelte sich hierbei um eine zusätzliche Klappenoperation, eine Carotis-Thrombendarteriektomie (Carotis-TEA), eine Myektomie oder einen Aorta-ascendens-Ersatz.
Blutungskomplikationen, die einer operativen Revision bedurften, waren in 3,0% der Fälle notwendig. In keinem der Fälle kam es zu einer Blutung an der Amputationsstelle. Auch die POAF-Inzidenz der vorliegenden Studie (27,5%) entspricht den in der Literatur (19-60%) beobachteten Werten nach kardiochirurgischen Operationen. Die postoperative Schlaganfallinzidenz betrug 2,1%(5/234). Es konnte kein signifikanter Zusammenhang zwischen dem Auftreten eines peri-/postoperativen Schlaganfalls und der POAF-Inzidenz festgestellt werden (p=0,129). Im Laufe der Anschlussbehandlung sind fünf Patienten verstorben (5/234, 2,1%). Drei dieser Patienten erhielten eine isolierte Bypassoperation und zwei eine Kombinationsoperation (Mitralklappenrekonstruktion/Myektomie bei HOCM). Keiner der verstorbenen Patienten erlitt postoperativ einen Schlaganfall.
Die Ergebnisse dieser Studie legen nahe, dass die Durchführbarkeit der LAA Amputation zum Zeitpunkt einer kardiochirurgischen Operation einen sicheren Eingriff darstellt.
Background
The impact of sex on cardiac morphology and function in chronic volume overload has been described in detail. However, the relation between sex and contractile properties at the actin-myosin level has not been well defined. Therefore, we evaluated the influence of sex on the contractile capacities of patients with chronic volume overload.
Methods
In 36 patients (18 males, 65 ± 9 years; 18 females, 65 ± 13 years) scheduled for elective mitral valve surgery due to severe mitral regurgitation (MR) with preserved left ventricular function, right auricle samples were obtained prior to extracorporal circulation. The fibers were prepared and skinned and exposed to a gradual increase in the calcium concentration (from pCa of 6.5–4.0) for calcium-induced force-developing measurements. Calcium sensitivity was also measured and recorded.
Results
The pCa-force relationship of the fibers obtained from males and females was significantly different, with the force values of the female fibers greater than those of male fibers at maximum calcium concentrations (pCa of 4.0: 3.6 ± 0.3 mN versus 3.2 ± 0.4 mN, p 0.02) and pCa of 4.5 2.6 ± 0.6 versus 2.0 ± 0.5, p 0.002). In contrast, the force values of female fibers were lower at mean calcium concentrations compared to those of male fibers (at 5.5 and pCa of 6.0: 1.0 ± 0.3 mN versus 1.2 ± 0.5 mN, p 0.04; 0.61 ± 0.05 versus 0.88 ± 0.09, p 0.04).
Calcium sensitivity was observed at pCa of 5.0 in females and pCa of 4.5 in males.
Conclusion
This study demonstrated that female fibers from patients exposed to chronic volume overload developed higher force values at a given calcium concentration compared to fibers from male patients. We assume that female patients might tap the full force potential, which is required when exposed to the highest calcium concentrations in our experimental cycle. The calcium sensitivity among genders was significantly different, with the results suggesting that males have higher calcium sensitivity and might compensate for lower force values at maximal calcium concentrations by a higher affinity for calcium. Hence, female patients with MR seem to work more “energy efficient”.
